Abstract

Description
本发明涉及一种有效抗齿斑和龈炎的组合物,尤其涉及对齿斑和龈炎有效的天然芳香剂的组合物。
自从1860年含丁香和没药天然产物提取物的医用洗剂的美国专利30834被授予J.F.PoPP以来,在这一领域就不断有这方面的实践和资料的记载,因而在治疗牙齿和牙龈的医用洗剂中使用天然产物就已不新奇。从那时起,许多专利公开了含有天然产物提取物的口腔用产品。例如,颁发给C、E、Blake的美国专利69388是关于玫瑰、香柠檬或冬青油香型的牙粉。颁发给J.O.Draper的美国专利115719是有关含没药和菖蒲根粉酊剂的牙皂。然而,在本领域的现有技术文献中却没有一篇公开了对人类齿斑和龈炎有效的临床验证记录。
牙齿表面的菌斑是龈炎和牙周炎的一个主要病因,如下列文献所示:M.M.Ash等,J.Periodontal,35:424~429,1964;J.Periodontal,36:177~187,1965;和E.Theilade等,J.Periodont.Res.1:1~13,1966。最近,慢性牙周疾病牵涉到齿斑中特定的口腔微生物。一种缓和龈炎和牙周炎发展的方法是防止或减少齿斑的形成。也有证据表明抗菌剂作为辅剂对机械除菌斑是有益于的,而且最近的人体临床研究表明含有抗菌化学物质(商业已知的如三氯生Triclosan)的牙膏和漱口组合物能减少齿斑和龈炎。
本发明的一个目的是提供一种改良的口用组合物,如牙膏和口腔漱口剂。
本发明的另一个目的是提供一种含有天然芳香剂组分的改良性口用组合物。
对本领域熟练技术人员而言,进一步阅读了说明书之后,本发明的其它目的将会变得很清楚。
现在人们已经发现含有百里酚和丁子香酚或百里酚、丁子香酚和一种倍半萜烯醇组合物的感官上可接受的口用产品(如牙膏或漱口剂)对人的齿斑和龈炎有效。典型的倍半萜烯醇包括麝子油醇、苦橙油醇、红没药醇、檀香醇(Santolol)等。也可加入芳香剂来改善口味,例如白花母菊(Chammomile)酊、没药树胶酊、娜檀根、澳大利亚茶树油、桉树脑、鼠尾草油等,且当在口用产品中使用澳大利亚茶树油、桉树脑和鼠尾草油中的一种或多种时,提高了百里酚和丁子香酚及百里酚、丁子香酚和倍半萜烯醇的口用组合物的抗齿斑和龈炎的疗效。
百里酚、丁子香酚和任选的倍半萜烯醇的量的变化可以相当大。然而,每百万份(以口用组合物的总重量计)优选使用至少约5份的量(PPm)。例如在漱口剂中,尤其是如果不含澳大利亚茶树油时,特别适用的范围是约1000到约20000PPm的百里酚和约1000到约20000PPm的丁子香酚。在漱口剂中使用澳大利亚茶油时,另一个特别适用的范围是约5到约500PPm的百里酚和约5到约500PPm的丁子香酚。当漱口剂中使用澳大利亚茶树油时,它的用量为约50到5000PPm。
使用上述列举的任何其它芳香剂,例如桉树脑、鼠尾草及其它时,用量范围在约50到约10000PPm之间,优选约5000PPm。在不含澳大利亚茶树油组合物如牙膏中,特别适用的丁子香酚范围为约5000到约20000PPm。当组合物含有澳大利亚茶树油时,使用的百里酚和丁子香酚每种的典型用量为约500到约30000PPm。
在不含澳大利亚茶树油的漱口剂和牙膏中,所含的倍半萜烯醇的典型用量分别为约1000到约20000PPm和约5000到约20000PPm,在含有澳大利亚油的漱口剂和牙膏中,所含的信半萜烯醇的用量分别为约5到约500PPm和约500到约3000PPm。
通常,本发明的抗齿斑抗龈炎组合物可以混入口用剂型中。它们除用于漱口剂和牙膏外,还可用于锭剂,口香糖或含有牙科载体的牙科絮状物。
在牙粉中牙科载体分为固体类和液体类。一般,液体类载体包括水和/或致湿剂,例如甘油、山梨醇、聚乙二醇或丙二醇及其适宜的混合物。使用水和一种或多种致湿剂的混合物作载体通常是有益的。全部液体的含量通常占载体重量的约5~90%,在透明和半透明的载体中,牙膏的液体含量可以为5-90%左右(按重量计),但在不透明的载体中,全部液体量一般为约5~50%(按重量计)。优选的致湿剂是甘油、山梨醇和聚乙二醇。
载体中的固体部分是胶凝剂。在本发明中,该胶凝剂包括羧甲基纤维素、羟乙基纤维素和羟甲基纤维素金属盐,它们的用量至少占载体重量的0.5%。也可含有其它的胶凝剂。可另外存在的胶凝剂包括黄原胶、Viscarin、明胶、淀粉、葡萄糖、蔗糖、聚乙烯吡咯烷酮、聚乙烯醇、黄蓍胶、刺梧桐树胶、羟丙基纤维素、甲基纤维素、羧乙基纤维素(CCMC)和藻酸钠。Laponite CP或SP,它们各自都是合成的无机复合硅酸盐粘土,以Laporte Induslries,LTD.商标销售及硅酸镁铝凝胶。该载体的固体部分或胶凝剂的典型用量约为牙膏重量的0.5~5.0%,优选约为0.5~2.0(按重量计)。
任何适宜的几乎不溶于水的磨光剂可加到牙膏或牙粉的凝胶载体中。本领域的现有技术中已知许多这类材料。有代表性的材料包括,例如磷酸二钙、磷酸三钙、氢氧化铝、碳酸镁、碳酸钙。焦磷酸钙、硫酸钙、膨润土、氧化铝、水合氧化铝、硅酸铝、硅酸锌、硅石,及其混合物。
也可使用水溶性的磨光剂,例如碳酸氢钠。依据本发明,当有碳酸氢钠存在时,利于齿斑和龋齿的减少。
通常,这类磨光剂含有大部分重量比的固态组份。这类磨光剂的含量是可变的,虽然可使用较少量的磨光剂,例如20%或更多的碳酸氢钠,但通常该量多达组合物总重量的75%,优选约20~75%。
任何适宜的表面性物质都可以混入凝胶载体中。这类可相容的物质是为提供去垢和起泡性能所需,这些特性取决于所选择的特定类型的表面活性物质。这些去垢剂大部分是水溶性有机化合物,在结构上可以是阴离子型的、阳离子型的或非离子型的。优选使用水溶性非皂类或合成的有机去垢剂。已知的适宜的去垢物质是,包括水溶性的高级脂肪酸单甘油酯单硫酸阳离子盐去垢剂(如可可脂肪酸单甘油酯单磺酸钠)、高级烷基硫酸盐(如月桂基硫酸钠)、烷基芳基硫酸盐(如十二烷基苯磺酸钠)、1,2一二羟基丙烷的高级脂肪酸酯磺酸盐等。其它适宜的表面活性物质包括非离子型表面活性剂,例如环氧乙烷和环氧丙烷的缩合物和丙二醇的缩合物(Pluronics)。
可以任意适宜量使用各种表面活性物质,通常是占牙粉组合物重量的0.05到10%左右,优选0.5~左右5%。
本发明的口用产品也可含有最低量达约5%(按重量计)的抗齿斑活性辅助剂,例如,聚乙烯马来酸/马来酸酐共聚物或聚乙烯膦酸,它们沉积在牙齿表面,抑制齿斑附着在牙齿表面。百里酚、丁子香酚和倍半萜烯醇是以不同的机制工作,即,作为抗菌剂,它们抑制细菌的再生并由此抑制齿斑生长。因此,这两种不同的抗齿斑机制是补充性的,表现出叠加的甚至是协同的功效。
本发明的口用产品也可含常规组份,例如,着色剂或增白剂(如二氧化钛),芳香剂和/或甜味剂,氟化物,如氟化钠,氟化锡和单氟磷酸钠等。无论化合物中是否含氟,这类附加的每种组份都可加少量到牙粉中,最多到约5%总重量,优选最多到约1%,该量提供约100到10000PPm的氟,优选约500到约2000PPm的氟,只要它们不会影响成品的抗齿斑特性即可。口用产品也可以是液态的,例如漱口剂,它通常含有约60~99%(按重量计)的含水无毒低级脂肪醇,优选有约5~30%(按重量计)的无毒醇(如乙醇、正丙醇或异丙醇)和水,且通常有约5~35%的致润剂。
这类口用制剂,一般通过用牙粉或牙膏刷洗与天然或人造牙齿和牙龈接触来施用,或通过漱洗口腔15~90秒与牙齿和牙龈接触来施用,或以锭剂、糖果或口香糖的形式,通过在口腔中吮吸或咀嚼来施用或以口腔雾化剂的形式,通过至少每天一次喷洒口腔表面来施用。
下述实施例将进一步描述本发明。除非另有说明,所有的份数和百分数均按重量计。
实施例1。天然芳香剂剂对口腔齿斑细菌的抗菌活性。
根据N.Nabi等在Am.J.Dent.2:197:206,1989(这里引用为参考文献)中所描述的标准系列稀释方法,通过测定各个试剂的体外最低抑制浓浓(此后称为MIC)来评价了许多天然芳香剂的抗Actinomyces Viscosus(一种齿斑细菌)的抗菌活性。为本发明的目的定义的MIC是指观察到没有细菌生长时的一个特定试剂的浓度。所用在有机溶剂或水溶液中的天然芳香剂是从便利的商业来源得到的。使用抗菌化合物,Triclosan作为阳性对照。Triclosan是2,4,4′一三氯-2′-羟基二苯基醚。

实施例3,天然产物漱口剂的体外抗菌活性。
通过测定表2所示的漱口剂的体外最低抑制浓度(MIC)来评价天然产生漱口剂和安慰剂抗两种齿斑细菌,即A.viscosus和Streptococcus Sanquis的抗菌活性。表4中所描述的结果显示天然产物漱口剂(1)抑制这两种齿斑细菌(A.viscosus和S.sanquis)的生长,并且该抑制作用超过了作对比的安慰漱口剂。还采用短时间杀灭试验(SIKT)来评价该漱口剂的抗菌活性。在这一试验中,将0.1ml漱口剂与1.9ml A.viscosas(106个细胞/ml)混合并于37℃培养2分钟。加入冷的胰蛋蛋白酶大豆肉汤(tryptic soya broth),停止培养并根据胰蛋白酶大豆琼脂平板上细菌的生长来测定细菌的存活率。通过把用漱口剂处理过的菌落形成数与用对照处理过的菌落形成数相比较计算灭A.viscosus的百分数。表5中的结果显示用天然产物漱口剂处理过的100%的A.viscosus被杀灭,但经安慰漱口剂处理的竟没有细菌被杀灭。

实施例4,天然产物漱口剂的体外抗齿斑活性
按上述实施例1中所述方法评价天然产物漱口剂的体外抗菌活性。通过温和的措施和高压灭菌器清洁拔出的人牙。然后用漱口剂处理,再用A.viscosus和Streptococcus mutans处理以使牙斑生长于牙齿表面。把该步骤重复三天。分别用安慰漱口剂和无菌水作为安慰对照和阴性对照。表6中的结果表明天然产物漱口剂(1)与水对照相比,对牙齿表面的齿斑形成抑制达39.7%。在相似条件下,安慰漱口剂对齿斑形成的抑制仅为5.8%。

实施例5.在猴子的临床研究中天然产物漱口剂的抗龈炎和抗齿斑活性
在猴子的临床研究中以体内龈炎和齿斑的减少来评价天然产物漱口剂的功效。两组猴子(在每组中N=6)用天然产物漱口剂(1)和安慰剂处理6周。开始在3周和6周时间点上算出龈炎和齿斑指数。结果表明6周后与安慰剂组相比较,天然产物漱口剂使龈炎减少25%(表7)。在相似条件下,天然产物漱口剂使齿斑评分减少0.2%(表8)。

实施例6,对人的临床研究中天然产物漱口剂对齿斑减少的影响。
对人的临床研究中评价天然产物漱口剂对齿斑减少的效果。研究是随机盲目并3室(cells)交叉设计进行来测定每种漱口剂对由零基线开始4天的齿斑生长的影响。十五名志愿者用每种漱口剂每天漱洗两次,记录齿斑的面积并评分(Addy等J.Clin.Perio.10,P.89-99(1983),Quigley和Hein J.A.D.A.65,p.26~29(1962)。使用三种漱口剂,即,天然产物漱口剂(1)(试验漱口剂),0.2%洗泌太(阳性对照)和0.9%盐水(阴性对照)。表9和10分别显示了各个漱口剂的平均斑划痕和齿斑面积。结果显示,与0.9%盐水漱口剂相比,处理4天后,由齿斑划痕和面积测得的天然产物漱口剂(1)对齿斑再生的抑制分别为27.7%和59.2%。在另一方面,临床上证明对齿斑和龈炎有效的0.2%洗泌太漱口剂在相似条件下,由齿斑划痕和面积测得的对齿斑的再生抑制分别为48.0%和78.0%。

实施例7天然产物牙膏(1)的抗齿斑细菌的抗菌活性
通过测定体外MIC和SIKT来评价成品牙粉产品中的天然芳香剂/产物对抗A.viscosus(一种齿斑细菌)的抗菌活性。MIC和SIKT的结果都表明天然产物牙粉(1)对齿斑细菌具有强的抗菌活性,并且该抗菌活性可与Colgate-Palmolive Company的牙龈保护配方(GPF)牙粉(一种临床证明抗齿斑和龈炎,含Triclosan的牙粉)相比,还比对照的安慰剂要好且超过了它(表11)。这些结果还暗示天然芳香剂/产物的抗菌活性与牙粉组份是相容的。

实施例8改善了口味和功效的优选漱口剂和牙粉组合物
因为对一个具有临床效果的新产品而言,一个先决条件是患者在感官上对产品的全面接受,通过加入澳大利亚茶树油来进一步改善实际上合有较低量的抗齿斑和抗龈炎组分产品口味和功效来说,本发明漱口剂和牙粉的组合物是最理想的。也可以通过加入桉叶油素和/或鼠尾草油来改善口味和功效。表12和16分别描述了最佳的漱口剂和牙粉配方。采用如下实施例9和10所示的体外方法评价该产品的功效。

实施例9最佳天然产物漱口剂的体外抗菌活性
在体外通过测定MIC和SIKT来评估最佳天然产物漱口剂(2)的抗菌活性。表13中的结果表明,与两种商业上的漱口剂Actibrush(含Triclosan)和Listerine(含酚类芳香剂)相比,天然产物漱口剂(2)抗四种齿斑菌的MIC是最低的。这些结果暗示,与商业上的Actibrush和Listerine相比,天然产物漱口剂(2)的抗齿斑细菌活性最强。SIKT结果(表14)表明天然产物漱口剂(2)杀灭的89%范围内的齿斑和唾液细菌,该范围与相似条件下Actibrush的活性相当。

实施例10在流动细胞体系中最佳天然产物漱口剂(2)对体外菌斑的功效。
接A.Graffar等在Am.J.Demt.,3,P.S7-S14,1990中所述方法,用Chemostat齿斑模型来评估天然产物漱口剂(2)对体外细菌齿斑形成的抑制效果。这一体外模型在生物物理和表面化学水平上模拟了与齿斑形成有关的口腔中实际唾液和牙齿表面的唾液覆盖的细菌对具有相似于牙釉质表面能的唾液覆盖的表面间的相互作用,并评价了这种相互作用。表15中所示结果表明,与对照的安慰剂相比,天然产物漱口剂(2)有效地减少了64%的齿斑划痕、67%的细菌数量、60%的齿斑面积和71%的碳水化合物含量。这些结果综合到一起暗示优选选的天然产物洗口剂(2)不仅减少了齿斑评分,而且通过减少细菌数和碳水化合物含量改善了齿斑的性质。因此发现在天然产物漱口剂(2)中加入约0.5Wt%的桉叶油素或约0.01Wt%的鼠草油提高了其抗齿斑活性(以体外Chemostat齿斑模型评价)。

本发明的组合物对牙斑和龈炎在约4到约10.5的PH范围内最为有效,优选约6到约10的PH范围,最优选约7到约10的PH范围。
除澳大利亚茶树油、桉树脑、鼠尾草油那些天然芳香剂/产品外,上面讨论过的芳香剂也可加到本发明的组合物中,只要它们不抑制本发明组合物中决定性的百里酚和丁子香酚的协同功效或百里酚、丁子香酚和倍半萜烯醇的协同功效。在这些加入的试剂中可以使用的是:Tulsi油印度楝树油、Eichinacea酊、迷迭香提取物、goldenseed提取物、西番莲提取物、姜黄提取物、槟榔果(betelnut)提取物、sunphenon茶提取物、蒲公英根提取物等。
虽然用了一定数量的特定实施来描述本发明,但应明白,本发明公开的优选形式是仅用于举例目的,在不超出本发明的构思和范围内,可进行许多的变化。
Claims (9)
1.一种具有抗齿斑作用的牙用组合物,它包括百里酚、丁子香酚和倍半萜烯醇的组合。
2.根据权利要求1的组合物,其中百里酚、丁子香酚和倍半萜烯醇的组合量至少为全部组合物的5ppm。
3.根据权利要求1的组合物,其中百里酚的含量为牙用组合物重量的1000PPM到20000PPM,丁子香酚的含量为牙用组合物重量的1000PPM到20000PPM,并且所述组合物是一种漱口剂。
4.根据权利要求1的组合物,其中百里酚的含量是口用组合物重的5000PPM到20000PPM,丁子香酚的含量是口用组合物重的5000PPM和倍半萜烯醇的用量是口用组合物重的1000PPM到20000PPM,并且所述组合物是一种牙膏。
5.一种含有权利要求1组合物的感官上可以接受的产品,其还包括芳香剂,所述芳香剂是澳大利亚茶树油、桉树脑、鼠尾草油或其混合物。
6.根据权利要求5的产品,其中澳大利亚茶树油的含量为50到5000PPM。
7.根据权利要求6的产品,它分别含有5到500PPM的百里酚和丁子香酚,并且所述组合物为一种漱口剂。
8.根据权利要求6的产品,它分别有500到3000PPM的百里酚和丁子香酚,且所述组合物是一种牙膏。
9.权利要求1要求的组合物,其中存在最多达5wt%聚乙烯甲基醚/马来酸或马来酸酐共聚物或聚乙烯磷酸酯抗斑活性增强剂。
