背景技术
吸入器或喷气器(puffers)用于经由肺将药物递送进入人体。它们可用于,例如治疗哮喘和慢性阻塞性肺病(COPD)。吸入器的类型可以包括定量吸入器(metered doseinhalers)(MDIs),干粉吸入器(DPIs)和喷雾器(nebulisers)
在呼吸药物递送中面临的常见问题是如何监控病人坚持性和依从性。坚持性按照药方标签(prescription label)处理病人,例如,每天服用规定数目的剂量。例如,如果药方要求每天两个剂量,并且病人一天正在服用两个剂量,它们被认为是100%坚持。如果病人一天正在仅仅服用一个剂量,它们被认为是50%坚持。在后种情况中,病人没有得到他们的医生规定的治疗。
依从性,在另一方面,涉及病人如何使用他们的药物递送装置。如果以有效治疗推荐的方式使用,他们是100%依从性。但是如果没有适当地使用,他们小于100%依从。呼吸驱动吸入器(例如干粉吸入器(DPI))的使用涉及以特定方式吸入;例如,吸入可能需要足够长时间和足够努力以夹带全部剂量的药剂。对一些病人来说,例如,小孩和老人,满足完全依从性的要求可能会非常困难。不能到达100%依从性会降低规定药剂的效力。
对病人来说确定他或她是否吸入了规定剂量的药物,因此核实药方的依从性非常困难。特别是对DPIs,病人可能没有立即注意到药物正在被吸入(例如,因为颗粒太小他们可能没有感觉到或品尝到)。病人可能看到药效之后知道吸入,但是仍然可能不知道吸入的药物的量是否符合药方。
发明内容
本公开大体上涉及辅助病人依从通过吸入器的药剂给予。例如,本公开可涉及使用指示器指示什么时候吸入器准备好释放剂量和什么时候病人已经吸入足够接收推荐的剂量。
吸入器可以包括吸口盖子(mouthpiece cover)、压力传感器、第一指示器和第二指示器。第一指示器可以是第一灯,第二指示器可以是第二灯。第一指示器可以是灯的第一状态,第二指示器可以是灯的第二状态。第一指示器可以被配置用以根据吸口盖子的状态指示。例如,第一指示器可以被配置用以根据吸口盖子的打开状态照亮,例如,当根据吸口盖子的打开状态药物可能准备吸入时。例如,根据吸口盖子的打开状态药物可以从容器被转移到给药杯。第二指示器可以被配置用以根据压力传感器的输出指示。例如,第二指示器可以被配置用以根据吸口或吸入器的别处中的压力测量值超过预定阀值照亮。预定阀值可以与药物给予有关。

具体实施方式

可以载有处理器或发送器的PCB 150可以被并入吸入器100壳体的顶部。围绕PCB150的颈圈197可以在吸入器100的制造即将结束之际被固定在轭(未显示)的顶部。这可以是在灭菌吸入器壳体的部分之后进行的。这样可能比较有利,因为灭菌过程可能破坏PCB150上的敏感电子元件(sensitive electronics)。

图2A-C显示实例性吸入器(例如吸入器100)的内部结构的顶部部分的示例性装置。连接到铰链式吸口帽子(未显示)的轭1090载有波纹管(bellows)1091,该波纹管由例如部分顺应性材料(compliant material)制成。图2A显示了当盖子被关闭时的波纹管的位置。弹簧臂292的脚可以被接收在波纹管的上部壁中的凹处293中。因此,凹处293的底部可以推动脚的下表面,这样的弹簧臂向上偏。这可以造成弹簧臂292的头关闭开关295,这将PCB 250保持在睡眠模式。
图2B显示了当轭290和波纹管291因此稍微向上移动时装置随盖子的打开开始。弹簧臂292可以保持与开关295接触,顺应性的波纹管材料可以缓解任何额外的张力,该额外的张力否则会打开开关因为凹处293的底部可以弯曲以承担张力。
图2C显示了当盖子完全打开时的装置。轭290和波纹管291可以向下移动脱离弹簧臂292,该弹簧臂向下放松远离开关295。开关1095可因此被打开,唤醒PCB 250。
图3显示了吸入器(例如吸入器100)内部结构的示例图。例如,打开吸口盖子可以造成药物从容器转移到给药杯。吸入器300可以具有弹簧392和波纹管391。盖子380的打开可以造成弹簧392的压缩或解缩。当盖子380从吸口370打开时弹簧392可以压缩波纹管391。压缩的波纹管391可以提供压力使得容器378中的药物流向给药杯375。在容器378和给药杯375之间,可以存在允许药物流动的料斗。料斗可固定在例如,仅仅当盖子380打开时允许药物从容器378转移进入给药杯375的位置。当使用者使用吸口370吸入时来自给药杯的药物可流过气流道(flow channel)320。
图4是吸入器(例如吸入器100)中的实例性电子元件的示意图。吸入器可以包括电子模块400。电子模块400可以具有处理器457、和电源455、存储器445、指示器452、开关495、传感器410和/或收发器460。处理器457可以被连接到每个部件。开关495可通过轭被连接到吸口,例如关于图1B描述的。开关495不局限为机械开关,它可以是例如,电子开关。传感器410可向处理器457提供有关吸入器的吸口(例如,或吸入器的其他部分)中的压力变化(例如,压力不同)的信息。例如,传感器410可以向处理器提供瞬时压力读数或随时间的合计压力读数。例如,压力读数可以是峰值吸入气流(Peak Inspiratory Flow)、加速度、总体积和/或诸如此类方面。压力读数可以是振幅、加速度、体积和/或诸如此类的一个或组合方面。压力读数(例如,压力降)可以在吸入器700中的别处被测量,并且吸入器700可以因此计算吸口中的气流。压力读数可表示经过吸入器的吸口的一定量的空气流,其例如,可以表示药物正在被使用者吸入。同样地,传感器410可向处理器457提供有关压力读数、经过吸口(例如,和/或吸入器的其他部分)的一定量的空气流和/或药物被使用者吸入的信息。处理器457可根据传感器410的压力读数做出决定(例如,关于一个或多个指示)。例如,处理器457可根据传感器410提供的压力读数计算吸入的空气或药物的量。传感器410可以具有分开的处理器用以计算吸入的空气或药物水平,并将它提供给处理器457。
根据从开关495和/或传感器410接收的信息,处理器457可以确定压力读数(例如,该压力读数可表示被使用者吸入的药物)达到预定水平。可能有用于预定水平的查找表,利用该查找表处理器对比压力读数。当达到预定水平时,处理器457可以将信号发送到一个或多个指示器452用以指示吸入器的状态。处理器457可以在存储器中储存压力读数、压力读数的时间戳和/或根据压力读数推导的信息(例如给药的药物、给予给使用者的药物、全部给予给使用者的药物、经过吸口的空气流,等等)。例如,处理器457可以存取来自任何类型的合适存储器(例如不可移动存储器和/或可移动存储器)的信息(例如用于预定水平药物的查找表),并且将数据储存在任何类型的合适存储器(例如不可移动存储器和/或可移动存储器)中。不可移动存储器可以包括随机存取存储器(RAM)、只读存储器(ROM)、硬盘或任何其他类型的存储器储存装置。可移动存储器可以包括用户标识模块(SIM)卡、存储棒(memory stick)、安全数字(SD)存储卡等等。处理器可以存取来自存储器的信息,并且将数据储存在存储器中,该存储器没有物理定位在吸入器之中,比如在服务器上。
处理器457可以包括微控制器、可编程逻辑装置(PLD)、微处理器、专用集成电路(ASIC)、现场可编程门阵列(FPGA)或任何合适的处理装置、控制器、或控制电路。处理器457可以包括内部存储器。处理器457可以从电源455接收电力,并且可以被配置用以分配和/或控制电力到吸入器400中的其他部件。电源供应器可以是任何用于给吸入器供电的合适的装置。开关459可不被连接到处理器457,但是连接到电源455。电源可以被直接连接到传感器410、存储器445、指示器452和收发器460中的一个或多个。
指示器和电子模块可以永久地连接吸入器。如图1B中显示的,指示器和电子模块可位于吸入器的顶部。在一些实施方案中,电子模块可位于不同位置。例如吸入器可以具有可安装帽子以将它安装到吸入器的顶部。帽子可以包含指示器和控制指示器的电子模块。指示器可添加到现有吸入器或从现有吸入器中移除,而不影响它的例如通过使用可安装帽子的操作。
图5阐明了电子模块550(例如,依从性模块)如何可以被并入吸入器(例如吸入器100)的顶部,不管指示器和电子模块是否是永久的或可去除的。电子模块550可以是图4的电子模块400的一个例子。图5A显示了轭(例如,固定圈(retainer ring))590的示例性默认位置590,向上推触觉开关595以打开它。随着开关595打开,依从性模块550和电池555(例如纽扣电池)之间可能没有电连接。图5B显示了当吸入器被启动供使用时固定圈590的实例性位置,相对开关595被降低以关闭它以便依从性模块550被供电。
图5C阐明了图5A和B中显示的吸入器的制造的最后阶段。依从性模块550可以被降低到吸入器壳体之上,然后帽子598可以被固定在适当的位置。正如以前的实施例,可以提供LED指示器552。
下面提供了使用传感器检测吸入的实例性方法。虽然下面的例子使用压力传感器,特别是气压传感器,吸入器(例如吸入器100)可使用其他类型的传感器测量吸入。
呼吸量计是用于测量由病人的肺吸入和呼出的空气的体积的仪器。呼吸量计测量换气,进入和呼出肺的空气的运动。从被称为呼吸描记图(呼吸量计的输出)的跟踪,有可能鉴定异常(例如,阻塞性或限制性)换气模式。呼吸量计可以使用各种各样的不同测量方法,包括压力转换器(pressure transducers)、超声波和水位表。
为了监控与呼吸有关的气流,压力传感器可能是最方便的,因为压力信息能被用于确定流量,该流量然后能被用于确定体积。
用于呼吸检测的压力传感器可以测量贯穿病人气道(airway)截面的压力差异。这可以使用两个连接来进行,通过管子或其他合适的管道,将传感器连接到气道。也有可能使用通向气道的单个连接,具有开向大气的另一个端口。如果气道内的压力在气流被施加之前和之后被测量,单个端口计量式传感器(single port gauge type sensor)也能被使用,读数的不同代表在空气通道阻力(air path resistance)上形成的想要压力降。
除了差别(两个端口)式(differential(two port)type)压力传感器和单个端口计量式传感器,利用在使用之前和之后做出的分开的测量值之外,绝对或气压传感器可以使用。气压传感器参考真空。它们有时候被称为高度计,因为高度能从气压读数推断出来。
但是,随着微型化,包括MEMS和NEMS技术的引入,很多改进的传感器现在是可用的。MEMS气压传感器可能够从20kPa到110kPa操作,并且当气动地结合到具有已知气流阻力的气流通道时能检测少于30lpm(升/分钟)的气流速度。
使用气压传感器可以使气压能够作为贯穿测量周期的基线使用,由此这可以解决其他单个端口方法的不确定性。
并且,知道局部气压可以提供对病人肺功能的一些了解。人们怀疑大气压力的变化,例如与接近风暴锋面有关的变化,可能对病人呼吸有影响,甚至可能与哮喘和COPD事件有关。
气压传感器可以已经在应力状态下,其具有在真空的装置中密封的集成参考端口(integral reference port)。这意味着他们在感兴趣区域具有低滞后。
由于它们的敏感元件极其小的尺寸和质量,MEMS传感器可能够反应极其小的压力变化。一些能够分辨低达1Pa的压力变化。
例如,Freescale MPL3115A2MEMS气压计/测高仪芯片(压力20传感器)是数字的,使用I2C界面向主人微型计算机(host micro-computer)传达压力信息。
MEMS压力传感器能被包装在金属中。这提供RF屏蔽和用于温度补偿的优良导热性。
MEMS压力传感器还是成本低的,展示出低功耗,并且非常小。这使它们特别适合用在手提和/或一次性装置中,该装置,例如可以由电池(例如纽扣电池)提供电力。
MEMS压力传感器的小尺寸可以让它们并入现有吸入器设计变得很容易。也可很容易将它们并入吸口或靠近吸口以更准确地测量由病人的吸入或呼出造成的压力变化。
微型气压传感器能仅仅使用通向空气通道的小孔被直接连接到病人气道,这不要求任何类型的管子。这可以减少水分凝结和与弹性管子有关的潜在细菌生长的可能性。可以包括内部密封物,例如凝胶密封物以保护传感器元件不受污染。
图6显示了具有实例性配置中的传感器的吸入器吸口的横截面图。微型气压传感器610靠着气流道620(通过该气流道病人呼吸)被放置。空气流基本上是箭头630指示的轴向。传感器端口611可以是密封的,与气流道壁622中的开口621一致,通过气动的(例如气密的)密封物640。(注意,只要传感器端口和气流道之间存在气动连接,密封物就不需要完全气密)。传感器端口611可以包括过滤器,例如空气能透过,水不可透过过滤器。气流道和密封物可通过两次注模工艺(two-shot molding process)制成。压力传感器610可以被安装在印刷电路板(PCB)650上以提供到电源和其他电子元件的连接。
微型压力传感器(例如,整个微型压力传感器)可被封装在邻近气流道的腔室中,例如,不是围绕开口621和传感器端口611之间的通道定位密封物640。气动密封物可以位于传感器占用空间(footprint)的外部,并且可以从气流道壁的外部一直延伸到其上可安装传感器的表面(例如,PCB的部件表面)。
具有内置温度补偿的MEMS传感器是可用的。在一个实施例中,可以使用外部热传感器。在一个实施例中,可以不使用外部热传感器。补偿可正好在测量位点被提供,增加补偿的准确性。具有内置温度补偿的MEMS传感器还可以充当紧凑呼吸温度计,向病人和/或他们的照料者提供进一步的信息。如果传感器的外壳是金属的,那么不仅仅可能是敏感内部电路与RF射频场(RF fields)(例如与移动电话或附近干扰有关的RF射频场)隔离,而且传感器还可能迅速平衡局部温度以便提供最佳温度补偿。
在穿过吸入器的空气流通道中的任何地方或与空气流通道流体连通的任何地方添加微型气压传感器可使依从性监控能够进行,由于这样的微型传感器可收集足够的数据用以指示病人是否是以适当的方式(例如,足够努力和足够长时间)吸入以携带全部剂量的药剂。结合源于剂量计量系统的指示在吸入之前大剂量药剂可使用于气流道(通过该气流道病人吸入)的信号,这个信息可能足以确认剂量已经被成功地给予。

如果来自微型压力传感器的输出是数字的,全部低水平信号处理可在传感器中进行,保护它不受外部干扰。这使利用大约几十帕斯卡的信号的工作有可能没有很多具有外部电路的传统传感器会挑战的困难。
传感器可以,例如,用在呼吸驱动干粉吸入器中。这些吸入器可以被配置使得使用者通过吸口的吸入导致经过装置的空气流夹带干粉药剂。吸入还可能导致另一个空气流从外部进入吸入器。吸入器可以包括涡流室和室壁,在该涡流室中两个空气流彼此碰撞,室壁用以分解干粉药剂的聚集用于更有效的递送。
例如,传感器可被用在呼吸驱动加压气溶胶吸入器中。这些吸入器包括用于释放测量剂量的药剂的工具,该释放工具包括用于通过施加能够驱动递送工具的预载荷(preload)启动装置的工具,用于施加能够防止递送工具驱动的抗气动力的工具和能够释放抗气动力以允许预载荷驱动递送工具和分发药剂的释放装置。气动抗力能通过包括例如膜片、活塞筒、波纹管或弹簧的装置来建立。经过阀或穿过叶片装置(vane mechanism)的吸入允许预载荷驱动气溶胶阀用以释放药剂。
对这样的吸入器来说通过确定什么时候装置被启动和/或什么时候气溶胶阀打开可监控坚持性。再次,在穿过吸入器的空气流通道中的任何位置或与空气流通道流体连通的任何位置引入MEMS气压传感器,结合用于确定什么时候装置已经被启动和/或什么时候气溶胶阀打开的工具可使依从性监控能够进行。
启动装置可以导致预载荷被施加到递送工具和载荷(load)被施加到电子开关。这个开关可以被连接到处理器的输入电路使得当装置被启动时处理器接受电子脉冲。或者或此外,电子开关可被安排以通过气溶胶阀的运动或气溶胶阀之前的阀或叶片装置的运动被驱动。这个开关可被连接到处理器的输入电路使得当气溶胶被释放到气流道(通过该气流道病人吸入)时处理器接受电子脉冲。开关可以是例如,机械的、光学的、接近性(proximity-based)的或是加速计。
应该注意到因为MEMS气压传感器响应可随时间改变的环境气压,注意力应该放在任何随后的传感器输出信号分析基于的原始读数上。自动零读数(即皮重(tare))可以在监控任何吸入信号之前被立即执行。虽然对这个数值来说有可能在响应局部环境气压变化中随时间改变,但是如果治疗在几分钟之内完成可能不期望造成任何问题。第二气压计芯片可被用于记录气压活动,允许主芯片专门被用于呼吸检测。
给药完成处的点(肺活量峰值的地方)可以对应气流倒转方向处的点。因此,当来自压力传感器的数据指示气流方向已经倒转时处理器可以做出给药完成的决定。
不是全部处理都需要模块去做。任何或全部处理都可交给外部数据处理装置。无线方案(例如包括BLE模块)可以用于将病人气流属性发送到应用(app),该应用然后可计算具体呼吸参数。吸入器可以因此将这样的任务要求的处理交给,例如智能电话处理器。这可以促进对吸入器来说可能的最小形态因素。这个方法的进一步的优势可能是在智能电话上运行的软件比在吸入器上运行的软件可以更容易被更换。
处理器可以通过收发器将由传感器集中的和由处理器处理的信息提供给遥控装置。下面详细地描述了收发器。虽然下面的实施例使用无线通信,吸入器可经由其他模式,包括使用电线进行通信。
添加收发器使监控病人坚持性和依从性并将这样的信息,例如包括病人气流属性,传达到使用者装置,例如智能电话或平板电脑或计算机成为可能。该信息可以直接从吸入器或从使用者装置被发送到服务器。从使用者装置数据可被传达到照料者的装置,例如,医生的个人电脑(PC)。这可使用有线连接,例如经由通过串行总线(USB)端口来进行。利用无线技术,有可能将结果传达到外部世界,而不以任何有效方式干扰外壳内的产品。合适的无线技术可以包括,例如,WiFi技术(例如IEEE 802.11)、医疗体域网(Medical Body AreaNetwork)(MBAN)技术(例如IEEE 802.15),近场通讯(Near Field Communication)(NFC)技术、移动技术(例如3G和4G)和BluetoothTM技术(例如BluetoothTM低能耗(BLE))。无线收发器,例如呈BLE芯片形式,可被连接到微型传感器或与它集成在一起。
这样的无线连接可被用于,例如实时报告装置驱动和/或具有日期和时间戳的测知的吸入。这个日期可以是外部处理过的,并且如果这样的处理结果是确定药方应该被再装满,警报可以被发送到病人和/或照料者和/或药剂师。警报可经由吸入器的一个或多个用户界面(例如LED和/或蜂鸣器)或经由文字信息或电子信函来提供。作为一个实施例,如果在排定给药时间之后的预先确定了的期间内没有接收到给药报告,提示可被发送到病人和/或照料者。例如如果使用频率超过安全阀值也可以产生警报。
依从性模块可直接或间接与下列的一个或多个通讯:病人的或照料者(例如医生、护士、药剂师、家庭成员或看护者)的用户装置(比如移动手机,例如智能手机、平板电脑、笔记本电脑或台式电脑),例如健康服务提供商或吸入器或药物制造商或经销商的服务器,或云存储系统。这样的通讯可以是经由网络(比如互联网)的,并且可能涉及例如在病人的智能手机上的专业的应用(dedicated app)。
依从性监控工具(比如一个或多个传感器,例如装置驱动传感器比如检测坚持性和依从性报告工具的机械开关,例如检测用于适当剂量递送的足够气流的微型压力传感器)和依从性报告工具(比如无线发送器或有线输出端口)可以被包括在单个模块中。这个模块可以作为用于附接到现有的或稍稍改进的吸入器设计的分开的吸入器配件/升级版被销售。依从性监控模块可以在制造过程中被并入吸入器。依从性监控模块可在单个物理单元中。依从性监控模块可在多个物理单元中。在吸入器配件版本的情况中,模块可以由一个或多个可连接单元组成。在模块并入吸入器的情况中,独立部件可以位于吸入器之中或之上的任何适当的位置,并且不需要比他们运行要求任何更进一步的组合在一起或连接。

Claims (20)

1.吸入器,其包括:
