技术领域
本发明涉及一种内窥镜,它具有一个柔性的、具有多个腔的导管探头。
背景技术
这种内窥镜已由DE 100 45 036 C1公开。公开的该内窥镜具有一个多腔探头以及一个设置在近侧探头端部上的手柄。在至少一个探头腔中有一光具。此外,设置了一个用于外科器械的工作腔。一个例如呈金属拉线或拉绳形式的控制元件与远侧的探头端部连接并且可沿轴向方向在该探头上运动地导向。由此可使远侧具有一定长度的探头部分弯曲和转动。以这种方式获得一种治疗用的内窥镜,该内窥镜在外科手术时可简单地操作。

发明内容
因此,本发明的任务在于,提供一种具有可控制的柔性的多腔探头的内窥镜,其中,可实现简单的维护,尤其是排除消毒时所出现的困难。
根据本发明,该任务通过下述方案解决。
按照本发明,提出了一种内窥镜,具有:一个柔性的并且具有多个腔的导管探头、一个设置在近侧探头端部上的手柄、一个设置在该导管探头的至少一个光具腔中的光具、至少一个用于外科器械的工作腔和一个控制元件,该控制元件固定在远侧的探头端部上或该探头端部附近以使该探头端部弯曲并且该控制元件在轴向上在该探头上可运动地被导向,其中:突出于导管探头的近侧端部之外的所述光具在一个柔性的软管中可运动地被导向,该软管在其纵向上弹簧弹性地构造并且在一个固定部位上与该光具固定连接,并且该光具的远侧端部通过该软管被压在一个封闭所述光具腔的远侧端部的可透光的盖上。
在本发明中,光具的突出于导管探头近侧端部之外的近侧端部段在一个柔性的软管中被导向。该光具尤其是构造为光纤光具,优选包括观察光具和照明光具。可弯曲的软管在其纵向上弹性地构成并且可例如用硅橡胶制成。在一个远离导管探头近侧端部的固定部位上,软管与光具固定连接。软管与光具的固定部位优选位于光具的近侧端部上。由于软管的弹性,在光具上在其纵向上作用一个预压力,使得光具的远侧端部被压在一个可透光的盖上,该盖作为封闭元件设置在导管探头内的光具腔的远侧端部上。该弹簧弹性的软管在其远侧的软管端部和与光具固定的部位之间被夹紧,该远侧的软管端部可拆开地固定在近侧的探头端部的区域中。

例如呈玻璃挡板或塑料挡板形式的可透光的密封装置或者说盖密封地嵌入到腔材料中。光具腔在远侧端部上被相对于目标地点密封地封闭。该可透光的盖也可具有光学特性、尤其是成像特性并且例如构造成透镜。呈挡板或透镜形式的该可透光的盖可例如通过粘接、焊入到尤其用塑料制成的探头材料中或者在探头模塑成形时通过挤压、压塑或类似方法密闭密封地嵌入到光具腔的远侧开口中。该光具、尤其是照明光具和观察光具布置得可在光具腔中和在软管中移动并且可从该光具腔和软管中取出。
在光具的近侧端部上可设置一个接头件,在该接头件上可连接照明装置和/或目镜。为了连接照明装置和目镜,该接头件具有合适的转接装置。也可以在接头件上、从而在光具的照明光纤上连接一个应急照明装置作为照明装置,该应急照明装置在需要时用蓄电池工作。目镜通过该接头件连接在光具的观察光纤股上。
软管的近侧端部与光具连接的固定部位可以设置在该接头件上并且优选构造为路厄锁连接。由于光具与软管之间的连接可拆开,光具可以从导管探头的光具腔中和软管中取出。除了作用在光具上的预压作用外软管还在导管探头的近侧端部与接头件之间对光具提供保护。
软管基于其弹性构成一个长度补偿装置,该长度补偿装置以一定预压力如弹簧一样作用在该光具的光导体束/光纤束上。远侧的光具端部被压在光具腔远侧端部上的可透光的盖上并且在远侧的探头部分弯曲时通过该弹性的预压力来补偿长度改变。该光具保持以一个确定的压紧力靠置在所述盖上。当该远侧的探头端部返回与其余探头部分对正的起始位置时,该光具股通过在该光具腔中逆着该弹性预压力移动又复位到起始位置中。
导管探头优选可转动地支承在手柄上。为此导管探头的近侧端部可以支承在手柄上的一个枢轴承中。该枢轴承优选具有一个纵向孔,该纵向孔与导管的控制装置腔对齐,控制元件布置在该控制装置腔中。这样,控制元件可通过该枢轴承被一直引导到手柄中或手柄上的一个滑动件。控制元件与该滑动件固定连接。通过该滑动件,控制元件可在轴向上移动以便使远侧的探头部分弯曲。通过导管探头相对于手柄的转动可以附加控制弯曲的探头端部。该枢轴承使导管探头可相对于手柄360°旋转。
导管探头在枢轴承上的固定借助可拆开的固定装置、尤其是路厄锁连接实现。控制元件在滑动件上的固定也借助可拆开的固定装置、例如借助夹紧螺钉实现。
导管探头优选只在引导线腔的区域内与手柄上的枢轴承可拆开地连接。其它探头腔的近侧出口不与手柄连接。这些探头腔出口独立于手柄并且在手柄外部可连接在对应的终端仪器上。通过软管导向的光具如上面已解释过的那样在手柄外部连接在照明系统、例如应急照明装置上和连接在目镜上,该目镜优选可固定在手柄上。观察光具也可以公知方式通过目镜连接到照相机/监视器系统上或连接到合适的观察装置上。
此外,探头的冲洗腔的近侧端部上的冲洗出口可连接在一个冲洗/抽吸系统上。此外,用于一个或多个外科器械的一个或多个工作腔可连接在一些操作元件上,借助这些操作元件操作相应的外科元件。为此,相应的外科元件可取出地在对应的工作腔中被导向。
该多腔导管探头优选构造成一次性部件。为此,该探头可通过压塑技术或挤压技术或通过其它合适的成型技术制造并且用塑料制成。此外,在该探头的近侧端部上可设置一个用坚固材料、例如塑料制成的附件,多个探头腔的腔出口和用于控制元件的可与手柄上的枢轴承连接的引导装置出口设置在该附件上。这些腔出口和该引导装置出口可优选具有用于路厄锁、卡接式锁或类似装置的接头件或者构造成这类闭锁装置的联接元件并且必要时用该近侧附件的坚固材料构成。优选该近侧附件也可构造成压塑成型件或挤压成型件,它与柔性探头共同构成一个一次性部件。
可从工作腔中取出的外科器械的消毒可以以简单的方式实施。从光具腔中取出的光具不会在外科手术期间被污染,因为该光具腔的远侧端部被该远侧端部上的可透光的密封装置相对于目标地点保护,周围的探头材料在纵向延伸长度上保护该光具。因为该导管探头优选构造成一次性部件,所以为了重新使用可将一个还未使用过的新的探头与手柄如上所述地连接,其中,光具可以可重复使用地装到光具腔中。
该导管探头优选可具有一个气球,通过导管探头中的气球腔可对该气球导送膨胀介质例如一种气体或一种液体,以使气球扩张。

控制元件可在导管探头内的一个柔性支撑管内被导向。该支撑管在导管探头的远侧端部前面离开距离地终止。由此,待弯曲的导管部分的柔性和可弯曲性不会受到该支撑管的影响。到远侧端部的距离大致相当于要通过该控制元件弯曲的远侧探头部分的长度。
有利的是,用于不同使用目的的不同导管探头可以与所述手柄、光具和目镜一起用在诊断和治疗中。手柄、光具和目镜构成可重复使用的模件式组件,它们可与长度、直径不同以及腔数量不同和腔的直径尺寸不同、尤其是工作腔直径尺寸不同的导管一起组合成可多重使用的内窥镜(多用途内窥镜)。例如也可以在工作腔内直接加工上、尤其是装配上极不相同的器械,这样,内窥镜导管与器械一起构成一个一次性系统,该系统不会引起消毒问题。通过该可导入的模件式光具得到一种可控制和可旋转的内窥镜检查器械(可视器械)。此外可实现作为一次性部件的气球导管,它带有可重复使用的不被污染的光具,该光具可从气球导管中取出。

具体实施方式
在图1中示出的内窥镜实施例具有一个手柄12和一个可拆卸地固定在该手柄上的导管探头1。该探头1被构造成多腔探头(图3)并且可具有例如一个或多个用于外科器械的工作腔29及至少一个用于光具6的光具腔5。此外还可设置一个用于气球17的膨胀的气球腔18(图4)。也可为由照明光具和观察光具组成的光具6设置分开的光具腔。同样可以为冲洗和抽吸在该探头1中设置独立的腔。
此外,该导管探头1包括一个例如呈拉绳或拉线形式的控制元件13。如例如由DE 100 45 036 C1所公开的那样,该长形的控制元件与远侧的探头端部20固定连接或固定在该探头端部附近并且在轴向方向上沿着该探头延伸并且可在该探头上运动地被导向。通过该控制元件13可使导管探头1的远侧端部段弯曲。从图5和6中可看到,控制元件13穿过支撑管23延伸,该支撑管布置在导管探头的控制装置腔25内。支撑管23也柔性地构成。支撑管23与远侧的探头端部20隔开一定距离地终止。该距离大致相当于要通过控制元件13弯曲的探头部分24的长度。该要弯曲的远侧的探头部分24比导管探头的支撑管23在其内延伸的那个探头部分具有更高的柔性。支撑管23在其远侧端部的区域中例如借助胶粘剂在固定部位28处固定在导管探头1上。固定部位28仅延伸在支撑管23的一个小的轴向段上。支撑管23的其余长度可运动地布置在导管探头的控制装置腔25内。由此,该导管探头在其长度的极大部分上保持柔性。导管探头在要弯曲的远侧探头部分24的区域中比支撑管23在其内延伸的其余探头部分具有更高的柔性和可弯曲性。
优选构造为拉线的控制元件13固定在远侧的探头端部20上。在图5所示实施例中该固定借助胶粘剂27实现。在图6所示实施例中该固定借助热缩软管26实现。通过热缩软管26使从内部穿过探头壁的开口30引出的拉线端部31被压到远侧探头端部的外侧面上。贯通的控制装置腔25的远侧端部被一个封闭元件32封闭。可借助控制元件13弯曲的远侧的探头部分24也可以按由DE 201 18 886 U或DE 19928 272 A1公开的方式构造。
该导管探头1由一种可弯曲的材料、尤其是由生物相容的塑料制成。优选该导管探头作为一次性部件构成,它在外科使用后被从手柄12上拆下并处理掉。在重新进行外科手术时,在手柄12上固定一个新的导管探头,它作为一次性部件无菌地保持待用。
该导管探头1在近侧的端部上具有一个由一种刚性的坚固材料构成的探头附件33。该材料同样也可为一种塑料材料。在该探头附件33上具有一些近侧的腔出口34,35,36和37。控制元件13穿过腔出口34。腔出口35例如配置给气球腔18,腔出口36例如配置给工作腔29,腔出口37例如配置给光具腔5。这些出口以公知方式装备有例如用于卡接式锁、路厄锁的联接元件或装备有类似的联接和接头件。
该导管探头1通过用于控制元件13的刚性的、细管状的腔出口34无相对转动地与手柄12上、尤其是手柄壳体上的枢轴承14连接(图2)。在所示出的实施例中手柄12在其前端部上具有枢轴承14,该枢轴承可通过一个可拆开的固定装置15与设置在探头附件33上的腔出口34连接。该可拆开的固定装置15可构造为卡接式锁或路厄锁或类似装置。通过该可拆开的固定装置15在手柄12上的枢轴承14与该导管探头1之间构成无相对转动的连接。以这种方式可使枢轴承14的例如用手操作的转动件38的旋转运动传递到导管探头1上并且尤其是传递到远侧的探头部分24上,该探头部分带有必要时弯曲的、远侧的探头端部20。基于例如探头附件33的造型或者基于导管探头1上或探头附件33上的标记可以识别导管探头的角度位置、特别是远侧的探头端部20的位置和光具6绕探头轴线的转角位置。导管探头1可以相对于手柄12在360°角度内旋转。可拆开的固定装置15具有一个固定在转动件38上联接元件40和一个固定在腔出口34上的联接元件39。在此可涉及卡接式锁或路厄锁或其它可拆开的连接装置的联接元件39,40。以此方式构成的可拆开的固定装置15具有贯通的轴向孔,该孔也穿过枢轴承14。控制元件13穿过该轴向孔延伸。
控制元件13的通过固定装置15和枢轴承14引导的近侧端部借助可拆开的、例如呈夹紧螺钉形式的固定器件41与一个支承得在轴向方向上可移动的滑移件16固定连接。
滑移件16在手柄12的壳体中可轴向运动地逆着一个预压力、例如一个预压弹簧42的预压力可在轴向方向上运动地被导向。在此,该预压力在朝向手柄12的前(远侧的)端部的方向上作用。在滑移件处于最前端位置时,远侧的探头端部20相对于其余的探头区域不弯曲。在滑移件16抵抗弹簧42的预压力朝向手柄12的后(近侧的)端部方向移动时,该运动通过控制元件13传递到远侧的探头端部20上并且该远侧的探头端部根据该移动运动的距离弯曲。滑移件16的操作可例如借助一个与固定器件41连接的操作元件进行。例如夹紧螺钉的头可构造为操作元件。但也可以设置一个附加的操作元件。该操作元件为此可通过一个在手柄12的轴向方向上延伸的纵向缝槽43伸入手柄壳体中,如在图2中对起到可拆开的固定器件41的作用的夹紧螺钉所示出的那样。该纵向缝槽43可在其两个端部上构成止挡,用于限制滑移件16的运动和限制控制元件13的轴向控制运动。
为了产生轴向的滑移件运动,也可设置一个曲柄传动装置,借助该曲柄传动装置将在手柄外部上产生的摆动运动或者旋转运动转换为轴向的、直线的滑移件运动。带有这种曲柄传动装置的手柄已在DE103 51 013 A1(PCT/EP03/12897)中说明并且可用在图1的实施例中。
此外可在手柄12上、尤其在后(近侧的)端部上优选可摆动地布置一个未详细示出的目镜架。
如图1所示,该导管探头1借助可拆开的固定装置15仅通过用于控制元件13的近侧探头出口34与手柄12连接。其余的出口35、36及37独立于手柄12并且可直接通过合适的连接装置与对应的终端仪器连接。例如用于气球腔18的近侧腔出口35借助一个相应的接头件连接在未详细示出的膨胀装置上,该膨胀装置提供液体的或气体的膨胀介质。此外,通过近侧的腔出口36可在工作腔29中可取出地插入外科器械,例如具有柔性轴的钻头,如同例如由DE 101 078 156 A1所公开的那样,或者具有用于取出异物及破碎异物的器械,尤其是在机械碎石时使用的器械。
此外可通过腔出口36在工作腔29上连接一个冲洗或抽吸装置。
通过腔出口37可将该包括一个照明光具和一个观察光具的光具6插入到探头1的光具腔5中。可以为这两股照明光具和观察光具设置分开的光具腔。光具腔5的相应的远侧端部被过一个可透光的盖7密封地封闭。以这种方式可避免光具6在目标地点被污染。光具6的其余部分通过由导管探头1构成的外罩保护,以免被污染。光具6可例如借助一个放置在光具出口11上的光具滑移件26在光具通道5中向前被一直移动到可透光的盖7,该可透光的盖可构成为玻璃挡板。

当连接软管的远侧端部与腔出口37和近侧的软管端部8与接头件10时,将软管2旋转,使得在光具6上在其纵向上作用一个预压力。通过该预压力使远侧的光具端部4压到远侧光具腔端部上的盖7上。通过该预压力可保证即使在远侧的探头部分24弯曲时远侧的光具端部4也始终压在可透光的盖7上,由此保证可无缺陷地观察目标地点。通过由软管2促成的弹性预压力保证了在远侧的探头端部20弯曲时光具可自动移动。当探头端部20返回到其直线位置时,光具6逆着预压力移动回来。由此保证了光具6始终用其远侧端部以一定压力靠置在盖7上。
照明光具股的近侧端部可通过接头件10与一个照明装置、例如一个应急照明装置连接。接头件10为此具有一个相应的转接装置44。光具6的观察光具股的近侧端部可与一个观察装置连接。该观察装置包含一个目镜,该目镜可通过接头件10借助合适的转接装置45、例如卡接式锁连接到光具6的观察光具股上。此外,观察装置可构造成照相机/监视器系统。

引导线11可穿过在远侧的探头端部20上敞开的引导线腔19。由此可以将导管探头1的远侧区域和尤其是要弯曲的远侧探头部分24以及气球17的区域借助该引导线11导入病人的身体中。引导线腔19从远侧探头端部20看在气球17的后面终止并通过导管壁22中的出口孔21通向外部。该出口孔可规定在近侧探头端部的附近。从远侧探头端部20到出口孔21的距离可以例如为230mm。
由于导管探头1和近侧的探头附件33连同从属的腔出口34至37可作为一次性部件例如用压塑技术制造,因此只有每次使用过的外科器械需要消毒处理。当集成有或加工有器械时,还可省去对器械的消毒处理。手柄12这样构造,使得它可与不同构造的、具有根据控制元件13类型的控制元件的导管探头一起使用。

Claims (19)
1. 内窥镜,具有:一个柔性的并且具有多个腔的导管探头、一个设置在近侧探头端部上的手柄、一个设置在该导管探头的至少一个光具腔中的光具、至少一个用于外科器械的工作腔和一个控制元件,该控制元件固定在远侧的探头端部上或该探头端部附近以使该探头端部弯曲并且该控制元件在轴向上在该探头上可运动地被导向,其特征在于,
突出于导管探头(1)的近侧端部之外的所述光具(6)在一个柔性的软管(2)中可运动地被导向,
该软管(2)在其纵向上弹簧弹性地构造并且在一个固定部位(3)上与该光具(6)固定连接,并且
该光具(6)的远侧端部通过该软管(2)被压在一个封闭所述光具腔(5)的远侧端部(4)的可透光的盖(7)上。
2. 根据权利要求1的内窥镜,其特征在于,该固定部位(3)设置在软管(2)的近侧端部(8)上。
3. 根据权利要求1的内窥镜,其特征在于,在光具(6)的近侧端部(9)上设置一个接头件(10),它可连接到照明装置上和/或目镜上。
4. 根据权利要求3的内窥镜,其特征在于,固定部位(3)设置在接头件(10)上。
5. 根据权利要求1至4之一的内窥镜,其特征在于,柔性的软管(2)安置在手柄(12)的外部。
6. 根据权利要求1至4之一的内窥镜,其特征在于,导管探头(1)在其近侧端部上可转动地支承在手柄(12)上的一个枢轴承(14)中,控制元件(13)通过该枢轴承被可移动地导向。
7. 根据权利要求6的内窥镜,其特征在于,在枢轴承(14)上设置一个用于导管探头(1)的可拆开的固定装置(15)。
8. 根据权利要求6的内窥镜,其特征在于,该枢轴承具有一个可用手操作的转动件(38),该转动件不相对转动地与导管探头(1)连接。
9. 根据权利要求7的内窥镜,其特征在于,控制元件(13)穿过所述固定装置(15)。
10. 根据权利要求1至4之一的内窥镜,其特征在于,控制元件(13)的近侧端部与一个支承在手柄(12)上的滑动件(16)可拆开地连接。
11. 根据权利要求1至4之一的内窥镜,其特征在于,导管探头(1)具有一个气球(17),通过导管探头(1)中的气球腔(18)可对该气球导送膨胀介质。
12. 根据权利要求11的内窥镜,其特征在于,一个引导线(11)通过引导线腔(19)被导向,该引导线腔从远侧的探头端部(20)起一直延伸到导管壁(22)内一个位于气球(17)后面的出口孔(21)。
13. 根据权利要求1至4之一的内窥镜,其特征在于,控制元件(13)安置在柔性的支撑管(23)内并且与远侧的探头端部(20)离开一个确定距离地终止,该支撑管安装在导管探头(1)的控制装置腔(25)内,其中,该确定距离大致相当于要通过该控制元件(13)弯曲的远侧探头部分(24)的长度。
14. 根据权利要求1至4之一的内窥镜,其特征在于,控制元件(13)通过热缩软管(26)或通过胶粘剂(27)固定在导管探头(1)的远侧端部上。
15. 根据权利要求13的内窥镜,其特征在于,支撑管(23)在固定部位(28)上在轴向上固定并且该支撑管(23)的其余管部分被支承得相对于控制装置腔(25)的内壁可运动。
16. 根据权利要求15的内窥镜,其特征在于,固定部位(28)设置在支撑管(23)的远侧端部上或该端部附近。
17. 根据权利要求1至4之一的内窥镜,其特征在于,所述外科器械可从所述至少一个工作腔(29)中取出或者加工到或集成到导管探头(1)中。
18. 根据权利要求1至4之一的内窥镜,其特征在于,导管探头(1)构造为一次性部件。
19. 根据权利要求1至4之一的内窥镜,其特征在于,导管探头(1)构造为压塑成型件或挤压成型件。
