Description
- Hintergrund der Erfindung
- Die vorliegende Erfindung bezieht sich allgemein auf chirurgische Instrumente und bezieht sich insbesondere auf ein Ablationssystem zur Behandlung von Vorhofflimmern und anderen Erkrankungen bzw. Störungen.
- Bei Patienten mit chronischem Vorhofflimmern mit Tachykardie, die auf medizinische Behandlung nicht anspricht, ist die Maze-Operation bzw. -Behandlung durchgeführt worden. Dieser Eingriff steuert die Ausbreitung der Depolarisations-Wellenfronten im rechten und linken Vorhof mittels chirurgischer Schnitte durch die Wände des rechten und linken Vorhofs. Diese Schnitte schaffen Blind- oder Sackgassen-Leitungswege, die das Auftreten von Kreiserregungs-Vorhofflimmern verhindern. Die Maze-Operation ist zwar bei der Behandlung von Vorhofflimmern erfolgreich, jedoch ist der Eingriff sehr kompliziert und wird derzeit nur von wenigen sehr erfahrenen Herzchirurgen in Verbindung mit anderen Eingriffen am offenen Herzen durchgeführt. Der Eingriff ist für das Herz auch sehr traumatisch, da im wesentlichen der rechte und der linke Vorhof zerschnitten und wieder zusammengenäht werden, so dass Läsionen definiert werden, über die hinweg sich keine Depolarisations-Wellenfronten ausbreiten.
- Es wurde vorgeschlagen, Eingriffe ähnlich der Maze-Operation stattdessen mittels Elektroablation durchzuführen, beispielsweise durch Aufbringen von RF- bzw. HF-Energie auf die Innen- oder Außenoberfläche des Vorhofs, um Läsionen zu erzeugen, über die sich Depolarisations-Wellenfronten nicht ausbreiten. Solche Verfahren sind im US-Patent Nr. 5,895,417, erteilt für Pomeranz et al., US-Patent Nr. 5,575,766, erteilt für Swartz et al., US-Patent Nr. 6,032,077, erteilt für Pomeranz, US-Patent Nr. 6,142,944, erteilt für Swanson et al. und US-Patent Nr. 5,871,523, erteilt für Fleischman et al. offenbart.
- Vorrichtungen nach Art einer Gefäßklemme für elektrochirurgische oder Kälteablation zur Verwendung bei solchen Eingriffen sind im US-Patent Nr. 5,733,280 erteilt für Avitall, US-Patent Nr. 6,237,605 erteilt für Vaska et al, US-Patent Nr. 6,161,543 erteilt für Cox et al., in der offengelegten PCT-Anmeldung Nr. WO99/59486 von Wang und in dem anhängigen US-Patent Nr. 6,546,935, eingereicht am 22. Dezember 2000 von Hooven et al. beschrieben.
- Die WO-A-01/05306 offenbart ein Ablationssystem, umfassend:
ein erstes langgestrecktes Ablationselement, welches eine sich in Längsrichtung erstreckende erste Einrichtung zur Abgabe von Ablationsenergie hält; ein zweites langgestrecktes und bezüglich des ersten Ablationselements bewegliches Ablationselement; und an dem ersten und zweiten Element angebrachte Einrichtungen zur magnetischen Anziehung des ersten und des zweiten Ablationselements zueinander zur Abgabe von Ablationsenergie. - Damit solche Eingriffe wirksam sind, ist es vorteilhaft, dass die elektrochirurgisch erzeugten Läsionen über ihre Länge kontinuierlich verlaufen und sich vollständig durch das Herzgewebe erstrecken. Das gleiche Problem tritt bei dem Versuch auf, kontinuierliche Läsionslinien durch die Wände anderer Herzkammern oder anderer Organe zu erzeugen.
- Zusammenfassung der Erfindung
- Die Erfindung stellt ein Ablationssystem bereit, umfassend:
ein erstes langgestrecktes Ablationselement, welches eine sich in Längsrichtung erstreckende erste Einrichtung zur Abgabe von Ablationsenergie hält; ein zweites langgestrecktes und bezüglich des ersten Ablationselements bewegliches Ablationselement; und an dem ersten und dem zweiten Element angebrachte und sich entlang dieser erstreckende Einrichtungen zur magnetischen Anziehung des ersten und des zweiten Ablationselements zueinander entlang der Länge der ersten Einrichtung zur Abgabe von Ablationsenergie; wobei das erste Element manuell formbar ist und das zweite Element so flexibel ist, dass es mittels der Anziehungseinrichtung verbogen und dadurch an dem ersten Element ausgerichtet werden kann. - Mit einem erfindungsgemäßen System können langgestreckte Läsionen erzeugt werden, wie sie bei einem Maze-Eingriff oder einem anderen Eingriff erforderlich sind, mittels eines Satzes zweier langgestreckter Ablationselement-Haltevorrichtungen (z.B. eine Elektrode oder Elektroden) zum Aufbringen von Ablationsenergie (z.B. HF-Energie) über ihre Länge. Die Ablationselemente sind auf entgegengesetzten Seiten der Wände des Vorhofs oder anderer Hohlorgane anzuordnen, jeweils auf einer Seite der Organwände, um das Gewebe zwischen den Elementen abzutragen oder Läsionen darin zu erzeugen. Die Ablationselemente können auch auf entgegengesetzten Außenflächen eines Organs angeordnet werden, beispielsweise auf entgegengesetzten Seiten einer Auricula cordis oder entlang entgegengesetzter Seiten des Gewebes neben der Basis der rechten oder linken Pulmonalvene.
- Die Ablationselemente sind mit einem Magnetsystem versehen, welches die Elemente zueinander zieht, so dass diese die Wand bzw. die Wände eines dazwischenliegenden Vorhofs oder anderen Hohlorgans längs des Elements zusammendrücken. Bei diesen Systemen ist wenigstens ein Element mit einem Magneten oder einer Reihe von Magneten entlang des Elements versehen. Das andere Element ist mit einem ferromagnetischen Element oder vorzugsweise einem anderen Magneten oder einer Reihe von Magneten versehen, die sich entlang des Elements erstrecken, wobei die Polaritäten entsprechend gewählt sind, um eine Anziehung zwischen den beiden Elementen sicherzustellen. Der Magnet bzw. die Magnete können starr oder biegsam sein und können aus magnetischem Werkstoff bestehen, z.B. Seltenerdmagnete, oder alternativ auch Elektromagnete.
- Bei einer bevorzugten erfindungsgemäßen Ausführungsform weisen die beiden Elemente gegenüberliegende Backen einer elektrochirurgischen Gefäßklemme auf, sind mit langgestreckten HF-Elektroden versehen sowie gerade oder gekrümmt gestaltet. Erfindungsgemäß ist die eine Backe formbar, so dass der Arzt die gewünschte Gestalt wählen kann, wobei die andere Backe flexibel ist. Das Magnetsystem erlaubt es der flexiblen Backe, automatisch eine der formbaren Backe entsprechende Gestalt anzunehmen.
- Es können ähnlich ausgeführte Einheiten vorgesehen sein, wobei die beiden Elemente voneinander separat, beispielsweise an separaten Griffen angebracht sind. Alternativ kann ein erstes Außenelement an einem Griff angebracht sein, den der Arzt hält, während ein zweites Innenelement auf einem perkutan eingeführten Katheter angeordnet sein kann. Bei diesen Ausführungsformen wäre das Innenelement typischerweise äußerst flexibel, während das Außenelement formbar wäre. Bei diesen Ausführungsformen kann mittels des Magnetsystems das Innenelement automatisch eine dem Außenelement entsprechende Gestalt annehmen, nachdem das Innenelement in das Hohlorgan eingeführt wurde.
- Bevorzugte Ausführungsformen werden nun ausschließlich beispielhaft mit Bezug auf die Zeichnungen beschrieben.

1 ist eine Draufsicht einer bipolaren elektrochirurgischen Gefäßklemme einer Art, bei welcher die vorliegende Erfindung zweckmäßigerweise eingesetzt werden kann. Die Gefäßklemme ist mit Griffen14 und12 versehen, die mit den schwenkbaren Backen16 bzw.18 verbunden sind. Entlang der Backen16 und18 befinden sich Ablationselektroden20 und22 , welche wie unten erörtert als HF-Elektroden ausgebildet sind. Bei alternativen Ausführungsformen können die Elektroden20 und22 zum Aufbringen von Mikrowellenstrahlung eingesetzt werden, oder sie können durch langgestreckte Heiz- oder Kühlelemente ersetzt werden, um über deren Länge Wärme- oder Kälteablation bereitzustellen. Bei der dargestellten Ausführungsform sind die Elektroden gespülte HF-Elektroden, welche die Zuführung von Salzlösung oder anderen leitenden Fluiden über deren Länge ermöglichen, im wesentlichen gemäß dem in US-Patent Nr. 6,096,037 von Mulier beschriebenen Mechanismus. Jede Elektrode ist mit einem Fluidzuführlumen30 ,32 versehen, durch welches die Salzlösung oder anderes leitendes Fluid den Elektroden zugeführt wird. Die Lumina30 und32 sind mit einem Luer-Anschlussstück34 verbunden, welches an eine Quelle für leitendes Fluid anschließbar ist. Alternativ können separate Luer-Anschlussstücke für jeweils ein Lumen30 ,32 vorgesehen sein. Gleichermaßen ist jede Elektrode mit Leitern24 ,26 versehen, zur Herstellung einer Verbindung der Elektroden mit einer Quelle für Ablationsenergie über den Elektroanschluss28 , wie oben angegeben. Die Ablationdenergiequelle kann HF-Energie oder Mikrowellenenergie liefern. Bei alternativen Ausführungsformen, wobei die Elektroden20 und22 durch Heizelemente ersetzt werden, können anstelle von Fluidzuführlumen Elektroleiter24 und26 mit zwei langgestreckten Widerstandsheizelementen vorgesehen sein, die auf den Backen16 und18 angeordnet und über den Anschluss28 an eine Stromquelle angeschlossen sind. Bei alternativen Ausführungsformen, wobei langgestreckte Kühlelemente anstelle der Elektroden20 und22 verwendet werden, kann Kühlfluid über die Fluidlumina30 und32 den Elektroden zugeführt werden, oder alternativ, falls elektrische Kühleinrichtungen vorgesehen sind, kann durch den Elektroanschluss28 über die Anschlüsse24 und26 den Kühleinrichtungen Strom zugeführt werden. - Die nachfolgende Erörterung ist zwar auf Ablationssysteme gerichtet, bei denen die zugeführte Ablationsenergie insbesondere HF-Energie ist, die über gespülte Elektroden zugeführt wird, jedoch versteht sich, dass die vorliegende Erfindung zweckmäßigerweise in Verbindung mit den anderen oben erwähnten Arten von Ablationsenergie eingesetzt werden kann. Demnach sind in der nachfolgenden Erörterung die dargestellten und beschriebenen gespülten HF-Elektroden als beispielhaft für einen Mechanismus zum Aufbringen von Ablationsenergie gemäß der vorliegenden Erfindung anzusehen, nicht als Beschränkung darauf.
- Die Backen
16 und18 können gerade gestaltet sein wie dargestellt, oder können gekrümmt sein. Die Backen16 und18 bestehen vorzugsweise aus einem nicht-ferromagnetischen Werkstoff wie biokompatiblem Kunststoff, und wie unten erörtert halten sie einen langgestreckten Magneten oder eine Reihe von Magneten, die sich entlang der Elektroden20 und22 erstrecken und bei der Ausrichtung der Elektroden zueinander auf gegenüberliegenden Seiten des zu abladierenden Gewebes mitwirken und beim Zusammendrücken von Gewebe zwischen den Elektroden mitwirken, so dass auf der ganzen Länge guter Kontakt sichergestellt ist. -
2A mit2G stellen verschiedene erfindungsgemäße alternative Ausführungsformen dar, wobei verschiedene Magnetausrichtungssysteme und verschiedene Ausgestaltungen der ersten und der zweiten Komponente eingesetzt werden (in diesem Fall die Backen16 und18 ), entlang derer Ablationsenergie aufzubringen ist.2A ist eine Querschnittsansicht durch die Backen16 und18 der Gefäßklemme aus1 , wobei die Elektroden20 und22 als langgestreckte Elektrodenspulen100 bzw.102 ausgebildet sind und durchlässige Innenrohre104 und106 halten. Die Rohre104 und106 können beispielsweise aus durchlässigem Polytetrafluorethylen (PTFE) bestehen und ihre Innenlumen können mit den in1 dargestellten Fluidlumen30 und32 in Verbindung stehen. Mittels dieses Mechanismus kann die Abgabe eines leitenden Fluids, z. B. einer Salzlösung entlang der Elektrodenspulen101 und102 bewirkt werden. Wie beschrieben, umfassen die Elektroden20 und22 zwar jeweils eine einzige langgestreckte Elektrodenspule, jedoch liegen Ausführungsformen, bei denen die Elemente (Backen16 und18 ) mit Mehrfachelektroden über ihre Länge versehen sind, ebenso im Umfang der vorliegenden Erfindung. - Wie dargestellt, sind die Backen
16 und18 jeweils mit einem Magnetpaar oder einer Reihe von Magneten108 ,110 ,112 ,114 versehen, welche sich entlang der Backen16 und18 erstrecken. Diese im Querschnitt gezeigten Magnete können entweder einzelne langgestreckte Magnete oder eine Reihe kürzerer Magnete sein, die sich entlang der Backen erstrecken. Die Polarität von Magneten entspricht der Kennzeichnung "N" bzw. "S" wie dargestellt, und zwar in einer solchen Anordnung, dass die Backen16 und18 jeweils über ihre Länge zueinander angezogen werden. Das Bereitstellen von Magneten zu beiden Seiten der Elektroden18 und20 bewirkt mit, dass die Elektroden sich zueinander zentrieren, so dass die Elektroden direkt gegenüber einander zu liegen kommen, wenn sie auf gegenüberliegenden Seiten des zu abladierenden Gewebes angeordnet werden. Die Magnete unterstützen auch das Zusammenpressen der Backen der Gefäßklemme über ihre Länge und stellen somit guten Kontakt mit dem Gewebe über die Länge der Backen sicher. - Die Backen
16 und18 bestehen vorzugsweise aus einem nicht-ferromagnetischem Werkstoff wie beispielsweise Kunststoff, um die Magnete und die Elektrodenspulen wie gezeigt voneinander isolieren zu können. - Eine der beiden Backen
16 und18 kann durch den Arzt in eine gewünschte Gestalt geformt werden, wobei die jeweils andere Backe flexibel ist. Die flexible Backe wird parallel zu der formbaren Backe ausgerichtet und gestaltet, wenn die beiden Backen jeweils auf einer Seite der Wand des zu abladierenden Hohlorgans aufeinander zu genähert werden. Die formbare Backe kann aufgrund des Werkstoffs, aus dem die Backe besteht, verformbar sein, oder mittels eines formbaren Einsatzes, beispielsweise eines langgestreckten Stabs aus Nitinol, nichtrostendem Stahl, oder einem anderen in2A nicht dargestellten formbaren Metall. -
2B stellt eine erfindungsgemäße alternative Ausführungsform dar und zeigt in ähnlicher Weise einen Querschnitt durch die Backen16 und18 der Gefäßklemme aus1 . Alle Elemente entsprechen den Elementen mit denselben Bezugszeichen in2A . Bei dieser Ausführungsform ist für die beiden Backen16 bzw.18 jeweils nur ein einziger langgestreckter Magnet oder eine Reihe von Magneten116 ,118 vorgesehen. Diese Ausgestaltung erlaubt eine reduzierte Gesamtgröße der Backe, funktioniert jedoch ansonsten wie im Zusammenhang mit2A beschrieben. In2B ist ein optionaler Metallformdraht120 gezeigt, der neben dem bzw. den Magneten118 angebracht ist, damit der Arzt die Backe18 formen kann. Bei Ausführungsformen mit diesem Formdraht ist es naheliegend, dass die Backe16 flexibel ist und nach dem Platzieren der Backen auf gegenüberliegenden Seiten des zu abladierenden Gewebes der Form entspricht, mit welcher der Arzt die Backe18 versehen hat. -
2C zeigt eine dritte erfindungsgemäße alternative Ausführungsform, die ebenso als Querschnitt durch die Backen16 und18 der Gefäßklemme aus1 dargestellt ist. Identisch nummerierte Elemente entsprechen den in2A dargestellten. Bei dieser Ausführungsform sind langgestreckte Magnete oder Magnetreihen122 und124 in den durchlässigen Fluidlumen106 und104 angeordnet, so dass magnetische Kraft, die aufgebracht wird, um die Backen16 und18 zueinander zu ziehen, in der Mitte zwischen den Elektrodenspulen100 und102 aufgebracht wird. Die verschiedenen im Zusammenhang mit den2A und2B oben erörterten alternativen Ausführungsformen können dementsprechend so vorgesehen sein, dass die Backen allgemein die in2C dargestellte Gestalt aufweisen. - Wie in den
2A ,2B und2C dargestellt, sind die Magnete so angeordnet, dass der (die) Magnetsüdpol(e) einer Backe zu dem (den) Magnetnordpol(en) der anderen Backe benachbart sind. Diese Ausgestaltung ist besonders vorteilhaft in Verbindung mit Ausführungsformen, bei denen jeweils ein langgestreckter Magnet vorgesehen ist, der sich im wesentlichen über die Länge der Backen erstreckt, und ebenso bei Ausführungsformen, bei denen eine Reihe kürzerer, gering beabstandeter Magnete vorgesehen ist, die sich entlang der Backen erstrecken. Bei Ausführungsformen, bei denen sich Magnete längs der Backe erstrecken, jedoch weiter voneinander beabstandet sind, kann die Magnetpolarität geändert werden, so dass sich längs der einen Backe am distalen Ende der Magnete die magnetischen Nordpole befinden und die Südpole an den proximalen Enden, wobei sich auf der anderen Backe die magnetischen Südpole jeweils am distalen Ende und die magnetischen Nordpole jeweils am proximalen Ende befinden. Alternative Magnetausgestaltungen dieser Art sind bei jeder der in den2A ,2B und2C dargestellten Ausführungsformen einsetzbar, wobei die Magnete als eine Reihe beabstandeter Magnete entlang der Länge der Backen ausgebildet sind. - Die Magnete selbst können aus jedem geeigneten magnetischen Werkstoff bestehen. Eine besonders vorteilhafte magnetische Werkstoffart zur erfindungsgemäßen Verwendung sind beispielsweise Seltenerdmagnete, wegen ihrer außergewöhnlichen Festigkeit bei relativ geringen Größen und Gewichten. Jedoch können stattdessen langgestreckte flexible Magnete eingesetzt werden sowie keramische Magnete. Wie unten ausführlicher erörtert wird, können die Magnete außerdem durch elektromagnetische Spulen ersetzt werden. Bei weiteren alternativen Ausführungsformen ist es denkbar, Magnete nur an einer Backe anzuordnen, wobei ein ferromagnetischer Werkstoff wie magnetischer nichtrostender Stahl anstelle des anderen Magneten eingesetzt wird. Beispielsweise bei der in
2A dargestellten Ausführungsform können die Magnete108 und110 durch langgestreckte Magnetelemente aus nichtrostendem Stahl ersetzt werden. Bei einer solchen Ausführungsform würden die langgestreckten nichtrostenden Stahlelemente von den Magneten112 und114 angezogen werden, wie unten beschrieben, und sind auch dazu einsetzbar, um der Backe116 die Verformbarkeit in die vorgesehene Gestalt zu verleihen. Die Verwendung nicht-magnetisierter ferromagnetischer Werkstoffe statt der in2B und2C dargestellten magnetischen Werkstoffe soll ebenso in dem Umfang der vorliegenden Erfindung enthalten sein. -
2D ist ein Längsschnitt durch die Backe18 der Gefäßklemme aus1 . Bei dieser Ausführungsform sind die Magnete112 und114 als Magnetreihe ausgebildet und innerhalb der Backe18 angeordnet. Die Elektrodenspule102 und das Fluidlumen106 sind ebenfalls im Längsschnitt dargestellt. -
2E zeigt einen alternativen Längsschnitt durch die Backe18 , die im übrigen der Darstellung in den1 und2A entspricht. Die Komponenten entsprechen den Komponenten mit denselben Bezugszeichen in2A . Bei dieser Ausführungsform ist jedoch die Backe18 mit Einkerbungen126 zwischen den einzelnen Magneten114 und112 versehen. Diese Einkerbungen stellen in Verbindung mit dem flexiblen Material der Backe18 Gelenkpunkte dar, die ein Verbiegen der Backe18 erleichtern. Diese Ausgestaltung ist besonders vorteilhaft für den Fall, dass die Backe16 wie in den1 und2A dargestellt formbar ausgebildet ist, wobei die Backe18 genügend flexibel ist, um die Gestalt der Backe16 anzunehmen, wenn sie auf der anderen Seite des zu abladierenden Gewebes platziert wird. -
2F ist ein Längsschnitt durch eine Gefäßklemme mit einer Backe mit der in2C dargestellten Gestalt. Die Komponenten entsprechen Komponenten mit denselben Bezugszeichen in2C . Bei dieser Ansicht ist der Magnet122 als innerhalb des Fluidlumens104 angeordnete Magnetreihe ausgebildet. -
2G ist ein Längsschnitt durch eine erfindungsgemäße Ausführungsform mit einer Backe mit der in2B dargestellten Gestalt. Bei dieser Ausführungsform ist der Magnet118 als entlang des Formdrahtes120 angeordnete Reihe von Magneten118 ausgebildet. Die Elektrodenspule102 und das Fluidlumen104 sind ebenfalls zu sehen. - Bei den Ausführungsformen der
2D ,2F und2G versteht sich, dass langgestreckte, kontinuierliche, flexible oder starre Magnete anstelle der dargestellten Reihe von Einzelmagneten verwendet werden können. Außerdem versteht sich, dass bei einigen Ausführungsformen die dargestellten Magnete weiter voneinander beabstandet und so angeordnet sein können, dass ihre Nord-Süd-Magnetachse sich jeweils längs der Backen erstreckt, wie oben in Verbindung mit den2A bis2C beschrieben. Bei diesen Ausführungsformen würde die Nord-Süd-Magnetachse der Magnete in einer Backe derjenigen der Magnete in der anderen Backe gegenüberliegen. Backen mit dieser Magnetanordnung können auch in Verbindung mit einer Backe oder einem anderen Ablationselement verwendet werden, welche(s) als Elektromagnetreihe ausgebildet ist, beispielsweise Spulen, deren Achsen sich entlang der jeweiligen Achse der Backe oder des anderen Ablationselements erstrecken. -
3 ist eine perspektivische Ansicht einer bipolaren elektrochirurgischen Gefäßklemme einer zweiten Art, welche zur Verwendung in Verbindung mit der vorliegenden Erfindung geeignet ist. Bei dieser Ausführungsform ist die Gefäßklemme mit Griffen212 und214 und langgestreckten Backen216 und218 versehen. In diesem Fall hält die Backe218 ein kreisrundes Ablationselement238 , entlang dem eine Elektrode220 angeordnet ist. Die Backe216 ist mit einem hakenförmigen Ablationselement236 versehen, welches eine der Elektrode220 gegenüberliegende korrespondierende Elektrode hält. Das Instrument aus3 ist insbesondere für Ablationen und zum Umringen der Basis der Pulmonalvenen ausgelegt, im Zusammenhang mit elektrochirurgischen Eingriffen entsprechend der oben erörterten Maze-Operation. Bei dieser Ausführungsform ist das kreisrunde Ablationselement238 durch den Arzt formbar, um eine Anpassung der Gestalt des Elements an die individuelle Anatomie des betroffenen Patienten zu ermöglichen. Das Element236 ist vorzugsweise wenigstens so flexibel, dass es leicht gespreizt werden kann, um ein Platzieren der Backe um die Basis der Pulmonalvenen zu erleichtern, und kann sehr flexibel sein, so dass in Abhängigkeit von der magnetischen Anziehung zwischen den Elementen236 und238 das Element236 eine zu dem Element238 parallele Gestalt annimmt. Wie bei der in1 dargestellten Gefäßklemme sind die Fluidlumina230 und232 zur Abgabe eines leitenden Fluids an die Elektroden über das Luer-Anschlussstück234 ausgelegt. Die elektrischen Leiter224 und226 sind zum Zuführen elektrischer Energie zu den Elektroden über die Elektroanschlüsse228 ausgelegt. Wie oben in Verbindung mit der Gefäßklemme aus1 erörtert, können alternative Mittel zum Aufbringen von Ablationsenergie wie Mikrowellenantennen oder Heiz- oder Kühlelemente zur Verabreichung von thermischer oder Kälteablation anstelle der Elektroden vorgesehen sein. -
4 zeigt eine weitere alternative erfindungsgemäße Ausführungsform, wobei die beiden Ablationselemente voneinander getrennt sind statt zusammenhängend wie bei den Gefäßklemmen aus den1A und3 . Bei dieser Ausführungsform entspricht das erste Element allgemein der Backe216 der Gefäßklemme aus3 , in diesem Fall mit einem Griff312 versehen, mittels dem der Arzt die Vorrichtung handhaben kann. Eine Elektrode320 erstreckt sich um das gekrümmte Ablationselement318 und kann wie oben erörtert eine gespülte elektrochirurgische Elektrode sein, die über das Lumen332 und das Luer-Anschlussstück334 mit Fluid versorgt wird und über die Leiter326 und den Elektroanschluss328 mit elektrischem Strom versorgt wird. Beim Gebrauch wird das gekrümmte Ablationselement318 auf der Außenoberfläche des zu abladierenden Organs platziert, beispielsweise um die Basis der Pulmonalvenen eines Patienten. Bei dieser speziellen Ausführungsform ist das gekrümmte Ablationselement318 verformbar, so wie das innere Ablationselement304 wie unten erörtert sehr flexibel ist. - Das innere Ablationselement
304 ist als Katheter ausgebildet und hat einen langgestreckten Katheterkörper414 , der längs seines distalen Abschnitts420 eine Elektrode hält. Der Aufbau des distalen Abschnitts420 kann allgemein dem Aufbau der Backen der Gefäßklemme gemäß der Darstellung in den2A bis2G entsprechen, unter der Voraussetzung, dass der distale Abschnitt420 des Katheters aus einem genügend flexiblen Material hergestellt ist, so dass er perkutan eingeführt und zu der vorgesehenen Stelle in dem zu abladierenden Organ geschoben werden kann. Beispielsweise könnte der Katheter durch das Gefäßsystem in den linken Vorhof hinein geschoben werden bis zu einer Position neben den Öffnungen in die Pulmonalvenen. Alternativ, wie in den5A bis D unten dargestellt, kann der distale Abschnitt420 des Katheters speziell zum Anordnen an dem distalen Abschnitt eines Katheters optimiert sein. Wie dargestellt, ist das proximale Ende des Katheters mit einem Anschlussstück416 versehen, das eine Fluidkupplung434 hält, die das Zuführen einer Salzlösung oder eines anderen leitenden Fluids zur Elektrode ermöglicht, die entlang des distalen Abschnitts420 des Katheters angeordnet ist. Elektrischer Strom wird der Elektrode mittels des Leiters426 und des Anschlussstücks428 zugeführt, entsprechend der oben für die anderen Ausführungsformen beschriebenen Weise. - Die
5A –5D zeigen verschiedene alternative Ausgestaltungen für den distalen Abschnitt420 des in4 dargestellten Katheters304 . Die in den5A –5D dargestellten erfindungsgemäßen Ausführungsformen können auch bei externen Ablationselementen verwendet werden, wie in4 dargestellt, oder bei Gefäßklemmvorrichtungen wie in den1 und3 dargestellt. -
5A ist ein Querschnitt durch den distalen Abschnitt420 des in4 dargestellten Katheters und zeigt eine erste erfindungsgemäße Ausführungsform, die insbesondere zur Verwendung als Teil eines perkutan eingeführten Katheters optimiert ist. Bei dieser Ausführungsform weist die Außenfläche des distalen Abschnitts ein durchlässiges Rohr404 auf, das wie oben erörtert aus PTFE bestehen kann und eine Elektrodenspule402 umgibt. Ein Magnet oder eine Reihe von Magneten406 ist in dem Lumen der Elektrodenspule402 angebracht. Bei dieser Ausführungsform wird durch das Lumen der Elektrodenspule402 ein Fluid zugeführt, welches in die durchlässige Wand des Rohrs404 eindiffundiert, und der durch die Elektrode402 zugeführte elektrische Strom steht mittels des leitenden Fluids in der Wand und auf der Oberfläche des Rohrs404 mit dem zu abladierenden Gewebe in Verbindung. Wie dargestellt, weist die Elektrode eine solche Magnetpolarität auf, dass ihre Nord-Süd-Achse quer zur Katheterachse verläuft. Jedoch liegen alternative Ausführungsformen unter Verwendung einer Reihe beabstandeter Magnete, deren Nord-Süd-Achse entlang der Katheterachse verläuft, ebenfalls im Umfang der Erfindung. -
5B zeigt einen alternativen Querschnitt durch den distalen Abschnitt420 des Katheters aus4 . Nummerierte Elemente entsprechen den Elementen mit denselben Bezugszeichen in5A . Bei dieser Ausführungsform ist jedoch ein Formdraht410 gezeigt, mittels dem der Arzt den distalen Abschnitt420 des Katheters beliebig gestalten kann. Beispielsweise kann der Katheter in einer im wesentlichen kreisrunden Gestalt vorgespannt sein, die während des Durchführens des Katheters durch das Gefäßsystem ausgestreckt wird, wobei er mittels des Formdrahtes410 in seine vorgegebene Gestalt zurückgestellt wird, sobald er nicht mehr durch das Gefäßsystem gehalten wird. -
5C zeigt einen weiteren alternativen Querschnitt durch den distalen Abschnitt420 des Katheters aus4 . Nummerierte Elemente entsprechen den Elementen mit denselben Bezugszeichen in5A . Bei dieser Ausführungsform ist die Spule412 jedoch keine Ablationselektrode, sondern wird stattdessen als Elektromagnet verwendet, um den Katheter zu einem zugehörigen externen Ablationselement anzuziehen. Die Abgabe von Ablationsenergie, z.B. HF oder Mikrowelle, wird durch den Mitteldraht418 bewirkt. -
5D ist ein Längsschnitt durch den distalen Abschnitt420 eines Katheters mit dem in5C dargestellten Querschnitt. Nummerierte Elemente entsprechen den Elementen mit denselben Bezugszeichen in5C . Bei dieser Ansicht ist zu sehen, dass die Spule412 eine von einer Reihe beabstandeter Elektromagnetspulen ist, die entlang des distalen Abschnitts420 des Katheters beabstandet angeordnet sind. Wie dargestellt, sind die Spulen412 in Reihe geschaltet, jedoch können sie bei alternativen Ausführungsformen für individuelle Betätigung geschaltet sein.
Claims (13)
- Ablationssystem, umfassend: ein erstes langgestrecktes Ablationselement (
14 ,16 ), welches eine sich in Längsrichtung erstreckende erste Einrichtung (20 ) zur Abgabe von Ablationsenergie trägt; ein zweites langgestrecktes und bezüglich des ersten Ablationselements bewegliches Ablationselement (12 ,18 ); und an dem ersten und zweiten Element angebrachte und sich entlang dieser erstreckende Einrichtungen (108 ,110 ,112 ,114 ) zur magnetischen Anziehung des ersten und des zweiten Ablationselements zueinander entlang der Länge der ersten Einrichtung zur Abgabe von Ablationsenergie; wobei das erste Element manuell formbar ist und das zweite Element ausreichend flexibel ist, so dass es mittels der Anziehungseinrichtung verbogen bzw. abgelenkt und dadurch bezüglich des ersten Elements ausgerichtet werden kann. - System gemäß Anspruch 1, wobei das zweite langgestreckte Ablationselement eine sich in Längsrichtung erstreckende zweite Einrichtung (
22 ) zur Abgabe von Ablationsenergie trägt. - System gemäß Anspruch 1 oder Anspruch 2, wobei die Anziehungseinrichtung einen Magneten umfasst welcher an einem der ersten oder zweiten Elemente angebracht ist.
- System gemäß Anspruch 3, wobei der Magnet einen Seltenerdmagneten aufweist.
- System gemäß Anspruch 3, wobei der Magnet einen Elektromagneten aufweist.
- System gemäß Anspruch 1 oder Anspruch 2, wobei die Anziehungseinrichtung Magneten aufweist, welche an beiden, der ersten und zweiten Elemente angebracht sind.
- System gemäß einem der vorangehenden Ansprüche, wobei das erste und das zweite Element miteinander verbunden sind.
- System gemäß Anspruch 7, wobei das erste und das zweite Element gelenkig miteinander verbunden sind.
- System gemäß Anspruch 7, wobei das erste und das zweite Element an den Backen einer elektrochirurgischen Gefäßklemme angebracht sind.
- System gemäß einem der Ansprüche 1 bis 6, wobei das erste und das zweite Element nicht miteinander verbunden sind.
- System gemäß Anspruch 10, wobei entweder das erste oder das zweite Element ein Katheter ist.
- System gemäß einem der vorangehenden Ansprüche, wobei entweder das erste oder das zweite Element mit einer vorgeformten Krümmung ausgebildet ist.
- System gemäß einem der vorangehenden Ansprüche, wobei das erste und das zweite Element beide mit einer vorgeformten Krümmung ausgebildet sind.
