背景技术
存在许多情况需要在体腔中插入导管以达到所需的医疗目标。一种相对普通的情况是直接将营养液或药物提供到胃或肠中。在胃或肠壁中形成开孔并且通过开孔放置导管。这种外科开口和/或产生开口的程序通常称为“胃造口术”。饲喂液可以通过导管注入以直接将营养提供给胃或肠(被称为肠内饲喂)。用于肠内饲喂的各种不同导管已经发展多年,包括相对于位于患者皮肤上的导管部分具有“低型面”的一些导管,以及那些具有更传统或非低型面构形的导管。这些经皮的跨导导管(有时称为“经皮的跨导管”)通常称为“胃造口术导管”、“经皮的胃造口术导管”、“PEG导管”或“肠内饲喂导管”。Picha等人在2000年2月1日发表的名称为“低型面气囊饲喂装置”的美国专利6,019,746提供了一种装置的实例。
这些导管通常用于被称为经皮的内窥镜胃造口术(通常称为PEG)的程序中。传统上,利用内窥镜引导或x射线引导放置PEG管。在将PEG管放置到患者胃中的传统PEG程序中,使用内窥镜观察患者的食道没有阻塞并且检查胃部和给胃部充气以观察选择进行胃造口术的区域是否可以被扩大。
如果位置是合适的,该点即被选定。在有些类型的程序中,在放置任何饲喂管之前,已经发现特别需要将胃腔(例如胃部)的前壁固定到腹壁上,作为产生通过两者的开孔通道之前的步骤。因此发现附加装置是关键的,因为其帮助防止意外的分离和腹腔暴露于污染和可能的腹膜炎。这个程序还适用于空肠造口术或胃空肠造口术以及上述的胃造口术程序。类似的程序还可以适用于或希望用于其它的导管例如腹腔引流管。

当从留在患者皮肤外部的末端拉出引导线时,PEG管被引导到患者的口腔中(同时内窥镜完全移离患者)并且被拉入患者的胃腔。一旦PEG管处于胃腔中,就将其部分地拉过胃和腹壁直到PEG管的缓冲器紧贴在胃粘膜上。然而,为了将PEG管部分地拉过胃和腹壁和皮肤,必须扩张最初的针通道。利用在PEG管的远端采用锥形扩张器的传统扩张装置进行该扩张以使得当其被拉过胃粘膜时使开口扩张。在这种扩张中,内窥镜再次进入患者并且随后被用于视觉观察PEG管的缓冲器紧贴在胃粘膜上。
在其它传统的PEG管放置程序中,根本不使用内窥镜检查。替代地,用x射线技术帮助选择患者体内用于引入PEG管的特别合适的位置(例如胃部),x射线被用于引导PEG管的放置和用于检查PEG管的最终位置。
有许多与这些传统程序相关的问题,包括:与内窥镜多次进入和离开患者相关的更高的食道创伤危险;将PEG放置在不正确的位置,大导管例如PEG通过食道;和/或将腔壁固定到腹部的另外的并发症和/或损伤。虽然可能希望避免这些问题,但是缺乏合适的装置或程序。
因此,需要将非血管导管例如PEG管放置在患者中并且减少这些危险和损伤且容易执行的装置、系统和方法。
发明内容
针对在此讨论的困难和问题,本发明提供了扩张装置和扩张系统。扩张装置是用于在非血管腔中放置导管的可充气装置,放置步骤期望地通过使用内窥镜在直接目视下进行。由于胃部是非血管腔的常见实例,为了说明本发明的目的,使用术语“胃腔”或“胃部”代表所有其它的非血管腔或空间(例如,十二指肠、空肠、回肠、腹腔等等),除非另外说明。
根据本发明,使传统的内窥镜进入胃部以吹入治疗并容许触诊以定位合适的位置。一旦合适的位置被定位,则从身体外部通过腹部将针插入胃部以形成针通道。然后通过针将引导线引入胃部,并且提供系统用于:在针通道中放置扩张装置;将扩张装置保持在所需的位置;将针通道扩张,和取出扩张装置。
扩张装置至少包括可充气扩张气囊和可充气固位气囊,将扩张气囊充气和放气的充气腔,将固位气囊充气和放气的充气腔,管状支撑件,和容纳引导线的通过该装置的连续通道。扩张气囊可以是顺应、半顺应、或非顺应。
该装置可以具有远端和近端。至少一个扩张气囊定位朝向装置的远端。扩张气囊(多个)在充满气时具有预定的直径的长度以适合具体尺寸的导管装置。替代地,可以分别使用不同的充气压力将扩张气囊(多个)扩张为各种有效直径以适合各种导管。装置的近端部分(位于非血管腔中的扩张装置部分)包括至少一个固位气囊(也称为“近端固位气囊”),其具有比扩张气囊(多个)的任何直径基本上更大的直径。一旦将该固位气囊充气,其就起作用以提供扩张装置在非血管腔(例如胃部)中的固位。近端固位气囊组件可以是顺应、半顺应、或非顺应。扩张气囊和固位气囊可以由相同的材料形成或者它们各自可以由不同的材料形成。每个气囊期望包括两个相对的开口端,开口端可以安装到管状支撑件上。

根据本发明,扩张装置可以用于“从内到外”或“从外到内”的扩张程序中。从内到外的扩张程序涉及将扩张装置安装到患者口腔外或非血管腔(例如胃部或其它空间)内的引导线。在患者口腔外的安装的非限制性实例可以涉及下列步骤:插入从口腔外延伸到胃部内的内窥镜;使用针通过皮肤、腹壁和胃壁传统地放置引导线;通过内窥镜的工作通道插入标准的内镜钳或内窥镜用绞断器;使用内镜钳或绞断器抓住在胃部中的引导线部分,然后通过内窥镜的工作通道拉引导线并将其拉出患者的口腔(与目前将整个内窥镜从患者取出的惯例不同);将最接近扩张气囊(不是扩张装置的固位气囊部分)的扩张装置的那端稳固地安装到从患者口腔伸出的引导线的那端;通过内窥镜的工作通道将引导线和安装的扩张装置拉回从而通过保留在皮肤外部的引导线部分使扩张气囊退出工作通道进入胃部。将扩张装置安装到患者胃部内的引导线上的非限制性实例可以涉及下列特征和/或步骤:扩张装置在最接近扩张气囊的那端(首先进入口腔的那侧)包括固定装置(磁铁、钩子、环、绞断器,等等);通过内窥镜的工作通道将扩张装置推入从而使固定装置退出工作通道;在内窥镜的目视下通过将固定装置连接到引导线(其通过针插入)而安装固定装置;拉动保留在皮肤外部的引导线部分从而将扩张装置拉过工作通道并使其进入胃部。与用于将扩张装置放置于胃部中的步骤无关,在放置于胃部中之后,将其拉入并且部分地拉过针通道从而使放气的扩张气囊的至少一部分延伸穿过腹部组织和皮肤并且固位气囊留在胃部中。
从外到内的扩张程序与从内到外的程序的不同之处在于其不涉及为了将扩张装置放置在胃部中而在取出内窥镜或使扩张装置通过内窥镜的工作通道之后通过身体将扩张器拉入合适位置中,也无需将扩张装置安装到从患者的胃部延伸通过口部的引导线上。从外到内的程序可以涉及下列步骤:插入从口腔外延伸到胃部内的内窥镜;最初通过插入的针通过皮肤、腹壁和胃壁传统地放置引导线然后取出针;在患者皮肤外部的引导线端安装扩张装置;将扩张装置部分地插入针通道从而使固位气囊在扩张气囊的任一部分之前进入胃部。
在放置扩张装置时,扩张气囊和固位气囊必须处于放了气的状态以使得扩张装置易于滑过内窥镜的工作通道和/或其在没有过多的力的情况下穿透针通道。优选,在这种放了气的状态的扩张装置尽量缠绕和折叠在管状支撑件周围以在通过内窥镜和/或针通道插入期间将扩张装置的有效横截面积减到最小。通过使用打褶机和/或折叠件生产设备以预先计划的安排有意地折叠气囊壁,或者通过由气囊壁的柔韧性和薄而提供的随意重叠和折叠,实现这种缠绕和折叠。

根据本发明,可充气的固位气囊108构形成在充满的、不受限的充气状态下其有效横截面大于可充气扩张气囊106在充气状态下的最大横截面,如图1中大体上示出的。扩张气囊(多个)具有长度和圆形横截面,其圆形横截面在充满气时沿着长度具有预定直径以适合具体尺寸的导管装置。替代地,可以分别利用不同的充气压力将扩张气囊(多个)扩张成各种有效直径以适应各种导管。作为非限制性的实例,扩张气囊的有效充气直径的范围可以从大约3到大约10毫米。作为另一个非限制性的实例,扩张气囊的有效充气直径的范围可以从大约2到大约8毫米。也可想到具有长度且沿着长度具有非圆形横截面的充气的扩张气囊,例如椭圆形或卵形。
装置的近端部分(扩张装置的位于非血管腔中的那个部分)结合有至少一个固位气囊(也被称为“近端固位气囊”),其具有比扩张气囊(多个)的任何直径基本上更大的横截面或直径。一般而言,固位气囊的横截面或直径是扩张气囊直径的大约1.5倍到大约3倍。一旦将该固位气囊充气,它就起作用以稳定腔壁和/或提供扩张装置在非血管腔(例如胃部)中的固位。近端固位气囊108可以具有圆形或非圆形的横截面,只要它能如上所述地起作用即可。固位气囊可以有、或没有具有一个对称轴的横截面。例如,近端固位气囊108可以具有正方形、矩形、三角形、椭圆形、卵形形状或其他形状。替代地和/或另外,近端固位气囊108可以结合有构成其整个横截面一部分的凸角、指状部或突起,因此其大于扩张气囊106的直径。
每个气囊希望地包括两个相对的开口端。开口端可以安装到管状支撑件上。参考图1,扩张气囊106可以具有开口端118和120。固位气囊108可以具有开口端122和124。所希望的是,气囊的位置尽量靠近在一起。在这个方面,固位气囊108的开口端122可以内翻以相对扩张气囊106提供较近的安装。能想到扩张气囊106的开口端120也可以内翻以提供靠近的安装。
固位气囊组件和扩张气囊可以由合适材料形成以使得气囊是顺应、半顺应或非顺应。即,气囊可以是比较弹性的(例如,顺应)以使得它在充气时伸展以及膨胀,气囊也可以是有点弹性的(例如,半顺应)以使得它在充气时会膨胀但只有有限的伸展,气囊可以是无弹性的(例如,非顺应)以使得它在充气时膨胀而没有显著的伸展。气囊可以各自由不同材料制成从而一个气囊可以是顺应,一个气囊可以是非顺应。能想到各种组合。所希望的是,气囊中的一个或两个气囊都可以由标识为2363-90A的聚氨酯材料形成,其可从Lubrizol Advanced Materials,Inc.,ThermedicsTM Polymer Products获得。
根据本发明的一个方面,扩张装置包括具有长度、宽度和纵轴的管状支撑件,管状支撑件限定了通过装置的连续通道。装置进一步包括至少两个可充气气囊,至少第一气囊轴向地定向在管状支撑件上形成装置的扩张区域,且至少第二气囊形成固位区域限定装置的第二部分。为每个可充气气囊提供至少一个气囊充气腔以使得固位区域构形成在充满的、不受限的充气状态下具有至少一个有效横截面和/或直径,该有效横截面和/或直径大于扩张区域在充气状态时的最大横截面和/或直径。
如图1中所示,扩张气囊106是轴向地定向在管状支撑件102上的第一气囊,固位气囊108可以如所希望地轴向地定向在管状支撑件102上。然而,能想到其他构形。例如,可以将多个固位气囊安装到管状支撑件上以从管状支撑件径向地突出。
本发明还覆盖了用于扩张开孔并插入非血管导管的系统,该系统包括如上所述的开孔扩张装置。系统还包括非血管导管,其构形成通过扩张的开孔通道安装在完全或部分充气的扩张气囊上面并进入由固位气囊稳定的非血管腔部分。根据该系统,开孔扩张装置构形成放气的且装置的至少一部分通过非血管导管收回。
在对该装置进行放置的示例性和非限制性的描述中,内窥镜可以前进到非血管腔(例如胃部)中以进行吹入并容许触诊以对导管定位位置(例如,PEG定位位置)进行定位。一旦位置被定位,就可以通过腹部将针插入胃部并可以通过针将引导线引入胃部。
可以通过内窥镜的工作通道插入标准的内镜钳、内窥镜用绞断器或气囊安装固定装置,使用内镜钳、绞断器或固定装置抓住引导线并通过内窥镜的工作通道拉起引导线并将其拉出患者的口腔。
将具有其附装的充气腔的扩张装置固定到引导线的端部并利用引导线将扩张装置拉过内窥镜的工作通道并使其进入胃部。扩张装置可以具有扩张气囊和固位气囊,扩张气囊在充满气时具有预定的体积和直径,固位气囊在充满气时其直径大于扩张气囊的最大直径。当这些气囊处于折叠或紧密缠绕的状态中时,扩张装置的总体直径适合在内窥镜的工作通道内。通常,该直径处于大约2毫米的范围内或更小。
从胃部取出针,同时将引导线留在针通道内。将扩张装置拉起到针通道中并使其部分地通过针通道以使得它到达腹部组织和患者外部的皮肤,如图3中所示。
现在参考图4A和4B,然后通过逐渐引入受控数量的流体(例如液体或气体)给扩张装置100的扩张气囊106充气以增加气囊中的压力,因此它使针通道平稳地和逐渐地膨胀成开孔通道。然后,也通过逐渐引入受控数量的流体(例如液体或气体)给扩张装置100的固位气囊108充气以增加气囊中的压力,因此它平稳地和逐渐地膨胀。当固位气囊108变得大于扩张气囊106并膨胀到充满气的状态时,它通过使胃壁“SW”紧靠在腹壁“AW”上而稳定胃壁“SW”,如图4B中所示。根据本发明的一个方面,该气囊充满气的直径可以从与待插入的导管装置(例如,PEG装置)的直径相匹配的范围中选择。扩张装置可以具有两个不同的串联的气囊;位于远端的扩张气囊(理想地是非顺应),和位于近端的单独的固位气囊(其也可以是非顺应)。具有非顺应气囊和单独的固位气囊的扩张装置的一个实例是单独的固位气囊固装到扩张装置的近端部分以帮助将装置留在患者胃部中,非顺应气囊固装在远端并且非顺应气囊用来使针通道膨胀成开孔通道,其中当非顺应气囊和单独的固位气囊都充满气时,非顺应气囊小于单独的固位气囊。
在扩张装置的固装的气囊充满气时,将剥脱鞘放置在扩张装置的最远端部分上面(即,从患者的外部)。将扩张装置的扩张气囊部分地放出少量的气以容许剥脱鞘越过扩张装置的远端和扩张气囊并通过开孔通道进入胃部。
接着,扩张装置让其气囊完全放气。因为它仍然安装在引导线上,所以通过经由内窥镜的工作通道收回引导线,可以通过内窥镜的工作通道取出扩张装置。替代地,可以通过在口腔从充气腔切断注射器充气连接器而取出扩张装置(如果使用这种充气腔的话)并经由鞘通过开孔位置将扩张装置拉出。注意,在图1和2中示出了扩张装置的不同充气腔构形。
然后,使导管(例如PEG装置)在引导线上面通过并通过剥脱鞘插入PEG装置的远端。将剥脱鞘与开孔通道分开并取出,取出任何其他的放置工具,并且PEG装置的远侧的停留端上的保持器将PEG装置保持在合适位置中。
替代地,可以不用剥脱鞘而将导管放到位。在扩张装置的固装的气囊充满气之后,扩张装置的扩张气囊仅仅放少量的气以容许导管越过扩张装置的远端并通过开孔通道进入胃部中。
接着,使扩张装置的气囊或多个气囊完全放气。由于它仍然安装在引导线上,所以可以通过经由内窥镜的工作通道收回引导线而经由内窥镜的工作通道取出扩张装置。替代地,可以通过扩张装置在口腔从充气腔切断注射器充气连接器而取出扩张装置(如果使用这种充气腔的话)并经由导管通过开孔位置将扩张装置拉出。注意,在图1和2中示出了扩张装置的不同充气腔构形。
尽管已经结合某些优选实施例描述了本发明,但应该懂得,本发明包含的主题不局限于那些具体实施例,相反,发明的主题应包括可包含在下面权利要求的精神和范围内的所有替换物、变型和等同方案。
Claims (14)
1.一种扩张装置,用于将至胃腔的针通道膨胀成开孔通道,所述扩张装置包括:
具有长度、宽度和纵轴的管状支撑件,所述管状支撑件限定了通过该装置的连续通道;
位于所述管状支撑件上的可充气扩张气囊,所述扩张气囊轴向地定向在所述管状支撑件上用于将至胃腔的针通道膨胀成开孔通道,所述可充气扩张气囊具有在3到10毫米范围内的充气直径;
扩张气囊充气腔;
位于所述管状支撑件上、与所述扩张气囊相邻的可充气固位气囊,所述固位气囊在充满的、不受限的充气状态下具有一直径以便稳定腔壁,该直径大于所述扩张气囊在充气状态时的最大直径;和
固位气囊充气腔;
所述可充气扩张气囊具有安装到所述管状支撑件上的两个开口端并且所述可充气固位气囊具有安装到所述管状支撑件上的两个开口端,至少一个开口端内翻以在所述扩张气囊和所述固位气囊之间提供较近的安装。
2.如权利要求1所述的扩张装置,其中所述可充气固位气囊构形成在充满的、不受限的充气状态下其直径比所述可充气扩张气囊在充气状态下的最大直径大1.5倍到3倍。
3.如权利要求1所述的扩张装置,其中所述可充气扩张气囊具有安装到所述管状支撑件上的两个开口端并且至少一个开口端内翻以在充气状态时提供与所述可充气固位气囊较近的安装。
4.如权利要求1所述的扩张装置,其中所述可充气固位气囊具有安装到所述管状支撑件上的两个开口端并且至少一个开口端内翻以在充气状态时提供与所述可充气扩张气囊较近的安装。
5.如权利要求1所述的扩张装置,其中所述可充气扩张气囊和所述可充气固位气囊各自由不同材料制成。
6.一种扩张装置,用于将至胃腔的针通道膨胀成开孔通道,所述扩张装置包括:
具有长度、宽度和纵轴的管状支撑件,所述管状支撑件限定了通过该装置的连续通道;
至少两个串联的可充气气囊,至少第一气囊轴向地定向在所述管状支撑件上并且具有安装到所述管状支撑件上的两个开口端,形成该装置的扩张区域用于将至胃腔的针通道膨胀成开孔通道,所述第一气囊具有在3到10毫米范围内的充气直径,以及至少第二气囊具有安装到所述管状支撑件上的两个开口端,形成固位区域限定该装置的第二部分;
用于每个可充气气囊的至少一个气囊充气腔,
其中至少一个开口端内翻以在所述第一气囊和所述第二气囊之间提供较近的安装并且所述固位区域构形成在充满的、不受限的充气状态下具有一直径,该直径比所述扩张区域在充气状态时的最大直径大1.5倍到3倍。
7.如权利要求6所述的扩张装置,其中所述第二气囊具有安装到所述管状支撑件上的两个开口端并且至少一个开口端内翻以在充气状态时提供与所述第一气囊较近的安装。
8.如权利要求7所述的扩张装置,其中所述第二气囊轴向地定向在所述管状支撑件上。
9.一种用于扩张至胃腔的开孔并插入饲喂管的系统,该系统包括:
开孔扩张装置,包括:
具有长度、宽度和纵轴的管状支撑件,所述管状支撑件限定通过该装置的连续通道;
可充气扩张气囊,其位于所述管状支撑件上用于扩张开孔通道;
扩张气囊充气腔;
可充气固位气囊,其位于所述管状支撑件上、邻近所述扩张气囊,
所述固位气囊用于稳定与所述开孔通道连通的胃腔的一部分;和
固位气囊充气腔;和
饲喂管,其构形成通过扩张的开孔通道安装在所述完全或部分充气的扩张气囊上面并进入由所述固位气囊稳定的所述胃腔部分,
其中所述开孔扩张装置构形成被放气且该装置的至少一部分通过所述饲喂管收回。
10.如权利要求9所述的系统,其中所述扩张气囊轴向地定向在所述管状支撑件上。
11.一种用于扩张至胃腔的开孔并插入饲喂管的系统,该系统包括:
开孔扩张装置,包括:
具有长度、宽度和纵轴的管状支撑件,所述管状支撑件限定了通过该装置的连续通道;
可充气扩张气囊,其位于所述管状支撑件上用于扩张开孔通道,所述可充气扩张气囊具有在3到10毫米范围内的充气直径;
扩张气囊充气腔;
可充气固位气囊,其位于所述管状支撑件上、邻近所述扩张气囊,所述固位气囊构形成通过在充满的、不受限的充气状态下具有一直径来稳定与所述开孔通道连通的胃腔的一部分以便稳定腔壁,该直径大于所述扩张气囊的最大直径;和
固位气囊充气腔;和
饲喂管,其构形成通过扩张的开孔通道安装在所述完全或部分充气的扩张气囊上面并进入由所述固位气囊稳定的所述胃腔部分,
其中所述开孔扩张装置构形成被放气且该装置的至少一部分通过所述饲喂管收回。
12.如权利要求11所述的系统,其中所述可充气扩张气囊轴向地定向在所述管状支撑件上。
13.如权利要求11所述的系统,其中所述可充气固位气囊轴向地定向在所述管状支撑件上。
14.如权利要求11所述的系统,其中所述可充气固位气囊具有安装到所述管状支撑件上的两个开口端并且至少一个开口端内翻以在充气状态时提供与所述可充气扩张气囊较近的安装。
