Claims (28)
- 患者の左心房内の組織の非線状領域を含むターゲット組織をアブレートする方法において、
組織係合部分を有するアブレータを備えるアブレーション装置を選ぶステップと、
患者の胸腔に侵入するステップと、
ターゲット組織を識別するステップと、
組織係合部分がアブレーションエネルギをターゲット組織に伝達することができるようにアブレーション装置をターゲット組織に隣接する位置に配置するステップと、
組織係合部分をアブレーションエネルギにて励起して左心房内の組織の非線状領域にフートプリントを形成し且つ左心房内の励起可能な組織の全質量を減少させるステップとを備える、ターゲット組織をアブレートする方法。 - 請求項1に記載の方法において、高周波エネルギ、熱エネルギ、低温エネルギ、化学的エネルギ、薬理学的エネルギ、超音波エネルギ、マイクロ波エネルギ、レーザエネルギ及び放射線エネルギの少なくとも1つを使用するアブレーション装置を選ぶステップを更に備える、方法。
- 請求項1に記載の方法において、バルーン、金網、パッチ、ロール状電極、ファン、双極電極、ワイパー及びクリスタル送信機の少なくとも1つを含むアブレーション装置を選ぶステップを更に備える、方法。
- 請求項1に記載の方法において、剣状突起下切開、肋骨下切開、胸骨切開、開胸切開及び経静脈穿刺の少なくとも1つを通じて入ることにより胸腔に侵入するステップを更に備える、方法。
- 請求項1に記載の方法において、直接可視化、X線、超音波、磁気共鳴画像法、陽電子放出断層撮影、コンピュータ断層撮影、蛍光透視、内視鏡的観測、心臓内エコー及び経食道エコーの少なくとも1つを使用してターゲット組織を識別するステップを更に備える、方法。

- 請求項8に記載の方法において、組織係合部分を超音波エネルギにて励起するステップを更に備える、方法。

組織係合部分をターゲット組織と接触させ得るようにアブレーション装置を調節するステップとを更に備える、方法。 - 請求項1に記載の方法において、生物接着剤、真空圧、拡張可能なバルーン、機械的偏向、磁界、形状記憶合金及び超弾性合金の少なくとも1つにより組織係合部分をターゲット組織に固定するステップを更に備える、方法。
- 請求項1に記載の方法において、ターゲット組織に隣接する組織の損傷を防止し得るよう絶縁体をアブレーション装置内に提供するステップを更に備える、方法。
- 患者のターゲット組織をアブレートするアブレーション装置において、
基端、末端、管腔を有する挿入ツールと、
導体及び組織係合部分を有するアブレータであって、導体は挿入ツールの基端から伸びる供給端と、組織係合部分に連結された送り出し端部とを有し、管腔内に除去可能に挿入された前記アブレータと、
導体と接続されたエネルギ源と、
末端がターゲット組織に隣接するように患者の体内に挿入される挿入ツールと、
アブレータを管腔から外に押し出して、ターゲット組織と係合させる導体とを備え、
エネルギはエネルギ源からアブレータまで伝達されて、ターゲット組織にフートプリントを形成し、励起可能な組織の全質量を減少させるようにした、アブレーション装置。 - 請求項15に記載のアブレーション装置において、ターゲット組織を検知し得るようアブレータと接続されたセンサを更に備える、アブレーション装置。
- 請求項15に記載のアブレーション装置において、アブレータの組織係合部分を可視化するマッピングツールを更に備える、アブレーション装置。
- 請求項15に記載のアブレーション装置において、挿入ツールはカテーテルを有する、アブレーション装置。
- 請求項15に記載のアブレーション装置において、アブレータは、ターゲット組織に除去可能に取り付けることのできるパッチ(patch)を有し、該パッチは、接着剤パッチ、舌状体パッチ、円形パッチ、バルーンパッチ、吸引パッチ、化学剤解放パッチ、ロール状パッチ、炭素パッチ、ウェブ付きパッチの少なくとも1つを含む、アブレーション装置。
- 請求項15に記載のアブレーション装置において、アブレータは、ターゲット組織に対して吸引することのできるスカート部を有し、該スカート部はエネルギをターゲット組織に伝達すべく流体を受け入れる、アブレーション装置。
- 請求項20に記載のアブレーション装置において、流体は導電性である、アブレーション装置。

- 請求項15に記載のアブレーション装置において、アブレータは、バルーンを有し、該バルーンは拡張してターゲット組織に隣接する患者のキャビティを占め、バルーンは挿入ツールの末端内に除去可能に格納され、バルーンは拡張してバルーンが挿入ツールから出るようにし、また、バルーンは組織係合部分がターゲット組織に対して当接するよう配置可能である、アブレーション装置。
- 請求項15に記載のアブレーション装置において、アブレータは化学剤送り出しシステムを有する、アブレーション装置。
- 請求項25に記載のアブレーション装置において、化学剤送り出しシステムは、拡散、イオン導入、機械的注射、無針注射、及び解放速度が遅いように制御された送り出し、の少なくとも1つの送り出し方法を含む、アブレーション装置。
- 請求項15に記載のアブレーション装置において、アブレータは、スカート部と、該スカート部内の接着剤舌状体とを有し、該舌状体はスカート部内にて動いてターゲット組織と係合し、スカート部はアブレータを真空圧によりターゲット組織に固定するようにした、アブレーション装置。
- 請求項15に記載のアブレーション装置において、組織係合部分は第一の双極組織電極と、第二の双極組織電極とを有し、第一の双極組織電極は左心房の内部の心内膜に取り付けられ、第二の双極組織電極は左心房の外側の心外膜に取り付けられ、第一及び第二の双極組織電極の各々は、エネルギ源と接続され、エネルギはエネルギ源から付与されて第一の双極組織電極から第二の双極組織電極まで伝達され、第一及び第二の双極組織電極の間に配置された左心房の組織の質量を収縮不能、作用不能、動作電位を伝搬不能であることの少なくとも1つとなるようにすることができる、アブレーション装置。
