内視鏡用処置具
 本発明は、内視鏡用処置具に関するものである。
 胆管や膀胱等の臓器内に発生した結石を破砕する場合に、内視鏡のチャンネルに挿入して経内視鏡的に処置を行う内視鏡用処置具が知られている(例えば、特許文献1参照。)。
 また、可撓管の先端に固定された短管状で硬質の先端チップの内孔からバスケットワイヤを突出させる内視鏡用処置具において、バスケットワイヤ内に収容した結石を破砕する場合に、バスケットワイヤが先端チップと結石との間に挟まれて、バスケットワイヤに力が伝わらなくなるという不都合を解決するために、先端チップに、バスケットワイヤが嵌入する逃げ溝を設けた内視鏡用処置具が知られている(例えば、特許文献2参照。)。

 特許文献2の内視鏡用処置具によれば、先端チップに設けられた逃げ溝にバスケットワイヤを構成する各ワイヤ素線を配置することにより、結石と先端チップとの間にバスケットワイヤが挟まれることは回避される。しかし、包囲した結石よりも基端側に位置するバスケットワイヤが、先端チップの内孔内に引き込まれる位置で、逃げ溝の基端側の溝壁に強く押し付けられるため、バスケットワイヤと先端チップとの間の摩擦を効果的に低減することが困難である。
 本発明は、ワイヤが強くしごかれることがなく、ワイヤの変形を抑え、ワイヤにかかる摩擦力を低減して小さい牽引力で効果的に結石を破砕することができる内視鏡用処置具を提供することを目的としている。
 本発明の一態様は、長手軸を有する管状のシースと、該シースの内部に前記長手軸に沿って移動可能に配置された操作ワイヤと、該操作ワイヤの先端に設けられ結石を把持するための1以上のワイヤを有する把持部と、前記シースの先端に取り付けられた筒状の金属製の先端チップと、該先端チップの径方向内方または外方に隣接する位置に配置される樹脂部材とを備え、前記先端チップに、該先端チップの先端から該先端チップの基端に向かって延びており、該先端チップの内周面から外周面まで貫通し、前記把持部の前記ワイヤが挿通可能な寸法を有する逃げ溝が設けられ、前記先端チップの径方向において前記逃げ溝の少なくとも基端側と前記樹脂部材とが重なっており、前記樹脂部材の先端が、前記逃げ溝の基端よりも先端側に配置されている内視鏡用処置具である。
 本態様によれば、先端チップおよびシースの先端部を体内に配置し、体外に配置されたシースの基端側において、操作ワイヤを操作することにより、操作ワイヤの先端に設けられた把持部を構成するワイヤによって結石を把持した状態で、操作ワイヤを基端側に牽引すると、結石が先端チップの先端に押し付けられる。把持部を構成するワイヤは先端チップの先端に設けられた逃げ溝内に入るので、把持した結石と先端チップとの間に挟まれることなく、先端チップの内部から先端側に向かって広がる形態で傾斜して逃げ溝を挿通する。
 この場合において、逃げ溝は先端チップの先端から該先端チップの基端に向かって延びているので、逃げ溝を挿通するワイヤの角度によってはワイヤが逃げ溝の基端側の溝壁に近接する。この場合においても、樹脂部材が先端チップの径方向内方または外方に隣接して配置され、かつ、先端チップの径方向において逃げ溝の少なくとも基端側と樹脂部材とが重なっており、樹脂部材の先端が、逃げ溝の基端よりも先端側に配置されているので、ワイヤは樹脂部材に接触し、金属製の先端チップに設けられた逃げ溝の基端に接触することが阻止される。これにより、ワイヤが硬い先端チップと接触しないため、ワイヤが強くしごかれることがなく、ワイヤの変形を抑えることができる。
 すなわち、牽引されることにより大きな張力がワイヤに付与された状態であっても、ワイヤに作用する摩擦力は、樹脂部材との接触によって小さく維持される。これにより、操作ワイヤに加えた牽引力を、把持部を構成するワイヤに効率よく伝達することができ、把持部によって把持した結石をより小さい牽引力で破砕することができる。
 上記態様においては、前記樹脂部材が、前記先端チップの径方向内方に配置されていてもよい。
 この構成により、先端チップの内側から先端に向かって広がる形態で傾斜するワイヤは樹脂部材の内面側に径方向外方に向かって押し付けられる。樹脂部材が先端チップの径方向内方に隣接する位置に配置されていることにより、外側に押された樹脂部材が先端チップとワイヤとの間に挟まれるので、より確実にワイヤと先端チップとの接触を阻止することができる。
 また、上記態様においては、前記樹脂部材の先端が、前記先端チップの先端よりも基端側に配置されていてもよい。
 また、上記態様においては、前記樹脂部材の先端が、前記先端チップの先端よりも前記先端チップの基端側、かつ前記逃げ溝の基端よりも前記先端チップの先端側に位置していてもよい。
 また、上記態様においては、前記樹脂部材が管状に形成されており、前記ワイヤが前記逃げ溝に挿通された状態において、前記ワイヤと前記逃げ溝の基端との間に前記樹脂部材が位置していてもよい。
 また、上記態様においては、前記樹脂部材が、前記先端チップの内周面または外周面の少なくとも一部に装着されている管状に形成されていてもよい。
 この構成により、逃げ溝の少なくとも基端側に重なる樹脂部材を単純な形状で実現することができる。
 また、上記態様においては、前記シースが、素線をコイル状に巻いたコイルシースであり、前記樹脂部材が、前記コイルシースの内周面または外周面に装着されていてもよい。
 この構成により、コイルシースの内周面または外周面に装着される樹脂部材の先端部をワイヤと先端チップとの接触を阻止するために利用することができる。樹脂部材をコイルシースの内周面側に配置する場合には、コイルシースに対する操作ワイヤの摺動摩擦を低減するライナとして機能させることができる。樹脂部材をコイルシースの外周面側に配置する場合には、コイルシースを被覆する外装として機能させることができる。
 また、上記態様においては、前記把持部が、バスケットワイヤであってもよい。
 また、上記態様においては、前記把持部が、スネアであってもよい。
 また、上記態様においては、前記把持部の前記ワイヤは、逃げ溝内を挿通する位置に配置された状態で、前記シースに対して前記操作ワイヤを基端側に牽引すると、前記樹脂部材に接触した状態に維持されてもよい。
 本発明によれば、ワイヤが硬い先端チップと接触しないため、ワイヤが強くしごかれることがなく、ワイヤの変形を抑え、ワイヤにかかる摩擦力を低減して小さい牽引力で効果的に結石を破砕することができるという効果を奏する。

 本発明の第1の実施形態に係る内視鏡用処置具1について図面を参照して以下に説明する。
 本実施形態に係る内視鏡用処置具1として、患者の結石Xを収容するバスケットを備える結石破砕装置を用いて説明する。本実施形態に係る内視鏡用処置具1は、図1に示されるように、長尺の挿入部2と、該挿入部2の基端に固定された操作部3とを備えている。
 挿入部2は、管状のシース4と、シース4内部にシース4の長手軸方向に沿って移動可能に配置された操作ワイヤ5と、操作ワイヤ5の先端に設けられたバスケットワイヤ(把持部)6と、シース4の先端に固定された先端チップ7と、シース4内に配置され、先端が先端チップ7の外面に固定されたチューブ(樹脂部材)8とを備えている。
 シース4は、例えば、挿入経路に倣って湾曲可能な可撓性を有し、かつ、圧縮強度の高いコイルシースである。
 先端チップ7は、金属製の円筒状部材であって、円筒状の接続部材によってシース4の先端に同軸に固定されている。
 操作ワイヤ5は、シース4の全長にわたって配置され、シース4の基端側に取り付けられた操作部3によって基端側に牽引される。
 バスケットワイヤ6は、例えば、図1および図2に示されるように、操作ワイヤ5の先端において基端部が束ねられる4本(図では3本のみ表示)のワイヤ9と、これらのワイヤ9の先端部を束ねる先端部材10とを備え、各ワイヤ9が折り曲げられることによってシース4内に収容可能であるとともに、シース4内から前方に押し出されることにより、図1に示されるように広がって、所定の空間を取り囲むバスケットを構成する。
 先端チップ7には、図2および図3に示されるように、先端から基端側に向かって延びる複数の逃げ溝11が設けられている。各逃げ溝11は、先端チップ7の長手軸に平行に長手軸方向の所定位置まで延びている。図2および図3に示す例では、逃げ溝11は、バスケットワイヤ6を構成する4本のワイヤ9をそれぞれ収容するために周方向に間隔を空けて4箇所に設けられている。
 各逃げ溝11は、バスケットワイヤ6を構成する各ワイヤ9の直径よりも大きい溝幅寸法を有している。
 チューブ8は、PEEKあるいはフッ素樹脂(例えば、4フッ化エチレン樹脂等)の滑り性の高い樹脂により構成されている。
 チューブ8はシース4内部に配置されることにより、操作ワイヤ5とシース4との間に配置され、シース4内面に内張りして操作ワイヤ5とシース4との間に発生する摩擦を低減する。
 本実施形態においては、チューブ8は、図2に示されるように、先端チップ7の基端側において先端チップ7の外周面を部分的に被覆する位置に配置されている。すなわち、チューブ8は、先端チップ7の長手軸方向の途中位置に先端面が配置される位置まで先端チップ7の外周面を被覆している。チューブ8の先端面は、図2および図3に示されるように、先端チップ7に設けられた逃げ溝11の基端12よりも先端側に配置されている。
 より具体的には、先端チップ7の径方向において、逃げ溝11の少なくとも基端側とチューブ8とが重なっている。そして、チューブ8の先端面は、先端チップ7の先端よりも先端チップ7の基端側、かつ、逃げ溝11の基端12よりも先端チップ7の先端側に位置している。
 この場合には、図4に示されるように、先端チップ7の先端とチューブ8の先端面との間の距離E1が、バスケットワイヤ6を構成するワイヤ9の外径寸法D1よりも大きいことが望ましい。
 操作部3は、例えば、図1および図5から図7に示されるように、シース4の基端に接続される本体13と、本体13に対して前後方向に移動可能に設けられ操作ワイヤ5に接続された結合部材14と、結合部材14に設けられたラックギヤ15に噛み合うピニオンギヤ16を備える第1ハンドル17と、結合部材14に固定された第2ハンドル18と、第2ハンドル18よりも本体13の基端側に固定された支持部19とを備えている。
 操作部3を操作する際には、図7に示されるように、左手の親指を支持部19に掛け、左手の人差し指から小指を第2ハンドル18に掛けて、左手の握力によって第2ハンドル18を支持部19に近づける方向に移動させる。これにより、第2ハンドル18が固定されている結合部材14が本体13に対して基端側に移動させられ、結合部材14に固定された操作ワイヤ5が基端側に牽引される。
 また、本体13に対して、右手で第1ハンドル17を回転させることにより、第1ハンドル17に固定されたピニオンギヤ16が回転し、ピニオンギヤ16が噛み合っているラックギヤ15が本体13の基端側に移動させられる。これにより、ラックギヤ15が固定されている結合部材14が本体13の基端側に移動させられ、結合部材14に固定された操作ワイヤ5が基端側に牽引される。
 このように構成された本実施形態に係る内視鏡用処置具1の作用について以下に説明する。
 本実施形態に係る内視鏡用処置具1を用いて患者の体内の結石Xを破砕するには、シース4の基端に配置されている操作部3を操作して、シース4内にバスケットワイヤ6を引き込むことにより、バスケットワイヤ6を収縮状態として、体腔に挿入された図示しない内視鏡のチャンネルに挿入部2を挿入し、内視鏡用処置具1の先端チップ7の先端を体内の結石Xに近接させた状態に配置する。
 この状態で、術者が、患者の体外に配置されている操作部3を操作することにより、操作ワイヤ5を先端側に押し出して、先端チップ7の先端からバスケットワイヤ6を前方に突出させる。バスケットワイヤ6は先端チップ7から前方に押し出されることにより、収縮状態から解放されて展開される。この状態で、バスケットワイヤ6内部に結石Xを収容し、操作部3を操作して操作ワイヤ5を基端側に牽引する。
 操作ワイヤ5が基端側に牽引されると、バスケットワイヤ6を構成している各ワイヤ9が先端チップ7内に引き込まれていく。そして、バスケットワイヤ6に把持された結石Xが、先端チップ7に突き当たる位置まで牽引された後、さらに操作ワイヤ5に牽引力を加える。これにより、バスケットワイヤ6と先端チップ7の先端との間で緊縛された結石Xが破砕される。
 この場合において、先端チップ7には逃げ溝11が設けられ、各ワイヤ9が逃げ溝11内に収容されるので、結石Xが先端チップ7の先端に突き当たるまで操作ワイヤ5を牽引しても、バスケットワイヤ6を構成している各ワイヤ9が結石Xと先端チップ7との間に挟まれることが防止される。
 そして、この状態において、チューブ8の先端が、逃げ溝11の基端12よりも先端側に配置されているので、逃げ溝11内に収容された各ワイヤ9はチューブ8の先端に接触し、逃げ溝11の基端12に接触することが防止される。すなわち、牽引によって大きな張力が作用している状態のワイヤ9が金属製の先端チップ7に押し付けられることが防止される。これにより、ワイヤ9が硬い先端チップ7と接触しないため、ワイヤ9が強くしごかれることがなく、ワイヤ9の変形を抑えることができる。
 ワイヤ9によってチューブ8の先端が押し付けられた場合にも、チューブ8が先端チップ7の内周面により保持されており、チューブ8が逃げ溝11内に入り込むような変形が生じ難いため、ワイヤ9とチューブ8との間に発生する摩擦力を小さく維持することができる。
 また、ワイヤ9が押し付けられるチューブ8は樹脂製であるため、ワイヤ9とチューブ8との間に発生する摩擦力を小さく維持することができる。これにより、操作ワイヤ5に加えた牽引力を、摩擦による損失を低減してバスケットワイヤ6に効率よく伝達することができ、バスケットワイヤ6によって把持した結石Xをより小さい牽引力で破砕することができるという利点がある。
 また、操作部3が、例えば、左手で支持部19と第2ハンドル18とを握り、左手の握力によって第2ハンドル18を支持部19側に引くとともに、右手で第1ハンドル17を回転させることにより、両手で、結合部材14を基端側に牽引する操作を行うので、より強い力で操作ワイヤ5を牽引することができ、より容易に結石Xを破砕することができる。
 また、第1ハンドル17を矢印Aの方向に回転させる場合、張力が大きくなって第1ハンドル17を回転させ難くなってくると、本体を矢印Cの方向に傾けるモーメントが作用する。本実施形態の操作部3によれば、第2ハンドル18が第1ハンドル17の回転中心よりも前側に配置されているので、第2ハンドル18を把持する左手によって、本体13に矢印Cとは逆方向のモーメントを付与することができる。これにより、第1ハンドル17の回転方向に本体13が傾くことを防止することができ、第1ハンドル17に力を掛け易くなるという利点がある。
 次に、本発明の第2の実施形態に係る内視鏡用処置具について、図面を参照して以下に説明する。
 本実施形態の説明において、上述した第1の実施形態に係る内視鏡用処置具1と構成を共通とする箇所には同一符号を付して説明を省略する。
 本実施形態に係る内視鏡用処置具は、図8から図11に示されるように、チューブ8とシース4とが固定されていない点で、第1の実施形態と相違している。チューブ8は先端チップ7の内面側に長手軸方向に移動可能に配置されている。
 コイルシースからなるシース4は湾曲することによって、コイルシースを構成している線材同士の間隔が広がるためシース4の中心軸線に沿う長さが長くなる。
 例えば、真っ直ぐな状態において、図10に示されるように、チューブ8の先端が先端チップ7の先端に一致していても、シース4が湾曲すると、チューブ8がシース4内において移動し、図11に示されるように、チューブ8の先端が先端チップ7の先端よりも基端側に移動する。
 本実施形態においては、チューブ8とシース4とは、想定される使用状態において、シース4に対してチューブ8の先端が最も基端側に移動した状態であっても、ワイヤ9とシース4の逃げ溝11の基端12との間にチューブ8が挟まれる位置関係となっている。
 チューブ8の先端が最も基端側に移動した状態としては、例えば、図9に示されるように、チューブ8の先端が逃げ溝11の基端側の側面と一致する位置となった状態である。
 このように構成された本実施形態に係る内視鏡用処置具の作用について以下に説明する。
 本実施形態に係る内視鏡用処置具においても、操作ワイヤ5が牽引されることによりワイヤ9に張力が作用すると、バスケットワイヤ6に取り込まれた結石Xが先端チップ7の先端に突き当たる位置まで引き寄せられる。この場合においても、バスケットワイヤ6を構成している各ワイヤ9は先端チップ7の逃げ溝11に収容されるので、先端チップ7と結石Xとの間に挟まれることが防止される。
 そして、本実施形態においても、バスケットワイヤ6を行使している各ワイヤ9に大きな張力が作用しても、ワイヤ9と先端チップ7との間にはチューブ8が配置されるので、ワイヤ9が逃げ溝11の基端側の側壁に直接押し付けられることが防止される。すなわち、ワイヤ9は金属製の先端チップ7よりも滑り性の高い樹脂製のチューブ8に接触するので、摩擦による損失を低減してバスケットワイヤ6に効率よく伝達することができ、バスケットワイヤ6によって把持した結石Xをより小さい牽引力で破砕することができる。
 本実施形態に係る内視鏡用処置具によれば、チューブ8が先端チップ7の内面側に配置されているので、ワイヤ9と先端チップ7との間にチューブ8をより確実に配置することができるという利点がある。すなわち、ワイヤ9に大きな張力が作用してチューブ8に押し付けられると、チューブ8は径方向外方に変形するが、先端チップ7はチューブ8の径方向外方に配置されているため、チューブ8が変形してもワイヤ9と先端チップ7との接触をより確実に防止することができる。
 なお、本実施形態においては、チューブ8とシース4との位置関係として、想定される使用状態において、シース4に対してチューブ8の先端が最も基端側に移動した状態であっても、ワイヤ9とシース4の逃げ溝11の基端12との間にチューブ8が挟まれる位置関係であるとした。すなわち、チューブ8の先端の基端側位置について制限したが、先端側位置については任意でよい。
 例えば、図10に示されるように、先端チップ7の先端と同一位置であってもよいし、図12に示されるように、先端チップ7の先端から突出する位置に配置されていてもよい。
 これら図10および図12の状態で、ワイヤ9に大きな張力が作用した場合であっても、図13および図14に示されるように、ワイヤ9が押し付けられたチューブ8は逃げ溝11内に入り込むように部分的に変形して、ワイヤ9が逃げ溝11内に収容されるので、結石Xと先端チップ7との間にワイヤ9が挟まれることが防止され、かつ、逃げ溝11にワイヤ9が直接接触することも防止される。
 また、図15から図17に示されるように、バスケットワイヤ6によって結石Xを補捉した状態で、操作ワイヤ5を牽引したときに、チューブ8がシース4内に座屈することなく引き込まれる量であれば、結石Xを破砕する前に、チューブ8の先端と先端チップ7の先端とが略一致するまで、チューブ8が引き込まれるので、チューブ8の先端が先端チップ7の先端を大きく超えて突出していてもよい。
 また、チューブ8が変形しなくてもワイヤ9が逃げ溝11内に収容されるように、チューブ8の先端の逃げ溝11に一致する位相にワイヤ9の外径寸法よりも大きな切り欠き20を設けてもよい。
 切り欠き20の形状は、図18および図19に示されるようにV溝型であってもよいし、図20に示されるように、逃げ溝11と同様の形状であってもよい。図20において、逃げ溝の幅H1、切り欠きの幅H2とすると、ワイヤ9と先端チップ7との接触を可能な限りなくすために、H1>H2であることが好ましい。
 また、本実施形態においては、シース4に対して長手方向に移動可能なチューブ8がシース4の内部に配置されている場合について説明したが、これに代えて、図21に示されるように、先端チップ7および該先端チップ7の内面に配置される短尺のチューブ8がシース4の先端に固定されていてもよい。この場合においても、チューブ8の先端面が、先端チップ7の逃げ溝11の基端12よりも先端側に配置されていることが好ましい。図中、符号21は、シース4の外面を被覆する被覆チューブである。被覆チューブ21の先端は、逃げ溝11の基端12よりも先端チップ7の基端側に位置している。
 また、本実施形態に係る内視鏡用処置具1として、シース4の内側にチューブ8が配置されている例を示したが、これに代えて、図22および図23に示されるように、シース4の外側にチューブ8が配置されていてもよい。
 具体的には、チューブ8は、シース4の基端から先端チップ7の逃げ溝11の途中位置までにおいて、シース4の外面および先端チップ7の外面の一部を被覆している。この場合においても、チューブ8の先端面は、先端チップ7の逃げ溝11の基端12よりも先端側に配置されていることが必要である。
 また、操作部3としては、図24に示されるように、本体13に第1ハンドル17を備え、第2ハンドル18および支持部19を有しない構造のものを採用してもよい。
 また、本実施形態においては、把持部としてバスケットワイヤ6を用いたものを例示したが、これに代えて、図25に示されるように、スネア20を用いてもよい。

  1.  長手軸を有する管状のシースと、
     該シースの内部に前記長手軸に沿って移動可能に配置された操作ワイヤと、
     該操作ワイヤの先端に設けられ結石を把持するための1以上のワイヤを有する把持部と、
     前記シースの先端に取り付けられた筒状の金属製の先端チップと、
     該先端チップの径方向内方または外方に隣接する位置に配置される樹脂部材とを備え、
     前記先端チップに、該先端チップの先端から該先端チップの基端に向かって延びており、該先端チップの内周面から外周面まで貫通し、前記把持部の前記ワイヤが挿通可能な寸法を有する逃げ溝が設けられ、
     前記先端チップの径方向において前記逃げ溝の少なくとも基端側と前記樹脂部材とが重なっており、前記樹脂部材の先端が、前記逃げ溝の基端よりも先端側に配置されている内視鏡用処置具。

  7.  前記シースが、素線をコイル状に巻いたコイルシースであり、
     前記樹脂部材が、前記コイルシースの内周面または外周面に装着される請求項6に記載の内視鏡用処置具。
  8.  前記把持部が、バスケットワイヤである請求項1に記載の内視鏡用処置具。
  9.  前記把持部が、スネアである請求項1に記載の内視鏡用処置具。
  10.  前記把持部の前記ワイヤは、前記逃げ溝内を挿通する位置に配置された状態で、前記シースに対して前記操作ワイヤを基端側に牽引すると、前記樹脂部材に接触した状態に維持される請求項1に記載の内視鏡用処置具。
