Abstract
Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung (12), mit:
– einem Anker (14), der zur Befestigung des Sensors (28) dieser Anordnung (12) an einer anatomischen Struktur ausgebildet ist;
dadurch gekennzeichnet, dass der Sensor (28) aufweist:
– einen Empfänger zum Empfang von Bezugsignalen, die
– von einem chirurgischen Lenkungssystem (34, 39) erzeugt werden;
– einen mit dem Empfänger gekoppelten Sender (30) zur Übertragung von von dem Empfänger empfangenen Bezugsignalen zu einer chirurgischen Navigationsschaltung (32) des chirurgischen Lenkungssystems (34, 39); und
– einen Verbinder mit Gehäuse (26) zur Befestigung zumindest des Empfängers an dem Anker (14), welcher
– Verbinder mit Gehäuse (26) so konfiguriert ist, dass er den Sensor (28) in einer festen Position bezüglich des Ankers hält, so dass die von dem Sender (30) übertragenen Signale eine derzeitige Position des Ankers (14) als Funktion der von dem Empfänger empfangenen Bezugssignale anzeigen.
– einem Anker (14), der zur Befestigung des Sensors (28) dieser Anordnung (12) an einer anatomischen Struktur ausgebildet ist;
dadurch gekennzeichnet, dass der Sensor (28) aufweist:
– einen Empfänger zum Empfang von Bezugsignalen, die
– von einem chirurgischen Lenkungssystem (34, 39) erzeugt werden;
– einen mit dem Empfänger gekoppelten Sender (30) zur Übertragung von von dem Empfänger empfangenen Bezugsignalen zu einer chirurgischen Navigationsschaltung (32) des chirurgischen Lenkungssystems (34, 39); und
– einen Verbinder mit Gehäuse (26) zur Befestigung zumindest des Empfängers an dem Anker (14), welcher
– Verbinder mit Gehäuse (26) so konfiguriert ist, dass er den Sensor (28) in einer festen Position bezüglich des Ankers hält, so dass die von dem Sender (30) übertragenen Signale eine derzeitige Position des Ankers (14) als Funktion der von dem Empfänger empfangenen Bezugssignale anzeigen.
Description
- Die vorliegende Erfindung bezieht sich auf eine integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung mit einem Anker, der zur Befestigung des Sensors dieser Anordnung an einer anatomischen Struktur ausgebildet ist. Diese Anordnung hat spezielle Anwendbarkeit auf chirurgische Verfahren, bei denen es erwünscht ist, die Relativbewegung einer oder mehrerer Strukturen zu verfolgen.
- Viele chirurgische Verfahren werden unter Verwendung von Bildern geplant und gelenkt, die von bildgebenden Systemen gewonnen werden, wie z. B. von Magnetresonanz-Bilderzeugungssystemen (MRI), Computertomographie-Bilderzeugungssystemen (CT), Röntgenstrahl-Bilderzeugungssystemen, Positionsemissions-Tomographie-Abtastgeräten (PET) und Photoemissions-Computertechnologien (SPECT). Diese Systeme ermöglichen es Ärzten, ausführliche voroperative (oder intraoperative) Ansichten von anatomischen Strukturen unter Verwendung von nichtinvasiven Verfahren zu gewinnen. Sobald diese Bilder erzeugt wurden, verwendet der Arzt typischerweise die Bilder dazu, ein korrigierendes chirurgisches Verfahren zu planen. Wenn der Patient auf einem Operationstisch liegt, können die Bilder mit dem entsprechenden physikalischen Raum des Patienten „in Deckung gebracht” und auf einem Bildschirm im Operationsraum (OR) angezeigt werden. Während der Arzt Sonden oder andere medizinische Instrumente im Inneren des Patienten gesteuert bewegt, leiten Sensoren auf den Instrumenten Positionsinformationen an einen Computer weiter. Der Computer überlagert seinerseits eine Anzeige der Position des Instrumentes über das Bild der anatomischen Struktur. Auf diese Weise kann der Arzt sich in gesteuerter Weise durch ein chirurgisches Verfahren dadurch bewegen, dass er einen Anzeigebildschirm in dem OR betrachtet. Ein Beispiel eines einen verwandten Stand der Technik darstellenden Systems findet sich in dem US-Patent
US 6 236 875 B1 mit dem Titel „Surgical Navigation System Including Reference and Localization Frames”, deren Inhalt durch diese Bezugnahme hier vollständig mit aufgenommen wird. - Bisher war die Verfolgung der Position von anatomischen Strukturen weitgehend auf die externe Verfolgung beschränkt, entweder durch Befestigen eines Sensors an der Haut eines Patienten durch Klebebänder, oder durch Befestigen einer externen Klammer an dem Patienten, wie z. B. einer Mayfield-Klammer, die von außen an dem Kopf eines Patienten befestigt wird. Das
US-Patent US 6 347 240 B1 mit dem Titel „System and Method for use in Displaying Images of a Body Part” offenbart ein System, das Schrauben verwendet, die sich von einem Knochenfragment durch die Haut eines Patienten erstrecken und mit einer Plattform außerhalb des Patienten verbunden sind. Nachführ- oder Verfolgungselemente, wie z. B. Sender, sind auf der Plattform angeordnet, so dass, wenn sich ein Knochenfragment bewegt, dies auch die Plattform mit den damit verbundenen Nachführelementen tut. Eine Anordnung in dem OR verfolgt die Bewegung der Nachführelemente, und diese Bewegung wird mit der Bewegung des Knochenfragmentes korreliert, um präzise die Bewegung des Knochenfragmentes zu verfolgen. Alternativ können Klammern anstelle von Schrauben verwendet werden, um eine Gruppe von Nachführ- oder Verfolgungselementen an einer Knochenstruktur zu befestigen. Obwohl diese bekannten Systeme allgemein zuverlässig sein können, ist ihre Struktur in gewisser Weise unhandlich, insbesondere dann, wenn die Bewegung mehrerer anatomischer Strukturen verfolgt werden muss. Zusätzlich erfordert die Verwendung der Nachführ- oder Verfolgungselemente und Empfangsgruppen eine unbehinderte Sichtlinie zwischen diesen Teilen, was nicht nur die Implantation in einem Patienten beschränkt sondern auch zu Störungen führen kann. - Aus diesen Gründen war es bei Verfahren, wie z. B. die, die die Wirbelsäule oder die Rekonstruktion oder Reparatur von Wirbelsäulenkörpern, Schädelbrüchen, gebrochenen Knochen oder anderen beschädigten knochenartigen Strukturen betreffen, etwas schwierig, die relative Bewegung der Anzahl von anatomischen Strukturen zu verfolgen.
- Aus der
DE 42 33 987 C1 ist eine Vorrichtung zum Markieren von Körperstellen für medizinische Untersuchungen bekannt, bei der ein oder mehrere Bezugsmarker verwendet werden, die mit einem bilderzeugenden System, beispielsweise einem Kernspintomographie-, Computertomographie- oder Röntgenstrahlsystem oder anderen Bilderzeugungssystemen zusammenwirken, die ein Bild eines Körpers erzeugen, an dem der oder die Marker befestigt sind. Die einzelnen Vorrichtungen zum Markieren von Körperstellen umfassen ein Befestigungselement oder Befestigungselemente, an denen ein Marker einsteckbar angebracht werden kann. Auf diese Weise können an einer einzigen Befestigung unterschiedliche Marker befestigt werden, um eine Abbildung mit unterschiedlichen bilderzeugenden Systemen zu ermöglichen. Diese Marker sind passive Marker. - Weiterhin ist aus der
WO 97/29710 A1 - Der Erfindung liegt die Aufgabe zugrunde, eine integrierte chirurgische Anker-/Lokalisierungsanordnung der eingangs genannten Art zu schaffen, die die Feststellung einer Bewegung von anatomischen Strukturen während medizinischer Verfahren ohne die Verwendung von unhandlichen externen, an dem Patienten zu befestigenden Geräten ermöglicht, wobei keine unbehinderte Sichtlinie zwischen einem Positionssensor und einem Detektor erforderlich ist.
- Diese Aufgabe wird durch die im Patentanspruch 1 angegebenen gegenständlichen Merkmale gelöst.
- Vorteilhafte Ausgestaltungen und Weiterbildungen der Erfindung ergeben sich aus den Unteransprüchen.
- Durch die erfindungsgemäße Ausgestaltung der integrierten chirurgischen Anker-/Lokalisierungssensor-Anordnung kann die Bewegung von anatomischen Strukturen während medizinischer Verfahren ohne die Verwendung von unhandlichen externen, an dem Patienten zu befestigenden Geräten festgestellt werden, wobei keine unbehinderte Sichtlinie zwischen einem Lokalisierungssensor und einem Detektor erforderlich ist.
- Die Anker können gegebenenfalls in einem Patienten vor einem Verfahren angeordnet werden und in dem Patienten für eine gewisse Zeit nach dem Verfahren verbleiben Hierbei wird weiterhin vorzugsweise die Erfassung einer Bewegung mit fünf oder sechs Freiheitsgraden einer anatomischen Struktur oder eines chirurgischen Instrumentes ermöglicht.
- Auf diese Weise wird ein Lokalisierungssystem für interne anatomische Strukturen geschaffen, das mit minimal-invasiven Verfahren verwendet werden kann.
- Die integrierte Anker-/Lokalisierungssensor-Anordnung kann sowohl vor als auch während der einzelnen Verfahren der Referenzmarkierung verwendet werden, wobei die einzelnen Anker in einem Patienten vor einem Verfahren angeordnet werden können und in dem Patienten für eine gewisse Zeit nach dem Verfahren verbleiben können.
- Da die einzelnen Sensoren vorzugsweise eine Bewegung mit fünf oder sechs Freiheitsgraden einer anatomischen Struktur oder eines chirurgischen Instrumentes erkennen können, steht eine ausreichende Information über die Bewegung dieser anatomischen Struktur oder des chirurgischen Instrumentes zur Verfügung.
- Die erfindungsgemäße integrierte Anker-/Lokalisierungssensor-Anordnung ermöglicht damit die Feststellung einer Bewegung von internen und/oder externen anatomischen Strukturen während medizinischer Verfahren ohne die Verwendung von unhandlichen externen Geräten, die an dem Patienten befestigt sind, und ohne dass eine unbehinderte Sichtlinie zwischen einem Positionssensor und einem Detektor erforderlich ist.
- Auf diese Weise wird eine integrierte Anker- und Lokalisierungssensor-Anordnung zu schaffen, die relativ einfach bei minimal-invasiven Verfahren verwendet werden kann.
- Der anatomische Anker kann sowohl vor als auch während der einzelnen Verfahren als Referenzmarker dienen.
- Die Erfassung einer Bewegung mit fünf oder sechs Freiheitsgraden einer anatomischen Struktur oder eines chirurgischen Instrumentes wird unabhängig davon ermöglicht, ob das Instrument ein Anker, ein Katheter oder irgend ein anderes medizinisches Instrument ist.
- Die Erfindung wird im Folgenden anhand von in der Zeichnung dargestellten Ausführungsbeispielen noch näher erläutert.

- Gemäß der Erfindung wird eine integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung geschaffen, wobei ein Beispiel einer derartigen integrierten Einheit in
1 mit der Bezugsziffer12 bezeichnet ist. - Gemäß der Erfindung ist der Anker so konfiguriert, dass er an einer anatomischen Struktur befestigt werden kann. Wie dies in
1 gezeigt ist, kann ein Anker gemäß der Erfindung als Beispiel eine chirurgische Schraube14 einschließen. Die Schraube14 weist einen Kopfabschnitt16 und einen Gewindeabschnitt18 auf. Der Gewindeabschnitt18 ist so ausgestaltet, dass er in einer knochenartigen Struktur befestigt werden kann, wie z. B. in Teilen von langen Knochen, Wirbelkörpern, den Schädel oder irgendeiner anderen knochenartigen anatomischen Struktur. Bei einer bevorzugten Ausführungsform kann der Anker eine Kreuzschlitzschraube mit einem Durchmesser von 2,2 mm und einer Länge von 3–7 mm sein. Vorzugsweise hat die Schraube unrunde oder auf andere Weise geformte Abschnitte15 , so dass ein Verbinder fest mit diesen verbunden werden kann. Die Schraube15 kann weiterhin Schlitze13 enthalten, die es ermöglichen, dass die Schraube mit Hilfe verschiedener üblicher chirurgischer Schraubendreher eingeschraubt werden kann. Es ist weiterhin vorzuziehen, dass die Schraube14 aus einem Material hergestellt ist, das auf einer Bildabtastung erscheint und keine Störungen mit dem chirurgischen Lenkungssystem hervorruft, mit dem sie verwendet werden soll. Wenn der Anker durch Abtastung erfassbar ist, kann er alternativ als Lokalisierungsmarker verwendet werden. Beispielsweise kann die Schraube bei Verwendung mit elektromagnetischen Lenkungssystemen aus Aluminium hergestellt sein. - Obwohl Gesichtspunkte der Erfindung hier in Verbindung mit chirurgischen Schrauben beschrieben werden, ist die Erfindung im weitesten Sinne nicht in dieser Hinsicht beschränkt. Es können andere Anker in Verbindung mit der Erfindung verwendet werden. Lediglich als Beispiel können derartige andere Anker chirurgische Klammern, Stifte, Stäbe, Nadeln, Weichgewebe-Anker, wie z. B. Schlaufen, und Kopfrahmen-(beispielsweise Mayfield-)Stifte einschließen.
- Gemäß der Erfindung wird weiterhin ein Empfänger zur Messung von Bezugssignalen bereitgestellt, die von einem chirurgischen Lenkungssystem erzeugt werden. Ein derartiges System schließt typischerweise einen Bezugssignalgenerator
39 ein, wie er schematisch in3 gezeigt ist. Wenn ein elektromagnetisches Lenkungssystem in Verbindung mit der Erfindung verwendet werden soll, kann der Empfänger zumindest einen elektromagnetischen Sensor28 einschließen, wie dies schematisch in den2A –2C gezeigt ist. Die Erfindung ist nicht auf irgendein bestimmtes Lokalisierungs-Lenkungssystem oder einen bestimmten Algorithmus beschränkt. Dennoch findet sich ein Beispiel eines brauchbaren Systems und Algorithmus in demUS-Patent US 5 592 939 A mit dem Titel „Method and System for Navigating a Catheter Probe”, dessen Inhalt durch diese Bezugnahme hier vollständig mit aufgenommen wird. -
2A zeigt einen Sensor28 mit einer einzigen Spule oder Wicklung20 . In Abhängigkeit von den speziellen Eigenschaften des verwendeten Lenkungssystems kann ein derartiger Sensor typischerweise die Position mit entweder drei oder fünf Freiheitsgraden erfassen. Im Gegensatz hierzu zeigen die2B und2C Sensoren28 mit mehrfachen Spulen, die in der Lage sind, die Position mit bis zu sechs Freiheitsgraden festzustellen. Insbesondere ermöglicht es die Spulenanordnung, die in3D gezeigt ist, daß ein Bezugssignal mit zwei orthogonalen Meßspulen22 erfasst wird. Alternativ kann eine Mehrzahl von kollinearen Spulen verwendet werden. Beispielsweise sind in der koaxialen Anordnung, die in2C gezeigt ist, zwei kollineare Messspulen mit unterschiedlichen oder entgegengesetzten Winkeln gewickelt. Obwohl irgendwelche entgegengesetzten Winkel funktionieren, ist ein bevorzugter Winkel gleich 90°. Auf diese Weise liefert jede Spule ein eindeutiges Rückführungssignal als Antwort auf das gleiche Bezugssignal zurück, das von einem elektromagnetischen Lenkungssystem erzeugt wird. - Die Spulenanordnung nach
2C ist weiterhin in Verbindung mit anderen medizinischen Geräten als Ankern verwendbar. Beispielsweise macht die kollineare Natur der Anordnung diese besonders für Geräte geeignet, die Arbeitskanäle aufweisen, wie z. B. Katheter, bei denen Spulen mit unterschiedlichen Winkeln um den Arbeitskanal herum gewickelt werden können, um die Größe des Gerätes zu einem Minimum zu machen. - Die Spule oder die Spulen des Sensors
28 können aus einem 40 AWG-Draht hergestellt werden, der mit ungefähr 200 Windungen gewickelt ist. Alternativ können die Sensoren oberflächenmontierte Induktivitäten mit 10 μH–1000 μH sein, die vorzugsweise nicht abgeschirmt sind. Bei einer alternativen Ausführungsform kann ein leitendes Lokalisierungssystem verwendet werden. In diesem Fall kann der Sensor28 eine eine leitende Elektrode aufweisenden Empfänger einschließen. Obwohl das bevorzugte elektromagnetische System so beschrieben wird, dass es einen Spulensensor einschließt, kann irgendein elektromagnetischer Sensor verwendet werden, unter Einschluss von Magnetflusssensoren und Reed-Schaltern ohne Beschränkung hierauf verwendet werden. - Die Erfindung schließt weiterhin einen Sender zur Übertragung von von dem Empfänger empfangenen Signalen an einen Prozessor ein. Bei einer bevorzugten Ausführungsform kann der Sender einfach zwei Drähte
30 zur Festverdrahtung des Sensors28 mit der Elektronik32 eines chirurgischen Lenkungssystems34 einschließen (das schematisch in3 gezeigt ist). Die Sender-Drähte30 können an ihren fernliegenden (nicht gezeigten) Enden Steckverbinder für ihre selektive Verbindung mit dem chirurgischen Lenkungssystem34 einschließen. Vorzugsweise schließen die Sender-Drähte30 zwei Paare eines verdrillten bifilaren 40 AWG-Drahtes mit einem Außendurchmesser von weniger als ungefähr 0,062 Zoll (1,575 mm) ein. Es wird weiterhin bevorzugt, dass die Sender-Drähte mit Sterilisationsverfahren kompatibel sind, so dass sie sicher im Inneren eines anatomischen Körpers verwendet werden können. - Bei einer alternativen Ausführungsform kann der Sender drahtlos sein und Signale an das chirurgische Lenkungssystem beispielsweise über Hochfrequenzsignale übertragen. Bei einer derartigen Ausführungsform kann eine Sendeschaltung und eine Antenne ebenfalls Teil des Sensors
28 sein. Weil die Einzelheiten von Funksendesystemen in der Technik bekannt sind, werden sie aus Gründen der Kürze hier nicht wiederholt. Der Sensor28 kann weiterhin eine (nicht gezeigte) Batterie zur Leistungsversorgung des Senders einschließen. Alternativ kann eine Spannung an den Sender über Induktion unter Verwendung einer externen Spule geliefert werden, die als Teil eines Sensors28 eingeschlossen ist. Beispiele derartiger Systeme werden in dem gleichzeitig eingereichten US-PatentUS 6 474 341 B1 mit dem Titel „Surgical Communication and Power System” beschrieben. - Ferner kann eine leitende Elektrode an einem automatischen Anker befestigt sein.
- Wie dies weiter oben beschrieben wurde, kann ein auf einem Anker angeordneter Sensor Signale von einem Signalgenerator extern von dem Patienten empfangen.
- Die Erfindung kann jedoch auch in einem System mit einer umgekehrten Anordnung verwirklicht werden, wobei beispielsweise das Element
28 ein Signalgenerator im Inneren des Patienten ist, während sich der Sensor außerhalb des Patienten befindet. Bei diesem Szenarium, und wenn die Erfindung ein elektromagnetisches Lenkungssystem verwendet, kann der interne Signalgenerator in seiner einfachsten Form ein Magnet sein. - Weiterhin ist gemäß der Erfindung ein Verbinder zur Befestigung des Empfängers an den Anker vorgesehen, wobei der Verbinder derart konfiguriert ist, dass die von dem Sender ausgesandten Signale eine derzeitige Position des Ankers anzeigen. Wie dies hier verwirklicht und in
1A gezeigt ist, schließt der Verbinder ein Gehäuse26 ein, das an den Kopf16 der Schraube14 befestigt ist. Das Gehäuse26 kann eine Drahtentlastungsnut33 für Sender-Drähte30 einschließen. Das Gehäuse26 kann weiterhin eine unrunde Öffnung27 zur Aufnahme eines eine entsprechende unrunde Form aufweisenden Schraubenkopfes16 einschließen. Diese Verkeilungsanordnung verhindert, dass sich das Gehäuse26 auf der Schraube14 dreht, wodurch sichergestellt wird, dass der Sensor28 in einer festen Position bleibt, um eine genaue Anzeige der Position (d. h. der Lage und/oder Ausrichtung) der Schraube14 zu liefern. Das Gehäuse26 weist vorzugsweise eine Vielzahl von abgeflachten Greifoberflächen31 auf, die es ermöglichen, daß die Schraube in eine knochenartige Struktur durch Handhabung des Gehäuses26 eingetrieben wird. Das Gehäuse26 kann entweder lösbar von der Schraube14 , fest an der Schraube14 befestigt oder einstückig mit dieser ausgebildet sein, solange es die Funktion der Befestigung eines Sensors an der Schraube erfüllt. Zu diesem Zweck kann das Gehäuse26 eine Öffnung29 entgegengesetzt zu der unrunden Öffnung27 aufweisen, um einen Sensor28 aufzunehmen. Wenn das Gehäuse26 einstückig mit einer Schraube14 ausgeformt ist, können die Schlitze13 fortgelassen werden oder sich alternativ an der Oberfläche der Öffnung29 befinden. -
1B zeigt eine der1A ähnliche Anordnung, jedoch mit der Ausnahme, dass eine Gewindekappe17 auf ein Gewindeende19 des Gehäuses26 aufgeschraubt ist, wodurch der Kopf16 der Schraube14 an dem Gehäuse26 befestigt wird. Bei dieser Anordnung kann das Sensorgehäuse26 selektiv von der Schraube14 getrennt werden. - Der Sensor
28 kann an dem Gehäuse26 in irgendeiner geeigneten Weise befestigt werden. Beispielsweise kann er in der Öffnung29 eingerastet und/oder mit dieser verklebt werden. Alternativ können Spulen oder andere Sensoren in der Öffnung29 abgeschieden werden, worauf die Öffnung danach mit einem geeigneten Vergussmaterial, wie z. B. chirurgischem Zement gefüllt wird. Im breitesten Sinne kann der Verbinder des Ankers aus irgendeinem Material oder einem Mechanismus bestehen, der in der Lage ist, den Empfänger mit dem Anker zu verbinden, wobei dies Material von einer Menge an Vergussmaterial bis zu Strukturen reicht, die abgeformt, mechanisch an dem Anker befestigt oder mit diesem verbunden sind, oder die einstückig mit diesem ausgebildet sind. - Bei einer alternativen (nicht gezeigten) Ausführungsform kann die Schraube
14 eine teilweise ausgehöhlte Konstruktion anstelle des Gehäuses16 aufweisen, und der Empfänger kann in den Hohlraum eingeordnet sein. Bei einer derartigen Ausführungsform kann der Verbinder Vergussmaterial zur Befestigung des Empfängers in dem Hohlraum sein, oder eine Kassette zur lösbaren Befestigung des Sensors in dem Hohlraum einschließen. Tatsächlich kann die Entfernbarkeit des Sensors aus dem Anker in nützlicher Weise in die mechanischen Verbindungen eingefügt werden, um es dem Arzt in flexibler Weise zu ermöglichen, den Sensor anzubringen und abzutrennen, wie es der Arzt für erforderlich hält. - Obwohl der Anker/Sensor der Erfindung als ein Positionierungsmarker verwendet werden kann, hat er besondere Vorteile bei der Verwendung von knochenartigen anatomischen Strukturen, wie z. B. der Wirbelsäulen-Körper während Wirbelsäulenverfahren oder von Knochenfragmenten während rekonstruktiver Verfahren. Beispielsweise kann ein Arzt ein Bild eines gebrochenen langen Knochens unter Verwendung der Fluoroskopie oder irgendeines anderen Bilderzeugungsgerätes gewinnen. Unter Verwendung eines Gerätes, wie z. B. des mit einem Kanal versehenen Schraubendrehers
40 (in den4A und4B gezeigt), kann der Arzt eine Schrauben-/Sensor-Anordnung in die knochenartige Struktur implantieren. Im einzelnen schließt der Schraubendreher40 einen Handgriff42 , einen Halsabschnitt44 und eine Buchse46 ein. Die Buchse46 schließt eine Öffnung48 ein, die so geformt ist, daß sie mit Greifoberflächen31 einer integrierten Schrauben-/Sensor-Einheit12 in Eingriff kommt. Der Schraubendreher kann einen Kanal oder eine Öffnung50 aufweisen, die sich durch diese hindurch erstreckt, um die Sendedrähte30 zu berücksichtigen. Alternativ kann anstelle der Öffnung50 ein Schlitz52 (4B ) in einer Kante der Buchse46 vorgesehen sein, um die Hindurchleitung von Drähten30 durch diesen zu ermöglichen. Im Gebrauch lädt der Arzt eine Schraube-/Sensor-Einheit12 in die Buchse48 und führt die geladene Buchse durch einen Einschnitt benachbart zu einer Knochenstruktur ein, in die die Schraube eingeschraubt werden soll. Unter Verwendung des Handgriffes42 dreht der Arzt die Schraube, wobei diese an der Knochenstruktur befestigt wird. Danach entfernt der Arzt den Schraubendreher40 und zieht ihn über die Sende-Drähte, die sich durch die Öffnung50 (4A ) erstrecken, oder er zieht den Schlitz52 (4B ) von den Drähten30 fort. - Nach dem Einschrauben eines Ankers/Sensors
12 in jedes der Knochenfragmente werden die Sensoren dann in bekannter Weise mit dem Bild ausgerichtet. Während das abgetastete Bild auf einem Anzeigegerät36 erscheint, kann der Arzt die gebrochenen Knochenfragmente manipulieren, wobei er ihre Bewegung in Echtzeit verfolgt. Dies ist erreichbar, weil die chirurgische Navigationsschaltung32 , die Signale von den Anker-/Sensoreinheiten12 in jedem gebrochenen Knochensegment empfängt, das auf der Anzeige36 erscheinende Bild ändern kann, um eine derzeitige Position der Knochensegmente wiederzugeben. Beispielsweise kann das abgetastete Bild digitalisiert und mit den Sensoren28 so korreliert werden, dass, wenn die gebrochenen Teile eines Knochens bewegt werden, eine simulierte Bewegung dieser Teile auf der Anzeige36 erscheinen kann. Auf diese Weise kann ein Arzt präziser einen gebrochenen Knochen auf seine ursprüngliche Stellung zurückführen. Verwandte Verfahren sind in den hereib eingangs zitierten US-PatentenUS 6 236 875 B1 mit dem Titel „Surgical Navigation System including Reference Frame and Localization Frame”, undUS 6 347 240 B1 mit dem Titel „Bone Navigation System” offenbart.
Claims (14)
- Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung (
12 ), mit: – einem Anker (14 ), der zur Befestigung des Sensors (28 ) dieser Anordnung (12 ) an einer anatomischen Struktur ausgebildet ist; dadurch gekennzeichnet, dass der Sensor (28 ) aufweist: – einen Empfänger zum Empfang von Bezugsignalen, die – von einem chirurgischen Lenkungssystem (34 ,39 ) erzeugt werden; – einen mit dem Empfänger gekoppelten Sender (30 ) zur Übertragung von von dem Empfänger empfangenen Bezugsignalen zu einer chirurgischen Navigationsschaltung (32 ) des chirurgischen Lenkungssystems (34 ,39 ); und – einen Verbinder mit Gehäuse (26 ) zur Befestigung zumindest des Empfängers an dem Anker (14 ), welcher – Verbinder mit Gehäuse (26 ) so konfiguriert ist, dass er den Sensor (28 ) in einer festen Position bezüglich des Ankers hält, so dass die von dem Sender (30 ) übertragenen Signale eine derzeitige Position des Ankers (14 ) als Funktion der von dem Empfänger empfangenen Bezugssignale anzeigen. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach Anspruch 1, dadurch gekennzeichnet, dass der Empfänger zumindest einen elektromagnetischen Sensor (
20 ,22 ,24 ) einschließt. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass der Empfänger eine Anzahl von kollinearen elektromagnetischen Spulen (
24 ) einschließt, die mit unterschiedlichen Winkeln gewickelt sind. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass der Sender (
30 ) zumindest einen Draht einschließt, der mit dem Empfänger verbunden ist und den Empfänger fest mit der Navigationsschaltung (32 ) verdrahtet. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass der Sender eine drahtlose Verbindung zu dem Lenkungssystem (
34 ,39 ) aufweist. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Empfänger eine leitende Elektrode einschließt.
- Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Anker (
14 ) eine chirurgische Schraube ist. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass zumindest der Empfänger in einem einstückig mit dem Anker (
14 ) ausgebildeten Hohlraum (29 ) angeordnet ist. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass der Verbinder mit Gehäuse (
26 ) auf dem Anker (14 ) befestigt ist. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach Anspruch 7 oder 8 bei direktem Rückbezug des Anspruchs 8 auf den Anspruch 7, dadurch gekennzeichnet, dass die Schraube eine darin ausgebildete Öffnung enthält, und dass der Verbinder mit Gehäuse (
26 ) Vergussmaterial zur Befestigung zumindest des Empfängers in der Öffnung einschließt. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass zumindest der Empfänger selektiv von dem Anker (
14 ) trennbar ist. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der vorhergehenden Ansprüche, gekennzeichnet durch einen Gewindebereich (
14 ) und einen Greifbereich (31 ), der es ermöglicht, dass zumindest ein Teil des Gewindebereiches (14 ) in die anatomische Struktur eingeschraubt wird. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem de vorhergehenden Ansprüche, dadurch gekennzeichnet dass Verbinder mit Gehäuse (
26 ) selektiv von dem Anker (14 ) trennbar ist. - Integrierte chirurgische Anker-/Lokalisierungssensor-Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Verbinder mit Gehäuse (
26 ) über eine Verkeilungsanordnung an dem Anker (14 ) befestigbar ist.
