Abstract

Description

具有延长胃滞留作用之可膨胀的 胃滞留性治疗系统
本发明描述一种延缓口服吸收之药物通过幽门之装置,此药物包括可在与胃液接触后膨胀之成分及可容许液体渗透的密闭式聚合物涂层。此装置所含的一种活性化合物主要是由其结合的药物控制而释放至胃液中。相较于其它常见延缓通过幽门的药物,活性化合物在释放时较不受该聚合物涂层之种类及组合所影响,而主要是由其合并形式决定。依据本发明之装置可轻易的滚制或折叠而充填入胶囊。
US Patent 4,207,890描述一种控制活性化合物释放之方法,其系由在胃中滞留膨胀而延长了在胃中的滞留时间。此装置包括(a)在投药前为折叠形式的聚合物涂层。聚合物涂层本身并无孔洞,实际上是由仅可供体液渗透的非水合材料组成。此装置并包括(b)可控制活性化合物释放的要件。依据其权利要求2,此要件可为聚合物涂层。另一项要件(c)是与体液接触后可膨胀的成分。
滞留作用的主要动力来自于渗透作用,要件(c)为可供渗透的活性物质,驱使水通过聚合物涂层(a)并在装置中引起溶液形成。装置在膨胀时因此含有水溶液,且其浓度能使活性化合物自装置中扩散出,然其视密度约略相当于食糜。此装置因此会在食糜中浮动或沉降。滞留构造完全是仰赖装置的大小。文中并未提及抽取方式,因此对于如何从胃中去除装置及于投用活性化合物后移除此装置则未交代。
此种滞留构造的改良型则出现在EP 0307904 A1及US Patent4,996,058。含有可膨胀成分的物质在酸之下会形成气体成分(例如:二氧化碳或氮)使聚合物涂层扩张而形成一种能在食糜上浮动之装置。滞留成分(参见US Patent 4,996,058第三段,第6至7行)系浮在食糜上,因而使胃中的装置不致于经由幽门倒出。此类装置有严重的缺点。在此两案例中,活性化合物的释放基本上系由聚合涂层控制。同时,此装置的充气动力原则上系受聚合涂层之限制,因为水及水合氢离子必须由聚合涂层扩散出,而活性化合物分子却必须以相反方向扩散。在聚合物涂层上之扩散常数是决定性的参数。对受到机械或化学作用损伤的较薄而敏感之聚合物涂层而言,就有活性化合物迅速释放的药剂倾出危险。不过,因产生气体而造成的膨胀程度与环境中的酸度,尤其食糜之组成和胃酸分泌有关。在因强酸而产生大量气体的环境下,装置受到的机械张力不得达到其破裂限值,另一方面,在酸性较弱的环境下,却必须产生充分的气体。此种矛盾使此装置在胃酸全天候变化的情况下处于不稳定状态。
因此本发明目的系在提供一种胃滞留系统以避免现有技术之此类系统缺点。
依据本发明,此类问题系以一种胃滞留装置解决,其中含有下列要件:
(a)由微多孔细胞膜或部份微多孔细胞膜或两者与其他非多孔膜类聚合物组合成的聚合物涂层。依据本发明孔隙可以任何方法产生,例如撑开薄膜、使用多相系统并挥发一部份系统、控制聚合程度(例如:以离子体形式)、以机械方法(例如针刺)、加热方法(例如雷射)或以离子辐射线照射再予以蚀刻,等。依据本发明微多孔的细胞膜可用的材料为:聚氨基甲酸酯、聚丙烯、聚乙烯基醇、聚乙烯基乙酸酯、聚丙烯酸及衍生物、聚甲基甲基丙烯酸及衍生物、聚碳酸盐、聚亚乙烯基二氟化物、聚四氟乙烯、及任何其他可提供适当孔度的所需聚合物。依据本发明,此类孔度系介于0.1至20μm,较佳者0.3至10μm,尤佳者0.5至1μm。视需要,为求能使孔隙较湿润,依据本发明细胞膜可浸渍在湿润剂或其他亲水性的成份中。
(b)可膨胀成分,其与胃液接触后会产生气体,例如:二氧化碳或氮,尤其是在酸作用下。依据本发明之此类实例有,例如,碱金属及碱土金属的碳酸盐及碳酸氢盐、铵离子或叠氮化钠或其混合物。此类可膨胀成分可视需要地变化以改良其气体产生动力,例如涂覆或嵌进亲脂成分中(例如蜡类或脂肪类)或适当的包覆剂(例如聚丙烯酸甲酯或聚甲基丙烯酸甲酯及衍生物)或本领域技术人员已知的相似物质。依据本发明,此可膨胀成分亦可形成泡沫(例如在与聚乙烯醇合并后或为半固体、药膏类制剂的形式)以利于控制抽气动力。
(c)一种内含活性化合物之成分,例如:多重微粒制剂、药片、胶囊或半固体、药膏类制剂、或泡沫。可用之活性化合物基本上均属于具有生理作用,尤其是人类用医药学或兽医药物的物质,较佳者可由胃黏膜吸收或在胃黏膜表面上作用。此种医药学活性化合物系为熟悉此技艺的专业人士所周知。成分(b)及(c)亦可在制剂中同时使用。
此装置可充填在生理可接受之材质制成的容器中(例如硬明胶胶囊)以便有助于投药及使用。
此装置的膨胀形式以形成一种充气的三角形较佳,因其乃是具有最大体积的平面构造。当其飘浮在食糜上时,由于此平面性,两个较稳定的位置均能使食糜经过胃滞留装置而不致于让食糜将充气形式由幽门推出。在平面形式中,三角形在膨胀状态下能兼具体积庞大且构造坚硬的特质,因此适用作为滞留装置。在胃中,此种体积庞大的构造嵌在胃壁之间,使食糜得以由幽门流出。嵌在内腔中必须承受食糜的流体抗性。装置愈趋近球形,愈有可能在胃出口处形成灯泡状密闭,以致于排空的压力将装置经幽门驱出。不过,依据本发明亦可采用四边形及多边形构造及具有平滑棱角者。
在使用微多孔或多孔聚合涂层后,依据本发明可解决许多问题。由于液体表面张力所致,尤其是水溶性系统,须用一定的压力将充填着液体之孔隙排空,主要是依其孔隙直径、表面张力及界面张力及接触角度而定。在选用适当的孔隙直径之下,可设定给定系统所需的压力限值,低于此值即无法排空孔隙。因而细胞膜是密闭的。不过,液体却能轻易地通过细胞膜;此液流系由孔隙数目和孔隙大小及培养液之黏度所决定。在使用特定孔隙度之微多孔细胞膜时,可依据本发明产生一种可容许液体轻易渗透、但在压力限值以下气体无法渗透的装置。因而此装置的内压被限定在预设值下。超过此压力限值时,细胞膜便可容渗透,且可避免在渗透破裂压力值以上对此装置造成机械张力,作为非破坏性的安全设计。因此,依据本发明之装置在使用时能维持几近稳定的内压。此装置的使用期可由可膨胀成分之种类及数量控制。毛细作用意指在细胞膜上之颗粒系润湿状,且其以些微迟延的方式透过微多孔的细胞膜释放活性化合物。
依据本发明之系统可运用在人类及兽医药物方面。

Claims (8)

1.一种用以延缓口服吸收之药物通过幽门的装置,此装置包括可在与胃液接触后因形成气体可膨胀之成分,其周围系以多孔的或微多孔的细胞膜完全或部份包围。
2.一种用以延缓口服吸收之药物通过幽门的装置,此装置包括可在与胃液接触后因形成气体可膨胀之成分,其中该装置的形状约为平面三角形或平面三叉放射体或四面体。
3.如权利要求1或2的装置,其中至少一部份可膨胀成分或至少一部份活性化合物或两者均为半固体药膏类形式。
4.如权利要求1或2的装置,其中至少一部份可膨胀成分及/或一部份活性化合物系存在于多重微粒制剂中。
5.如权利要求1或2的装置,其中至少一部份可膨胀成分或一部份活性化合物或两者均存在于泡沫类或泡沫制剂中。
6.如权利要求1或2的装置,其中至少一部份可膨胀成分或一部份活性化合物或两者均存在于可缓慢溶解的结晶形式。
7.如前述任一权利要求的装置,其系含括在能于胃液中迅速崩解的涂层内,例如硬明胶胶囊。
8.如前述任一权利要求之装置,其系由胃液可溶解的聚合涂层折叠并限于折叠形式。
