Die Erfindung betrifft ein Instrument zum Einsetzen einer chirurgischen Befestigungseinrichtung in Körper
gewebe nach dem Oberbegriff des Patentanspruchs !.
Ein derartiges Instrument ist aus der US-PS 32 75 211
bekannt. Die chirurgischen Befestigungseinrichtungen, die mit dem vorbekannten Instrument in die Körpergewebe
eingesetzt werden, bestehen gewöhnlich aus einem Metalldraht, und zwar aus einem inerten Material,
wie Tantal oder nichtrostendem Stahl. Es ist auch die Verwendung von Metallen wie Magnesium bekannt, die
vom Körper allmählich absorbiert werden.
Bei manchen Operationen ist die Verwendung von nichtmetallischen Befestigungseinrichtungen zweckmäßig.
Beispielsweise können metallische Befestigungseinrichtungen eine Streuung von Röntgenstrahlen und daher
eine Minderung dei Qualität von Röntgenaufnahmen bewirken. In manchen Fällen können in den auf die
Operation folgenden Monaten oder Jahren metallische Befestigungseinrichtungen in dem Körper unerwünschterweise
wandern. Aus diesen und anderen Gründen kann es zweckmäßig sein, für die chirurgischen Befestigungseinrichtungen
nichtmetallische Materialien zu verwenden, beispielsweise natürliche oder synthetische
Polymere oder Harze oder Kollagen, welche den Durchtritt von Röntgenstrahlen nicht behindern und/
oder vom Körper relativ schnell absorbiert werden können. Nachstehend werden alle derartiger, nichtmetallischen,
relativ biegeelastischen Materialien für chirurgische Befestigungsetnrichtungen als »kunststoffartige
Materialien« bezeichnet.
Diese kunststoffartigen Materialien haben im allgemeinen andere physikalische Eigenschaften als Metalle.
Sie können in dem vorbekannten Instrument zum Einsetzen einer chirurgischen Befestigungseinrichtung in
Körpergewebe nicht an Stelle von Metalldraht verwendet werden, weil die aus diesen Materialien bestehenden
Befestigungseinrichtungen im allgemeinen für das Durchstechen oder Durchdringen des festzulegenden
Gewebes nicht genügend fest sind. Ferner können aus diesen Materialien bestehende Befestigungseinrichtungen
bei ihrem Einsetzen nicht in d'-r bei Metallklammern
üblichen Weise gebogen werden. Diese Materialien sind im allgemeinen so biegsam, daß sie die Form
einer fertigen Klammer nicht beibehalten.
Aufgabe der Erfindung ist es daher, ein Instrument zum Einsetzen einer chirurgischen Befestigungseinrichtung
in Körpergewebe der eingangs angegebenen Art so auszugestalten, daß damit auch aus kunststoffartigen
Materialien bestehende Befestigungselemente in Körpergewebe eingesetzt werden können.
Erfindungsgemäß wird diese Aufgabe gelöst durch die im kennzeichnenden Teil des Patentanspruchs I angegebenen
Merkmale.
Dadurch, daß die Schiebeanordnung zu einem Teil der Seitenfläche der Zinken parallele und an diesen Flächenteilen
anliegende Metallstifte besitzt, die sich über das freie Ende der zugeordneten Zinken hinaus erstrekken
und bei dem Vorwärtstreiben des Befestigungselements mit diesen zur Erleichterung des Durchtritts der
zugeordneten Zinken durch das Gewebe mitbewegbar sind, wird gewährleistet, daß auch aus kunststoffartigen
Materialien bestehende Befestigungselemente in das Körpergewebe eingesetzt werden können. Die Metallstifte
durchstechen das festzulegende Gewebe bzw. treten durch es hindurch und ermöglichen es damit den
Zinken der Befestigungseinrichtungen, ebenfalls durch das Gewebe hindurchzutreten und mit den Halleelementen
verriegelt zu werden. Die Meuillstiftc erleichtern also den Durchtritt der Zinken durch das Gewebe.
Dadurch, daß eine der Befestigungsaufnahme zugeordnete Einrichtung zum automatischen Zurückziehen
der Stifte in die Befestigungselementaufnahme beim Wegbewegen der Befestigungselementaufnahme von
dem festgelegten Gewebe vorgesehen ist, wird erreicht, daß die Stifte zurückgezogen werden, ehe das festgelegte
Gewebe von dem Instrument zum Einsetzen der chirurgischen Befestigungseinrichtungen abgenommen
werden kann.
Vorteilhafte Weiterbildungen der Erfindung sind in den Unteransprüchen beschrieben.
Nachstehend wird ein Ausführungsbeispiel der Erfindung anhand der Zeichnung ausführlich beschrieben. In
dieser zeigt

Das in der Fig.! gezeigte Instrument 10 besteht aus
zwei Hauptteüen, und zwar der Betätigungsvorrichtung
12 und der Patrone 14, die zum Einsetzen der Befestigungseinrichtung
dient. Die Betätigungsvorrichtung 12 ist von vorb^kannter Bauart und wird im folgenden daher
nicht ausführlich beschrieben.
Die Betätigungsvorrichtung 12 besitzt ein Gehäuse 20, einen Klemmbetätigungsmeclianismus mit einem
Schwenkglied 22 und einem diesem zugeordneten, hin- und herbewegbaren Glied 24, sowie ein schwenkbares
Schieberbe'ätigungsglied 26. Die Parrone 14 ist im distalen
Teil des Gehäuses 20 durch eine Rastverbindung oder auf ähnliche Weise zwischen dem distalen Schenkel
28 des Gehäuses und der Halteelementaufnahne 30 herausnehmbar montiert.
Am besten in den F i g. 2 und 3 erkennt man, daß die Patrone 14 eine langgestreckte Halteelementaufnahme
30 und eine Befestigungselementaufnahme 40 besitzt. Diese ist mit der Halteeler'entaufnahme 30 im Bereich
des einen Endes derselben mittels eines Zapfens 32 schwenkbar verbunden. Die Halteelementaufnahme 30
enthält mehrere in zwei parallelen Reihen angeordnete Halteelemente 80. Die Befestigungselementaufnahme
40 enthält mehrere, in zwei parallelen Reihen angeordnete Befestigungselemente 100. Nach dem Verschwenken
der Befestigungselementaufnahme 40 in eine zu der Halteelementaufnahme 30 im wesentlichen parallele
Stellung, die beispielsweise in der F i g. 2 gezeigt ist, liegt jedes Befestigungselement 100 einem der Halteelemente
80 gegenüber. Die Befestigungs- und Halteelemente jeder Reihe sind gegenüber den Befestigungs- und HaI-teelernenten
der anderer Reihe derart versetzt, daß die Befestigungseinrichtungen in dem festgelegten Gewebe
gestaffelt angeordnet sind.
Zum Festlegen von Körpergewebe wird das in der F i g. 1 gezeigte Instrument wie folgt verwendet. Durch
Aufwärtsschwenken des schwenkbaren Klemmbetätigungsgliedes 22 aus dem Gehäuse 20 heraus wird das
hin- und herbewegbare Klemmbetätigungsglied 24 in die in der F i g. 1 a gezeigte Stellung proximal zurückgezogen.
Dann wird eine Patrone 14 in das Gehäuse 20 eingesetzt. Bei proximal zurückgezogenem Glied 24
kann die Befestigungselementaufnahme 40 von der Halteelementaufnahme 30 weg in die in der F i g. 1 a gezeigte
Stellung verschwenkt werden, so daß die Patrone 14 jetzt offen ist. Nun wird das festzulegende Gewebe 16
zwischen die einander gegenüberliegenden Flächen der Aufnahmen 30 und 40 eingebracht und dann das
schwenkbare Klemmbetätigungsglied 22 in eine Stellung verschwenkt, in der es zu dem Gehäuse 20 parallel
ist Dadurch wird das Klemmbetätigungsglied 24 distal in die in der Fig. Ib gezeigte Stellung ausgefahren, so
daß sein distales Ende an der proximalen Seite der Befestigungselementaufnahme
40 angre^ü und diese zu der Halteelementaufnahme 30 hin verschw ;nkt. Dadurch
wird das festzulegende Gewebe zwischen den einander gegenüberliegenden Flächen der Aufnahmen 30 'jnd 40
eingeklemmt. Wenn die Befestigungselementaufnahme 40 im v, esentlichen parallel zu der Halteelementaufnahme
30 angeordnet ist, ist das Gewebe vollkommen eingeklemmt und festlegebereit. Diese Stellung der Patrone
14 ist am besten in den F i g. 2 und 3 gezeigt. Danach wird das schwenkbare Schieberbetätigungsglied 26 zum
hinteren Ende des Gehäuses 20 hin verschwenkt, um durch Betätigung der Befestigungselementaufnahme 40
Befestigungselemente 100 aus der Aufnahme 40 heraus und durch das Gewebe hindurch vorzutreiben, bis sie in
der in den F i g. 4 und 5 gezeigten Stellung in den Haheelementen
80 verriegelt sind. Jetzt ist das Gewebe vollständig festgelegt und kann aus dem instrument herausgenommen
werden. Das schwenkbare Schieberbetätigungsglied 26 wird freigegeben und kehrt in seine Ausgangsstellung
zurück. Durch Herausschwenken des schwenkbaren Klemmbetätigungsgliedes 22 aus dem
Gehäuse 20 wird das hin- und herbewegbare Klemmbetätigungsglied 24 zurückgezogen, so daß die Befestigungselementaufnahme
40 von der Halteelementaufnahme 30 weggeschwenkt und danacn -ias Instrument
von dem festgelegten Gewebe abgenommen werden kann. Dabei werden die Halteelemente 80 aus der Aufnahme
30 herausgezogen und bleiben zum Festlegen des Gewebes mit den Befestigungselemenicn 100 verriegelt.
Die jetzt leere Patrone 14 wird aus dem Gehäuse 20 herausgenommen und beseitigt. Zum erneuten
Festlegen von Gewebe kann eine neue Patrone in dai
Gehäuse eingesetzt werden.
Die Deiden Teile 80 und 100 der Befestigungseinrichtung
bestehen aus kunststoffartigem Material in dem vorstehend erläuterten Sinn. Zum Unterschied gegenüber
dem Metall, aus dem chirurgische Klammern gewöhnlich hergestellt werden, ist das kunststoffartige
Material der Eleninte 80 und 100 der Befestigungseinrichtung relativ weich und biegeelastisch. Infolgedessen
sind die Zinken 104 und 106 des Befestigungselements 100 im allgemeinen nicht imstande, das festzulegende
Gewebe zu durchstechen. Um den Durchtritt der Zinken des Befestigungselements durch das Gewebe zu
erleichtern, besitzt üe Befestigungselementaufnahme 40 zwei in den F i g. 2 und 6 gezeigte Metallstifte 52. die
sich längs je einer Zinke des Befestigungselements erstrecken und während des Vortriebshubes des Instru-
mems zusammen mit den zugeordneten Zinken bewegt werden. Jeder Stift 52 steht etwas über das freie Ende
der zugeordneten Zinke vor und ist scharf zugespitzt, um das Durchstechen des Gewebes zu erleichtern.
Die Stifte 52 können auf verschiedene Weise hergestellt werden. In der dargestellten Ausführungsform bildet
jeder Stift den Endteil eines am besten in der F i g. 6 gezeigten Bügels 50, der einen Steg 54 besitzt, von dessen
entgegengesetzten Enden zwei parallele, U-profilförmige
Schenkel 56 rechtwinklig abstehen. Jeder Stift 52 erstreckt sich von dem freien Ende eines der Schenkel
56. Fi g. 8 zeigt einen ebenen Zuschnitt 53, aus dem
der Bügel 50 hergestellt werden kann.
Nachstehend soll die Ausbildung und Wirkungsweise der Patrone 14 ausführlicher beschrieben werden. Gemäß
den F i g. 2 und 3 besitzt die Befestigungsaufnahme 40 ein Gehäuse mit einem Anschlag 62, das von einer
Seile in das Gehänseinnerp vorkragt Im Inneren des
Gehäuses 60 ist das U-Profil 64 gelagert, das parallel zu
den Zinken der Befestigungselemente 100 hin- und herbewegbar ist. Die beiden Flansche des U-Profils 64 sind
auf der proximaien bzw. der distalen Seite des Anschlages 62 angeordnet. Zwischen dem Anschlag 62 und dem
proximaien Flansch des U-Profils 64 ist die Druckblattfeder 66 eingespannt, die das U-Profil 64 elastisch in der
proximaien Richtung zu bewegen trachtet.
An dem distalen Flansch des U-Profils 64 ist durch eine Schwalbenschwanzverbindung zwischen dem U-Profil
und dem Rücken des Schiebers 68 dieser fest mit dem distalen Flansch des U-Profils 64 verbunden. Gemäß
der Fig.6 besitzt der Schieber 68 einen langgestreckten
Rücken 70 und mehrere von dem Rücken rechtwinklig abstehende Finger 72. die sich zu der proximaien
Seite des Rückens 102 je eines Befestigungselements 100 erstrecken. Die Finger 72 sind daher die Teile,
mit denen die Befestigungselemente 100 aus der Befestigungselementaufnahme 40 herausgetrieben werden,
wenn die Patrone 14 betätigt wird. Die Stege 54 des die Stifte 52 tragenden Bügels 50 sind zwischen dem Rükken
70 des Schiebers und dem distalen Schenkel des U-Profils 64 festgelegt. Jeder einen Stift 52 tragende,
U-profilförmige Schenkel 56 durchsetzt eine entsprechend geformte Ausnehmung 74 im Schieberrücken 70
(siehe auch F i g. 7). Die U-profilförmigen Schenkel 56 erstrecken sich ferner längs der Seiten der Schieberfinger
72 und enden im Bereich der Enden des Rückens 102 des zugeordneten Befestigungselements 100. An dem
Übergang zwischen jedem U-profiiförmigen Schenkel 56 und dem zugeordneten Stift 52 ist der Bügel 50 mit
einem Rastelement c-8 ausgebildet, das lösbar an einer
entsprechenden Rastfläche 124 des Befestigungselements 100 angreift. Das Zusammenwirken der Rastelemente
58 und 124 trägt dazu bei, daß die Befestigungselemente
100 in der Befestigungselementaufnahme 40 gehalten werden, ehe diese Aufnahme zum Vortreiben
der Befestigungselemente betätigt wird.
Auf der proximaien Seite des Gehäuses 60 ist im Bereich des Schwenkzapfens 32 eine Blattfeder 42 montiert,
deren der Befestigung der Feder 42 an dem Gehäuse 60 entgegengesetztes, freies Ende an einem Paßstift
44 angreift. Dieser erstreckt sich parallel zu den Zinken der Befestigungselemente durch den oberen Teil
des Gehäuses 60 und ist in diesem in der Längsrichtung hin- und herbewegbar. Normalerweise trachtet die Feder
42, von der proximaien Fläche des Gehäuses 60 weg in die in der Fig. la gezeigte Stellung zu federn und
dabei das nicht gezeigte, freie Ende des Paßstiftes 44 proximal in das Gehäuse 60 einzuziehen. Wenn sich dagegen
die Klemmbetätigungsglieder 22 und 24 der Betätigungsvorrichtung 12 in der in der Fig. Ib gezeigten
Stellung befinden, wird die Feder 42 gegen die proxima-Ic Fläche des Gehäuses 60 gedrückt, so daß sie den
Paßstift 44 distal bewegt. In dieser Stellung steht das distale Ende des Paßstiftes 44 von der distalen Seite des
Gehäuses 60 in eine Ausnehmung 34 vor, die in dem von dem Schwenkzapfen 32 entfernten Ende der Halteelementaufnahme
30 ausgebildet ist. Dadurch trägt der Paßstift 44 dazu bei. die Befestigungselementaufnahme
40 auf die Halteelementaufnahme 30 auszurichten, wenn diese beiden Aufnahmen im wesentlichen parallel
zueinander und bereit sind, Befestigungseinrichtungen in zwischen den Aufnahmen eingeklemmtes Gewebe
einzusetzen. Wenn die Befestigungselementaufnahme 40 zu der Halteelementaufnahme 30 nicht parallel ist,
zieht die Feder 42 das distale Ende des Paßstiftes 44 automatisch in das Gehäuse 60 zurück, so daß der Paßstift
das Einführen von Gewebe in das Instrument oder das Herausnehmen von Gewebe aus dem Instrument
nicht behindert.
Die Halteelemente 80 werden in Ausnehmungen der Halteelementaufnahme 30 auf beliebige geeignete Weise
herausnehmbar gehalten, beispielsweise durch eine schwache Reibungsberührung zwischen den Halteelementen
und der Halteelementaufnahme. Infolgedessen können d:>
Halteelemente relativ leicht aus der Halteelementaufnahme heraustreten, nachdem die Befestigungselemente
100 durch das Gewebe hindurch und in die Halteelemente hinein getrieben worden sind.
Wenn das festzulegende Gewebe zwischen den einander gegenüberliegenden Flächen der Halteelementaufnahme
30 und der Befestigungselementaufnahme 40 eingeklemmt ist und festgelegt werden soll, wird das in
der F i g. 1 gezeigte Schieberbetätigungsglied 26 zum hinteren Ende des Gehäuses 20 hin verschwenkt, so daß
der einen Teil der Betätigungsvorrichtung 12 bildende Schieberstößel 130 gemäß den Fig.4 und 5 distal bewegt
wird. Dabei tritt das distale Ende des Schiebersiößels
130 durch den Schlitz 46 der Feder 42 und den Schlitz 76 des Gehäuses 60 und greift dieses distale Ende
an dem proximaien Flansch des U-Profils 64 an, so daß dieses distal verschoben wird und dadurch die Druckblattfeder
66 zusammendrückt und auch den Schieber 68 distal verschiebt. Infolge der distalen Verschiebung
des Schiebers 68 werden die Stifte 52 sowie die Zinken 104,106 der Befestigungselemente 100 durch das Gewebe
hindurch in die Ausnehmungen 84, 86 der Halteelemente 80 vorgetrieben. Dabei treten die Enden der Stifte
52 in die Ausnehmungen 84,86 ein, die im Bereich der Stifte so groß sind, daß die Stifte von den Halteelementen
nicht festgehalten werden. Die Stifte 52 erleichtern den Durchtritt der Zinken 104, 106 durch das Gewebe
und versteifen die Zinken, so daß sie nicht knicken. Dagegen behindern die Stifte 52 nicht das vorstehend ausführlich
beschriebene Verdrehen der Zinken bei deren Eintritt in die Ausnehmungen 84 und 86 des Halteelements.
Wenn der Schieberstößel 130 seinen Vortriebshub vollständig durchgeführt hat, befindet sich der distale
Flansch des U-Profils 64 in der Nähe des Anschlages 62 (Fig.4 und 5) und sind die freien Endteile der Zinken
aller Befestigungselemente 100 in den zugeordneten Halteelementen 80 in der in den F i g. 13 und 14 gezeigten
Weise verriegelt. Jetzt ist das Gewebe festgelegt und kann das schwenkbare Betätigungsglied 26 freigegeben
werden, damit der Schieberstößel 130 proximal
zurückgezogen wird.
Sobald der Rückzug des Schieberstößels 130 beginnt, drückt die Druckblattfeder 66 das U-Profil 64 in seine in
den Fi g. 2 und 3 gezeigte Ausgangsstellung zurück, so
daß die Stifte 52 aus den fertiggestellten chirurgischen Befestigungseinrichtungen und dem festgelegten Gewebe
heraus und in das Gehäuse 60 zurückgezogen werden. A'.if diese Weise werden die Stifte 52 automatisch in
das Instrument zurückgezogen, bevor das festgelegte Gewebe aus dem Instrument herausgenommen werden
kann. Dadurch wird verhindert, daß das Gewebe bei seinem Herausnehmen aus dem Instrument durch die
Berührung mit den Stiften beschädigt wird.
Zum Herausnehmen des festgelegten Gewebes aus dem Instrument wird das schwenkbare Klemmbetätigungsglied
22 (Fig. 1) aus dem Gehäuse 20 herausgeschwenkt und dadurch das hin- und herbewegbare Betätigungsglied
24 zurückgezogen. Infolgedessen kann die Feder 42 den Paßstift 44 zurückziehen und kann die
Befestigungselementaufnahme 40 von der Halteelemcntaufnahme 30 wegschwenken. Infolge des Rückzuges
der Stifte 52 sind die Befestigungselemente 100 bereits freigegeben worden, jetzt werden die Halteelemcnte
80 aus der Halteelementaumahme 30 herausgezogen. Danach kann das festgelegte Gewebe aus dem
Instrument herausgenommen werden. Die verbrauchte Patrone 14 wird von der Betätigungsvorrichtung 12 abgenommen
und beseitigt. Nach dem Einsetzen einer neuen Patrone 14 in die Betätigungsvorrichtung ist das
Instrument erneut einsatzbereit jo
Verstehend wurde der Erfindungsgegenstand anhand
einer bestimmten Ausführungsform beschrieben, doch kann die Erfindung auch auf andere chirurgische Befestigungsinstrumente
angewendet werden. Beispielsweise kann man anstelle der vorstehend beschriebenen Patrone
14 mit einer Schwenklagerung auch eine Anordnung verwenden, in der die Haltee'ementaufnahme und
die Befestigungseiementaufnahme zwei voneinander vollkommen getrennte Patronenteile sind, die relativ zueinander
geradlinig bewegbar sind, analog der voneinander getrennten Amboß- und Klammernhalter der
chirurgischen Klammerinstrumente gemäß der US-PS 34 94 533. Die Erfindung ist auch nicht auf das Einsetzen
von chirurgischen Befestigungseinrichtungen in geraden Reihen eingeschränkt. Beisp,aisweise kann die Erfindung
auch auf das Einsetzen von chirurgischen Befestigungseinrichtungen in ringförmigen Mustern angewendet
werden, wie dies bei den in der US-PS 43 04 236 gezeigten chirurgischen Klammerinstrumenten zum
Herstellen von kreisförmigen Anastomosenähten der Fall ist. Bei der Anwendung der Erfindung auf Instrumente
dieser Art ist die Halteelementaufnahme von der Befestigungseiementaufnahme vollkommen getrennt,
analog der getrennten Anordnung des Amboßhalters und des Klammernhalters gemäß der US-PS 43 04 236.
Ferner ist die Erfindung nicht auf zum Einsetzen von Befestigungseinrichtungen dienende Instrumente eingeschränkt,
die aus einer wiederholt verwendbaren Betätigungsvorrichtung und einer davon getrennten Einwegpatrone
bestehen. Der zum Einsetzen der Befestigungseinrichtungen dienende Teil kann auch mit der Betätigungsvorrichtung
integriert und das ganze Instrument nach einmaligem Gebrauch beseitigt werden.
Die dargestellten und vorstehend beschriebenen Ausführungsbeispiele
der Erfindung können im Rahmen des Erfindungsgedankens vom Fachmann verschiedentlich
abgeändert werden. Beispielsweise können die chirurgischen Befestigungseinrichtungen gemäß der Erfindung
auch in andersartigen Instrumenten zum Einsetzen von Befestigungseinrichtungen verwendet werden. Ausführungsbeispiele
derartiger Instrumente wurden vorstehend beschrieben.

Claims (4)
1. Instrument zum Einsetzen einer chirurgischen Befestigungseinrichtung in Körpergewebe, bestehend
aus einer Befestigungsaufnahme (40), die mindestens ein Befestigungselement (100) mit einem
langgestreckten Rücken (102) und mindestens zwei im Abstand voneinander angeordnete Zinken (104,
106), die sich parallel zueinander rechtwinklig zu der Längsachse des Rückens von diesem weg erstrekken,
enthält, und aus einer der Befestigungsaufnahmen (40) zugeordneten Schieberanordnung (68),
durch die das Befestigungselement (100) aus der Befestigungsaufnahme (40) zum Festlegen des Gewebes
vortreibbar ist, wobei die Zinken (104, 106) durch das Körpergewebe hindurchtreten, gekennzeichnet
durch eine Haltelementaufnahme (30), in der mindestens ein Haltelement (80) herausnehmbar angeordnet ist, das mindestens zwei
Ausnehmunjren (84.86) besitzt, durch die jeweils das
freie Ende einer Zinke (104, 106), aufnehmbar und festhaltbar ist, wobei die Zinken durch das sich zwischen
der Befestigungselementaufnahme (40) und der Haltelementaufnahme (30) befindende Körpergewebe
hindurchtreten und die freien Endteile der Zinken in die Ausnehmungen (84, 86) des Haltelementes
(80) eintreten und darin festgehalten werden, wobei die Schiebeanordnung (68) zu einem Teil
der Seitenfläche (108) der Zinken (104,106) parallele
und an diesen Flächenteilen anliegende Metallstifte (52) besitzt, die sich über das freie Ende der zugeordneten
Zinken hinaus erstrecken und bei dem Vorwärtstreiben des ßefestigungselements (100) mit diesem
zur Erleichterung des Durc.tritts der zugeordneten Zinken durch das Gewebe mitbewegbar sind;
und durch eine der Befestigungsaufnahme (40) zugeordnete Einrichtung (22,24) zum automatischen Zurückziehen
der Stifte (52) in die Befestigungselementaufnahme beim Wegbewegen der Befestigungselementaufnahme
von dem festgelegten Gewebe.
2. Instrument nach Anspruch 1, dadurch gekennzeichnet, daß die Befestigungselementaufnahme (40)
im Bereich des einen Endes der Haltelementaufnahme
(30) schwenkbar gelagert ist.
3. Instrument nach einem der vorangehenden Ansprüche,
dadurch gekennzeichnet, daß jeder Stift (52) an die ihm zugeordnete Zinke (104, 106) diese
versteifend und dadurch die Längsknickfestigkeit der Zinke erhöhend angreift, aber die zum Verriegeln
des Befestigungselements (100) und des Halteelements (80) erforderliche Drehbarkeit der Zinke
nicht beeinträchtigt.
4. Instrument nach einem der vorangehenden An-Sprüche, dadurch gekennzeichnet, daß jeder Stift
(52) und die ihm zugeordnete Zinke (104, 106) miteinander zusammenwirkende Rastmittel (58, 124)
aufweisen, durch die vor der Betätigung der Schieberanordnung (68) das Befestigungselement (100) in bo
der Befestigungselementaufnahme (40) festhalibar ist.
