Abstract
본 발명은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)의 신장독성 완화 및 신장 섬유화 질환의 치료 용도에 관한 것으로, 본 발명의 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드는 산화 스트레스를 감소시키고 염증성 사이토카인을 감소시켜 신장독성 및 신장 섬유화 질환의 예방 및 치료에 유용하게 이용될 수 있다.
Description
본 발명은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)의 신장독성 완화 및 신장 섬유화 질환의 치료 용도에 대한 것이다.
신장은 생체의 항상성(homeostasis)을 유지하는 중요한 장기로, 체내 체액량, 혈액 내의 이온 농도와 pH를 조절하고, 대사성 노폐물, 독소, 약물 등의 노폐물을 배설하며, 혈압 조절 및 기타 대사성, 내분비 기능을 수행한다. 또한, 비타민 D를 활성화시켜서 소장에서 칼슘이 흡수되도록 도와주며 여러 가지 호르몬의 합성에도 관여한다.
이러한 신장이 배설, 조절, 대사 및 내분비적 기능을 정상적으로 수행하지 못하고 전체적으로 기능이 저하되거나 이상이 초래된 상태를 신장 질환이라고 한다. 신장의 손상으로 인한 기능의 저하는 신장 및 관련 구조의 증대, 신장의 위축, 체액량의 변화, 전해질 불균형, 대사성 산증, 가스교환장애, 항감염 기능 손상, 요독성 독소의 축적 등을 초래한다.
신장 질환은 진행 상태에 따라 급성신부전증, 만성신부전증으로 분류되며, 또는 발병 원인에 따라 혈관 복합체의 침착으로 인한 사구체 신염, 당뇨병에 수반되는 당뇨병성 신장 질환 또는 고혈압에 수반되는 고혈압성 신장 질환, 항생제 또는 항암제 등의 약물투여에 의한 독성신병증, 세균 감염 등으로 나뉜다. 원인이 되는 신장 질환의 종류에 관계없이 만성적으로 신기능 장애가 진행되어 사구체 여과율이 50% 이하로 감소하면, 대부분의 경우 계속적으로 사구체 여과율이 감소하게 되며, 궁극적으로 말기 신부전증에 도달하게 되고 혈액학적 이상, 신경계 합병증, 위장관계 합병증, 면역학적 합병증, 감염 또는 골이영양증 등의 합병증이 일어나 심한 경우 죽음에 이르게 된다.
신장질환의 유발원인은 매우 다양하고 복잡할 뿐 만 아니라 진행 단계에 따라 다른 기전이 관여하므로 질환의 진행 시점에 따른 치료제의 적용이 필요하다. 하지만 대부분의 약제는 질환의 진행과정 등을 고려하지 않은 것들이 많아 적절한 치료제가 없는 실정이다. 산화 스트레스는 중요한 신장질환의 유발원인일 뿐 만 아니라 심혈관손상의 중요위험 병인으로 밝혀져 있으나 이를 타겟팅하는 방법을 이용한 약제의 효과는 아직 명확하게 증명되지 않았다.
특히, 만성신장병의 발생기전 및 치료에 관한 기존의 연구는 주로 신장 섬유화 및 세포외 기질합성의 증가단계, 즉 신장의 조직학적 변화가 초래된 후기단계에 대한 실험연구에 집중되어 있다. 하지만 후기단계의 신장은 비가역적인 신장의 섬유화가 일어나 있어, 만성신장병을 보다 적절히 치료하기 위해서는 신장질환이 발생되고 초기 단계의 치료가 요구된다.
신장 섬유화증(신장 섬유화 질환)은 신장질환의 중요한 표지자이며, 말기 신부전의 매우 흔한 증상중의 하나이다. 일측성 요관폐쇄 동물모델은 신장의 섬유화를 유발하고, 세뇨관 세포의 손상, 상피 중배엽 세포 전이, 염증반응의 증가, 대식세포의 유입, 간질 섬유화세포의 증식 및 신장 기능의 저하하는 등의 특징을 보인다.
황칠나무(Dendropanax morbifera)는 두릅 나무과 (오갈피나무과) 황칠나무속의 한국 특산종으로서, 상록교목이며 잎은 표면에 털이 없고 매끈하며 어긋나고 달걀형 또는 타원형으로 잎몸이 전혀 갈라지지 않거나 3-5갈래로 손가락처럼 깊게 갈라지는 것도 있다. 우리나라의 황칠 나무의 분포도를 보면 제주도, 전남 완도, 대흑산도, 거문도 등 바닷가 일대에서 많이 자라고 있다. 황칠의 속명인 덴드로파낙스(Dendropanax)는 그리스어의 덴드로(dendro, 나무)와 파낙스(panax, 전지전능한 약)의 합성어이다. 황칠나무의 학명인 덴드로파낙스 모비페라는 만병통치나무라는 뜻을 가지고 있으며 나무 인삼이라고 불리우기도 한다. 또한 황칠이라는 이름은 황칠나무껍질에 상처를 내면 노란색의 액체가 마치 옻나무의 옻칠처럼 나온다고 하여 붙여진 이름이며 황칠은 공예품을 만드는 과정에서 표면을 가공하거나 색을 칠할 때 사용하고, 황제의 옷인 곤룡포, 나전칠기, 고려 불상 외 각종 가구 및 금속 등에 다양하게 사용되어져 왔다. 한국 본초 도감에 따르면 황칠나무의 뿌리와 줄기는 맛이 달고 성질은 따뜻하며 거풍습과 황혈맥 등에 효능이 있다.
또한, 최근 연구에 따르면 황칠과 황칠 속 단일 성분이 항균, 항암, 고지혈증, 항혈전 항염증 효과 및 항산화 효과 등의 약리효과가 밝혀지면서 더욱 관심을 가지고 많은 소재로 이용되고 있다.
이에, 본 발명의 발명자들은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드가 신장독성을 완화시키고, 신장 섬유화 질환에 치료 효능이 있음을 확인하여 본 발명을 완성하였다.
본 발명의 목적은 신장독성 또는 신장 섬유화 질환 예방 또는 치료용 약학적 조성물을 제공하는 것이다.
본 발명의 다른 목적은 신장독성 또는 신장 섬유화 질환 예방 또는 개선용 건강기능식품 조성물을 제공하는 것이다.
본 발명의 또 다른 목적은 신장독성 또는 신장 섬유화 질환 예방 또는 치료 방법을 제공하는 것이다.
상기와 같은 목적을 달성하기 위하여,
본 발명은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장독성 예방 또는 치료용 약학적 조성물을 제공한다.
또한, 본 발명은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장 섬유화 질환 예방 또는 치료용 약학적 조성물을 제공한다.
나아가, 본 발명은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장독성 예방 또는 개선용 건강기능식품을 제공한다.

본 발명의 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드는 천연물 유래 성분으로 인체에 독성이 없어 안전하다. 또한, 본 발명의 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드는 산화 스트레스를 감소시키고 염증성 사이토카인을 감소시켜 신장독성 및 신장 섬유화 질환의 예방, 개선 및 치료에 유용하게 이용 될 수 있는 효과가 있다.

이하에서는 도면을 참조하여 본 발명을 보다 상세하게 설명한다.
본 발명은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장독성 예방 또는 치료용 약학적 조성물 및 신장 섬유화 질환 예방 또는 치료용 약학적 조성물에 관한 것이다.
또한, 본 발명은 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장독성 또는 신장 섬유화 질환 예방 또는 개선용 건강기능식품에 관한 것이다.
본 발명에서 상기 "황칠나무"는 황칠나무(Dendropanax morbifera)의 수피, 꽃, 잎, 줄기, 열매, 가지, 종자 및 뿌리로 이루어진 군에서 선택된 것일 수 있으나, 이에 제한되지는 않는다. 바람직하게는 황칠나무의 잎(leaf) 및 줄기(stem)를 포함하는 지상부(aerial part)를 사용하는 것일 수 있다.
본 발명에서 상기 황칠나무 추출물은 황칠나무의 지상부를 적절한 용매로 추출한 것으로, 상기 추출하기 위한 적절한 용매는 약학적으로 허용되는 유기용매라면 어느 것을 사용해도 무방하며, 물 또는 유기용매를 사용할 수 있다. 예를 들어, 추출 용매로서 정제수, 메탄올(methanol), 에탄올(ethanol), 프로판올(propanol), 이소프로판올(isopropanol), 부탄올(butanol) 등을 포함하는 탄소수 1-4개의 무수 또는 함수 저급 알코올, 프로필렌글리콜, 부틸렌글리콜, 글리세린, 아세톤, 에틸 아세테이트, 부틸 아세테이트, 클로로포름, 디에틸에테르, 디클로로메탄, 헥산, 에테르, 벤젠, 메틸렌클로라이드 및 시클로헥산으로 이루어진 군으로부터 선택되는 어느 하나 이상의 용매를 단독으로 혹은 혼합하여 사용할 수 있다. 바람직하게는 물 또는 에탄올을 단독으로 혹은 혼합하여 사용할 수 있다.
추출 방법으로는 열수추출법, 냉침추출법, 환류냉각추출법, 용매추출법, 수증기증류법, 초음파추출법, 용출법, 압착법 등의 방법 중 어느 하나를 선택하여 사용할 수 있다. 또한, 목적하는 추출물은 추가로 통상의 분획 공정을 수행할 수도 있으며, 통상의 정제 방법을 이용하여 정제될 수도 있다. 본 발명의 황칠나무 추출물의 제조방법에는 제한이 없으며, 공지되어 있는 어떠한 방법도 이용될 수 있다.
본 발명에서 상기 덴드로파녹시드(dendropanoxide)는 도 1에 도시된 바와 같은 구조를 갖는 화합물로서, 황칠나무의 에탄올 추출물로부터 용매분획하여 분리될 수 있다. 예를 들어, 황칠나무의 지상부 에탄올 추출물의 디클로로메탄 분획물로부터 분리된 것일 수 있으나, 이제 제한되지 않는다.
본 발명에서 상기 신장독성 및 신장 섬유화는 항암제 또는 당뇨병에 의해 유발되는 것일 수 있다.
상기 항암제는 바람직하게는 시스플라틴(cisplatin)일 수 있고, 상기 당뇨병은 스트렙토조토신(streptozotocin)에 의해 유발되는 것일 수 있다.
본 발명의 일실시예에서는, 황칠나무 물추출물은 시스플라틴을 투여로 동물모델에서 유발된 급성 신장 독성(acute kidney injury) 관련 바이오마커인 혈액 요소 질소(blood urea nitrogen, BUN), 혈청 크레아티닌(serum creatinine, sCr) 및 전염증성 사이토카인(pro-inflammatory cytokines; IL-6 및 TNF-α)의 증가된 수준을 감소시키며, 증가된 신장 섬유증 관련 인자들의 발현을 감소시키고, 감소된 항산화 효소 활성을 증가시키며, 신장 자기세포사멸을 억제 및 근위 세뇨관에서 조직병리학적으로 확인된 신장 구조의 손상을 개선 시킬 수 있는 효과가 있음을 확인하였고, 시스플라틴의 항암 효과를 향상시키면서도 시스플라틴으로 유발된 신장독성(신독성)에 대한 신장 보호효과를 나타냄을 확인하였다(실시예 1 및 3 참조).
본 발명의 다른 일실시예에서는, 본 발명의 황칠나무 물 추출물이 스트렙토조토신에 의해 당뇨병이 유발된 동물모델에서 신장 및 췌장의 손상과 신장 섬유증 및 신장 자기세포사멸로부터 보호 효과를 나타내며, 관련 인자인 BUN, sCr, KIM-1, SBP1, PKM2 등의 섬유증 마커와 전염증성 사이토카인의 수준 또는 발현 증가를 감소시키고, 항산화 효소 활성 감소를 증가시킴으로써, 스트렙토조토신에 의한 당뇨병으로 유발된 신장 섬유증 및 당뇨병성 신병증(신장병증)에 대한 보효 효과를 나타냄을 확인하였다(실시예 1 및 4 참조).
본 발명의 또 다른 일실시예에서는, 본 발명의 황칠나무 에탄올 추출물로부터 분리된 덴드로파녹시드는 시스플라틴에 의해 유도된 세포 독성 및 세포 사멸로부터 NRK-52E 세포를 보호하고, 시스플라틴에 의해 혈청 또는 뇨(소변)에서 증가된 급성 신장 손상 관련 바이오마커(BUN, sCr,Kim-1, SBP-1, NGAL 등) 및 전염증성 사이토카인 수준을 유의하게 감소시켰으며, 항산화 효소 활성이 증가하고, 지질 과산화 수준을 개선시키고, AMPK/mTOR 신호 전달의 조절을 통해 신장 세포의 세포자기사멸을 억제시킴으로써, 덴드로파녹시드가 시스플라틴으로 유발된 신장독성(신독성)을 개선 및 치료할 수 있음을 확인하였다(실시예 2 및 5 참조).
따라서 본 발명의 조성물은 항암 보조제 또는 당뇨병 치료 보조제로도 적용하여 사용할 수 있다.
본 발명에서 "항암 보조제"는 항암제의 항암 치료 효과를 증대시키고, 항암제의 부작용을 억제하거나 개선시키기 위하여 사용될 수 있으며, 항암제와 병용하여 환자에게 투여될 수 있다.
본 발명의 상기 항암 보조제는 항암제인 시스플라틴에 의해 유발되는 신장독성 또는 신장 섬유화 증상을 억제하거나 개선시키는 효과를 나타내면서도 항암제의 항암 효과를 증대 또는 유지시키는 효과를 나타내는 것일 수 있다.
본 발명의 "당뇨병 치료 보조제"는 당뇨병 치료제의 당뇨병 질환 치료 효과를 증대시키거나, 당뇨병 합병증을 억제 또는 개선시키기 위하여 사용될 수 있으며, 당뇨병 치료제와 병용하여 환자에게 투여될 수 있다.
본 발명의 당뇨병 치료 보조제는 당뇨병 합병증으로 유발되는 신장독성 또는 신장 섬유화 증상을 억제하거나 개선시키는 효과를 발휘하는 것일 수 있다.
본 발명의 조성물은 약학적으로 유효한 양의 화합물 또는 추출물을 단독으로 포함하거나 하나 이상의 약학적으로 허용되는 담체, 부형제 또는 희석제를 포함할 수 있다. 상기에서 약학적으로 유효한 양이란 질환의 증상을 예방, 개선 및 치료하기에 충분한 양을 말한다.
또한, 상기에서 "약학적으로 허용되는"이란 생리학적으로 허용되고 인간에게 투여될 때, 통상적으로 위장 장애, 현기증과 같은 알레르기 반응 또는 이와 유사한 반응을 일으키지 않는 조성물을 말한다. 상기 담체, 부형제 및 희석제의 예로는, 락토즈, 덱스트로즈, 수크로즈, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로즈, 메틸 셀룰로즈, 폴리비닐피롤리돈, 물, 메틸하이드록시벤조에이트, 프로필하이드록시벤조에이트, 탈크, 마그네슘 스테아레이트 및 광물유를 들 수 있다. 또한, 충진제, 항응집제, 윤활제, 습윤제, 향료, 유화제 및 방부제 등을 추가로 포함할 수 있다.
또한, 본 발명의 조성물은 포유동물에 투여된 후 활성 성분의 신속, 지속 또는 지연된 방출을 제공할 수 있도록 당업계에 공지된 방법을 사용하여 제형화될 수 있다. 제형은 분말, 과립, 정제, 에멀젼, 시럽, 에어로졸, 연질 또는 경질 젤라틴 캅셀, 멸균 주사용액, 멸균 분말의 형태일 수 있다.
또한, 본 발명의 조성물의 활성 성분의 투여량은 투여 경로, 환자의 연령, 성별, 체중 및 환자의 중증도 등의 여러 인자에 따라 적절히 선택될 수 있고, 본 발명에 따른 조성물은 골관절염의 증상을 예방, 개선 또는 치료하는 효과를 가지는 공지의 화합물과 병행하여 투여할 수 있다.
본 발명의 약제학적 조성물의 적합한 투여량은 제제화 방법, 투여방식, 수용자의 연령, 체중, 성, 병적 상태, 음식, 투여 시간, 투여 경로, 배설 속도 및 반응 감응성과 같은 요인들에 의해 다양하게 처방될 수 있다. 본 발명의 약제학적 조성물의 투여량은 바람직하게는 1일 당 1 x 103 ~1 x 1012 세포/kg 이다.
나아가 본 발명에 따른 조성물은 상기 기술한 바와 같이 약학적 조성물로 사용할 수 있을 뿐만 아니라, 건강기능식품으로도 사용할 수 있다. 예컨대, 식품의 주원료, 부원료, 식품 첨가제, 기능성 식품 또는 음료로 용이하게 활용할 수 있다.
상기 “식품”이란, 영양소를 한 가지 또는 그 이상 함유하고 있는 천연물 또는 가공품을 의미하며, 바람직하게는 어느 정도의 가공 공정을 거쳐 직접 먹을 수 있는 상태가 된 것을 의미하며, 통상적인 의미로서, 식품, 식품 첨가제, 기능성식품 및 음료를 모두 포함하는 것을 말한다.
상기 식품용 조성물을 첨가할 수 있는 식품으로는 예를 들어, 각종 식품류, 음료, 껌, 차, 비타민 복합체, 기능성 식품 등이 있다. 추가로, 특수영양식품(예, 조제유류, 영, 유아식 등), 식육가공품, 어육제품, 두부류, 묵류, 면류(예, 라면류, 국수류 등), 빵류, 건강보조식품, 조미식품(예, 간장, 된장, 고추장, 혼합장 등), 소스류, 과자류(예, 스낵류), 캔디류, 쵸코렛류, 껌류, 아이스크림류, 유가공품(예, 발효유, 치즈 등), 기타 가공식품, 김치, 절임식품(각종 김치류, 장아찌 등), 음료(예, 과실음료, 채소류 음료, 두유류, 발효음료류 등), 천연조미료(예, 라면스프 등)를 포함하나 이에 한정되지 않는다. 상기 식품, 음료 또는 식품첨가제는 통상의 제조방법으로 제조될 수 있다.
또한, 상기 “기능성 식품”또는 “건강기능식품”이란 식품에 물리적, 생화학적, 생물 공학적 수법 등을 이용하여 해당 식품의 기능을 특정 목적에 작용, 발현하도록 부가가치를 부여한 식품군이나 식품 조성이 갖는 생체방어리듬조절, 질병방지와 회복 등에 관한 체내조절기능을 생체에 대하여 충분히 발현하도록 설계하여 가공한 식품을 의미하며, 구체적으로는 건강 기능성 식품일 수 있다. 상기 기능성 식품에는 식품학적으로 허용 가능한 식품 보조 첨가제를 포함할 수 있으며, 기능성 식품의 제조에 통상적으로 사용되는 적절한 담체, 부형제 및 희석제를 더욱 포함할 수 있다.
상기 건강보조식품의 종류로는 이에 제한되지는 않으나, 분말, 과립, 정제, 캡슐 또는 음료 형태 일 수 있다.
또한, 본 발명은 본 발명의 약학적 조성물을 치료가 필요한 개체에게 투여하는 것을 포함하는, 신장독성 예방 또는 치료 방법 및 신장 섬유화 질환 예방 또는 치료 방법에 관한 것이다.
본 발명의 예방 또는 치료 방법은 상기 약학적 조성물을 치료적 유효량으로 개체에 투여하는 것을 포함한다. 특정 개체에 대한 구체적인 치료적 유효량은 달성하고자 하는 반응의 종류와 정도, 경우에 따라 다른 제제가 사용되는지의 여부를 비롯한 구체적 조성물, 개체의 연령, 체중, 일반건강 상태, 성별 및 식이, 투여 시간, 투여 경로 및 조성물의 분비율, 치료기간, 구체적 조성물과 함께 사용되거나 동시 사용되는 약물을 비롯한 다양한 인자와 의약 분야에 잘 알려진 유사 인자에 따라 다르게 적용하는 것이 바람직하다. 따라서 본 발명의 목적에 적합한 조성물의 유효량은 전술한 사항을 고려하여 결정하는 것이 바람직하다.
상기 치료가 필요한 개체는 환자를 의미하는 것일 수 있고, 상기 개체는 임의의 포유동물에 적용가능하며, 상기 포유동물은 인간 및 영장류뿐만 아니라, 소, 돼지, 양, 말, 개 및 고양이 등의 가축을 포함한다.
이하, 본 발명을 실시예에 의해 상세히 설명하기로 한다. 그러나 이들 실시예는 본 발명을 보다 구체적으로 설명하기 위한 것으로서, 본 발명의 범위가 이들 실시예에 한정되는 것은 아니다.
실시예 1. 황칠나무 물추출물의 제조
황칠나무(Dendropanax morbifera)의 지상부는 전라남도 광양에서 채집한 것을 사용하였으며, 성균관대학교 약학대학의 증거표본으로 보관하였다(SKKU-Ph-16-031 및 SKKU-Ph-17-021). 건조한 황칠나무 지상부 100g을 물 1L에서 95℃에서 5시간 동안 2회 추출하고 여과한 후 감압농축하여 황칠나무 물추출물을 얻었다.
실시예 2. 황칠나무의 추출, 분획 및 덴드로파녹시드(dendropanoxide)의 분리
2-1. 황칠나무 에탄올 추출물 및 용매 분획물의 제조
황칠나무(Dendropanax morbifera)는 전라남도 고흥군에서 지상부를 채집(SKKU-Ph-18-012)하여 세절한 다음 52℃에서 12시간동안 건조하였다. 건조한 황칠나무 지상부(2.27kg)를 95% 에탄올(95% ethanol) 수용액으로 실온에서 24시간 동안 2회 추출 및 60℃에서 5시간 동안 1회 추출하고 여과한 후 모든 추출액을 40℃에서 감압 농축하여 황칠나무 에탄올 추출물(81.9g)을 얻었다. 수득된 에탄올 추출물을 물(증류수, 900mL)에 현탁한 후 상법에 따라 유기용매 분획하여 디클로로메탄(21.1g), 에틸아세테이트(3.2g), n-부탄올(7.2g) 및 물(63.5g) 분획을 얻었다.
2-2. 덴드로파녹시드의 분리
상기 2-1에서 얻어진 디클로메탄(CH2Cl2) 분획물을 반복된 실리카겔 컬럼 크로마토그래피와 재결정 과정을 통하여 백색 결정의 덴드로파녹시드(dendropanoxide; DPx)를 얻었으며, 1H-NMR과 13C-NMR 및 GC-MS 분석을 통하여 그 구조를 확인하였다(도 1).
실시예 3. 황칠나무 물추출물의 시스플라틴(cisplatin) 유도 신장독성 억제 효과
3-1. 동물실험
동물모델에서 황칠나무(Dendropanax morbifera) 물추출물의 시스플라틴 유도 신장 독성에 대한 보호 효과를 확인하는 실험을 실시하였다.
랫드를 이용한 동물실험을 수행하기 위해, 수컷 Sprague-Dawley 쥐(6 주령, 200±5g)는 Charles River Laboratory에서 구입하여 자동 제어 온도(23±0.5℃) 및 습도(55±2%) 조건에서 12시간 명/암 주기가 있는 공간에서 사육하였고, 물과 사료는 자유롭게 제공하였다. 동물실험은 각 군당 6마리씩 4개 그룹으로 나누었고, 각 그룹은 식염수 투여 대조군(control; i.p.), 시스플라틴 처리군(CDDP; 6 mg/kg, i.p.), 황칠나무 물추출물군(DM; 25mg/kg, p.o.) 및 복합군으로서 2시간전 시스플라틴(6mg/kg, i.p.) 투여 후 황칠나무 물추출물(25mg/kg, p.o.)투여군(CDDP+DM)으로 구성되었다. 본 실험에서 신장 손상을 유도하기 위해 시스플라틴(CDDP, 6mg/kg)을 생리식염수(0.9% 염화나트륨)에 용해시켜 10일에 걸쳐 한번 복강내로 투여(i.p.)하였다. 황칠나무 물추출물(DM, 25mg/kg)은 탈이온 증류수에 용해시켜 10일동안 매일 경구투여(p.o.)하였다.
실험이 끝난 후 모든 동물은 24시간 금식한 후 희생시켰으며, 조직학 및 기타 분석을 위해 신장을 분리하였고, 혈액 샘플을 채취한 후 60분에 4000rpm에서 원심분리로 혈청을 분리하였다. 분리된 신장 및 혈청 샘플은 분석전까지 -80℃에서 보관하였다.
3-2. 뇨 및 혈청 분석
뇨는 24시간 간격으로 수집하였고, 혈액은 복부대동맥에서 채취한 후 혈청을 분리하여 사용하였다. 뇨 및 혈청의 BUN(blood urea nitrogen)과 크레아티닌을 포함한 각종 지표 물질의 수준은 Hitachi 912 자동화분석장치 (Roche Diagnostics, Sandhofer, Mannheim, Germany)를 이용하여 분석하였다.
3-3. 웨스턴 블롯(Western blot) 분석
뇨 및 신장 조직에서 Protein Extraction Solution (1 ml)을 이용하여 단백질을 추출하였다. 추출된 단백질을 용해시키고, SDS-PAGE(sodium dodecyl sulfate polyacrylamide gel electrophoresis), Transfer, Blocking, 항원-항체반응, Detection의 순서로 실험을 진행하였다. 6~15% SDS Gel을 이용하여 electrophoresis를 실시하였고, 그 후에 단백질들을 polyvinylidene difluoride(PVDF) membrane에 전달하였다. 단백질이 전달된 membrane을 blocking buffer(5% Skim milk)로 blocking 과정을 진행하였으며, 그 후에 1차 항체를 밤새 반응시키고, 세척 버퍼(wash buffer)로 헹구어 낸 후 2차 항체를 반응시켰다. 2차 항체 반응 후, 다시 세척 버퍼로 결합되지 않은 2차 항체를 씻어낸 뒤, enhanced chemiluminescence(ECL)-plus kit을 이용하여 각 단백질들의 발현량을 비교하였다.
3-4. 신장 조직학적 염색 (H&E 염색)
신장을 10% neutral phosphate-buffered formalin액(pH 7.4)에서 고정하였고, 에탄올을 이용하여 탈수화 과정을 진행한 후, 파라핀을 침투시키고 조직을 5㎛ 두께로 절단하였다. 자일렌을 이용하여 조직 중의 파라핀을 제거하고 헤마톡실린(hematoxylin)과 에오신(Eosin)을 이용하여 H&E 염색을 진행하였다. Mounting solution을 이용하여 mounting을 진행한 후 광학현미경을 이용하여 관찰하였다.
3-5. 신장 면역조직화학 염색 (IHC)
상기 3-4에서 만들어진 슬라이드를 xylene, 에탄올, 가열된 Sodium citrate 버퍼, 5% 과산화수소수의 순서로 처리한 후 조직을 blocking을 하고, 그리고 필요한 1차 항체와 반응을 진행하였으며 또한 2차 항체와의 반응을 실시하였다. HRP-steptavidin 시약을 30분간 반응시켜주고, DAB와 Hematoxylin 용액으로 염색한 후 confocal K1-fluo microscope(Nanoscope Systems, Daejeon, Korea)로 관찰하였다.
3-6. 염증성 사이토카인 및 항산화 효소 활성 측정
랫드의 혈액 (Serum)에서 TNF-α, IL-1β IL-6, IL-10의 수준은 ELISA assay kits(Abcam)로 450 nm로 설정된 마이크로 플레이트 판독기를 사용하여 측정하였다.
카탈라아제(Catalase)와 Superoxide dismutase (SOD) 함량은 신장 조직에서 ELISA kit를 사용하여 측정되었다. 카탈라아제(Catalase)는 570nm에서 흡광도를 측정하였고, 카탈라아제는 nmol/min/mL 단백질로 표시하고 표준 곡선을 사용하여 정량하였다. Superoxide dismutase(SOD)의 함량은 440 nm에서 측정 하였으며, SOD는 U/ml로 표현되었고 표준 곡선을 사용하여 정량화되었다.
3-7. 세포자기사멸 확인(TUNEL assay)
세포자기사멸(apoptosis) 유도를 조사하기 위해 TUNEL 분석을 수행했다. 파라핀이 제거된 신장 조직 절편을 20μg/mL 프로테이나제 K(Proteinase K)와 평형 완충액(Equilibration solution)으로 반응한 후 TdT reaction mix을 이용하여 조직을 incubation 시켜주고 Stop buffer, peroxidase blocking, HRP-streptavidin HRP solution 순서로 처리한 후 DAB 및 헤마톡실린을 제 시간에 반응시키고 confocal K1-fluo microscope에서 관찰하였다.
3-8. 생체내 종양 이종 이식(Xenograft) 모델 실험
약 20g 무게의 4주령 수컷 BALB/c 누드 마우스(Japan SLC Inc., Hamamatsu, Shizuoka, Japan)를 제어된 온도 조건(22±2℃) 및 공기 여과 장치가 달린 사육시설에서 12시간 명/암 주기로 수용했다. 마우스는 무균 절차를 사용하여 처리되었고, 실험 절차는 성균관대학교 동물 실험위원회(SKKUIACUC2018-11-01-1)의 승인을 받았다. 마우스에 50% Matrigel™을 함유하는 무혈청 배지(BD Biosciences, Franklin Lakes, NJ, USA)에 HCT-116 세포(2×107 세포/0.1mL)를 피하 주사했다. 마우스를 4개의 그룹으로 나누었다(그룹당 6마리): (a) 0.2mL PBS 비히클(vehicle) 처리군(대조군, control); (b) CDDP 투여군(4mg/kg, i.p.); (c) DM(25mg/kg/일, oral gavage) + CDDP(4mg/kg, i.p.) 투여군; 및 (d) DM(25mg/kg/일, oral gavage) 투여군. 시스플라틴(CDDP)은 30일 동안 일주일에 한 번 주입되었고, 황칠나무 물추출물(DM)은 경구 위관 영양법(oral gavage)으로 30일 동안 매일 투여되었다. 종양 부피(V)는 표준 공식 V(mm3)=0.52(ab
2)를 사용하여 계산되었으며, 여기서 a는 종양의 길이이고 b는 종양의 너비이다. 약물 투여 전과 실험 종료시에 체중을 기록하였다. 31일에 마우스를 CO2 질식으로 안락사시켰다.
3-9. 황칠나무 물추출물의 시스플라틴 유도 신장 독성에 대한 보호 효과
시스플라틴의 투여로 인해 신장의 조직학적 구조 변화와 함께 체중 감소, 혈액 요소 질소(BUN) 및 크레아티닌 수준이 증가되었으며, 뇨를 이용한 웨스턴 블롯에서 신장 섬유증의 바이오마커로 알려진 SBP-1, KIM-1, NGAL 및 TIMP-1과 같은 단백질 기반 신장 독성 생체 지표 물질의 뇨 배설이 증가하였고, 신장 조직을 이용한 면역학적 염색 실험에서 SBP-1, KIM-1, NGAL 또한 시스플라틴에 의해 증가함을 확인하였다. 이러한 신장 손상이 유발된 상태에서, 황칠나무 물추출물은 신장 손상 생체 표지 물질인 BUN, 크레아티닌 및 KIM-1, NGAL 및 SBP1의 현저한 감소를 나타내었으며, 특히, 황칠나무 물추출물의 보호 효과는 조직 병리학적 검사에서 명확하게 관찰되었으며, 물추출물은 시스플라틴에 의해 유발된 신장 근위 세뇨관에서의 심한 손상을 현저하게 감소시켰다(도 2 내지 도 4).
황칠나무 물추출물은 내인성 항산화제 활성(SOD 및 Catalase)을 증가시킴으로써 신장에서 시스플라틴에 의해 유도된 산화 스트레스를 현저하게 감소시켰고 또한, 염증성 사이토카인인 TNF-α, IL-1β, IL-6, IL-10의 수준을 감소시켰다(도 5).
또한, 신장 조직에서 세포자기사멸 단백질인 P53과 Bax는 시스플라틴 투여군(CDDP)에 비해 황칠나무 물추출물을 투여한 복합군(CDDP+DM)에서 감소하였고, Bcl-2는 증가하였으며, 신장 조직을 이용한 TUNEL assay에서도 시스플라틴 군(CDDP)에 비해 물추출물 투여한 복합군(CDDP+DM)에서 TUNEL-positive 세포 수가 감소하여 신장 손상에 의한 세포자멸사가 억제됨을 확인하였다(도 6).
이상의 결과로부터 황칠나무 물추출물은 시스플라틴에 의해 유발된 신장 독성에 대하여 확실한 보호 효과를 나타냄을 확인할 수 있었다.
3-10. 황칠나무 물추출물의 시스플라틴 유도 종양 이종 이식 모델에서의 신장 보호 효과
이종 이식 종양 모델에서 시스플라틴 처리된 그룹(CDDP)은 체중이 크게 감소하였으며, 종양 부피와 종양 무게는 대조군에 비해 현저하게 감소했다. 시스플라틴 및 황칠나무 물추출물(DM)을 함께 투여시(CDDP+DM) CDDP 군에서 관찰된 것과 유사한 패턴으로 종양 성장을 극적으로 억제하였다. 면역 화학적 염색 결과 결장 종양 세포 증식에 대한 시스플라틴 및 황칠나무 물추출물의 항종양 활성이 종양 조직에서 Ki-67의 감소와 관련이 있음을 확인하였다. 이러한 결과는 황칠나무 물추출물이 종양 이종 이식 모델에서 시스플라틴의 항암 효능을 방해하지 않고, 오히려 황칠나무 물추출물과 시스플라틴의 병용 치료가 시스플라틴 매개 항암 활성을 강화한다는 것을 나타낸다(도 7).
한편, 종양 이종 이식 모델에서 시스플라틴 처리군(CDDP)은 대조군(control)에 비해 신장 무게가 유의하게 증가하였고, 혈청 BUN 및 sCr 수준과 KIM-1 및 SBP1의 발현 역시 증가하여, 심각한 신장 손상 및 신장 섬유화가 발생한 것으로 나타났다. 반면에, 시스플라틴과 황칠나무 물추출물 함께 투여한 경우(CDDP+DM), 신장 무게가 대조군과 유사한 수준으로 감소되었고, 증가된 BUN 및 sCr의 수준 및 NGAL 및 SBP1 발현 역시 상당히 감소되어, 황칠나무 물추출물이 종양 모델에서 시스플라틴에 의해 유도된 근위관 손상 및 신장 구조 파괴로부터 보호하며, 신장 근위 세뇨관의 손상을 보호함을 확인하였다(도 8 내지 도 9).
실시예 4. 황칠나무 물추출물의 스트렙토조토신(streptozotocin) 유도 신섬유화 억제 효과
4-1. 동물실험
수컷 Sprague-Dawley 쥐(4주령)를 Charles River Laboratory에서 구입하여 조건에서 12시간 명/암 주기가 있는 공간에서 사육하였고, 물과 사료는 자유롭게 제공하였다. 단일 용량의 스트렙토조신(STZ, 60mg/kg)을 차가운 시트레이트 완충액(0.1mmol/L, pH 4.5)에 용해시켜 복강내로 투여(i.p.)하였다. STZ 투여 5일 후 공복 혈당 수치가 300mg/dL 이상인 랫드를 당뇨병으로 진단하여, 추가 실험에 사용하였다. SD 랫드를 각각 5마리씩 5개 그룹으로 나누어, 생리 식염수를 경구 투여(i.p.)한 대조군(NC), 스트렙토조토신 단일 투여(60mg/kg, i.p.)한 당뇨병군(STZ), 스트렙토조토신과 황칠나무 물추출물(25mg/kg, p.o.)을 함께 투여한 복합군(STZ+DP), 황칠나무 물추출물 단일 투여(25mg/kg, p.o.)한 정상군(DP) 및 스트렙토조토신과 매트포민(metformin, 50mg/kg, p.o.)를 함께 투여한 양성대조군(STZ+Mef)으로 구성되었다. 복합군 및 양성대조군에서 황칠나무 물추출물(DP) 및 매트포민(Mef)은 각각 스트렙토조토신(STZ) 투여 5일 후부터 4주간 매일 경구투여(25mg/kg/day 또는 50mg/kg/day, p.o.) 하였다.
4-2. 뇨 및 혈청 분석
뇨는 metabolic cage를 사용하여 수집하였고, 혈액은 최종 약물 투여후 모든 쥐를 밤새 단식하고 케타민/자일라진(3.25mg/1.25mg/100g 중량기준, i.p.)으로 마취한 뒤, 흉강을 열어 하대정맥에서 채취한 다음 혈청을 원심분리(1500×g)하여 사용하였다. 뇨 및 혈청의 BUN(blood urea nitrogen)과 크레아티닌을 포함한 각종 지표 물질의 수준은 VetScan analyzer(Abaxis Inc., CA, USA) 와 automatic urine analyzer(Sysmex, Japan)를 이용하여 분석하였다.
4-3. 간, 신장, 췌장의 조직학적 염색 (H&E 염색)
왼쪽 신장, 간 및 췌장의 모든 조직을 neutral buffered formalin액에서 고정하였고, 에탄올을 이용하여 탈수화 과정을 진행한 후, 파라핀을 침투시키고 조직을 5㎛ 두께로 절단하였다. 자일렌(Xylene)을 이용하여 조직 중의 파라핀을 제거하고 헤마톡실린(hematoxylin)과 에오신(Eosin)을 이용하여 H&E 염색을 진행하였다. Mounting solution을 이용하여 mounting을 진행한 후 광학현미경을 이용하여 관찰하였다. 특히, 신섬유화 형태 분석을 위해서는 마손 삼색(Masson’s trichrome; MT) 염색을 실시하였다.
4-4. 신장의 면역조직화학 분석(IHC)
면역조직화학 분석(IHC)은 상기 4-3에서 만들어진 슬라이드를 자일렌을 처리하여 조직 중의 파라핀을 제거하고, TGF-β1, 피브로넥틴(fibronectin), 콜라겐 I(collagen I) 및 α-SMA를 포함한 1차 항체와 반응을 진행하였으며 또한 HRP 중합 2차 항체와의 반응을 실시하였다. DAB(diaminobenzidine tetrahydrochloride)와 헤마톡실린 용액으로 염색한 후 confocal K1-fluo microscope(Nanoscope Systems, Daejeon, Korea)로 신장 조직 절편을 관찰하였다.
4-5. 최종 당화 산물의 분석
최종 당화 산물(Advanced Glycation end Products)의 분석은 ELISA assay로 450 nm로 설정된 마이크로 플레이트 판독기를 사용하여 측정하였다.
4-6. 3-Indoxyl Sulfate (3-IS) 및 4-Hydroxyproline 함량의 측정
뇨, 혈청, 신장조직에서 트립토판 아미노산의 대사산물인 3-IS(3-인독실설페이트)의 양을 HPLC 분석법(Gilson, LC-321 322 350)으로 측정하였고, 신장조직 중 4-Hydroxyproline(4-하이드록시프롤린; 콜라겐 수준을 나타내는 마커)의 함량은 colorimetric assay kit (Cell Biolabs, San Diego, CA, USA)으로 측정하였다.
4-7. 염증성 사이토카인 및 항산화 효소 활성 측정
혈청(Serum)에서 전염증성 인자인 TGF-β1, IL-1β 및 IL-6과 항염증 인자인 IL-10의 수준은 ELISA assay kits (Abcam)로 450nm로 설정된 마이크로 플레이트 판독기를 사용하여 측정 하였다.
총 글루타티온(glutathione)과 산화 환원 글루타티온(GSH)(GSSG)의 양은 Glutathione Assay Kit를 사용하여 정량하였고, 산화된 형태와 환원된 형태의 글루타티온 비율(GSH/GSSG)을 계산하였다.
카탈라아제(Catalase; CAT)와 초과산화물 불균등화효소(Superoxide dismutase; SOD) 함량은 신장 조직에서 ELISA kit를 사용하여 측정되었다. 지질 과산화(Lipid peroxidation)는 혈중의 타티와비투르산(thiobarbituric acid; TBA) reactive substances 측정을 위한 assay kit(TBARS assay kit, Cayman, USA)를 사용하였고, 뇨 중의 8-Hydroxy-2’-deoxyguanosine (8-OHdG)은 ELISA assay kit (Cell Biolabs, San Diego, CA, USA)로 측정하였다.
4-8. 세포자기사멸 확인 (TUNEL assay)
세포자기사멸(apoptosis) 유도를 조사하기 위해 TUNEL 분석을 수행했다. 신장 조직을 DeadEndTM colorimetric system (Promega, Madison, WI, USA)에 적용하여 apoptotic cell을 검출하였다.
4-9. 웨스턴 블롯 (Western blot) 분석
신장 조직에서 Protein Extraction Solution (1 ml)을 이용하여 단백질을 추출하였다. 추출된 단백질을 용해시키고, SDS-PAGE(sodium dodecyl sulfate polyacrylamide gel electrophoresis), Transfer, Blocking, 항원-항체반응, Detection의 순서로 실험을 진행하였다. 6~15% SDS Gel을 이용하여 electrophoresis를 실시하였고, 그 후에 단백질들을 polyvinylidene difluoride(PVDF) membrane에 전달하였다. 단백질이 전달된 membrane을 blocking buffer(5% 탈지유)로 blocking 과정을 진행하였으며, 그 후에 1차 항체를 밤새 반응시키고, 세척 버퍼(wash buffer)로 헹구어 낸 후 2차 항체를 반응시켰다. 2차 항체 반응 후, 다시 세척 버퍼로 결합되지 않은 2차 항체를 씻어낸 뒤, enhanced chemiluminescence(ECL)-plus kit을 이용하여 각 단백질들의 발현량을 비교하였다.
4-10. 황칠나무 물추출물의 스트렙토조토신 유도 신섬유화 억제 효과
황칠나무 물추출물은 스트렙토조토신(streptozotocin)의 투여로 유발된 당뇨쥐의 체중 및 장기 무게 감소를 억제하였으며, 신장 손상 생체 표지 물질인 BUN, 크레아티닌, 각종 산화적 스트레스 지표물질 및 KIM-1, SBP1, PKM2의 현저한 감소를 나타내었고, 신장과 췌장의 조직병리학적 손상을 차단하였다. 구체적으로, 황칠나무 물추출물은 스트렙토조토신 투여로 증가된 내인성 ROS의 양을 현저히 감소시켰으며, GSH를 증가시키고, GSSG를 감소시켰다. 또한, 황칠나무 물추출물은 당뇨 쥐에서 내인성 항산화제 활성(SOD 및 Catalase 등)을 증가시킴으로써 신장에서 스트렙토조토신에 의해 유도된 산화 스트레스를 현저하게 감소시켰고, 증가된 MDA 및 8-OHdG를 효과적으로 감소시켰으며, 염증성 사이토카인인 TGF-β1, IL-1β, IL-6의 수준을 감소시켰으며 IL-10의 수준을 증가시켰다. 특히, 황칠나무 물추출물의 스트렙토조토신 유발 당뇨 쥐에서의 신장 보호 효과는 조직 병리학적 검사에서 명확하게 관찰되었다(도 10 내지 도 13).
또한 황칠나무 물추출물을 투여한 복합군(STZ+DP)의 신장 조직에서 세포자기사멸 단백질인 Bax, cleaved-caspase 3 및 cleaved-caspase 9가 감소하였고, Bcl-2는 증가하였다. 신장 조직을 이용한 TUNEL assay에서도 STZ+DP군에서 황칠나무 물추출물 투여에 의해 TUNEL-positive 세포 수가 감소함을 확인하였다(도 14).
당뇨 유발 신섬유화에 대한 억제 효과를 평가한 결과, 당뇨에 의해 신장에서 collagen-1, fibronectin 및 vimentin이 증가 및 α-SMA와 E-cadherin이 감소되었으나, 황칠나무 물추출물의 투여에 의해 collagen-1, fibronectin 및 α-SMA의 발현이 다시 감소하고 ECM 수준이 회복되는 효과가 나타남을 확인하였으며, 이로부터 황칠나무 물추출물이 스트렙토조토신에 의한 당뇨로 유발된 신섬유화를 효과적으로 억제함을 알 수 있었다. 또한, 당뇨에 의해 증가된 신장의 4-hydroxyproline 양을 효과적으로 감소시켰으며 스트렙토조토신에 의해 유발된 콜라겐 섬유(collagen fiber)의 축적 또한 현저히 감소시켰다(도 15).
이러한 결과로부터 황칠나무 물추출물이 스트렙토조토신 유발 신장독성 및 신섬유화에 대하여 현저한 보호 효과를 나타냄을 확인하였다.
실시예 5. 황칠나무로부터 분리된 덴드로파녹시드(dendropanoxide)의 시스플라틴(cisplatin) 유도 신장독성 억제 효과
5-1. 동물실험
랫드를 이용한 동물실험을 수행하기 위해, 수컷 Sprague-Dawley 쥐를 Charles River Laboratory에서 구입하여 자동 제어 온도(23±0.5℃) 및 습도(55±2%) 조건에서 12시간 명/암 주기가 있는 공간에서 사육하였고, 물과 사료는 자유롭게 제공하였다. 동물실험은 4개 그룹으로 나누었고, 각 그룹은 식염수 투여 대조군(Con; i.p.), 시스플라틴 처리군(CDDP; 6mg/kg, i.p.), 복합군으로서 1시간전 시스플라틴(6mg/kg, i.p.) 투여 후 덴드로파녹시드(5 및 10mg/kg, p.o.)투여군(CDDP+DPx)으로 구성되었다. 본 실험에서 신장 손상을 유도하기 위해 시스플라틴(CDDP, 6mg/kg)은 실험 시작 시점에 한번 복강내로 투여(i.p.)하였고, 덴드로파녹시드(DPx)는 5mg/kg 또는 10mg/kg을 7일간 매일 경구투여(p.o.)하였다.
실험이 끝난 후 모든 동물은 24시간 금식한 후 희생시켰으며, 조직학 및 기타 분석을 위해 신장을 분리하였고, 혈액 샘플을 채취한 후 1500g에서 10분간 원심분리하여 혈청을 분리하였다. 또한, 뇨는 희생시키기 전(8일째에) 24시간 동안 수집하여 3000g에서 10분간 원심분리하였다. 분리된 신장, 혈청 및 뇨 샘플은 분석전까지 -80℃에서 보관하였다.
5-2. 세포 생존력 시험
덴드로파녹시드의 유효 및 독성 농도를 확인하기 위해 랫드 신장 상피 세포 (NRK-52E)를 96 well plate(4×103세포/Well)에 24시간 배양한 후, 덴드로파녹시드를 1 내지 400μg/ml의 농도 범위로 48시간 동안 처리하여 세포 생존력을 관찰하였다. 세포 생존력 확인은 MTT assay를 통해 진행되었다. 시스플라틴의 세포독성에 대한 덴드로파녹시드의 보호효과를 확인하기 위하여 세포에 덴드로파녹시드(5, 10, 또는 20μg/ml)를 6시간 동안 먼저 노출시켰다. 이후, 시스플라틴 20μM을 48시간 동안 처리하였다. 5mg/ml 농도의 MTT시약(3-[4,5-dimethylthiazol-2-yl]-2,5diphenyltetrazoliumbromide)을 100 μL씩 각 well에 적용하였다. 37℃에서 3시간이 지난 후, 상층액을 흡인한 다음, 포르마잔(formazan) 결정을 10분 동안 37℃에서 DMSO 100μL로 용해했다. VERSA max 마이크로 플레이트 판독기 (Molecular Devices Corp., Sunnyvale, CA, USA)를 사용하여 540nm에서 well 당 흡광도를 측정하였다.
5-3. 아넥신 V와 PI 염색을 이용한 apoptosis의 확인
랫드 신장 상피 세포(NRK-52E)에서 시스플라틴 처리에 의한 세포 사멸이 덴드로파녹시드에 의해 억제되는지 확인하는 실험을 수행하였다. 우선, 랫드 신장 상피 세포(NRK-52E)를 6 well plate에10% 소 태아 혈청을 포함한 배지를 이용하여 16시간동안 배양한 뒤 혈청이 첨가되지 않은 배지로 교체하여 12시간 동안 배양하였다. 이후 덴드로파녹시드는 5, 10 μg/ml의 농도로 처리하였으며 30분 후 20μM의 시스플라틴을 24시간 동안 처리하였다. 24시간 후 배지를 제거하고, DPBS로 2회 세척한 후, 트립신 1 mL를 각 well에 넣은 뒤 세포를 분리하고, 배지 5 mL씩 넣어 트립신을 희석한 후, 1000rpm으로 5분 동안 원심분리하여 세포를 준비하였다. 세포를 수확한 뒤 Annexin V-FITC/PI Apoptosis Detection Kit(BD Biosciences, Inc)를 이용하여 각 시료 당 1×Binding buffer 1ml에 아넥신(annexin) V 5μl 및 PI( propidium iodide) 5μl를 가하고 차광 후 실온 보관하고 유체분석기(Flow cytometer)를 이용하여 측정하였다.
5-4. 혈청 내 ALT, AST, BUN 및 크레아티닌 활성분석을 통한 장기손상지표 분석
SD 랫드에 시스플라틴(6mg/kg) 복강 투여 후 조제용매 또는 덴드로파녹시드(5, 10mg/kg)를 7일간 매일 경구 투여하였다. 복부 대동맥에서 혈액을 채취하여 혈청을 분리하여 분리한 혈청에서 알라닌 아미노트랜스퍼레이즈 (ALT;alanine aminotransferase, 이하 ALT라고 함) 및 아스파테이트 아미노트랜스퍼레이즈 (AST;aspartate aminotransferase, 이하 AST라 함)를 측정하여 간장기능의 손상 정도를 평가하였고, 혈액 내 요소 질소(blood urea nitrogen, 이하 BUN이라고 함) 및 혈청 크레아티닌(serum creatinine, 이하 sCr이라 함) 농도를 측정하여 신장기능의 손상 정도를 관찰하였다. 혈청의 ALT, AST, BUN, sCr 수준은 Hitachi 912 자동화분석장치(Roche Diagnostics, Sandhofer, Mannheim, Germany)를 이용하여 분석하였다. 또한 7일차에 랫드를 대사케이지에 넣어 24시간 동안의 뇨를 채취하였다.
5-5. 항산화 효소 활성 및 산화적 스트레스 확인 시험
PBS로 세척한 신장조직을 HEPES 완충액 (EGTA, mannitol, sucrose, pH 7.2)로 균질화한 후, 균질액을 1500 x g에서 5분간 원심분리하여 상층액을 취하여 증류수로 50배 희석하였다. 신장 조직내 초과산화물 불균등화효소(Superoxide dismutase), 카탈라아제(Catalase), 글루타티온(Glutathione), 말론디알데히드(Malondialdehyde) 및 8-OHdG(8-Hydroxy-2′-deoxyguanosine)의 수준은 각각 Superoxide Dismutase Assay Kit(Cayman chemical, Ann Arbor, Michigan, USA), Catalase Colorimetric Activity Kit(Abcam, Cambridge, MA, USA), Glutathione Assay Kit(Cayman Chemicals Company, Michigan, USA), MDA Colorimetric Assay Kit(MDA-586, OxisResearCh™, OR, USA) 및 8-OHDG ELISA Kit(Cell Biolabs, San Diego, CA, USA)를 통해 측정하였다.
5-6. 혈중 염증성 사이토카인 농도 분석
시스플라틴 투여시 나타나는 염증 반응을 관찰하고 덴드로파녹시드 투여에 따른 항염증 효과를 염증 관련 사이토카인(TNF-α, 인터루킨-1β, 인터루킨-6, 인터루킨-10)의 농도 측정을 통하여 확인하였다. 즉, 혈액을 채취하여 혈청을 분리하여 분리한 혈청에서 TNF-α, 인터루킨-1β(IL-1β), 인터루킨-6(IL-6), 인터루킨 -10(IL-10) 농도를 ELISA 어세이 키트(Abcam, Cambridge, MA, USA)를 사용하여 측정하였다. 주요 사이토카인의 수준은 표준 곡선으로부터 계산하였다.
5-7. 웨스턴 블롯(Western blot) 분석
신장 조직에서 Protein Extraction Solution(1 ml)을 이용하여 단백질을 추출하였다. 추출된 단백질을 용해시키고, SDS-PAGE(sodium dodecyl sulfate polyacrylamide gel electrophoresis), Transfer, Blocking, 항원-항체반응, Detection의 순서로 실험을 진행하였다. 6~15% SDS Gel을 이용하여 전기영동법(electrophoresis)을 실시하였고, 그 후에 단백질들을 polyvinylidene difluoride(PVDF) membrane에 전달하였다. 단백질이 전달된 membrane을 blocking buffer(5% Skim milk)로 blocking과정을 진행하였으며, 그 후에 1차 항체를 밤새 반응시키고, 세척 버퍼(wash buffer)로 헹구어 낸 후 2차 항체를 반응시켰다. 2차 항체 반응 후, 다시 세척 버퍼로 결합되지 않은 2차 항체를 씻어낸 뒤, enhanced chemiluminescence(ECL)-plus kit을 이용하여 각 단백질들의 발현량을 비교하였다.
5-8. 덴드로파녹시드의 시스플라틴 유도 신장독성 억제 효과
황칠나무 추출물로부터 분리된 덴드로파녹시드(DPx)의 세포 독성 유도 여부를 확인하기 위하여, NRK-52E 세포의 생존력을 평가한 결과, 덴드로파녹시드(1~400μg/mL)에 의한 세포 독성이 유발되지 않았고, 시스플라틴(CDDP)으로 유도된 신장 세포 독성에 대한 덴드로파녹시드(DPx)의 보호 효과를 평가하기 위해 NRK-52E 세포를 DPx(5, 10, 20μg/mL)로 6시간 동안 전처리 한 후 CDDP(20 μM)로 48시간 처리한 결과, DPx는 CDDP에 의한 세포 독성 및 신장 손상으로부터 NRK-52E 세포를 보호하는 효과가 있음을 확인하였다(도 16). 시스플라틴으로 유도된 세포자멸사에 의해 증가된 Annexin V-양성 세포 수가 덴드로파녹시드에 의해 크게 감소하였고, 세포자기사멸 신호 전달 경로에 Bax 및 cleaved-PARP의 발현 증가를 감소시키고, Bcl-2의 단백질 발현을 증가시켜 세포자멸사로부터 세포를 보호하였다(도 17).
또한, 랫드를 이용한 동물 실험에서, 간 및 신장의 무게가 시스플라틴 투여에 의해 대조군 대비 유의하게 증가되었으나, 덴드로파녹시드 투여에 따라 유의하게 감소하였고, 덴드로파녹시드는 시스플라틴에 의해 유발된 신장의 조직학적 손상과 BUN, creatinine의 증가를 효과적으로 억제하였으며 KIM-1, NGAL, SBP1 또한 회복효과를 나타내었다(도 18 내지 도 19).
덴드로파녹시드는 시스플라틴에 의해 유도된 p53 및 Bax를 감소시켰고, 감소된 Bcl-2는 유의한 회복 효능을 나타내었다(도 20). 항산화효소의 활성에 있어 덴드로파녹시드는 시스플라틴에 의해 감소된 SOD 및 CAT를 증가시켰으며, 반면에 증가된 MDA와 8-OHdG를 감소시켰다(도 21).
시스플라틴에 의해 증가된 혈중 염증성 사이토카인의 분석에서 덴드로파녹시드는 TNF-α, IL-1β, IL-6 및 IL-10을 효과적으로 저하시켰으며, AKT의 감소, p-AKT의 증가, AMPK의 증가, p-AMPK의 감소, mTOR의 감소 및 p-mTOR의 증가 효능을 나타내었다(도 22 내지 도 23).
이상의 결과로부터 황칠나무 추출물로부터 분리된 덴드로파녹시드는 시스플라틴에 의해 유도된 신독성을 in vitro와 in vivo에서 산화적 스트레스와 염증의 억제를 통하여 효과적으로 회복시킬 수 있었다. 따라서 덴드로파녹시드는 시스플라틴 유발 급성 신독성을 치료할 수 있는 효과적인 치료제가 될 수 있음을 확인하였다.
이제까지 본 발명에 대하여 그 바람직한 실시예들을 중심으로 살펴보았다. 본 발명이 속하는 기술 분야에서 통상의 지식을 가진 자는 본 발명이 본 발명의 본질적인 특성에서 벗어나지 않는 범위에서 변형된 형태로 구현될 수 있음을 이해할 수 있을 것이다. 그러므로 개시된 실시예들은 한정적인 관점이 아니라 설명적인 관점에서 고려되어야 한다. 본 발명의 범위는 전술한 설명이 아니라 특허 청구범위에 나타나 있으며, 그와 동등한 범위 내에 있는 모든 차이점은 본 발명에 포함된 것으로 해석되어야 할 것이다.
Claims (10)
- 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장독성 예방 또는 개선용 약학적 조성물.
- 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장 섬유화 질환 예방 또는 치료용 약학적 조성물.
- 제1항 또는 제2항에 있어서, 상기 황칠나무 추출물은 물 또는 유기용매에 의해 추출된 것인 약학적 조성물.
- 제3항에 있어서, 상기 유기용매는 메탄올, 에탄올, 프로판올, 이소프로판올, 부탄올, 아세톤, 에테르, 벤젠, 클로로포름, 디클로로메탄, 에틸아세테이트, 메틸렌클로라이드, 헥산 및 시클로헥산으로 이루어진 군 중에서 선택되는 어느 하나 이상인 것인 약학적 조성물.
- 제1항에 있어서,상기 신장독성은 항암제 투여 또는 당뇨병에 의해 유발된 것인 약학적 조성물.
- 제2항에 있어서,상기 신장 섬유화는 항암제 투여 또는 당뇨병에 의해 유발된 것인 약학적 조성물.
- 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장독성 예방 또는 개선용 건강기능식품.
- 황칠나무 추출물 또는 이로부터 분리된 덴드로파녹시드(dendropanoxide)를 유효성분으로 포함하는 신장 섬유화 질환 예방 또는 개선용 건강기능식품.
- 제1항의 신장독성 예방 또는 개선용 약학적 조성물을 치료가 필요한 개체에게 투여하는 것을 포함하는 신장독성 예방 또는 치료 방법.
- 제2항의 신장 섬유화 질환 예방 또는 치료용 약학적 조성물을 치료가 필요한 개체에게 투여하는 것을 포함하는 신장 섬유화 질환 예방 또는 치료 방법.
