Abstract
本发明提供了用于癌抗原肽疫苗和病毒抗原肽的佐剂,其含有百日咳疫苗作为主要组分。本发明还提供了用于癌症或病毒感染性疾病的治疗剂以及用于癌症转移或复发或病毒诱导的肿瘤发生的预防剂,其含有癌抗原肽或病毒抗原肽和百日咳疫苗。可以被适当地应用的百日咳疫苗是全细胞体百日咳疫苗。本发明的试剂可以被安全地以多个剂量给药。
Description

技术领域
本发明涉及帮助治疗肿瘤和病毒感染性疾病以及预防癌症转移和复发及病毒诱导的肿瘤发生的疫苗,以及涉及用于制备该疫苗的佐剂。
背景技术
已经进行了为了试验目的的应用HLAI类结合肽对肿瘤的肽免疫治疗。然而,从目前获得的肽免疫治疗结果判断,预期单独给药肿瘤抗原肽并不那么有效(就所涉及的过去的报告而言,没有一例在RECIST评价中SD的比值超过10%)(非专利参考文献1、3)。
因此,为了刺激细胞毒性的T淋巴细胞(CTL),广泛地使用了一种方法,其中给药被弗氏不完全佐剂(FIA)乳化的肽(非专利参考文献1-3)。如从MHC四聚体阳性CD8T细胞或分泌IFN-γ的细胞的增加所证实的那样,悬浮于FIA中的肽允许肽特异性的T细胞增殖。然而,已经增殖的CD8T细胞不太可能作为效应子被完全地活化;它们的治疗效果受到了限制(非专利参考文献4-6)。在一些情况下,悬浮在FIA中的肽甚至能够引起抗原特异性的免疫耐受(非专利参考文献7)。存在使患者的QOL显著恶化的另一问题,因为FIA在皮肤下长期存在,并且还因为丘疹持续约2年,并且当给药次数增加时皮肤硬化进展,尽管在1-2个月内发红会消失。
为了增加肽免疫原性,在临床研究中测试了许多佐剂。应用这些佐剂的目的是通过活化抗原呈递细胞(APC)和/或辅助T细胞(Th)从而提供具有抗炎性环境的CTL。将树状细胞用作肽的抗原呈递细胞,以诱导更好的肿瘤控制(非专利参考文献3、8)。已经引入了非甲基化的去氧-CpG、Toll样受体配体(非专利文献9)和活化APC的配体,例如Flt3(非专利文献10)。就辅助细胞活性而言,Th1型反应诱导最佳的细胞免疫(非专利文献11、12)。由Th细胞和其它免疫增强细胞分泌的重组细胞因子被用于临床研究中。这些细胞因子包括IL-2(非专利文献13)、GM-CSF(非专利文献8)、IFN-α(非专利文献14)和IL-12(非专利文献15)。由于可获得GMP(GoodManufacturingPractice)级别的商业化产品,细胞因子允许容易地进行试验治疗。然而,这些只能替代一些免疫反应,并且主要只刺激APC或Th细胞。此外,细胞因子的生物半衰期是很有限的。
为了在更自然的条件下活化APC和Th细胞两者,开发了结核杆菌细胞壁骨架(BCG-CWS)的用途(非专利参考文献16)。然而,BCG-CWS有效地活化细胞免疫,因此无法避免开口性溃疡的发生。因为这一原因,BCG-CWS不能在免疫力降低的患者,例如白血病患者中使用。BCG-CWS也难以在重复接种中使用。由于肽易于被血清中的蛋白酶分解,除了与细胞表面上的MHC分子结合的那些外,将在数天内消失,因此需要频繁地免疫。基本上应答原本为免疫耐受对象的肿瘤自身抗原蛋白质的T细胞很可能丧失活性;其细胞毒活性会变弱,除非每周给药一次。因此,由于难以重复地给药,作为肿瘤免疫治疗的佐剂,BCG-CWS是有严重缺陷的。

发明内容
本发明要解决的问题
本发明的一个目的是提供用于癌抗原肽或病毒抗原肽疫苗的佐剂,其中该佐剂在肿瘤特异性免疫治疗或病毒感染性疾病的免疫治疗中有效,并且因此提供在免疫治疗中有效的用于癌症或病毒感染性疾病的治疗剂。
解决问题的手段
就上述问题,本申请人深入地进行了研究,并且发现全细胞百日咳疫苗活化抗原呈递细胞(APS)和辅助T细胞(Th)两者,并且包含肿瘤抗原肽或病毒抗原肽以及作为佐剂的全细胞百日咳疫苗的疫苗显示出出色的抗肿瘤活性或抗病毒活性。此外,本申请人确认,与BCG-CWS不同,百日咳疫苗可以以一周的间隔被重复地给药,因为该疫苗不需要如FIA那样在使用前乳化,并且几乎不产生发红和肿胀,并且开发了本发明。
因此,本发明提供了下列:

本发明的效果
本发明的用于癌症或病毒感染性疾病的治疗剂和用于癌症转移或复发或病毒诱导的恶性肿瘤发生的预防剂包含癌抗原肽或病毒肽以及百日咳疫苗。在给药癌抗原肽和病毒肽中,百日咳疫苗显示出出色的佐剂活性。百日咳疫苗可以就其作为疫苗本身被广泛地应用,并且可获得GMP级别的疫苗。因此,本发明提供了有效且安全的用于癌症和病毒感染性疾病的治疗剂。

[图4]显示在用有或没有百日咳疫苗的OVA-II肽免疫的DO11.10小鼠中产生的Th1/Th2细胞因子的图。在第三次每周一次的免疫一周之后,收集脾细胞,用OVA-II肽激发,并检查细胞因子的产生。应用Luminex100和FluorokineMAP试剂盒测量细胞因子IFN-γ、IL-4和IL-5。此处显示了在用抗原刺激后,在孵育48小时后,在培养基上清液中细胞因子的浓度。Ac疫苗诱导了较大量的Th2型细胞因子(IL-4和IL-5)的产生,而Wc疫苗促进了较大量的Th1型细胞因子(IFN-γ)的产生并倾向于产生较小量的Th2细胞因子。
[图5]显示了在用有或没有百日咳疫苗的OVA-I肽免疫的小鼠中体内肿瘤增殖的图。在第三次每周一次的免疫一周之后,将EG7和EL4细胞接种在B6小鼠背部共同的相反侧皮内。每两天测量肿瘤大小,表示为大直径和与其垂直的小直径的乘积。数据显示为5只小鼠的平均值。

具体实施方式
本发明提供了用于癌(或病毒感染性疾病)的治疗剂和用于癌转移和复发(或病毒诱导的恶性肿瘤发生)的预防剂,所述试剂包含癌抗原肽(或衍生自引起病毒感染性疾病的病毒的肽)和百日咳疫苗(下文也称为“本发明的试剂”)。在本文中提到“治疗”时,应当理解为包括“减轻症状”和“抑制进展”。
在本发明中,“癌抗原肽”是指具有诱导免疫的免疫反应例如抗体产生和细胞活性的蛋白质(肽);具体而言,尤其优选具有T细胞诱导活性以刺激细胞毒性的T淋巴细胞的蛋白质(肽)。对癌抗原肽没有特别的限制,只要其为具有T细胞诱导活性的蛋白质(肽)即可;例如,可提及HER-2/neu、MART-1、NY-ESO-1、Gp-100、MUC-1、p53、前列腺特异性抗原(PSA)、hTERT、WT1、存活蛋白、CEA、MAGE等,癌抗原肽优选为WT1、存活蛋白、PSA等,更优选为WT1。具体而言,作为WT1抗原肽,可优选使用在WO00/06602、WO00/26249、WO2006/030770等中描述的那些。作为存活蛋白抗原肽,优选使用描述于2006/080142等中的那些。
同时,在本发明中,“病毒肽”是指衍生自病毒的蛋白质或包含其部分氨基酸序列的肽,所述病毒通过感染包括人类在内的哺乳动物而引起一些疾病或病症;其实例包括,但不限于,丙型肝炎病毒、乙型肝炎病毒、流感病毒、登革病毒、成年人T细胞病毒、人类免疫缺陷病毒、乳头瘤病毒等。在优选的实施方案中,病毒肽是衍生自与肿瘤相关的(致癌的)病毒的抗原肽。在本文中,“与肿瘤相关的肽”是指衍生自一组与恶性肿瘤发生密切相关的病毒(包括丙型肝炎病毒)的蛋白质;或包含其氨基酸序列一部分的肽。具体地,作为丙型肝炎病毒肽,优选应用在WO2005/105993等中描述的那些。
只要其拥有针对抗原的免疫原性,癌抗原肽可以是癌抗原肽或病毒抗原肽的片段。
例如可通过Sambrook等(1989分子克隆:试验手册,第二版ColdSpringHarborLab.,ColdSpringHarbor,NewYork.)描述的方法来制备癌抗原肽。具体而言,例如可通过如下方法制备癌抗原肽:培养掺入了表达载体的转化体以产生癌抗原蛋白质(肽),所述表达载体包含编码癌抗原蛋白质(肽)的核酸;以及从获得的培养物中分离并纯化该癌抗原蛋白质(肽)。还可通过公知的肽合成方法来制备抗原蛋白质(肽)。肽合成方法可以是,例如,固相合成方法或液相合成方法。
作为“百日咳疫苗”,可以提及:包含甲醛处理的或热失活的百日咳杆菌(Bordetellapertussis)细胞壁部分的全细胞百日咳疫苗、包含从百日咳杆菌半纯化或纯化的组分(例如,百日咳毒素、纤维血细胞凝集素等)的无细胞百日咳疫苗等。优选地,该百日咳疫苗是全细胞百日咳疫苗,但取决于受试患者和条件,无细胞百日咳疫苗有时是有用的。
至于如何制备百日咳疫苗,可以无限制地使用公知的方法。可以应用百日咳疫苗的商业产品,其例如可从BioFarma(Indonesia)等获得。
在本发明的试剂中含有的癌抗原肽(或病毒肽)与百日咳疫苗的比例依据抗原的选择和百日咳疫苗的性质而不同,并且不能一概而论;例如,当使用WT1抗原和全细胞百日咳疫苗时,在用于成年人的单剂量中,作为实例性的比例,可以提及0.1-5mg肽:1×108-1×109个全细胞百日咳疫苗细胞,并且该比值优选为0.5-4mg肽:1-7×108个全细胞百日咳疫苗细胞。
本发明的试剂还可以包含可药用载体。作为可药用载体,可以没有限制地应用对于疫苗产品常用的任何载体;具体而言,可以提及:盐水、缓冲盐水、葡萄糖、水、甘油、等渗的水性缓冲溶液及其组合。该载体优选为消毒的载体。合适时,用乳化剂、防腐剂(例如,硫柳汞)、等渗剂、pH调节剂和减活化剂(例如,甲醛)等对其进行配制。
对给药本发明试剂的受试者没有特别地限制,只要其为患有癌症的动物,其中所述癌症高度地表达从其中衍生活性组分癌抗原肽的蛋白;或感染了从其中衍生病毒肽的病毒;例如,可以提及包括人在内的哺乳动物、鸟等。
本发明的试剂优选具有适于疫苗给药模式的形式;例如,可注射的形式,包括溶液、悬浮液或乳液。备选地,作为可以被处理以得到流体溶液、悬浮液或乳液的形式,可以提及诸如冻干制备物的固体形式。
可应用作为活性组分的前述癌抗原肽或病毒肽和作为佐剂组分的百日咳疫苗与前述载体一起通过常规方法制备本发明的试剂。对于成年人的单剂量,可以以0.1-5mg肽和1×108-1×109个全细胞百日咳疫苗细胞,优选为0.5-4mg肽和1-7×108个全细胞百日咳疫苗细胞将癌抗原肽或病毒肽和百日咳疫苗包含在该试剂中。
作为优选的制备实例,可以提及这样的方法:其中将单剂量的肽(例如,3mg)溶解于300μl溶液(例如,5%葡萄糖溶液)中,并且就在使用前,将其与悬浮于100μl悬浮剂(例如,生理盐水)中的5×108个全细胞百日咳疫苗细胞混合,以得到总共400μl的疫苗溶液,但这一方法不能被解释为是限制性的。
对给药本发明试剂的途径没有特别的限制,但是优选为局部给药。例如,可以提及肌肉内、皮下、皮内、腹膜内和其它给药途径。皮肤、鼻内和口内给药也是可能的,作为给药方法,可以提及喷雾、涂布等。
根据受试者年龄、性别、体重、药物耐受性等确定给药的量;对于成年人的单剂量,通常可一次、两次或多次给药0.1-5mg肽和1×108-1×109个全细胞百日咳疫苗的细胞,优选0.5-4mg肽和1-7×108个全细胞百日咳疫苗的细胞。优选地,给药多个剂量;在这种情况下,期望以1-4周、优选1-2周、更优选1周的间隔给药本发明的试剂3次或更多次、优选6次或更多次、优选10次或更多次、以及甚至更优选12次或更多次。在多剂量给药中,可以在上述范围内适当地改变肽和/或百日咳的单剂量。通常,采用这样的剂量方案:其中为了安全测试的目的,首先测试低剂量若干次,并且在证实了安全性后,增加剂量。在与百日咳疫苗一起给药之前,可以进行单独给药所述肽的安全性测试。此外,如果在给药百日咳疫苗后观察到诸如发红等有害作用,期望在该时间之后使用转为暂时单独给药肽的剂量方案,或停止使用。
在给药中,以这样的方式给药试剂是重要的,其中所述方法在对患者的负担尽可能低的范围内使得肽能够到达尽可能多的淋巴结。由于与抗原呈递细胞例如树突细胞的MHC分子结合的每个细胞的肽量、以及呈递肽并迁移至淋巴结的树突细胞总量是很重要的,因此改变部位以允许该肽到达一些淋巴结,而不是在一个部位给药大量的试剂,是更有效的。然而,由于经验地猜测,如果在宽表面积的部分中以高密度存在相同抗原的条件下进行免疫,其很可能诱导I型过敏反应(立即过敏:荨麻疹、过敏性休克和哮喘等),通常,在接近每一四肢根部淋巴结附近的1-2处,总共4-8处给药试剂。
优选的给药模式的实例包括,但不限于这样的方法:其中通过皮内注射往接近两个腋窝和两个腹股沟区域的淋巴结的共4处每处给药100μl,等等,只要可以达到上述目的,可以进行任意修改。例如,对于头颈部肿瘤患者,可以在颈根部左侧和右侧的2处(为了将抗原递送至锁骨下淋巴结)以代替腹股沟区域,以及靠近每一腋窝的1处,其四处给药试剂。就皮下注射而言,在一处超过100μl的注射量可能会有微痛,并且可能导致在注射后从针孔倒流及溢出;因此,优选一处的剂量体积为100μl或更少。
百日咳疫苗的特征在于比公知的常规肽免疫治疗佐剂更低的由于给药的副作用流行,诸如发红、肿胀、溃疡和硬化(产生约5-20mm的红斑,然而,其在约48小时内基本上消失)。特别地,在癌患者中,担心溃疡会导致致命的结果,因此这一特点使得本发明具有有利的效果。由于本发明的试剂不需要如FIA那样在使用前乳化,其给药不会产生持久的发红或丘疹。此外,由于该试剂不会产生溃疡(使用BCG-CWS时总会产生溃疡),一周一次的多剂量给药是可能的,因此可以将由于停药导致的症状加重的风险最小化。
在特别优选的实施方案中,本发明的试剂包含作为癌抗原肽的衍生自WT1蛋白的肽。在约70%的自发发展的恶性肿瘤中高度地表达WT1蛋白,并且其特别能用于治疗表达高水平肿瘤抗原的恶性实体瘤,诸如肾癌、肺癌、食道癌和其它消化系统癌、卵巢癌、乳腺癌、脑肿瘤和肉瘤、白血病和其它血液恶性肿瘤患者。在另一优选的实施方案中,本发明的试剂包含作为病毒肽的衍生自丙型肝炎病毒的抗原肽,并且能用于治疗丙型肝炎和预防丙型肝炎病毒诱导的恶性肿瘤(肝癌等)。
本发明还提供了由1或2或更多个容器组成的试剂盒,所述容器含有本发明试剂的1种或2种或更多种组分。
应用本发明的试剂和试剂盒,能够治疗癌症或病毒感染性疾病或减轻其症状。因此,本发明提供了治疗癌症或病毒感染性疾病的方法,其包括给受试者给药免疫敏化有效量的本发明的试剂。
下文将参照以下实施例详细描述本发明,然而,本发明决不限制于这些实施例。
实施例
(材料和方法)
细胞和抗体
卵清蛋白转染的EL4细胞系E.G7-OVA(Moore,M.W.等,(1988),Cell54:777-785)由MichaelBevan博士通过HermanN.Eisen博士赠与。红白血病细胞系(H-2b)FBL3由MasaakiMiyazawa博士提供,这得到了BruceChesebro博士的好意允许。
小鼠
OT1转基因小鼠(OT-1tg)表达对卵清蛋白肽257-264特异的TCR对,所述肽与H-2Kb分子结合(Hogquist,K.A.等,(1994),Cell76:17-27),该小鼠由F.Carbone博士好意提供。使OT-1tg与由M.Okabe博士提供的EGFP转基因小鼠(Okabe,M.等(1997),FEBSLetters407:313-319)交配。使该两个转基因小鼠系与C57BL/6(B6)背景反复回交。B6小鼠购自SLC(Shizuoka,日本),并且在Kochi大学医学院的SPF场所繁殖。使由D.Y.Loh博士提供的DO11.10转基因小鼠与BALB/c交配,并且维持作为就编码对与I-Ad(卵清蛋白323-339)结合的OVA-II肽特异的TCR对而言的纯合子(Murphy,K.M.等,(1990),Science250:1720-1723)。
肽
通过Fmoc方法手工合成OVA-I(SIINFEKL(SEQIDNO:1))肽和Db126(RMFPNAPYL(SEQIDNO:2))肽。每一肽均通过反相HPLC被纯化为>95%的纯度,应用MALDITOF质量分析器(VoyagerDE-RP,AppliedBiosystems,Fostercity,CA)确定其分子量。应用MicroBCA分析(Pierce,RockfordIL)肽浓度。
佐剂
百日咳疫苗是从百日咳杆菌TohamaI期菌株细胞制备的,并且由Osaka大学(Bikenkai,Kannonji,日本)的微生物疾病研究会提供。无细胞的疫苗由失活的百日咳毒素和纤维血细胞凝集素组成,并且将其与磷酸铝混合。全细胞疫苗包含悬浮在PBS中的失活的细菌细胞。
在用Ac或Wc百日咳疫苗免疫的小鼠中Ig-G抗体的产生。
用5μg吸附至磷酸铝的无细胞(Ac)疫苗或2×109个失活全细胞(Wc)皮下免疫B6小鼠,每一种疫苗均在弗氏不完全佐剂(FIA)中被乳化。在第一、第二和第三次每周一次的免疫一周后,杀死小鼠,收集血清用于ELISA分析。
ELISA
用20μg/ml浓度的、没有铝盐的Ac疫苗包被平底96孔板(SUMILON,Akita,日本),并且用PBS中的5%脱脂奶封闭。从1/10稀释比系列稀释每一小鼠血清,并在4℃温育1小时。将HRP-标记的山羊抗-小鼠IgG(γ链特异的)抗体(Zymed,SouthSanFrancisco,CA)用作二抗,在4℃温育血清30分钟。添加OPD底物,并且在490nm处测量吸光度。
体内CTL增殖试验
静脉内注射衍生自OT1tg的10,000,000个脾细胞。次日,用50nmole的OVA-I肽在背部皮肤中免疫每一小鼠。作为佐剂添加的百日咳疫苗是10μg/小鼠的吸附至磷酸铝的Ac疫苗,或2×107个失活的全细菌细胞体。在下一周,用相同的抗原加强免疫该小鼠。在第二次免疫一周后,杀死小鼠,并通过流式细胞术分析脾细胞。为了鉴定转基因TCR,应用了藻红蛋白(PE)标记的单克隆抗-Vβ5-特异性抗体MR9-4(Kanagawa,O.等,(1991),J.Immunol.147:1307-1314)。
用MHCI类结合肽免疫,用于细胞毒性分析
用或不用佐剂,应用溶解于PBS中的50nmole的OVA-I肽或100nmole的Db126肽(Udaka,K.等,(2000),JImmunol164:1873-1880),添加5μg无细胞疫苗或衍生自全细胞疫苗的1×107个失活的细菌细胞,免疫B6小鼠。将免疫原皮内注射至背部皮肤的两个部位。将每周一次的肽免疫重复4次。
细胞毒性分析
如上所述在B6小鼠中进行4次每周一次的免疫。在最后一次免疫一周之后,在体外用EG7细胞刺激脾细胞。简而言之,将来自一个脾的脾细胞悬浮于含有5%大鼠ConA上清液的10%FCSDMEM中(32ml)。用来自137Cs源的60Gy辐射EG7细胞,并且以1×105细胞/ml的密度添加至脾细胞。在刺激后的第五天,以不同的E/T比值,通过对10,000个EG7细胞5个小时的51Cr释放分析测量细胞毒活性。特异性裂解%是(试验性释放-自发释放)/(总释放-自发释放)×100的结果。
细胞因子分析
应用Luminex100(LuminexCorporation,Austin,TX)和FluorokineMAP小鼠试剂盒(R&DSystemsInc.,Minneapolis,MN)测量Th1/Th2细胞因子的分泌。以每只小鼠100nmole的在PBS中的有或没有佐剂的OVA-II肽,在背部皮肤的两个部位皮内注射DO11.10小鼠。应用的佐剂是10μg/小鼠的吸附至磷酸铝的Ac疫苗或2×107个细胞/小鼠的Wc疫苗。在第三次每周一次的免疫一周之后,杀死小鼠,用OVA-II肽刺激脾细胞。简而言之,用在含有DMEM(200μl/孔)的10%PBS中的OVA-II系列稀释液(200μl/孔)刺激2×106DO11.10脾细胞/孔。应用针对IFN-γ、IL-4和IL-5的Fluorokine试剂盒量化在37℃温育48小时期间分泌在上清液中的细胞因子。通过存在于该分析中的KJ1.26+、CD4+T细胞(保留DO11.10转基因TCR的细胞)数校正细胞因子浓度(Haskins,K.等,(1983),J.Exp.Med.157:1149-1169)。
肿瘤接种试验
将3,000,000个EL4细胞和4,000,000个EG7在B6小鼠的经剃毛的背部共同相对的侧面皮内接种至每一B6小鼠。在接种7天后开始每两天测量共同垂直的最长的直径。预先确定肿瘤细胞的数目,并且该两个细胞系具有大约相同的肿瘤生长速率。通过每周一次皮内注射50nmole溶于PBS中的、有或没有佐剂(5μg吸附至磷酸铝的Ac或1×107个失活的百日咳全细胞体)的OVA-I对每一组的5只小鼠免疫3次。将除了肿瘤接种部位之外的背部皮肤用于免疫。在第三次免疫一周后,接种肿瘤细胞。再进行两次每周一次的免疫,在肿瘤接种后一周开始。为了检查用WT1肽免疫的效果,在肿瘤攻击试验中应用了FBL3红白血病细胞(Udaka,K.等,(2000),JImmunol164:1873-1880)。通过皮内注射100nmole的Db126肽(RMFPNAPYL(SEQIDNO:2),(Udaka,K.等,(2000),JImmunol164:1873-1880)),有或没有佐剂,每周一次地免疫小鼠。在第三次免疫一周后,腹膜内注射5,000,000个FBL3细胞,并且检查存活天数。在肿瘤接种一周后,再次开始一周一次的肽免疫。
用Wc疫苗和Db126肽进行的长期免疫
用1×109个失活的百日咳全细胞体(是在CTL诱导和肿瘤接种试验中使用量的100倍)、在有或没有50nmole的Db126肽的情况下,一周一次地皮内免疫每一只B6小鼠。在第九次免疫一周后,收集尿液,杀死小鼠,并收集血液和组织(肺、肾、卵巢、睾丸、骨髓)。在10%甲醛中固定组织,并用苏木精和曙红(Haematoxylin&Eosin)染色。将从腋窝大血管和腔静脉中收集的血液立即在含有10mMEDTA的PBS中稀释10倍,并且在合适的稀释后,应用血细胞计数室计数红细胞(RBC)。在Turk溶液中稀释10倍以裂解RBC后,以相同的方式计数白细胞。应用Micro-TP测试试剂盒(WakoPureChemicalIndustries,Osaka)测量尿液总蛋白含量。
(结果)
百日咳疫苗的佐剂活性
为了检查百日咳疫苗的佐剂活性,应用了包含卵清蛋白(OVA)作为内源肿瘤抗原的模型抗原系统(Moore,M.W.等,(1988),Cell54:777-785,Hogquist,K.A.等,(1994),Cell76:17-27)。已知Kb-结合的OVA-I(SIINFEKL(SEQIDNO:1))肽是H-2b小鼠中的主要CTL表位(Moore,M.W.等,(1988),Cell54:777-785)。首先,进行了测试以确定是否能通过添加百日咳疫苗增强OVA-I肽免疫诱导的CTL。首先,在CTL诱导试验之前,测试了百日咳疫苗的诱导辅助细胞的活性。通过测试针对PT和FHA混合物的IgG抗体的产生,因此主要测试Th2偏位活性,检查了该辅助细胞活性。如图1所示,在用Ac疫苗免疫3周之内,非常好地产生了IgG抗体。Wc疫苗也诱导IgG抗体的产生,但是产量比应用Ac疫苗获得的产量小。因此,其显示Ac疫苗和Wc疫苗均能够刺激Th细胞活性。
然后,用OVA-I肽和Ac疫苗或Wc疫苗一起免疫C57BL/6(B6)小鼠。图2显示了应用通过皮内注射肽(有或没有百日咳疫苗)免疫4次的脾细胞作为效应子的细胞毒性测试结果。将OVA-转化的细胞E.G7-OVA(EG7)(Moore,M.W.等,(1988),Cell54:777-785)及其母细胞系EL4用作靶。当单独应用预期缺乏辅助细胞活性的OVA-I肽时,仅诱导了低细胞毒活性。偶尔,能观察到OVA-特异性裂解(在图2c中5只小鼠的1只,对于重复试验,平均值为10-20%)。这使得想起偶尔在临床研究中发现的喜欢应答治疗的抗肿瘤效果的患者。平均几至10%的接受了单独的WT1肽或以在弗氏不完全佐剂(FIA)中的乳剂形式的WT1肽(与本发明的肽不同的肽)的癌症患者显示出证实肿瘤抑制的临床反应(Oka,Y.等,(2004),Proc.Natl.Acad.Sci.USA101:13885-13890,Rosenberg,S.等,(1998),Nat.Med.4:321-327)。这些偶然的反应可能是具有在之前感染等之下,在免疫部位具有促进炎症的环境的那些,并且偶然地刺激其CTL。当添加能促进Th2反应的Ac疫苗时,细胞毒活性升高。同时,Wc疫苗在所有小鼠中均诱导非常强的细胞毒活性。以非常高的重复性观察到用Wc疫苗时CTL活性的很强的诱导。Wc免疫的小鼠的反应是抗原特异性的,并且没有观察到针对EL4的细胞毒性。因此,发现所述强细胞毒活性不是由于如同LAK细胞的非特异性细胞毒活性,所述LAK细胞是由大量细胞因子诱导的。在用Ac和Wc免疫的小鼠的脾细胞中,特别是在具有针对EG7的高细胞毒活性的细胞系中,CD8T细胞(CTL)比例倾向于升高(数据未显示)。因此,然后,检查了小鼠身体中抗原特异性CD8T细胞的增殖度。
抗原特异性CD8T细胞的体内增殖
为了监测肿瘤抗原特异性T细胞的体内增殖,将与表达EGFP(在鸡β-肌动蛋白启动子和巨细胞病毒增强子的控制下)的转基因小鼠交配的OVA-I-特异性TCR转基因小鼠(OT1)的脾细胞接种至正常小鼠。在这些EGFP转基因小鼠中,并非所有的T细胞均表达EGFP,尽管其原因是不清楚的(Okabe,M.等,(1997),FEBSLetters407:313-319)。在脾中的CD8T细胞中,80±10(SD)%的细胞是EGFP表达阳性的。应用单克隆抗-Vβ5-特异性抗体MR9-4(Kanagawa,O.等,(1991),J.Immunol.147:1307-1314)鉴定转基因TCR。如图3中所示,在静脉内注射15天后,脾内的OT1-TCR+、EGFP+CD8T细胞几乎减少至检测的极限。然而,当用OVA-I肽单独皮内免疫2次后,转基因OT1-TCR+、EGFP+、CD8T细胞增殖至可检测水平。当将百日咳疫苗添加至OVA-I肽时,OT1细胞展现出进一步增殖。有趣的是,高重复性地发现,采用Ac疫苗的OT1细胞的增殖比通过Wc的增殖要稍强。从这一结果判断,Ac拥有增加抗原特异性T细胞数的活性,然而,如图2所示,其细胞毒活性没有如此强。相反,Wc疫苗在诱导强细胞毒性方面非常有效。为了产生细胞毒活性,有必要使细胞进行新蛋白质合成以产生作为细胞毒颗粒的成分的穿孔素和粒酶。由Wc疫苗诱导的免疫刺激环境可能在这一蛋白质合成诱导中起着作用。
用百日咳疫苗刺激的CD4T细胞的Th1/Th2细胞因子谱
接下来,尝试确定作为佐剂添加的百日咳疫苗是否会影响由用与百日咳杆菌不相关的异源肽刺激的T细胞分泌的细胞因子谱,所述T细胞存在于百日咳杆菌附近。对于模型抗原系统,用DO11.10TCR转基因(DO11.10tg)CD4T细胞作为响应子。DO11.10TCR对与I-Ad分子结合的OVA-II肽(323-339,ISQAVHAAHAEINEAGR(SEQIDNO:3)是特异的(Hunt,D.F.等,(1992),Science256:1817-1820))。如在材料和方法中所描述的那样,用有或没有百日咳疫苗的溶解于PBS中的OVA-II肽皮内免疫DO11.10tg小鼠。在三次每周一次的免疫一周之后,收集脾并用OVA-II肽刺激。48小时后,回收培养物上清液,并测量细胞因子的含量。如图4所示,通过IL-2-产生活性而第一次被鉴定的DO11.10T细胞(Haskins,K.等,(1983),J.Exp.Med.157:1149-1169)产生了大量的干扰素γ(IFN-γ)(图4a)。当用Wc疫苗免疫该小鼠时,以高重复性观察到产生更高的IFN-γ(一类Th1细胞因子)。相反,用Ac疫苗的肽免疫诱导较高产量的IL-5(一类Th2细胞因子)。在不同的免疫组中,另一类Th2细胞因子IL-4的产量没有如此大的差异。然而,当单独给药或与OVA-II肽一起给药时,Ac疫苗倾向于诱导较高的IL-4产量。这些结果提示由百日咳疫苗诱导的炎症环境可能还影响存在于附近的对不相关抗原特异的T细胞。考虑到作为Th细胞对肿瘤抗原(多数是自身蛋白)活性刺激结果诱导的自身攻击反应的危险性,用外来的抗原特异性的Th细胞刺激静止CTL是较完全的策略。对外来抗原的免疫反应逃脱了自身耐受,并且一般更强。此外,如果免疫反应过量,并且是在外来抗原的情况下,可通过停止免疫很容易地停止抗原供应。
体内肿瘤排斥反应
接下来,确定采用百日咳疫苗的肽免疫是否促进体内肿瘤排斥。在注射了OVA-I肽(有或没有百日咳疫苗)后,每周一次地用EG7和母EL4肿瘤细胞皮内接种小鼠,接种3次。接种一周后,重新开始每周一次的肽免疫。如图5所示,单独用肽的免疫没有显著地影响肿瘤增殖。这让人想起在临床研究当中,当单独用肽或与几乎没有佐剂活性的佐剂(如FIA)一起使用肽免疫肿瘤患者时,观察到很低的临床反应(Rosenberg,S.A.等,(2004),Nat.Med.10:909-915,Oka,Y.等,(2004),Proc.Natl,Acad.Sci.USA101:13885-13890,Nencioni,A.等,(2004),Ann.Oncol.15:153-160,Romero,P.等,(2004),CancerImmunol.Immunother.53:249-255)。当添加Ac作为佐剂时,肿瘤增殖在某种程度上变得更慢。相反,Wc疫苗很强地抑制了肿瘤增殖。与其它诱导Th1的佐剂诸如结核杆菌组分不同,即使当在重复的免疫中给药时,Wc疫苗也不会产生开放性溃疡。当在临床中使用时,这又是Wc疫苗的优点。
体内诱导针对WT1肿瘤自抗原的抗肿瘤反应
在上述试验中,应用外来抗原OVA作为模型肿瘤抗原,使得较容易消除免疫反应。接下来,确定百日咳疫苗是否有助于诱导针对肿瘤自抗原的抗肿瘤反应。为了这一目的,作为肿瘤抗原,靶向Wilms肿瘤1(WT1)基因产物(Udaka,K.等,(2000),JImmunol164:1873-1880)。发明人之前鉴定了衍生自小鼠WT1产物的表位Db126(126-134,RMFPNAPYL(SEQIDNO:2)),其由Db分子呈递并诱导抗肿瘤CTL反应。将FBL3红白血病细胞用作自发表达高水平WT1产物的肿瘤。由于当皮内注射时,FBL3细胞不会充分地增殖,因此监测腹膜内注射了致死剂量FBL3细胞的小鼠的生存活率。如图6所示,单独用肽免疫诱导极微弱的抗肿瘤活性。当给药Ac疫苗时,死亡的小鼠的数目减少。相反,用Wc疫苗和肽免疫的所有小鼠均存活。结果,显示在其中靶向肿瘤自身肽的肿瘤排斥试验中,Wc疫苗显示出强的佐剂活性。
用肿瘤肽和百日咳疫苗重复免疫的长期效果
已证实Wc疫苗的佐剂活性非常强;作为结果,提示Wc疫苗可能诱导针对自身蛋白质的攻击反应,从而引起有害作用。对于将来的人类应用,这将是关键点。因此,在添加了Wc疫苗的情况下,将采用WT1肽的免疫重复很长时间,并检查反应。即使在正常组织中,也表达WT1肿瘤抗原,尽管是小量的,具体而言是在骨髓的造血干细胞、间皮起源的胸膜和腹膜、肾足细胞、睾丸、卵巢和可能的其它干细胞中。每周一次地重复给药单独的Db126WT1肽、或单独的Wc、或Db126和Wc的混合物,总共8次免疫,并且检查有害作用。用于该免疫的Wc疫苗量比之前使用的多100倍(1×109个失活的细菌细胞)。在最后一次免疫一周后,计数尿液蛋白质和外周血细胞以及检查组织。结果,在任一个免疫组中,没有在所检查的任何组织中观察到异常。没有在尿液蛋白或血液学中观察到异常(图7)。
图8显示了每周一次地用Db126肽和Wc疫苗免疫9次的小鼠的肾和肺。在任一免疫组中,没有自身免疫反应诸如淋巴细胞浸润或组织损伤的迹象。从这些结果,发现即使长时间重复靶向WT1肿瘤抗原(自身蛋白)的肽免疫治疗,也不会发生有害的自身攻击反应。
应用WT1肿瘤抗原肽的恶性肿瘤临床分析
在Kochi大学医学院医院(KochiUniversitySchoolofMedicineHospital),应用W10肽(ALLPAVPSL(SEQIDNO:4))作为WT1肿瘤抗原肽、并应用全细胞体百日咳疫苗(Wc)作为佐剂,在患有各种恶性实体瘤的患者中进行了临床I/II期研究(有些正在进行中)。将单剂量的肽(1或3mg)溶解于300μl5%葡萄糖溶液中,并就在给药前将其与悬浮于100μl生理盐水中的5×108个Wc混合,得到总共400μl疫苗悬浮液。应用用于自己注射胰岛素的注射器和针头,将100μl该疫苗悬浮液皮下注射至两个腋窝和两个腹股沟区域中四个皮肤部位的每一个中。
剂量方案根据患者而不同;一般而言,在3次单独给药W10一周后,检查安全性测试并评估治疗效果,召开数据管理委员会以确定继续治疗的可接受性,并且对于确定继续治疗的患者,将方案调整为以1或2周的间隔给药W10+Wc。对于个例的特定剂量方案如下:

从一开始,给药W103mg+5×108Wc,给药10次。7周后,确定等级为PD。10次给药之后,等级变为PS3,并且完成了测试治疗。
●测试号KB07-009:
从一开始,给药W103mg+5×108Wc,给药6次。肿瘤坏死并脱落,从伤口发生感染,一般状况恶化,没有继续治疗。
●测试号KB07-011:
单独给药W10(3mg)5次→4周间隔,给药1次W101mg+5×108Wc→随后连续给药W103mg+5×108Wc,在第21周后,等级变为PD,治疗结束。
●测试号KB07-012和KB07-013:
从一开始,给药W103mg+5×108Wc,并且这还在进行中。
●测试号KB07-017
从一开始,给药6次W103mg+5×108Wc。在第六周时诊断成像(PET-CT)确定等级为PD。治疗完成。
通过JCOG/JSCO-2004年10月27日作为用于有害事件的评价标准应用CTCAE(CommonTerminologyCriteriaforAdverseEventsv3.0)评估安全性;如果等级为2级,暂停治疗;如果等级为3级或更高,不管与该测试治疗的因果关系,停止治疗。
根据RECIST标准(Theresse,P.等,JNatlCancerInst.2000,92,205-216)评估治疗的有效性。具体而言,如通过螺旋CT或MRI确定的那样,如果所有的靶病变消失,则等级为CR(完全消失);如果靶病症的最长直径总和降低30%或更多,则等级为PR(部分反应);如果靶病症的最长直径总和升高20%或更多,则等级为PD(进展疾病);如果结果不符合PR或PD的标准,则等级为SD(稳定疾病)。如果出现了新的病变,不管存在靶病变与否,将等级确定为PD。结果显示于表1和图9-11中。
在完成了6次或更多次W10+Wc给药的11个患者中,10个允许通过RECIST标准进行功效评估,其中半数5个给出了等级SD(表1)。对于KB07-003(图10),在开始加入Wc后,在14周期间的SLD的改变是58%,得到根据RECIST标注的PR等级;然而,由于该患者具有对于靶骨损伤的放疗史,因此不能排除其可能的迟发效果;根据标准“PSA水平保持在测量的极限”,确定其等级为SD(表1)。对于KB07-009,存在严重的发红和肿胀,以及在癌转移部分疼痛,这在给药48小时后达到峰值,但是这些在约2天内减轻,并且这甚至在接下来的肽免疫之后仍然被重复。在肽免疫治疗开始之后,成像显示戏剧性的改变,其中影响颞骨内部和口腔至颈部皮肤的肿瘤质量快速溶解,但是形成了从口腔穿透至颈部皮肤的洞,从该伤口发生了感染,并且其一般状况恶化,因此中断了治疗,并且没有能够进行RECIST评分(表1)。

Claims (18)
1.用于癌症的治疗剂或用于癌症转移或复发的预防剂,其包含癌抗原肽和百日咳疫苗。
2.权利要求1的试剂,其中所述百日咳疫苗是佐剂。
3.权利要求1或2的试剂,其中所述百日咳疫苗是全细胞百日咳疫苗。
4.权利要求1-3中任一项的试剂,其中所述癌抗原肽是选自以下的蛋白:WT1、存活蛋白和前列腺特异性抗原、或包含其氨基酸序列一部分的肽。
5.权利要求1-4中任一项的试剂,其中所述癌是高度地表达选自WT1、存活蛋白和前列腺特异性抗原的蛋白的恶性肿瘤。
6.权利要求1-5中任一项的试剂,其中所述试剂允许至少3次的免疫接种。
7.用于病毒感染性疾病的治疗剂,其包含衍生自引起病毒感染性疾病的病毒的肽以及百日咳疫苗。
8.权利要求7的试剂,其中所述百日咳疫苗是佐剂。
9.权利要求7或8的试剂,其中所述百日咳疫苗是全细胞百日咳疫苗。
10.根据权利要求7-9中任一项的试剂,其中所述病毒是诱导恶性肿瘤的病毒。
11.权利要求10的试剂,其中所述试剂用于预防/治疗病毒诱导的恶性肿瘤。
12.权利要求7-9中任一项的试剂,其中所述病毒选自:丙型肝炎病毒、流感病毒和登革病毒。
13.权利要求7-12中任一项的试剂,其中所述试剂允许至少3次的免疫接种。
14.用于疫苗的佐剂,其包含作为主要成分的百日咳疫苗以及作为活性成分的癌抗原肽或病毒肽。
15.权利要求14的佐剂,其中所述百日咳疫苗是全细胞百日咳疫苗。
16.权利要求14或15的佐剂,其中所述病毒是诱导恶性肿瘤的病毒。
17.权利要求14或15的佐剂,其中所述肽是选自以下的蛋白:WT1、存活蛋白和前列腺特异性抗原、或包含其氨基酸序列一部分的肽。
18.权利要求14或15的佐剂,其中所述病毒选自:丙型肝炎病毒、流感病毒和登革病毒。
