Description
- Die Erfindung bezieht sich auf die Verwendung von Strontiumverbindungen für die Herstellung von Medikamenten zur Verwendung in Verfahren zur Behandlung von subdermalem Weichgewebeschmerz bzw. Weichteilschmerz, speziell chronischem Schmerz, und auf neuartige strontiumhaltige pharmazeutische Zusammensetzungen.
- Schmerz ist eine subjektive Erfahrung. Gemäß der International Association for the Study of Pain (IASP) ist Schmerz eine unbehagliche sensorische und emotionelle Erfahrung, die einer Gewebeschädigung zugeordnet ist. Der Körper besitzt seine eigenen inhibitorischen Mechanismen, die zur Unterdrückung von Schmerzimpulsen Enkephaline und Endorphine verwenden. Andere Mechanismen des Körpers wirken in bestimmten Situationen schmerzverstärkend bzw. schmerzintensivierend. Weil Schmerz eine derart komplexe Erfahrung ist, überrascht es nicht, dass er sich von Person zu Person unterscheidet oder dass er sich im selben Individuum in Abhängigkeit von der Zeit, vom Ort und der Situation verändern kann.
- Schmerz kann im allgemeinen als einem der nachfolgenden Typen zughörig kategorisiert werden:
nocizeptiver Schmerz;
neuropathischer Schmerz;
psychogener Schmerz; und
idiopathischer Schmerz. - Schmerz kann zudem als akut oder chronisch kategorisiert werden. Mit chronischem Schmerz ist Schmerz gemeint, der über mehr als 6 Monate anhält oder Wiederauftritt, und gegenüber dem keine kausale Therapie gegeben werden kann. Unter chronischem Schmerz Leidende können als Individuen definiert werden, die Schmerz als ein Ergebnis von beispielsweise einer muskuloskelettären Erkrankung, von Unfällen (beispielsweise Sportverletzungen), chirurgischen Eingriffen, Weichgewebekrebs (beispielsweise Rachenkrebs), Dysfunktion der Symphysis pubis, Narbengewebe oder einer Krankheit oder einem Leiden, beispielsweise Rheumatismus, erfahren.
- Auf der ganzen Welt gibt es ein zunehmendes Bedürfnis nach Behandlungen zum Reduzieren oder Eliminieren von chronischem oder akutem Schmerz. Gegenwärtige Schmerzlinderungsbehandlungen beinhalten häufig eine Medikamentengabe oder die Zerstörung von Nerven. Es bleibt jedoch ein Bedürfnis nach Verfahren zur Linderung von chronischem und akutem Schmerz, speziell von subdermalem Weichgewebeschmerz, beispielsweise Muskel- oder Sehnenschmerz, Schmerz im Narbengewebe oder an Orten chirurgischer Einschnitte, Gelenkschmerz, Brustschmerz, Rückenschmerz, Schleimbeutelschmerz (beispielsweise im Zusammenhang mit einer Schleimbeutelentzündung etc.) und speziell nach Behandlungsverfahren, die durch den am Schmerz Leidenden ohne medizinische Unterstützung durchgeführt werden können.
- Strontiumverbindungen waren für topische Verwendung zur Behandlung von Jucken oder Irritationen der Haut und Schleimhäute sowie der Zähne bekannt. Radioaktive Strontiumverbindungen waren zudem zur Verwendung zur Behandlung von Schmerzen bekannt, die mit metastasierendem Knochenkrebs einhergehen (kraft der Effekte der Strahlung, die durch die Strontium-Radionuklide emittiert wird).
- Die
US-A-5,866,168 beschreibt die Verwendung von dermatologischen/pharmazeutischen Zusammensetzungen, die Strontium enthalten können zur Behandlung und/oder Erleichterung von Schmerz, der mit unterschiedlichen Hautfehlfunktionen wie Gürtelrose bzw. Zona, Postzoster, Verbrühungen, Verbrennungen, Demodicidose, Ulkus der Haut, Fibrose, hypertrophem Narbenwachstum, Rosacea etc. einhergeht. - Die
US-A-5,851,556 bezieht sich auf die Verwendung von Erdalkalimetallsalzen, hierunter unter anderem Strontiumsalze, in Zusammensetzungen für die Behandlung von sensibler Haut. - Wir haben jedoch jetzt überraschenderweise herausgefunden, dass Strontiumverbindungen auch wirksam zu Reduzierung oder Eliminierung subdermaler Weichgewebeschmerzen verwandt werden können.
- Daher stellt die Erfindung, aus einem Aspekt heraus betrachtet, die Verwendung einer topischen Zusammensetzung bereit, die eine physiologisch verträgliche Strontiumverbindung, einen physiologisch verträglichen Träger bzw. Carrier und ein physiologisch verträgliches Mittel zur Verbesserung der Hautpenetration enthält, zur Herstellung eines Medikaments zur Verwendung in der Behandlung von subdermalem Weichgewebeschmerz (beispielsweise Schmerz in Muskeln, Sehnen, Bändern oder Schleimbeuteln, speziell Muskelschmerz), besonders von chronischem oder akutem Schmerz, speziell von chronischem Schmerz.
- Der Schmerz, der entsprechend der Erfindung behandelt wird, kann beispielsweise einer Schleimbeutelentzündung, einer Sehnenentzündung, Rheumatismus, Neuropathie, chirurgischen Eingriffen, Verletzungen (beispielsweise Sportverletzungen), Narbengewebe, mutmaßlichem Herzinfarkt, Rückenmuskeln, Weichgewebekrebs, Dysfunktion der Sympysis pubis und muskuloskelettären Krankheiten zugeordnet sein. Typischerweise wird sie jedoch nicht zur Behandlung von Kopfschmerzen oder Bauchschmerzen verwendet werden, könnte es jedoch, falls erwünscht.
- Die Strontiumverbindung, die entsprechend der Erfindung verwendet wird, ist vorzugsweise nichtradioaktiv. Mit "nichtradioaktiv" ist hier gemeint, dass die Strontiumverbindung nicht derart an radioaktiven Strontiumisotopen angereichert ist, dass sie sich als ein radioaktives Material für medizinische Zwecke qualifizieren würde. Wenngleich ein winziger Anteil des Strontiums, das in der Strontiumverbindung vorliegt, selbstverständlich radioaktiv sein kann, sollte der Gehalt des radioaktiven Strontiumisotops in der Strontiumverbindung im allgemeinen nicht mehr als dem 1000fachen der natürlichen Abundanz, vorzugsweise nicht mehr als dem 100fachen, noch eher vorzugsweise nicht mehr als dem 5fachen entsprechen. Ganz besonders vorzugsweise enthält die Strontiumverbindung radioaktive Strontiumisotope nicht in größerem Umfang als dem ihrer natürlichen Abundanz.
- Die Strontiumverbindung, wie sie entsprechend der vorliegenden Verbindung verwendet wird, kann jede physiologisch verträgliche Strontiumverbindung, die als eine Quelle zur Gabe von Strontiumionen dienen kann, sein. Typischerweise wird die Verbindung ein anorganisches oder organisches Salz oder ein Komplex sein, beispielsweise mit einem Chelator. Beispiele bevorzugter Verbindungen beinhalten Chlorid, Nitrat, Sulfat, Malat, Citrat, Lactat, Oxalat, Malat, Fumarat, Tartrat, Malonat, Acetat, Gluconat, Glutaconat, p-Aminohippurat, Succinat, Phosphat, Hydrogenphosphat, Glyce rophosphat, Aminocaproat, Mandelat, Dibenzoyltartrat, Stearat, Ascorbat, Benzoat, 3,4-Dimethoxybenzoat und Methotrexat, sowie Komplexe mit Penicillamin, Tyrosin, Leucin etc. Besonders bevorzugt liegt die Strontiumverbindung, wenn als Salz, in der Form des Chlorids, Nitrats, Acetats, Citrats, Lactats oder Hydrogenphosphats, speziell des Chlorids, Acetats, Citrats, Lactats oder Hydrogenphosphats, noch spezieller des Chlorids vor. Jedoch kann die Strontiumverbindung alternativ dazu in der Form eines Chelatkomplexes vorliegen, beispielsweise mit einer Polycarboxylsäure- oder einer Polyphosphorsäureverbindung oder einem zyklischen Polyether. Beispiele geeigneter Chelatoren sind in den Bereichen der Nuklearmedizin und der Kernresonanzbildgebung wohlbekannt (siehe beispielsweise die wissenschaftliche sowie die Patentliteratur von Amersham, Nycomed, Schering, Salutar, Bracco, Sterling Winthrop, Mallinckrodt, etc.). Die Verwendung linearer oder zyklischer Polychelatoren wie EDTA, DTPA, EGTA, DTPA-BMA, DOTA, DO3A, 1,2-Di(aminoethoxy)ethan-N,N,N',N'-tetraessigsäure, Kryptofix 5 und Kryptofix 222, speziell EDTA, ist besonders bevorzugt.
- Es ist besonders bevorzugt, dass die Strontiumverbindung zusammen mit einem weiteren Analgetikum, beispielsweise Aspirin, Ibuprofen oder anderen nichtsteroidalen antiinflamatorischen Medikamenten (NSAID) oder COX-2-Inhibitoren oder als ein Salz oder Komplex eines solchen Analgetikums verabreicht wird.
- Wenn erwünscht, kann die Strontiumverbindung als ein Salz oder ein Komplex einer Medikamentenverbindung mit einer aziden Gruppe oder einer Aminogruppe verabreicht werden, vorzugsweise einer Verbindung mit einem physiologischen Effekt, der für ein Leiden, von dem der Patient betroffen ist, förderlich ist, beispielsweise eine solche, die in der Behandlung der zugrundeliegenden Krankheit, die für die Schmerzen verantwortlich ist, wirksam ist. Im Falle von Aminomedikamenten kann die resultierende Strontiumverbindung typischerweise ein Strontiumchelat mit dem Aminomedikament als Gegenion sein. Beispiele solcher Medikamentenverbindungen beinhalten Nystatin, Mesalazin, Sulfasalazin, Olsalazin, Glutaminsäure, Repaglinid, Pantothensäure, Epoprostenol, Iloprost, Tirofiban, Tranexaminsäure, Folsäure, Furosemid, Bumetanid, Kaurensäure, Captopril, Enalapril, Lisinopril, Ramipril, Fosinopril, Trandolapril, Valsartan, Telmisartan, Pravastatin, Fluvastatin, Atorvastatin, Cerivastatin, Sulfadiazin, Tretinoin, Adapalen, Azelainsäure, Dinoproston, Levotyroxin, Lityronin, Doxycyclin, Lymecyclin, Oxytetracyclin, Tetracyclin, Ampicillin, Amoxicillin, Mecillinam, Benzylpenicillin, Phenoxymethylpenicillin, Diclosacillin, Clocsacillin, Piperacillin, Clavulansäure, Tazobactam, Cefalexin, Cefalotin, Cefoxitin, Cefuroxim, Ceftazidim, Ceftriaxon, Aztreonam, Meropenem, Imipenem, Cilastatin, Ciprafloxacin, Nalidixinsäure, Fusidinsäure, Phoscarnet und Zanamivir.
- Strontiumverbindungen, die in der vorliegenden Erfindung nützlich sein können, beinhalten Salze oder Komplexe von Strontium mit Cyclooxygenaseinhibitoren (anderen als Salicylaten (beispielsweise Acetylsalicylsäure) und Oxicamen (beispielsweise Piroxicam und Tenoxicam)) mit Aminosäuren und mit mehrzähnigen Chelatoren (anderen als EDTA oder EGTA), die die Fähigkeit besitzen, mehr als 3, vorzugsweise mehr als 4 Koordinationsbindungen mit Metallen auszubilden.
- Beispiele geeigneter Cyclooxygenaseinhibitoren (beispielsweise COX1- und/oder COX2-Inhibitoren) beinhalten NSAIDe wie Amfenac, Bendazac, Bufexamac, Cinmetacin, Diclofenac, Etodolac, Felbinac, Fenbufen, Fenoprofen, Fentiazac, Flufenaminsäure, Flunixin, Flunoxaprofen, Flurbiprofen, Furprofen, Ibuprofen, Indomethazin, Ketoprofen, Lonazolac, Loxoprofen, Mefenaminsäure, Mofezolac, Naproxen und Niflumin säure. Die Strontiumsalze oder -komplexe können einfach zubereitet werden, indem man Strontiumcarbonat mit der Säureform dieser Verbindungen in Lösung reagieren läßt.
- Beispiele von Aminosäuren, die zur Bildung von Strontiumverbindungen zur Verwendung entsprechend der Erfindung verwendet werden können, beinhalten alle natürlichen Alpha-Aminosäuren, beispielsweise Tyrosin, Leucin, Lysin etc. Wie es auch bei den COX-Inhibitoren der Fall ist, können die Verbindungen in Lösung zubereitet werden, indem Strontiumcarbonat und die Aminosäure verwendet wird. Es können jedoch auch andere Strontiumsalze verwendet werden, beispielsweise das Chlorid, das Acetat und das Hydroxid.
- Beispiele für Chelatoren, die zur Herstellung von Strontiumverbindungen zur Verwendung in der vorliegenden Erfindung verwendet werden können, beinhalten jene mit einem Diethylentriamin- oder einem Tetraazacyclododecanrückgrat, das wenigstens eine metallbindende Gruppe auf den Stickstoffen des Rückgrates in Form einer Oxosäure (beispielsweise Carboxyl- oder Phosphorsäure) trägt, beispielsweise DTPA, DTPA-Bismethylamid, DOTA, DO3A, Hydroxypropyl-DO3A etc. Diese sind aus dem Gebiet der Kontrastmittel für diagnostische Bildgebung wohlbekannt, und auch hier können die Strontiumverbindungen einfach in Lösung aus Strontiumcarbonat zubereitet werden.
- Die Strontiumverbindung wird in einer pharmazeutischen Zusammensetzung verabreicht, die wenigstens einen physiologischen Carrier bzw. Träger oder Hilfsstoff enthält. Die Strontiumverbindung kann bis zu 100 Gew.-% der Zusammensetzung ausmachen, vorzugsweise 0,005 bis 50 Gew.-%, eher vorzugsweise 0,05 bis 20 Gew.-%, besonders 0,1 bis 10 Gew.-%, insbesondere 0,1 bis 3 Gew.-%. Konventionelle pharmazeutische Carrier und Hilfsstoffe können verwendet werden, bei spielsweise Lösungsmittel (beispielsweise Wasser, Ethanol etc.), Gelbildner, Konservierungsmittel, Emulgatoren, Redoxmittel (beispielsweise Antioxidantien), Schäumungsmittel, Verdickungsmittel, Viskositätsmodifikatoren, pH-Modifikatoren etc.
- Die Strontiumzusammensetzungen zur Verwendung in der Erfindung können jede praktische Form zur topischen Gabe einnehmen. Daher können die Zusammensetzungen beispielsweise in der Form von Lösungen, Dispersionen, Suspensionen, Gelen, Flüssigkristallsystemen und Flüssigkristallvorläufern, Emulsionen, Sirupen, Cremes, Pasten, Salben, Wundsalben bzw. Unguenta, Sprays etc. vorliegen. Zur topischen Gabe werden Lösungen, Cremes, Pasten, Salben, Emulsionen und Gele bevorzugt.
- Die Zusammensetzungen enthalten ein Mittel zur Verbesserung der Hautpenetration.
- Beispiele geeigneter Mittel zur Verbesserung der Hautpenetration beinhalten Propylenglycol-Laurat, Propylenglycol-Monolaurat, Propylenglycol-Monocaprylat, Isopropylmyristat, Natriumlaurylsulfat, Dodecylpyridiniumchlorid, Ölsäure, Propylenglycol, Diethylenglycolmonoethylether, Nicotinsäureester, hydrogenierte Soja-Phospholipide, essentielle öle, Alkohole (wie Ethanol, Isopropanol, n-Octanol und Decanol), Terpene, N-Methyl-2-pyrrolidin, Alpha-Tocopherol, Polyethylenglycolsuccinat (TPGS), Tween 80 und andere Tenside, Dimethyl-beta-cyclodextrin und Dimethylsulfoxid, speziell DMSO.
- Die topische pharmazeutische Verwendung von Strontiumverbindungen, die ein Mittel zur Verbesserung der Hautpenetration enthalten, welches Dimethylsulfoxid ist, ist neu und bildet einen weiteren Aspekt der Erfindung.
- Daher stellt die Erfindung, aus einem weiteren Aspekt heraus betrachtet, eine Zusammensetzung zur Verwendung als topisches Pharmazeutikum (beispielsweise zur Linderung von Schmerz) bereit, das eine physiologisch verträgliche Strontiumverbindung, einen physiologisch verträglichen Träger (beispielsweise ein wässriges Lösungsmittel, ein Gel, eine Paste, eine Emulsion oder eine Creme) und ein physiologisch verträgliches Mittel zur Verbesserung der Hautpenetration enthält, welches Dimethylsulfoxid ist.
- Bestimmte Strontiumzusammensetzungen, die ein Mittel zur Verbesserung der Penetration enthalten, welches Dimethylsulfoxid ist, sind neu und bilden einen weiteren Aspekt der Erfindung, wie in Anspruch 15 definiert.
- Die Erfindung wird weiter durch die nachfolgenden, nicht einschränkenden Beispiele veranschaulicht. Beispiele 1 bis 17 veranschaulichen strontiumhaltige Zusammensetzungen und Strontiumverbindungen, die für die Verwendung in der Zubereitung pharmazeutischer Zusammensetzungen entsprechend der Erfindung geeignet sind.

- Eine strontiumhaltige Zusammensetzung wurde als 0,1 Gew.-%ige Lösung von Strontiumchlorid-Hexahydrat in Wasser zubereitet.
- Beispiel 2
- Zusammensetzung
- Eine strontiumhaltige Zusammensetzung wurde als eine Lösung von 0,1 Gew.-% Strontiumchlorid-Hexahydrat, 0,1 Gew.-% Magnesiumchlorid-Hexahydrat und 0,1 Gew.-% Calciumchlorid-Dihydrat in Wasser zubereitet.
- Beispiel 3
- Herstellung eines Strontium (II)-Komplexes von Ethylendiaminetetraessigsäure (SrEDTA)
- Eine Suspension von Strontiumcarbonat (1,0 g, 6,77 mmol) und Ethylendiaminetetraessigsäure (1,98 g, 6,77 mmol) in Wasser (25 ml) wurde bei 70°C für 30 Minuten gerührt. Die klare Lösung wurde bis zur Trockne eingeengt und im Vakuum bei Umgebungstemperatur getrocknet. Die Zielverbindung wurde als weißes, kristallines Material isoliert. Ausbeute 2,79 g (109%, berechnet als wasserfreies Produkt). Schmelzpunkt oberhalb von 250°C.
- Beispiel 4
- Herstellung des Strontium (II)-Komplexes von Ethylen-bis(oxyethylennitrilo)tetraessigsäure (SrEGTA)
- Eine Suspension von Strontiumcarbonat (1,0 g, 6,77 mmol) und Ethylen-bis(oxyethylennitrilo)tetraessigsäure (2,58 g, 6,77 mmol) in Wasser (25 ml) wurde bei 70°C für 6,5 Stunden gerührt. Die Lösung wurde beinahe klar. Die Lösung wurde bei Raumtemperatur gefiltert und das Filtrat wurde zur Trockne eingeengt und im Vakuum bei Umgebungstemperatur getrocknet. Die Verbindung wurde als weißes, kristallines Material isoliert. Ausbeute 1,54 g (49%).
- Beispiel 5
- Herstellung von Strontium (II)-Salicylat
- Eine Suspension von Strontiumcarbonat (1,0 g, 6,77 mmol) und Salicylsäure (1,87 g, 13,5 mmol) in Wasser (25 ml) wurde für 4 Stunden gerührt. Die Lösung wurde blassgelb und beinahe klar. Die Lösung wurde bei Raumtemperatur filtriert und das Filtrat wurde zur Trockne eingeengt und in Vakuum bei Umgebungstemperatur getrocknet. Die Zielverbindung wurde als blassrotes Pulver isoliert. Ausbeute 2,1 g (86%). Schmelzgrad oberhalb 300°C.
- Beispiel 6
- Herstellung des Strontium (II)-Komplexes von Diethylentriaminpentaessigsäure (SrDTPA)
- Eine Suspension von Strontiumcarbonat (1,0 g, 6,77 mmol) und Diethylentriaminpentaessigsäure (2,67 g, 6,77 mmol) in Wasser (25 ml) wurde bei 80°C für 19 Stunden gerührt. Die unlösliche Fraktion wurde bei Raumtemperatur abfiltriert und das Filtrat wurde zur Trockne eingeengt und im Vakuum bei Umgebungstemperatur getrocknet. Die Zielverbindung wurde als weißes/blassgelbes, kristallines Material isoliert. Ausbeute 1,6 g (49%). Schmelzgrad etwa 250°C.
- Die Zielverbindung wurde ebenso mit 51% Ausbeute durch ein ähnliches Verfahren mit Strontiumacetat anstelle von Strontiumcarbonat zubereitet.
- Beispiel 7
- Herstellung des Strontium (II)-Komplexes von L-Ascorbinsäure
- Eine Suspension von Strontiumcarbonat (1,0 g, 6,77 mmol) und L-Ascorbinsäure (2,39 g, 13,5 mmol) in Wasser (25 ml) wurde bei 80°C für 7 Stunden gerührt. Die Mischung wurde gelb. Die Mischung wurde bei Raumtemperatur filtriert, und das Filtrat wurde zur Trockne eingeengt und im Vakuum bei Umgebungstemperatur getrocknet. Die Zielverbindung wurde als gelbes Pulver isoliert. Ausbeute 2,50 g (78%). Schmelzpunkt etwa 250°C.
- Beispiel 8
- Herstellung des Strontium (II)-Komplexes von L-Ascorbinsäure-6-Palmitat
- Eine Lösung von Strontiumchlorid-Hexahydrat (0,32 g, 1,2 mmol) in Wasser (3 ml) wurde unter Rühren zu einer Lösung von L-Ascorbinsäure-6-Palmitat (1,0 g, 2,4 mmol) in Ethanol/Wasser (100 ml, 50:50, v/v) bei Raumtemperatur gegeben. Die Mischung wurde für 5 Minuten gerührt und die Zielverbindung wurde durch Filtration isoliert und durch Gefriertrocknung getrocknet. Ausbeute 0,514 g (47%). Weißes Pulver.
- Beispiel 9
- Herstellung des Strontiumibuprofensalzes
- Ibuprofen (2,59 g, 12,5 mmol) wurde in Wasser (100 ml), das Natriumhydroxid (0,503 g, 12,5 mmol) enthielt, gelöst. Eine Lösung von Strontiumchlorid-Hexahydrat (1,68 g, 6,3 mmol) in Wasser (5 ml) wurde zugegeben. Die Mischung wurde für 10 Minuten bei Raumtemperatur gerührt und die Zielverbindung wurde durch Filtration isoliert und getrocknet. Ausbeute 1,30 g (44%). Schmelzpunkt > 300°C.
- Beispiel 10
- Herstellung des Strontiumdiclofenacsalzes
- Diclofenac (0,35 g, 1,18 mmol) wurde in Wasser/Ethanol (30 ml, 50:50, v/v), das Natriumhydroxid (25 mg, 0,59 mmol) enthielt, gelöst. Eine Lösung von Strontiumchlorid-Hexahydrat (0,16 g, 0,59 mmol) in Wasser (3 ml) wurde zugegeben. Die Mischung wurde für 20 Minuten gerührt und die Zielverbindung wurde durch Filtration isoliert und getrocknet. Ausbeute 0,122 g (15%).
- Beispiel 11
- Herstellung von Strontiumstearat
- Stearinsäure (2,97 g, 10,4 mmol) wurde in Wasser/Ethanol (100 ml, 50:50 (Volumen)), das Natriumhydroxid (0,417 g, 10,4 mmol) enthielt, gelöst. Die Mischung wurde auf 70°C erhitzt und eine Lösung von Strontiumchlorid-Hexahydrat (1,39 g, 5,2 mmol) in Wasser (3 ml) wurde zugegeben. Die Zielverbindung wurde durch Zentrifugation des gebildeten Präzipitats isoliert. Ausbeute 1,6 g (46%).
- Beispiel 12
- Herstellung des SrEDTA-Dimegluminsalzes
- Strontium-EDTA (1 g, 2,65 mmol) (aus Beispiel 4) und N-Methyl-D-glucamin (1,03 g, 5,29 mmol) wurden in Wasser (10 ml) gelöst und bei 70°C für 30 Minuten gerührt. Die Mischung wurde filtriert und das Filtrat wurde zur Trockne eingeengt und in Vakuum bei Umgebungstemperatur getrocknet. Die Zielverbindung wurde als weißes, kristallines Material isoliert. Ausbeute 0,722 g (36%).
- Beispiel 13
- Herstellung von Strontiumbenzoat
- Strontiumcarbonat (1,0 g, 6,77 mmol) und Benzoesäure (1,65 g, 13,5 mmol) in Wasser (30 ml) wurden für 4 Stunden bei 70°C gerührt. Die Mischung wurde filtriert und das Filtrat wurde zur Trockne eingeengt und im Vakuum bei Umgebungstemperatur getrocknet. Die Zielverbindung wurde isoliert. Ausbeute 1,8 g (81%).
- Beispiel 14
- Herstellung von Strontiumglutarat
- Strontiumcarbonat (1,0 g, 6,77 mmol) und Glutarsäure (0,89 g, 6,77 mmol) in Wasser (30 ml) wurden über Nacht bei 70°C gerührt. Die Mischung wurde filtriert und das Filtrat wurde zur Trockne eingeengt und in Vakuum bei Umgebungstemperatur getrocknet. Die Zielverbindung wurde isoliert. Ausbeute 1,23 g (83%).
- Beispiel 15
- Herstellung des Strontiumalaninsalzes
- Strontiumhydroxid-Octahydrat (1,0 g, 3,79 mmol) und L-Alanin (0,67 g, 7,52 mmol) in Wasser (30 ml) wurden für 4 Stunden bei Raumtemperatur gerührt. Die Mischung wurde filtriert und das Filtrat wurde zur Trockne eingeengt und in Vakuum bei Raumtemperatur getrocknet. Ausbeute 0,81 g (63%).
- Beispiel 16
- Herstellung von Strontiumhippurat
- Strontiumcarbonat (0,5 g, 3,39 mmol) und Hippursäure (1,215 g, 6,77 mmol) in Wasser (30 ml) wurden bei 70°C für 5 Stunden gerührt. Die Mischung wurde filtriert und das Filtrat wurde zur Trockne eingeengt und im Vakuum bei Umgebungstemperatur getrocknet. Ausbeute 1,25 g (83%).
- Beispiel 17
- Herstellung eines Strontiumchelates mit 1,2-Di(2-aminoethoxy)ethan-N,N,N',N'-tetraessigsäure
- Strontiumcarbonat (1,0 g, 6,77 mmol) und 1,2-Di(2-aminoethoxy)ethan-N,N,N',N'-tetraessigsäure (2,58 g, 6,77 mmol) in Wasser (30 ml) wurden bei 85°C für 48 Stunden gerührt. Die Mischung wurde eingeengt und die Zielverbindung wurde im Vakuum bei Umgebungstemperatur getrocknet. Ausbeute 2,55 g (81%).
- Die Verbindungen der Beispiele 3 bis 17 können zur topischen Gabe in jeder praktischen Form (beispielsweise Gele, Cremes, Lösungen etc.) durch Verwendung konventioneller pharmazeutischer Träger und Hilfsstoffe formuliert werden.
- Beispiel 18
- Hautpenetrationszusammensetzung
- Eine strontiumhaltige Zusammensetzung wurde durch Lösen von 40 g Strontiumchlorid-Hexahydrat in 1000 ml Lösungsmittel hergestellt. Die Zusammensetzung des Lösungsmittels war:
50 Vol.-% destilliertes Wasser
25 Vol.-% Tetraglycol® (Glycofurol)
25 Vol.-% DMSO - Zwei Patienten mit Morbus Bechterew hatten nichtsteriodale antiinflammatorische Medikamente und Opioide ohne eine Linderung der Schmerzen in den Iliosacralgelenken verwendet. Die Zusammensetzung dieses Beispiels wurde dermal an den Iliosacralgelenken zwei- bis dreimal täglich verabreicht. Beide Patienten beobachteten eine komplette Linderung der Schmerzen.
- Beispiel 19
- Hydrogel mit Ibuprofen, Strontiumascorbat und einem Hautpenetrationsverbesserer
- Strontiumascorbat (900 mg) aus Beispiel 7 und Natriumlaurylsulfat (450 mg) wurden in Ibux-Gel 5% (hergestellt von Weifa AS, Oslo, Norwegen) unter Verwendung von Mörser und Pistill untergemischt. (Ibux-Gel enthält 5% Ibuprofen in einem Hydrogel mit Hydroxyethylcellulose, Benzylalkohol, Isopropanol, Natriumhydroxyl und gereinigtem Wasser). Das erhaltene Gel enthielt 1,2 Gew.-% Strontium.

- Creme mit Strontiumchlorid und einem Mittel zur Verbesserung der Hautpenetration
- Strontiumchlorid-Hexahydrat (1,2 g) und Natriumlaurylsulfat (0,3 g) wurden in Unguentum Merck (13,5 g) unter Verwendung von Möreser und Pistill untergemischt. Die Creme enthielt 2,6 Gew.-% Strontium in der Form von Strontiumchlorid.

- Salbe mit Hydrocortison und Strontiumstearat
- Strontiumstearat (60 mg) aus Beispiel 11 wurde in Hydrocortison 1%-Salbe (Galderma Nordic AB) unter Verwendung von Mörser und Pistill untergemischt (Hydrocortison 1% Salbe enthält 1% Hydrocortison, Propylenglycol, flüssiges Paraffin, Cetylalkohol und Vaseline®). Die erhaltene Salbe enthielt 1 Gew.-% Hydrocortison und 3 Gew.-% Strontiumstearat.
- Beispiel 22
- Klinische Erprobung der Zusammensetzung aus Beispiel 18
- Eine 35 Jahre alte Frau war 7 Jahre zuvor in einen Autounfall verwickelt und hatte schwere Schmerzen im Genick und den Schultern entwickelt. Die Patientin hatte nichtsteriodale antiinflammatorische Medikamente über einen langen Zeitraum ohne signifikanten Effekt verwendet. Seit dem Unfall hatte sie keine Nacht durchgehend geschlafen.
- Die Patientin testete die Zusammensetzung des Beispiels 18 durch topische Gabe in den Regionen des Schmerzes. Sie gab einen schmerzlindernden Effekt nach einer Minute an und hatte für die folgenden 2 bis 3 Tage keine oder sehr geringe Schmerzen. Nach Gabe der Zusammensetzung des Beispiels 18 war sie jede der beiden folgenden Nächte in der Lage, für etwa 10 Stunden zu schlafen.
- Beispiel 23
- Klinische Erprobung der Zusammensetzung des Beispiels 18
- Neun Boxer mit Schmerzen im Gesicht und/oder den Fingern haben den Effekt der Zusammensetzung des Beispiels 18 er probt. Die Zusammensetzung wurde direkt auf die schmerzenden Regionen gegeben. Eine sofortige Linderung des Schmerzes wurde in allen schmerzenden Regionen durch diese Boxer beobachtet. Die Schmerzreduktion hielt für eine lange Zeit an.
- Vier Boxer mit Schmerzen in der Nase und der Augenregion testeten die Effekte einer Zusammensetzung, die eine 2 Gew.-%ige Formulierung von Strontiumchlorid-Hexahydrat im gleichen Lösungsmittel wie in Beispiel 18 war (d. h. 50% der Strontiumkonzentration in Beispiel 18). Diese Formulierung zeigte ebenso gute klinische Effekte sowohl was die Schmerzlinderung, als auch die Reduktion der Schwellung der schmerzenden Region anging.

- Klinische Erprobung der Zusammensetzung des Beispiels 18
- Eine Patientin mit schmerzendem Beckenbereich als einem Ergebnis eines Bänderdefektes während der Schwangerschaft hatte eine Behandlung mit nichtsteroidalen antiinflammatorischen Medikamenten für einige Zeit ohne jeden Effekt versucht.
- Die topische Formulierung des Beispiels 18 wurde aufgetragen und resultierte in einer sofortigen Linderung der Schmerzen. Der Schmerzlinderungseffekt erstreckte sich über etwa 6 Stunden pro Anwendung.
Claims (15)
- Verwendung einer topischen Zusammensetzung mit einer physiologisch verträglichen Strontiumverbindung, einem physiologisch verträglichen Träger bzw. Carrier und einem physiologisch verträglichen Mittel zur Verbesserung der Hautpenetration für die Herstellung eines Medikaments zur Verwendung bei der Behandlung von Schmerz im subdermalen Weichgewebe bzw. subdermalem Weichgewebeschmerz.
- Verwendung nach Anspruch 1, bei der das Mittel zur Verbesserung der Hautpenetration Dimethylsulfoxid ist.
- Verwendung nach einem der Ansprüche 1 oder 2, bei dem die Strontiumverbindung Strontiumchlorid oder Strontiumnitrat ist.
- Verwendung nach einem der Ansprüche 1 oder 2, bei der die Strontiumverbindung Strontiumchlorid ist.
- Verwendung nach einem der Ansprüche 1 bis 4, bei der der Träger Wasser ist.
- Verwendung nach Anspruch 1, bei der die Zusammensetzung 40 g Strontiumchlorid-Hexahydrat pro 1000 ml Lösungsmittel enthält, wobei das Lösungsmittel 50% (Volumen) destilliertes Wasser, 25% (Volumen) Glycofurol und 25% (Volumen) Dimethylsulfoxid enthält.
- Verwendung nach einem der Ansprüche 1 bis 5 bei der Behandlung von chronischem subdermalem Weichgewebeschmerz.
- Verwendung nach einem der vorstehenden Ansprüche, bei der das Medikament auf die Oberfläche der Haut gegeben wird.
- Verwendung nach einem der Ansprüche 1 bis 5, 7 und 8, wobei das Weichgewebe Muskelgewebe bzw. ein Muskel ist.
- Zusammensetzung zur Verwendung als topisches Pharmazeutikum mit einer physiologisch verträglichen Strontiumzusammensetzung, einem physiologisch verträglichen Träger und einem physiologisch verträglichen Mittel zur Verbesserung der Hautpenetration, welches Dimethylsulfoxid ist.
- Zusammensetzung nach Anspruch 10 zur Schmerzlinderung.
- Zusammensetzung nach Anspruch 10 oder Anspruch 11, bei der die Strontiumverbindung Strontiumchlorid oder Strontiumnitrat ist.
- Zusammensetzung nach Anspruch 10 oder Anspruch 11, bei der die Strontiumverbindung Strontiumchlorid ist.
- Zusammensetzung nach einem der Ansprüche 10 bis 13, bei der der Träger Wasser ist.
- Schmerzlindernde topische pharmazeutische Zusammensetzung, mit 40 g Strontiumchlorid-Hexahydrat pro 1000 ml Lösungsmittel, wobei das Lösungsmittel 50% (Volumen) destilliertes Wasser, 25% (Volumen) Glycofurol und 25% (Volumen) Dimethylsulfoxid aufweist.
