Abstract
Ein Implantat (1) weist eine flächige Grundstruktur aus einem Gestricke oder Gewirke auf, das in einem zentralen Bereich (3) eine größere Dehnbarkeit hat als im peripheren Bereich (4) des Implantats (1). Die Grundstruktur kann im zentralen Bereich (3) des Implantats (1) zu einem Vorsprung (30) ins Dreidimensionale verformt sein. Vorzugsweise weist das Implantat (1) im Bereich des freien Endes (32) des Vorsprungs (30) eine Versteifungsstruktur (22, 24) auf. Das Implantat (1) eignet sich besonders zur Behandlung von Hernien.
Description
Die Erfindung betrifft ein chirurgisches Implantat, dessen
bevorzugte Ausführungsformen zur Behandlung von Leistenhernien
verwendet werden können.
Aus der WO 92/19162 ist ein Implantat zum Verschließen einer
durch eine Hernie verursachten Öffnung in einer Körperwand
bekannt. Das Implantat besteht aus einer flächigen Komponente,
von deren zentralem Bereich eine Art Stopfen ausgeht. Optional
ist am freien Ende des Stopfens eine weitere flächige Komponente
angebracht. Dieses Implantat lässt sich zusammenfalten und mit
Hilfe eines Rohres an die Operationsstelle heranführen. Dort wird
das Implantat aus dem Rohr herausgeschoben, so dass der Stopfen
die zu verschließende Öffnung ausfüllt, während sich die flächige
Komponente und gegebenenfalls die optionale zusätzliche flächige
Komponente seitlich am Gewebe anlegen. Damit sich das Implantat
nach dem Ausschieben aus dem Rohr selbsttätig entfaltet, können
federnde Versteifungsstrukturen vorgesehen sein.
Ein weiteres Implantat zur Reparatur einer Hernie in einer
Muskelwandung ist in der WO 92/13500 beschrieben. Dieses
Implantat enthält eine flächige Komponente, von der eine
Ausstülpung ausgeht, die mit Hilfe einer Klappe verschließbar
ist. Die Ausstülpung wird in den Herniendefekt eingeführt und zur
Stabilisierung mit einer viskosen Flüssigkeit gefüllt, deren
Auslaufen die Klappe verhindert.
Die vorbekannten Implantate haben eine relativ große Masse und
bilden daher im Patienten einen relativ großen Fremdkörper, was
für den Heilungsprozess nicht vorteilhaft ist.
Es ist Aufgabe der Erfindung, ein Implantat mit relativ geringer
Masse zu schaffen, das als sicheres und problemlos einsetzbares
Implantat zur Behandlung einer Hernie ausgestaltet sein kann.
Diese Aufgabe wird gelöst durch ein Implantat mit den Merkmalen
des Anspruchs 1. Vorteilhafte Ausgestaltungen der Erfindung
ergeben sich aus den Unteransprüchen.
Das erfindungsgemäße Implantat weist eine flächige Grundstruktur
aus einem Gestricke oder Gewirke auf. Das Gestricke oder Gewirke
der flächigen Grundstruktur hat in einem zentralen Bereich des
Implantats eine größere Dehnbarkeit als im peripheren Bereich des
Implantats; der Unterschied in den Dehnbarkeiten beträgt
vorzugsweise zumindest in einer Richtung mindestens 20%. (Die
Dehnbarkeit einer Probe ist umgekehrt proportional zu der zur
Erzielung einer vorgegebenen relativen Längenänderung von z. B.
5% erforderlichen Kraft.) Diese größere Dehnbarkeit wird vorzugs
weise bereitgestellt, indem die Masse pro Flächeneinheit des
Gestrickes oder Gewirkes der flächigen Grundstruktur in einem
zentralen Bereich des Implantats geringer ist als im peripheren
Bereich des Implantats. Die geringere Masse pro Flächeneinheit
kann dadurch erreicht werden, dass die Maschendichte des
Gestrickes oder Gewirkes der flächigen Grundstruktur in einem
zentralen Bereich des Implantats geringer ist als im peripheren
Bereich des Implantats. Vorzugsweise ist das Gestricke oder
Gewirke einstückig gefertigt (z. B. unter Verwendung eines
einheitlichen Garns). Für die Form des Bereichs der Grundstruktur
mit größerer Dehnbarkeit sind viele Möglichkeiten denkbar; so
kann dieser Bereich z. B. quadratisch, rechteckig, kreuzförmig,
vieleckig, rund oder auch in anderer Form ausgestaltet sein.
Die flächige Grundstruktur des Implantats ist also in einem
zentralen Bereich dehnbarer und vorzugsweise leichter oder
weniger dicht als im Randbereich, hat aber dort auch bei einer
geringeren Materialmenge pro Flächeneinheit eine ausreichende
Stabilität. Der Begriff "zentraler Bereich" ist weit gefasst zu
verstehen und nicht auf das geometrische Zentrum beschränkt.
Ein besonderer Vorteil des Implantats kommt dann zum Tragen, wenn
bei bevorzugten Ausführungsformen die Grundstruktur in einem
zentralen Bereich des Implantats (also in einem Bereich mit
größerer Dehnbarkeit der Grundstruktur) zu einem Vorsprung ins
Dreidimensionale verformt ist. Der Vorsprung ist vorzugsweise als
hohle Ausstülpung gestaltet und kann im Bereich seines freien
Endes pilzartig verbreitert sein. Wegen der größeren Dehnbarkeit
der Grundstruktur in dem zentralen Bereich des Implantats lässt
sich der Vorsprung ausformen, ohne dabei das Material des
Implantats zu überdehnen, was zu einem unerwünschten Festigkeits
verlust führen würde. Aus einer gleichmäßig dichten flächigen
Grundstruktur, etwa einem herkömmlichen Implantatnetz, könnte ein
vergleichbarer Vorsprung nicht ohne weiteres gestaltet werden;
es käme vielmehr zu einer unerwünschten Faltenbildung oder zu
einer lokalen Überdehnung des Materials bis hin zur Rissbildung
und einem damit verbundenen unerwünschten Festigkeitsabfall.
Vorzugsweise ist der Vorsprung des erfindungsgemäßen Implantats
formstabilisiert. Dies lässt sich z. B. mit Hilfe einer Wärmebe
handlung erreichen, der das Implantat nach Ausbildung des
Vorsprungs unterzogen wird. (Der oben verwendete Begriff "Dehn
barkeit" bezieht sich auf die Grundstruktur, nicht auf den
fertig ausgebildeten Vorsprung, dessen Materialeigenschaften
durch den Formungs- und Stabilisierungsvorgang beeinflußt werden
können.)
Wenn das Implantat zur Behandlung einer Hernie, z. B. eines
Leistenbruchs, vorgesehen ist, wird der Vorsprung in den
Herniendefekt eingeführt, während die flächige Grundstruktur im
weitgehend ebenen peripheren Bereich des Implantats in der
Umgebung der Hernie an den Körperwandstrukturen angelegt und
gegebenenfalls daran befestigt wird. Da die Grundstruktur aus
einem Gestricke oder Gewirke hergestellt ist, ist sie flexibel,
und auch der Vorsprung hat selbst nach einer formstabilisierenden
Wärmebehandlung eine ausreichende Flexibilität. Im Laufe des
Heilungsprozesses werden die Maschen des Gestrickes oder Gewirkes
von Gewebe durchwachsen. Dabei wirkt sich außer der Flexibilität
des Implantats auch eine relative geringe Masse günstig aus.
Bei bevorzugten Ausführungsformen weist das erfindungsgemäße
Implantat im Bereich des freien Endes des Vorsprungs eine
Versteifungsstruktur auf. Diese Versteifungsstruktur kann ring-
oder scheibenartig sein und ist vorzugsweise faltbar. Dabei ist
es vorteilhaft, wenn die Versteifungsstruktur aus einem gefalte
ten Zustand selbstständig in einen entfalteten Zustand expandier
bar ist, z. B. durch die Wirkung einer Feder oder aufgrund des
Elastizitätsverhaltens des für die Versteifungsstruktur ver
wendeten Materials.
Die Versteifungsstruktur verstärkt den Vorsprung. Wenn der
Vorsprung im Bereich seines freien Endes pilzartig verbreitert
und dort mit der Versteifungsstruktur versehen ist, lässt sich
ein Defekt in einem Körpergewebe besonders zuverlässig verschlie
ßen. Denn in diesem Fall liegt an einer Seite des Gewebes der
periphere Bereich der Grundstruktur des Implantats an, während
der Vorsprung an der anderen Seite des Gewebes durch die
pilzartige Verbreiterung gesichert ist. Wenn sich die Ver
steifungsstruktur selbstständig entfaltet, lässt sich das
Implantat auf besonders schnelle und in einer den Patienten
schonenden Weise einsetzen. Dazu kann ein Rohr als Hilfsmittel
verwendet werden, in das das Implantat in zusammengefaltetem
Zustand eingesetzt ist. Nachdem das Rohr an die Operationsstelle
herangeführt ist, wird das Implantat z. B. mit Hilfe eines Stabes
aus dem Rohr ausgeschoben, so dass der Vorsprung in die zu
verschließende Öffnung eindringen und sich die Versteifungs
struktur entfalten kann.
Bei einer vorteilhaften Ausgestaltung weist die Versteifungs
struktur ein ring- oder scheibenartiges Trägerteil auf, von
dessen Peripherie über den Umfang verteilt mehrere elastische
Bögen ausgehen. Die elastischen Bögen bestehen vorzugsweise aus
Fadenmaterial, z. B. aus monofilem Polypropylen. Für das Träger
teil sind viele Ausgestaltungen denkbar. Es kann flach sein, aber
auch kalottenartig gekrümmt. Vorzugsweise ist es als Spritzteil
hergestellt, z. B. aus Polypropylen, wobei gegebenenfalls die
erwähnten elastischen Bögen während des Spritzvorgangs in ihren
Endbereichen miteingegossen werden.
Das erfindungsgemäße Implantat kann im Hinblick auf den be
absichtigten Verwendungszweck vollständig aus nichtresorbierbarem
Material, vollständig aus resorbierbarem Material oder sowohl aus
resorbierbarem als auch aus nichtresorbierbarem Material
bestehen. Für das Gestricke oder Gewirke der Grundstruktur kommen
Multifilamente und Monofilamente in Betracht. Ein bevorzugtes
nichtresorbierbares Material ist Polypropylen, ein bevorzugtes
resorbierbares Polyglactin 910 (ein Copolymer aus Glykolid und
L-Lactid im Verhältnis 9 : 1). Andere Polymere und Copolymere
lassen sich ebenfalls verwenden. Ein Gesichtspunkt für die
Materialwahl ist der Einsatzzweck des Implantats, von dem z. B.
die erforderliche Flexibilität und Reißkraft und das benötigte
Langzeitverhalten im Körper abhängen.
Im Folgenden wird das erfindungsgemäße Implantat anhand von
Ausführungsbeispielen näher beschrieben. Die Zeichnungen zeigen
in
Fig. 1 eine Fadenlaufdarstellung einer ersten Ausführungsform
des erfindungsgemäßen Implantats,
Fig. 2 eine schematische Darstellung der Fadenführereinteilung

ausgebildeten Vorsprung, der durch die Versteifungs
struktur gemäß Fig. 6 stabilisiert ist, wobei der
Vorsprung in einem Herniendefekt eingesetzt ist, und
Fig. 8 eine Maschendarstellung eines Gestrickes, das für den
zentralen Bereich der Grundstruktur eines erfindungs
gemäßen Implantats geeignet ist.
In Fig. 1 ist in einer dem Fachmann geläufigen Weise eine
Fadenlaufdarstellung einer ersten Ausführungsform eines Im
plantats 1 gezeigt. Das Implantat 1 weist eine Grundstruktur 2
auf, die im Ausführungsbeispiel aus einem Gewirke besteht, und
zwar aus Häkelgalonware. In einem zentralen Bereich 3 der
Grundstruktur 2, der im Ausführungsbeispiel kreuzförmig gestaltet
ist, ist die Masse pro Fächeneinheit geringer als im peripheren
Bereich 4 der Grundstruktur 2. Im Ausführungsbeispiel wird dieser
Unterschied dadurch erreicht, dass die Anzahl der eingelegten
Teilschussfäden in die Franse des Gewirkes im zentralen Bereich 3
geringer ist als im peripheren Bereich 4, wie in Fig. 1
veranschaulicht. Das Gewirke der Grundstruktur 2 ist im Aus
führungsbeispiel in einem Stück gefertigt. Der periphere
Bereich 4 der Grundstruktur 2 kann sich über den in Fig. 1
eingezeichneten Umfang hinaus erstrecken.
Fig. 2 zeigt in einer dem Fachmann bekannten Darstellungsart die
Musterpatrone und die Fadenführereinteilung für die Ausführungs
form gemäß Fig. 1. Das Implantat 1 wurde als Häkelgalonware auf
einer Häkelgalonmaschine vom Typ "Raschelina RD3MT3" der Firma
Müller, Frick (Schweiz) hergestellt. Dabei wurden Polypropylenfä
den von 60 den in Franse und Polypropylenfäden von 140 den in
Schuss gearbeitet. Die Musterschüsse bilden den kreuzförmigen
zentralen Bereich 3 des Implantats aus. Der Verlauf der Fäden
ergibt sich aus der Musterpatrone und aus der Fadenführerein
teilung.
Die Fig. 3 veranschaulicht die Musterpatrone und die Faden
führereinteilung einer zweiten Ausführungsform des Implantats,
bei der der zentrale Bereich mit 3' und der periphere Bereich mit
4' bezeichnet sind. Durch die Verwendung zusätzlicher Fadenführer
ergibt sich eine andere Umrisslinie des zentralen Bereichs 3' als
bei der Ausführungsform gemäß den Fig. 1 und 2. Die Her
stellung erfolgte auf derselben Häkelgolonmaschine und mit
demselben Fadenmaterial wie bei der ersten Ausführungsform.
Fig. 4 zeigt die Musterpatrone und die Fadenführereinteilung für
eine dritten Ausführungsform des Implantats, dessen zentraler und
peripherer Bereich hier mit 3" bzw. 4" bezeichnet sind. Es wird
deutlich, wie sich mit einer noch größeren Zahl von Fadenführern
eine Umrisslinie des zentralen Bereichs 3" mit einer noch
größeren Zahl von Ecken erreichen lässt, so dass sich hier quasi
die Umrisslinie einer auf der Spitze stehenden Raute ergibt. Es
wurden wieder die Häkelgalonmaschine und das Fadenmaterial wie
bei der ersten Ausführungsform verwendet.
Für die Form der zentralen Grundstruktur des Implantats sind
viele Gestaltungen denkbar, so außer den bereits betrachteten
kreuzförmigen und vieleckigen Formen auch eine rechteckige oder
quadratische Form, aber auch runde Formen. Letztere lassen sich
näherungsweise durch vieleckige Formen mit einer großen Zahl von
Ecken erreichen. Die flächige Grundstruktur des Implantats hat
bei den oben gezeigten Ausführungsformen in dem betrachteten
zentralen Bereich (der nicht zwingend um das geometrische Zentrum
herum angeordnet sein muss) eine geringere Masse als im periphe
ren Bereich. Dadurch ergibt sich für den zentralen Bereich eine
größere Dehnbarkeit der Grundstruktur. Grundsätzlich lässt sich
eine größere Dehnbarkeit im zentralen Bereich jedoch auch durch
Unterschiede in der Wirkart oder Strickart oder in der Material
wahl erreichen, sogar wenn die Masse pro Flächeneinheit im
zentralen Bereich des Implantats größer ist als im peripheren
Bereich.
Bei den Ausführungsformen gemäß den Fig. 1 bis 4 ist die
flächige Grundstruktur in einem Stück gefertigt. Es ist aber auch
denkbar, die Materialien für den zentralen Bereich und den
peripheren Bereich getrennt herzustellen und ein Materialstück
für den zentralen Bereich in eine Aussparung einzusetzen (z. B.
durch Aufsticken, Aufnähen oder Aufkleben), die in einem
Materialstück für den peripheren Bereich ausgebildet ist.
In der folgenden Tabelle 1 sind für verschiedene Gewirke, die mit
"Gewirke 1" bis "Gewirke 10" bezeichnet sind, das Material und
die Maschenstruktur angegeben.
"Pronova" ist eine Markenbezeichnung der Ethicon GmbH für
Poly(vinylidenfluorid-co-hexafluorpropylen).
Die für eine Dehnung der Gewirke aus Tabelle 1 in Höhe von 5%
erforderliche Kraft in verschiedenen Richtungen ist in Tabelle 2
dargestellt. Ferner zeigt die Tabelle 2 die Dicke und das
Flächengewicht dieser Gewirke und zusätzlich die Vergleichsdaten
für zwei herkömmliche Implantatnetze, die in der Tabelle mit A
und B bezeichnet sind. Das Netz A (von der Ethicon GmbH unter der
Bezeichnung "Prolene Netz" vertrieben) ist aus einem Polypropy
len-Monofilament mit einer Stärke von 6 mil gefertigt. Das Netz B
besteht ebenfalls aus Polypropylen-Monofilamenten und wird von
der Atrium Medical Corporation, 5 Wentworth Drive, Hudson, NH
03051, U.S.A., unter der Bezeichnung "atrium POLYPROPYLENE MESH"
vertrieben.
Die für eine Dehnung von 5% erforderliche Kraft wurde mit Hilfe
eines Prüfgeräts "ZWICKI 1120" der Firma Zwick GmbH, Ulm, an
Proben einer Breite von 60 mm und einer Länge von 100 mm
bestimmt. Die in Längsrichtung gemessene Einspannlänge der
jeweiligen Probe betrug dabei 25 mm. Die sich in Querrichtung
erstreckende Probenbreite entsprach der Breite von 60 mm der
gummierten Backen des Prüfgeräts. Mit einer Geschwindigkeit von
2 mm/min wurde eine Vorkraft von 0,1 Newton aufgebaut. Danach
wurde der Abstand der Backen mit einer Prüfgeschwindigkeit von
20 mm/min solange vergrößert, bis eine Probendehnung von 5%
erreicht war. Die dabei auftretende Kraft wurde gemessen und ist
in Tabelle 2 für verschiedene Richtungen der Wirkstruktur der
einzelnen Proben angegeben.
Die Struktur des Gewirkes 5 stimmt mit der Wirkstruktur des
zentralen Bereichs 3 des Implantats 1 gemäß den Fig. 1 und 2
überein. Die Tabelle 2 zeigt, dass die Dehnbarkeit des Gewirkes 5
besonders hoch ist. Auch das Gewirke 9 hat eine große Dehn
barkeit. Die Strukturen der Gewirke 5 und 9 eignen sich daher
besonders für den zentralen Bereich eines Implantats der hier
betrachteten Art. Die Strukturen der übrigen Gewirke, die eine
geringere Dehnbarkeit und im Allgemeinen auch ein höheres
Flächengewicht haben, kommen insbesondere für den peripheren
Bereich eines derartigen Implantats in Frage. Für den peripheren
Bereich sind jedoch auch Strukturen wie bei den herkömmlichen
Netzen, deren Dehnbarkeit generell noch geringer ist, denkbar.
Bei bevorzugten Ausführungsformen des Implantats ist die
Grundstruktur in dem besonders dehnbaren zentralen Bereich zu
einem Vorsprung ins Dreidimensionale verformt. Einen schemati
schen Schnitt durch ein fertig ausgeformtes Implantat 1 zeigt die
Fig. 7; zunächst werden aber anhand von Fig. 5 verschiedene
Gestaltungen für Formen zur Ausbildung des Vorsprungs erläutert.
Die Fig. 5 zeigt in den Teilen (a) bis (d) vier Beispiele für
derartige Formen in schematischem Längsschnitt. Dabei ist jeweils
eine rotationssymmetrische Form 10a, 10b, 10c bzw. 10d auf einer
Basisplatte 12 angebracht. Vorzugsweise bestehen die jeweilige
Form 10a, 10b, 10c bzw. 10d und die Basisplatte 12 aus Metall.
Um die nach ihrer Herstellung flächige Grundstruktur eines
Implantats in Dreidimensionale zu verformen, wird die Grund
struktur zunächst auf eine Temperatur erwärmt, die noch deutlich
(z. B. mindestens 10 K) unter dem (niedrigsten) Schmelzpunkt des
Materials liegt, aus dem die Grundstruktur besteht. Anschließend
wird die Grundstruktur in einem zentralen Bereich an einer der
Formen 10a bis 10d (oder auch an eine anders gestaltete Form)
angedrückt, während der periphere Bereich der Grundstruktur an
der Basisplatte 12 an liegt.
Bei der erhöhten Temperatur erweicht das Material der Grund
struktur, so dass durch die Form 10a, 10b, 10c bzw. 10d ein
Vorsprung in Gestalt einer hohlen Ausstülpung in der Grund
struktur ausgebildet wird, ohne dass das Material der Grund
struktur dabei infolge der auftretenden Dehnungen überbeansprucht
wird oder gar reißt. Bei der Formgebung erweist es sich als
günstig, dass das Gestricke oder Gewirke in der Zone, die von der
Form 10a, 10b, 10c bzw. 10d gedehnt wird, von vornherein für eine
größere Dehnbarkeit konzipiert ist.
Vorzugsweise wird nach dem Formen des Vorsprungs eine Wärmebe
handlung an dem Implantat durchgeführt, um den Vorsprung zu
stabilisieren. Wenn das Implantat (wie in den Ausführungsbei
spielen) aus Polypropylen gefertigt ist, ist eine thermische
Behandlung im Ofen bei 150°C für 2 Stunden besonders geeignet.
Nach Formung des Vorsprungs und Beendigung der thermischen
Behandlung behält das Implantat seine Form mit dem dreidimensio
nalen Vorsprung und ist sowohl im peripheren Bereich als auch im
Bereich des Vorsprungs flexibel.
In Fig. 6 ist ein Beispiel für eine Versteifungsstruktur 20
dargestellt, mit deren Hilfe der Vorsprung im Bereich seines
freien Endes stabilisiert werden kann, wie weiter unten anhand
von Fig. 7 näher erläutert. Die Versteifungsstruktur 20 enthält
im Ausführungsbeispiel ein ringartiges Trägerteil 22, das als
Spritzteil aus Polypropylen hergestellt ist. Beim Spritzvorgang
werden in den Randbereich des Trägerteils 22 elastische Bögen 24
eingegossen, die im Ausführungsbeispiel aus monofilem Polypropy
len bestehen. Dabei sind die insgesamt acht Bögen 24 gleichmäßig
über den Umfang des Trägerteils 22 verteilt, siehe Fig. 6 (b).
Das Trägerteil 22 ist leicht kalottenartig nach unten gekrümmt,
so dass auch die Bögen 24 in der Darstellung nach Fig. 6 (a)
etwas nach unten weisen.
Die Versteifungsstruktur 20 lässt sich zusammenfalten oder
zusammenlegen, indem die flexiblen Bögen 24 auf das Zentrum des
Trägerteils 22 zu gedrückt werden. Werden die Bögen 24 wieder
freigegeben oder losgelassen, dehnen sie sich selbsttätig zu
ihrer ursprünglichen Form (oder zumindest weitgehend zu ihrer
ursprünglichen Form) aus, so dass die Versteifungsstruktur 20
wieder den in der Fig. 6 dargestellten entfalteten Zustand
einnimmt.
Die Fig. 7 veranschaulicht ein Implantat 1 mit der Grundstruktur
gemäß den Fig. 1 und 2, bei dem in dem zentralen Bereich 3 in
der oben beschriebenen Weise ein Vorsprung 30 ausgebildet ist.
Das Gewirke hat im Bereich des Vorsprungs 30 eine physiologisch
akzeptable Porengröße. Zum Formen des Vorsprungs 30 wurde die
pilzartige Form 10b verwendet. Der Vorsprung 30 ist daher im
Bereich seines freien Endes 32 pilzartig verbreitert. In diesem
Bereich ist die anhand von Fig. 6 beschriebene Versteifungs
struktur 20 in den Vorsprung 30 eingesetzt. Der periphere
Bereich 4 der Grundstruktur bildet den weitgehend ebenen
Randbereich 34 des Implantats 1.
Die Fig. 7 zeigt, wie das Implantat 1 in einen Herniendefekt 40
eingesetzt ist, der zwischen Gewebestrukturen 42 vorliegt. Andere
Gewebestrukturen, wie Haut oder Peritoneum, sind in Fig. 7 nicht
eingezeichnet. Vorzugsweise kommt die pilzartige Verbreiterung
des Vorsprungs 30, die durch die Versteifungsstruktur 20
stabilisiert ist, zwischen den Gewebestrukturen 42 und dem
Peritoneum zu liegen. Um das Implantat 1 in den Herniendefekt 40
einzusetzen, kann das Implantat 1 zusammengefaltet und in ein
Rohr gesteckt werden, dessen Außendurchmesser ungefähr so groß
ist wie der Herniendefekt 40. Mit Hilfe dieses Rohres lässt sich
das Implantat 1 durch den Herniendefekt 40 führen. Wenn das
Implantat 1 z. B. mit Hilfe eines Stabs aus dem Rohr ausgeschoben
wird, entfaltet sich die Versteifungsstruktur 20 selbsttätig und
gibt dabei dem Vorsprung 30 die in Fig. 7 dargestellte Form.
Bei den bisher beschriebenen Ausführungsformen ist die Grund
struktur des Implantats gewirkt. Gestricke können aber ebenso
verwendet werden. Die Fig. 8 zeigt die Maschendarstellung eines
Gestrickes, das für den zentralen Bereich eines Implantats
geeignet ist. Dieses Gestricke ist eine kombinierte Einfaden-
Maschenware 50, die unterschiedlich große Fadenschleifen
aufweist. Im Ausführungsbeispiel ist als Material Polypropylen
verwendet (60 den für die Franse und 140 den für den Schuss).
Claims (16)
1. Implantat, mit einer flächigen Grundstruktur (2) aus einem
Gestricke oder Gewirke, wobei das Gestricke oder Gewirke der
flächigen Grundstruktur (2) in einem zentralen Bereich (3)
des Implantats (1) eine größere Dehnbarkeit aufweist als im
peripheren Bereich (4) des Implantats (1).
2. Implantat nach Anspruch 1, dadurch gekennzeichnet, dass die
Masse pro Flächeneinheit des Gestrickes oder Gewirkes der
flächigen Grundstruktur (2) in einem zentralen Bereich (3)
des Implantats (1) geringer ist als im peripheren Bereich
(4) des Implantats (1).
3. Implantat nach Anspruch 1 oder 2, dadurch gekennzeichnet,
dass die Maschendichte des Gestrickes oder Gewirkes der
flächigen Grundstruktur (2) in einem zentralen Bereich (3)
des Implantats (1) geringer ist als im peripheren Bereich
(4) des Implantats (1).
4. Implantat nach einem der Ansprüche 1 bis 3, dadurch gekenn
zeichnet, dass das Gestricke oder Gewirke einstückig gefer
tigt ist.
5. Implantat nach einem der Ansprüche 1 bis 4, dadurch gekenn
zeichnet, dass die Grundstruktur (2) in einem zentralen Be
reich (3) des Implantats (1) zu einem Vorsprung (30) ins
Dreidimensionale verformt ist.
6. Implantat nach Anspruch 5, dadurch gekennzeichnet, dass der
Vorsprung (30) als hohle Ausstülpung gestaltet ist.
7. Implantat nach Anspruch 5 oder 6, dadurch gekennzeichnet,
dass der Vorsprung (30) im Bereich seines freien Endes (32)
pilzartig verbreitert ist.
8. Implantat nach einem der Ansprüche 5 bis 7, dadurch gekenn
zeichnet, dass der Vorsprung (30) formstabilisiert ist.
9. Implantat nach einem der Ansprüche 5 bis 8, dadurch gekenn
zeichnet, dass das Implantat (1) im Bereich des freien
Endes (32) des Vorsprungs (30) eine Versteifungsstruk
tur (20) aufweist.
10. Implantat nach Anspruch 9, dadurch gekennzeichnet, dass die
Versteifungsstruktur (20) ring- oder scheibenartig ist.
11. Implantat nach Anspruch 9 oder 10, dadurch gekennzeichnet,
dass die Versteifungsstruktur (20) faltbar ist.
12. Implantat nach Anspruch 11, dadurch gekennzeichnet, dass die
Versteifungsstruktur (20) aus einem gefalteten Zustand
selbstständig in einen entfalteten Zustand expandierbar ist.
13. Implantat nach Anspruch 12, dadurch gekennzeichnet, dass die
Versteifungsstruktur (20) ein ring- oder scheibenartiges
Trägerteil (22) aufweist, von dessen Peripherie über den
Umfang verteilt mehrere elastische Bögen (24) ausgehen.
14. Implantat nach Anspruch 13, dadurch gekennzeichnet, dass die
elastischen Bögen (24) aus Fadenmaterial bestehen, vorzugs
weise aus monofilem Polypropylen.
15. Implantat nach Anspruch 13 oder 14, dadurch gekennzeichnet,
dass das Trägerteil (22) kalottenartig gekrümmt ist.
16. Implantat nach einem der Ansprüche 1 bis 15, dadurch gekenn
zeichnet, dass die Form des Bereichs (3) der Grund
struktur (2) mit größerer Dehnbarkeit ausgewählt ist aus der
folgenden Gruppe von Formen: quadratisch, rechteckig,
kreuzförmig (3), vieleckig (3', 3"), rund.
