技术领域
本发明涉及含有来源于植物、动物或矿物组份的保健品,特别涉及一种防治阴道炎症的妇科洗液及其制备方法。
背景技术
调查显示,全世界约87%的成年女性不同程度的感染过妇科炎症,临床常见的妇科炎症有阴道炎、宫颈炎、子宫内膜炎等。临床上妇科炎症主要表现为阴道瘙痒、灼热肿痛、白带增多、尿频、尿急、尿痛等症状,反复发作,经久不愈,影响生活质量和夫妻生活。
健康的女性,外阴只用清水清洗即可,阴道内部有自洁功能,不必清洗。如果没病,却天天使用洗液清洁,就会刺激阴道,影响乳酸的分泌,打破正常菌群和酸性环境,从而降低人体的抵抗力。
妇科炎症往往预示着疾病,所以必须正规看医生,并在医生指导下治疗。洗液仅仅是在病症明确之后的辅助保健措施,不可代替医生治疗。现在的医药厂家纷纷推出自己的妇女洗液品牌,其中大多属于中药制剂,能起到杀菌、清洁的作用。
此外,部分妇科洗液中还有以下一些西药成分,主要作用是抑制病菌滋生,维持阴道内正常的弱酸性环境:醋酸氯已定:白色念珠菌,淋病双球菌的抑菌杀菌。醋酸洗必泰:适用于皮肤及粘膜的消毒;创面感染、阴道感染和子宫颈糜烂的冲洗。
外阴炎是妇科常见病,现代医学认为,本病可分为特异性与非特异性感染两种。特异性外阴炎以霉菌、滴虫感染为主,非特异性外阴炎以葡萄球菌、大肠杆菌、链球菌感染等为主,临床以非特异性感染比较多见,此外还可以继发于其他局部或全身疾病,如宫颈炎、阴道炎、宫颈癌等病,因分泌物增多的刺激,或经血、恶露过多过久的刺激,均可引发不同程度的炎症。
女性特殊生理条件致病菌不少以女性阴道为孳生地,可因女性每月内分泌变迁,影响阴道PH条件,有利于这类寄生菌活跃繁殖,数量增多,成为发病要素,产生局部瘙痒,灼热,黏膜红肿,充血,甚而可使黏膜发疹或糜烂。随之刺激阴道分泌物增多,微生物(真菌或滴虫)的产物假膜形成絮状白带,或泡沫样分泌溢出,腥臭味日渐增重。在女性阴道微生物繁殖期间,男女性交,极易成为男性被感染的机会,导致包皮龟头炎的发生。感染局部产生红 肿,发疹、糜烂、渗液及灼热、发痒一系列症状。
其中部分患者原发病(如霉菌或滴虫性阴道炎)治愈后,外阴瘙痒症状仍不能缓解。目前西医尚无特效疗法。抗生素治疗妇科的各种生殖泌尿系统炎症及痛经带下等效果很不理想,用西药治疗此类妇科病,不但疗效差、易复发,而且副作用大,又没有特效的中成药。临床以中医师随病证外方作汤剂口服,以及一些中药洗剂,但治疗效果均不甚理想,存在着可诱发耐药性、复发率高、用药不便、制剂困难等不足。而目前已经公开地用于妇科洗液的药物,如:“恩威牌”洁尔阴洗液,仍然不能达到理想的抗炎、止痒效果。
许多患者迫切希望能有一种使用方便而效果好的外用药物,用于防治各种类型的阴道炎症,能达到标本兼治,以内养外,且无毒副作用,使用方便,治愈时间短,治愈后不易复发的妇科洗液。
发明内容
本发明所要解决的技术问题在于,由于阴道特殊结构,妇女平时忽视卫生,或久居阴湿之地,以致湿邪侵入阴道,或情怀不畅,肝郁化火,脾虚生湿,湿热郁注下焦,均可发为阴痒。治疗妇科的西药药物很多,有抗菌药物如甲硝唑、环丙沙星等,微生态治疗制剂如乳酸杆菌、地衣芽孢杆菌。抗生素治疗妇科的各种生殖泌尿系统炎症及痛经带下等效果很不理想,用西药治疗此类妇科病,不但疗效差、易复发,而且副作用大,又没有特效的中成药,临床以中医师随病证外方作汤剂口服,以及一些中药洗剂,但治疗效果均不甚理想,存在着可诱发耐药性、复发率高、用药不便、制剂困难等不足。现在迫切需要提供了一种见效快、副作用小、抑菌性强,止痒杀虫效果迅速的的洗液,能有效解除患者的痛苦,治疗妇科阴道炎症。

为解决上述技术问题,本发明还提供一种防治阴道炎症的妇科外用洗液的制备方法,其特征在于,所述洗液的制备步骤包括:
a.将所述原料药除白及、薄荷、艾叶和苍术外放入粉碎机粉碎后过筛,
b.将过筛后剩下的颗粒物放入乙醇中加热回流提取2次;
c.再取上述乙醇提取后的药渣,加水加热回流提取2次;
d.将上述两种提取液合并浓缩;
e.将白及、薄荷、艾叶和苍术用蒸馏法汽化成白色结晶粉末;
f.将汽化的白色结晶粉末倒入浓缩液中混匀,加甲硝唑溶液搅拌均匀,紫外线消毒后罐装。
所述步骤a中,可以将所述原料药除白及、薄荷、艾叶和苍术外放入粉碎机粉碎后过100目细筛,使能通过100目筛的细粉量在达到30%,收集通过100目筛的原料细粉备用。
所述步骤b中,可以将过筛后剩下的粗颗粒物放入乙醇中加热回流提取2次,每次1~2小时,将2次提取液合并静置;
所述步骤c中,可以将上述乙醇提取过的药渣加10倍量水加热回流提取2次,每次1~2小时,将2次提取液合并静置。
所述步骤d中,可以将上述两种提取液合并,减压浓缩相对密度为80℃时1.37的滤液,回收乙醇,将所得滤液调整比重到1.06,加入过筛的细粉后搅匀。倒入0.5%甲硝唑溶液中。
所述步骤e中,可以将白及、薄荷、艾叶和苍术用蒸馏法汽化成白色结晶,干燥后成粉末状,收集备用。
所述步骤f中,可以将浓缩的药液加上汽化的白色结晶粉末混匀,倒入0.5%甲硝唑溶液中搅拌均匀,用紫外线消毒后罐装。
治疗采取外洗或坐浴,每次20~30mL,每日2次。一般以10d为1疗程,2~3个疗程后观察疗效。
与现有技术相比教,本发明具有止痒迅速,杀菌彻底,消炎除虫效果强,抗炎、止痒效果好;并且给药方便,药液易于制造,成本低廉等优点。且安全,毒副作用小,使用方便,治愈时间短,治愈后不易复发。能创造较好的社会价值及经济利益。
具体实施方式
从解剖上来讲,女性的外阴处于一个自然闭合的状态,保证阴道不和外界相通。阴道壁在平时前后壁也是闭合的状态,宫颈也是一个闭合状态。除 此以外另外,间接接触传染也是霉菌性阴道炎的一条传播途径。接触哪些物品可能被传染阴道炎接触被霉菌患者感染的共厕所的坐便器、浴盆、浴池坐椅、毛巾,使用不洁卫生纸,都可以造成传播,当被感染者外阴阴道的念珠菌达到一定数量时,即可发生霉菌性外阴阴道炎。其他可导致阴道炎的行为抗菌素、消炎药是罪魁大量服用抗菌素也可以导致阴道炎。只要用了抗生素无论是口服还是打针都会抑制阴道的乳酸杆菌扰乱阴道的自然生态平衡,改变了阴道的微环境,致病的细菌病源体就可能繁殖,最终导致局部的念珠菌性阴道炎发作。所以一般情况下不要大量服用抗菌素类药物。
阴道各种炎症属中医妇科杂病阴痒和带下范畴,两者常相互伴见。阴痒和带下的发生有两方面原因:一方面与内因:肝、肾、脾功能失常关,肝脉绕阴器,又主藏血,为风木之脏;肾主藏精,主生殖,开窍于二阴;脾主运化水湿。如若肝经湿热或肝郁脾虚化火生湿,湿热之邪随经下注,蕴结阴器故而阴痒;或肝肾不足,精血亏虚,生风化燥,阴道肌肤失养,不荣而痒;另外一方面与外因感染湿邪病虫有关,因摄身不慎,致湿邪病虫侵入,虫扰阴道,发为阴痒。《医宗金鉴·妇科心法要诀》所说“妇人阴痒,多因湿热生虫”即是此意。显然,引发感染性阴道炎的原因应属后者。按照传统中医理论,由内因所致者以内服药调整功能为主,而因外感病虫所致者或辅以外治或以外治为主要手段。
由于感染湿邪病虫所致之阴痒、带下者多表现为肝经湿热,因而治疗应以清热除湿、杀虫止痒为原则。本病主要是因湿热蕴结,虫蚀阴中所致湿热之邪有内外之分。如久居湿地等致湿邪外侵,郁而化热,或经期、产后,湿热邪毒乘虚而入,此为外感湿热。若素体脾气虚弱,或肝气郁结,木克脾土,脾失健运,水湿内留,停注下焦,蕴而化热,则为内生湿热。湿热蕴结,韧带不固,则带下增多,色黄。下焦湿热,膀优失约则并发淋证;湿邪浸淫日久成毒,或感染热毒邪气,热盛迫血妄行,或湿毒腐蚀肌肤,则可见带下脓血,或见″阴蚀″、″阴疮″。湿腐生虫,或摄生不慎,虫邪直犯阴器,虫蚀阴中则阴痒,阴中灼痛本病虽病在阴器,但每与肝脾等脏腑有关。多因湿邪为病。显然,引发感染性阴道炎的原因应属后者。
白及性苦、甘、涩,微寒。归肺、肝、胃经。功能主治收敛止血,消肿生肌。用于咳血吐血,外伤出血,疮疡肿毒,皮肤皲裂;肺结核咳血,溃疡病出血。《神农本草经》:“主痈肿、恶疮、败疽,伤阴死肌,胃中邪气”;《本草纲目》:“气味(根)苦、平、无毒。
地肤子清热利湿,祛风止痒。用于小便涩痛、阴痒带下、风疹、湿疹、皮肤瘙痒。《药性论》:″与阳起石同服,主丈夫阴痿不起,补气益力;治阴卵癀疾,去热风,可作汤沐浴。″地肤子煎剂2g/kg给家兔灌胃,6小时内尿中氯化物增加127.1%~131.6%,但总尿量无变化.有报告指出,地肤子对大白鼠尿量及尿钠排出均有影响。
黄柏清热燥湿,泻火除蒸,解毒疗疮。用于湿热泻痢,黄疸,带下,热 淋,脚气,痿辟,骨蒸劳热,盗汗,遗精,疮疡肿毒,湿疹瘙痒。盐黄柏滋阴降火。用于阴虚火旺,盗汗骨蒸。《中国药典》载黄柏清热燥湿之力,与黄芩、黄连相似,但以除下焦之湿热为佳。
苦参性苦,寒。归心、肝、胃、大肠、膀胱经清热燥湿,杀虫,利尿。用于热痢,便血,黄疸尿闭,赤白带下,阴肿阴痒,湿疹,湿疮,皮肤瘙痒,疥癣麻风;外治霉菌性阴道炎,常与黄柏结合使用,是治疗阴道炎的外用妇科良药。
百部性味甘苦,微温。《别录》:微温。《药性论》:味甘,无毒。《日华子本草》:味苦,无毒。性味甘、苦,微温。归肺经。功能主治润肺下气止咳,杀虫。用于新久咳嗽,肺痨咳嗽,百日咳;外用于头虱,体虱,蛲虫病,阴道骚痒。
白藓皮祛风,燥湿,清热,解毒。治风热疮毒,疥癣,皮肤痒疹,风湿痹痛,黄疸。《本经》:主头风,黄疸,咳逆,淋沥,女子阴中肿痛,湿痹死肌,不可屈伸、起止、行步。
土茯苓性味:甘淡,平。归经:肝经;胃经;脾经。功能:解毒,除湿,利关节。主治:治梅毒,淋浊,筋骨挛痛,脚气,疔疗疮,痈肿,瘰疬。
当归,其味甘而重,故专能补血,其气轻而辛,故又能行血,补中有动,行中有补,诚血中之气药,亦血中之圣药。大约佐之以补则补,故能养营养血,补气生精,安五脏,强形体,益神志,凡有形虚损之病,无所不宜。佐之以攻则通,故能祛痛通便,利筋骨,治拘挛、瘫痪、燥、涩等证。营虚而表不解者,佐以柴、葛、麻、桂等剂,大能散表卫热,而表不敛者,佐以大黄之类,又能固表。惟其气辛而动,故欲其静者当避之,性滑善行,大便不固者当避之。凡阴中火盛者,当归能动血,亦非所宜,阴中阳虚者,当归能养血,乃不可少。
黄芩有清热燥湿,凉血安胎,解毒功效。主治温热病、上呼吸道感染、肺热咳嗽、湿热黄胆、肺炎、痢疾、咳血、目赤、胎动不安、高血压、痈肿疖疮等症。黄芩的临床应用抗菌比黄连还好,而且不产生抗药性。我们借助广谱抗菌作用强的特点,用在真菌培养杂菌感染特厉害,用黄芩提取液效果很好。《本草经疏》:黄芩,其性清肃,所以除邪:味苦所以燥湿;阴寒所以胜热,故主诸热。诸热者,邪热与遍热也,黄疸、肠僻、泄痢,皆湿热胜之病也,析其本,则诸病自瘳矣。苦寒能除湿热,所以小肠利而水自逐,源清则流洁也。血闭者,实热在血分,即热人血室,令人经闭不通,湿热解,则荣气清而自行也。恶疮疽蚀者,血热则留结,而为痈肿渍烂也:火疡者,火气伤血也,凉血除热,则自愈也。黄芩有抗变态反应及抗炎作用:黄芩所含黄芩甙与黄芩素能明显抑制致敏豚鼠离体小肠与离体气管对抗原所产生的过敏性收缩反应,对豚鼠与小鼠的被动全身性变态反应以及豚鼠被动性皮肤变态反应,黄芩甙与黄芩素亦有抑制作用。黄芩有解热作用,多数试验证明,黄芩的有效成份黄芩甙元有解热作用。黄芩有解毒作用,黄芩甙有明显 的解毒作用,这可能是由于在β-葡萄糖醛酸甙酶的作用下,黄芩甙分解出葡萄糖醛酸,并与多种毒物结合所致。黄芩甙10毫克能提高士的宁的LD50达2.5倍,并且使四氯化碳中毒小鼠的肝糖原含量增多。
益母草味辛苦、凉。活血、祛淤、调经、消水。治疗妇女月经不调,胎漏难产,胞衣不下,产后血晕,瘀血腹痛,崩中漏下,尿血、泻血,痈肿疮疡。益母草碱性皮下注射有中枢抑制作用,MLD为0.4-0.6g/kg。兔静脉注射益母草碱1mg/kg,可见尿量显著增加。益母草碱在较高浓度时能使兔血悬液发生溶血作用。益母草碱水浸液用试管稀释法1∶13-1:10,对许兰氏毛菌,羊毛样小孢子菌、红色表皮癣菌、星状奴卡氏菌均有抑制作用。益母草煎剂用平板纸片法,对大肠杆菌、志贺氏痢疾杆菌有抑制作用。益母草能去淤生新,活血调经,含有硒、锰等多种微量元素,能抗氧化、防衰老,具有相当不错的益颜美容、抗衰防老功效。
黄花败酱草性味苦平,具有清热解毒、排脓破瘀的功效。能医肠痈、下痢、赤白带下、产后瘀滞腹痛、目赤肿痛。入胃、大肠、肝经。主治清热解毒,消痈排脓,活血行瘀。用于肠痈、肺痈及疮痈肿毒,实热瘀滞所致的胸腹疼痛,产后瘀滞腹痛等症。
凌霄花一种传统中药材,拉丁名Campsis grandiflora。具有行血去瘀、凉血祛风利湿,清热解毒,味辛酸,性微寒归肝、心包经、功能活血散瘀,凉血祛风。主治月经不调、经闭症瘕、产后乳肿、皮肤瘙痒.《本草衍义补遗》:凌霄花,治血中痛之要药也,且补阴捷甚,盖有守而独行,妇人方中多用何哉。《南宁市药物志》:有消炎解毒、止血止痛、排脓生肌之功,治红白痢。
鸡血藤性苦、甘,温。归肝、肾经,补血,活血,通络。用于月经不调,血虚萎黄,麻木瘫痪,风湿痹痛活血舒筋;养血调经,《本草纲目拾遗》载其能医手足麻木;肢体瘫痪;风湿痹痛;凌霄花,鸡血藤内服是治疗血淤不散,养血化淤活血的良药。
紫草性寒,味甘、咸。归心、肝经。主治凉血,活血,解毒透疹。用于血热毒盛、斑疹紫黑、麻疹不透、疮疡、真菌性皮肤病、水火烫伤。清热凉血,用于麻疹,热病癍疹,真菌性皮肤病,尿血,血淋,血痢,疮疡,解丹毒。《本草正义》载:紫草,气味苦寒,而色紫入血,故清理血分之热。古以治脏腑之热结,后人则专治痘疡,而兼疗癍疹,皆凉血清热之正旨。
千里光,又名九里明、九里光。拉丁名称Herba Senecionis Scandentis。是一种中草药,生于山坡、疏林下、林边、路旁、沟边草丛中。其性寒,味苦,具有清热解毒,明目,止痒等功效。多用于风热感冒、目赤肿痛、泄泻痢疾、皮肤湿疹疮疖。《本草拾遗》:主疫气,结黄,疟瘴,盅毒,煮服之吐下,亦捣敷疮、虫蛇犬等咬伤处。
紫花地丁苦、辛、寒,无毒;归心、肝经。功能主治:可入药,其性寒味微苦,清热解毒,凉血消肿。主治黄疸、痢疾、乳腺炎、目赤肿痛、咽炎;外敷治跌打损伤、痈肿、毒蛇咬伤等。紫花地丁多用于热毒壅盛之时, 不但可以配合备注中所说的银花、连翘、野菊花等药材同用,更可与败酱草和黄芪同用,不但加强紫花地丁的消肿、治痈等效力时还起到一个托毒的功效。可谓之一举两得啊。紫花地丁在试管内有抑制结核杆菌生长的作用。直接镜检法和试管培养法表明:紫花地丁(醇提物31mg/ml,水煎剂62mg/ml)对钩端螺旋体有抑制作用。紫花地丁水浸剂(1∶4)在试管内对堇色毛癣菌有抑制作用。此外,认为紫花地丁尚有清热、消肿、消炎等作用。
蒲公英植物体中含有蒲公英醇、蒲公英素、胆碱、有机酸、菊糖等多种健康营养成分,有利尿、缓泻、退黄疸、利胆等功效。蒲公英同时含有蛋白质、脂肪、碳水化合物、微量元素及维生素等,有丰富的营养价值,可生吃、炒食、做汤,是药食兼用的植物。蒲公英植物体中含有蒲公英醇、蒲公英素、胆碱、有机酸、菊糖等多种健康营养成分,有利尿、缓泻、退黄疸、利胆等功效。蒲公英同时含有蛋白质、脂肪、碳水化合物、微量元素及维生素等,有丰富的营养价值。其中含有的胡萝卜素和C及矿物质,对消化不良、便秘都有改善的作用。另外叶子还有改善湿疹、舒缓皮肤炎的功效,根则具有消炎作用,花朵煎成药汁可以去除雀斑。
艾叶,该品为菊科植物艾Artemisia argyi Levl.et Vant.的干燥叶。夏季花未开时采摘,除去杂质,晒干。主治吐血;衄血;吃力血;便血;崩漏;妊娠下血;月经不调;痛经;胎动不安;心腹冷痛;泄泻久痢;霍乱转筋;带下;湿疹;疥癣;痔疮;痈疡。散寒止痛,温经止血。用于少腹冷痛,经寒不调,宫冷不孕,吐血,衄血,崩漏经多,妊娠下血;外治皮肤瘙痒,脱皮。醋艾炭温经止血。用于虚寒性出血。《本草纲目》载其理气血,逐寒湿;温经,止血,安胎。治心腹冷痛,泄泻转筋,久痢,吐衄,下血,月经不调,崩漏,带下,胎动不安,痈疡,疥癣。
蛇床子性辛、苦,温;有小毒。归肾经。主治温肾壮阳,燥湿,祛风,杀虫。用于阳痿,宫冷,寒湿带下,湿痹腰痛;外治外阴湿疹,妇人阴痒;阴囊湿疹,是妇科外用良药。《本草经疏》载:蛇床子,味苦平;《别录》辛甘无毒;今详其气味,当必兼温燥,阳也。故主妇人阴中肿痛,男子阴痿湿痒,除痹气,利关节,恶疮。《别录》温中下气,令妇人子服热,男子阴强,令人有子。盖以舌能除湿,温能散寒,辛能润肾,甘能益脾,故能除妇人男子一切虚寒湿所生病。寒湿既除,则病去,性能益阳,故能已疾,而又有补益也
薄荷是中华常用中药之一。它是辛凉性发汗解热药,治流行性感冒、头疼、目赤、身热、咽喉、牙床肿痛等症。外用可治神经痛、皮肤瘙痒、皮疹和湿疹等。疏风、散热、辟秽、解毒、外感风热、头痛、咽喉肿痛、食滞气胀、口疮、牙痛、疮疥、瘾疹、温病初起、风疹瘙痒、肝郁气滞、胸闷胁痛。杀菌作用:薄荷脑有很强的杀菌作用,d-薄荷脑比1-薄荷脑的抑菌作用强。促进透皮吸收作用:国外研究表明,薄荷醇能显著促进醋氨酚透皮吸收作用,对曲安缩松有显著促进吸收作用,可使柴胡的生物利用度增加,能显著促进 扑热息痛的透皮吸收。这种促进透皮吸收作用可能是由于薄荷醇通过改变角质层脂质的结构改变药物的透皮渗透能力。
苍术燥湿健脾,祛风散寒,明目。用于脘腹胀满,泄泻水肿,脚气痿躄,风湿痹痛,风寒感冒,夜盲。苍术具有燥湿健脾,祛风湿的功效。主治湿阻中焦,风寒湿痹,脚膝肿痛,痿软无力,雀目夜盲。《珍珠囊》日:“能健胃安脾,诸湿肿非此不能除。”《本草纲目》曰:“治湿痰留饮......及脾湿下流,浊沥带下,滑泻肠风。”《新修本草》称其能“利小便”。
实施例1:本发明洗液的制作过程为:将百部1000g,黄柏1000g,苦参1000g,地肤子1000g,白藓皮1000g,当归1000g,千里光1000g,土茯苓1000g,凌霄花1000g,紫草1000g,益母草1000g,紫花地丁1000g,鸡血藤1000g,蛇床子1000g,黄花败酱草1000g,黄芩1000g,蒲公英1000g;先将所述原料药粉碎成粗粉,再进一步粉碎,过100目筛,使能通过100目筛的细粉量在达到30%,收集通过100目筛的细粉得细粉a;再将不能通过100目筛的粗粉投入多功能提取罐提取两次,第一次加8-10倍量乙醇煎煮1-2小时,第二次加4-8倍量水煎煮1-2小时,合并两次煎液,过滤得滤液;加热浓缩至糊状,再将薄荷1000g、白及1000g、苍术1000g和艾叶1000g蒸馏法汽化成白色结晶,干燥成粉末,加入制备的细粉a与粗颗粒提取液b,水浴搅拌均匀,混合成粘稠的液体,加入5%甲硝锉溶液中,搅拌均匀,紫外线杀菌消毒后装罐。
实施例2:本发明洗液的制作过程还可以为:将百部1000g,黄柏1000g,苦参1000g,地肤子1000g,白藓皮1000g,当归1000g,千里光1000g,土茯苓1000g,凌霄花1000g,紫草1000g,益母草1000g,紫花地丁1000g,鸡血藤1000g,蛇床子1000g,黄花败酱草1000g,黄芩1000g,蒲公英1000g,混合,切碎,浸泡。将将浸泡好的原料药加热回流提取2次,每次1~2小时,将2次提取液合并静置。再将上述水提取过的药渣加5~8倍量60~70%乙醇加热回流提取2次,每次1~2小时,将2次提取液合并静置。将上述两种提取液合并,减压浓缩相对密度为1.35(80℃)的滤液,回收乙醇,将所得滤液调整比重到1.06,减压得浸膏,将薄荷1000g,白及1000g、苍术1000g和艾叶1000g蒸馏法汽化成白色结晶粉末,再将5%甲硝锉溶液50℃~60℃加热融化,加入制备得到的浸膏和白色粉末,水浴搅拌均匀,混合成粘稠的液体,灌注,紫外线杀菌消毒后使用。
实施例3:本发明洗液的制备方法也可以为:百部1500g,黄柏1500g,苦参1300g,地肤子1400g,白藓皮1200g,当归1300g,千里光1400g,土茯苓1400g,凌霄花1400g,紫草1500g,益母草1500g,紫花地丁1400g,鸡血藤1300g,蛇床子1400g,黄花败酱草1400g,黄芩1300g,蒲公英1300g,加入5-10倍量的水浸泡1-2小时,提取2次;加热提取1-2小时,浓缩至浸膏状,成为组份1;剩余药渣加60-90%乙醇浸泡0.5-1.5小时,提取两次,每次1-2小时,合并提取液,滤过,浓缩,80-160目滤过,6000-10000转/分钟离心后的上清液,经截流分子量为5000-10000的超滤柱超滤,超滤液减压浓缩相对密 度为1.35(80℃)的浸膏,加热回流提取2次,每次30分钟~45分钟,提取活性成份,将2次提取液合并静置成浸膏状,作为组份2。再将薄荷1300g,白及1300g、苍术1200g和艾叶1400g蒸馏法汽化成白色结晶粉末,再将5%甲硝锉溶液50℃~60℃加热融化,加入制备得到的浸膏,水浴搅拌均匀,混合成粘稠的液体,灌注紫外线杀菌消毒后使用。
实施例4:取百部1300g,黄柏1400g,苦参1300g,地肤子1400g,白藓皮1200g,当归1300g,千里光1400g,土茯苓1400g,凌霄花1400g,紫草1500g,益母草1500g,紫花地丁1400g,鸡血藤1300g,蛇床子1400g,黄花败酱草1400g,黄芩1300g,蒲公英1300g,薄荷1300g,白及1300g、苍术1200g和艾叶1400g加水煎煮2次,合并煎液,去除沉渣,上清液加入乙醇并过滤,滤液回收乙醇至净,最后用纯化水调整药液体积至每毫升药液相当于1g原药材,药液为黄褐色至棕褐色液体,有明显薄荷香气。
治疗采取外洗或坐浴,每次20~30mL,每日2次。一般以10d为1疗程,2~3个疗程后观察疗效
毒理学试验:
试验例1:本发明急性毒性试验
一、试验材料:
动物:昆明种小鼠,体重21-24g,雌雄各半,山东大学生物试验室育种。药物:本发明洗液剂。
二、方法:
1、LD50计算:采用改良寇氏法,将小鼠随机分成5组,每组10只,雌雄各半,将洗液剂溶解,配成最大浓度,按小鼠最大允许容量给药,所给剂量按生药量依次为18,14.4,11.5,9.2,7.4(g.kg-1),在动物禁食(不禁水)18小时后,一日内分两次给药(间隔半小时),每次0.5ml,观察动物死亡情况。
2、最大耐受剂量测定(MTD值):取小鼠20只,雌雄各10只。将片剂加蒸馏水溶解,配成最高浓度,按动物的最大耐受量,以注射灌喂器能抽动为准。在动物禁食(不禁水)18小时后,一日内分两次给药(间隔半小时),每次0.5ml(每ml含生药0.36g),总药量为18g生药/kg.d,相当临床成人50Kg体重用量的300倍。给药后连续观察7天。
三、试验结果:
在LD50计算中当用最大允许浓度和最大允许容量给予小鼠时(18g/Kg.d),未见小鼠死亡,即未测出LD50,只可求最大耐受剂量,在7天观察期中,动物其食欲、活动、毛色、精神状态等皆正常,发育正常,未见有死亡。即选用相当于临床剂量的300倍药量,并无不良反应发生,表明急性毒性极小,MTD>18g/Kg.d。
试验例2:本发明急性皮肤刺激试验报告
1、实验目的:检测洗液剂对实验动物皮肤的刺激/腐蚀作用和强度
2、材料和方法:
2.1.受试物:名称:本发明洗液剂(实施例方法制得)
2.2.实验动物:壹级大耳白种家兔,10只,雌性,体重2.1-2.3kg;由青岛大学医学院生物系实验动物中心提供;
2.3.试验方法:
选用皮肤完好的健康家兔10只,于试验前24小时将其背部脊柱两侧被毛剪掉,去毛面积左、右各约4cm×4cm。试验时取受试物0.2ml涂于2.5cm×2.5cm的二层纱布上,敷贴于一侧去毛皮肤表面,油纸覆盖,胶布固定。另一侧皮肤作为空白对照。封闭4小时后,去除覆盖物,并用温水清洗去残留受试物,于去除受试物后的1、24和48小时观察涂抹部位皮肤反应,并评分。
3、实验结果:实验动物皮肤未出现任何红斑,红肿,发炎现象。
4、小结:根据《消毒技术规范》(1999)中皮肤刺激反应评分标准判定,本发明软膏剂对动物皮肤刺激强度属无刺激性。
试验例3:阴道粘膜刺激试验结果
1.试验方法:
实验动物检疫3天后,选用18只上述健康家兔,随机分为三组,每组各6只,分别为:空白对照、赋形剂对照、本发明洗液组(按应用液5倍浓度)。分别用适当大小的灭菌棉条浸以灭菌生理盐水和受试物置于赋形剂对照组和受试物组动物阴道内,与阴道粘膜接触6小时,24小时后处死动物。取出局部阴道组织,观察病变情况。
2、试验结果:阴道粘膜刺激试验结果
试验例4:皮肤过敏试验结果
用本发明洗液,进行诱导和激发接触试验,在24小时和48小时后观察试验小鼠,均不见皮肤有红斑及水肿等现象,与阴性对照组无差异。结果表明,该膏剂对皮肤反应强度近于零,即无皮肤变态反应。
试验例5:长期毒性实验资料
1、实验方法
将豚鼠随机份成4组,每组15只。在豚鼠背部脊柱两侧将脱毛剂均匀涂上,使其脱毛范围约40平方厘米。洗净脱毛剂,观察24小时后每组豚鼠分别涂本发明洗液0.2、0.4和0.8ml,分别含生药92、184和368mg,另一组涂溶媒0.8ml每日二次,连续30天,观察豚鼠的一般情况,实验结束后处死动物进行血液学、血液生化及病理学检查。
2、结果
上述三组用药豚鼠躯干去毛区,未见局部皮肤有水肿、充血、红斑、出血点及溃疡。用药组与对照组动物毛发色泽、摄食、四肢活动等对照组无明显差异,血液生化检查,用药组与对照组均在正常范围。病理组织学检查,实验各组心、肝、肾及局部皮肤均未见明显病变。提示,本发明中药洗液长期用药对局部皮肤及全身重要脏器均未见明显的毒性作用。
试验结果表明:本发明中药洗液实验安全。该药对乙型链球菌、金黄色葡萄球菌、大肠杆菌的最小抑制浓度为1∶150,淋球菌为1∶150,白色念珠菌为1∶300,对1∶20稀释度时的杀灭时间为5分钟,1∶50时为10分钟,1∶100时为20分钟,1∶200时为40分钟,1∶400时为8小时。
药效学试验
1.本发明药物对白念珠性阴道炎模型小鼠阴道的影响
实验动物:ICR小鼠70只,全部雌性,体重18g-22g。
试验药物:本发明洗液(实施例2所用方法制备);对比组:洁尔阴洗液,市售。
造模:取健康小鼠70只,雌性,体重18-22g,其中10只小鼠做为正常对照组,其余60只均皮下注射苯甲酸雌二醇5mg/kg,隔天一次,共3次,第6天小鼠阴道接种白色念珠菌,接种的菌液密度为9×108个/ml,每只20μL,每天一次,共三次,造模10天后,第11天观察造模结果,方法是用无菌的PBS(磷酸缓冲液)冲洗小鼠阴道,收集灌洗液,涂片,染色,镜检,主要在显微镜下找白色念珠菌的假菌丝,因为白色念珠菌的假菌丝才有定居阴道粘膜的能力,从而造成阴道炎症,造模结果较成功。在60只造模小鼠中挑选50只假菌丝较多的小鼠进行试验。分组给药:第一组正常对照组,灌胃去离子水10ml/kg,取50只造模成功的小鼠随机分为5组,每组10只,即为第二组阴道炎模型对照组,每天灌胃去离子水10ml/kg,第三组阳性对照组,每日妇炎洁洗液冲洗小鼠阴道22.5mg/kg(相当人用量9倍),第四-第六组为本发明药物大、中、小剂量组,分别每天冲洗小鼠阴道6.0g/kg、3.0g/kg、1.5g/kg(相当于临床人用量的18倍、9倍、4.5倍),给药体积均为10ml/kg,连续给药7天。
检测:末次给药8小时后,开始用无菌的PBS液冲洗小鼠阴道,每只20μL,收集灌洗液,其中10μL涂片、染色、镜检,观察孢子、假菌丝、假菌丝团形成情况并对其量进行分级,以判断感染严重程度。划分标准为:整张涂片未见假菌丝记为0,假菌丝量<10记为1,在10-30之间记为2,有一个假菌丝团记为3,假菌丝团>1记为4;剩余10μL加入1ml无菌PBS液中稀释, 再在稀释液中取50μL,接种于沙堡培养基内,30℃培养48小时,进行CFU计数(即菌落形成单位,以菌落个数/L)。每只小鼠阴道灌洗液取后,处死,取小鼠念珠菌感染阴道组织,经10%福尔马林固定,取材,常规石蜡包埋,连续切片,厚度5μm,HE染色,用LEICA生物显微镜进行观察。指标的划分标准:炎性浸润:+间质散在中性白细胞或淋巴细胞;++中性白细胞在血管周围明显增多;+++多数视野中有较多炎细胞,但无明显坏死及钙化;++++有明显的脓肿或钙化。菌丝:+阴道腔可见白色念珠菌菌丝。粘膜下组织水肿:+有轻度水肿;++水肿明显;+++水肿波及肌肉浅层;++++水肿波及肌肉深层。模型组的假菌丝量明显高于正常对照组(P<0.01),说明小鼠阴道炎模型成。与模型组比较,本发明药物6.0g/kg、3.0g/kg、1.5g/kg剂量及阳性药洁尔阴组,可使小鼠阴道灌洗液中的假菌丝显著减少(P<0.01,P<0.05)。
2.体外抗菌试验
培养基:肉汤培养基、肉汤琼脂培养基、血平板、巧克力平板;菌种:选用大肠杆菌、绿脓杆菌、金黄色葡萄球菌、白色念珠菌、变形杆菌、淋球菌、丙型链球菌、卡他球菌等菌种;方法:稀释法,将本发明洗液高、中、低剂量用肉汤自1∶5,1∶50至1∶100作倍比稀释,每管含药液2ml,第9管为无药对照,每管中加入稀释菌液1滴,37℃孵育18h,肉眼观察结果,含药最低浓度尚呈明显不混浊管为该药的最小抑菌浓度(MIC)。淋球菌在肉汤中生长不良,改用巧克力平板稀释法测最小抑菌浓度。如下表1:

结果表明本发明洗液均对大肠杆菌、绿脓杆菌、金黄色葡萄球菌、白色念珠菌、变形杆菌、卡他球菌、丙型链球菌的最小抑菌浓度为5mg/ml,淋球菌为10mg/ml。对大肠杆菌、绿脓杆菌、金黄色葡萄球菌、白色念珠菌、变形杆菌、卡他球菌、丙型链球菌、淋球菌均有明显的抑菌作用。
3.抗阴道霉菌试验
将培养基按比例配制,分装4瓶,分别装入等容生理盐水(空白对照组)、0.5%、1%、2%的本发明洗液,分装于无菌试验管内,每管约2ml。取新鲜的镜检证明霉菌阳性的标本0.2ml,加入上述7支不同成分的试管内,立即送入37℃温箱置24~48小时,取出后观察到液体混浊,无膜状物、极小沉淀,然后用无菌的长吸管吸取每根试管的上、中、下层观察结果。结果表明本发明洗液对霉菌有很强的杀灭作用。
4、本发明对组胺致豚鼠阴道瘙痒的影响
(1)、受试物及药品:(1)、供试品为本发明药物洗液,(2)、复方黄柏洗液;赣卫消证字(2004)第0237号(3)、妇炎洁洗液,市售。
(2)、动物:豚鼠,普通级,体重150~250g,山东大学生物实验动物室提供
取豚鼠50只,每只豚鼠砂纸擦伤阴道皮肤,面积约1cm2,随后动物随机分组:(1)空白对照组(10ml/kg,ig);(2)复方黄柏洗液(4mg/kg,ig);(3)妇炎洁组(1g/kg,ig);(4)本发明药物洗液剂低剂量组(2.3g/kg,ig);(5)本发明药物外用洗液高剂量组(4.5g/kg,ig)。30分钟后,在阴道创面滴0.01%组胺液,此后每隔3分钟依0.01%、0.02%、0.03%、0.04%......递增浓度,每次均为0.05ml/只,直致出现豚鼠回头舔阴道,以最后出现豚鼠回头舔阴道时所给予的组胺总量为致痒阈。

结果表明,本发明药物4.5g/kg剂量组可明显提高致痒阈,见表2。与空白对照组比较有显著性差异(P<0.01)。
通过上述药效学试验,证实了本发明洗液对大肠杆菌、绿脓杆菌、金黄色葡萄球菌、白色念珠菌、变形杆菌、卡他球菌、丙型链球菌、淋球菌均有明显的抑菌作用;对霉菌有杀灭作用,为本发明洗液针对妇科的阴道炎、宫颈炎、子宫内膜炎等阴道瘙痒、灼热肿痛、白带增多、尿频、尿急、尿痛等症状有效性提供了可靠的依据。
临床资料
1一般资料
所选38例均为我院妇科门诊患者,年龄22~55岁;病程2个月~3年,平均病程6个月;所有病例均表现为外阴瘙痒,甚则痒痛红肿,坐立不安,带下量多,色黄,气味腥臭,舌红、苔黄腻,脉弦数。
2治疗方法
予以本院中药制剂妇科洗液坐浴治疗。药物组成:百部13g,黄柏13g, 苦参14g,地肤子12g,白藓皮12g,当归11g,千里光13g,土茯苓13g,凌霄花12g,薄荷11g,紫草13g,白及12g,益母草13g,紫花地丁13g,鸡血藤14g,蛇床子14g,黄花败酱草13g,黄芩13g,艾叶12g,蒲公英12g,苍术13g。用法:诸药加水煎煮2次,合并煎液,去除沉渣,上清液加入乙醇并过滤,滤液回收乙醇至净,最后用纯化水调整药液体积至每毫升药液相当于1g原药材,药液为黄褐色至棕褐色液体,有明显薄荷香气。
治疗采取外洗或坐浴,每次20~30mL,每日2次。一般以10d为1疗程,2~3个疗程后观察疗效。
3治疗结果
38例中,显效(症状消失,随访半年无复发)25例,有效(症状消失,半年内有复发)7例,无效(症状较前减轻,但未消失,时重时轻)6例,总有效率84.2%。

外阴炎是妇科常见病,现代医学认为,本病可分为特异性与非特异性感染两种。特异性外阴炎以霉菌、滴虫感染为主,非特异性外阴炎以葡萄球菌、大肠杆菌、链球菌感染等为主,临床以非特异性感染比较多见,此外还可以继发于其他局部或全身疾病,如宫颈炎、阴道炎、宫颈癌等病,因分泌物增多的刺激,或经血、恶露过多过久的刺激,均可引发不同程度的炎症。其中部分患者原发病(如霉菌或滴虫性阴道炎)治愈后,外阴瘙痒症状仍不能缓解。目前西医尚无特效疗法。
本病可归属于中医学“阴痒”、“阴肿”等范畴。中医学认为外阴炎的急性阶段属于实证,主要是由于湿热下注外阴所致。即所谓:“妇人阴痒,多由于湿热所化。”妇女平时忽视卫生,或久居阴湿之地,以致湿邪侵入阴道,或情怀不畅,肝郁化火,脾虚生湿,湿热郁注下焦,均可发为阴痒。至于湿热的生成,又与肝脾二脏功能失常有关。因脾主水液代谢,脾之功能失常,水液代谢不能正常进行,就会导致水湿积聚,日久化热,形成湿热;肝主疏泄,对水液的运行具有调节作用,肝之疏泄功能失常就会形成肝气郁滞,不仅影响水液的运行而生湿,同时日久也能化热,形成湿热。治疗以清利湿热,杀虫止痒为主。我院中药制剂妇科洗液由蛇床子、苦参、百部、苍术、白鲜皮、地肤子、薄荷等组成,其中蛇床子杀虫止痒、温肾燥湿;苦参、白鲜皮、地肤子清热燥湿,杀虫利尿;百部解毒杀虫;苍术燥湿健脾;薄荷散郁火,消肿止痛。诸药合用,共奏清利湿热、杀虫止痒之效,故疗效满意。
具体实施例1:陈某,32岁,已婚,自述时常外阴瘙痒,可能其丈夫性器不洁,阴道灼热,白带异常,尿频、尿急,下身坠胀,经医生确诊为霉菌性阴道炎,反复发作,来我院就诊,使用本发明洗液,方用:百部13g,黄柏13g,苦参14g,地肤子12g,白藓皮12g,当归11g,千里光13g,土茯苓13g,凌霄花12g,薄荷12g,紫草13g,白及12g,益母草13g,紫花地丁13g,鸡血藤14g,蛇床子14g,黄花败酱草13g,黄芩13g,艾叶12g,蒲公英12g,苍术 12g,治疗采取外洗或坐浴,每次20~30mL,每日2次。以10d为1疗程。1个疗程后自述瘙痒消失,其丈夫也开始在夫妻生活前使用洗液,两个疗程后阴道无灼热感,白带正常,小便正常,至今未复发。
具体实施例2:韩某,34岁,来本院妇检时,症状有脓性白带、阴道粘膜红肿,伴有尿急、尿频、尿痛等,确诊为淋菌性阴道炎,盆腔炎、淋菌性阴道炎易侵犯子宫颈、子宫内膜、输卵管甚至腹腔,迁延反复难以根治。结婚4年未生育,使用本发明洗液:方用:百部13g,黄柏13g,苦参14g,地肤子14g,白藓皮12g,当归11g,千里光13g,土茯苓13g,凌霄花14g,薄荷13g,紫草13g,白及12g,益母草13g,紫花地丁13g,鸡血藤14g,蛇床子14g,黄花败酱草14g,黄芩13g,艾叶12g,蒲公英12g,苍术12g,按本发明实施例所述方法制成洗液。治疗采取外洗或坐浴,每次20~30mL,每日2次。10天后瘙痒消失、白带正常,14天后阴道无灼热感、小便正常,症状消失,白带正常,异味消失,瘙痒停止。巩固5个疗程,现已经成功受孕。随访3年无复发。

1.一种防治阴道炎症的妇科外用洗液,其特征在于,所述洗液包括以下重量份数的原料药:百部10~20份,黄柏10~20份,苦参10~20份,地肤子10~20份,白藓皮10~20份,当归10~20份,千里光10~20份,土茯苓10~20份,凌霄花10~20份,薄荷10~20份,紫草10~20份,白及10~20份,益母草10~20份,紫花地丁10~20份,鸡血藤10~20份,蛇床子10~20份,黄花败酱草10~20份,黄芩10~20份,艾叶10~20份,蒲公英10~20份,苍术10~20份。
2.根据权利要求1所述防治阴道炎症的妇科外用洗液,其特征在于,所述洗液中各种原料药的重量份数为:百部10~15份,黄柏10~15份,苦参10~15份,地肤子10~15份,白藓皮10~15份,当归10~15份,千里光10~15份,土茯苓10~15份,凌霄花10~15份,薄荷10~15份,紫草10~20份,白及10~20份,益母草10~20份,紫花地丁10~20份,鸡血藤10~20份,蛇床子10~20份,黄花败酱草10~20份,黄芩10~20份,艾叶10~20份,蒲公英10~20份,苍术10~20份。
3.根据权利要求1所述防治阴道炎症的妇科外用洗液,其特征在于,所述洗液中各种原料药的重量份数为:百部10~20份,黄柏10~20份,苦参10~20份,地肤子10~20份,白藓皮10~20份,当归10~20份,千里光10~20份,土茯苓10~20份,凌霄花10~20份,薄荷10~20份,紫草10~15份,白及10~15份,益母草10~15份,紫花地丁10~15份,鸡血藤10~15份,蛇床子10~15份,黄花败酱草10~15份,黄芩10~15份,艾叶10~15份,蒲公英10~15份,苍术10~15份。
4.一种如权利要求1~3中任一项所述防治阴道炎症的妇科外用洗液的制备方法,其特征在于,所述洗液的制备步骤包括:
a.将所述原料药除白及、薄荷、艾叶和苍术外放入粉碎机粉碎后过筛,
b.将过筛后剩下的颗粒物放入乙醇中加热回流提取2次;
c.再取上述乙醇提取后的药渣,加水加热回流提取2次;
d.将上述两种提取液合并浓缩;
e.将白及、薄荷、艾叶和苍术用蒸馏法汽化成白色结晶粉末;
f.将汽化的白色结晶粉末倒入浓缩液中混匀,加甲硝唑溶液搅拌均匀,紫外线消毒后罐装。
5.根据权利要求4所述妇科洗液的制备方法,其特征在于,所述步骤a中,将所述原料药除白及、薄荷、艾叶和苍术外放入粉碎机粉碎后过100目细筛,使能通过100目筛的细粉量在达到30%,收集通过100目筛的原料细粉备用。
6.根据权利要求4所述妇科洗液的制备方法,其特征在于,所述步骤b中,将过筛后剩下的粗颗粒物放入乙醇中加热回流提取2次,每次1~2小时,将2次提取液合并静置。
7.根据权利要求4所述妇科洗液的制备方法,其特征在于,所述步骤c中,将上述乙醇提取过的药渣加水加热回流提取2次,每次1~2小时,将2次提取液合并静置。
8.根据权利要求4所述妇科洗液的制备方法,其特征在于,所述步骤d中,将上述两种提取液合并,减压浓缩相对密度为80℃时1.37的滤液,回收乙醇,将所得滤液调整比重到1.60,加入过筛的细粉后搅匀。
9.根据权利要求4所述妇科洗液的制备方法,其特征在于,所述步骤e中,将白及、薄荷、艾叶和苍术用蒸馏法汽化成白色结晶,干燥后成粉末状,收集备用。
10.根据权利要求4所述妇科洗液的制备方法,其特征在于,所述步骤f中,将浓缩的药液加上汽化的白色结晶粉末混匀,倒入0.5%甲硝唑溶液中搅拌均匀,用紫外线消毒后罐装。
