Abstract
本発明は、難治性がん、特に免疫チェックポイント阻害薬の単独療法やそれと他剤との併用が無効ながんに対して治療効果を示す、新規かつ有効ながん免疫療法剤を提供する。具体的には、免疫チェックポイント阻害薬と、インターロイキン-18(IL-18)と、T細胞増殖因子(例、インターロイキン-2(IL-2))とを組み合わせてなる、がん治療剤を提供する。
Description
本発明は、インターロイキン-2(IL-2)等のT細胞増殖因子と、インターロイキン-18(IL-18)と、免疫チェックポイント阻害薬とを組み合わせてなる、がんの治療剤に関する。
肝臓がんは、肝切除や肝移植、穿刺局所療法、肝動脈化学塞栓療法(TACE)など非薬物療法が中心で、それらに適応がない進行性の肝細胞がんに対しては、血管内皮増殖因子(VEGF)阻害薬であるソラフェニブやレンバチニブによる薬物療法が行われているが、最新薬のレンバチニブでも奏効率は40%程度である。最近になって、免疫チェックポイント阻害薬である抗PD-1抗体(ニボルマブ、ペムブロリズマブ)が、ソラフェニブ投与歴のある肝細胞がんに対する2次治療薬として米国で承認されたが、ペムブロリズマブは、プラセボ対照の第III相試験で、全生存期間と無増悪生存期間という主要評価項目を達成できなかった。VEGF阻害薬との併用等も研究されているが、ペムブロリズマブとレンバチニブとの併用での奏効率は6割程度にとどまり、未だ十分な効果があるとはいえない。
炎症性サイトカインは、がん免疫に重要なT細胞やナチュラルキラー(NK)細胞を強力に活性化するため、抗腫瘍効果を期待してさまざまなサイトカイン療法が検討されてきた。IL-18はNK細胞、CD8陽性キラーT細胞(CTL)、γδT細胞などの増殖を著明に亢進する。しかし、IL-18の単独投与は転移性メラノーマに対する第II相試験で十分な有効性を示せなかった。本発明者らの研究グループは以前、免疫チェックポイント阻害薬にIL-18を併用することにより、獲得免疫を亢進する能力が期待されるNK細胞(ヘルパーNK細胞)やCTLが増加し、免疫チェックポイント阻害薬の抗腫瘍効果が増強されることを報告している(特許文献1、非特許文献1)。
一方、IL-2は活性化T細胞・NK細胞・樹状細胞などにより産生され、CTLやNK細胞といった殺腫瘍作用を示す細胞の増殖・活性化を引き起こすことから、単独投与やIL-2により活性化したNK細胞との併用により、転移性腎癌や悪性黒色腫などの治療に臨床的に用いられている(非特許文献2)。本発明者らは、末梢血由来NK細胞をIL-18とIL-2を併用して培養すると、NK細胞の増殖・活性化を著明に促進することを見出した(非特許文献3)。
しかしながら、免疫賦活化作用を示すサイトカインが、一方で腫瘍増殖を促進する作用を示す場合がある。IL-2はナイーブT細胞からTh1、Th2、Th17、制御性T細胞(Treg)の分化・活性化を誘導するが、TregはCTL活性の抑制を増強させるため、in vivo投与によりin vitroでの効果が再現し得るか否かは全く予測できない。IL-18についても、抗腫瘍効果と腫瘍増殖促進効果の二面性があることが報告されている(非特許文献4)。また、がん微小環境では、IL-18に極めて高親和性のIL-18結合タンパク質(IL-18BP)の発現が増大しており、IL-18や抗PD-L1抗体による治療によりIL-18BPの発現がさらに増大することも報告されている(非特許文献5)。従って、免疫チェックポイント阻害薬とIL-18との併用が、実際のヒト臨床においてどれだけ有効であるかは未知数である。

本発明の目的は、難治性がん、特に免疫チェックポイント阻害薬の単独療法やそれと他剤との併用が無効ながんに対して治療効果を示す、新規かつ有効ながん免疫療法剤を提供することである。
治療困難な肝臓がんモデルとして自然発症型のマウスが使用されているが、当該モデルに対して有効な治療薬はほとんどなく、進行肝細胞がんに対する一次治療の標準薬であるレンバチニブも無効である。本発明者らは、P-糖タンパク質をコードするmdr2遺伝子を欠損させた自然発症型肝細胞がんモデルマウス(Mdr2 KOマウス)を用い、大腸がん細胞移入による腹膜播種モデルやメラノーマ細胞移入による肺転移モデルにおいて効果を認めた免疫チェックポイント阻害薬(抗PD-L1抗体)とIL-18の併用投与の治療効果を検討した。その結果、腫瘍サイズの増大傾向を認め、免疫チェックポイント阻害薬とIL-18の併用は、難治性の肝臓がんに対しては十分な効果を奏さないことが示唆された。
そこで、本発明者らは鋭意検討を重ねた結果、Mdr2 KOマウスに対して、上記2剤に加えてさらにT細胞増殖因子(例えば、IL-2)を併用投与すると、腫瘍サイズが縮小傾向を示すことを見出した。免疫チェックポイント阻害薬とIL-2の2剤併用によっては、腫瘍サイズの増大傾向を認め、十分な治療効果が得られなかった。3剤併用により、いずれの2剤併用と比較しても腫瘍マーカーであるα-フェトプロテイン(AFP)の血清レベルは有意に低下した。また、3剤併用により抗腫瘍性サイトカイン(インターフェロン-γ(IFN-γ)や腫瘍壊死因子-α(TNF-α))の産生は有意に増大し、いずれの2剤併用と比較しても増大傾向を示した。
3剤併用による腫瘍縮小、AFP値の低下、IFN-γ・TNF-α産生の増大効果は、抗CD8抗体の投与により無効化されたことから、3剤併用による治療効果には、少なくともCD8陽性T細胞が重要であると考えられた。また、3剤併用による腫瘍縮小効果は、抗アシアロGM1抗体の投与により無効化されたことから、3剤併用による治療効果には、NK細胞の関与も示唆された。
そこで、本発明者らは鋭意検討を重ねた結果、Mdr2 KOマウスに対して、上記2剤に加えてさらにT細胞増殖因子(例えば、IL-2)を併用投与すると、腫瘍サイズが縮小傾向を示すことを見出した。免疫チェックポイント阻害薬とIL-2の2剤併用によっては、腫瘍サイズの増大傾向を認め、十分な治療効果が得られなかった。3剤併用により、いずれの2剤併用と比較しても腫瘍マーカーであるα-フェトプロテイン(AFP)の血清レベルは有意に低下した。また、3剤併用により抗腫瘍性サイトカイン(インターフェロン-γ(IFN-γ)や腫瘍壊死因子-α(TNF-α))の産生は有意に増大し、いずれの2剤併用と比較しても増大傾向を示した。
3剤併用による腫瘍縮小、AFP値の低下、IFN-γ・TNF-α産生の増大効果は、抗CD8抗体の投与により無効化されたことから、3剤併用による治療効果には、少なくともCD8陽性T細胞が重要であると考えられた。また、3剤併用による腫瘍縮小効果は、抗アシアロGM1抗体の投与により無効化されたことから、3剤併用による治療効果には、NK細胞の関与も示唆された。
IL-18によるがん治療の障壁となっていることが示唆されるIL-18BPのMdr2 KOマウスにおける発現を調べると、血清中のIL-18BPのタンパク質レベルは、抗PD-L1抗体とIL-18との2剤併用では変化はなく、抗PD-L1抗体とIL-2との2剤併用ではむしろ増加したのに対し、3剤併用では有意に低下した。肝細胞がん患者及び肝内胆管がん患者の血清中のIL-18BPタンパク質レベルを調べたところ、肝細胞がん患者において、健常者のそれと比較して著明にIL-18BPレベルが上昇していたが、患者間でも最大4倍以上の差異が認められたことから、3剤併用療法はIL-18BPの血清レベルが上昇しているがん患者に有効であり得ることが示唆された。
本発明者らは、これらの知見に基づいてさらに研究の重ねた結果、本発明を完成するに至った。

本発明によれば、肝臓がんなどの難治性がん、特に免疫チェックポイント阻害薬の単独療法やそれと他剤との併用が無効ながんに対して有効な複合がん免疫療法剤が提供される。
本発明は、免疫チェックポイント阻害薬と、IL-18と、T細胞増殖因子とを組み合わせてなる、がん治療剤(以下、「本発明の併用剤」ともいう。)を提供する。即ち、本発明の併用剤は、有効成分として(a)免疫チェックポイント阻害薬、(b)IL-18、及び(c)T細胞増殖因子を含む複合がん免疫療法剤である。
(I)有効成分
(a)免疫チェックポイント阻害薬
本明細書において「免疫チェックポイント阻害薬」とは、T細胞又はNK細胞上に発現する免疫抑制性の共刺激分子(免疫チェックポイント分子)と、がん細胞や抗原提示細胞上に発現するそれらのリガンドとの結合を阻害し、免疫抑制シグナルの伝達を遮断してT細胞又はNK細胞の活性化抑制を解除する薬剤を意味する。
(a)免疫チェックポイント阻害薬
本明細書において「免疫チェックポイント阻害薬」とは、T細胞又はNK細胞上に発現する免疫抑制性の共刺激分子(免疫チェックポイント分子)と、がん細胞や抗原提示細胞上に発現するそれらのリガンドとの結合を阻害し、免疫抑制シグナルの伝達を遮断してT細胞又はNK細胞の活性化抑制を解除する薬剤を意味する。
本発明の併用剤の有効成分として用いられる免疫チェックポイント阻害薬としては、T細胞又はNK細胞上に発現する抑制性の共刺激分子(例えば、PD-1、CTLA4、TIM-3、LAG-3、TIGIT、CD96、BTLA、VISTA、KIR等)に結合して、それらのリガンド(例えば、PD-L1、PD-L2、CD80/86、CEACAM1、Galectin-9、MHCクラス-II分子、LSECtin、Galectin-3、CD155、CD112、CD113、CD111、HVEM、VSIG3等)との結合を阻害する物質や、該リガンドに結合して免疫チェックポイント分子との結合を阻害する物質であれば、特に制限はないが、好ましい一実施態様においては、免疫チェックポイント分子又はそのリガンドに対するブロッキング抗体を挙げることができる。好ましくは、抗PD-1抗体、抗PD-L1抗体、抗CTLA4抗体、抗PD-L2抗体、抗TIM-3抗体、抗LAG-3抗体、抗KIR抗体等、より好ましくは抗PD-1抗体、抗PD-L1抗体、抗CTLA4抗体等が挙げられる。
免疫チェックポイント分子又はそのリガンドに対する抗体は、ポリクローナル抗体、モノクローナル抗体の何れであってもよいが、好ましくはモノクローナル抗体である。これらの抗体は、自体公知の抗体または抗血清の製造法に従って製造することができる。抗体のアイソタイプは特に限定されないが、好ましくはIgG、IgMまたはIgA、特に好ましくはIgGが挙げられる。IgG1やIgG2のFc領域はADCC、ADCP、CDC等のエフェクター機能を有するので、がん細胞に発現するリガンドを標的とする抗体において好ましく用いられ得る。一方、IgG4はエフェクター機能が低いので、T細胞やNK細胞上に発現する免疫チェックポイント分子を標的とする抗体において好ましく用いられ得る。尚、CTLA4は制御性T細胞(Treg)でも強く発現しており、Tregの除去作用を期待して、抗CTLA4抗体としてIgG1やIgG2サブタイプを用いることもできる。
免疫チェックポイント分子又はそのリガンドに対する抗体は、標的抗原を特異的に認識し結合するための相補性決定領域(CDR)を少なくとも有するものであれば特に制限はなく、完全抗体分子の他、例えばFab、Fab'、F(ab’)2等のフラグメント、scFv、scFv-Fc、ミニボディー、ダイアボディー等の遺伝子工学的に作製されたコンジュゲート分子、あるいはポリエチレングリコール(PEG)等のタンパク質安定化作用を有する分子等で修飾されたそれらの誘導体などであってもよい。
好ましい一実施態様において、免疫チェックポイント分子又はそのリガンドに対する抗体は、ヒトを投与対象とする医薬品として使用されることから、該抗体(好ましくはモノクローナル抗体)はヒトに投与した場合に抗原性を示す危険性が低減された抗体、具体的には、完全ヒト抗体、ヒト化抗体、マウス-ヒトキメラ抗体などであり、特に好ましくは完全ヒト抗体である。ヒト化抗体およびキメラ抗体は、常法に従って遺伝子工学的に作製することができる。また、完全ヒト抗体は、ヒト-ヒト(もしくはマウス)ハイブリドーマより製造することも可能ではあるが、大量の抗体を安定に且つ低コストで提供するためには、ヒト抗体産生マウスやファージディスプレイ法を用いて製造することが望ましい。
免疫チェックポイント分子又はそのリガンドに対するモノクローナル抗体のいくつかは、既に医薬品として上市されているか臨床試験中であり、それらを使用することができる。例えば、抗PD-1抗体として、ニボルマブ(オプジーボ(登録商標))、ペンブロリズマブ(Keytruda(登録商標))、AMP-514(MEDI0680)、ビジリズマブ(CT-011)等、抗PD-L1抗体として、アテゾリズマブ(RG7446、MPDL3280A)、デュルバルマブ(MEDI4736)、アベルマブ(PF-06834635、MSB0010718C)、BMS-936559(MDX1105)等、抗CTLA4抗体として、イピリムマブ(ヤーボイ(登録商標))、トレメリムマブ等、抗TIM-3抗体として、MBG453等、抗LAG-3抗体として、BMS-986016、LAG525等、抗KIR抗体として、リリルマブ等が挙げられる。
あるいは、免疫チェックポイント分子に結合してがん細胞や腫瘍浸潤マクロファージ等の抗原提示細胞上に発現するそのリガンドとの結合を阻害する物質として、該リガンドの免疫チェックポイント分子との結合に必要な部分(例、細胞外ドメイン)を含み、かつ免疫抑制性シグナルを伝達する能力を有しないフラグメント、さらには、該フラグメントに免疫疲弊したエフェクター細胞を除去し得る分子をコンジュゲートさせた物質を用いることができる。後者の例として、PD-L1やPD-L2の細胞外ドメインとIgG1やIgG2抗体のFc領域との融合タンパク質(例、AMP-224等)を挙げることができる。
免疫チェックポイント分子に結合してそのリガンドとの結合を阻害する物質はまた、該リガンドと競合的に免疫チェックポイント分子に結合するアンタゴニストであってもよい。そのようなアンタゴニストは、免疫チェックポイント分子とそのリガンドとを用いた競合アッセイ系を構築し、化合物ライブラリーをスクリーニングすることにより取得することができる。
本発明の併用剤は、上記の免疫チェックポイント阻害薬のいずれか1種又は2種以上を組み合わせて用いることができる。
(b)IL-18

本発明の併用剤の有効成分として用いられるIL-18は、配列番号2に示されるアミノ酸配列と同一もしくは実質的に同一のアミノ酸配列を含む蛋白質である。IL-18は、ヒトや他の哺乳動物(例えば、ラット、マウス、サル、イヌ、ウシ、ウサギ、ブタ、ヒツジなど)のIL-18産生細胞(例、マクロファージ、樹状細胞、ミクログリア、滑膜線維芽細胞、上皮細胞等)もしくはそれを含む組織から単離・精製される蛋白質であってもよい。また、化学合成もしくは無細胞翻訳系で生化学的に合成された蛋白質であってもよいし、あるいは上記アミノ酸配列をコードする塩基配列を有する核酸を導入された形質転換体から産生される組換え蛋白質であってもよい。
配列番号2に示されるアミノ酸配列と実質的に同一のアミノ酸配列としては、配列番号2に示されるアミノ酸配列と80%以上、好ましくは90%以上、より好ましくは95%以上、特に好ましくは97%以上、最も好ましくは98%以上の同一性を有するアミノ酸配列などが挙げられる。ここで「同一性」とは、当該技術分野において公知の数学的アルゴリズムを用いて2つのアミノ酸配列をアラインさせた場合の、最適なアラインメント(好ましくは、該アルゴリズムは最適なアラインメントのために配列の一方もしくは両方へのギャップの導入を考慮し得るものである)における、オーバーラップする全アミノ酸残基に対する同一アミノ酸残基の割合(%)を意味する。本明細書におけるアミノ酸配列の同一性は、相同性計算アルゴリズムNCBI BLAST(National Center for Biotechnology Information Basic Local Alignment Search Tool)を用い、以下の条件(期待値=10;ギャップを許す;マトリクス=BLOSUM62;フィルタリング=OFF)にて計算することができる。
IL-18は、配列番号2に示されるアミノ酸配列と実質的に同一のアミノ酸配列を含み、かつ配列番号2に示されるアミノ酸配列を含む蛋白質と同質の活性を有する蛋白質である。ここで「活性」とは、例えば、受容体との結合活性や、T細胞やNK細胞の増殖・活性化促進活性等、がんの抑制に寄与する任意の活性を意味する。ここで「同質」とは、それらの活性が定性的に同じであること意味する。したがって、IL-18の活性は野生型IL-18と同等もしくはそれ以上であることが好ましいが、これらの活性の程度は異なっていてもよい。
あるいは、本発明で用いられるIL-18としては、例えば、(i)配列番号2に示されるアミノ酸配列中の1または2個以上(例えば1~30個程度、好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が欠失したアミノ酸配列、(ii)配列番号2に示されるアミノ酸配列に1または2個以上(例えば1~30個程度、好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が付加したアミノ酸配列、(iii)配列番号2に示されるアミノ酸配列に1または2個以上(例えば1~30個程度、好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が挿入されたアミノ酸配列、(iv)配列番号2に示されるアミノ酸配列中の1または2個以上(例えば1~30個程度、より好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が他のアミノ酸で置換されたアミノ酸配列、または(v)それらを組み合わせたアミノ酸配列を含有する蛋白質なども含まれる。
上記のようにアミノ酸配列が挿入、欠失または置換されている場合、その挿入、欠失または置換の位置は特に制限されないが、例えば、IL-18変異体として、配列番号2に示されるアミノ酸配列中、38位、68位、76位及び127位のシステイン(Cys)残基の少なくとも1つが他のアミノ酸(例、Ser、Ala、Asp、Thr、Val、Leu等)で置換され、安定性が向上した変異体(例えば、特許第4024366号参照)、C38S、C68S又はC68Dに加え、L144C又はD157Cのアミノ酸置換を有し、IL-18受容体及び/又はIL-18BPとの親和性が改変された変異体(例えば、特許第4753867号参照)、IL-18BPとの結合に重要なアミノ酸残基、例えば、配列番号2に示されるアミノ酸配列中、42位のGlu、85位のIle、87位のMet、89位のLys、96位のMet、130位のAsp、132位のLys、143位のPro、149位のMet及び189位のLeuの少なくとも1つのアミノ酸残基、好ましくはG42及び/又はK89が他のアミノ酸で置換され、IL-18BPとの親和性が低下した変異体(例えば、特表2004-530432号公報参照)、Y1X、L5X、K8X、M51X、K53X、S55X、Q56X、P57X、G59X、M60X、E77X、Q103X、S105X、D110X、N111X、M113X、V153X及びN155X(Xは野生型IL-18のアミノ酸残基とは異なるアミノ酸を示す)のうちの少なくとも1つの置換、好ましくはM51X、M60X、S105X、D110X及びN111X、あるいはM51X、K53X、Q56X、S105X及びN111Xの置換を有するIL-18BP低親和性変異体(例えば、特表2020-533301号公報参照)を挙げることができる。あるいは、本発明者らが創製したIL-18変異体(即ち、C38/68/76/127S-E6A-K53A、C68/76/127S-E6A-K53A-C38M、C38/68/76/127S-E6A-K53A-G3Y、C38/68/76/127S-E6A-K53A-G3L、C38/68/76/127S-E6A-K53A-S72Y、C38/68/76/127S-E6A-K53A-S72M、C38/68/76/127S-E6A-K53A-S72F)も好ましく用いられ得る。
本発明で用いられるIL-18は遊離体であってもよいし、塩であってもよい。そのような塩としては、生理学的に許容される酸(例、無機酸、有機酸)や塩基(例、アルカリ金属、アルカリ土類金属)などとの塩が用いられ、とりわけ生理学的に許容される酸付加塩が好ましい。このような塩としては、例えば、無機酸(例、塩酸、リン酸、臭化水素酸、硫酸)との塩、あるいは有機酸(例、酢酸、ギ酸、プロピオン酸、フマル酸、マレイン酸、コハク酸、酒石酸、クエン酸、リンゴ酸、蓚酸、安息香酸、メタンスルホン酸、ベンゼンスルホン酸)との塩などが用いられる。
IL-18は、前述したヒトや他の哺乳動物のIL-18産生細胞もしくは組織の細胞外マトリクスや培養上清から、自体公知の蛋白質の精製方法、例えば、逆相クロマトグラフィー、イオン交換クロマトグラフィー、アフィニティークロマトグラフィーなどのクロマトグラフィー等を用いて単離することができる。また、IL-18は、公知のペプチド合成法に従って製造することもできる。ペプチドの合成法としては、例えば、固相合成法、液相合成法のいずれによってもよい。即ち、IL-18を構成し得る部分ペプチドもしくはアミノ酸と残余部分とを縮合させ、生成物が保護基を有する場合は保護基を脱離することにより製造することができる。
好ましい実施態様においては、IL-18は、それをコードする核酸を含有する形質転換体を培養し、得られる培養物から分離精製することによって製造することができる。ここで核酸はDNAであってもRNAであってもよい。DNAの場合は、好ましくは二本鎖DNAである。IL-18をコードするDNAは、例えば、そのcDNA配列情報に基づいてオリゴDNAプライマーを合成し、IL-18を産生する細胞より調製した全RNAもしくはmRNA画分を鋳型として用い、RT-PCR法によって増幅することにより、クローニングすることができる。得られたcDNAを鋳型にして、自体公知の部位特異的変異誘発法を用いて、上記した各種変異を導入することができる。
IL-18をコードするDNAとしては、例えば、配列番号1に示される塩基配列を含有するDNA、または配列番号1に示される塩基配列とストリンジェントな条件下でハイブリダイズする塩基配列を含有し、前記した野生型IL-18と同質の活性を有する蛋白質をコードするDNAなどが挙げられる。配列番号1に示される塩基配列とストリンジェントな条件下でハイブリダイズできるDNAとしては、例えば、配列番号1に示される塩基配列と80%以上、好ましくは90%以上、さらに好ましくは95%以上、特に好ましくは97%以上、最も好ましくは98%以上の同一性を有する塩基配列を含有するDNAなどが用いられる。
本明細書における塩基配列の同一性は、相同性計算アルゴリズムNCBI BLAST(National Center for Biotechnology Information Basic Local Alignment Search Tool)を用い、以下の条件(期待値=10;ギャップを許す;フィルタリング=ON;マッチスコア=1;ミスマッチスコア=-3)にて計算することができる。
IL-18をコードするDNAは、好ましくは、配列番号1に示される塩基配列を有する野生型ヒトIL-18をコードするDNA、又は上述の種々のヒトIL-18変異体タンパク質をコードするDNAである。
本明細書における塩基配列の同一性は、相同性計算アルゴリズムNCBI BLAST(National Center for Biotechnology Information Basic Local Alignment Search Tool)を用い、以下の条件(期待値=10;ギャップを許す;フィルタリング=ON;マッチスコア=1;ミスマッチスコア=-3)にて計算することができる。
IL-18をコードするDNAは、好ましくは、配列番号1に示される塩基配列を有する野生型ヒトIL-18をコードするDNA、又は上述の種々のヒトIL-18変異体タンパク質をコードするDNAである。
IL-18をコードするDNAは、化学的にDNA鎖を合成するか、もしくは合成した一部オーバーラップするオリゴDNA短鎖を、PCR法やGibson Assembly法を利用して接続することにより、その全長をコードするDNAを構築することも可能である。化学合成又はPCR法もしくはGibson Assembly法との組み合わせで全長DNAを構築することの利点は、該DNAを導入する宿主に合わせて使用コドンをCDS全長にわたり設計できる点にある。異種DNAの発現に際し、そのDNA配列を宿主生物において使用頻度の高いコドンに変換することで、タンパク質発現量の増大が期待できる。使用する宿主におけるコドン使用頻度のデータは、例えば(公財)かずさDNA研究所のホームページに公開されている遺伝暗号使用頻度データベース(http://www.kazusa.or.jp/codon/index.html)を用いることができ、または各宿主におけるコドン使用頻度を記した文献を参照してもよい。
クローン化されたDNAは、目的によりそのまま、または所望により制限酵素で消化するか、リンカーを付加した後に、使用することができる。該DNAはその5’末端側に翻訳開始コドンとしてのATGを有し、また3’末端側には翻訳終止コドンとしてのTAA、TGAまたはTAGを有していてもよい。これらの翻訳開始コドンや翻訳終止コドンは、適当な合成DNAアダプターを用いて付加することができる。
IL-18をコードするDNAを含む発現ベクターは、例えば、IL-18をコードするDNAから目的とするDNA断片を切り出し、該DNA断片を適当な発現ベクター中のプロモーターの下流に連結することにより製造することができる。発現ベクターとしては、大腸菌由来のプラスミド(例、pBR322,pBR325,pUC12,pUC13);枯草菌由来のプラスミド(例、pUB110,pTP5,pC194);酵母由来プラスミド(例、pSH19,pSH15);昆虫細胞発現プラスミド(例、pFast-Bac);動物細胞発現プラスミド(例、pA1-11,pXT1,pRc/CMV,pRc/RSV,pcDNAI/Neo);λファージなどのバクテリオファージ;バキュロウイルスなどの昆虫ウイルスベクター(例、BmNPV,AcNPV);レトロウイルス、レンチウイルス、ワクシニアウイルス、アデノウイルス、アデノ随伴ウイルス、ヘルペスウイルスなどの動物ウイルスベクターなどが用いられる。
プロモーターとしては、遺伝子の発現に用いる宿主に対応して適切なプロモーターであればいかなるものでもよい。例えば、宿主が動物細胞である場合、SRαプロモーター、SV40プロモーター、LTRプロモーター、CMV(サイトメガロウイルス)プロモーター、RSV(ラウス肉腫ウイルス)プロモーター、MoMuLV(モロニーマウス白血病ウイルス)LTR、HSV-TK(単純ヘルペスウイルスチミジンキナーゼ)プロモーターなどが用いられる。他の宿主においても自体公知のプロモーターを適宜選択することができる。
プロモーターとしては、遺伝子の発現に用いる宿主に対応して適切なプロモーターであればいかなるものでもよい。例えば、宿主が動物細胞である場合、SRαプロモーター、SV40プロモーター、LTRプロモーター、CMV(サイトメガロウイルス)プロモーター、RSV(ラウス肉腫ウイルス)プロモーター、MoMuLV(モロニーマウス白血病ウイルス)LTR、HSV-TK(単純ヘルペスウイルスチミジンキナーゼ)プロモーターなどが用いられる。他の宿主においても自体公知のプロモーターを適宜選択することができる。
発現ベクターとしては、上記の他に、所望によりエンハンサー、スプライシングシグナル、ポリA付加シグナル、選択マーカー、SV40複製起点(以下、SV40 oriと略称する場合がある)などを含有しているものを用いることができる。選択マーカーとしては、例えば、ジヒドロ葉酸還元酵素遺伝子、アンピシリン耐性遺伝子、ネオマイシン耐性遺伝子等が挙げられる。
上記したIL-18をコードするDNAを含む発現ベクターで宿主を形質転換し、得られる形質転換体を培養することによって、IL-18を製造することができる。宿主としては、例えば、エシェリヒア属菌、バチルス属菌、酵母、昆虫細胞、昆虫、動物細胞などが用いられる。哺乳動物細胞としては、例えば、サルCOS-7細胞、サルVero細胞、チャイニーズハムスター卵巣細胞(以下、CHO細胞と略記)、dhfr遺伝子欠損CHO細胞(以下、CHO(dhfr-)細胞と略記)、マウスL細胞,マウスAtT-20細胞、マウスミエローマ細胞,ラットGH3細胞、ヒトFL細胞、HeLa細胞、HepG2細胞、HEK293細胞などが用いられる。他の宿主についても自体公知の細胞をそれぞれ適宜選択することができる。
形質転換は、宿主の種類に応じ、公知の方法に従って実施することができる。動物細胞は、例えば、細胞工学別冊8 新細胞工学実験プロトコール,263-267 (1995)(秀潤社発行)、ヴィロロジー(Virology),52巻,456 (1973)に記載の方法に従って形質転換することができる。
形質転換体の培養は、宿主の種類に応じ、公知の方法に従って実施することができる。
例えば、宿主が動物細胞である形質転換体を培養する場合の培地としては、例えば、約5~約20%の胎児ウシ血清を含む最小必須培地(MEM)、ダルベッコ改変イーグル培地(DMEM)、RPMI 1640培地、199培地、Ham’s F-12培地などが用いられる。培地のpHは、好ましくは約6~約8である。培養は、通常約30℃~約40℃で、約15~約60時間行なわれる。必要に応じて通気や撹拌を行ってもよい。
以上のようにして、形質転換体の細胞内または細胞外にIL-18を製造せしめることができる。
例えば、宿主が動物細胞である形質転換体を培養する場合の培地としては、例えば、約5~約20%の胎児ウシ血清を含む最小必須培地(MEM)、ダルベッコ改変イーグル培地(DMEM)、RPMI 1640培地、199培地、Ham’s F-12培地などが用いられる。培地のpHは、好ましくは約6~約8である。培養は、通常約30℃~約40℃で、約15~約60時間行なわれる。必要に応じて通気や撹拌を行ってもよい。
以上のようにして、形質転換体の細胞内または細胞外にIL-18を製造せしめることができる。
前記形質転換体を培養して得られる培養物からIL-18を自体公知の方法に従って分離精製することができる。このような方法としては、塩析や溶媒沈澱法などの溶解度を利用する方法;透析法、限外ろ過法、ゲルろ過法、およびSDS-ポリアクリルアミドゲル電気泳動法などの主として分子量の差を利用する方法;イオン交換クロマトグラフィーなどの荷電の差を利用する方法;アフィニティークロマトグラフィーなどの特異的親和性を利用する方法;逆相高速液体クロマトグラフィーなどの疎水性の差を利用する方法;等電点電気泳動法などの等電点の差を利用する方法;などが用いられる。これらの方法は、適宜組み合わせることもできる。
かくして得られるIL-18が遊離体である場合には、自体公知の方法あるいはそれに準じる方法によって、該遊離体を塩に変換することができ、IL-18が塩として得られた場合には、自体公知の方法あるいはそれに準じる方法により、該塩を遊離体または他の塩に変換することができる。
(c)T細胞増殖因子
本発明の併用剤に用いられるT細胞増殖因子としては、CTLやTh1細胞をはじめとするT細胞の増殖・活性化を引き起こす液性因子であれば特に制限はなく、例えば、IL-2、IL-15、IL-7、IL-9、IL-21等のサイトカインを挙げることができるが、好ましくはIL-2、IL-15、IL-7、より好ましくはIL-2が挙げられる。これらのT細胞増殖因子は1種のみを用いてもよいし、2種以上を組み合わせて用いてもよい。一実施態様においては、T細胞増殖因子はNK細胞の増殖も引き起こすことができる(例、IL-2、IL-15)。この場合、該T細胞増殖因子を「NK細胞及びT細胞増殖因子」と称することがある。
本発明の併用剤に用いられるT細胞増殖因子としては、CTLやTh1細胞をはじめとするT細胞の増殖・活性化を引き起こす液性因子であれば特に制限はなく、例えば、IL-2、IL-15、IL-7、IL-9、IL-21等のサイトカインを挙げることができるが、好ましくはIL-2、IL-15、IL-7、より好ましくはIL-2が挙げられる。これらのT細胞増殖因子は1種のみを用いてもよいし、2種以上を組み合わせて用いてもよい。一実施態様においては、T細胞増殖因子はNK細胞の増殖も引き起こすことができる(例、IL-2、IL-15)。この場合、該T細胞増殖因子を「NK細胞及びT細胞増殖因子」と称することがある。
IL-2は、活性化T細胞・NK細胞・樹状細胞により産生され、CTLやNK細胞といった殺腫瘍作用を示す細胞の増殖・活性化を引き起こす。ヒトIL-2は153アミノ酸からなる前駆体として産生され、N末端の20アミノ酸からなるシグナル配列が切断されて、133アミノ酸からなる成熟IL-2として分泌される。ヒトIL-2のアミノ酸配列情報は、例えば、UniProtKB(accession番号:P60568)を参照することができる。ヒト成熟IL-2のアミノ酸配列を配列番号4に示す。
IL-15は、単球、マクロファージ、樹状細胞等により産生され、CTLやNK細胞に対してIL-2と類似の作用を示す。ヒトIL-15は162アミノ酸からなる前駆体として産生され、N末端の29アミノ酸からなるシグナル配列が切断されて不活性なプロ体として分泌され、さらに19アミノ酸からなるプロペプチドが切断されて、114アミノ酸からなる成熟IL-15(活性体)となる。ヒトIL-15のアミノ酸配列情報は、例えば、UniProtKB(accession番号:P40933)を参照することができる。ヒト成熟IL-15のアミノ酸配列を配列番号6に示す。
IL-7は、間質細胞、細網線維芽細胞、胸腺上皮細胞等により産生され、T細胞の初期分化の促進、末梢T細胞の量的維持等に寄与している。ヒトIL-7は177アミノ酸からなる前駆体として産生され、N末端の25アミノ酸からなるシグナル配列が切断されて、152アミノ酸からなる成熟IL-7として分泌される。ヒトIL-7のアミノ酸配列情報は、例えば、UniProtKB(accession番号:P13232)を参照することができる。ヒト成熟IL-7のアミノ酸配列を配列番号8に示す。
本発明の併用剤の有効成分として用いられるIL-2、IL-15及びIL-7は、それぞれ配列番号4、6及び8に示される各アミノ酸配列と同一もしくは実質的に同一のアミノ酸配列を含む蛋白質である。IL-2、IL-15及びIL-7は、ヒトや他の哺乳動物(例えば、ラット、マウス、サル、イヌ、ウシ、ウサギ、ブタ、ヒツジなど)の各サイトカイン産生細胞(例えば、IL-2産生細胞として、T細胞、NK細胞、NKT細胞、活性化樹状細胞、肥満細胞等、IL-15産生細胞として、単球、マクロファージ、樹状細胞等、IL-7産生細胞として、間質細胞、細網線維芽細胞、胸腺上皮細胞等)もしくはそれを含む組織から単離・精製される蛋白質であってもよい。また、化学合成もしくは無細胞翻訳系で生化学的に合成された蛋白質であってもよいし、あるいは上記各アミノ酸配列をコードする塩基配列を有する核酸を導入された形質転換体から産生される組換え蛋白質であってもよい。
配列番号4、6又は8に示されるアミノ酸配列と実質的に同一のアミノ酸配列としては、配列番号4、6又は8に示されるアミノ酸配列と80%以上、好ましくは90%以上、より好ましくは95%以上、特に好ましくは97%以上、最も好ましくは98%以上の同一性を有するアミノ酸配列などが挙げられる。ここで「同一性」とは、IL-18について前記したのと同義である。
IL-2、IL-15及びIL-7は、それぞれ配列番号4、6及び8に示される各アミノ酸配列と実質的に同一のアミノ酸配列を含み、かつ配列番号4、6及び8に示される各アミノ酸配列を含む蛋白質と同質の活性を有する蛋白質である。ここで「活性」とは、例えば、受容体との結合活性や、T細胞やNK細胞の分化・増殖・活性化促進活性等、がんの抑制に寄与する任意の活性を意味する。ここで「同質」とは、それらの活性が定性的に同じであること意味する。したがって、IL-2、IL-15及びIL-7の活性は、それぞれ野生型IL-2、野生型IL-15及び野生型IL-7と同等もしくはそれ以上であることが好ましいが、これらの活性の程度は異なっていてもよい。
あるいは、本発明で用いられるIL-2、IL-15及びIL-7としては、例えば、(i)配列番号4、6及び8に示される各アミノ酸配列中の1または2個以上(例えば1~30個程度、好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が欠失したアミノ酸配列、(ii)配列番号4、6及び8に示される各アミノ酸配列に1または2個以上(例えば1~30個程度、好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が付加したアミノ酸配列、(iii)配列番号4、6及び8に示される各アミノ酸配列に1または2個以上(例えば1~30個程度、好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が挿入されたアミノ酸配列、(iv)配列番号4、6及び8に示される各アミノ酸配列中の1または2個以上(例えば1~30個程度、より好ましくは1~10個程度、さらに好ましくは1~数(5、4、3もしくは2)個)のアミノ酸が他のアミノ酸で置換されたアミノ酸配列、または(v)それらを組み合わせたアミノ酸配列を含有する蛋白質なども含まれる。
上記のようにアミノ酸配列が挿入、欠失または置換されている場合、その挿入、欠失または置換の位置は特に制限されないが、例えば、IL-2変異体として、配列番号4に示されるアミノ酸配列中、3位のThr、42位のPhe、45位のTyr及び72位のLeu残基の少なくとも1つが他のアミノ酸(例、Ala、Ser、Val等)で置換され、Treg上で発現する高親和性のαβγc受容体のIL-2Rα鎖に対する親和性が低下した変異体(例えば、特2020-33362号公報参照)等を挙げることができる。IL-15、IL-7についても、野生型と同等もしくはそれ以上の活性を有する自体公知の変異体を用いることができる。
本発明で用いられるIL-2、IL-15及びIL-7は遊離体であってもよいし、塩であってもよい。そのような塩としては、IL-18について前記したのと同様の塩が挙げられる。
IL-2、IL-15及びIL-7は、前述したヒトや他の哺乳動物の各サイトカイン産生細胞もしくは組織の細胞外マトリクスや培養上清から、自体公知の蛋白質の精製方法を用いて単離することができる。また、IL-2、IL-15及びIL-7は、公知のペプチド合成法に従って製造することもできる。このような蛋白質の精製方法、ペプチド合成法としては、IL-18について前記したのと同様の方法を用いることができる。
好ましい実施態様においては、IL-2、IL-15及びIL-7は、それをコードする核酸を含有する形質転換体を培養し、得られる培養物から分離精製することによって製造することができる。
IL-2、IL-15及びIL-7をコードするDNAとしては、例えば、それぞれ配列番号3、5及び7に示される各塩基配列を含有するDNA、または配列番号3、5及び7に示される各塩基配列とストリンジェントな条件下でハイブリダイズする塩基配列を含有し、前記した野生型IL-2、IL-15及びIL-7と同質の活性を有する蛋白質をコードするDNAなどが挙げられる。配列番号3、5及び7に示される各塩基配列とストリンジェントな条件下でハイブリダイズできるDNAとしては、例えば、配列番号3、5及び7に示される各塩基配列と80%以上、好ましくは90%以上、さらに好ましくは95%以上、特に好ましくは97%以上、最も好ましくは98%以上の同一性を有する塩基配列を含有するDNAなどが用いられる。
本明細書における塩基配列の同一性は、IL-18をコードするDNAの場合と同様にして計算することができる。
IL-2、IL-15及びIL-7をコードするDNAは、好ましくは、それぞれ配列番号3、5及び7に示される各塩基配列を有する野生型ヒトIL-2、IL-15及びIL-7をコードするDNA、又は上述の種々のヒトIL-2、IL-15及びIL-7変異体タンパク質をコードするDNAである。
本明細書における塩基配列の同一性は、IL-18をコードするDNAの場合と同様にして計算することができる。
IL-2、IL-15及びIL-7をコードするDNAは、好ましくは、それぞれ配列番号3、5及び7に示される各塩基配列を有する野生型ヒトIL-2、IL-15及びIL-7をコードするDNA、又は上述の種々のヒトIL-2、IL-15及びIL-7変異体タンパク質をコードするDNAである。
IL-2、IL-15及びIL-7をコードするDNAは、化学的にDNA鎖を合成するか、もしくは合成した一部オーバーラップするオリゴDNA短鎖を、PCR法やGibson Assembly法を利用して接続することにより、その全長をコードするDNAを構築することも可能である。
クローン化されたDNAは、目的によりそのまま、または所望により制限酵素で消化するか、リンカーを付加した後に、使用することができる。該DNAはその5’末端側に翻訳開始コドンとしてのATGを有し、また3’末端側には翻訳終止コドンとしてのTAA、TGAまたはTAGを有していてもよい。これらの翻訳開始コドンや翻訳終止コドンは、適当な合成DNAアダプターを用いて付加することができる。
IL-2、IL-15又はIL-7をコードするDNAを含む発現ベクターは、例えば、IL-2、IL-15又はIL-7をコードするDNAから目的とするDNA断片を切り出し、該DNA断片を適当な発現ベクター中のプロモーターの下流に連結することにより製造することができる。発現ベクター、プロモーターとしては、IL-18について前記したものが同様に好ましく用いられる。また、発現ベクターは、所望によりエンハンサー、スプライシングシグナル、ポリA付加シグナル、選択マーカー、SV40 oriなどを含有していてもよい。選択マーカーとしては、例えば、ジヒドロ葉酸還元酵素遺伝子、アンピシリン耐性遺伝子、ネオマイシン耐性遺伝子等が挙げられる。
上記したIL-2、IL-15又はIL-7をコードするDNAを含む発現ベクターで宿主を形質転換し、得られる形質転換体を培養することによって、IL-2、IL-15又はIL-7を製造することができる。宿主、形質転換法、形質転換体の培養法、得られたIL-2タンパク質の精製方法、遊離体から塩もしくは塩から遊離体への変換方法等は、IL-18について前記したもの又は方法が同様に好ましく用いられる。
T細胞増殖因子として、IL-2、Il-15及びIL-7を例に挙げて説明してきたが、当業者であれば、本明細書の記載に基づいて、他のT細胞増殖因子についても容易にそれらを取得することができる。
(II)本発明の併用剤
後述の実施例に示されるとおり、免疫チェックポイント阻害薬、IL-18及びT細胞増殖因子の3剤を組み合わせて用いることにより、治療効果を奏する薬剤がほとんどなく、難治性がんのモデルとなる自然発症型のがんモデルマウスにおいて、免疫チェックポイント阻害薬とIL-18、免疫チェックポイント阻害薬とT細胞増殖因子との併用に比べて、顕著に腫瘍の増殖を抑制することができる。従って、本発明の併用剤は、がんの治療剤として使用することができる。ここで「がんの治療」とは、腫瘍の増殖、がんの進展、及び浸潤・転移の抑制、がんの発症遅延、再発の抑制等をすべて包含する概念として用いられる。本発明の併用剤は、特に、免疫チェックポイント阻害薬の単独療法や免疫チェックポイント阻害薬とIL-18との併用療法を含む既存のがん治療に抵抗性であるか、当該がん治療が不適応のがんを有する対象(ヒト又は他の哺乳動物、好ましくはヒト)に対して、好ましく使用することができる。
後述の実施例に示されるとおり、免疫チェックポイント阻害薬、IL-18及びT細胞増殖因子の3剤を組み合わせて用いることにより、治療効果を奏する薬剤がほとんどなく、難治性がんのモデルとなる自然発症型のがんモデルマウスにおいて、免疫チェックポイント阻害薬とIL-18、免疫チェックポイント阻害薬とT細胞増殖因子との併用に比べて、顕著に腫瘍の増殖を抑制することができる。従って、本発明の併用剤は、がんの治療剤として使用することができる。ここで「がんの治療」とは、腫瘍の増殖、がんの進展、及び浸潤・転移の抑制、がんの発症遅延、再発の抑制等をすべて包含する概念として用いられる。本発明の併用剤は、特に、免疫チェックポイント阻害薬の単独療法や免疫チェックポイント阻害薬とIL-18との併用療法を含む既存のがん治療に抵抗性であるか、当該がん治療が不適応のがんを有する対象(ヒト又は他の哺乳動物、好ましくはヒト)に対して、好ましく使用することができる。
本発明の併用剤を用いることができるがん種は特に制限されず、任意のがんが挙げられる。例えば、上皮細胞由来の癌であり得るが、非上皮性の肉腫や血液がんであってもよい。より具体的には、例えば、消化器のがん(例えば、肝がん(肝細胞がん、胆管細胞がん)、胆嚢がん、胆管がん、膵がん、十二指腸がん、食道がん、胃がん、大腸がん(結腸がん、直腸がん)、肛門がん)、泌尿器のがん(例えば、腎がん、尿管がん、膀胱がん、前立腺がん、陰茎がん、精巣(睾丸)がん)、胸部のがん(例えば、乳がん、肺がん(非小細胞肺がん、小細胞肺がん))、生殖器のがん(例えば、子宮がん(子宮頸がん、子宮体がん)、卵巣がん、外陰がん、膣がん)、頭頸部のがん(例えば、上顎がん、咽頭がん、喉頭がん、舌がん、甲状腺がん)、皮膚のがん(例えば、基底細胞がん、有棘細胞がん)を含むが、これらに限定されない。
後述の実施例で使用したMdr2 KOマウスは、mdr2遺伝子によってコードされる、ATP結合カセット(ABC)トランスポーターのスーパーファミリーのメンバーであるiAbc4タンパク質の欠損により、慢性の胆管周囲性炎症及び胆汁うっ滞性肝臓疾患を発症し、肝細胞がんを自然発症する(Katzenellengoben et al., Mol. Cancer Res. 2007, 5(11): 1159-1170)。そのため、本発明の好ましい一実施態様においては、本発明の併用剤が対象とするがんは、炎症関連がん、好ましくは、胆汁、膵液などの消化液による慢性炎症が原因となりうる肝臓がん、膵臓がん、胆管がん、胆嚢がん、十二指腸がん等の消化器がん、あるいはB型・C型肝炎ウイルスなどのウイルス感染による炎症・線維症・肝硬変由来の肝臓がん(それらを原発性とする転移がんを含む)であり得る。
免疫チェックポイント阻害薬、IL-18及びT細胞増殖因子は低毒性であり、そのまま液剤として、または適当な剤型の医薬組成物として、ヒト又は他の哺乳動物に対して経口的または非経口的(例、血管内投与(静脈内投与、動脈内投与等)、皮下投与、皮内投与、腹腔内投与、筋肉注射、局所投与など)に投与することができる。
本発明の併用剤は、有効成分3剤をそれぞれ別個に製剤化してもよいし、それらの2剤以上を単剤として製剤化してもよい。
非経口投与のための組成物としては、例えば、注射剤、坐剤等が用いられ、注射剤は静脈注射剤、皮下注射剤、皮内注射剤、筋肉注射剤、点滴注射剤等の剤形を包含しても良い。このような注射剤は、公知の方法に従って調製できる。注射剤の調製方法としては、例えば、1以上の有効成分を、通常注射剤に用いられる無菌の水性液、または油性液に溶解、懸濁または乳化することによって調製できる。注射用の水性液としては、例えば、生理食塩水、ブドウ糖やその他の補助薬を含む等張液等が用いられ、適当な溶解補助剤、例えば、アルコール(例、エタノール)、ポリアルコール(例、プロピレングリコール、ポリエチレングリコール)、非イオン界面活性剤〔例、ポリソルベート80、HCO-50(polyoxyethylene(50mol)adduct of hydrogenated castor oil)〕等と併用してもよい。油性液としては、例えば、ゴマ油、大豆油等が用いられ、溶解補助剤として安息香酸ベンジル、ベンジルアルコール等を併用してもよい。調製された注射液は、適当なアンプルに充填されることが好ましい。直腸投与に用いられる坐剤は、有効成分を通常の坐薬用基剤に混合することによって調製されてもよい。
経口投与のための組成物としては、固体または液体の剤形、具体的には錠剤(糖衣錠、フィルムコーティング錠を含む)、丸剤、顆粒剤、散剤、カプセル剤(ソフトカプセル剤を含む)、シロップ剤、乳剤、懸濁剤等が挙げられる。このような組成物は公知の方法によって製造され、製剤分野において通常用いられる担体、希釈剤もしくは賦形剤を含有していてもよい。錠剤用の担体、賦形剤としては、例えば、乳糖、でんぷん、蔗糖、ステアリン酸マグネシウムが用いられる。
上記の非経口用または経口用医薬組成物は、有効成分の投与量に適合するような投薬単位の剤形に調製されることが好都合である。このような投薬単位の剤形としては、例えば、錠剤、丸剤、カプセル剤、注射剤(アンプル)、坐剤が挙げられる。免疫チェックポイント阻害薬は、投薬単位剤形当たり通常10~10000mg、好ましくは50~1000mg、より好ましくは100~1000mg含有されている。IL-18は、投薬単位剤形当たり通常1~100mg、好ましくは5~50mg含有されている。IL-2は、投薬単位剤形当たり通常104~106単位、好ましくは5×104~5×105単位含有されている。
各有効成分の投与量は、投与対象、対象疾患、症状、投与ルートなどによっても異なるが、例えば、成人の肝臓がんの治療のために使用する場合には、免疫チェックポイント阻害薬を1回量として、通常0.5~500mg/kg体重程度、好ましくは2.5~50mg/kg体重程度、より好ましくは5~50mg/kg体重程度、IL-18を1回量として、通常0.05~1mg/kg体重程度、好ましくは0.25~2.5mg/kg体重程度、IL-2を1回量として、通常5×102~5×104単位/kg体重程度、好ましくは2.5×103~2.5×104単位/kg体重程度を、1日1回~週1回程度、好ましくは週2~3回程度、静脈内又は腹腔内投与により投与するのが好都合である。他の非経口投与および経口投与の場合もこれに準ずる量を投与することができる。症状が特に重い場合には、その症状に応じて増量してもよい。
本発明の併用剤における有効成分の比率は、各有効成分の投与量が上記の範囲内にあれば特に制限はないが、例えば、IL-18の投与量を1とすると、免疫チェックポイント阻害薬の投与量は5~200程度、好ましくは10~50程度であり得る。また、T細胞増殖因子の投与量は、例えばIL-2の場合、1単位0.5ng換算で、IL-18の投与量1に対して、0.1~1程度、好ましくは0.2~0.5程度であり得る。
本発明の併用剤が、各有効成分がそれぞれ別個の組成物の形態、あるいは、いずれか2剤が単一の組成物の形態で、残りの1剤が別の組成物の形態で提供される場合、各組成物は、同時に又は時間差をおいて、同一経路又は別経路で対象に投与することができる。
IL-18BPはがん微小環境において発現が上昇しており、高い親和性でIL-18に結合してT細胞やNK細胞などのエフェクター細胞上のIL-18受容体との結合及びシグナル伝達を遮断して該エフェクター細胞の増殖・活性化を抑制する、可溶性の免疫チェックポイント分子として機能し得る。また、IL-18によるサイトカイン療法を受けたがん患者の血清中では、IL-18BPレベルが著明に上昇していることが報告されている。後述の実施例に示されるように、自然発症肝臓がんモデルマウスの血清ではIL-18BPレベルが上昇しており、免疫チェックポイント阻害薬とIL-18もしくはIL-2との2剤併用ではIL-BPレベルを低下させることはできないのに対し、3剤を組み合わせた本発明の併用剤は、IL-18BPレベルを著明に低下させることができる。また、IL-18療法を受けていない肝細胞がん患者の血清IL-18BPレベルは健常者のそれよりも著明に上昇しているが、患者間でもその数値は大きく異なることが明らかとなった。血清IL-18BPレベルの高値は、がん微小環境下でIL-18BPによる免疫抑制状態が成立していることを反映していると考えられる。そのため、血清IL-18BPレベルが特に高いがん患者に対して、IL-18BPの発現を低下させることのできる本発明の併用剤は、当該患者におけるIL-18による免疫抑制(T細胞やNK細胞等の免疫疲弊)を解除し得ると考えられるので、特に有用であり得る。実際、後述の実施例でも、個体数は少ないものの、自然発症肝臓がんモデルマウスにおいて、投与開始時の血清IL-18BPレベルが特に高い個体において、3剤投与後に顕著なIL-18BPレベルの低下が認められており、血清IL-18BPレベルを指標として本発明の併用剤が著効する対象を特定できる可能性が強く示唆される。
IL-18BPは健常者でも恒常的に発現しており、正常血清レベルは数pg/mL程度と言われている。従って、好ましい一実施態様において、本発明の併用剤は、例えば、血清IL-18BPレベルが10 pg/mL以上、好ましくは20 pg/mL以上のがん患者に対して投与され得る。血清IL-18BPレベルは、例えば、抗IL-18BP抗体を用いた免疫化学的アッセイ(例、ELISA等)により測定することができ、そのようなELISAキットは市販されている。
以下、実施例により、本発明を詳しく説明するが、本発明はこれらに限定されるものではない。
実施例1 Mdr2-/- KOマウスに対する免疫チェックポイント阻害薬とIL-18及び/又はIL-2との併用療法
肝細胞がんを自然発症したMdr2-/-KOマウス(60~64週齢、雌;The Jackson Laboratoryより入手)を4群に分け、
1)抗マウスPD-L1抗体(クローン10F.9G2;BioXcellより購入)200 μg/マウス、IL-18(組換えマウスIL-18;グラクソスミスクライン社製)10 μg/マウス、及びIL-2(遺伝子組換え型IL-2(イムネース注35);塩野義製薬株式会社製)5,000単位/kg(IL-2+IL-18+PD-L1投与群);
2)抗マウスPD-L1抗体 200 μg/マウス、及びIL-2 5,000単位/kg(IL-2+PD-L1投与群);
3)抗マウスPD-L1抗体 200 μg/マウス、及びIL-18 10 μg/マウス(IL-18+PD-L1投与群);又は
4)生理食塩水(Control(PBS投与)群);
を週2回の間隔で、4週間腹腔内投与した。薬剤投与開始4週間後に各併用剤の効果を調べた。本実験のスケジュールを図1に示す。
肝細胞がんを自然発症したMdr2-/-KOマウス(60~64週齢、雌;The Jackson Laboratoryより入手)を4群に分け、
1)抗マウスPD-L1抗体(クローン10F.9G2;BioXcellより購入)200 μg/マウス、IL-18(組換えマウスIL-18;グラクソスミスクライン社製)10 μg/マウス、及びIL-2(遺伝子組換え型IL-2(イムネース注35);塩野義製薬株式会社製)5,000単位/kg(IL-2+IL-18+PD-L1投与群);
2)抗マウスPD-L1抗体 200 μg/マウス、及びIL-2 5,000単位/kg(IL-2+PD-L1投与群);
3)抗マウスPD-L1抗体 200 μg/マウス、及びIL-18 10 μg/マウス(IL-18+PD-L1投与群);又は
4)生理食塩水(Control(PBS投与)群);
を週2回の間隔で、4週間腹腔内投与した。薬剤投与開始4週間後に各併用剤の効果を調べた。本実験のスケジュールを図1に示す。
(1)腫瘍増殖抑制効果
CTスキャン(実験動物用3DマイクロX線CT装置(R_mCT2-SP2):リガク社製)を用いて、薬剤投与開始時及び投与開始4週間後の肝臓のCT画像を撮影した。その結果、Control群では腫瘍サイズの増大傾向を認めたのに対し(図2-1)、IL-2+IL-18+PD-L1投与群では腫瘍サイズの縮小傾向が認められた(図3-1)。他方、IL-2+PD-L1投与群(図4)及びIL-18+PD-L1投与群(図5)では、腫瘍サイズの増大傾向を認め、免疫チェックポイント阻害薬といずれか一方のサイトカインとの2剤併用では腫瘍増殖抑制効果を奏しないことが明らかとなった。
CTスキャン(実験動物用3DマイクロX線CT装置(R_mCT2-SP2):リガク社製)を用いて、薬剤投与開始時及び投与開始4週間後の肝臓のCT画像を撮影した。その結果、Control群では腫瘍サイズの増大傾向を認めたのに対し(図2-1)、IL-2+IL-18+PD-L1投与群では腫瘍サイズの縮小傾向が認められた(図3-1)。他方、IL-2+PD-L1投与群(図4)及びIL-18+PD-L1投与群(図5)では、腫瘍サイズの増大傾向を認め、免疫チェックポイント阻害薬といずれか一方のサイトカインとの2剤併用では腫瘍増殖抑制効果を奏しないことが明らかとなった。
肝臓組織切片のHE染色の結果、Control群では、肝実質内に腺管様構造を伴う肝細胞がんを認めたが、腫瘍周辺への炎症細胞の浸潤は明らかでなかった(図2-2、A~C)。一方、IL-2+IL-18+PD-L1投与群では、肝実質内に腺管様構造を伴う肝細胞がんや壊死した組織を認めた。また、腫瘍周辺への炎症細胞の浸潤が顕著であり、リンパ球や単核球を認めた。さらに腫瘍内にも炎症細胞の浸潤を認め、壊死組織の周囲に肝細胞の再生像やリンパ球の浸潤を認めた(図3-2、A~C)。
(2)腫瘍マーカーの発現低下
投与開始4週間後に各群のマウスから採血し、常法により血清AFPレベルを測定した。その結果、IL-2+IL-18+PD-L1投与群では、IL-2+PD-L1投与群及びIL-18+PD-L1投与群と比較して、有意にAFPレベルの低下を認めた(図6A)。薬剤投与前後を比較すると、IL-2+IL-18+PD-L1投与群で併用投与によりAFP値の低下傾向を示した(図6B)。
投与開始4週間後に各群のマウスから採血し、常法により血清AFPレベルを測定した。その結果、IL-2+IL-18+PD-L1投与群では、IL-2+PD-L1投与群及びIL-18+PD-L1投与群と比較して、有意にAFPレベルの低下を認めた(図6A)。薬剤投与前後を比較すると、IL-2+IL-18+PD-L1投与群で併用投与によりAFP値の低下傾向を示した(図6B)。
(3)サイトカインの産生増強効果
投与開始4週間後に各薬剤投与群における血清中のIFN-γ及びTNF-αレベルを測定・比較した。その結果、2剤併用(IL-2+PD-L1投与群及びIL-18+PD-L1投与群)では、IFN-γ、TNF-αのいずれも有意にその産生を増大させることはできなかったが、3剤を併用(IL-2+IL-18+PD-L1投与群)すると、FN-γ及びTNF-αの産生が有意に増大した(図7、左)。薬剤投与前後を比較すると、IL-2+IL-18+PD-L1投与群で併用投与によりIFN-γ、TNF-αのいずれの血清レベルも有意に上昇した(図7、右)。
投与開始4週間後に各薬剤投与群における血清中のIFN-γ及びTNF-αレベルを測定・比較した。その結果、2剤併用(IL-2+PD-L1投与群及びIL-18+PD-L1投与群)では、IFN-γ、TNF-αのいずれも有意にその産生を増大させることはできなかったが、3剤を併用(IL-2+IL-18+PD-L1投与群)すると、FN-γ及びTNF-αの産生が有意に増大した(図7、左)。薬剤投与前後を比較すると、IL-2+IL-18+PD-L1投与群で併用投与によりIFN-γ、TNF-αのいずれの血清レベルも有意に上昇した(図7、右)。
(4)3剤併用の治療効果に対するT細胞及びNK細胞の寄与
IL-2+IL-18+PD-L1に加えて、NK細胞表面上に発現する糖脂質のアシアロGM1に対する抗体を投与して、NK細胞を除去したところ、薬剤投与前後の画像診断により腫瘍サイズが増大することが明らかとなった(図8)。このことから、3剤併用による腫瘍増殖抑制効果にNK細胞が役割を果たしていることが示唆された。
また、IL-2+IL-18+PD-L1に加えて、キラーT細胞(CTL)表面上に発現するCD8に対する抗体を投与して、CTLを除去したところ、薬剤投与前後の画像診断により腫瘍サイズが増大することが明らかとなった(図9-1)。さらに、投与開始4週間後の血清中のAFP、IFN-γ、TNF-αのレベルを比較すると、IL-2+IL-18+PD-L1+抗CD8抗体投与群では、IL-2+IL-18+PD-L1投与群と比較して、AFPレベルの有意な上昇と、IFN-γ、TNF-αレベルの有意な低下を認めた(図9-2)。このことから、3剤併用による腫瘍増殖抑制効果に、CTLが少なくとも炎症性サイトカインの産生増強作用を介して重要な役割を果たしていることが示された。
IL-2+IL-18+PD-L1に加えて、NK細胞表面上に発現する糖脂質のアシアロGM1に対する抗体を投与して、NK細胞を除去したところ、薬剤投与前後の画像診断により腫瘍サイズが増大することが明らかとなった(図8)。このことから、3剤併用による腫瘍増殖抑制効果にNK細胞が役割を果たしていることが示唆された。
また、IL-2+IL-18+PD-L1に加えて、キラーT細胞(CTL)表面上に発現するCD8に対する抗体を投与して、CTLを除去したところ、薬剤投与前後の画像診断により腫瘍サイズが増大することが明らかとなった(図9-1)。さらに、投与開始4週間後の血清中のAFP、IFN-γ、TNF-αのレベルを比較すると、IL-2+IL-18+PD-L1+抗CD8抗体投与群では、IL-2+IL-18+PD-L1投与群と比較して、AFPレベルの有意な上昇と、IFN-γ、TNF-αレベルの有意な低下を認めた(図9-2)。このことから、3剤併用による腫瘍増殖抑制効果に、CTLが少なくとも炎症性サイトカインの産生増強作用を介して重要な役割を果たしていることが示された。
(5)IL-18BPとの関係
投与開始4週間後に各薬剤投与群のマウスから血清を採取し、血清中のIL-18BPレベルを測定・比較した。その結果、Mdr2 KOマウスの血清中でIL-18BPレベルが上昇しており、がん微小環境においてIL-18BPが高発現していることが示唆された。2剤併用(IL-2+PD-L1投与群及びIL-18+PD-L1投与群)では、IL-18BPレベルを低下させることはできず、N数は少ないものの、IL-2+PD-L1投与群では薬剤投与によりむしろIL-18BPレベルは上昇した。これに対し、IL-2+IL-18+PD-L1投与群ではIL-18BPレベルを有意に低下させることができた(図10-1、左)。薬剤投与前後を比較すると、IL-2+IL-18+PD-L1投与群で併用投与によりIL-18BPレベルが有意に低下した(図10-1、右)。
3剤併用によるIL-18BP発現低下作用は、抗CD8抗体の投与により無効化された(図10-2)。このことは、IL-18BP発現低下作用にCTLが重要な役割を果たしていることを示唆している。
投与開始4週間後に各薬剤投与群のマウスから血清を採取し、血清中のIL-18BPレベルを測定・比較した。その結果、Mdr2 KOマウスの血清中でIL-18BPレベルが上昇しており、がん微小環境においてIL-18BPが高発現していることが示唆された。2剤併用(IL-2+PD-L1投与群及びIL-18+PD-L1投与群)では、IL-18BPレベルを低下させることはできず、N数は少ないものの、IL-2+PD-L1投与群では薬剤投与によりむしろIL-18BPレベルは上昇した。これに対し、IL-2+IL-18+PD-L1投与群ではIL-18BPレベルを有意に低下させることができた(図10-1、左)。薬剤投与前後を比較すると、IL-2+IL-18+PD-L1投与群で併用投与によりIL-18BPレベルが有意に低下した(図10-1、右)。
3剤併用によるIL-18BP発現低下作用は、抗CD8抗体の投与により無効化された(図10-2)。このことは、IL-18BP発現低下作用にCTLが重要な役割を果たしていることを示唆している。
東京都立駒込病院を受診したC型肝炎ウイルス(HCV)陽性の肝細胞がん患者69人、肝内胆管がん患者8人及び健常ボランティア24人から、インフォームドコンセントを得て採取した血清中のIL-18BPレベルを測定した。その結果、肝細胞がん患者では、健常人、肝内胆管がん患者に比べて血清IL-18BPレベルが著明に上昇していることが明らかとなった(図11-1)。また、肝細胞がん患者間でもIL-18BPレベルに最大4倍以上の差異が認められた。
一方、前記肝細胞がん患者69人と健康ボランティア24人から採取した血清中のIL-18レベルを測定したところ、肝細胞がん患者では、健常人に比べて血清IL-18レベルが有意に上昇していた(図11-2;慢性肝炎患者30人、肝硬変患者15人から採取した血清中のIL-18レベルの結果を合わせて示す)。しかし、肝細胞がん患者における血清IL-18レベルと血清IL-18BPレベルとの間には相関が認められなかった(図11-3)。
一方、前記肝細胞がん患者69人と健康ボランティア24人から採取した血清中のIL-18レベルを測定したところ、肝細胞がん患者では、健常人に比べて血清IL-18レベルが有意に上昇していた(図11-2;慢性肝炎患者30人、肝硬変患者15人から採取した血清中のIL-18レベルの結果を合わせて示す)。しかし、肝細胞がん患者における血清IL-18レベルと血清IL-18BPレベルとの間には相関が認められなかった(図11-3)。
本発明の併用剤は、肝臓がんなどの難治性がん、特に免疫チェックポイント阻害薬の単独療法やそれと他剤との併用が無効もしくは不適応ながんに対して有効な複合がん免疫療法剤となり得る点で極めて有用である。また、血清IL-18BPレベルを測定するコンパニオン診断の実施により、本併用剤がより著効するがん患者を予測することも可能となる。
本出願は日本で出願された特願2021-130294(出願日:2021年8月6日)を基礎としており、その内容は本明細書に全て包含されるものである。
Claims (8)
- 免疫チェックポイント阻害薬と、インターロイキン-18(IL-18)と、T細胞増殖因子とを組み合わせてなる、がん治療剤。
- T細胞増殖因子が、インターロイキン-2(IL-2)、インターロイキン-15(IL-15)及びインターロイキン-7(IL-7)からなる群より選択される1以上のサイトカインである、請求項1に記載の剤。
- T細胞増殖因子としてIL-2を含む、請求項2に記載の剤。
- 免疫チェックポイント阻害薬が、抗PD-L1抗体、抗PD-1抗体、抗CTLA4抗体、抗LAG-3抗体及び抗TIM-3抗体からなる群より選択される1以上の抗体である、請求項1~3のいずれか1項に記載の剤。
- 免疫チェックポイント阻害薬の単独療法又は免疫チェックポイント阻害薬とIL-18との併用療法に抵抗性のがんに対する、請求項1~4のいずれか1項に記載の剤。
- 血清IL-18結合タンパク質(IL-18BP)レベルが10 pg/mL以上である対象に投与される、請求項1~5のいずれか1項に記載の剤。
- がんが炎症関連がんである、請求項1~6のいずれか1項に記載の剤。
- 炎症関連がんが消化器がんである、請求項7に記載の剤。
