(3)多孔質材料を含む経口投与製剤であって、被吸着物質としてメチレンブルーを用いた吸着試験において、メチレンブルー濃度40mg/Lの試験液500mLを用いて製剤(製剤が、顆粒剤の場合、多孔質材料として50mg相当量、錠剤の場合1錠、カプセル剤の場合1カプセル分)を溶出試験器に入れて試験した場合、pH1.2の試験液で2時間後の吸着量が、多孔質材料1g当たり50mg未満であり、かつpH7.5の試験液で6時間後の吸着量が多孔質材料1g当たり80mg以上である製剤。(好ましくは(1)または(2)の製剤。)

(吸着試験法)
サンプル(腸溶性ポリマーによりコーティングされた多孔質材料を含む顆粒剤であれば、多孔質材料の重量換算で50mg相当の量、錠剤であれば1錠、カプセル自体が腸溶性のものであれば1カプセル、カプセルは腸溶性ではない通常のもので内容物に腸溶性ポリマーでコーティングされているようであれば内容物を取り出し1カプセル分)を用いて評価する。
a)腸溶性ポリマーによりコーティングされた多孔質材料を含む顆粒剤、カプセル剤または錠剤;
b)多孔質材料が腸溶性カプセルに封入されたカプセル剤;および
c)多孔質材料の圧縮成形物が腸溶性ポリマーでコーティングされた錠剤
が挙げられる。
本製剤の第1の態様は、腸溶性ポリマーによりコーティングされた多孔質材料を含む顆粒剤である。
第2の態様は、通常のカプセルに第1態様の顆粒剤を封入したカプセル剤である。
第3の態様は、腸溶性カプセルに被覆されていない多孔質材料を封入したカプセル剤である。
第4の態様は、腸溶性カプセルに、第1態様の顆粒剤を封入したカプセル剤である。
第5の態様は、第1態様の顆粒剤を圧縮成形した錠剤である。
第6の態様は、多孔質材料の圧縮成形物が腸溶性ポリマーでコーティングされた錠剤である。
第7の態様は、上記第2~4の態様に併用薬を配合したカプセル剤である。
(製法A)
本発明の第1の態様である「腸溶性ポリマーによりコーティングされた多孔質材料を含む顆粒剤」は、例えば以下の方法で製することができる。
(製法B)
第2の態様であるカプセル剤は、製法Aを用いて製造される顆粒剤を、定法にしたがって、ゼラチンカプセル、セルロース誘導体やでんぷんなどを使用した通常のカプセルに、必要に応じて添加剤などと共に封入することで製することができる。カプセルには第2の薬剤を封入して配合剤とすることもできる。
(製法C)
第3の態様である「腸溶性カプセルに被覆されていない多孔質材料を封入したカプセル剤」は、例えば以下の方法で製することができる。
(製法D)
第4の態様である「腸溶性カプセルに、第1態様の顆粒剤を封入したカプセル剤」を製造する場合は、製法Aと製法Cを組み合わせて製造すればよい。多孔質材料のコーティング量は、カプセルの性質に合せて調節する。
また、更に他の薬剤を封入して、配合カプセル剤とすることもできる。
(製法E)
第5の態様である、第1態様の顆粒剤を圧縮成形した錠剤は、製法Aにより得られた顆粒に適宜、常用の賦形剤、結合剤、崩壊剤、滑沢剤等を配合して圧縮成形することで得ることができる。
(製法F)
第6の態様である、多孔質材料の圧縮成形物が腸溶性ポリマーでコーティングされた錠剤は、多孔質材料を適宜、常用の賦形剤、結合剤、崩壊剤、滑沢剤等を配合して圧縮成形した後、汎用の錠剤のコーティング装置を用いて腸溶性ポリマーでコーティングすることによって得ることができる。腸溶性ポリマーによるコーティングの前にポリマーのコーティング適性の向上などの目的でサブコーティングを施すこともできる。
1)クレメジン細粒(以下、AST-120細粒、AST-120、KremezinあるいはKreと称する場合がある)
2)球形吸着炭細粒「マイラン」(以下、球形吸着炭「マイラン」、Merchmezinと称する場合がある)
3)球形吸着炭細粒「日医工」(以下、球形吸着炭「日医工」、Kyucalと称する場合がある)
9週齢の雄性Crlj:ZUC-Lepr[fa]ラットを二酸化炭素で安楽死させた後、胃の幽門下(十二指腸)から60cmの消化管(盲腸まで残り約30cm)を採取した。採取消化管に対して正中線に沿ってメスを入れて、氷上の50mL容ポリプロピレン製チューブに移した後、10mLの生理食塩水を加えた。容器を密封し、4℃、3000rpmで10分間の遠心分離を行い、上清をIntestinal Lavage Fluidとした。
AST-120(細粒、1g/5mLの経口投与用懸濁液を1.25%トラガカントで調製)を10週齢の雄性Crlj:ZUC-Lepr[fa]ラットに0.5mL経口投与し、50分後に二酸化炭素で安楽死させた。続いて消化管よりAST-120を回収し、After administrationサンプルとした。また経口投与前の投与用懸濁液をBefore administrationサンプルとした。
100mgのAST-120(細粒)を5号ゼラチンカプセル(カプスゲルジャパン)に手充填した。

AST-120に対し、下記の溶媒系処方に基づきコーティングを実施した。腸溶性ポリマーとしてはオイドラギットL30D55(コーティング液の組成を表13に示す)、オイドラギットS100(コーティング液の組成を表15に示す)、オイドラギットS100:L100=1:1(コーティング液の組成を表17に示す)およびHPMC-AS-HG(コーティング液の組成を表19に示す)を用いた。サンプルはポリマー固形分として対AST-120として10~100%(表4に示すとおり)までコーティングを実施した。オイドラギットL30D55、オイドラギットS100、オイドラギットS100:L100=1:1を30%までコーティングした製剤(製剤例16、製剤例19、製剤例20)の処方を表14、表16、表18に示し、HPMC-AS-HGを17%までコーティングした製剤(製剤例22)の処方を表20に示す。コーティング時の製造条件を表21に示す。

腸溶性ポリマーでコーティングした球形吸着炭(実施例3で製造された顆粒剤;製剤例4~11)とコーティングしていない球形吸着炭(AST-120 細粒)について、2種のpH条件つまり消化管上部を模倣した酸性~弱酸性領域(pH5.6)、消化管下部を模倣した中性領域(pH7.8)で吸着性能を評価した。Indole吸着量はIndoleを0.1g/Lの濃度で400mMのMES緩衝液(pH5.6)または50mMのリン酸緩衝液(pH7.8)に溶解し、9~15mgの範囲で予め秤量しておいた腸溶性ポリマーでコーティングした球形吸着炭とコーティングしていない球形吸着炭にそれぞれ40mL添加し、密封容器中で37℃、3時間、74rpmで往復攪拌した後、上清中のIndole濃度を測定することで行った。上清中のIndole濃度を測定後、沈殿物(腸溶性ポリマーでコーティングした球形吸着炭とコーティングしていない球形吸着炭)の重量を測定し、吸着性能を吸着Indole量を沈殿物重量で除算することによって算出した。結果を図1(pH5.6)、図2(pH7.8)に示す。
また、腸溶性ポリマーでコーティングした球形吸着炭(実施例3で製造された顆粒剤;製剤例12~14)を用い、緩衝液として400mMのMES緩衝液(pH5.8)および50mMのリン酸緩衝液(pH7.7)を用いた以外は上記と同様にして、吸着性能を算出した。結果を、図3(pH5.8)および図4(pH7.7)に示す。
実施例2で製造したエナラプリル配合の球形吸着炭カプセル剤(製剤例2)を、Cynomolgus monkey1頭につき1個(計5頭)を経口投与した。経口投与前と投与後0.5、1、2、4、8、24時間後に採血を行い、血漿中エナラプリラト(エナラプリルの活性代謝物)濃度を測定、5頭の平均値をグラフにまとめた。比較対照として球形吸着炭を腸溶性ポリマーでコーティングしていない5号カプセルに充填したものを用いるほかは実施例3と同様に製造した配合カプセル剤を用いた。またエナラプリル単独投与時の血漿中エナラプリラト濃度を測定するためにはエナラプリルのみを0号カプセル1個に充填したものを投与した。結果を図5に示す。
実施例2で製造したロサルタン配合の球形吸着炭カプセル剤(製剤例2)を用いて、実施例5と同様におこなった。ただし、ロサルタンの用量は10mgで行い、血漿中濃度の測定はロサルタンを検出した。結果を図6に示す。
本試験では、実施例3で製造した顆粒剤のうち、腸溶性ポリマーとしてオイドラギット S100を用いて、対AST-120比としてポリマー固形分40%までコーティングした顆粒剤(製剤例7)を用いた。当該顆粒剤とコーティングされていない球形吸着炭(AST-120(細粒))をそれぞれ、球形吸着炭の重量換算で0.3gを10mLの1.25%トラガカント溶液に懸濁して投与液を調製した。11週齢の雄性Crlj:ZUC-Lepr[fa]ラットに 1kgにつき10mLをそれぞれ経口投与した(N=5)。投与後に絶食し、経口投与前と投与後1、2、4、6時間後に尾静脈から採血を行い、尿毒素の一つであるIndoxyl sulfate(IS) の血漿中濃度を測定し、経口投与前の時点からの血漿中IS低下度(Δ Plasma IS、mg/dL)を算出した。各群、各時点における平均値±標準誤差を図7に示した。本発明の顆粒剤(製剤例7)はコーティングされていない球形吸着炭よりも高度な血漿中Indoxyl sulfate濃度低下を示した。
本試験では、実施例3で製造した顆粒剤のうち、腸溶性ポリマーとしてオイドラギット S100およびL100(3:1)を用いて、対AST-120比としてポリマー固形分30%までコーティングした顆粒剤(製剤例12)を用いた。当該顆粒剤とコーティングされていない球形吸着炭(AST-120(細粒))をそれぞれ、球形吸着炭の重量換算で100 mgあるいは300 mgを10 mLの1.25%トラガカント溶液に懸濁して投与液を調製した。SD (Slc:SD)ラットに1 kgにつき10 mLをそれぞれ経口投与した(N=8~9)。投与後に絶食し、経口投与2時間後にProbenecid溶液(50 mg/mL)をラット1 kgにつき1 mLを静脈内投与した。経口投与前、静脈内投与前、静脈内投与後1、2、3、4時間後に静脈から採血を行い、尿毒素の一つであるIndoxyl sulfate(IS)の血漿中濃度を測定し、経口投与前の時点からの血漿中IS低下度(Δplasma IS、mg/dL)を算出した。各群、各時点における平均値±標準誤差を図8に示した。本発明の顆粒剤(製剤例12)はコーティングされていない球形吸着炭の1/3の投与用量で同等の血漿中Indoxyl sulfate濃度低下を示した。
本試験では、実施例3で製造した顆粒剤のうち、腸溶性ポリマーとしてオイドラギット S100およびL100(3:1)を用いて、対AST-120比としてポリマー固形分30%までコーティングした顆粒剤(製剤例12)を用いた。当該顆粒剤とコーティングされていない球形吸着炭(AST-120(細粒))をそれぞれ、球形吸着炭の重量換算でBeagle dogに1 kgにつき100 mgあるいは300 mgとなるようにゼラチンカプセル(No.12(1/4oz))に充填し、それぞれを経口投与した。経口投与1時間後にProbenecid溶液(20 mg/mL)をBeagle dog 1 kgにつき2.5 mLを経口投与した。球形吸着炭の経口投与から2時間後に再給餌(DS-A, 250 g/dog)を行った。給餌前、給餌後2、4、6、8時間後に静脈から採血を行い、尿毒素の一つであるIndoxyl sulfate(IS)の血漿中濃度を測定し、給餌前の時点からの血漿中IS低下度(Δplasma IS、mg/dLおよびΔplasma IS AUC(0~8h)、mg・h/dL)を算出した。各群、各時点における平均値±標準誤差を図9、給餌前(0h)から8hのΔplasma IS AUCを図10に示した。本発明の顆粒剤(製剤例12)はコーティングされていない球形吸着炭の1/3の投与用量で同等の血漿中Indoxyl sulfate濃度低下を示した。
本試験では、実施例3で製造した顆粒剤のうち、表22に示した腸溶性ポリマーを用い、対AST-120比(または球形吸着炭細粒「マイラン」比)としてポリマー固形分10~30%までコーティングした顆粒剤(製剤例15、16、12、19、22および13)を用いた。当該顆粒剤とコーティングされていない球形吸着炭(AST-120(細粒))をそれぞれ、球形吸着炭の重量換算で100 mgを10 mLの1.25%トラガカント溶液に懸濁して投与液を調製した。SD (Slc:SD)ラットに1 kgにつき10 mLをそれぞれ経口投与した(N=6~19)。投与後に絶食し、経口投与2時間後にProbenecid溶液(50 mg/mL)をラット1 kgにつき1 mLを静脈内投与した。経口投与前、静脈内投与前、静脈内投与後1、2、3、4時間後に静脈から採血を行い、尿毒素の一つであるIndoxyl sulfate(IS)の血漿中濃度を測定し、経口投与前の時点からの血漿中IS低下度(Δplasma IS AUC(-2h~4h))を算出した。溶媒対照群(1.25%トラガカント溶液投与群)を1とした時のratioを算出し、各群の平均値を表22に示した。本発明の顆粒剤(製剤例15、16、12、19、22および13)はコーティングされていない球形吸着炭よりも高度な血漿中Indoxyl sulfate濃度低下を示した。

本試験では、実施例3で製造した顆粒剤のうち、腸溶性ポリマーとしてオイドラギット S100を用いて、対AST-120比としてポリマー固形分40%までコーティングした顆粒剤(製剤例7)を用いた。当該顆粒剤を重量換算で0.3g秤量し10mLの1.25%トラガカント溶液に懸濁して投与液を調製した。11週齢の雄性Crlj:ZUC-Lepr[fa]ラット 1kgにつき10mLを経口投与した後絶食し、1および4時間後に二酸化炭素で安楽死させて開腹し、消化管内の球形吸着炭の状態を観察した。投与1時間後では球形吸着炭は小腸に存在していた。これは図11に示すように灰白色であり、肉眼で観察することによってコーティングが維持されていることが確認できた。投与4時間後では球形吸着炭は大腸に存在し、コーティングが溶解して黒色に戻っており、中身であるAST-120が露出していることが確認できた(図12)。以上より腸溶性ポリマーが小腸では溶解せず大腸において溶解し、中身であるAST-120が大腸で露出することが示された。
実施例3の製剤例12と同じ方法で製造した顆粒剤(腸溶性ポリマーとしてオイドラギット S100およびL100(3:1)を用いて、対AST-120比として30%までコーティングした顆粒剤(表11の処方、表9に記載の製造条件で製造)171 ± 4 mgとワルファリン0.5 mg(ワルファリン5 mg錠をすりつぶし、ワルファリン量にして0.5mg分)を充填した0号カプセル1個を、Cynomolgus monkey1頭につき1個(計6頭)を経口投与した。経口投与前と投与後0.5、1、2、4、8、24時間後に採血を行い、血漿中ワルファリン濃度を測定、6頭の平均値をグラフにまとめた。前述投与から8日後、比較対照として腸溶性ポリマーでコーティングしていない顆粒AST-120(120 ± 2 mg)とワルファリン0.5 mg(ワルファリン5 mg錠をすりつぶし、ワルファリン量にして0.5mg分)を充填した0号カプセル1個を経口投与し、同様に採血し血漿中ワルファリン濃度を測定、6頭の平均値をグラフにまとめた。さらに前述投与から12日目にワルファリン単独投与時の血漿中ワルファリン濃度を測定するためにワルファリン0.5 mg(ワルファリン5 mg錠をすりつぶし、ワルファリン量にして0.5mg分)を0号カプセル1個に充填したものを経口投与し、同様に採血し血漿中ワルファリン濃度を測定、6頭の平均値をグラフにまとめた。計3回の結果を図13に示す。
製剤例12、製剤例13および15~22の顆粒剤について、球形吸着炭換算で50mg相当を秤量し、試験に使用した。

Claims (23)
- 多孔質材料を含む経口投与用製剤であって、多孔質材料に大腸または小腸下部ではじめて多孔質材料の表面を露出させる手段が施されている製剤。
- 当該製剤の吸着試験における被吸着物質の吸着量が、当該手段が施されていない多孔質材料の吸着量に対して、pH5以下の環境下で20%未満であり、pH7以上の環境下で60%以上である請求項1に記載の製剤。
- 被吸着物質としてメチレンブルーを用いた吸着試験において、メチレンブルー濃度40mg/Lの試験液500mLを用いて製剤(製剤が、顆粒剤の場合、多孔質材料として50mg相当量、錠剤の場合1錠、カプセル剤の場合1カプセル分)を溶出試験器に入れて試験した場合、pH1.2の試験液で2時間後の吸着量が、多孔質材料1g当たり50mg未満であり、かつpH7.5の試験液で6時間後の吸着量が多孔質材料1g当たり80mg以上である請求項1または2に記載の製剤。
- 大腸または小腸下部ではじめて多孔質材料の表面を露出させる手段が施されている製剤が、下記a)~c)から選ばれるものである請求項1~3のいずれか1項に記載の製剤。
a)腸溶性ポリマーによりコーティングされた多孔質材料を含む顆粒剤、カプセル剤または錠剤
b)多孔質材料が腸溶性カプセルに封入されたカプセル剤
c)多孔質材料の圧縮成形物が腸溶性ポリマーでコーティングされた錠剤 - 多孔質材料が有害物質の経口吸着薬である請求項1~4のいずれか1項に記載の製剤。
- 多孔質材料が球形吸着炭である請求項1~5のいずれか1項に記載の製剤。
- 多孔質材料が、当該手段を施す前において、その平均粒子径X50がレーザー回折法で測定するとき100~500μmであり、比表面積がBET多点法で測定するとき800m2/g以上であるものである請求項1~6のいずれか1項に記載の製剤。
- 多孔質材料が、当該手段を施す前において、その平均粒子径X50がレーザー回折法で測定するとき200~400μmであり、比表面積がBET多点法で測定するとき1000~1700m2/gであるものである請求項1~7のいずれか1項に記載の製剤。
- 腸溶性ポリマーまたは腸溶性カプセルが、pH5~8で溶解するものである請求項1~8のいずれか1項に記載の製剤。
- 腸溶性ポリマーがアクリル系ポリマーまたはセルロース系ポリマーである請求項1~9のいずれか1項に記載の製剤。
- 腸溶性ポリマーが、メタクリル酸およびエチルアクリレートのコポリマー、メタクリル酸およびメチルメタクリレートのコポリマー、メタクリル酸とメチルアクリレートおよびメチルメタクリレートのコポリマー、セルロースアセテートフタレート、セルロースアセテートトリメリテート、ヒドロキシプロピルメチルセルロースフタレート、ヒドロキシプロピルメチルセルロースアセテートサクシネート、カルボキシメチルエチルセルロース、セルロースアセテートサクシネート、メチルセルロースフタレート、ヒドロキシメチルセルロースフタレート、ヒドロキシプロピルメチルアセテートマレエート、ヒドロキシプロピルメチルトリメリテート、ポリビニルアセテートフタレート、及びポリビニルブチレートフタレートからなる群から選ばれる1種以上である請求項1~10のいずれか1項に記載の製剤。

- 腸溶性ポリマーが多孔質材料に対して10~100重量%コーティングされている請求項13に記載の製剤。

- 他の薬剤と同時に服用するための請求項1~18のいずれか1項に記載の製剤。
- 更に他の薬剤が配合されている請求項1~19のいずれか1項に記載の製剤。
- 他の薬剤が、アンジオテンシンII受容体拮抗薬、アンジオテンシン変換酵素阻害薬、カルシウム拮抗薬、利尿薬、高尿酸血症治療薬、高脂血症治療薬、糖尿病治療薬、ステロイド・免疫抑制薬、抗血小板薬・抗凝固薬、高リン血症治療薬、赤血球造血刺激因子製剤、鎮痛剤、抗不整脈薬、抗うつ薬、アルツハイマー型認知症治療薬、パーキンソン病薬、プロトンポンプ阻害薬(PPI)、抗アレルギー薬および抗菌薬から選ばれる1種以上である請求項19または20に記載の製剤。
- 他の薬剤が、アンジオテンシンII受容体拮抗薬、アンジオテンシン変換酵素阻害薬、カルシウム拮抗薬、利尿薬および抗血小板薬・抗凝固薬から選ばれる1種以上である請求項19または20に記載の製剤。
