Abstract
选择性地激活已受爱滋病病毒感染的或具有感染危险的人体细胞内潜在天然防御系统。本发明描述了从微妙的早期免疫学损害到晚期疾病状况的临床各期爱滋病病毒相关病变的特异性治疗手段。同时还叙述了预防或阻止爱滋病病毒相关病变的方法,如将缺对ds RNA引入用于人体的(如输血)或环绕人体(如透析)的各种血液产品或生物液。
Description
爱滋病(AIDS.获得性免疫缺陷综合症)已构成了二十世纪对于人类健康的主要威胁。由于它的生物学可变性,它在人类个体之间难以捉模的传播方式,以及它的使快速检测它的存在的方法无能为力的潜伏期,爱滋病已经迅速发展成为一种前所未有的流行病疑难病症。实际上上,即使采用那些已经应用到提高诊断能力中去的值得考虑的对策,现有技术仍不能在世界上任何地方使这种疾病得以有效地控制。(参见国家卫生研究所〔Bethesda,Maryland〕疾病控制中心和国家卫生研究所药物与生物制品中心于1985年7月31日召集的题目为“Experienee With HTLV-Ⅲ Antibody Testing,an Update on Screening,Laboratory and Epidemiological Correlations”的专题讨论会的会议记录,并参见Budiansky,S.《Nature》,316卷,96页,1985年7月11日)对于爱滋病病毒的存在有两种不同的标志。LAV是对法国巴黎巴士德研究所分离出来的爱滋病病毒的简称,HTLV-Ⅲ是美国马里兰洲贝塞斯达国立卫生研究所分离出来的爱滋病病毒的简称。本文中经常提到的爱滋病病毒是一般的或是指定名称的。
现行治疗方案采用下列两种方法之一:免疫学方法(疫苗产生)或直接攻击病毒本身(抗病毒疗法)。虽然疫苗产生方法开始时显示出极大的前途(至少在理论上如此),但目前有关病毒组成的新的知识却在很大程度上打破了这种希望:即该病毒显然很容易突变或改变它的基本生化结构,从而关健性的病毒抗原“决定基”-疫苗发挥有效作用所必需的结构-就很容易发生突变或改变。例如,最近对加利福尼亚洲、马里兰洲和英格兰分离出来的HTLV-Ⅲ病毒的基因组成进行精细分析后,发现它们彼此明显不同。这些研究暗示,通过接种疫苗来抵抗爱滋病。将不会具有象直接抗脊髓灰质炎病毒,麻疹病毒等的大部分其它疫苗一样的广泛而持久效果。
对于另一种疗法,即直接抗病毒疗法,其它科学家已经试验过几种化合物,包括HPA-23,苏拉明、ribavirin。干扰素和fascarnet等。这些药物已经引入到实验室或者临床研究中,然而迄今几乎没有获得任何治疗成功的证据。事实上,在几乎每一种情况下都有关于其高毒性和/或剧烈付作用的报导(如见1985年8月5日由W.Clack在《Newsweek》〔第71页〕上发表的综述和由C.Wallis于1985年8月12日在《Time》上发表的文章〔40-47页〕)。实际上这些毒药中没有一种是新的或能够有选择性地直接抵抗潜在紊乱的,即爱滋病毒病毒在某些细胞中繁殖的。例如:HPA-23是一种含重金属的组合物(使人联想到二十年代或者青霉素问世前的人们利用砷剂治疗性病);苏拉明实际上是一种抗寄生(睡眠病)化合物,而fascarnet是一种抗疱疹病毒化合物。后两种化合物可以抑制一种与爱滋病病毒相关的酶的活性,这种酶被称作“逆转录酶”,但是,实际上还没有证据证明象这样一种酶促抑制会导致任何治疗或疾病的予防/改善作用。与此类似,干扰素作为一种象上面提到过的化合物一样的有毒的和无特异的化合物,对于与爱滋病病毒有关的肿瘤的治疗可能具有有限的作用,但是大量的研究表明它作为一种抵抗爱滋病病毒或大部分其它病毒的抗病毒剂,其前途是渺茫的。
缺对双链RNA是大分子RNA的已知形式之一(见美国专利4,024,222号和美国专利4,130,641号),在该形式中,由于双螺旋不稳定,而妨碍了碱基配对。人们所熟知的缺对双链RNA的干扰素诱导特性表明了一种与干扰素的诱导本身无关的机制(如,见1983年8月15日递交的题为“双链RNA对肿瘤细胞的抗增殖作用”的欧洲专利申请833054265号)。一个典型的用于治疗的缺对双链RNA是通过多次黄嘌呤核糖核苷酸与多胞嘧啶/尿嘧啶核糖核苷酸的复合所形成的双股RNA,例如,美国马里兰洲Rockville HEW Research公司的商标为“Ampligen”的产品rIn·r(C12U)n,其中的n为4至29的数值。已研究了许多与Ampligen具有相似行为的缺对dsRNA聚合物。与天然和/或合成的其它形成的dsRNA相比,缺对dsRNA在治疗上的关健优点是它们在动物或人体内的毒性较小。例如,Lampson等人在美国专利3,666,646号中提到双股RNA(dsRNA)的早期复合体,它们虽然能够引出与干扰素相关的各种效应,但是它们的毒性排除了它们在癌症或相关机能失调治疗上的任何临床可用性。
本发明人在其最近的教科书上已经详细叙述了各种干扰素、干扰素诱导物的已知活性(如见R.W.Ottenfite和G.B.Buttes.Wauel Dekkes主编的《Anticancer and Interferon Agents》一书中由发明人所撰写的一章,301-319页。New Yock,1984;另见由P.E.Come和W.A.Cactes编写的《Handbook of Experimental Pharmacology on Interferon》,1984由Spcinges Vechag.New Yock和Heidelkeg出版,535-555页详细叙述了dsRNAs的已知性质)。
现在已经知道缺对dsRNA如Ampligen具有抗某些人类肿瘤,特别是肾癌的临床效果。虽然目前认为dsRNA为一种纯粹的“干扰素诱导物”,但正是欧洲专利申请83305426.5号(本申请人是其发明人之一)中所充分描述的,dsRNA对某些完全抗干扰素本身的人类肿瘤是有效的。
与缺乏抗某些肿瘤的活性相似,现已证明所有类型干扰素实际上在对付爱滋病病毒方面均没有明显的活性
。特别使人感到惊奇的是,已知作为“干扰素诱导物”的缺对dsRNAs在治疗与爱滋病相关的机能失调中具有应用价值。
本发明叙述了以Amplgen作为举例的缺对dsRNA的另一个新的和使人意想不到的特性,该特性对于与爱滋病相关的机能调的治疗具有使人惊奇的广泛用途。
本发明公开了有选择性的抑制人类病毒,尤其是爱滋病病毒,而对正常细胞无显著毒性的治疗方法;阻止、推延或预防人体受爱滋病病毒感染的方法;一种预防或治疗与爱滋病相关的机能失调的方法;处理人体细胞和组织产物以预防或阻止被爱滋病病毒感染或毒害的方法;以及纠正与爱滋病病毒有关的特定损害的方法,包括免疫系统内细胞上干扰素受体的丧失,细胞内2′-5′A合成酶的减少和免疫系统中被选择细胞内dsRNAs的减少或丧失。另外,也揭示了用于上述方法的医药组合物。
本发明的方法适合于纠正与爱滋病病毒相关特定损伤,包括免疫系统中的各种关健细胞上干扰素受体的丧失,免疫/机体防御系统以及免疫/机体防御系统中各种关键细胞内dsRNA的减少/丧失。
本发明所指的缺对dsRNA是一类选自poly(Cn,U)和poly(Cn,G)的共多聚核苷酸(其中的n取4到29的任何整数),同时是多次黄嘌呤核苷酸和多胞嘧啶核苷酸复合物的类似物,是通过修变rIn·rCn以沿多胞嘧啶核苷酸链(rCn)掺入未配对碱基(尿嘧啶或鸟嘌呤)而形成的,或者是通过修饰多次黄嘌呤核苷酸(rIn)的核糖基骨架而形成的。这些用rInr(C12,U)n来表示的rIn的缺对类似物已在Caites和Ts′o的美国专利4,130,641号和4,024,222号中叙述过,在此列入本文的参考文献。
本发明所指的药物组合物包括缺对dsRNA,也可以是干扰素,以及药理上可接受的载体或稀释剂。本发明所采用的药物组合物是适于在合适的药物载体中通过非肠道给药的。这样,就可按照现有医药技术的要求,用无菌水或无热原水作为溶剂或稀释剂来制备非肠道溶液,悬浮液和分散体系。另外,也可用生理状态可接受的盐作为稀释剂。
很清楚,存在于任何剂量单位中活性成分的绝对量不应超过与所用给药的比率及方式相符的数量,但另一方面,其用量也应足够以小的剂量达到预期的给药比率。此外,给药比率还将取决于所期望的药理作用。
给与的缺对dsRNA的量达到每毫升体液0.01到1000毫克是足够的。当与干扰素同时给药时则所加干扰素的量应达到使其最终浓度为每毫升体液0.1-100,000IRU。这里所指的体液是血清、盐、维生素等的溶液。它们在体内循环且浸润组织。
按下列要求将缺对dsRNA配成水溶液,即将其加入到各种人体细胞中达到其暂时浓度为每毫升液体0.01-250毫克。研究中使用了各种不同的细胞,各种用于HTLV-Ⅲ(爱滋病病毒)感染的潜在靶细胞。
下面所叙述的是用H-9细胞进行的典型实验,该细胞是一种能被HTLV急性或慢性感染的人淋巴样细胞。将其在标准条件下培养(如见Mitsuya等人,Science volame226,papage172,1984)着重对HTLV酶或HTLV特异性蛋白的存在进行几周的分析,在加入各种浓度的Ampligen之前,之后或同时加入病毒以模拟各种临床状态。最重要的是仔细分析细胞的数量,形态学特征等,以检测是否象Mitsuya所述使用其它抑制剂一样,ds RNA对于淋巴样细胞的生长具有各种非特异性效应。在实验过程的不同时间,分离细胞采用lee等人(Biochemistry,24卷,551页,1985)所述的方法,对2′5′-A寡聚物进行HPLC分析。某些A寡聚物当以自然状态存在时,能够使细胞获得抗病毒特性,但是以前并没叙述过或预见到一种试验化合物(人工合成的)能够精确地引发这种有选择性的自然反应,从而增强抗一般的以及特别是爱滋病病毒的自然防御机制。

实验A是用25倍于靶细胞的病毒(称为感染单位)作的,而实验B是用10倍于病毒的细胞(指定为H-9细胞)作的。阳性细胞百分率是指那些用免疫萤光法测定的定名为p24和p18的,表达了HTLV-Ⅲ标记物的细胞;逆转录酶是指一种采用被称为poly rA/dT的标准模板在细胞上清液中所测得的病毒酶。同时,进行细胞计数:在所试验的各种浓度Ampligen时,细胞以正常的生长速度繁殖(高达每毫升300毫克)。在实验A和B中,Ampligen(50毫克/毫升)在第一天被加进去。HPLC分析显示(在实验A或B的3-7天)在2′,5′-oligomer的图形有一特异的偏移,其结果是高分子量的寡聚物(很少抗病毒活性)转变为低分子量的(最大抗病毒活性)对爱滋病病毒具有选择性的和强大的抵抗作用的寡聚物。
用各种可能是人体内爱滋病病毒靶细胞的其它人细胞进行了类似实验,这些细胞包括:NK细胞(自然杀伤细胞),T辅助细胞,T抑制细胞和单核细胞等。在所有情况下,各种浓度的缺对dsRNA能够选择性地抑制和/或阻止HTLV的繁殖,而对正常细胞的生长和成熟无任何影响。培养的T细胞的增殖是采用所熟知的细胞生物学技术里面的IL-2因子的标准加入方法而维持的。缺对dsRNA对HTLV的选择性抑制始终与通过HPLC分析所测定的2′,5′-寡聚物的图形的特定偏移或提高有关。
因为dsRNA是“干扰素诱导物”,所以可以认为它是通过一种干扰素作用机制生效的,或者可将干扰素本身的活性看作是由dsRNA引出的。因此,下述实验通过证明面对由Ampligen所表现的十分有效而特异的抗爱滋病病毒活性,所有类型的干扰素实际上没有明显活性,而确证了本发明的独到之处。
在用LAV感染前18小时,用250I.U./毫升的重组α-干扰素或250I.U./毫升的天然β-干扰素或50I.U./毫升的天然γ-干扰素或50微克/毫升的Ampligen,将CEM(另一种人T细胞系)处理,培养15天后,就得到下列数据:

上述数据表明了Ampligen对于病毒复制的极度抑制作用。左边指示培养液内的逆转录酶活性。右边则指示含病毒抗原的培养物中细胞的百分数。
可见dsRNA(Ampligen)抑制(>99%)H9(T4)细胞和CEM(T4)细胞中的HTLV-Ⅲ/LAN感染。干扰素α、β、γ介于活性与非活性之间。Ampligen对病毒的抑制作用有很好的选择,而对细胞的生长则没有任何可测知的影响。
所以说,Ampligen是通过一种不同于干扰素的机制来抑制HTLV-Ⅲ/LAV病毒的复制的。这一发现提出了Ampligen和干扰素或其它药物能够结合使用。在此,Ampligen将抑制病毒的复制而干扰素则可用于刺激免疫系统,使其恢复疾病前的功能。
Ampligen对于天然抗病毒状态的恢复作用以图解的形式表示如下,Ampligen以抗病毒方式作用于AIDS/ARC组,恢复破坏病毒基因组的天然抗病毒RNA酶活性。
因此,该药物具有抑制病毒制的潜在能力,以及扩大免疫应答能力。因此,不能将Ampligen简单地看作是一种病毒的逆转录酶抑制剂,而是通过刺激人体免疫系统和在靶细胞内建立一种抗病毒状态的双重机制发挥作用。鉴于Ampligen能够治疗外源干扰素很难起作用的动物的病毒感染和病毒诱发的肿瘤,所以很容易把它与干扰素区别开来。通过这些观察即可将Ampligen与许多其它用于治疗这种疾病的药物区别开来。
在相关的研究中,已在爱滋病易感者(同性恋者)身上通过爱滋病的病理学研究,检定出意想不到的和以前未检出的生化损伤。这些损伤可被Ampligen纠正,并且解释了相对于干扰素或已被试验过的大部分其它化合物的微小活性(如果有的话),Ampligen的有效的抗病毒活性。
首先,在每六个爱滋病易感者(同性恋者)和每六个爱滋病受害者的T淋巴细胞中观察到关键性生化共因子dsRNA的丢失。在破碎细胞后按标准技术检测dsRNA。由此,可将Ampligen的效果准确地看作是人体在亚细细胞水平上抵抗病毒侵袭所必需的关键性生化统一体的特异性替代疗法。当天然存在有dsRNA或象通过Ampligen疗法一样加入dsRNA时,即产生一种活性RNase L以“吃掉”(水解)或破坏病毒mRNA,从而终止或遏制病毒感染。继在美国首先申请专利后,pseekle等人得出一个补充性的结论(见1985年9月发行的《Journal of Infectious Diseases》,457-465页,题目为“Interferon-Induced2′-5′Oligoadenylate Synthetase During Interferon Alpha Therapy inHomosexual Men With Kaposi′s Sarcoma:Marked Deficiency in Biochemical Response to Interferon in Patients With Acquired Immunodeficiency Syndrome”)。他们指出,向取自爱滋病受害者的免疫细胞中加入干扰素并没有导致预期的这种特异性细胞内防御途径(抵抗各种病毒的第一道防线)的增强。
另外,也发现爱滋病病毒感染本身导致了干扰素受体的进一步丧失或改变(图解显示于细胞表面上),从而使干扰素不能很好地结合并进而不能发动导致病毒感染被抑制和免疫能力的恢复/维持的关键生化过程的“级联”。还进一步观察到,异常的低水平2′-5′A合成酶可带来HTLV-Ⅲ的感染。据此,由本申请人认定爱滋病病毒感染本身的特征在于至少有下列三个方面的损害:(1)干扰素受体的丧失,(2)2′-5′A合成酶的减少,(3)细胞内dsRNA的异常降低;而这些有害的改变可特异地使用Ampligen的dsRNA疗法加以纠正。
对于危险个体(被或未被爱滋病病毒感染),dsRNA作用于生化损伤的下游或远端,从而恢复正常的抗病毒反应能力。
dsRNA可被细胞有效地摄入,因而不需要完整的IFN受体。更进一步,缺对dsRNA也可被用作促进细胞内的摄取和分配的生物活性片段。因此,dsRNA具有很容易地通过血脑屏障的潜力,在某些病人体内这种屏障可能是残存的爱滋病病毒储库。
dsRNA激活2′-5′A聚合酶,其促进活性抗病毒寡核苷酸合成能力较单独使用干扰素可提高500倍(dsBAN可使1.6×106个细胞产生2250毫微摩尔的2′-5′Aoligo A,而在同样情况下200单位/毫升的IFN被导致不到0.5毫微摩尔2-5oligo A的产生)。
本申请人在病情有进一步发展危险的病人或具有明显爱滋病症状者的NK细胞中观察到相似现象,即就AIDS/ARC病人自然杀伤细胞(NK)的活性水平来说存在有相似状况。然而,对NK调节的机制却所知甚少。
AIDS/ARC病人和“危险”人群中的健康成员通常具有微弱的免疫监视能力(功能性NK和T淋巴细胞)并且不能被干扰素重新激活。Ampligen则远离病变部位发挥作用且能激活细胞毒性淋巴细胞。
血液产品包括用于输血的全血或其一部分、“包装”(浓缩)的红细胞、包装的白细胞、血小板浓缩物或象免疫球蛋白一类的血清蛋白部分。这些血液产品在开始分离时或冷冻保藏前应加进适当浓度的缺对dsRNA。或者,在血液产品注射进接受者体内前立即加入适当浓度的dsRNA。在这种情况下操作人员只需简单地查阅一下接受者体重与其体液量具有内在联系的体液量标准表,该体液量是与dsRNA的必要量相平衡的血液液和体液量的总和。一个60至70公斤的病人大约具有10至12品脱或5至6升的体液量。
以上所述的血液产品可能含有未检测出的逆病毒,特别是与其有关的爱滋病相关病毒。目的是为上述的血液产品提供一个最终浓度以防止存在于该血液产品接受者的各种组织中的隐蔽逆病毒的“接种”。当污染的输液细胞或细胞产物在接受者的系统内混合和/或分布时,通过防止该逆病毒的成功接种,我们就可以防止威胁生命的病毒的繁殖。
另一些重要用途是病人的血液暂时地与献血者的血液或血液产品相遇的情形下,如通过有损伤的或用于传递逆病毒的膜时。这种设备的例子包括在心脏手术、心脏分流手术、器官移植等时所用的体外心泵和相关装置。这不同于全血或血液产品被输入病人体内的情形。通过一细胞膜的暂时暴露仅仅包括暂时暴露给可能污染的血液,暴露持续短至几分钟或长至几小时则依赖于外科的或其它的医疗步骤的长短。利用肾透析装置进行相似的暴露是可能的,其中存在的有效浓度的dsRNA将阻止未检测出的逆病毒经过细胞膜且在与这种设备相联的个体体内接种的可能性。
医学文献也证实了爱滋病相关病毒的通道经过鼻和泪分泌物。具有在患者之间转移逆病毒潜力的医疗设备和装置也使用本发明的缺对dsRNAs进行了充分处理。作为例证的例子包括用于帮助呼吸的吸入剂和吸入剂治疗设备以及用于检测和治疗眼睛的设备。含dsRNA的气雾剂可以喷到该设备的接触部分以防止由于大量使用而污染设备。
包含在或加进全血或血液产品中的dsRNA的量将依赖于所用血液或血液产品的整个稀释度。首先根据病人的大小测定出病人的组织容积,然后计算出dsRNA的量。被输入的血液产品需要更大浓度的dsRNA,因为与使用体外血泵或透析设备相比其稀释量相当大。用于不稀释的使用量最好在0.01-200mcg/每毫升体液。与此相比,对于通常由350和500毫升容器提供的全血,一个60公斤的带有5至6升体液量的病人将需要大约200毫克的dsRNA,以达到40mcg的平衡浓度。
实施例:
对一个带有与爱滋病相关的中间体严格的复合物的60公斤成年男子(淋巴结增大并且由于淋巴结的增大而几乎有一年不能吃固体食物,爱滋病病毒的浓度大于每毫升血液105病毒颗粒而没有肿瘤或其它感染迹象)采用静脉滴注法用200毫克与生理盐水混合的缺对dsRNA(Ampligen)处理30分钟。这种灌输导致当dsRNA在体内完全循环且与细胞处的体液充分平衡时其浓度达到大约每毫升体液40mcg(0.04毫克)。dsRNA在间隙为2至3天的6个连续疗程中被输入病人体内以足够清除病人体内的所有可检测的与爱滋病相关病毒和纠正病人的退化了的免疫功能。T4/T8淋巴细胞的比值提高50%提示了免疫功能的恢复,该比值通常被认为是对于逆病毒的总的免疫力的可靠尺度。进而言之,在治疗结束时,通过皮试所测定的病人免疫能力从服用缺对dsRNA之前所测定的无反应状态恢复到正常水平。病人在治疗结束时又能够吃固体食物且病情一步一步好转。
所观察到的病人以服用Ampligen前的无反应状态恢复转变成治疗结束时的正常状态是一个显著结果,具有令人信服的价值。
Claims (15)
1、一种制备用于预防或治疗爱滋病的组合物的方法,它是由抑制爱滋病量的dsRNA和抑制爱滋病量的干扰素组合而成。
2、如权利要求1所述的方法,其特征在于其中的dsRNA的量应在给药后使病人体内达到每毫升体液0.01至1000毫克的水平,干扰素的量达到每毫升体液0.01至1000毫克的水平。
3、如权利要求1所述的方法,其中dsRNA是缺对dsRNA。
4、如权利要求3所述的方法,其中缺对dsRNA是rIn·(C12I)n。
5、一种制备组合物的方法,其特征在于该组合物包括人血或其一部分以及抑制爱滋病有效量的缺对dsRNA。
6、一种制备组合物的方法,其特征在于该组合物包括人细胞和抑制爱滋病有效量的缺对dsRNA。
7、一种赋予人体来源的生物液以抵抗爱滋病病毒感染能力的方法,其特征在于使所说的生物液与以抑制爱滋病量的dsRNA混合或接触。
