本发明涉及一种治疗妇科阴道炎的中药洗液,该中药清洗液由如下重量配比的原料组成:酢浆草110~130份、溪黄草90~110份、椿皮80~100份、番石榴叶80~100份、秦皮50~70份、大叶桉叶50~70份、瓜子金30~50份、桃枝30~50份、蓍草40~60份、三白草40~60份、天葵子30~50份、骨碎补50~70份、蛇床子30~50份、土荆皮70~90份、皂角刺20~40份、山药30~50份、干漆20~40份、刘寄奴20~40份、鸡血藤30~50份、扶桑根10~30份、红泽兰10~30份;其具有清热燥湿、化瘀止痛、清热解毒、杀虫止痒、补益脾肾、调经止带之功。

技术领域
本发明涉及一种治疗妇科阴道炎的中药洗液,属于中药领域。
背景技术
妇科阴道炎是妇科常见病、多发病,发病率高,严重影响了广大妇女的工作、学习和身体健康。阴道炎是阴道黏膜及黏膜下结缔组织的炎症,常见的有细菌性阴道病、滴虫性阴道炎、霉菌性阴道炎、老年性阴道炎等。临床上以白带的性状发生改变以及外阴瘙痒灼痛为主要临床特点。感染阴道炎后长期不进行有效治疗,会导致子宫内膜抵抗能力减弱,细菌易上行感染,透过子宫内膜,引起宫颈炎、盆腔炎、附件炎等妇科炎症,还有可能会引起不孕。
目前临床上治疗西药的药物制剂很多,但患者长期使用这些西药抗生素会产生毒副作用。且这些药物在抑制或杀灭病原微生物的同时杀灭有益菌群,破坏引道菌群的平衡,导致妇科疾病反复发作的缺点。传统的中医药相对于西药,其具有疗效确切,复发率低,毒副小作用的优势,中医采用辨证论治进行治疗,可以达到标本兼治的目的。
发明内容
本发明的目的是提供一种治疗妇科阴道炎的中药洗液。该中药洗液是经过本发明人经过多年临床研究,终于找到了疗效显著,复发率低的中药外用洗液。
本发明所述的治疗妇科阴道炎的中药洗液,它是由如下原料药组成:酢浆草、溪黄草、椿皮、番石榴叶、秦皮、大叶桉叶、瓜子金、桃枝、蓍草、三白草、天葵子、骨碎补、蛇床子、土荆皮、皂角刺、山药、干漆、刘寄奴、鸡血藤、扶桑根、红泽兰。
本发明的中药洗液的组方用量在下述重量份范围都具有较好的疗效。
作为本发明的优选,它是由如下重量配比的原料药组成:酢浆草110~130份、溪黄草90~110份、椿皮80~100份、番石榴叶80~100份、秦皮50~70份、大叶桉叶50~70份、瓜子金30~50份、桃枝30~50份、蓍草40~60份、三白草40~60份、天葵子30~50份、骨碎补50~70份、蛇床子30~50份、土荆皮70~90份、皂角刺20~40份、山药30~50份、干漆20~40份、刘寄奴20~40份、鸡血藤30~50份、扶桑根10~30份、红泽兰10~30份。

取上述重量配比的酢浆草、溪黄草、椿皮、番石榴叶、秦皮、大叶桉叶、瓜子金、桃枝、蓍草、三白草、天葵子、骨碎补、蛇床子、土荆皮、皂角刺、山药、干漆、刘寄奴、鸡血藤、扶桑根、红泽兰,加水煎煮二次,第一次加药材重量8~12倍量的水,煎煮1~2小时,第二次加药材重量6~10倍量的水,煎煮0.5~1.5小时,合并两次煎液,滤过,滤液浓缩成清膏,加入乙醇,使含醇量达40%~60%,静置8~24小时,滤过,回收乙醇,即得。

刘寄奴,来源,菊科植物奇蒿ArtemisiaanomalaS.Moore的地上部分。性味,性温,味辛、微苦。功效,活血通经、消积、止痛。
鸡血藤,来源,豆科植物密花豆SpatholobussuberectsDunn.的藤茎。性味,性温,味苦、甘。功效,补血、活血、通络,具抗炎作用。
扶桑根,来源,为锦葵科植物朱槿的根。性味,味涩,性平。功效,治月经不调,血崩,白浊,白带。
红泽兰,来源,爵床科红泽兰属植物红泽兰Championellajaponica(Thunb.)Brem.以全草入药。性味,辛,温。功效,活血通经,去瘀止痛。
阴道炎属于祖国医学的“带下病”,根据临床表现和舌脉象,临床上常分为脾阳虚、肾阳虚、阴虚挟湿、湿热下注、湿毒蕴结五种证型,水失健运、湿热(毒)下注是其主要病机,清热利湿、解毒杀虫为主要疗法。方中选择酢浆草,溪黄草共为君药,配以椿皮,番石榴叶,秦皮,大叶桉叶,六药合用可使清热燥湿之功,辅以瓜子金,桃枝,蓍草,三白草,天葵子,四药相须为用,共为臣药,佐以骨碎补,蛇床子,土荆皮,皂角刺,山药,干漆,刘寄奴,鸡血藤,八药相配为佐药,以达补益脾肾,杀虫止痒之功。最后配以扶桑根,红泽兰,以加强活血止痛,调经止带。诸药合用,共奏清热燥湿、化瘀止痛、清热解毒、杀虫止痒、补益脾肾、调经止带之功。
药理实验表明,本发明中药洗液具有广谱抗菌、抗滴虫、抗炎、镇痛、止痒等药理作用。经临床试验验证,本发明的中药洗液对细菌性阴道炎的总有效率为92.45%,对白色念珠菌性阴道炎总有效率为89.58%,对滴虫性阴道炎的总有效率为92.73%,其临床有效率优于对照组(洁尔阴)。由此可知,本发明中药洗液具有疗效显著,副作用小,复发率低的优点。

取酢浆草120g、溪黄草100g、椿皮90g、番石榴叶90g、秦皮60g、大叶桉叶60g、瓜子金40g、桃枝40g、蓍草50g、三白草50g、天葵子40g、骨碎补60g、蛇床子40g、土荆皮80g、皂角刺30g、山药40g、干漆30g、刘寄奴30g、鸡血藤40g、扶桑根20g、红泽兰20g;加水煎煮二次,第一次加药材重量10倍量的水,煎煮1.5小时,第二次加药材重量8倍量的水,煎煮1小时,合并两次煎液,滤过,滤液浓缩成清膏,加入乙醇,使含醇量达50%,静置12小时,滤过,回收乙醇,即得。
洁尔阴洗液(成都恩威制药有限公司);盐酸吗啡注射液(东北制药集团公司沈阳第一制药厂),甲硝唑(亚宝药业集团股份有限公司),磷酸组胺(Sigma公司),巴豆油。
1.2动物及菌株
昆明种小鼠,清洁级,雌雄兼用,体重20~23g;豚鼠,普通级,雌雄兼用,体重300~400g;均为西安医学院实验动物中心提供。标准菌株:大肠埃希菌、绿假单胞菌、金黄色葡萄球菌、白色念珠菌(中国药品生物制品检定所提供);阴道毛滴虫(临床株):西安高新医院检验科提供。
2.试验方法
2.1体外抑茵实验
将本发明中药洗液用相应培养基1:2、1:4、1:8、1:16、1:32、1:64、1:128、1:256、1:512、1:1024稀释,每一稀释管内加入浓度为1×10个/ml的金葡菌、绿脓杆菌、大肠杆菌、白色念珠菌0.5ml,37℃培养24小时,记录最低抑菌浓度(MIC)。

由表1结果可知,本发明中药洗液对金黄色葡萄球菌、绿脓杆菌、大肠杆菌和白色念珠菌均有抑制作用,尤其对白色念珠菌的抑制作用最强。这也说明,本发明中药洗液对这些病菌所致感染性疾病有良好的治疗作用。
2.2体外抗滴虫试验
将本发明中药洗液和甲硝唑分别加入阴道滴虫培养液中,稀释成1000、500、250、125、62.5mg/kg本发明中药洗液及甲硝唑液(20μg/kg),将上述药物培养液分别加入培养管中,同时设空白对照管。将培养48小时的阴道滴虫(1.8×10爪/rn1),取0.2ml,分别加入上述培养管中,再置于37℃孵箱中继续培养。分别于l0分钟、2、18、24小时取出各管,摇匀培养液,随机取悬液,滴片,镜检,观察计数虫体,计算虫体死亡率。24小时后,再转种培养72小时,观察虫体生长情况。

“—”表示无虫体生长,“++”表示有一定的虫体生长,“++++”表示虫体生长旺盛。
实验结果表2说明,本发明中药洗液对阴道滴虫的药物抑制浓度为125mg/kg。这些也进一步提示,本发明中药洗液对阴道滴虫引起的阴道炎具有治疗作用。
2.3抗炎作用实验
取昆明种雄性小鼠40只,体重25~27g,随机分为4组,每组10只。本发明中药洗液,30%本发明中药洗液两个剂量组,阳性药组(洁尔阴洗液)及正常对照组。分别外涂本发明中药洗液、洁尔阴洗液和蒸馏水,每只涂耳50μl,每天1次,连涂3天。末次涂药后30分钟,用巴豆油混合致炎剂涂右耳两面(50μl/只)致炎,隔4小时用打孔器切下同部位等面积两圆耳片称重,以左右耳片重量差作为耳肿胀指标。

结果表3证实,本发明中药洗液对小鼠巴豆油所致耳肿胀有明显的抑制作用,其抑制效果优于对照组。
2.4镇痛试验
2.4.1热板法
选取基础痛阈为10~30秒昆明种雌性小鼠40只,体重(23±2)g,随机分为4组,每组10只。本发明中药洗液,30%本发明中药洗液两个剂量组和空白对照组分别用本发明中药洗液、30%本发明中药洗液和蒸馏水,泡小鼠四肢至踝关节处,间隔1小时重复给药1次,共2次,泡10秒。另设吗啡对照组,于测痛阈前30分钟ip1次(5mg/kg)。末次药后30分钟用热板测痛仪测定痛阈,每隔30分钟测1次,共测4次。

表4结果表明,与空白组比较,本发明中药洗液对小鼠热板痛阈值有明显的提高(P<0.05)。
2.4.2对小鼠辐射热致痛的影响
选取基础痛阈为2~4秒的昆明种小鼠,40只,雌雄各半,随机分为4组,每组10只。分组和给药同“2.4.1”,给药方法自尾根部至尾尖分别涂药,隔1小时1次,共涂2次,末次涂药后30分钟,以12V,50W放影灯泡制成的光热致痛仪,刺激尾端1/3处,以甩尾时间为指标,超过10秒停止照射按10秒计,测30~60分钟的痛阈值。

由表5结果可知,和空白组比较,30%本发明中药洗液及本发明中药洗液在30分钟可显著提高小鼠辐射热痛阙(P<0.05)。这也提示,本发明中药洗液对辐射热致痛的小鼠具有明显的镇痛作用。
2.5止痒作用实验
选取豚鼠40只,雌雄各半,随机为4组,每10只。即30%本发明中药洗液及本发明中药洗液的两个剂量组,阳性药组(洁尔阴洗液)及空白对照组。每只鼠右后肢足背脱毛,24小时后用本发明中药洗液及30%本发明中药洗液、洁尔阴洗液、蒸馏水泡右后肢10秒,每天1次,共3天。实验当日用粗砂纸擦伤右后足背脱毛处,面积约1cm2。于末次药后10分钟,在擦伤处滴0.01%、0.02%、0.03%递增浓度的磷酸组胺60μL/只,间隔3分钟,直至出现豚鼠回头舔右后足背为止,以所给磷酸组胺总量为致痒阈。

由表6结果可知,与空白组比较,本发明中药洗液及洁尔阴洗液组能显著提高豚鼠致痒阈值,且本发明中药洗液药效优于洁尔阴洗液。这说明,本发明中药洗液有显著的止痒作用。本实验的研究结果,为本发明中药洗液的临床应用提供了进一步实验依据。
具体实施方式
以下是本发明内容的具体实施例,用于阐述本申请文件中所要解决技术问题的技术方案,有助于本领域技术人员理解本发明内容,但本发明技术方案的实现并不限于这些实施例。
实施例1
取酢浆草120g、溪黄草100g、椿皮90g、番石榴叶90g、秦皮60g、大叶桉叶60g、瓜子金40g、桃枝40g、蓍草50g、三白草50g、天葵子40g、骨碎补60g、蛇床子40g、土荆皮80g、皂角刺30g、山药40g、干漆30g、刘寄奴30g、鸡血藤40g、扶桑根20g、红泽兰20g;加水煎煮二次,第一次加药材重量10倍量的水,煎煮1.5小时,第二次加药材重量8倍量的水,煎煮1小时,合并两次煎液,滤过,滤液浓缩成清膏,加入乙醇,使含醇量达50%,静置12小时,滤过,回收乙醇,即得。
实施例2
取酢浆草130g、溪黄草90g、椿皮100g、番石榴叶100g、秦皮50g、大叶桉叶70g、瓜子金30g、桃枝50g、蓍草60g、三白草40g、天葵子30g、骨碎补50g、蛇床子30g、土荆皮90g、皂角刺40g、山药30g、干漆40g、刘寄奴20g、鸡血藤50g、扶桑根30g、红泽兰10g;
加水煎煮二次,第一次加药材重量12倍量的水,煎煮2小时,第二次加药材重量6倍量的水,煎煮0.5小时,合并两次煎液,滤过,滤液浓缩成清膏,加入乙醇,使含醇量达60%,静置24小时,滤过,回收乙醇,即得。
实施例3
取酢浆草110g、溪黄草110g、椿皮80g、番石榴叶80g、秦皮70g、大叶桉叶50g、瓜子金50g、桃枝30g、蓍草40g、三白草60g、天葵子50g、骨碎补70g、蛇床子50g、土荆皮70g、皂角刺20g、山药50g、干漆20g、刘寄奴40g、鸡血藤30g、扶桑根10g、红泽兰30g,加水煎煮二次,第一次加药材重量8倍量的水,煎煮1小时,第二次加药材重量10倍量的水,煎煮1.5小时,合并两次煎液,滤过,滤液浓缩成清膏,加入乙醇,使含醇量达40%,静置8小时,滤过,回收乙醇,即得。
为了进一步验证本发明成品制剂的疗效,我们将上述具体实施例1中制备的中药清洗液,进行了相应的临床试验,现将结果报告如下。
2临床观察及疗效
2.1一般资料
采用随机、对照、双盲原则,收集妇科门诊阴道炎患者302例。将其分为3组,细菌性阴道炎组102例,其中治疗组53例、对照组49例,年龄在23~58岁,平均年龄(37.3±1.9)岁;白色念珠菌性阴道炎组94例,其中治疗组48例、对照组46例,年龄在25~61岁,平均年龄(39.2±2.1)岁;滴虫性阴道炎组106例,其中治疗组55例、对照组51例,年龄在21~63岁,平均年龄(38.5±2.2)岁;各组患者在年龄、病程等方面比较差异无统计学意义(P>0.05),具有可比性。
2.2诊断标准
2.2.1中医湿热瘀阻证的诊断标准
主症:外阴瘙痒,带下量多,色黄或黄白相兼,有异味,质黏稠,小腹坠胀或疼痛。次症:纳食较差,头晕目眩,口苦咽干,心烦不宁,小便短赤。舌苔脉象:舌质红,苔黄腻或有瘀斑,脉濡数或弦滑而数。以上主症具备2项以上,或主症具备1项以上,次症具备2项,再结合舌象、脉象均可确诊。
2.2.2西医诊断标准
①临床表现:外阴及阴道瘙痒、灼痛、白带增多,或伴有尿频、尿急、尿痛等尿道刺激症状,或伴有局部刺激症状。②妇科检查:分泌物异常:滴虫性阴道炎分泌物呈灰黄色、乳白色或黄绿色脓性,呈泡沫状,有腥臭味;念珠菌性阴道炎分泌物为白色、凝乳块或豆腐渣样,略带异味;细菌性阴道炎分泌物呈灰白色,有鱼腥臭味。③实验室检查:滴虫性阴道炎,在阴道分泌物中显微镜下可找到滴虫;念珠菌性阴道炎在分泌物中可查到念珠菌;细菌性阴道病在分泌物中可见到线索细胞等。
2.3治疗方法
2.3.1对照组
细菌性阴道炎组、白色念珠菌性组、滴虫性阴道炎组,3组中对照组的患者采用洁尔阴冲洗后阴道,避开经期,2周为1个疗程,连续治疗3个疗程。
2.3.2治疗组
细菌性阴道炎组、白色念珠菌性组、滴虫性阴道炎组,3组患者以本发明实施例1方法制备的中药洗液冲洗阴道,每日2次,治疗期间,严禁房事。用药期间每天更换内裤,并将用过的内裤、毛巾进行消毒,禁食辛辣刺激食品。禁行房事,禁止盆浴、游泳。如遇经期仅以温开水淋洗外阴,禁行阴道冲洗,月经干净2d后可继续治疗。2周为1个疗程,连续治疗3个疗程后,判断疗效。
2.4疗效判定标准
痊愈:疗效指数≥95%,用药后中医主症消失,局部体征消失,分泌物检查转为正常。显效:疗效指数≥70%,<95%,用药后中医主症基本消失,次症明显减轻,局部体征明显减轻,阴道清洁度变为Ⅰ级,滴虫、霉菌、唾液酸苷酶、过氧化氢浓度、阴道分泌物胺试验、线索细胞转为阴性,阴道PH值正常。有效:疗效指数≥30%,<70%,中医主症有所好转,局部体征有所减轻,阴道清洁度变为Ⅰ或Ⅱ级,滴虫、白色念珠菌、唾液酸苷酶、过氧化氢浓度,阴道分泌物胺试验,线索细胞时有阴性。无效:疗效指数<30%,中医主症无明显改善,局部体征无明显改善,分泌物检查异常、无明显改善。
2.5统计学处理
采用统计软件SPSS13.0处理数据,采用检验,P<0.05有统计学意义。
3结果
3.1试验结果见表7-9。

由表7的结果可知,细菌性阴道炎患者经过3个疗程的治疗后,治疗组患者的总有效率为92.45%,优于对照组。

由表8的结果可知,白色念珠菌性组患者中的治疗组和对照组的总有效率分别为89.58%,67.39%,治疗组的效果优于对照组(P<0.05)。

由表9的结果可知,患滴虫性阴道炎组的患者,其中治疗组和对照组的总有效率分别为92.73%,72.55%,治疗组的效果优于对照组(P<0.05)。
3.2复发情况
经随访6个月,治疗组(细菌性阴道炎,白色念珠菌性阴道炎,滴虫性阴道炎)痊愈的86例中,复发2例,复发率2.33%;对照组(洁尔阴)痊愈55例中,复发8例,复发率14.55%。治疗组与对照组患者复发率比较显著性差异(P<0.05)。
综上所述,本发明的中药洗液对细菌性阴道炎、白色念珠菌性患者、滴虫性阴道炎患者具有显著的疗效,其制备工艺简单,稳定,可行。这也进一步说明本发明的中药洗液具有良好的市场应用前景。
Claims (6)
1.一种治疗妇科阴道炎的中药洗液,其特征在于,它是由如下原料药组成:酢浆草、溪黄草、椿皮、番石榴叶、秦皮、大叶桉叶、瓜子金、桃枝、蓍草、三白草、天葵子、骨碎补、蛇床子、土荆皮、皂角刺、山药、干漆、刘寄奴、鸡血藤、扶桑根、红泽兰。
2.如权利要求1所述的治疗妇科阴道炎的中药洗液,其特征在于,它是由如下重量配比的原料药组成:酢浆草110~130份、溪黄草90~110份、椿皮80~100份、番石榴叶80~100份、秦皮50~70份、大叶桉叶50~70份、瓜子金30~50份、桃枝30~50份、蓍草40~60份、三白草40~60份、天葵子30~50份、骨碎补50~70份、蛇床子30~50份、土荆皮70~90份、皂角刺20~40份、山药30~50份、干漆20~40份、刘寄奴20~40份、鸡血藤30~50份、扶桑根10~30份、红泽兰10~30份。
3.如权利要求2所述治疗妇科阴道炎的中药洗液,其特征在于,它是由如下重量配比的原料药组成:酢浆草120份、溪黄草100份、椿皮90份、番石榴叶90份、秦皮60份、大叶桉叶60份、瓜子金40份、桃枝40份、蓍草50份、三白草50份、天葵子40份、骨碎补60份、蛇床子40份、土荆皮80份、皂角刺30份、山药40份、干漆30份、刘寄奴30份、鸡血藤40份、扶桑根20份、红泽兰20份。
4.如权利要求1~3中任一所述中药洗液的制备方法,其特征在于,它是由下述工艺步骤组成:
取上述重量配比的酢浆草、溪黄草、椿皮、番石榴叶、秦皮、大叶桉叶、瓜子金、桃枝、蓍草、三白草、天葵子、骨碎补、蛇床子、土荆皮、皂角刺、山药、干漆、刘寄奴、鸡血藤、扶桑根、红泽兰,加水煎煮二次,第一次加药材重量8~12倍量的水,煎煮1~2小时,第二次加药材重量6~10倍量的水,煎煮0.5~1.5小时,合并两次煎液,滤过,滤液浓缩成清膏,加入乙醇,使含醇量达40%~60%,静置8~24小时,滤过,回收乙醇,即得。
5.如权利要求4所述中药洗液的制备方法,其特征在于,它是由下述工艺步骤组成:
取上述重量配比的酢浆草、溪黄草、椿皮、番石榴叶、秦皮、大叶桉叶、瓜子金、桃枝、蓍草、三白草、天葵子、骨碎补、蛇床子、土荆皮、皂角刺、山药、干漆、刘寄奴、鸡血藤、扶桑根、红泽兰,加水煎煮二次,第一次加药材重量10倍量的水,煎煮1.5小时,第二次加药材重量8倍量的水,煎煮1小时,合并两次煎液,滤过,滤液浓缩成清膏,加入乙醇,使含醇量达50%,静置12小时,滤过,回收乙醇,即得。
6.如权利要求1~3中任一所述中药洗液在制备治疗妇科阴道炎中的应用。
