すなわち、本発明による、悪性腫瘍の治療または転移もしくは再発の予防剤は、悪性腫瘍抗原ペプチドおよび百日咳ワクチンを含み、前記悪性腫瘍抗原ペプチドは、WT1タンパク質またはそのアミノ酸配列の一部を含むペプチドであり、かつ、前記百日咳ワクチンは、全菌体百日咳ワクチンアジュバントである、悪性腫瘍の治療または転移もしくは再発の予防剤である。また、本発明のアジュバントは、悪性腫瘍抗原ペプチドを有効成分とするワクチン用のアジュバントであって、前記悪性腫瘍抗原ペプチドは、WT1タンパク質またはそのアミノ酸配列の一部を含むペプチドであり、かつ、前記アジュバントは、全菌体百日咳ワクチンを主成分とすることを特徴とするアジュバントである。

好ましい製剤例の1つとして、ペプチドの1回投与量(例えば、3mg)を300μlの溶剤(例、5%ブドウ糖液)に溶解し、これに5×108個の全菌体百日咳ワクチンを100μlの懸濁剤(例、生理的食塩水)に懸濁したものを、投与直前に混合し、合計400μlのワクチン液とする方法が挙げられるが、これに限定されない。
細胞および抗体
オボアルブミンをトランスフェクトしたEL4 細胞株であるE.G7-OVA(Moore, M. W. et al., (1988), Cell 54: 777-785)は、Herman N. Eisen博士を介してMichael Bevan博士からの厚意で贈られた。FBL3は、赤白血病系統(H-2b)であり、Bruce Chesebro博士の厚意で許可を得てMasaaki Miyazawa博士から提供された。
OT1トランスジェニックマウス(OT-1 tg)は、H-2 Kb 分子に結合するオボアルブミンペプチド、257-264に特異的な一対のTCRを発現し(Hogquist, K. A. et al., (1994), Cell 76: 17-27)、これはF. Carbone博士から厚意で贈られた。OT1 tgは、M. Okabe博士の厚意で提供されたEGFPトランスジェニックマウス(Okabe, M. et al, (1997), FEBS Letters 407: 313-319)と交配させた。両方のトランスジェニックマウス系統は、C57BL/6(B6)バックグラウンドと何度もバッククロスさせた。B6マウスはSLC(Shizuoka, Japan)から購入し、高知大学医学部のSPF施設で繁殖させた。D.Y. Loh博士からの厚意の贈与であるDO11.10トランスジェニックマウスは、BALB/cと交配させ、I-Adに結合するOVA-IIペプチド(オボアルブミン 323-339)に特異的なTCRの対をコードするトランスジーンについてホモ接合体として維持した(Murphy, K. M. et al., (1990), Science 250: 1720-1723)。
OVA-I(SIINFEKL(配列番号:1))ペプチドおよびDb126(RMFPNAPYL(配列番号:2))ペプチドは、Fmoc法にて手動で合成した。ペプチドは、>95%の純度まで逆相HPLCで精製し、その分子量はMALDI TOF質量分析器(Voyager DE-RP, Applied Biosystems, Foster city, CA)で確認した。ペプチドの濃度は、MicroBCAアッセイ(Pierce, Rockford IL)で決定した。
百日咳ワクチンは、Bordetella Pertussis東浜I相株細菌から製造し、(財)阪大微生物病研究会(Bikenkai, Kannonji, Japan)により提供された。無細胞ワクチンは不活化百日咳毒素と繊維状血球凝集素とからなり、リン酸アルミニウムと混合されている。全菌体ワクチンは、PBSに懸濁した不活化細菌である。
B6マウスを、共にフロイント不完全アジュバント(FIA)中に乳化させた、リン酸アルミニウムに吸着させた無細胞(Ac)ワクチン5 μgまたは2×109の不活化全菌体(Wc)で皮下免疫した。週一回の免疫を1回、2回および3回した1週間後に、マウスを屠殺し、血清をELISA分析のために採取した。
平底96ウェルプレート(SUMILON, Akita, Japan)を、アルミニウム塩なしで20 μg/mlの濃度のAcワクチンでコーティングし、PBS中5%のスキムミルクでブロッキングした。マウス血清を1/10から始めて連続希釈し、4℃で1時間インキュベートした。HRP標識したヤギ抗マウスIgG (γ鎖特異的) 抗体 (Zymed, South San Francisco, CA)を二次試薬として使用し、4℃で30分間インキュベートした。OPD基質を添加し、吸光度を490 nmで測定した。
OT1tg由来の脾臓細胞10,000,000個を静脈内注射した。次の日、マウスを、50 nmolesの OVA-Iペプチドで背部皮膚に免疫した。アジュバントとして添加した百日咳ワクチンは、リン酸アルミニウムに吸着させたAcワクチン10μg/マウス又は2×107の不活化全菌体細菌であった。マウスを、次の週に同じ抗原で追加免疫した。2回目の免疫の1週間後、マウスを屠殺し、脾臓細胞をフローサイトメトリーによって分析した。トランスジェニックTCRを同定するために、フィコエリトリン(PE)標識したモノクローナル抗Vβ5特異的抗体であるMR9-4(Kanagawa, O. et al., (1991), J. Immunol. 147: 1307-1314)を使用した。
B6マウスを、アジュバント有り無しで、PBSに溶かした50 nmolesのOVA-Iペプチドまたは100 nmolesのDb126ペプチド(Udaka, K. et al, (2000), J Immunol 164: 1873-1880)に、5 μgの無細胞ワクチン又は全菌体ワクチン由来の1×107の不活化細菌を添加して免疫した。免疫原を背部皮膚の2部位に皮内注射した。週一回のペプチド免疫を4回繰り返した。
4回の週一回の免疫を、上述のようにB6マウスで実施した。最後の免疫の1週間後、脾臓細胞をEG7細胞に対し、インビトロで刺激した。簡潔に述べると、1つの脾臓由来の脾細胞を、5%ラットConA上清を含む10%FCS DMEM(32 ml)中に懸濁した。EG7細胞に137Cs源からの60 Gyを照射し、1×105細胞/mlの密度で脾臓細胞に添加した。刺激から5日目に、様々なE/T比で、10,000個の EG7細胞に対する5時間の51Cr放出アッセイによって細胞溶解活性を測定した。%特異的溶解は、(実験的放出-自発的放出)/(総放出-自発的放出)×100の積である。
Th1/Th2サイトカイン分泌を、Luminex 100 (Luminex Corporation, Austin, TX) およびFluorokine MAPマウスキット (R&D Systems Inc., Minneapolis, MN)を使用して測定した。DO11.10マウスに、マウス1匹当たり100 nmolesのPBS 中OVA-II ペプチドを、アジュバント有り無しで、背部皮膚の2部位に皮内注射した。使用したアジュバントは、10 μg/マウスのリン酸アルミニウムに吸着させたAcワクチン又は2×107細胞/マウスのWcワクチンであった。週一回の免疫の3回目の1週間後、マウスを屠殺し、脾臓細胞をOVA-IIペプチドで刺激した。簡潔に述べると、2×106のDO11.10脾細胞/ウェルを、DMEMを含む10% FCS(200 μl/ウェル)中のOVA-IIの連続希釈物で刺激した。37℃で48時間のインキュベーション期間中に上清中に分泌されたサイトカインを、IFN-γ、IL-4、およびIL-5に対するFluorokineキットで測定した。サイトカイン濃度は、アッセイ中に存在するKJ1.26+、CD4+T細胞 (DO11.10 トランスジェニックTCR 保有細胞(Haskins, K. et al., (1983), J. Exp. Med. 157: 1149-1169))の数で補正した。
3,000,000個のEL4 細胞および4,000,000個のEG7細胞を、B6マウスの剃毛した背部脇の対側に皮内接種した。互いに垂直な最大寸法を、接種後7日目から開始して一日おきに測定した。腫瘍細胞数を予め決定して、2つの細胞系統について同程度の腫瘍成長速度を与えた。5匹のマウスの群を、アジュバント(リン酸アルミニウムに吸着させたAcワクチン5 μg又は1×107の不活化百日咳全菌体)有り無しで、PBS中に溶かした50 nmoles のOVA-Iに加えて、週一回の皮内注射により3回免疫した。腫瘍接種部位から離れた背部皮膚を免疫に使用した。3回目の免疫の1週間後に、腫瘍細胞を接種した。週一回の免疫をさらに2回、腫瘍接種の1週間後に開始して実施した。 WT1 ペプチドでの免疫効果を調べるためには、FBL3 赤白血病細胞を、腫瘍チャレンジ実験に使用した(Udaka, K. et al., (2000), J Immunol 164: 1873-1880)。マウスを、アジュバント有り無しで、100 nmoles のDb126 ペプチド (RMFPNAPYL(配列番号:2),(Udaka, K. et al., (2000), J Immunol 164: 1873-1880))と皮内注射することによって、週一回免疫した。3回目の免疫の1週間後、5,000,000個のFBL3細胞を腹腔内注射して生存日数を調べた。週一回のペプチド免疫を、腫瘍接種の1週間後に再開した。
B6マウスを、50 nmoles のDb126 ペプチド有り無しで、1×109の不活化百日咳全菌体 (CTL誘導および腫瘍接種実験で使用した量の100倍)と共に、週一回皮内免疫した。9回目の免疫の1週間後、尿を採り、マウスを屠殺して血液および組織(肺、腎臓、卵巣、精巣、骨髄)を採取した。組織を10%ホルマリンで固定し、そのパラフィン切片をHaematoxylin & Eosinで染色した。腋下大動脈および大静脈から採取した血液を、10mM EDTAを含むPBS中に即座に10倍希釈し、適切な希釈の後、赤血球(RBC)を、血球計算板を使用して計数した。白血球を、Turk溶液中で10倍希釈してRBCを溶解後、同様に計数した。尿総蛋白質量は、マイクロTPテストキット(和光純薬工業,大阪)を用いて測定した。
百日咳ワクチンのアジュバント活性
百日咳ワクチンのアジュバント活性を調べるために、内因性腫瘍抗原としてオボアルブミン (OVA)を導入したモデル抗原系を利用した(Moore, M. W. et al., (1988), Cell 54: 777-785、Hogquist, K. A. et al., (1994), Cell 76: 17-27)。Kb結合OVA-I (SIINFEKL(配列番号:1)) ペプチドは、H-2bマウスにおける主要なCTLエピトープであることが知られている(Moore, M. W. et al., (1988), Cell 54: 777-785)。OVA-I ペプチド免疫によるCTLの誘導が、百日咳ワクチンの添加によって増強できるかどうかを最初に試験した。まず、CTL誘導実験の前に、百日咳ワクチンのヘルパー誘導活性を試験した。ヘルパー活性は、PTとFHAとの混合物に対するIgG抗体の産生によって、従って主にTh2偏位した活性を試験することによって調査した。図1に示されるように、IgG抗体は、Acワクチンで免疫したときには3週間以内に非常に良好に産生された。WcワクチンもまたIgG抗体産生を誘導したが、その量はAcワクチンを用いたときよりも少なかった。これにより、AcワクチンおよびWcワクチンの両方が、Th細胞活性を刺激できることが示された。
腫瘍抗原特異的T細胞の生体内増殖をモニターするために、ニワトリβ−アクチンプロモーターおよびサイトメガロウイルスエンハンサーの下でEGFPを発現するトランスジェニックマウスと交配したOVA-I特異的TCRトランスジェニックマウス (OT1)の脾細胞を、正常のマウスに養子移入した。このEGFPトランスジェニックマウスでは、理由は不明だが、全てのT細胞がEGFPを発現するわけではない(Okabe, M. et al., (1997), FEBS Letters 407: 313-319)。脾臓中の80±10(SD)%のCD8 T細胞がEGFP発現について陽性であった。モノクローナル抗Vβ5特異的抗体、MR9-4(Kanagawa, O. et al., (1991), J. Immunol. 147: 1307-1314)を、トランスジェニックTCRを特定するために使用した。図3に示すように、OT1-TCR+、EGFP+CD8 T細胞は、静脈内注射の15日後には、脾臓において、検出限界近くまで減少していた。しかし、マウスをOVA-Iペプチド単独で皮内に2回免疫すると、トランスジェニックOT1-TCR+、EGFP+、CD8 T細胞は、観察できる程度に増えていた。百日咳ワクチンをOVA-I ペプチドに添加したところ、OT1細胞はさらに増殖していた。興味深いことに、AcワクチンによるOT1細胞の増殖は、Wcよりも、若干強いことが再現性よく観察された。この結果により、Acは抗原特異的T細胞の数を増大する活性はあるが、図2で観察されたように、細胞傷害活性はあまり強くなかった。これに対して、Wcワクチンは強い細胞傷害活性を誘導することにおいて非常に有効であった。細胞傷害活性を生じさせるためには、新たなタンパク質合成をして、細胞傷害性顆粒の成分であるパーフォリンおよびグランザイムを産生する必要がある。Wcワクチンによって誘導された免疫刺激環境は、このようなタンパク質合成の誘導において一役を果たした可能性がある。
次に、アジュバントとして添加した百日咳ワクチンが、近傍にいるB. Pertussisとは無関係な異種ペプチド抗原で刺激したT細胞が分泌するサイトカインプロフィールにも影響を与えるか否かを試した。モデル抗原系として、レスポンダーにはDO11.10 TCRトランスジェニック (DO11.10tg) CD4 T細胞を使用した。DO11.10 TCRは、I-Ad 分子に結合したOVA-II ペプチド (323-339, ISQAVHAAHAEINEAGR(配列番号:3),(Hunt, D. F. et al, (1992), Science 256: 1817-1820))に特異的である。DO11.10tgマウスを、材料と方法に記載したように、百日咳ワクチン有り無しで、PBSに溶かしたOVA-IIペプチドと共に皮内免疫した。週一回、3回免疫して1週間後、脾臓を採取し、OVA-II ペプチドで刺激した。48時間後、培養上清を回収し、サイトカイン含量を測定した。図4に示すように、IL-2を産生する活性によって最初に同定されたDO11.10 T細胞(Haskins, K. et al., (1983), J. Exp. Med. 157: 1149-1169)は、かなりの量のインターフェロン−γ(IFN-γ)を産生した(図4a)。Wcワクチンで免疫した場合、Th1型サイトカインであるIFN-γのより高い産生が再現性よく観察された。対照的に、Acワクチンでのペプチド免疫は、Th2型サイトカインであるIL-5のより高い産生を誘導した。別のTh2型サイトカインであるIL-4の産生は、異なる免疫群の間でそれほど差異がなかった。しかし、単独又はACワクチンをOVA-IIペプチドと共に投与すると、より高いIL-4産生を誘導する傾向があった。これらの結果は、百日咳ワクチンによって誘導された炎症環境が、近傍にいる、無関係な抗原に特異的なT細胞に対しても影響を持つことを示唆する。腫瘍抗原(その殆どが自己タンパク質である)に対するTh細胞を能動的に刺激することによって自己攻撃的な応答を誘導する危険性を考慮すると、外来抗原特異的Th細胞によって静止CTLを刺激することは、より安全なストラテジーであろう。外来抗原に対する免疫応答は、自己寛容にかからず、一般により強力である。さらに、免疫応答が行き過ぎた場合には、外来抗原であれば免疫を停止することによって容易に抗原の供給を停止することができる。
次に、百日咳ワクチンでのペプチド免疫が生体内で腫瘍拒絶を促進するか否かを調査した。百日咳ワクチン有り無しでOVA-Iペプチドを週一回、3回注射後、マウスにEG7および親EL4腫瘍細胞を皮内接種した。接種の1週間後、週一回のペプチド免疫を再開した。図5に示すように、ペプチド単独での免疫は、腫瘍増殖に対して有意な影響を有さなかった。これは、臨床試験において、担がん患者を、ペプチド単独又はFIAのようなアジュバント活性がほとんどないアジュバントと共に免疫した場合に観察された、低い臨床応答を想起させる(Rosenberg, S. A. et al., (2004), Nat. Med. 10: 909-915、Oka, Y. et al., (2004), Proc. Natl. Acad. Sci. USA 101: 13885-13890、Nencioni, A. et al., (2004), Ann. Oncol. 15: 153-160、Romero, P. et al., (2004), Cancer Immunol. Immunother. 53: 249-255)。Acワクチンをアジュバントとして添加した場合、腫瘍増殖はある程度ゆっくりになった。これとは対照的に、Wcワクチンは、腫瘍増殖を強力に抑制した。結核菌成分のような他のTh1誘導アジュバントとは異なり、Wcワクチンは、反復免疫をしても開放性潰瘍を起こさなかった。これは、臨床に持ち込む場合に、Wcワクチンがさらに有利な点である。
上記実験は、免疫応答がより容易に惹起され得るモデル腫瘍抗原としての外来抗原OVAを使用した。次に、百日咳ワクチンが自己腫瘍抗原に対する抗腫瘍応答の誘導を助けるか否かを調査した。この目的のために、腫瘍抗原としてウィルムス腫瘍1(WT1)遺伝子産物(Udaka, K. et al., (2000), J Immunol 164: 1873-1880)を標的にした。本発明者らは、Db 分子によって提示され、抗腫瘍CTL応答を誘導したマウスWT1産物由来のエピトープDb126 (126-134, RMFPNAPYL(配列番号:2))を以前に同定した。FBL3 赤白血病細胞を、高レベルのWT1産物を自然に発現する腫瘍として使用した。FBL3細胞は、皮内注射によっては充分増殖しなかったので、致死用量のFBL3細胞を腹腔内注射したマウスの生存をモニターした。図6に示されるように、ペプチド免疫単独は、ごく弱い抗腫瘍活性を誘導した。Acワクチンを投与すると死ぬマウスの数が減った。これとは対照的に、Wcワクチンとペプチドを免疫したマウスでは、全てが生き残った。この結果、自己腫瘍ペプチドを標的とした腫瘍拒絶実験においても、Wcワクチンが強力なアジュバント活性を有することが示された。
Wcワクチンのアジュバント活性が非常に強いことが分かったが、その結果、Wcワクチンが自己タンパク質に対して攻撃的な応答を誘導し、有害な副作用を引き起こす可能性も出てきた。将来ヒトに適用するために、これは重要なポイントとなる。そこで、Wcワクチンを添加してWT1ペプチドで長期にわたり免疫を繰り返して反応を調べた。WT1腫瘍抗原は、正常組織でも、骨髄中の造血幹細胞、中皮起源の胸膜および腹膜、腎臓の有足細胞、精巣、卵巣および、おそらく種々の幹細胞にも、少量発現される。Db126 WT1 ペプチド単独あるいはWc単独、あるいはDb126とWcを混ぜて、毎週1回、合計8回免疫を繰り返して副作用が出るかどうか、調べた。これまでに使った量の、100倍過剰量(1×109の不活化細菌)のWcワクチンを免疫した。最後の免疫から1週間後、尿タンパクおよび末梢血の細胞数と組織を調べた。その結果、いずれの免疫群においても、組織に異常はみられなかった。尿タンパク、血液像にも異常が認められなかった(図7)。
高知大学医学部付属病院にて、WT1腫瘍抗原ペプチドとしてW10ペプチド(ALLPAVPSL(配列番号:4))を、アジュバントとして全菌体百日咳ワクチン(Wc)をそれぞれ用い、各種悪性固形腫瘍の患者における第I/II相臨床試験を実施した(一部継続中)。ペプチドの1回投与量(1または3mg)を300μlの5%ブドウ糖液に溶解し、これに、5×108個のWc菌体を100μlの生理的食塩水に懸濁したものを、投与直前に混合し、合計400μlのワクチン液とした。該ワクチン液を、インシュリンの自己注射用の注射筒と針を使って、両腋窩および両そけい部の皮膚、計4箇所に100μlずつ皮内注射した。
投与プロトコルは各症例に応じて異なるが、典型的には、W10単独投与3回後、1週間後に安全性試験のための検査と治療効果の評価を行い、データマネージメント委員会を開いて治療継続の可否を判断し、継続が決定したものについて、1、2週あけて、W10+Wc投与に移行した。各症例の具体的な投与プロトコルは以下の通りである。

最初から、W10 3mg+5×108 Wcで継続投与中
・試験番号KB07-017:
最初からW10 3mg+5×108 Wcを6回投与。6週目の画像診断(PET-CT)にてPDの判定。治療終了。
安全性の評価は、有害事象の判定基準としてCTCAE(Common Terminiology Criteria for Adverse Events v3.0) by JCOG/JSCO-Oct.27,2004を用い、grade 2で休止、grade 3以上で、試験治療との因果関係に関係なく中止とした。
また、治療の有効性の評価は、RECIST基準(Theresse, P. et al., J Natl Cancer Inst. 2000, 92,205-216)に従って行った。具体的には、helical CTまたはMRIにより、全ての標的病変の消失した場合をCR (complete response)、標的病変の最長径の和が、30%以上減少した場合をPR (partial response)、標的病変の最長径の和が、20%以上増加した場合をPD (progressive disease)、PRにもPDにも不十分である場合をSD (stable disease)として評価した。但し、標的病変の有無に関わらず、新病変が一個でも出現した場合は、PDと評価した。結果を表1および図9〜11に示す。
Claims (5)
- 悪性腫瘍抗原ペプチドおよび百日咳ワクチンを含み、
前記悪性腫瘍抗原ペプチドは、WT1タンパク質またはそのアミノ酸配列の一部を含むペプチドであり、かつ、
前記百日咳ワクチンは、全菌体百日咳ワクチンアジュバントである、
悪性腫瘍の治療または転移もしくは再発の予防剤。 - 前記悪性腫瘍が、WT1を高発現する悪性腫瘍である、請求項1に記載の剤。
- 少なくとも3回の免疫接種が可能である、請求項1または2に記載の剤。
- 前記悪性腫瘍抗原ペプチドが、配列番号2または配列番号4で表されるアミノ酸配列を有するペプチドである、請求項1から3のいずれか一項に記載の剤。
- 悪性腫瘍抗原ペプチドを有効成分とするワクチン用のアジュバントであって、
前記悪性腫瘍抗原ペプチドは、WT1タンパク質またはそのアミノ酸配列の一部を含むペプチドであり、かつ、
前記アジュバントは、全菌体百日咳ワクチンを主成分とすることを特徴とするアジュバント。
