Abstract

本発明の課題はハプトグロビンの添加に起因するヘモグロビンの測定値の低下を抑制して、ヘモグロビンの量をより正確に測定できるヘモグロビンの測定試薬及び測定方法を提供することである。本発明のヘモグロビンを測定するための試薬は、抗ヘモグロビン抗体を担持する不溶性担体と、抗ハプトグロビン抗体を担持する不溶性担体とを含む。本発明の検体中のヘモグロビンを測定するための方法は、(1)検体とハプトグロビンとを混合し、ヘモグロビン-ハプトグロビン複合体を形成させ、ヘモグロビン-ハプトグロビン複合体を含む試料を得る工程と、(2)工程(1)で得られた試料と、抗ヘモグロビン抗体を担持する不溶性担体、及び抗ハプトグロビン抗体を担持する不溶性担体とを接触させ、免疫凝集させる工程と、を含む。

Description

ヘモグロビンの測定試薬、測定キット及び測定方法
 本発明は、ヘモグロビンの測定試薬及び測定方法に関する。
 糞便、尿、唾液等の検体に含まれる血液の検出は、多くの疾患の診断に有用である。例えば、糞便中の血液を検出する便潜血検査は、大腸癌の検診に利用されている。潜血を検出する方法として、糞便等の検体中の潜血に含まれるヘモグロビンを、抗ヘモグロビン抗体を用いて検出する免疫学的手法が知られている。
 潜血検査に供される検体は、通常、被験者によって保存溶液の入った容器に採取され、病院等の検査機関に送られる。多くの場合、検体を含有する保存溶液は、実際に検査に供されるまで数日間保管され、その間、高温下に置かれることも度々ある。ヘモグロビンは溶液中で不安定であり、高温条件下では特に変性又は分解しやすい。ヘモグロビンが変性又は分解することによりエピトープ又はその周辺部位の構造が変化すると、抗体がヘモグロビンを認識できなくなり、よって、免疫学的手法によるヘモグロビンの検出の正確性が低下する。
 それゆえ、ヘモグロビンを安定化するために、検体の保存溶液にハプトグロビンを添加する方法が用いられている(例えば、特許文献1)。ハプトグロビンは、幅広い動物の血液中に存在し、赤血球の溶血によって血液中に放出されたヘモグロビンを回収する役割を担うタンパク質である。ハプトグロビンはヘモグロビンと速やかに不可逆的に結合し、安定なヘモグロビン-ハプトグロビン複合体を形成することが知られている。保存溶液等にあらかじめハプトグロビンを添加しておくことで、検体を保存溶液等に加えた際に、検体に含まれるヘモグロビンは、ヘモグロビン-ハプトグロビン複合体を形成することで安定化される。
 しかしながら、ハプトグロビンを添加する場合、特に検体中の遊離ヘモグロビンに対してハプトグロビンを過剰量で添加すると、ヘモグロビンの測定値の低下が認められることがある(特許文献1)。

 本発明は、ハプトグロビンの添加に起因するヘモグロビンの測定値の低下を抑制して、ヘモグロビンの量をより正確に測定できるヘモグロビンの測定試薬及び測定方法を提供することを目的とする。
 本発明者らは、上記従来技術の課題を解決すべく鋭意検討を重ねた結果、抗ヘモグロビン抗体が結合している不溶性担体と、抗ハプトグロビン抗体が結合している不溶性担体とを共存させることで、ハプトグロビンの添加に起因するヘモグロビンの測定値の低下を抑制でき、ヘモグロビンの量をより正確に測定できることを見出し、本発明の完成に至った。
 すなわち、本発明は、例えば、以下の各発明に関する。
[1] 抗ヘモグロビン抗体を担持する不溶性担体と、抗ハプトグロビン抗体を担持する不溶性担体とを含む、ヘモグロビンを測定するための試薬。
[2] 抗ヘモグロビン抗体が少なくとも2種である、[1]の試薬。
[3] 不溶性担体がラテックス粒子及び/又は金コロイド粒子である、[1]又は[2]のいずれかの試薬。
[4] 抗ヘモグロビン抗体及び抗ハプトグロビン抗体がモノクローナル抗体である、[1]~[3]のいずれかの試薬。
[5] 抗ハプトグロビン抗体が1種である、[1]~[4]のいずれかの試薬。
[6] 検体中のヘモグロビンを測定するための方法であって、
 (1)検体とハプトグロビンとを混合し、ヘモグロビン-ハプトグロビン複合体を形成させ、ヘモグロビン-ハプトグロビン複合体を含む試料を得る工程と、
 (2)工程(1)で得られた試料と、抗ヘモグロビン抗体を担持する不溶性担体、及び抗ハプトグロビン抗体を担持する不溶性担体とを接触させ、免疫凝集させる工程と、

 本発明によれば、ヘモグロビンの量をより正確に測定できる測定試薬及び測定方法が提供される。
<定義>
[ヘモグロビン-ハプトグロビン複合体]
 ヘモグロビンは、赤血球の中に存在するタンパク質であり、αサブユニット(又はα鎖)とβサブユニット(又はβ鎖)と呼ばれる2種類のサブユニットそれぞれ2つから構成される四量体構造[α2β2]を有している。ハプトグロビンは、血漿中に存在し、血中に遊離したヘモグロビンと結合するタンパク質であり、3つの型の構造を有する。例えば、1-1型ハプトグロビンは、αサブユニット(又はα鎖)とβサブユニット(又はβ鎖)と呼ばれる2種類のサブユニットそれぞれ2つから構成される四量体構造[α2β2]を有する。
 ヘモグロビンとハプトグロビンとは安定な複合体を形成する。通常ハプトグロビン1分子に対して、ヘモグロビン1分子が結合している。このような複合体は、ヘモグロビン-ハプトグロビン複合体と呼ばれており、本明細書では完全ヘモグロビン-ハプトグロビン複合体ともいう。
 一方、ハプトグロビンに対してヘモグロビンのモル比(ヘモグロビン:ハプトグロビン)が1未満となる場合、すなわち、ヘモグロビンに対してハプトグロビンが過剰量に存在する場合、ヘモグロビン-ハプトグロビン複合体中間体(intermediate)と呼ばれる複合体が形成される傾向にある(J. V. PASTEWKA et al.,Biochimica et Biophysica Acta, 386 (1975) 530-537)。ヘモグロビン-ハプトグロビン複合体中間体を形成する際、1分子の四量体ヘモグロビン[α2β2]は、2分子の二量体(αβ)へと解離して、それぞれのαβ二量体がハプトグロビン1分子と結合すると考えられている。すなわち、ヘモグロビン-ハプトグロビン複合体中間体は、1分子のハプトグロビン[α2β2]に1/2分子のヘモグロビン(αβ)が結合している複合体であると考えられている。
 本明細書において、ヘモグロビン-ハプトグロビン複合体は、特に明記しない限り、完全ヘモグロビン-ハプトグロビン複合体とヘモグロビン-ハプトグロビン複合体中間体との両方を含む。
[検体]
 本明細書における検体は、ヘモグロビンを含む又は含む可能性のある、被検対象から採取した生体サンプルである。検体は、糞便、唾液、又は尿であってよく、特に糞便であることが好ましい。また、全血、血清、血漿等を検体とすることもでき、これらのヘモグロビン量を測定することは、例えば、溶血やハプトグロビン製剤を用いた治療の指標として有用となりうる。
[試料]
 本明細書における試料は、検体を含むものであり、特に検体とハプトグロビンとを混合して得られたものである。検体中の遊離ヘモグロビンは、ハプトグロビンと混合することで、安定なヘモグロビン-ハプトグロビン複合体を形成する。
 すなわち、試料中において、ハプトグロビンと複合体を形成していない遊離ヘモグロビン、検体中に元々存在していたヘモグロビン-ハプトグロビン複合体(検体由来のヘモグロビン-ハプトグロビン複合体)、及び/又はハプトグロビンの添加によって形成されたヘモグロビン-ハプトグロビン複合体(調製ヘモグロビン-ハプトグロビン複合体)が含まれ得る。
 特に、安定性の点から、検体とハプトグロビンを混合した試料中において、全ての遊離ヘモグロビンがハプトグロビンと複合体を形成して調製ヘモグロビン-ハプトグロビン複合体を形成していることが好ましく、試料は、検体由来のヘモグロビン-ハプトグロビン複合体及び調製ヘモグロビン-ハプトグロビン複合体からなることが好ましい。
 試料は、例えば、被検者等が検体を採取してから、速やかにハプトグロビンを含有する検体保存用溶液(以下、場合によって「保存溶液」という)中に検体を添加することによって得られる。試料は、ヘモグロビンの測定までは常温で数日間保存してもよく、2~10℃で冷蔵保存されることが好ましい。検体が固形物を含む糞便等である場合、保存溶液をろ過して固形物を除去したものを試料とするようにしてもよい。
[保存溶液]
 保存溶液は、検体を保存するための検体保存用溶液を意味し、ハプトグロビンを含むものであることが好ましい。
 保存溶液は、さらに緩衝液等を含むことが好ましい。緩衝液等としては、2-モルホリノエタンスルホン酸(MES)、2-[4-(2-ヒドロキシエチル)-1-ピペラジニル]-エタンスルホン酸(HEPES)、ピペラジン-1,4-ビス(2-エタンスルホン酸)(PIPES)等のグッド緩衝剤を含む緩衝液であってよく、リン酸緩衝液、トリス緩衝液又はグリシン緩衝液であってもよい。保存溶液のpHは、5~10であってよく、6~8であってよい。
 保存溶液中のハプトグロビンの濃度は、検体の種類及び量にもよるが、例えば、0.05単位/L~50単位/L、0.1単位/L~10単位/L、又は、0.2単位/L~2単位/Lである。ここで、1単位は、1mgのヘモグロビンに結合するハプトグロビン量を意味する。上記範囲のハプトグロビン濃度は、検体中の全てのヘモグロビンを、ハプトグロビンと複合体を形成させるのに十分な濃度である。
 例えば、保存溶液0.2~20mLに対して、糞便の濃度が0.05%~25%(W/V%)となるよう懸濁する場合、検体対ハプトグロビンの量比は0.075g/単位~5000g/単位としてもよい。
 本発明におけるハプトグロビンは、測定対象であるヘモグロビンと複合化してヘモグロビン-ハプトグロビン複合体を生成するものであれば特に限定されない。ヘモグロビンとハプトグロビンの結合は、種特異性が小さいので幅広いハプトグロビンを利用することができる。ヒト・ヘモグロビンを測定対象としているときには、ヒトをはじめとして、ウマ、ブタ、サル、イヌ、ウサギ、ラット等に由来するハプトグロビンを利用できる。ハプトグロビンは必ずしも高度に精製された状態のものを用いる必要はない。
 保存溶液には、アジ化ナトリウム(NaN3)等の抗菌剤、pH調整剤、イオン強度を調節するための塩等、ヘモグロビンの保存時に使用され得る公知の添加剤をさらに添加してもよい。抗菌剤は、抗生物質及び溶菌酵素を含む。添加剤の例には、リジン、ヒスチジン等のアミノ酸、アルブミン、タンパク質分解酵素阻害剤、遷移金属イオンの水溶性錯体、エチレンジアミン四酢酸(EDTA)等、ヘモグロビンを安定化する作用があることが知られる公知の成分も含まれる。アルブミンの例には、ウシ血清アルブミン(BSA)等の血清アルブミン、及び卵白に由来するアルブミン(オボアルブミン)が含まれる。
<ヘモグロビンの測定試薬>
 本発明の一実施形態のヘモグロビンの測定試薬は、抗ヘモグロビン抗体を担持する不溶性担体と、抗ハプトグロビン抗体を担持する不溶性担体とを含む。本発明の試薬が用いられる測定法としては免疫学的手法が用いられ、免疫学的手法としては、抗ヘモグロビン抗体を利用する公知の免疫学的手法であってよく、例えば、免疫凝集法(例えば、ラテックス凝集法、又は金コロイド凝集法)、イムノクロマトグラフ法、ELISA法が挙げられる。本発明の試薬は特に免疫凝集法、より好ましくはラテックス凝集法に好適に用いられる。
 本発明の一実施形態のヘモグロビンの測定試薬は、測定時に遊離ヘモグロビン及びヘモグロビン-ハプトグロビン複合体の両方を測定対象として、ヘモグロビンの総量を測定する。本発明の試薬は、免疫学的測定法(特に免疫凝集法)において、遊離ヘモグロビン、完全ヘモグロビン-ハプトグロビン複合体、及びヘモグロビン-ハプトグロビン複合体中間体の各抗原に対し、ヘモグロビンとしてほぼ同等の反応性を示す。特に、不溶性担体に結合した抗ハプトグロビン抗体を共存させることで、完全ヘモグロビン-ハプトグロビン複合体とヘモグロビン-ハプトグロビン複合体中間体の両抗原に対し、ヘモグロビンとして同等の反応性(「ヘモグロビンに対する同値反応性」)を示すという優れた効果を奏する。
 なお、「ヘモグロビンとして同等の反応性」とは、試料中にヘモグロビンとして同濃度の、遊離ヘモグロビン、完全ヘモグロビン-ハプトグロビン複合体、ヘモグロビン-ハプトグロビン複合体中間体又はこれらの混合物の各抗原が存在する場合、不溶性担体の免疫凝集反応により測定されるヘモグロビン濃度が、各抗原において実質的に等しくなることをいう。ここで、不溶性担体の免疫凝集反応により測定されるヘモグロビン濃度が「実質的に等しい」とは、遊離ヘモグロビンの測定値を100%とした場合、ヘモグロビンとして同濃度であるその他の各種抗原を測定した際のヘモグロビン濃度の測定値の下限が80%以上であり、好ましくは90%以上、より好ましくは95%以上であり、また、測定値の上限値は120%以下であり、好ましくは110%以下であり、より好ましくは105%以下であることを意味する。
 本発明の試薬は、従来の抗ヘモグロビン抗体を担持する不溶性担体に加えて、抗ハプトグロビン抗体を担持する不溶性担体も含むことにより、従来のハプトグロビンの添加に起因するヘモグロビンの測定値の低下を抑制できる。特に、添加するハプトグロビンと検体中の遊離ヘモグロビンのモル比(ヘモグロビン/ハプトグロビン)が1未満となる場合、すなわち、ヘモグロビン-ハプトグロビン複合体中間体が存在する場合であっても、ヘモグロビンの測定値の低下を抑制して、検体中のヘモグロビンを正確に検出、測定できる。さらに、検体にハプトグロビンを添加しない場合において、検体中に元々ハプトグロビン(遊離ハプトグロビン、ヘモグロビン-ハプトグロビン複合体中間体、ヘモグロビン-ハプトグロビン複合体を問わず)が存在していても、ヘモグロビンの測定値の低下を抑制して、検体中のヘモグロビンを正確に検出、測定できる。
 本発明者らは、従来のハプトグロビンの添加に起因するヘモグロビンの測定値の低下は、ヘモグロビン-ハプトグロビン複合体中間体の存在が一因であると考え、抗ハプトグロビン抗体を担持する不溶性担体の添加でこの問題を解決した。
 不溶性担体としては、抗ヘモグロビン抗体又は抗ハプトグロビン抗体を担持できるものであればよく、免疫学的手法に使用できる不溶性粒子が好ましく、一般的に使用される金コロイド粒子等の金属コロイド粒子、ラテックス粒子、シリカ粒子、磁性粒子、蛍光粒子、赤血球等が挙げられるが、これらに限定されることはない。不溶性粒子としては、ラテックス粒子が好ましく、ポリスチレン系ラテックス粒子がさらに好ましい。不溶性担体は、好ましくは粒子状であり、その平均粒子径としては5~1000nmが好ましく、30~500nmがより好ましく、75~350nmがさらに好ましいが、特にこの範囲に捉われることなく、使用することが可能である。
 抗体を担持しているとは、抗体が不溶性担体の表面に物理吸着又は化学結合していることによって固定化されることをいう。担持方法(固定化方法)としては、例えば、公知の技術である、抗体と不溶性担体粒子とを混合して、不溶性担体粒子の表面に抗体を物理的に吸着させることにより不溶性担体粒子への抗体の固定化が可能である。また、表面にアミノ基やカルボキシル基を導入した不溶性担体粒子を用いる場合には、グルタルアルデヒドやカルボキシイミド試薬を用いた化学結合によって不溶性担体粒子の表面への抗体の固定化が可能である。
 抗体の担持量は、特に限定されないが、0.5~2000μg/mgラテックスであればよく、1~1000μg/mgラテックス、又は2~500μg/mgラテックスであってもよい。抗体の担持量は、不溶性担体に固定化する前の抗体量から固定化した後の抗体量を引いた量で算出することができる。
 抗ヘモグロビン抗体は、特に限定されないが、ヘモグロビン-ハプトグロビン複合体におけるヘモグロビンのエピトープを認識することができ、かつ、ハプトグロビンと交差反応を起こさないことが好ましい。ポリクローナル抗体又はモノクローナル抗体であってもよいが、特異性の点からモノクローナル抗体であることが好ましく、抗ヒト・ヘモグロビンモノクローナル抗体であることが好ましい。測定試薬に含まれる抗ヘモグロビン抗体は少なくとも1種、好ましくは少なくとも2種である。
 一方、抗ハプトグロビン抗体は、特に限定されないが、ヘモグロビン-ハプトグロビン複合体におけるハプトグロビンのエピトープを認識することができ、かつ、ヘモグロビンと交差反応を起こさないことが好ましい。特に、抗ハプトグロビン抗体は、少なくとも遊離のヘモグロビンがヘモグロビン-ハプトグロビン複合体を形成するために用いられる遊離のハプトグロビンの添加量の範囲で、遊離のハプトグロビンと抗ハプトグロビン抗体を担持した不溶性担体において凝集しないものが好ましい。さらに、抗ハプトグロビン抗体は、遊離のハプトグロビンと抗ハプトグロビン抗体を担持した不溶性担体において凝集しないものがより好ましい。抗ハプトグロビン抗体は、ポリクローナル抗体又はモノクローナル抗体であってもよいが、モノクローナル抗体であることが好ましく、抗ハプトグロビンモノクローナル抗体であることが好ましい。抗ハプトグロビン抗体がモノクローナル抗体である場合、その認識部位はハプトグロビンのα鎖(α鎖認識抗体)又はβ鎖(β鎖認識抗体)であってよい。測定試薬に含まれる抗ハプトグロビン抗体は、少なくとも1種である。
 本発明で使用できる上記抗体としては、由来する動物種は特に限定されず、例えば、ウサギ、ヤギ、マウス、ラット、ウマ、ヒツジ等の動物に由来する抗体が挙げられ、公知の方法により、測定対象物を免疫した動物の血清から得られるポリクローナル、測定対象物を免疫した動物の脾臓をミエローマ細胞と細胞融合して得られるモノクローナル抗体のいずれを用いてもよい。また、それらの断片[例えば、F(ab’)2、Fab、Fab’、又はFv]を用いることができる。
 抗ヘモグロビン抗体を担持するために用いられる不溶性担体は、少なくとも1種であるが、少なくとも2種であってもよい。1種の不溶性担体には1種又は2種以上の抗ヘモグロビン抗体が担持されていてもよい。また、1種の抗ヘモグロビン抗体が、1種又は2種以上の不溶性担体に担持されていてもよい。
 測定試薬に2種以上の抗ヘモグロビン抗体が含まれる場合、2種以上の抗ヘモグロビン抗体が共に同じ不溶性担体に担持されてもよい。あるいは、1種の不溶性担体に1種の抗ヘモグロビン抗体を担持させた不溶性担体を複数種混合して用いることもできる。このとき、異なる種の抗ヘモグロビン抗体を担持するために用いられる不溶性担体は、同種の不溶性担体であってもよく、材質又は粒径等が異なる異種の不溶性担体であってもよい。
 抗ハプトグロビン抗体を担持するために用いられる不溶性担体は、少なくとも1種であるが、少なくとも2種であってもよい。1種の不溶性担体には1種又は2種以上の抗ハプトグロビン抗体が担持されていてもよい。また、1種の抗ハプトグロビン抗体が、1種又は2種以上の不溶性担体に担持されていてもよい。測定試薬に2種以上の抗ハプトグロビン抗体が含まれる場合は、上述の測定試薬に2種以上の抗ヘモグロビン抗体が含まれる場合と同様である。
 測定試薬の形態は特に限定されず、例えば、不溶性担体を含有しない試薬(第1試薬)及び抗体を担持する不溶性担体(抗体担持不溶性担体)を含有する試薬(第2試薬)の二試薬系で構成されてもよく、抗体担持不溶性担体を含有する試薬のみの一試薬系で構成されるようにしてもよい。
 第1試薬は反応系における、測定対象物や夾雑物の濃度の調整や反応速度の調整等の点から希釈液として用いるなど、測定環境を調整するよう用いることができる。第2試薬は、抗体担持不溶性担体を含有しており、第1試薬及び試料と混合されて、免疫凝集反応を生じる。第1試薬及び第2試薬は、pH緩衝剤、塩、界面活性剤、凝集促進剤、防腐剤などを、適宜含むことができる。凝集反応時のpHとしては5~9が好ましい。
 また、測定試薬は保存溶液と混合して反応液を得るが、不溶性担体の反応液中の濃度は、使用する不溶性担体の粒径や測定系全体の設計にあわせ、例えば0.0001mg/mL~10mg/mLの範囲から適宜選択をすることができる。測定試薬における抗ヘモグロビン抗体を担持する不溶性担体の濃度は、0.01~20mg/mL、又は0.05~1mg/mLであってよく、抗ハプトグロビン抗体を担持する不溶性担体の濃度は、0.01~20mg/mL、又は0.05~1mg/mLであってよい。
 なお、不溶性担体の第二試薬中の濃度は、使用時には保存溶液と混合することにより、又は、第一試薬と保存溶液と混合することにより希釈されるため、その第二試薬中の不溶性担体の濃度は、希釈倍率に応じて適宜選択することができ、例えば、2倍希釈して用いる場合は、0.004mg/mL~80mg/mL、3倍希釈して用いる場合は、0.06mg/mL~120mg/mLとなるように適宜調整することができる。
<ヘモグロビンの測定方法>
[工程(1)]
 工程(1)において、検体とハプトグロビンとを混合し、ヘモグロビン-ハプトグロビン複合体を形成させ、ヘモグロビン-ハプトグロビン複合体を含む試料を得る。
 検体とハプトグロビンとの混合は、上記[試料]の項目に記載のとおりである。ここで、試料中に含まれるヘモグロビン-ハプトグロビン複合体は、完全ヘモグロビン-ハプトグロビン複合体及びヘモグロビン-ハプトグロビン複合体中間体の両方を含み得る。試料はまた、検体由来の遊離ヘモグロビン、検体由来のヘモグロビン-ハプトグロビン複合体、及び検体由来の他の成分を含み得、さらに、ヘモグロビンと複合体を形成しない遊離ハプトグロビンを含み得る。検体と混合するためのハプトグロビンは検体保存用溶液に含まれてもよく、ハプトグロビンが検体保存用溶液に含まれる形で添加される場合、試料はヘモグロビンと複合体を形成しない遊離ハプトグロビンのほか、保存溶液の他の成分も含み得る。検体由来の遊離ヘモグロビンが全てハプトグロビンと複合体を形成することが好ましいため、検体に混合するハプトグロビンの量が、検体中の遊離ヘモグロビンに対して過剰量であってよい。
[工程(2)]
 工程(2)において、工程(1)で得られた試料と、抗ヘモグロビン抗体を担持する不溶性担体、及び抗ハプトグロビン抗体を担持する不溶性担体とを接触させ、免疫凝集反応を生じさせる。
 上記接触によって、抗ヘモグロビン抗体が試料中のヘモグロビンを認識し、それによって抗ヘモグロビン抗体を担持する不溶性担体がヘモグロビン又はヘモグロビン-ハプトグロビン複合体に結合し、さらに不溶性担体同士が凝集する。不溶性担体の凝集により溶液の濁度が変化するため、溶液の濁度の変化を測定し、ヘモグロビン濃度既知のヘモグロビン又はヘモグロビン-ハプトグロビン複合体を用いて作成した検量線により、試料中のヘモグロビン濃度を求めることができる。
 濁度の変化の測定は、免疫反応液の吸光度や散乱光等を光学的手法により測定する方法があげられる。光学的手法としては、例えば、汎用の光学的測定装置を用いてもよく、例えば、生化学自動分析装置「JCA-BM2250」(日本電子株式会社製)又は「JCA-BM6070」(日本電子株式会社製)を用いて測定することができる。
 本発明の測定方法は、従来の抗ヘモグロビン抗体を担持する不溶性担体に加えて、抗ハプトグロビン抗体を担持する不溶性担体も試料に接触させることにより、従来のハプトグロビンの添加に起因するヘモグロビンの測定値の低下を抑制でき、検体中のヘモグロビンをより正確に検出及び測定することができる。なお、検体中のヘモグロビンを測定する際にハプトグロビンを添加しない場合(すなわち、工程(1)が行われない場合)において、検体中に元々ハプトグロビン(遊離ハプトグロビン、ヘモグロビン-ハプトグロビン複合体中間体、完全ヘモグロビン-ハプトグロビン複合体を問わず)が存在していても、検体と、抗ヘモグロビン抗体を担持する不溶性担体、及び抗ハプトグロビン抗体を担持する不溶性担体とを接触させ、免疫凝集させる(すなわち、工程(2)のみ)ことで、ヘモグロビンの測定値の低下を抑制して、検体中のヘモグロビンを正確に検出、測定できる。
 本発明は、上記の方法により検体中のヘモグロビンを検出する際に、使用することのできるキットをさらに提供する。キットは、抗ヘモグロビン抗体担持不溶性担体及び抗ハプトグロビン抗体担持不溶性担体を含む測定試薬、ハプトグロビン(好ましくは、ハプトグロビンを含む保存溶液)、キャリブレータ、及びコントロールなどの構成を含んでもよく、また、検体を採取するための器具及び容器、検体を保存するための保存溶液などの構成を含んでいてもよい。
<実施例1-1~4-2、比較例1>
[測定試薬の調製]
 測定試薬として、第1試薬及び第2試薬を調製した。
 第1試薬として、50mM HEPES緩衝液(pH7.4)を用いた。
 第2試薬として50mM HEPES緩衝液(pH7.4)と、複数種の抗ヘモグロビンモノクローナル抗体(抗Hb抗体)それぞれの抗体を担持する各ポリスチレンラテックス、及び抗ハプトグロビンモノクローナル抗体を担持するポリスチレンラテックスを、表1に示す濃度で混合して、表1に示す最終的なラテックス濃度を有する実施例1-1~1-4及び比較例1の免疫凝集反応測定用の試薬を調製した。同様に、第2試薬として、所定の濃度で混合し、表2に示す最終的なラテックス濃度を有する実施例2-1~2-3、実施例3-1~3-3、及び実施例4-1~4-2の免疫凝集反応測定用の試薬を調製した。
 抗ハプトグロビンモノクローナル抗体としてAO-53(α鎖認識)、AO-27(α鎖認識)、AO-35(α鎖認識)、若しくはAN12-8(β鎖認識)を使用した。抗ハプトグロビン抗体は、下記の[試料の調製]の項目において添加する量の遊離ハプトグロビンでは、抗ハプトグロビン抗体を担持した不溶性担体と凝集しないことを確認したものを用いた。
 抗体担持ポリスチレンラテックスは公知の方法により調製した。すなわち、各抗ヘモグロビンモノクローナル抗体又は抗ハプトグロビンモノクローナル抗体とポリスチレンラテックス粒子(平均粒径200nm)を混合して、ポリスチレンラテックス粒子表面に抗ヘモグロビンモノクローナル抗体又は抗ハプトグロビンモノクローナル抗体を担持することにより調製した。

[試料の調製]
 ヒト血液から精製したヒト・ヘモグロビン(hHb)、及びヒト・ハプトグロビン(hHp)(Haptoglobin Human, Phenotype 1-1(SIGMA社製))をHbキャリブレータ希釈液‘栄研’(栄研化学株式会社製)中に、表3に示す濃度となるように混合し、No.0~6の試料を調製した。No.3は、ヒト・ヘモグロビン800μg/Lに対してヒト・ハプトグロビン0.8単位/Lを混合しており、ヘモグロビンとハプトグロビンがすべて完全ヘモグロビン-ハプトグロビン複合体を形成している。
[免疫凝集]
 No.0~6の試料に、それぞれ実施例1-1~1-4、実施例2-1~2-3、実施例3-1~3-3、実施例4-1~4-2及び比較例1の測定試薬を添加し自動分析機BM2250を用いて濁度を測定し、あらかじめ作成した検量線に基づき、ヘモグロビンの濃度換算値を算出した。さらにNo.0(ヒト・ハプトグロビンを含まない試料)に対するヘモグロビンの相対値(%)も算出した。結果を表3に示す。
 JCA-BM2250における測定条件は次のとおりである。

 表3から、ヘモグロビンの測定に際してハプトグロビンを添加した場合において、抗ヘモグロビン抗体を担持するラテックスのみを添加し、抗ハプトグロビン抗体を担持するラテックスを添加しない比較例1と比べて、抗ヘモグロビン抗体を担持するラテックスとともに、抗ハプトグロビン抗体を担持するラテックスを添加する各実施例は、ハプトグロビン添加によるヘモグロビン測定値の低下を抑制できること、すなわち、実際の値により近い測定値が得られることを判明した。この結果は、使用したα鎖認識及びβ鎖認識のいずれの抗ハプトグロビン抗体においても得られたため、抗ハプトグロビン抗体が認識するサブユニットに依存しないことが証明された。
<比較例2-1~2-3、3-1~3-3、4~7>
 実施例1-1における抗ハプトグロビン抗体担持ラテックスの代わりに、ラテックスに担持されていない遊離の抗ハプトグロビン抗体を、抗ヘモグロビン抗体担持ラテックスと混合した試薬を調製し、No.0~6の試料に添加し、同様に凝集反応を生じさせ、ヘモグロビンの濃度換算値及びヘモグロビンの相対値(%)を算出した。各比較例の試薬の組成は表4に、結果は表5に示す。なお、抗ハプトグロビン抗体AN11-2及びAN12-3はβ鎖認識抗ハプトグロビンモノクローナル抗体であり、公知の方法により作製した。
 表5から、ラテックスに結合されていない遊離の抗ハプトグロビン抗体を添加しても、ハプトグロビン添加によるヘモグロビン測定値の低下を抑制できなかった。むしろ、より低値となる傾向を示した。その原因は恐らく、過剰に存在する抗ハプトグロビン抗体によって、ラテックスに結合されている抗ヘモグロビン抗体とヘモグロビン-ハプトグロビン複合体との結合を阻害しているものと推測される。

<実施例5-1~5-5、比較例8>
 [測定試薬の調製]
 測定試薬として、第1試薬及び第2試薬を調製した。
 第1試薬として、50mM HEPES緩衝液(pH7.4)を用いた。
 第2試薬として50mM HEPES緩衝液(pH7.4)と、複数種の抗ヘモグロビンモノクローナル抗体(抗Hb抗体)それぞれの抗体を担持する各ポリスチレンラテックス、及び抗ハプトグロビンモノクローナル抗体を担持するポリスチレンラテックスを、表6に示す濃度で混合して、表6に示す最終的なラテックス濃度を有する実施例5-1~5-5及び比較例8の免疫凝集反応測定用の試薬を調製した。
 抗ハプトグロビンモノクローナル抗体としてAO-53(α鎖認識)、若しくはAN12-8(β鎖認識)を使用した。抗ハプトグロビン抗体は、下記の[試料の調製]の項目において添加する量の遊離ハプトグロビンでは、抗ハプトグロビン抗体を担持した不溶性担体と凝集しないことを確認したものを用いた。
 確認の方法は、第2試薬として、AO-53(α鎖認識)、若しくはAN12-8(β鎖認識)を担持したポリスチレンラテックス1.0mg/mLを用いた点、試料として、遊離ハプトグロビンを0ng/mL若しくは24.5pmol/mLとなるように保存溶液(Hbキャリブレータ希釈液‘栄研’(栄研化学株式会社製))に添加したもの(ヘモグロビンを含まない)を用いた点を除いて、後述する試料の調製及び免疫凝集方法と同様に行った。なお、24.5pmol/mLのハプトグロビン濃度は、1.2単位/Lの遊離ハプトグロビン添加濃度に相当する。
 抗体担持ポリスチレンラテックスは公知の方法により調製した。すなわち、各抗ヘモグロビンモノクローナル抗体又は抗ハプトグロビンモノクローナル抗体とポリスチレンラテックス粒子(平均粒径200nm)を混合して、ポリスチレンラテックス粒子表面に抗ヘモグロビンモノクローナル抗体又は抗ハプトグロビンモノクローナル抗体を担持することにより調製した。

[試料の調製]
 健常者から糞便検体を2つ(糞便検体1及び2)採取した。それぞれの糞便検体を、糞便濃度が0.5%(W/V%)となるように、また、ヒト血液から精製したヒト・ヘモグロビン(hHb)、及びヒト・ハプトグロビン(hHp)(Haptoglobin Human, Phenotype 1-1(SIGMA社製))を、表7及び8に示す濃度となるように、保存溶液(Hbキャリブレータ希釈液‘栄研’(栄研化学株式会社製))中に混合し、No.0~5の試料を調製した。No.3は、ヒト・ヘモグロビン600μg/Lに対してヒト・ハプトグロビン0.6単位/Lを混合しており、ヘモグロビンとハプトグロビンがすべて完全ヘモグロビン-ハプトグロビン複合体を形成している。
[免疫凝集]
 糞便検体1及び2のそれぞれのNo.0~5の試料に、それぞれ実施例5-1~5-5及び比較例8の測定試薬を添加し自動分析機JCA-BM6070を用いて濁度を測定し、あらかじめ作成した検量線に基づき、ヘモグロビンの濃度換算値を算出した。さらにNo.0(ヒト・ハプトグロビンを含まない試料)に対するヘモグロビンの相対値(%)も算出した。結果を表7及び8に示す。
 JCA-BM6070における測定条件は次のとおりである。

Claims (15)

  1.  抗ヘモグロビン抗体を担持する不溶性担体と、抗ハプトグロビン抗体を担持する不溶性担体とを含む、ヘモグロビンを測定するための試薬。
  2.  抗ヘモグロビン抗体が少なくとも2種である、請求項1に記載の試薬。
  3.  不溶性担体がラテックス粒子及び/又は金コロイド粒子である、請求項1又は2に記載の試薬。
  4.  抗ヘモグロビン抗体及び抗ハプトグロビン抗体がモノクローナル抗体である、請求項1~3のいずれか一項に記載の試薬。
  5.  抗ハプトグロビン抗体が1種である、請求項1~4のいずれか一項に記載の試薬。
  6.  検体中のヘモグロビンを測定するための方法であって、
     (1)検体とハプトグロビンとを混合し、ヘモグロビン-ハプトグロビン複合体を形成させ、ヘモグロビン-ハプトグロビン複合体を含む試料を得る工程と、
     (2)工程(1)で得られた試料と、抗ヘモグロビン抗体を担持する不溶性担体、及び抗ハプトグロビン抗体を担持する不溶性担体とを接触させ、免疫凝集させる工程と
    を含むヘモグロビンの測定方法。
  7.  抗ヘモグロビン抗体が少なくとも2種である、請求項6に記載の測定方法。
  8.  不溶性担体がラテックス粒子及び/又は金コロイド粒子である、請求項6又は7に記載の測定方法。
  9.  抗ヘモグロビン抗体及び抗ハプトグロビン抗体がモノクローナル抗体である、請求項6~8のいずれか一項に記載の測定方法。
  10.  検体が糞便、唾液、又は尿である、請求項6~9のいずれか一項に記載の測定方法。
  11.  ハプトグロビンが検体保存用溶液に含まれる、請求項6~10のいずれか一項に記載の測定方法。
  12.  検体保存用溶液におけるハプトグロビン濃度が0.05単位/L~50単位/Lである、請求項11に記載の測定方法。
  13.  検体中のヘモグロビンが、遊離ヘモグロビン、ヘモグロビン-ハプトグロビン複合体中間体、及び完全ヘモグロビン-ハプトグロビン複合体からなる群から選ばれる少なくとも一つを含み、前記遊離ヘモグロビンの少なくとも一部は、工程(1)の後に、前記ハプトグロビンと、ヘモグロビン-ハプトグロビン複合体中間体及び/又は完全ヘモグロビン-ハプトグロビン複合体を形成する、請求項6~12のいずれか一項に記載の測定方法。
  14.  検体と、抗ヘモグロビン抗体を担持する不溶性担体、及び抗ハプトグロビン抗体を担持する不溶性担体とを接触させ、免疫凝集させる工程を含む、検体中のヘモグロビンを測定するための方法。
  15.  検体と、抗ヘモグロビン抗体を担持する不溶性担体、及び抗ハプトグロビン抗体を担持する不溶性担体とを接触させ、免疫凝集させる工程を含む、検体中のヘモグロビンの測定値の低下を抑制する方法。
