使用vActRIIB的体内处理
根据下文描述的操作,所有下述动物研究使用成熟的截短的vActRIIB-IgG2Fc(E28W)多肽(SEQIDNO:91)来进行。
在抑制素-α缺陷小鼠中的肌肉消耗的处理
抑制素-α是天然存在的激活素A抑制剂。缺乏抑制素-α的小鼠显示在循环中显著升高的激活素A水平,且患有与自发性肿瘤形成相关的致死性消耗综合征,所述肿瘤例如卵巢、睾丸癌和肾上腺癌(Matzuk等人,PNAS91(19):8817-21(1994),Cipriano等人Endocrinology121(7):2319-27(2000),Matzuk等人,Nature360(6402):313-9(1992))。对于下述实验,抑制素-α敲除小鼠(C57BL/6J)得自CharlesRiverLaboratories。在抑制素-α敲除小鼠中检查vActRIIB-IgG2FcE28W(SEQIDNO:91)(下文为E28W、或E28W多肽、或可溶性受体E28W)对体重和肌肉量的影响。执行在8周龄的雄性抑制素-α敲除小鼠中的14天单次注射研究。在8周龄时,与年龄匹配的野生型同窝对照小鼠比较,雄性抑制素-α敲除小鼠已失去超过25%的体重。在第0天时,5只敲除小鼠给予E28W(30mg/kg)的单次皮下注射,而5只敲除小鼠皮下施用相同体积的PBS(媒介物)。作为基线对照,在第0天时,5只年龄匹配的野生型小鼠施用媒介物的单次皮下注射。小鼠在第0天、第7天和第14天时进行称重。在第14天结束时,处死所有小鼠,并且经由尸检分析其无脂肪尸体重量和腓肠肌量。在14天的研究时期,媒介物处理的敲除小鼠的平均体重下降约1.1g,从第0天时的22.5g到第14天时的21.4g。相比之下,E28W处理的敲除小鼠的平均体重显示11g的显著增加,从第0天时的22.1g到第14天时的33.1g。末期尸检分析揭示在抑制素-α敲除小鼠中,E28W多肽事实上使无脂肪尸体重量和腓肠肌量加倍,如下所示。与媒介物处理的敲除小鼠的约8.0g和媒介物处理的野生型对照小鼠的约12.1g比较,E28W处理的敲除小鼠的平均无脂肪尸体重量是约14.9g。与媒介物处理的敲除小鼠的约209mg和媒介物处理的野生型对照小鼠的约324mg比较,E28W处理的敲除小鼠的平均腓肠肌重量(来自两条腿)是约426mg。这些结果证实E28W多肽用于处理重量减轻和肌肉消耗的疾病状态的有效性,并且概括于下表中。

在雄性和雌性抑制素-αKO小鼠中分别检查E28W多肽的施用对睾丸和卵巢肿瘤的形成率的影响。在这个研究中,11只抑制素-α敲除小鼠,包括8周龄的雄性(n=5)和9周龄的雌性(n=6),用E28W(30mg/kg)的单次皮下注射进行处理,而另外11只抑制素-α敲除小鼠,包括年龄匹配的雄性(n=5)和雌性(n=6),接受相同体积的PBS(媒介物)的单次注射。此外,11只野生型同窝对照小鼠,包括年龄匹配的雄性(n=5)和雌性(n=6),施用媒介物的单次注射。处理后2周,处死小鼠且实施尸检,以检查肉眼可鉴定的睾丸和卵巢肿瘤的形成率。观察到11只媒介物处理的敲除小鼠中的10只发展出可鉴定的肿瘤。特别地,分别在检查的5只雄性中的5只和6只雌性中的5只中发现睾丸和卵巢肿瘤形成。发现这些肿瘤的大小比野生型对照小鼠中的相应正常睾丸或卵巢大2-3倍。这显示于图3中。仅10%(11只中的1只)的E28W处理的抑制素-α敲除小鼠显示可见的肿瘤形成。特别地,在雌性中,6只E28W处理的敲除小鼠中的1只发展出可鉴定的卵巢肿瘤,而6只未处理的雌性敲除中的5只与年龄匹配的野生型对照比较,在卵巢的大小或总体形态学中具有很少的改变或无改变。5只E28W处理的雄性敲除小鼠中的5只都未显示出可见肿瘤,与年龄匹配的野生型对照比较,在睾丸的大小或总体形态学中具有很少的改变或无改变。这些结果证实E28W施用在减少抑制素-αKO小鼠中的睾丸和卵巢肿瘤形成中有效,提示可溶性受体疗法在癌症治疗中的临床功用。
在雄性抑制素-α敲除小鼠中检查E28W多肽在治疗厌食症中的有效性。在这个研究中,与年龄匹配的野生型小鼠(n=10)比较,抑制素-α敲除小鼠(n=5)中的食物消耗显著减少。观察到E28W处理的抑制素-α敲除小鼠的食物消耗在检查的3周时期过程中显著增加。E28W处理的敲除小鼠中的平均每周食物摄入增至略微高于年龄匹配的野生型对照小鼠中的摄入的水平,并且比用媒介物处理的敲除小鼠的平均每周食物摄入高约50%。因此,数据显示E28W处理在改善抑制素-αKO小鼠中的厌食症中高度有效。
在雄性和雌性抑制素-αKO小鼠中分别检查E28W处理对存活的影响。对于雄性,约50日龄的25只抑制素-αKO小鼠施用E28W多肽(10mg/kg/wk,SC),而26只年龄匹配的抑制素-αKO小鼠接受媒介物(PBS)。19只年龄匹配的野生型雄性小鼠接受媒介物且用作基线对照。媒介物处理的敲除小鼠在研究的第15天(约65日龄)时开始死亡。到实验的第34天(约84日龄)时,50%媒介物处理的敲除小鼠已死亡,并且到第78天(约128日龄)时,其中100%已死亡。相比之下,25只用E28W多肽处理的敲除小鼠或19只用媒介物处理的野生型对照小鼠无一在研究的第78天(约128日龄)前死亡。在E28W处理的敲除小鼠中,25只中的1只在研究的第78天(约128日龄)时死亡,并且25只中的24只存活超过第100天(约150日龄)。无媒介物处理的野生型小鼠在100天的研究阶段中死亡。类似存活研究结果在雌性抑制素-αKO小鼠中获得。约50日龄的22只雌性抑制素-αKO小鼠用E28W(10mg/kg/wk,SC)进行处理,而23只相同年龄的雌性抑制素-αKO小鼠用PBS(媒介物)进行处理。同时,17只野生型雌性对照小鼠用媒介物进行处理。媒介物处理的雌性敲除小鼠在研究的第40天(约90日龄)时开始死亡。到实验的第58天(约108日龄)时,50%媒介物处理的雌性敲除小鼠已死亡,并且到第86天(约136日龄)时,其中100%已死亡。相比之下,仅约5%(22只中的1只)E28W处理的雌性敲除小鼠已死亡,而约90%(22只的20只)存活超过研究的第120天(约170日龄)。无媒介物处理的野生型小鼠在120天研究阶段中死亡。因此,数据证实E28W多肽治疗在显著延长雄性和雌性抑制素-α敲除小鼠的存活中有效。关于雄性和雌性敲除小鼠的存活曲线的示意图表在图4中提供。
携带结肠-26肿瘤的小鼠中的肌肉消耗的治疗
携带结肠-26肿瘤的小鼠是广泛使用的用于研究癌症恶病质的临床前动物模型(Fujita等人,IntJCancer68(5):637-43(1996),Kwak等人,CancerResearch64(22):8193-8(2004))。E28W多肽对体重改变、肌肉量和存活率的影响在携带肿瘤的小鼠中进行研究。结肠-26(C-26)肿瘤细胞以0.5x106个细胞/小鼠皮下植入40只10周龄的、雄性CDF1小鼠内。肿瘤植入在第0天时执行。在肿瘤植入后第5天开始,20只C-26小鼠用10mg/kgvActRIIBIgG2FcE28W(SEQIDNO:91)的皮下注射每周进行处理,而20只C-26小鼠用媒介物(PBS)进行处理。同时,10只年龄和重量匹配的正常小鼠仅用媒介物(PBS)进行处理。体重和食物摄入每周测定3次。携带肿瘤的小鼠每天2次进行存活检查。使用与PC计算机连接的卡尺(Ultra-CalIVIP65电子测径器,FredVFowlerCo.BostonMA)测量肿瘤大小,并且数值自动记录至MicrosoftExcel数据文件中的工作表。如图5中所示,肿瘤植入后2周,携带C-26肿瘤的小鼠发展出严重恶病质,并且急剧丧失其体重。E28W处理有效缓和携带肿瘤的小鼠中的体重减轻。用E28W处理的携带肿瘤的小鼠的平均体重显著高于用媒介物处理的携带肿瘤的小鼠的平均体重(p<0.001,从肿瘤接种后第7天到第33天,不成对T检验,GraphpadSoftwareInc.SanDiegoCA)。
E28W多肽处理和媒介物处理的组之间的肿瘤大小不存在差异,表明治疗对C-26肿瘤生长无影响。末期尸检分析显示E28W处理的携带C-26肿瘤的小鼠的平均无脂肪尸体重量和腓肠肌重量显著高于用媒介物处理的携带肿瘤的小鼠的那些(对于无脂肪尸体和腓肠肌p<0.001)。E28W对携带C-26肿瘤的小鼠的存活的影响显示于图6中。媒介物处理的小鼠在肿瘤植入后约第14天时开始死亡。在肿瘤植入后第35天时,所有20只媒介物处理的携带C-26肿瘤的小鼠死亡;然而,20只用E28W处理的携带C-26肿瘤的小鼠中的17只仍存活。因此,E28W处理导致携带C-26肿瘤的小鼠的存活的显著延长(p<0.0001,卡方检验)。因此,E28W多肽不仅在维持体重和肌肉量中有效,而且在延长携带C-26肿瘤的小鼠的存活中有效。
后肢悬吊小鼠的处理
后肢悬吊小鼠模型用于检查vActRIIB-IgG2FcE28W(SEQIDNO:91)对疾病状态中的肌肉量的影响。后肢悬吊操作基本上与先前由CarlsonCJ等人(CarlsonCJ,BoothFW和GordonSE:AmJPhysiolRegulIntegrCompPhysiol.277:R601-RR606,1999)报告的相同。9周龄的雌性C57BL/6小鼠用于研究。总共60只小鼠如下分成3组:1.用媒介物(PBS)处理的未悬吊的基线对照组(20只小鼠),2.用媒介物处理的后肢悬吊组(20只小鼠),和3.用vActRIIB-IgG2Fc,E28W处理的后肢悬吊小鼠组(20只小鼠)。特别地,在后肢悬吊开始时,分别对上文描述的组给予30mg/kgvActRIIB-IgG2FcE28W或媒介物的单次SC注射。体重改变每周纵向测量2-3次。来自每个组的5只小鼠在下述4个不同时间点时处死:第1天、第3天、第7天和第14天。腓肠肌重量经由尸检进行测定。
如下表中所示,后肢悬吊导致体重最高达10%的显著减轻。用vActRIIB-IgG2FcE28W处理后肢悬吊小鼠导致显著的体重增加,其水平高于媒介物处理的后肢悬吊组或未悬吊的基线对照组,这是通过ANOVA测量分析的。在2周研究过程中,分别与媒介物处理的悬吊组中的0.2%下降和未悬吊基线对照组中的4.8%重量增加比较,vActRIIB-IgG2FcE28W(SEQIDNO:91)处理组的平均体重增加是12.6%。时程尸检结果显示与体重平行改变的后肢肌肉量。用vActRIIB-IgG2Fc(E28W)处理悬吊小鼠,完全缓解肌肉丧失。因此,这个实验的结果显示E28W在与废用相关的肌肉萎缩治疗中有效。

OVX小鼠的处理
切除卵巢的雌性C57B16小鼠(OVX)考虑作为用于雌性性腺机能减退和骨质疏松症的模型。24只雌性C57B16小鼠在3月龄时切除卵巢,并且允许恢复3个月。在6月龄时,24只OVX小鼠以及24只年龄匹配的假手术对照C57B16小鼠在3个月的处理阶段中通过NMR和骨量(PIXImus-GELUNARCorporation)测量体重、肌肉和脂肪量的纵向改变。在该阶段结束时,处死动物,并且在末期尸检过程中收获骨组织,且实施FaxitronX射线和显微CT(FaxitronX-rayCorporation和GEMedical系统)分析。证实E28W变体受体(SEQIDNO:91)在增加体重特别是无脂肪骨骼肌量和骨量方面有效,同时使小鼠的脂肪含量减少至未切除卵巢的小鼠中可见的水平。特别地,在12周的阶段中,与对用E28W处理的假手术小鼠比较,与对OVX加媒介物或假手术加媒介物的无脂肪肌肉量基本上无增加比较(对于OVX加媒介物约19克或对于野生型加媒介物约20克),无脂肪肌肉量对于用E28W处理的OVX小鼠从20g增至27.0g。在相同研究中,到12周研究结束时,用E28W处理的OVX小鼠显示减少的脂肪量,从平均8g/动物到平均约4g/动物,与假手术的动物相似。相比之下,用媒介物处理的OVX小鼠在研究过程中的任何时间时未丧失脂肪量。最后,与媒介物处理的OVX小鼠比较,骨量在用E28W处理的OVX小鼠中增加。通过pQCT(外周定量计算机体层摄影)分析测定在末期尸检过程中收获的解剖骨的股骨/胫骨BMC(骨矿物质含量)分析。在12周研究结束时,用E28W处理的OVX小鼠使BMC从约0.045g/cm增至约0.055g/cm,这与假手术的媒介物处理动物的最终BMC相似。在12周研究结束时,用媒介物处理的OVX小鼠显示约相同的0.045g/cm的BMC。在12周研究结束时,E28W处理的野生型小鼠显示BMC从约0.054g/cm到约0.065g/cm的增加。这些研究证实E28W多肽作为老年人中的脆性、骨质疏松症和肥胖的潜在治疗的有效性。
