背景技术
放射疗法在三种主要形式的癌症疗法(外科手术、化学疗法和放射疗法)中保持着独特且既定的作用。外科手术将承受癌症的组织从身体除去,破坏它们。化学疗法消除整个身体的显微型疾病。只有放射疗法可以既破坏癌组织又同时消除显微型疾病。实验的消融癌治疗技术(例如,超声波、高温热疗(hyperthermia)和冷冻手术)仅可以像外科手术一样破坏组织,而新的化疗剂不能有效地破坏实体肿瘤。放射疗法将保持并扩大其作为癌症治疗和消融治疗上的选择疗法的显著作用。
放射疗法的临床目标是将优化的电离放射剂量配给精确地输送到肿瘤和靶点,同时使周围的正常组织免受剂量的伤害。在输送电离放射时,临床医生尝试在根除疾病的可能性和由于对周围健康或功能组织的照射而出现致命的或使人衰弱的副作用的可能性之间做出权衡。
不管是消融组织还是给组织灭菌,电离放射都通过破坏DNA或细胞中的其他重要分子中的化学键而杀死细胞。放射疗法通过以迅速分裂的癌细胞为标靶而起作用,其中放射引起破坏DNA或细胞中的其他重要分子的反应,在细胞分裂时造成细胞死亡。和正常细胞不同,癌细胞迅速分裂并且不能容易地修复它们自身,由于放射导致的遗传损伤,它们比健康细胞更容易死亡。随着时间延长治疗并且分段次输送剂量容许健康细胞恢复,同时优先消除癌细胞。有时,健康细胞的较少恢复可以被接受,如果可以利用立体定向方法获得更大的定位和固定精度的话。
用电离放射疗法治疗癌症或消融组织通过破坏癌细胞或靶细胞的DNA或其他关键分子而起作用。当电离的带电粒子造成构成DNA链或其他重要细胞分子的原子的直接或间接电离时,会导致该DNA破坏。当DNA或其他关键细胞分子的原子通过入射放射而直接产生时会发生直接电离。间接电离由于含水的细胞组分的电离而发生,含水的细胞组分的电离形成自由基,然后其破坏DNA或其他关键细胞分子。细胞具有修复单链DNA损伤的机制,因而双链DNA断裂是导致细胞死亡的最重要的机制。癌细胞一般未分化且类似于干细胞,这使得它们比大多数健康的分化细胞繁殖得更多,也使得它们修复亚致死损伤的能力减弱。因而单链DNA损伤通过细胞分裂而被传递并且对癌细胞DNA的破坏积累,使得它们死亡或更慢地繁殖。
放射敏感度是细胞、组织、器官或生命体对于电离放射的有害效应的相对敏感性。有四种主要的放射敏感度修饰剂,通常称为“4R”:再氧合、细胞周期的再分布、亚致命损伤的修复、和再增殖。
肿瘤含有低氧区域(低水溶氧浓度),其中癌细胞被认为是耐受放射的。在分次放射治疗中,这些区域通过各种机制被再氧合,这些包括瘤内压力减小和脉管系统的正常化。放射分次之间的再氧合导致之前的低氧肿瘤区域的放射敏感度并且被认为是增加了放射治疗的效率。
哺乳动物细胞在细胞周期全程中表现出不同水平的放射耐受性。总体上,放射对于具有更大再生活性的细胞具有更大的效果。在S期晚期中的细胞是特别耐受的,而且在G2期和M期中的细胞对于电离放射是最敏感的。在分次放射过程中,在G2M期中的细胞是优先被杀灭。两分次之间的时间容许来自细胞周期的S期的耐受细胞重新分布到细胞更加放射敏感的细胞期中。
通过电离放射的细胞杀灭基于不可修复的病变的生成,包括DNA双链断裂(DSB)或其它关键分子的损坏。然而大多数放射引起的DNA损坏是亚致命的。虽然这种损坏总体上在较低剂量得以修复,然而亚致命病变的积累在较高剂量下也促成致命性。在放射治疗中采用了在放射分次之间的亚致命损坏的修复,因为关键的正常组织和肿瘤在其对于修复放射损伤的能力方面经常是不同的。
正常的和恶性的干细胞具有进行不对称细胞分裂的能力,这产生子代干细胞和定向祖细胞。对比而言,干细胞按照对称细胞分裂分裂成为两个定向祖细胞或两个子代干细胞。如果仅仅在干细胞分裂的1%中发生了后者,那么在20次细胞倍增之后干细胞数量将会是定向祖细胞数量的两倍。如此,干细胞分裂方式中的小变化可能对于组织或肿瘤的结构具有重大影响,并且被认为是加速再增殖背后的机制。
快速分裂的肿瘤细胞和肿瘤干细胞通常(然而并非总是)比大多数体细胞更敏感。以上提及的4R可能对于肿瘤和健康细胞两者的放射敏感度都具有显著影响,肿瘤和健康细胞可能是例如低氧的并且因此对于通过电离氧产生的自由基来调节其大多数功效的X射线较不敏感。
大多数敏感细胞是未分化、营养良好、快速分裂、和高代谢活性的那些。在体细胞中,最敏感的是精原细胞和幼红细胞、表皮干细胞、和胃肠干细胞。最不敏感的是神经细胞和肌肉纤维。非常敏感的细胞还包括卵母细胞和淋巴细胞,然而它们是静息细胞并且因此不符合上述标准。
细胞的损坏可能是致命的(细胞死亡)或亚致命的(细胞可以自我修复)。对细胞的影响可以是确定性的和/或随机性的。
确定性影响具有照射阈值,在此阈值下确定性影响不会出现并且是照射的必然结果。确定性影响引起的损坏大体取决于剂量。这些影响在大约0.25到2Sv(希沃特(Sievert))之间的剂量范围中通常是亚致命的(例如,其产生较不明显的疾病形式),并且在大约2到5Sv之间的剂量范围中通常是致命的(例如,一定百分比的人群在60天内死亡)。高于大约5Sv的剂量导致大多数人在60天内死亡,而高于6到7Sv的那些剂量导致所有人死亡。当然,对于任何个人的具体影响还取决于其它因素,例如年龄、性别、健康等等。
随机的或无规的影响,其可以归类为体质的或基因的影响,是巧合的并且不可避免。这些影响也没有阈值。在体质影响中,继发癌是最重要的。继发癌通常由于放射引起DNA直接和间接突变而发展。直接影响是通过电离粒子和射线本身引起的那些,而间接影响是由自由基引起的那些,自由基特别是在水放射分解和氧放射分解中产生的自由基。基因影响将癌症的诱因遗传给下一代。
一种类型的癌细胞对于放射的响应通过其放射敏感度来描述。高放射敏感度的癌细胞被温和剂量的放射快速杀灭。这类癌细胞包括白血病、大多数淋巴瘤、和生殖细胞肿瘤。大多数上皮癌仅是中等放射敏感度,并且需要显著更高剂量的放射,例如大约60到70Gy(戈端(Gray))以实现根治。有些类型的癌是显著放射耐受的,即,需要比临床实践中可能安全的剂量高得多的剂量以产生根治。肾细胞癌和黑色素癌通常被认为是放射耐受的。
肿瘤对放射治疗的响应也可能与肿瘤大小相关。由于复杂的原因,非常大的肿瘤比小肿瘤或显微型疾病对于放射响应较小。各种策略可以用于克服这种影响。最常用的技术是在放射治疗之前的外科手术切除。这种方法最常见于乳腺癌的治疗中,其中采用广泛性局部切除或乳房切除及随后的辅助性放射治疗。另一种方法涉及在根治性放射治疗之前用新辅助化疗收缩肿瘤。第三种技术涉及通过在放射治疗过程中给用某些药物以增强癌的放射敏感度。放射增敏药物的实例包括但不限于顺铂(Cisplatin)、尼莫拉唑(Nimorazole)、西妥昔单抗(Cetuximab),等等。
放射治疗本身对于患者是无痛苦的。许多低剂量的缓和治疗(例如、对骨转移瘤的放射治疗)引起微量副作用或没有副作用,然而在治疗后的几天里可能由于在治疗区域中水肿压迫神经而经历短期疼痛发作。更高的剂量可能在治疗过程中(急性副作用)、在治疗之后的数月或数年中(长期副作用)、或者在再次治疗之后(累积副作用)引起不同的副作用。副作用的性质、严重程度、时期等等取决于接受放射的器官的放射敏感度、治疗本身(放射类型、剂量、分次、同步化疗)和患者。由于放射的副作用通常限于患者身体接受治疗的区域。
在放射治疗的现有技术中观察到的主要副作用是疲劳和皮肤刺激性。疲劳通常在治疗过程的中期开始并且可能在治疗完毕之后持续数周。受到刺激的皮肤将会愈合,但是可能不会如其之前的那样有弹性。还观察到许多急性副作用。
急性副作用在照射开始之后立即或很快产生。这些作用可能包括肿胀(也称为水肿或浮肿)、反胃和呕吐、上皮面损伤、口腔和喉部疼痛、肠道不适、不孕等等。迟发作用在治疗之后数月或数年发生并且一般限于被治疗的区域。它们通常由于血管和结缔组织细胞损伤而引起。迟发作用的严重程度可以通过将治疗分次为较小的部分而减小。在器官中的损伤和垂死细胞将会对电离放射发出信号并产生炎症反应,这是以上列举的许多急性作用的潜在原因。
作为由于细胞的放射损伤而引起的一般炎症的一部分,软组织肿胀可能在放射治疗期间引起问题。这种急性作用可能在脑肿瘤和脑转移瘤的治疗中是一个担忧,特别是存在已有的升高的颅内压或者肿瘤正在引起管腔(例如,气管或主要支气管)接近完全阻断的的情况下。可以在放射治疗之前考虑手术介入。如果手术被断定为不必要或不适宜,患者可以在放射治疗过程中服用类固醇以减小肿胀。
反胃和呕吐通常仅仅与胃部或腹部的治疗关联(通常在治疗之后反应几个小时),或者在某些头部和颈部肿瘤的治疗期间,与对头部中的某些产生反胃的结构(最常见的是内耳的前庭部分)的放射治疗关联。正如所有的痛苦治疗,有些患者在放射治疗期间立即呕吐,或者甚至在想到它就呕吐,但这被认为是心理反应。任何原因引起的呕吐都可以用止吐药进行治疗。
上皮表面可以经受来自放射治疗的损伤。取决于被治疗的区域,这可以包括皮肤、口腔粘膜、咽部、肠粘膜、尿管等等。损伤开始和从这种损伤中恢复的速率取决于上皮细胞的更新速率。通常,在进入治疗的几周中皮肤开始变红和疼痛。这种反应在治疗过程中可能会变得更加严重并且持续到放射治疗完毕后大约一周,并且皮肤可能溃坏。虽然这种湿性脱皮是不舒服的,但是通常恢复快。在皮肤天然皱褶的区域中,例如女性乳房下部,耳朵后部,和在腹股沟中,皮肤反应有变得更糟的倾向性。
如果治疗头部和颈部区域,在口部和喉部中通常可能发生暂时的疼痛和溃疡。如果严重,这些作用可能影响吞咽,并且患者可能需要止痛药和营养支持/食物补充品。如果对食道直接治疗的话,或者正如常常发生的,如果其在肺癌治疗期间接受到伴随放射的话,食道也可能变得疼痛。
可以直接用放射治疗下肠道(处理直肠癌或肛门癌)或者通过放射治疗其它的骨盆结构(前列腺、膀胱、女性产道)下肠道可能会被放射。典型特征可能包括疼痛、腹泻和反胃。
生殖腺(卵巢和睾丸)对于放射非常敏感。其可能在直接暴露于大多数正常治疗剂量的放射之后无法产生配子。如果生殖腺不是主要治疗区域,那么对于所有身体部位的治疗计划被设计为对生殖腺剂量最小化,如果不能完全排除的话。通过保留至少一个生殖腺免受放射可以有效避免不育。
长期而言,由于细胞死亡和组织的放射变性或损伤的形态变化将会作为迟发副作用而出现,例如纤维化、脱发、干燥、淋巴水肿、癌、心脏病、认知下降、放射直肠炎等等。
纤维化是指由于扩散性性的结疤过程,被照射的组织趋于随时间变得较没有弹性。脱发(掉头发)可能在使用高于1Gy的剂量时发生在任何带有毛发的皮肤上。其仅仅发生在(一个或多个)放射场范围内。在使用10Gy的单次剂量时掉头发可能是永久的,但是如果剂量是分次的,永久掉发可能不会发生直至剂量超过45Gy。
唾液腺和泪腺在2Gy分次中具有大约30Gy的放射耐受力,大多数头部和颈部癌症的根治都超出这个剂量。口腔干燥(口干症)和眼部干燥(干眼症)可能变成令人烦扰的长期问题并且严重降低患者的生活质量。类似的,在治疗的皮肤(例如腋窝)中的汗腺有停止工作的倾向性,并且天然湿润的阴道粘膜在骨盆照射之后经常是干燥的。
淋巴水肿,是一种局部液体滞留和组织肿胀的情况,可能由于淋巴系统在放射治疗期间经受的损伤导致。这是在手术后接受佐剂腋下放射治疗以清理腋淋巴结的乳腺放射治疗患者中最常见报告的并发症。
放射,当用于治疗癌症时,同时也是癌症的潜在诱因,而且继发恶性肿瘤也见于极少量的患者中—通常少于1/1000。由于放射治疗导致的癌症通常在治疗之后20到30年发起,然而有些血液恶性肿瘤可能在5到10年之内发展。在大多数情况中,处理原发癌带来的危险降低远比这种危险重要。由于放射治疗导致的新发癌症通常发生在患者的治疗区域内。
放射具有潜在的由于心脏病的过多死亡风险,见于有些过往乳腺癌RT方案之后。
在将放射施加给头部的情况中,放射治疗可能导致认知下降。认知下降在年龄在5到11岁的年轻的孩子中特别明显。研究发现,例如,5岁孩子的IQ在治疗之后每年下降数个IQ点。
放射性直肠炎可能涉及对于直肠的长期作用,包括一种或多种出血、腹泻和急症,并且通常与骨盆器官的放射治疗关联。当膀胱受到感染时骨盆放射治疗也可以引起放射性膀胱炎。
发明内容
一方面,一种方法包括将在放射治疗过程的至少一部分之后或期间进行的随后的水肿分析和基准水肿分析(其在所述随后水肿分析之前进行)进行比较,以估计患者组织中由于放射治疗过程引起的水肿变化,至少部分基于患者组织中相关于放射治疗过程期间施用的放射剂量的水肿变化得出水肿对于施用剂量的相关性,并且基于水肿对于施用剂量的相关性进行一个或多个临床动作。

具体实施方式
由于电离放射而发生在组织中的生理变化产生能够采用磁共振成像(MRI)检测到的变化,磁共振成像将产生的变化检测为解剖形态学和信号强度中的变化。在MRI的诊断技术的现有技术中,这些变化通常被认为是对进一步诊断和跟进的阻碍并且被描述为要在MRI诊断研究中避免误解。例如,参考“MRI appearance of radiation-induced changesof normal cervical tissues,”A,Lell M,Sweeney R,Bautz W,Lukas P.EurRadiol.2001;11(9):1807-17。

在光子放射治疗中使用的放射量以戈瑞(Gy)测量,并且根据治疗的癌症类型和阶段而变化。对于有疗效的情况,实体上皮肿瘤的典型剂量范围从60到80Gy,而淋巴瘤采用20到40Gy治疗。预防性(辅助)剂量通常在45到60Gy,每分次1.8到2Gy(例如对于乳腺、头部、和颈部癌症)。放射肿瘤医生在选用剂量时可能考虑其它因素,包括患者是否正在接受化疗、患者并存病、在手术前还是手术后实施放射治疗、和手术成功的程度。
处方剂量的施用参数在治疗中确定(例如作为计量学分析或其它过程的一部分)。治疗计划通常在专用计算机上使用专门的治疗计划软件进行。根据放射施用方法,可以使用多个角度或放射源以合计达到总需要量。治疗计划人员通常寻求设计出将均匀的处方剂量施用到肿瘤上并且将最小化剂量施用到周围健康组织上的计划。控制或治愈疾病的可能性以及副作用的可能性通过评价已经通过临床经验和临床实验建立的剂量和剂量-体积标准而确定。
对放射治疗过程施用的总剂量可以在单次剂量中施用或分次施用(在一段时间分散施用)。分次施用容许正常细胞有时间恢复,而肿瘤细胞通常在分次之间修复效率较低。分次施用还容许使在一个治疗分次期间处于细胞周期的相对放射耐受期中的肿瘤细胞在下一个治疗分次施用之前循环进入周期的敏感期。类似的,慢性或急性低氧(因而更加放射耐受)的肿瘤细胞可以在分次之间再次氧合,提高肿瘤细胞杀灭。分次施用方案在不同的放射治疗中心之间和甚至在各医生之间是个性化的。成人的典型分次施用日程安排是每天1.8到2Gy,一周五天。在有些癌症类型中,分次施用日程安排延期过长时间可能让肿瘤开始再增殖,并且对于这些肿瘤类型,包括头颈部和宫颈鳞状细胞癌,放射治疗优选在特定量的时间内完成。对于孩子,典型的分次剂量可以是每天大约1.5到1.8Gy,因为更小的分次剂量关系到正常组织中迟发效应的减小的发生率和严重性。
在有些情况中,在治疗过程接近完毕时每天使用两分次。这种日程安排,称为同时追加剂量方案或亚分次施用,被用于当其较小时较快再生的肿瘤上。特别是,在头颈部中的肿瘤表现出这种行为。
近期,亚分次施用变得更常见。这是其中放射总剂量被分成多个大剂量的放射治疗,并且治疗低于每天一次给出。常用剂量由于癌症类型而显著不同,从每分次大约3Gy到每分次大约20Gy。亚分次施用法通常试图通过不给细胞足够的时间再生而减小癌症复发的可能性。对于单次剂量施用或亚分次施用,通常采用立体定位方法在限定患者位置和固定患者时通常采取额外的护理。
对于短程疗法,植入体可以连续地分次施用,持续数分钟或数小时,或者其可以是永久的种子,其缓慢地连续施予放射直到其变为无活性。
磁共振成像可以用于评估人体组织和肿瘤中的炎症和其它响应以提供比物理剂量分布更好的对细胞损伤和组织响应的测量。在肿瘤或器官微环境中可能存在的放射敏感度的差异,或者患者个体之间基因方面的放射敏感度的差异,将采用这种方法给出解释。
许多类型的MRI扫描可以评估电离放射引起的炎症反应和形态学变化。MRI扫描的实例包括基础MRI扫描(例如T1加权的MRI、T2加权的MRI、T*2加权的MRI、自旋密度加权的MRI等等)和专用MRI扫描(例如扩散MRI、磁化传递MRI、T1ρMRI、液体衰减反转恢复、磁共振血管造影、磁共振的封闭颅内CSF动力学法;磁共振光谱法、磁共振光谱成像、功能性MRI等等)。
T1加权的扫描是指一组标准扫描,其描绘了身体内各种组织的自旋-晶格(或T1)弛豫时间。T1加权的图像可以使用自旋回波或梯度回波序列获取。T1加权的对比度可以通过应用反转恢复RF脉冲而增强。由于其能够使用脉冲间的短重复时间(TR),基于梯度回波的T1加权序列可以非常快速地获得。T1加权的序列通常在缩短的T1的MRI造影剂注入之前和之后采集。在脑部中,T1加权的扫描提供灰质和白质之间的可感知的对比度。在身体中,T1加权的扫描很适用于区分脂肪和水,其中水显较暗而脂肪显较亮。
T2加权的扫描是指一组标准扫描,其描绘了身体内各种组织的自旋-自旋(或T2)弛豫时间。如同T1加权的扫描,脂肪与水区分被区分。然而在T2加权的扫描中,脂肪显较暗,而水显较亮。例如,在大脑和脊髓研究的情况中,在T2加权的图像中CSF(脑脊髓液体)将会更亮。因此这些扫描特别良好地适合于对水肿成像,具有长的回波时间(TE)和长TR。由于自旋回波序列对磁场中的不均匀性较不敏感,因此这些图像长期以来都是临床重点。
T*2(读作“读作T2星”)加权的扫描使用梯度回波(GRE)序列,具有长TE和长TR。使用的GRE序列不具有自旋回波中使用的额外的重聚脉冲,因此其受到高于正常T2衰退(表示为T2’)的附加的损失。这些附加的损失有使T*2更易于在空气-组织边界上损失磁化率的倾向性,但是能够增加某些类型的组织对比度,例如静脉血。
自旋密度,也称为质子密度,加权的扫描通常预期不具有来自T2或T1衰退的对比度,其中仅有的信号变化来自于可用自旋量(水中的氢原子核)方面的差别。这种方法使用自旋回波或有时使用梯度回波序列,具有短TE和长TR。
扩散MRI,一种专用的MRI扫描类型,其测量水分子在生物组织中的扩散。临床上,扩散MRI用于诊断病情(例如,中风)或神经失调(例如,多发性硬化症),并且有助于改进对于中枢神经系统中白质轴突的连接性的理解。在各向同性的介质中(例如在一杯水内部),水分子按照湍流和布朗运动而自然随机运动。然而,在生物组织中,雷诺数足够低以使流动为层流,扩散可能是各向异性的。例如在神经元的轴突中的分子具有穿越髓磷脂膜的低可能性。因此分子主要沿着神经纤维轴运动。如果已知在特定体元中的分子主要在一个方向上扩散,即可假定在这个区域中的大多数纤维平行于此方向排布。
扩散张量成像(DTI)的近期发展使扩散能够在多个方向上被测量并且可以计算每个体元的在每个方向上的各向异性分数。此发展使研究人员能够制作纤维方向的脑部图以检查大脑中不同区域的连接性(使用示踪成像术)或检查如多发性硬化症的神经变性和髓鞘脱失的疾病区域。
扩散MRI的另一种应用是扩散加权成像(DWI)。在缺血性中风发生之后,DWI对于病变中发生的变化特别高度敏感。据推测,由于细胞毒性水肿(细胞肿胀)的结果,水扩散的限制(障碍)增加可能是DWI扫描上的信号增强的原因。DWI增强在中风症状发作的5-10分钟内显现(与计算机体层摄影技术相比,其通常用时长达4-6个小时也不能检测急性梗死的变化),并且保持长达两周。结合脑灌注的成像,研究人员可以高亮出“灌注/扩散错配”区域,其可以指示能够通过再灌注治疗抢救的区域。
如同许多其它专门应用,这种技术通常结合快速图像采集序列,例如回波平面成像序列。
磁化传递(MT)是指在NMR和MRI中从游离水质子到水合水质子在NMR和MRI中纵向磁化的转移。在分子溶液的磁共振成像中,例如蛋白质溶液中,发现了两种类型的水分子,游离(体相)和水合(结合)的水分子。游离的水质子具有较快的平均旋转频率,并且因此具有较少的可导致局域不均匀性的固定水分子,固定水分子可导致局域不均匀性。由于这种均匀性,大多数游离水质子具有位于63MHz的标准质子共振频率(以1.5特斯拉)周围的窄段共振频率。这还产生更慢的横向磁化移相以及因而更长的T2。相反地,水合水分子由于与溶质分子相互作用而慢速被缓慢,并且因此产生导致更宽共振频率频谱的场不均匀性。
在游离液体中,质子,其可以经典地视为小磁偶极子,表现出平移的和旋转的运动。这些运动的偶极子扰动周围的磁场,然而在足够长的时间标度上(可以是几纳秒),由质子的运动引起的平均场是零。这种作用已知为“运动平均”或退缩(narrowing),并且是质子在液体相中自由移动的特性。另一方面,结合到大分子(例如蛋白质)上的质子,常常具有固定取向,所以紧密邻近这种结构的平均磁场没有平均为零。结果是磁场中的空间模式,其对于经受磁场的质子产生残留偶极耦合(进动频率的范围)。宽频率分布显示为可能是几kHz宽的宽频谱。由于自旋相干性的损失,即T2弛豫,来自这些质子的净信号很快消失,与宽度成反比。由于交换机制,例如自旋转移或质子化学交换,结合到大分子上的(不相干)自旋与体相介质中的(相干)自旋连续交换位置并且建立动态平衡。
虽然通常没有来自结合自旋,或交换到体相介质中的结合自旋的可测量信号,然而它们的纵向磁化得到保存并且通常可以仅仅通过T1弛豫的相对缓慢的过程而恢复。如果只有结合自旋的纵向磁化可以改变,那么由于交换过程可以在体相介质的自旋中测量这种效应。磁化转移序列在下述频率应用射频(RF)饱和:该频率远离体相水的窄线共振然而仍然对于具有kHz级别上的频谱线宽的结合质子发生共振。RF应用引起交换进入体相水中的结合自旋的饱和,导致纵向磁化的损耗并因此导致体相水中的信号下降,由此提供组织中大分子含量的间接测量。磁化转移的实施涉及选择合适的频率偏移和脉冲形状以便足够强烈地饱和结合自旋,在RF照射的特定吸收率的安全限值内。
T1ρMRI通常依靠下列事实:分子具有作为温度的函数并且表达为平移和旋转运动的动能,并且通过分子之间的碰撞表达。运动的偶极子扰动磁场然而运动的偶极子通常极为快速以致于长时间标度上的平均效应可能为零。然而根据时间标度,偶极子之间的相互作用并不是总是平均化而消散。在最慢极值处,相互作用时间实际上是无限的并且发生在存在大静态场扰动的地方(例如,金属植入体)。在这种情况中,相关性的损耗描述为“静态移相”。T*2是在包括所有相互作用(包括静态移相)的自旋系综中相关性损耗的测量。T2是排除静态移相的相关性损耗的测量,其中使用RF脉冲以反转最慢类型的偶极子相互作用。实际上在给定的生物样品中存在相互作用时间标度的连续体并且再聚焦RF脉冲的性质可以调变为再聚焦不仅限于静态移相。通常,自旋系综的衰减速率是相互作用时间的函数也是RF脉冲功率的函数。在RF影响下发生的旋转坐标系中自旋-晶格弛豫时间的测量称为T1ρ。其类似于T2衰减,但是其偶极相互作用再聚焦较慢,静态相互作用也较慢,因此T1ρ≥T2。
液体衰减反转恢复(FLAIR)是用于使来自液体的信号在零位的反转-恢复脉冲序列。例如,其可以用于脑部成像中以抑制脑脊髓液体(CSF)从而显出室周的超强病变,例如多发性硬化症的斑块。通过仔细地选择反转时间TI(在反转和激发脉冲之间的时间),来自任何特殊组织的信号可以被抑制。

磁共振的封闭颅内脑脊液(CSF)或液体动力学(MR-GILD)技术是一种基于双极性梯度脉冲的MR序列,用于演示心室的CSF脉动流、池囊、西尔维厄斯(Sylvius)导管和整个颅内CSF通路。这是分析带有CSF阻塞病变(例如常压性脑积水)的患者中CSF循环系统动态的方法。这还允许血管中动脉和静脉脉动血流的可视化而不使用造影剂。
磁共振波谱法(MRS)可以用于测量身体组织中不同代谢物的水平。MR信号产生对应于被激发的同位素的不同分子排布的共振的波谱。这种信号可以用于诊断某些代谢紊乱,特别是影响大脑的那些,并可用于提供关于肿瘤代谢的信息。磁共振波谱法成像(MRSI)结合波谱学和成像方法以产生来自样品或患者内部的空间局部化波谱。空间分辨率显著更低(受到可利用SNR的限制),但是每个体元中的波谱包括关于许多代谢物的信息。因为可利用的信号用于编码空间和波谱信息,所以MRSI要求只在较高场强(3T及以上)下才可获得的高SNR。
功能性MRI(fMRI)测量由于变化的神经活动的脑部中的信号变化。以低分辨率然而快速地扫描大脑(典型地,每2-3秒一次)。神经活动的增加经由T*2变化引起MR信号中的变化。这种机制称为血液-氧-水平依赖(BOLD)作用。增加的神经活动引起对于氧的增加需求,而血管系统实际上对此过度补偿,相对于脱氧血红蛋白的含量增加了氧合血红蛋白。由于脱氧血红蛋白衰减MR信号,因此血管响应导致有关神经活动的信号增加。神经活动和BOLD信号之间的关系的准确性质是当前研究的主题。BOLD作用也容许产生神经组织中的静脉血管的高分辨率3D图。
虽然BOLD信号是人类受测体的神经科学研究中最常用的方法,MR成像的灵活性质提供了感知血液供应的其它方面的信号的手段。替代的技术采用了动脉自旋标记(ASL)或通过脑血流量(CBF)和脑血容量(CBV)对MRI信号加权。CBV方法要求注射目前在人类临床试用中的某类的MRI造影剂。由于这种方法在临床前研究中已经显示为比BOLD技术更加灵敏,这可能潜在地扩大fMRI在临床应用中的作用。CBF方法提供了比BOLD信号更加定量的信息,然而却有检测灵敏度的显著损失。
人体组织对于电离放射的急性和迟发响应可以在治疗中采用MRI技术进行成像以评价组织和肿瘤对于施用的电离放射的真实放射灵敏度修正的响应。例如,上皮表面可能经受来自放射治疗的损伤而内表面可能增厚,其可通过T1、T2、T*2、或自旋密度成像检测到。在其它实例中,成像可以检测出由于扩散性结疤过程而随时间趋于变得弹性更差的经照射的组织。可以对纤维化响应以及淋巴水肿成像,淋巴水肿是一种局部液体滞留和组织肿胀的情况,其由于淋巴系统在放射治疗期间经受的损伤导致。淋巴水肿是在手术后接收佐剂腋下放射治疗以清理腋淋巴结的乳腺放射治疗患者中最常报告的并发症。
水肿是由于细胞的放射损伤而发生的一般炎症的一部分,如下进一步说明,水肿可以直接采用与本发明主题的实施方案一致的T1和T2加权的MRI进行测量,本发明主题的实施方式包括有关于经由表达为水肿的炎症响应来示踪放射损伤的技术、方法、系统、设备、物品等等。炎症响应的定量测量可以通过测量由于采用MRI放射,治疗患者组织内部急性炎症引起的肿胀而产生。
炎症是血管组织对于有害的刺激(例如电离放射)的复杂生物学响应的一部分。其它刺激例如癌肿瘤入侵、病原体、损伤的细胞或刺激物也可以引起炎症,但是基准测量和评估可以与电离放射引起的炎症分离。炎症是器官消除有害刺激、死亡细胞或物质以及启动愈合过程的保护性努力。炎症被认为是对于引起它的刺激的天然免疫机制或敏感性。这也是对于伤口和感染的愈合的重要机制。然而,慢性炎症也可能导致疾病的宿主,所述疾病例如枯草热、牙周炎、动脉粥样硬化、风湿性关节炎、和甚至癌症(例如胆囊癌)。因此,炎症由身体密切地控制。
炎症可以分类为急性或慢性。急性炎症是身体对于有害刺激的最初响应,并且通过血浆和白细胞(特别是粒细胞)从血液进入受伤组织的增加的移动而实现。生物化学事件的级联使炎症响应传播和熟化,涉及局部血管系统、免疫系统和受伤组织内部的各种细胞。持续时间久的炎症,称为慢性炎症,导致在炎症的部位存在细胞类型的渐进性转换并且特征为由于炎症过程所致的组织的同时破坏和愈合。
急性炎症的典型迹象和症状是发红、肿胀、发热、疼痛、和组织功能缺失。可以在具体病例中观察到任何迹象,但是,当然,没有单个迹象是必需出现的。通常,炎症是视觉观察的并且由医疗从业者的外部检查而定性。当急性炎症发生在身体表面上时典型迹象出现,然而内部器官的急性炎症可能不会产生全部迹象。疼痛通常仅仅产生于适当的感觉神经末梢存在于炎症区域中的位置。例如,肺部的急性炎症(肺炎)不会产生疼痛,除非炎症涉及胸膜壁层,这里确实有疼痛敏感的神经末梢。

除了细胞衍生的介质以外,由预成型血浆蛋白质组成的若干个非细胞性生物化学级联系统也平行作用以引发和传播炎症响应。这些包括由细菌激发的补体系统,以及由坏死(例如烧伤或创伤)激发的凝血和纤溶系统。
急性炎症响应要求维持持续的刺激。炎症介质具有短的半衰期并且在组织中快速降解。因此一旦刺激消除急性炎症就通常停止和消减。
渗出性组分涉及血浆液体(包括重要蛋白质例如纤维蛋白和免疫球蛋白(抗体))进入炎症组织的移动。这种移动通过化学引发的膨胀和血管的升高的渗透性而获得,其产生血液血浆的净损失。液体进入组织的这种升高的聚集使其肿胀(水肿)。这种渗出的液体由淋巴管导入局部淋巴结,带着大量细菌以启动适应性免疫系统的识别和攻击阶段。
急性炎症的特征是显著的血管变化,包括但不一定限于血管舒张、增加的渗透性、和升高的血流,这些由各种炎症介质的作用引起。血管舒张首先发生在微动脉的水平,然后进行到毛细血管水平,并且可能带来血液存在量的净增加,这因而能够引起炎症的发红和发热。血管的升高渗透性导致血浆移动进入组织,产生由于血液中细胞浓度增加而引起的停滞。填满细胞的扩张血管通常是这种病情的特征。停滞容许白血球沿着内皮边缘聚集(移动),这个过程对于它们重新补充到组织中很重要。正常流动的血液防止了这种情况,因为沿着血管周围的剪切力使血液中的细胞移动到血管的中间。
炎症精细调节肿瘤周围的微环境,这可以有助于增殖、幸存和转移。癌细胞使用选择素、趋化因子及其入侵、转移、远端转移等等。另一方面,免疫系统的许多细胞有助于癌症免疫,抑制癌症。类固醇激素受体之间的分子贯通,其对于细胞发展有重要影响,在炎症中有重要作用的转录因子,例如NF-KB,可以调节炎症刺激对于癌症细胞的有些最重要的作用。炎症的这种影响类固醇激素在细胞中的作用的调停能力很有可能影响有些实例中的致癌作用。另一方面,由于许多类固醇激素受体的模块化性质,这种相互作用可以提供干扰癌症进程的方法,例如通过靶向具体细胞类型中的具体蛋白质区域。这种方法可以限制与相关肿瘤没有关系的副作用,并且可以帮助保持器官的活跃原状稳态功能和发展过程。
通过发生了伤害的组织以及引起这种伤害的有害物质将会确定具体情况中的结果。炎症的可能结果可以包括解除、纤维化、脓肿形成、慢性炎症、肿胀等等。
解除是炎症组织完全恢复回到正常状态。炎症措施例如血管舒张、化学产物、和白细胞浸润停止、以及损坏的实质细胞再生。在有限或短期炎症发生的情况中,这是通常的结果。
大量组织破坏、或不可再生的组织损坏可能不能完全被身体再生。纤维化是指发生在这些损伤区域中形成主要由胶原组成的疤痕的纤维状疤痕。疤痕不会包括任何特殊结构,例如实质细胞。相应地,可能发生功能性伤害。
脓肿形成包括形成包括脓的空腔,脓是包括死亡白色血液细胞和细菌的不透明液体,带有来自破坏细胞的主要残余物。
如果引起急性炎症的有害物质持续,慢性炎症就来了。这种过程,特征为炎症持续多日、月或年,可能导致形成慢性伤。慢性炎症的特征是在受伤组织中主导性地存在巨噬分子。这些细胞是身体的有力保护性物质,但是它们释放出的毒素(包括反应性氧物种)对于器官自己的组织以及入侵物质都是有害的。因此,慢性炎症几乎总是伴有组织破坏。
在医疗用语中,肿胀、浮肿、或肿大是身体部位或区域的短暂反常变大且不是由于细胞增殖引起。这是由组织中的液体累积引起的。其可以发生在整个身体内(总体的),或发生在具体部位或可能受到影响的器官中(局部的)。肿胀被认为是炎症的五种特征之一,还伴有疼痛、发热、发红、和功能缺失。身体部位可能响应于损伤、感染、或疾病而肿胀。如果身体没有良好地循环液体,那么肿胀可能发生。
总体的肿胀,或大面积水肿(也称为普遍性水肿)是严重患病者的普遍迹象。虽然轻微的水肿对于没有培训过的观察者可能难以检查,特别是对于超重的人,然而大面积水肿一般是非常明显的。edema(美国英语)或oedema(英国英语),早期已知为积水(dropsy)或积液(hydropsy),是一种反常的皮下液体累积或者在一个或多个体腔内液体积累而产生肿胀。总体上来说,间质液体的量通过液体原状稳态平衡确定,并且增加的液体分泌进入间质或者这种液体的不良消除可能引起水肿。
皮肤水肿称为“凹陷”,当压力施加到小区域上以后,解除压力之后凹痕持续一段时间。外周凹陷水肿是更常见的类型,产生于水滞留并且可以由系统性疾病、有些女性的怀孕、直接由于心力衰竭或是其结果引起,或者由局部病情例如静脉曲张、血栓性静脉炎、昆虫叮咬、和皮炎引起。当凹痕不持续时观察到非凹陷的水肿。这关联于这些病情例如淋巴水肿、脂肪水肿和粘液水肿。
普遍发展到整个身体中的水肿的起因可能引起多个器官中和外周的水肿。例如严重的心力衰竭可能引起肺水肿、胸膜积液、腹水和外周水肿。
虽然对于肾病综合征的水肿广泛举用低血浆渗透压,大多数医生指出水肿可能发生在尿液中有显著蛋白质(蛋白尿)之前或者在血浆蛋白质水平下降之前。有利的是还有另外一种可用的解释。大多数形式的肾病综合征是由于肾小球中的毛细血管基底膜中的生物化学变化和结构变化,并且这些变化发生在身体的大多数其它组织的血管中,如果在更轻微程度上。因此如果所有其它血管也是更加可渗透的,那么导致尿液中蛋白质的渗透性增加能够解释水肿。
也是如上提及的病情,水肿通常发生在有些孕妇怀孕的晚期。这对于具有肺部问题或不良循环病史的那些女性是更常见的,如果关节炎已经存在于这类特殊女性中也会加重。由于过度反应的肿胀而引起的疼痛,已经具有关节炎问题的女性通常更需要寻求医疗帮助。在孕期发生的水肿通常出现在腿的下部,通常是小腿肚往下。
水肿可能发生在具体器官中,例如作为炎症、肌腱炎、或胰腺炎的一部分。某些器官通过组织特有机制发展水肿。例如脑水肿是大脑中的细胞外液体积累。其可以发生在有毒或反常代谢状态中和例如系统性红斑狼疮或高海拔位置缺氧的病情中。这引起困倦或意识丧失。肺水肿发生在当肺部血管中的压力由于阻塞而升高,通过肺静脉转移血液时。这通常是由于心脏左心室衰竭。这也可能发生在高海拔疾病或吸入有毒化学物质时。肺水肿产生急促的呼吸。当液体累积在胸膜腔中时胸膜积液可能发生,水肿也可能伴随青光眼、严重结膜炎或角膜炎或在手术后而出现在眼部的角膜中。这种水肿可能导致患者看见亮光周围的有色光圈。围绕眼周的水肿称为眶周水肿或肿眼泡。眶周组织是醒后短时间内最引起注意的肿胀,可能是由于处于水平位置中的液体由于重力作用而重新分布的结果。
皮肤水肿的常见表现随着蚊子叮咬、蜘蛛咬、蜜蜂蛰(疹块和红斑),以及皮肤接触某些植物例如毒葛(Poison Ivy)或西部毒橡树(Western Poison Oak)而观察到,其中的后者称为接触性皮炎。另一种皮肤水肿形式是粘液水肿,这是由于结缔组织的增加沉积而引起。在粘液水肿中(以及各种其它罕见的病情中)水肿是由组织在其细胞间隙中容纳水的升高趋势而引起。在粘液水肿中这是由于沉积在组织矩阵中的富含碳水化合物的亲水分子(可能大多数是透明质酸)增加引起。水肿更容易在老年人中在依赖区域中形成(在家中坐在椅子上或者在飞机上)而这还没有被良好地理解。雌激素改变身体重量部分是通过改变组织的含水量。由于组织亲水性的变化或者终端淋巴毛细血管的“灯芯”功能失效,水从组织矩阵到淋巴的转移受到损害,这其中有各种理解不全面的情况。
在淋巴水肿中,间质液体的反常脱除是由淋巴系统失效引起的。这可能是由于例如癌症产生的压力或变大的淋巴结,放射治疗引起的淋巴血管破坏,或者由感染引起的淋巴渗透(例如象皮病)导致的阻塞。最常见是由于不运动引起肌肉的泵送作用失效,最显著的是在例如多发性硬化症、或截瘫的病情中。淋巴液体回流也是依赖于已知为淋巴心结构的泵送作用。有人提出在使用阿司匹林类环加氧酶抑制剂例如布洛芬或吲哚美辛之后发生在有些人中的水肿可能是由于淋巴心作用的抑制。
可能有助于水肿形成的因素包括增加的静水压力、血管中降低的渗透压、升高的组织渗透压、升高的血管壁渗透性(例如炎症)、通过淋巴系统的液体清理阻塞、和组织本身的保水性质变化。升高的静水压力通常反应了肾的水和钠保持能力。
间质液体的产生受到斯塔林等式的力的调控,其可以表示为区室之间的液体净流动Jv,如下:

在血管中的静水压力趋于使水滤出进入组织。这导致血液血浆和组织之间的蛋白质浓度差。因此,血浆中更高水平的蛋白质的渗透压趋于使水从组织回到血管中。斯塔林等式表明液体渗漏速率由两种力之间的差值决定,并且也由血管壁对于水的渗透性决定,这决定了对于给定的力不平衡的流动速率。大多数的水渗漏发生在毛细血管中或毛细血管后的微静脉中,其具有容许水比蛋白质更自由地通过的半渗透膜壁。蛋白质据说是有反射性的并且反射系数由高于1的反射常数给出。如果血管壁细胞之间的空隙打开,那么水的渗透性首先增加,但是随着空隙大小增加,蛋白质的渗透性也增加且带有反射系数下降。
斯塔林等式中的变量值变化可能有助于水肿形成,通过增加血管中的静水压力、血管中的渗透压下降或血管壁渗透性增加。后者具有两种作用。其容许水更自由地流动和其通过容许蛋白质更容易离开血管而减小渗透压力差。
剂量-体积直方图(DVH)是在放射治疗计划中使用的概念。DVH是由MichaelGoitein引进的,他还引进了放射治疗概念例如“束眼观(beam’s-eye-view)”、“数码重建的射线照片”、计划和定位中的不确定/误差等,Verhey在1979年在Shipley等人的出版物,ADVH summarizes 3D dose distributions in a graphical 2D format中提出。在现代放射治疗中,基于CT或MR扫描的3D重建,3D剂量分布通常产生于计算机化的治疗计划系统中。在DVH分析中指出的“体积”可以是放射治疗的靶向物,靶向物附近的健康器官,或者是任意结构。
DVH可以按照两种方法中的任何一个而呈现:差分DVH和积累DVH。DVH通过首先确定直方图的剂量格大小而产生。剂量格可以是任意大小,例如,0到1Gy,1.001到2Gy,2.001到3Gy等等。在差分DVH中,条或柱高度表示接受剂量格给出的剂量的结构体积。各个剂量格的剂量沿着横轴,而结构体积(百分比或者是绝对体积)在纵轴上。
差分DVH采用典型直方图的外观。积累DVH也是沿着横轴按照剂量格的剂量绘制。然而,第一格的柱高度(例如0到1Gy)代表接受大于或等于这个剂量的结构体积。第二格的柱高度(例如1.001到2Gy)代表接受大于或等于这个剂量的结构体积,等等。采用非常细微(小)的剂量格大小,积累DVH采用光滑曲线图的外观。这条线总是倾斜并且从左上端开始到右下端。对于接受非常均匀剂量的结构,例如100%的体积接受正好10Gy,积累DVH将会表现为在图表上端的水平线,以100%体积垂直绘制,在横轴上有10Gy的垂直下降。
积累DVH广泛采用并且优先于差分DVH。DVH在放射肿瘤学医疗专业中广泛采用。临床使用的DVH通常包括放射治疗计划中的所有相关结构和靶向物,其中每条线绘制成不同颜色代表不同结构。也是,纵轴几乎总是按照体积百分比(而不是绝对体积)绘制。临床研究通常采用DVH指标以关联于患者的毒性和结果。
DVH方法的缺点是其没有提供空间信息。换句话说,DVH没有显示出结构中的什么位置接受剂量。还有,来自最初放射治疗计划的DVH代表在放射治疗开始时剂量相对于结构的关系。随着治疗进展和时间流逝,如果有变化(即,如果患者减重、如果肿瘤收缩、如果器官变形等等),最初的DVH丧失准确性,例如由于呈现的数据中所固有的一个或多个计算的分母发生变化。剂量分布的空间属性可以通过叠放剂量分布图而滚动正交图像而呈现。
目前的放射肿瘤治疗实践关联控制或治愈肿瘤的可能性以及在健康或功能器官中引入致命或衰弱副作用的可能性,采用由剂量体积直方图得出的数据,以及由放射治疗治疗计划计算出的点、平面、或体积或剂量分布图。
电离放射剂量是施用给患者或物体的每单位物质的电离放射能量。这预期是由放射对于DNA或癌症细胞或靶细胞的其它关键分子,以及不可避免受到放射的健康或功能细胞引起的损害量的定量测量。然而,放射生物学的4R已知在不同个体之间是不同的,由于基因、形态学、和病理学上的原因,这些原因能够显著影响由放射引起的对于DNA或癌症细胞或靶细胞的其它关键分子,以及不可避免受到放射的健康或功能细胞的实际损害。
当细胞被电离放射照射和损害或杀灭时,这种刺激在照射的组织中引起炎症响应。由于这种响应受到细胞和组织实际损害的调节,这比施用的物理电离放射剂量是更直接的实际损害和细胞死亡的测量。
炎症响应可以作为在放射施用之后或期间产生的急性水肿而测量。在应用电离放射之前和之后组织的MR成像信号变化是液体增加的直接定量测量,见到相同患者的相同解剖体中T1加权的MR扫描中的减少和相应的T2(或T*2)加权的MR扫描中的增加。这种T1中的减少、T2中的增加、或两者是炎症的直接定量测量。在测量中也应该考虑到从施用刺激开始的时间和扫描长度以容许引起的水肿建立或消散。信号强度因此可以表示为炎症分布以替代施用的细胞损害评价的剂量分布。类似的,可以产生炎症体积直方图并且关联于患者结果和毒性。这种工具提供了肿瘤控制或治愈可能性,以及正常或健康组织毒性作为炎症的可能性的更好预计,按照引起的水肿测量,直接响应于电离放射刺激。
在本发明主题的进一步实施方案中,观察预期接受来自电离放射损害的区域以外的炎症可以用作安全特性以提醒临床使用者关于电离放射的意外施用或偶发性施用。
炎症响应改变接受放射损害的组织液体含量。这种液体增加改变光束通路中的物质从而使有效原子数不同并且吸收不同类型放射位置也可能改变。例如液体含量的增加可以在光束通路中产生更少的“脂肪状”物质(例如CH2)和更多的“水状”物质。在质子治疗或重离子治疗中,施用的光束成分的阻止力由此可以在局部水平上改变,由此改变布拉格分子的范围,并且因此改变施用的剂量分布。类似的,氢含量的增加可以改变来自中子光束的剂量。因此,采用MRI扫描的水肿定量分析可以用于改进计算质子、重离子、和中子治疗剂量的能力。
本发明主题的实施可以采用能够捕获至少患者局部的MRI图像的系统或其它设备而实现,其中采用施用一个或多个放射光束的至少某些并发度。换句话说,一个或多个MRI图像可以在施用分次的放射治疗剂量期间捕获。这种技术也可以称为“分次内”MRI。在本发明主题的其它实施方案中,MRI可以周期使用,可选地但不必须在放射分次内,以收集在治疗期间表征患者水肿变化的差分数据。例如,可以对治疗的区域进行基准扫描,然后在整个治疗过程中可以收集一个或多个另外的扫描以对治疗组织和治疗组织周围或受到一个或多个放射光束的另外影响的那些组织中的水肿变化进行定量。一个或多个另外扫描可选包括一个或多个在至少一个分次内进行的扫描,在一系列分次中的一个或多个分次之间进行的扫描,在某些其它间隔(可以是固定的或可变的)中进行的扫描,等等。
能够进行波段内MRI成像的系统的非限制性实例描述于共有的美国专利号7,907,987中,其公开内容在此全部引入作为参考。图1到图5分别显示了视图100、200、300、400、500,说明了可以包含在这种系统中的特征实例。MRI机器的主要磁铁亥姆霍兹线圈对115可以设计成为分开的螺线管,从而使患者扫描床130在磁铁和放射源120(例如线性加速器、放射同位素源等等,能够施用一个或多个放射同位素光束、质子光束、重离子光束、中子光束、X-射线等等。)中间的圆柱孔中运行,放射源能够向下瞄准两个螺线管部分115之间的空隙到达患者扫描床130上的患者135。分开的螺线管MRI磁铁115可以保持静止,放射源120,其可以包括多叶准直器强度调节放射治疗(IMRT)单元,围绕机架125上的扫描床轴向旋转。可以有利地采用多于一个放射源120。患者135放置在患者扫描床130上同时(或至少近似同步或至少近似同时地)进行成像和治疗。
如图5所示,带有多叶准直器的放射源120可以包括放射同位素源515(或其它放射源),其可选地能够采用固定的主准直器520、双重分散的二级多叶准直器525、和三级多叶准直器530进行瞄准,以阻断来自二级多叶准直器525的叶间漏过。应当理解能够在一个分次期间或者基本上与该分次同时的其它情况(例如在分次期间、之前、或之后的短时间内)产生MRI成像的其它系统也可以用于实施本发明主题。另外,在使用分次内MRI扫描的本发明主题的实施方案中,也可以使用没有包含同步放射施用的传统MRI系统。
图6和图7显示了近期拍摄的受试患者的相同部位的两个MRI扫描图像600、700。图6的扫描图像600显示了其中包括更多游离氢的区域(例如具有更高含水量的组织)比包括更多固定氢的区域(例如脂肪组织)表现为更暗的T1加权的扫描。图7的扫描图像700显示了其中包括更多固定氢的区域(例如脂肪组织)比包括更多游离氢的区域(例如具有更高含水量的组织)表现为更暗的T2加权的扫描。扫描图像600、700显示了通过受试患者腹部的截面并且显示了肝602、肾604、和脊柱606,以及其它特征。肝602包括高含量的脂肪组织,因此在显示T1加权扫描结果的扫描图600中比在显示T2加权扫描结果的扫描图700中更暗。类似的,脊柱在图6中比在图7中更亮,而肾具有更高含水量,在图6中比在图7中更暗。在图6和图7中还描绘了放射通道610,在其上将放射治疗施用给患者。如图6和7中所示,扫描图像600、700指示了沿着放射通道610的液体增加,其在图6的T1加权的扫描中在更亮的肝602中显示为暗线并且在图7的T2加权的扫描中在更暗的肝中显示为亮线。这种增加的液体是由受到放射治疗影响的组织中的细胞损坏引起水肿而产生的。
在本发明公开之前,观察这种水肿的传统方法是忽略或试图纠正这种“人为结果”,其通常被认为是对于分析需要放射治疗的潜在病理学的干扰。对比而言,本发明的主题采用了水肿定量,基于患者治疗区域的一个或多个MRI扫描以便至少估计放射治疗剂量对于靶向结构(例如肿瘤或其它疾病组织)和周围组织的作用。采用这种方式定量的水肿通常被认为是急性的、短暂的水肿,产生于光束通过的组织中的细胞死亡。
在实践中,本发明主题的实施方案可以包括T1加权的和T2加权的扫描比值定量,以确定、或至少估计随患者组织内部的空间位置变化的游离氢比值。估计的游离氢比值增加可以解释为患者组织中水肿增加的指示。按照这种方式,水肿的变化可以用作估计在患者的给定组织区域中细胞损坏或死亡量的代表。如上所述,放射引起的(以及其它)损伤的细胞损坏或破坏产生启动细胞(例如巨噬细胞、树突细胞、组织细胞、枯氏细胞、肥大细胞等等)释放产生临床炎症迹象的炎症调节物质,包括但不限于水肿(血浆蛋白质和液体渗出进入组织)。这种液体渗出可以被检测到,至少在差分的基础上,与本发明主题的实施方案一致,通过使用MRI扫描检测组织中游离氢含量的变化。可以使用一个或多个以上讨论的扫描来表征这种变化。在其它实例中,两个或多个扫描可以结合以产生患者组织中游离氢含量的比值计量代表作为组织的空间位置函数。
与本发明主题的一个或多个实施方案一致,如图8的流程图800中说明的方法可以包括下列特征。在802中,在放射治疗过程中的至少一部分期间或之后进行的随后水肿分析(例如随后的基于MRI的水肿分析)与在随后水肿分析之前进行的基准水肿分析进行比较,以估计由放射治疗过程产生的患者组织中的水肿变化。在本发明主题的有些实施方案中,随后的水肿分析和基准水肿分析可以是MRI扫描,其可以提供细胞中流体含量的估计结果。
如上所述,例如,基准和随后水肿分析可以各自包括一个或多个MRI扫描。在本发明主题的有些实施方案中,一个或多个MRI扫描可以包括T1加权的和T2加权的扫描,并且这些扫描结果的比值可以预备给每个基准和随后扫描。水肿变化可以基于随后扫描和基准扫描的微分分析而得出(例如,估计、计算、确定等等)以确定放射治疗过程期间发生的变化。在某些实例中,游离氢的含量可以在随后扫描中相对于基准扫描而定量,并且游离氢相对含量的变化可以用作患者组织中的水肿变化的代表。
基准扫描可以用作与一个或多个随后的MRI扫描进行比较的参考,所述MRI扫描在放射治疗过程中施用给患者的至少一个放射分次期间或之后拍摄。换句话说,基准扫描说明了现有水肿的存在,例如由于最初病症、其它创伤而产生的水肿,等等。在治疗过程期间拍摄至少一个随后MRI扫描(例如一个或多个分次内或分次间MRI扫描),并且在基准和随后扫描之间发生在患者组织中的变化可以定量。在有些实例中,这种定量可以通过差分成像法而完成以指出来自患者组织的MRI信号的强度变化。
单一类型的MRI扫描可以用于基准扫描和至少一个随后扫描,并且对于这种扫描的MRI响应的变化可以在基准扫描和随后扫描之间定量。替代地,如上所述,每个扫描可以包括两个或多个类型的扫描比值,例如T1加权和T2加权的扫描。由于水肿通常产生液体增加,正如正常细胞损坏或细胞死亡恢复机制包括:通过有更高含水量(因此更高游离氢含量)的液体将死亡或损坏的细胞物质带走。在有些变更方案中,基准扫描可以在任何治疗之前拍摄,然后用于与在施用给患者的放射剂量分次期间或之间拍摄的随后扫描比较。在其它变更方案中,可以使用一系列扫描,差分分析可以在一系列中的两个或多个扫描之间应用,可能但不一定使用在放射治疗过程开始之前收集的扫描作为基准扫描。
在804中,基于患者组织中的水肿变化,关联于放射治疗过程期间的放射施用剂量,得出水肿对于施用剂量的相关性。换句话说,可以应用物理施用剂量的一个或多个计算或模型以得出(例如估计、计算、确定等等)实际施用给患者组织的一个或多个放射量,施用给患者组织的预期放射量,等等。得出的实际施用或预期已经施用的放射量可选地可以基于一个或多个输入。在有些变更方案中,放射前治疗计划可以提供这些数据。在其它变更方案中,结合的MRI和放射施用方法(例如描述于共有美国专利号7,907,987)可以用于计算疾病组织和其它组织结构接受的放射剂量的更准确测量,基于患者、患者器官等等的分次内动态。
患者组织中的水肿变化与施用剂量(或预期施用剂量)的相关性可能包括定量:水肿变化如何对应于疾病组织和周围组织的预期结果,相对于预期值(例如组织对于施用放射量的预期响应)。预期值可以根据经验、实验、通过应用一个或多个理论模型等等、或者通过一个或多个这种方法的结合而计算。
在806中,一个或多个临床动作基于水肿对于施用剂量的相关性而进行。临床动作可以包括一个或多个各种工作。例如患者经历的水肿,特别是目标疾病组织以外的组织结构中的水肿,超过预期值阈值用量以上,可以停止治疗过程以便进一步分析,可以检查放射施用系统以确定任何功能异常的构件,临床医生可以受到使用者界面或某些其它自动方法的提醒,在下一个分次中施用的放射量可以减少等等。在其它实例中,如果患者经历的水肿,特别是目标疾病组织中的水肿,低于预期值阈值用量以上,可以同样停止治疗过程以便进一步分析,可以检查放射施用系统以确定任何功能异常的构件,临床医生可以受到使用者界面或某些其它自动方法的提醒,在下一个分次中施用的放射量可以增加(例如假定在目标疾病组织以外的组织结构中患者经历的水肿在某种可接受的范围以内)等等。
如上所述,在一段时间上差分分析的包括至少一个放射治疗施用剂量的MRI扫描数据可以用于得出至少关于扫描组织中发生的细胞死亡或损坏的估计量。这类数据可以按照各种方式表达,包括但不限于视觉描绘、数值表达、等等,代表强度作为患者组织中部位的函数。例如,在用作扫描组织中的游离氢含量代表的T1加权和T2加权的扫描比值的情况中,这两种扫描比值中的差分变化可以表示为部位的函数。这种表示可以包括在目标疾病组织附近在患者组织某部分上的2D图或其它图像上使用体元以视觉描绘发生在治疗过程上的细胞死亡或损坏。
增加的游离氢比值可以用作给定部位中损坏组织升高量的代表,其可以临床用作吸收的活性放射剂量的实际相对剂量计。按照这种方式,可以提供下列测量:处理影响已经在目标疾病组织上进行的放射治疗的程度,以及周围其它组织已经受到影响的严重程度,等等。相应地,本发明主题的实施方案可以有助于医疗管理正在进行放射治疗的患者。比较计量学计划的“预期”结果与实际观察到的水肿变化能够容许临床医生更好地估计具体患者如何响应于具体的放射治疗过程。
图9显示了具有与本发明主题的实施方案共同的一个或多个特征的系统900的示意图。计算机系统902可以与MRI系统904通信,可选与放射施用系统906通信。使用者界面可选地可以包括显示器、使用者输入设备等等,以及以下讨论的其它实例用于将信息传送给临床医生或其它使用者和/或用于接受信息输入。计算机系统902可选地可以是MRI系统904的一部分或者以其它方式结合到其上和/或结合到放射施用系统上。在图1到图5的实例中,计算机系统902、MRI系统904、放射施用系统906都可以结合。在有些实例中,放射施用系统906可以包括它自己的计算机系统,例如剂量计划系统运行的剂量计划软件。在系统900的各种构件之间的数据连通可以在任何数据传输连接点(网络、计算机总线)上完成。计算机系统902可以可选地包括可编程处理器,其执行实施一个或多个以上所述特征的一个或多个软件模块。

Claims (12)
1.一种利用对放射疗法的生理反应的成像对治疗评估的系统,所述系统包括:
MRI系统;
放射源;以及
存储指令的至少一个非暂态机器可读介质,所述指令在由可编程处理器执行时使可编程处理器执行操作,所述操作包括:
捕获基准MRI扫描并且在治疗过程中捕获随后的MRI扫描;
将利用所述基准MRI扫描的基准水肿分析与利用所述随后的MRI扫描的随后的水肿分析进行比较以估计患者组织中的急性和/或短暂的水肿的变化;以及
当估计的急性和/或短暂的水肿的变化超过预期值时提醒临床医生或停止治疗。
2.根据权利要求1的所述的系统,其中,所述随后的MRI扫描和所述基准MRI扫描包括下列中的至少一个:T1加权的MRI扫描、T2加权的MRI扫描、T1加权MRI与T2加权MRI的扫描结果比以及MRI响应比。
3.根据权利要求1所述的系统,其中,所述比较包括量化游离氢含量的变化。
4.根据权利要求1所述的系统,所述操作还包括在显示器处生成患者组织中细胞死亡的量的估计的视觉描绘。
5.根据权利要求4所述的系统,所述视觉描绘包括在2D图上使用体元。
6.根据权利要求4所述的系统,其中,所述视觉描绘包括T1加权的MRI扫描与T2加权的MRI扫描的比的差分变化并且该差分变化被表示为部位的函数。
7.一种非暂态机器可读介质,所述非暂态机器可读介质存储指令,所述指令在由可编程处理器执行时使可编程处理器执行操作,所述操作包括:
捕获基准MRI扫描并且在治疗过程中捕获随后的MRI扫描;
将利用所述基准MRI扫描的基准水肿分析与利用所述随后的MRI扫描的随后的水肿分析进行比较以估计患者组织中的急性和/或短暂的水肿的变化;以及
当估计的急性和/或短暂的水肿的变化超过预期值时提醒临床医生或停止治疗。
8.根据权利要求7的所述的非暂态机器可读介质,其中,所述随后的MRI扫描和所述基准MRI扫描包括下列中的至少一个:T1加权的MRI扫描、T2加权的MRI扫描、T1加权MRI与T2加权MRI的扫描结果比以及MRI响应比。
9.根据权利要求7所述的非暂态机器可读介质,其中,所述比较包括量化游离氢含量的变化。
10.根据权利要求7所述的非暂态机器可读介质,所述操作还包括在显示器处生成患者组织中细胞死亡的量的估计的视觉描绘。
11.根据权利要求10所述的非暂态机器可读介质,所述视觉描绘包括在2D图上使用体元。
12.根据权利要求10所述的非暂态机器可读介质,其中,所述视觉描绘包括T1加权的MRI扫描与T2加权的MRI扫描的比的差分变化并且该差分变化被表示为部位的函数。
