Description
涙液の油層の異常により生じる「蒸発亢進型ドライアイ」のなかには、マイボーム腺機能不全、眼瞼炎がある。涙液の水層の異常により生じる「涙液減少型ドライアイ」には、シェーグレン症候群、スティーブンス・ジョンソン症候群などがある。
ドライアイ治療の最大の目的は、涙液の構造を正常に戻し、角結膜上皮障害や自覚症状を改善させることになる。
実際にドライアイの治療に用いられている点眼には、人工涙液、ヒアルロン酸ナトリウム、コンドロイチン硫酸ナトリウム、フラビンアデニンジヌクレオチドを含有する水性点眼液、血清点眼及び油性点眼などがあり、近年はジクアホソルナトリウムやレバミピドなどのドライアイ治療剤も販売されている。軽症のドライアイである場合、日本国内では人工涙液として、涙液の補助(目のかわき)を効能・効果としたOTCの眼科用薬が入手しやすい。
人工涙液の役割とは、不足している涙液を外部から補充して涙液の増加をはかる方法である。涙液量は正常人では6.5±0.3μL、ドライアイ患者では4.8±0.4μLと報告されており、結膜嚢の容量は20~30μL、点眼液1滴は約50μLとされているので、正常人、ドライアイ患者のいずれの場合も片眼に対し、人工涙液を1滴点眼すると結膜嚢内は水分で満たされることになる。
人工涙液としては、無機塩類を添加した水性点眼剤が挙げられる。しかしながら、このような水性点眼剤は点眼後鼻涙管を通して速やかに排出される、または眼表面から蒸発するといった現象が生じる。よって、眼表面の水分を維持するため、または自覚症状を軽減するためには、1回2~3滴、1日5~6回といった頻回点眼をしなければならない(非特許文献1~3)。
また、0.6%及び1.2%メチルセルロース含有生理食塩水よって、豚の眼球において角膜上皮創傷治癒効果が認められたことが知られている(非特許文献5)。
しかし、これらのメチルセルロース含有生理食塩水では、メチルセルロース特有の熱によってゲル化する性質を溶液上で確認しているものではなく、ゲル化による効果は全く考慮していない。
一方、ニューキノロン系抗菌剤を有効成分として、ゲル化温度が十分に低い、熱ゲル化水性医薬組成物が開示されている。また前記組成物として、オフロキサシンゲル化点眼液0.3%「わかもと」が角膜上皮障害に有効であることが報告されている(特許文献3、非特許文献6)。
前記は抗菌剤を含有するゲル化点眼液において、1種類のメチルセルロースを含有し、ゲル化する点眼液の効果を確認したものにすぎない。これらの特許文献に開示されている前記水性医薬組成物は、室温で保存した場合、組成物が徐々にゲル化し、点眼前に液体で投与しやすいという特徴が失われるため、通常、低温で保存することが想定されている。人工涙液の場合、前述の通り頻回点眼することが一般的であるが、可逆性熱ゲル化水性組成物は低温で保存する必要があるため、持ち歩くことは不適である。
メチルセルロース、ポリエチレングリコール及びクエン酸ナトリウム並びに糖アルコール、乳糖、カルメロースあるいはシクロデキストリンのいずれかを含有する水性医薬組成物として、室温でゲル化したとしても軽く振るなど弱い力を加えたときに急激に粘度が低下して液体に戻る組成物、すなわち、揺変性を持つ水性組成物が開示されている(特許文献4)。
ヒドロキシエチルセルロース並びにメチルセルロースあるいはヒプロメロースを含む水溶液からなる水性組成物では、ヒトの体温付近で瞬時に増粘してゲル化せず、軽く振るなどの弱い力を加えたときに急激に粘度が低下して流動性が高まる組成物が開示されている(特許文献5)。
このように携帯できる熱ゲル化製剤であったり、投与後の異物感を生じないような水性組成物であったりと製品の流通やQOLに着目した改良が進められてきた。しかし、ゾル-ゲル相転移を繰り返すことによって揺変性が再現可能な水性組成物については開示されていない。
本発明の課題はまた、角膜保護用、角膜上皮障害軽減用あるいは角膜損傷回復用眼科用水性組成物を提供することである。
本発明の課題はまた、角膜保護用、角膜上皮障害軽減用あるいは角膜損傷回復用眼科用水性組成物を調製するためのメチルセルロースの使用を提供することである。
1.(A) 少なくとも2種類のメチルセルロース、(B) ポリエチレングリコール、(C) ポリビニルピロリドン、及び(D) クエン酸又はその薬学的に許容される塩を含有し、ゲル化することを特徴とする眼科用水性組成物。
2.(A)成分が2種類のメチルセルロースである、前記1項記載の眼科用水性組成物。
3.2種類のメチルセルロースが1:30~30:1の質量比で含まれる、前記2項記載の眼科用水性組成物。
4.(A)成分が20℃における2w/v%水溶液の粘度が4~400mPa・sのメチルセルロースである、前記1~3のいずれか1項記載の眼科用水性組成物。
5.(A)成分が、20℃における2w/v%水溶液の粘度が4mPa・s、15mPa・s、25mPa・s、100mPa・s、400mPa・s、1500mPa・s又は4000mPa・sであるメチルセルロースからなる群から選ばれる2種である、前記1~3のいずれか1項記載の眼科用水性組成物。
6.(B)成分が、PEG8000、PEG4000、PEG800、PEG400及びPEG300からなる群から選ばれる少なくとも1種類である、前記1~5のいずれか1項に記載の眼科用水性組成物。
7.(C)成分が、PVP K17、PVP K25、PVP K30及びPVP K90からなる群から選ばれる少なくとも1種である、前記1~6のいずれか1項に記載の眼科用水性組成物。
8.熱ゲル化人工涙液である前記1~7に記載の眼科用水性組成物。
9.(A) 少なくとも2種類のメチルセルロース、(B)ポリエチレングリコール、(C) ポリビニルピロリドン、及び(D) クエン酸又はその薬学的に許容される塩を含有する、角膜保護用、角膜上皮障害軽減用又は角膜障害回復用眼科用水性組成物。
10.ポリエチレングリコール、ポリビニルピロリドン、及びクエン酸又はその薬学的に許容される塩と組み合わせて、角膜保護用、角膜上皮障害軽減用又は角膜障害回復用眼科用水性組成物を調製するための、少なくとも2種類のメチルセルロースの使用。
本発明に用いられるメチルセルロース(以下、MCともいう)は、医薬品添加物として市販されているものを、適宜使用することができる。例えば粘度で表せば2w/v%水溶液の20℃における粘度が12000mPa・s(ミリパスカル秒)以下であればよく、120mPa・s以下であるものがより好ましい。メトキシ基の含有率は26~33%の範囲が好ましい。さらにMCはその水溶液の粘度によって区別され、例えば、市販品の品種には表示粘度4、15、25、100、400、1500、4000(数字は2w/v%水溶液の20℃における粘度(単位はミリパスカル・秒)を示す。以降、単に「粘度」ないし「表示粘度」と述べた場合も同じである。例えば、「表示粘度が4」は、2w/v%水溶液の20℃における粘度が4ミリパスカル・秒であることを意味する)のものがあり、容易に入手可能である。表示粘度4~400のMCが取り扱いやすいため好ましい。表示粘度が4、15、25、100、400、1500及び4000からなる群から選ばれる2種のMCであるのがより好ましい。表示粘度が4のMC、表示粘度が15のMC、表示粘度が400のMCがさらに好ましく、特に、表示粘度が4のMCと表示粘度が15のMCとの組み合わせ、表示粘度が4のMCと表示粘度が400のMCとの組み合わせ、及び表示粘度が15のMCと表示粘度が400のMCとの組み合わせが好ましい。とりわけ、表示粘度が4のMCと表示粘度が15のMCとの組み合わせが好ましい。MCの概要、規格、用途、使用量及び商品名などについては医薬品添加物辞典(日本医薬品添加物協会編集、薬事日報社発行)に詳細に記載されている。また、MCは日本、米国、欧州の三薬局方での調査合意に基づき規定した医薬品各条として第十七改正日本薬局方に掲載されている。MCの規格の大部分は三薬局方で共通するものである。なお、MCの粘度は、第十七改正日本薬局方に記載の方法に従って測定できる。
本発明に用いられるポリエチレングリコール(以下、PEGともいう)は、医薬品添加物として市販されており、例えば、PEG-200、-300、-400、-600、-1000、-1500、-1540、-2000、-4000、-6000、-8000、-20000、-50000、-500000、-2000000及び-4000000の商品名で和光純薬工業(株)から、マクロゴール-200、-400、-1500、-4000、-6000または-20000の商品名で三洋化成工業(株)から、さらに、CARBOWAX(登録商標) PEG 200、300、400、540、600、1000、1450、3350、4000、4600、8000の商品名でDOW CHEMICAL COMPANYより販売されている。本発明の基剤に用いられるPEGの重量平均分子量は300~50000が好ましく、300~8000が特に好ましい。重量平均分子量が300以上の場合には体温によるゾル-ゲル相転移を起こしやすく、重量平均分子量が50000以下の場合には液体状態での粘度が高くなりすぎないため好ましい。とりわけ、重量平均分子量が4000又は8000のPEGが好ましい。また、2種以上のPEGを混合して重量平均分子量を上記の至適範囲内に調整することも可能である。PEGの概要、規格、用途、使用量及び商品名などについては医薬品添加物事典(日本医薬品添加物協会編集、薬事日報社発行)に詳細に記載されている。また、PEGの概要、規格は米国薬局方(2018 U.S. Pharmacopeia National Formulary, USP41 NF36、以下USP41(NF36)という)に詳細に記載されている。なお、PEG-400、-4000、-6000及び-20000の重量平均分子量は、第十七改正日本薬局方に記載の方法に従って測定できる。重量平均分子量が200から8000のPEGは、USP41(NF36)に従って粘度を測定することができ、粘度範囲が規定されている。
PEGに代えて又はPEGに追加して、ポリプロピレングリコールを用いることができる。ポリプロピレングリコールを含ませることにより、本発明の組成物の粘度を適度な範囲に調整することができる。本発明に用いることができるポリプロピレングリコールは、重量平均分子量が200~40000であるのが好ましく、200~1200であるのがより好ましく、200~700であるのが水への溶解性が高いためさらに好ましく、200~400が特に好ましい。なお、ポリプロピレングリコール2000の重量平均分子量は、医薬品添加物規格2013(薬事日報社発行)に記載の方法に従って測定できる。ポリプロピレングリコールは、例えば、ユニオール(登録商標)D-200、D-250、D-400、D-700、D-1000、D-1200、D-2000、D-4000の商品名で日油株式会社より販売されている。前記ユニオールDシリーズの重量平均分子量、動粘度は油化製品総合カタログ(日油株式会社油化事業部)に記載されている。
本発明に用いられるポリビニルピロリドン(以下、PVPともいう)は、医薬品添加物として市販されているものを、適宜使用することができる。具体的には、PVP K17、PVP K25、PVP K30、PVP K90等が挙げられる。本発明に用いられるPVPの重量平均分子量は2500~250000が好ましく、20000~150000が特に好ましい。PVP K17、PVP K25、PVP K30、PVP K90からなる群から選ばれる少なくとも1種が好ましい。とりわけ、PVP K25が好ましい。PVPの概要、規格、用途、使用量及び商品名などについては医薬品添加物事典(日本医薬品添加物協会編集、薬事日報社発行)に詳細に記載されている。PVP K25、PVP K30及びPVP K90はポビドンとして第十七改正日本薬局方に収載されている。ポビドンは日本、米国、欧州の三薬局方での調査合意に基づき規定した医薬品各条として第十七改正日本薬局方に掲載されている。ポビドンの規格の大部分は三薬局方で共通するものである。なお、本書ではPVP K25をPVP k25のように表記することがある。
本発明に用いられるクエン酸としては、クエン酸水和物及びクエン酸の薬学的に許容しうる塩が挙げられる。
前記クエン酸の薬学的に許容し得る塩としては、ナトリウム塩、カリウム塩などを例示できる。ナトリウム塩としては、クエン酸ナトリウム水和物(別名:クエン酸ナトリウム(日局)、以下、クエン酸ナトリウムとする)、クエン酸二水素ナトリウム、クエン酸二ナトリウム等が挙げられる。カリウム塩としては、クエン酸カリウム(クエン酸カリウム一水和物)があげられる。
本書でいうクエン酸ナトリウムは、第十七改正日本薬局方にクエン酸ナトリウム水和物としての概要、規格及び用途が詳細に記載されている。また、クエン酸ナトリウムとして、クエン酸ナトリウム2水和物の概要及び規格がUSP41(NF36)に詳細に記載されている。
MCの組み合わせが2種類である場合、前記組成物において、1種類のMCの濃度は0.1~3w/v%であるのが好ましい。MCの濃度が0.1w/v%以上の場合、ヒトの体温付近(34~38℃)での増粘がヒトの体温付近(34~38℃)で増粘するので好ましい。またMCの濃度が3w/v%以下の場合、室温(1~30℃)において粘性の低い水溶液を調製しやすいため好ましい。より好ましくは0.1w/v%以上、さらに好ましくは0.2w/v%以上である。より好ましくは2.5w/v%以上である。
MCの組み合わせが2種類である場合、異なる規格(好ましくは粘度)のMCを、質量比にして、1:30~30:1の割合で混合することが好ましく、より好ましくは1:24~24:1の割合、さらに好ましくは1:4~4:1である。特に、表示粘度が4のMCと表示粘度が15のMCとの組み合わせ、表示粘度が4のMCと表示粘度が400のMCとの組み合わせ、又は表示粘度が15のMCと表示粘度が400のMCとの組み合わせを、上記の質量比で併用するのが好ましい。とりわけ、表示粘度が4のMCと表示粘度が15のMCとの組み合わせを、上記の質量比で併用するのが好ましく、なかでも、1:4~4:1の質量比で併用するのが好ましい。
PEGの濃度は0.5~4.0w/v%の範囲で、濃度が0.5w/v%より低い場合は局所でのゲルが生成しにくいことから実用性に乏しく、また、4w/v%より高いとゲル化温度が低くなり好ましくない。
ポリプロピレングリコールの濃度は0.1~4w/v%の範囲で、濃度が4w/v%より高いと眼刺激の点で好ましくない。
PVPの濃度は0.5~4.0w/v%の範囲で、濃度が0.5w/v%より低い場合は局所でのゲルが生成しにくいことから実用性に乏しく、また、4w/v%より高いとゾルの粘度が高くなり好ましくない。
クエン酸の濃度は1.0~4.0w/v%の範囲で、濃度が1.0w/v%より低い場合は局所でのゲルが生成しにくいことから実用性に乏しく、また、4w/v%より高いと眼刺激の点で好ましくない。
(A)表示粘度が4のMCと、表示粘度が15のMCとを、質量比1:4~4:1で、合計濃度が0.2~5w/v%となる量と、
(B)0.5~4.0w/v%の、重量平均分子量が4000又は8000のPEGと、
(C)0.5~4.0w/v%のPVP K25と、
(D)1.0~4.0w/v%のクエン酸又はそのナトリウム塩を含むのが最も好ましい。この組成物のpH調整剤は、水酸化ナトリウム又は硫酸であるのがよい。
本発明の眼科用水性組成物はまた、乾性角結膜炎、涙液減少症における涙液補充のための人工涙液、涙液不足に伴う、コンタクトレンズ装用時の不快感、目のかわき、目の疲れ、目のかすみにおける涙液補充のための人工涙液、目の乾燥による、ヒリヒリする症状、刺激における涙液補充のための人工涙液として用いることができる。
本発明の眼科用水性組成物はまた、角膜の乾燥を抑制し、角膜を保護するための人工涙液又は涙液量を増大させる人工涙液として用いることができる。
本発明の眼科用水性組成物はまた、眼表面で滞留し、角膜上皮障害を軽減する人工涙液として用いることができる。
本発明の眼科用水性組成物はまた、角膜損傷を早期に回復させることで角膜障害を回復することができる人工涙液としての用いることができる。
点眼液で汎用性の高い防腐剤であるベンザルコニウム塩化物は、一般式:[C6H5CH2N(CH3)2R]Clで示される。なお、前記一般式において、Rはアルキル基を示す。本発明の組成物において、ベンザルコニウム塩化物は岡見化学工業株式会社から販売されているものを使用した。
メチルセルロース(信越化学工業(株)製、メトローズ(登録商標)SM-4及びSM-15)、ポリエチレングリコール(CARBOWAX(登録商標) PEG8000、ダウ・ケミカル社製)、ポリビニルピロリドン(PVP k25)及びクエン酸ナトリウムを所定量混合し、60~70℃に加熱した滅菌精製水へ添加し、攪拌することで分散させた。均一に分散したことを確認後、攪拌しながら10℃以下に冷却した。全体が澄明になったことを確認し、ベンザルコニウム塩化物を所定量添加し、溶解した。さらに、1Mの水酸化ナトリウム水溶液又は1Mの硫酸水溶液でpHを7.0に調整後、滅菌精製水で所定の容量にメスアップし、本発明の眼科用水性組成物を調製した(実施例1、2)。処方内容を表1に示した。
なお、表中、各成分の配合量はすべてw/v%で表した。
実施例1、2及び比較例1~4の組成物の20℃~40℃における粘度を測定して、温度と粘度の関係を評価した。アントンパール社製レオメーター (Modular Compact Rheometer 102) で粘度を測定した。調製した本発明の組成物、約1mLを直径約50 mmのパラレルプレートと温度コントロール用ペルチェの間にセットした。ペルチェとパラレルプレートのギャップは0.5 mmに設定した。測定開始前に試料を5分間5℃に保冷した。測定開始から、各測定温度まで徐々に昇温した。各測定温度で240秒間保持して粘度を測定した。結果を図1に示した。
比較例2、3はゲル化温度が本発明の組成物よりも室温に近く、比較例4では室温で大きく増粘するため、室温での貯法は難しいことが示された。一方で、本発明の眼科用水性組成物は、室温(1~30℃)で即時投与可能な液剤として保存できることが示された。
前述の実施例1,2と同様に次の通り実施例4~39を調製した。メチルセルロース(SM-4、SM-15及びSM-400)、ポリエチレングリコール(PEG8000、PEG4000、PEG400及びPEG300)、ポリビニルピロリドン(PVP k25、PVP k30及びPVP k90)及びクエン酸ナトリウムを所定量混合し、表6の実施例を調製する場合はホウ酸あるいはヒアルロン酸を所定量さらに混合し、60~70℃に加熱した滅菌精製水へ添加し、攪拌することで分散させた。均一に分散したことを確認後、攪拌しながら10℃以下に冷却した。全体が澄明になったことを確認し、表3~7に示す他の成分を所定量添加し、溶解した。さらに、1Mの水酸化ナトリウム水溶液又は1Mの硫酸水溶液でpHを調整後、滅菌精製水で所定の容量にメスアップし、本発明の眼科用水性組成物を調製した。
実施例と同様の手順で比較例Aを調製した。
処方内容を表3~7に示した。
表3から、本発明の組成物はMCの種類に関わらず調製可能であり、熱ゲルの特性を有することが示された。
表3の実施例8と実施例9を比較すると、表示粘度の大きいMCの割合が増えるほど低温での粘度が大きくなることが明らかであった。
表4から、PVPの種類ないし重量平均分子量に関わらず、本発明の組成物が調製可能であり、熱ゲルの特性を有することが示された。
表4の実施例13と実施例14を比較すると、クエン酸ナトリウムの濃度が低い組成物のほうが34~40℃において粘度の上昇が緩やかであった。
表4の実施例12と実施例13を比較すると、ポリエチレングリコールの濃度が低い組成物のほうが34~40℃において粘度の上昇が緩やかであった。
表4の実施例13と実施例15を比較すると、PVPの濃度が低い組成物のほうが34~40℃において粘度の上昇が緩やかであった。
表5から、本発明の組成物において各種防腐剤の添加が可能であり、熱ゲルの特性に影響を与えないことが明らかであった。
表6から、本発明の組成物においてホウ酸のような酸類、ヒアルロン酸のような増粘剤、トロメタモールのような塩基類,タウリン等のアミノ酸の添加が可能であり、熱ゲルの特性に影響を与えないことが明らかであった。
表6から、ポリエチレングリコールの種類ないし重量平均分子量に関わらず、本発明の組成物が調製可能であり、熱ゲルの特性を有することが示された。
表7から、本発明の組成物において各種無機塩類の添加が可能であり、熱ゲルの特性に影 響を与えないことが明らかであった。
表4から、前述のクエン酸ナトリウムまたはポリエチレングリコールほどではないものの、PVPが本発明の組成物での熱による粘度の上昇に影響を与えていることが明らかであった。
表5から、防腐剤の種類に限定せず添加可能であることが示されたので、本発明は水性医薬用組成物として適することが示された。
表6及び表7から、本発明の組成物が人工涙液用組成物として適することが示された。
表3~7から、本発明の組成物は、室温(1~30℃)で即時投与可能な液剤として保存できることが示された。
表3~7から、本発明の組成物は、眼科用水性組成物に適しており、投与後の眼表面の温度に反応し、即時に増粘して滞留性があることが示唆された。
前述の実施例1,2と同様の手順で実施例40~52を調製し、20℃~40℃における粘度を測定した。処方内容及び測定結果を表8に示した。
実施例49、51、52、53は30~34℃で粘度が低いものの、38、40℃で粘度が急激に上昇している。
本試験の粘度測定にて観察した粘度挙動をもって、本発明のほか実施例と同様の粘度推移を持つことから、実施例40~52でも本発明の特徴を備えていることが示された。
表1に示した実施例2及び表2に示した比較例2~4について、ゲル化時の揺変性を検討した。
実施例2及び比較例2~4の組成物について、アントンパール社製レオメーター (Modular Compact Rheometer 102) を用いてゲル化後の組成物に対してせん断応力とひずみの関係を求めた。せん断応力を増加させた時にひずみと比例の関係でなくなるポイントがあれば揺変性があると評価した。
調製した本発明及び比較例の組成物、約1mLを直径約50 mmのパラレルプレートと温度コントロール用ペルチェの間にセットした。ペルチェとパラレルプレートのギャップは0.5 mmに設定した。測定開始前に測定試料を5分間5℃で保冷した。測定開始から約10分間36℃に保持した。次に240秒間25℃に保持した後に、振動測定にて、周波数1 (Hz) 、せん断応力を0.01~10Paに変化させた時のひずみを求めた。結果を図2に示した。
表1に示した実施例1、2及び表2に示した比較例1~3について、一定期間で加熱と冷却を繰り返して粘度を測定し、再現性を評価した。
本発明の組成物、実施例2について、20℃と36℃へ温度調節を繰り返した際の粘度の変化を評価した。アントンパール社製レオメーター (Modular Compact Rheometer 102) で粘度を測定した。調製した本発明の組成物、約1mLを直径約50 mmのパラレルプレートと温度コントロール用ペルチェの間にセットした。ペルチェとパラレルプレートのギャップは0.5 mmに設定した。測定試料は測定開始前に10分間5℃で保冷した。測定開始から、36℃に昇温して240秒間保持した後に粘度を測定し、次に20℃に冷却して240秒間保持した後に粘度を測定した。この36℃と20℃の操作を4回半繰り返した。比較例4は36℃での粘度が非常に高く、ほかの試料と同条件で測定ができなかった。よって、実施例1、2および比較例1~3の結果を図3に示した。
表3に示した実施例9、10について、一定期間で加熱と冷却を繰り返して粘度を測定し、再現性を評価した。
試験操作は試験例3Aと同様である。結果を図4に示した。
表1に示した実施例2及び表2に示した比較例1~4の温度と溶液状態 (ゾル及びゲル) の関係を検討し、各水性組成物のゲル化再現性を求めた。
実施例2及び比較例1~4の組成物の30℃から36℃へ昇温した際の溶液状態 (ゾル及びゲル) の関係及び、30℃でゲル構造を崩した後のゲル化再現性を評価した。アントンパール社製レオメーター (Modular Compact Rheometer 102) を用いて振動測定によりゲル化の指標となる貯蔵弾性率 (G’) と損失弾性率 (G’’) を測定し、G’’/ G’の式で求められる損失正接(tan (δ) )の値を求めた。一般的に、tan (δ)>1を示すものをゾル、tan (δ)<1を示すものがゲルであるとされている(ゲル コントロール-ゲルの上手な作り方のゲル化の抑制-、2009年6月、発行:株式会社情報機構)。
調製した本発明の実施例及び比較例、約1mLをそれぞれ直径約50 mmのパラレルプレートと温度コントロール用ペルチェの間にセットした。ペルチェとパラレルプレートのギャップは0.5 mmに設定した。測定試料は測定開始前に10分間5℃で保冷した。測定開始から約1分間で30℃に昇温して30秒程度保持した。周波数1 (Hz) 、ひずみ5%の試験条件で振動測定し、次に約30秒間で36℃に測定試料を昇温し,そのまま150秒程度温度を維持した。その後、30℃、せん断応力10 Paの条件でパラレルプレートを回転させることによりゲル構造を崩した。再度36℃へ昇温しながら測定し、これらの操作を10回繰り返した。1回目のゲル化挙動と、ゲル化再現性の評価において5回目及び10回目の結果を図5~7に示した。
ゲル化再現性の評価では、比較例1~4は操作回数を重ねるごとにゾルへの転移が悪くなっていた。実施例2は、ゾルへの転移が弱くはなっているものの、10回目の操作においても比較例よりも長い時間ゾル状態を維持していた。
本発明の実施例によって、2種類のメチルセルロースを適当な割合で混合することで、ゲル化再現性が良くなることがこの結果より示された。
(1)試料溶液
表9の実施例3、比較例5及び比較例6を試験例1と同様に調製した。対照として生理食塩液(光製薬)(以下、生食ともいう)を加えた。
有色家兎(系統;Kbt:Dutch,搬入時体重1.5~2.0 kg、バイオテック)を安楽死させて角膜を採取した(n=3)。その後、採取した角膜を35℃で40分間、インキュベーター内で乾燥させた。乾燥開始直後から、室温で保存した実施例3及び生食と5℃で保存した後使用直前に室温に戻した比較例5と比較例6を、採取した角膜上に1分間隔でそれぞれ10μL×6回ずつ(計60μL)滴下した。乾燥後、角膜を生食で洗浄し、1%メチレンブルー(ナカライテスク)を用いて染色し、400μLの抽出液(アセトン(和光純薬工業):飽和硫酸ナトリウム(和光純薬工業)水溶液=7:3、容量比)に一昼夜以上浸漬して角膜に残留したメチレンブルーを抽出し、各抽出液の660nmにおける吸光度を測定した。
結果を図8に示した。
よって、本発明の組成物は角膜保護作用を有することが示唆された。
(1)試料溶液
表9の比較例5、表4の実施例17及び表6の実施例32を試験例1と同様に調製した。対照として生理食塩液(光製薬)を加えた。
有色家兎(系統;Kbt:Dutch,搬入時体重1.5~2.0 kg、バイオテック)を安楽死させて角膜を採取した。その後、採取した角膜に実施例17、実施例32及び比較例5を150μLずつ滴下し、35℃で40分間、インキュベーター内で乾燥させた。乾燥後、角膜を生食で洗浄し、1%メチレンブルー(ナカライテスク)を用いて染色し、400μLの抽出液(アセトン(和光純薬工業):飽和硫酸ナトリウム(和光純薬工業)水溶液=7:3、容量比)に一昼夜以上浸漬して角膜に残留したメチレンブルーを抽出し、各抽出液の660nmにおける吸光度を測定した。結果を図9に示した。
よって、本発明の組成物は角膜保護作用を有することが示唆された。
(1)試料溶液
実施例として、表9の実施例3を試験例1と同様に調製した。
比較例として、ノバルティスファーマ株式会社製のSYSTANE(登録商標) ULTRAを比較例アとした。または同社製のSYSTANE(登録商標) GEL DROPSを比較例イとして用いた。対照として生理食塩液(光製薬)を加えた。
白色家兎(系統;Kbs:JW,体重3.5kg以上、北山ラべス)を安楽死させ角膜(n=3~7)を採取した後、角膜上に生理食塩液、実施例3、比較例ア及び比較例イをそれぞれ150μLずつ滴下した。滴下後、角膜を35℃で40~50分間、インキュベーター内で乾燥させた。乾燥後、角膜を生理食塩液で洗浄し、1%メチレンブルー(ナカライテスク)を用いて染色し、400μLの抽出液(アセトン(和光純薬工業):飽和硫酸ナトリウム(和光純薬工業)水溶液=7:3、容量比)に一昼夜以上浸漬して角膜に残留したメチレンブルーを抽出し、各抽出液の660nmにおける吸光度を測定した。
結果を図10に示した。
(1)試料溶液
実施例として、表1の実施例2を試験例1と同様に調製し、点眼ボトルに充填した。
比較例として、試験例6と同様に比較例ア及び比較例イを用いた。
これらの試料溶液に1mg/mLとなるようにフルオレサイト(登録商標)静注(500mg)を加えた。
白色家兎(系統;Kbs:JW、北山ラべス)の眼瞼を緩やかに眼球から引き離し、角膜上に実施例2、比較例ア及び比較例イをそれぞれ30μL滴下後、上下両眼瞼を軽くつまんで強制的に30秒閉眼させた。点眼30分後に眼表面を生理食塩液100μLでよくすすいだ後、洗浄液を回収し検体とした。同時に、点眼後強制瞬き下における眼表面の洗浄液の回収についても以下の手順で行った。眼瞼を緩やかに眼球から引き離し、角膜上に30μLを滴下後、上下両眼瞼を軽くつまんで強制的に30秒閉眼させ、10秒に1回の頻度で、強制瞬きさせた。点眼30分後に眼表面を生理食塩液100μLでよくすすいだ後、洗浄液を回収し検体とした。
各検体の480nm(励起波長)及び520nm(吸収波長)を測定し、蛍光色素濃度を算出した。
結果を図11に示した。
実施例として、表1の実施例2を試験例1と同様に調製し、点眼ボトルに充填した。
対照として、リン酸緩衝生理食塩水(GIBCO(登録商標)PBS buffers、ライフ テクノロジーズ コーポレーション)を用いた。
所定量のベンザルコニウム塩化物[アルキル基の炭素数:14]( ベンジルジメチルテトラデシルアンモニウムクロリド水和物、東京化成工業株式会社)を微量のエタノールに溶解した後、生理食塩水でメスアップして0.1(w/v)%BAC溶液を調製した。
0.1(w/v)%BAC溶液を白色家兎(系統;Kbs:JW、北山ラべス)の両眼にマイクロチューブで20μLずつ、1日2回(朝、夕)、2週間連日投与した。
前記と投与時間をずらし、朝、夕の間、3時間ごとになるよう、実施例2またはリン酸緩衝生理食塩水を30μLずつ両眼に、1日3回、2週間連日投与した。実施例2は点眼ボトルで投与した。リン酸緩衝生理食塩水はマイクロピペットで投与した。試験開始前と1週間後及び2週間後にシルメル試験紙(あゆみ製薬株式会社)を用いて涙液を採取した。結果を図12及び表10に示した。
リン酸緩衝生理食塩水投与群では、2週間後に涙液量のわずかな増加が認められた。
一方、本発明の実施例2投与群では、経時的な涙液量の増加が認められた。
実施例として、表9の実施例3を試験例1と同様に調製した。
対照として、生理食塩水(大塚生食注(500mL)(登録商標)、大塚製薬株式会社)を用いた。
比較例として、ノバルティスファーマ社製のSYSTANE(登録商標) ULTRAを比較例アとした。
所定量のベンザルコニウム塩化物(東京化成株式会社)を微量のエタノールに溶解した後、生理食塩水でメスアップして0.1(w/v)%BAC溶液を調製した。
本試験方法はYuqiu Zhangらの方法(Drug and chemical toxicology 2016.39(4) 455-460)を参考とした。
1日に3回(朝から夕方までの4時間毎)、0.1(w/v)%BAC溶液を白色家兎(系統;Kbs:JW、北山ラべス)の両眼にマイクロピペットで20μLずつ、連日投与した。投与期間は投与日を含めて42日間とした。投与前及び42日間投与後にポータブルスリットランプ コーワ SL-17(興和株式会社製、以下SL-17という)を用いて角膜表面の障害の程度を観察した。
0.1(w/v)%BAC溶液を42日間投与後に、1日に4回(朝から夕方までの2.5~3時間毎)、実施例3、比較例ア及び生理食塩水のそれぞれをマイクロピペットで30μLずつ白色家兎の両眼(1群各5羽、n=10眼)に連日投与した。投与期間は投与日を含めて12日間とした。角膜をフルオレセイン染色してから3分経過後に生理食塩水で洗浄した。SL-17を用いて角膜表面の障害の程度を観察した。各群について、角膜損傷スコアを算出した。結果を図13,14に示した。図14には、無処置(Control)、生理食塩水、実施例3あるいは比較例アの投与開始から12日目に角膜障害を観察した写真を示した。
Claims (10)
- (A) 少なくとも2種類のメチルセルロース、(B) ポリエチレングリコール、(C) ポリビニルピロリドン、及び(D) クエン酸又はその薬学的に許容される塩を含有し、ゲル化することを特徴とする眼科用水性組成物。
- (A)成分が2種類のメチルセルロースである、請求項1記載の眼科用水性組成物。
- 2種類のメチルセルロースが1:30~30:1の質量比で含まれる、請求項2記載の眼科用水性組成物。
- (A)成分が20℃における2w/v%水溶液の粘度が4~400mPa・sのメチルセルロースである、請求項1~3のいずれか1項記載の眼科用水性組成物。
- (A)成分が、20℃における2w/v%水溶液の粘度が4mPa・s、15mPa・s、25mPa・s、100mPa・s、400mPa・s、1500mPa・s又は4000mPa・sであるメチルセルロースからなる群から選ばれる2種である、請求項1~3のいずれか1項記載の眼科用水性組成物。
- (B)成分が、PEG8000、PEG4000、PEG800、PEG400及びPEG300からなる群から選ばれる少なくとも1種類である、請求項1~5のいずれか1項に記載の眼科用水性組成物。
- (C)成分が、PVP K17、PVP K25、PVP K30及びPVP K90からなる群から選ばれる少なくとも1種である、請求項1~6のいずれか1項に記載の眼科用水性組成物。
- 熱ゲル化人工涙液である請求項1~7に記載の眼科用水性組成物。
- (A) 少なくとも2種類のメチルセルロース、(B)ポリエチレングリコール、(C) ポリビニルピロリドン、及び(D) クエン酸又はその薬学的に許容される塩を含有する、角膜保護用、角膜上皮障害軽減用又は角膜障害回復用眼科用水性組成物。
- ポリエチレングリコール、ポリビニルピロリドン、及びクエン酸又はその薬学的に許容される塩と組み合わせて、角膜保護用、角膜上皮障害軽減用又は角膜障害回復用眼科用水性組成物を調製するための、少なくとも2種類のメチルセルロースの使用。
