Abstract
Description
本発明は、硫酸化グリコサミノグリカンが有する出血活性を抑制する技術、硫酸化グリコサミノグリカンを利用しつつこれに起因する出血の問題がなく、かつ優れた生体組織治療効果を発揮しうる素材、医療材料、医薬を提供することを課題とする。
[1]カチオン化キトサンを有効成分とする、硫酸化グリコサミノグリカンに由来する出血活性の抑制剤(以下、「本発明抑制剤」という。)。このカチオン化キトサンは、4級アンモニウム基を有することが好ましい。この硫酸化グリコサミノグリカンは、ヘパリン、ヘパラン硫酸、ケラタン硫酸、コンドロイチン硫酸又は硫酸化ヒアルロン酸であることが好ましい。
[2]カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体(以下「本発明複合体」という。)。この両者はイオン結合していることが好ましい。
[3]本発明複合体を含有する創傷被覆材(以下「本発明被覆材」という。)。
[4]本発明複合体を有効成分とする、生体組織治療用の医薬(以下「本発明医薬」という。)。本発明医薬は、創傷治療剤であることが好ましい。
本発明被覆材は、優れた創傷被覆特性を有し、生体適合性も高く、しかも簡単かつ衛生的に除去することができ、極めて有用である。また本発明被覆材及び本発明医薬ともに、硫酸化グリコサミノグリカンに由来する出血を顕著に抑制しつつ、優れた生体組織治療効果を発揮することから極めて有用である。
本発明複合体は、このような本発明被覆材や本発明医薬の素材とすることができることから、極めて有用である。
なお、本明細書において使用する略語は以下の通りである。

本発明抑制剤は、カチオン化キトサンを有効成分とする、硫酸化グリコサミノグリカンに由来する出血活性の抑制剤である。
本発明抑制剤の有効成分であるカチオン化キトサンは、キトサン分子中に正の電荷を有しているものであれば特に限定されない。例えば、キトサンに正の電荷を与えるよう、正電荷を有した官能基を導入したものでもよい。正電荷を有した官能基の導入は、例えばキトサン分子中のアミノ基や水酸基に1級、2級もしくは3級のアミノ基、または、4級アンモニウム基を導入すること、又はキトサン分子中のアミノ基における水素原子を炭化水素基で置換して2級もしくは3級のアミノ基、または、4級アンモニウム基とすることによって行うことができる。例えば、エチレンジアミンやスペルミジン等を導入することによって正電荷を有したキトサンが得られる。共有結合によって導入する場合、その方法は、化学的な反応によって共有結合が形成されるものであれば特に限定されない。
なお、2級もしくは3級のアミノ基、または、4級アンモニウム基は、特に限定されない。すなわち、硫酸化グリコサミノグリカンに由来する出血活性の抑制効果を発揮し得るように、硫酸化グリコサミノグリカンとの複合体が維持されるものであればよい。硫酸化グリコサミノグリカンに由来する出血活性の抑制効果については、本明細書の実施例に記載の方法により、確認することができる。例えば、2級もしくは3級のアミノ基、または、4級アンモニウム基における炭化水素基としては、炭素数1~4の直鎖又は分岐状のアルキル基、炭素数2~4の直鎖又は分岐状のアルケニル基、炭素数2~4の直鎖又は分岐状のアルキニル基、炭素数6~10のアリール基等の炭化水素基を挙げることができる。
本発明には、これらの1種類又は2種以上のカチオン化キトサンを組み合わせて、用いることができる。
このようにして製造された本発明抑制剤は、硫酸化グリコサミノグリカンに由来する出血活性を抑制するために用いられる。
これらのなかでも、ヘパリン、ヘパラン硫酸、ケラタン硫酸又はコンドロイチン硫酸が好ましい。ケラタン硫酸としては、ケラタンポリ硫酸が好ましい。本発明には、これらの1種類又は2種以上を組み合わせて、用いることができる。
またコンドロイチン硫酸としては、コンドロイチン硫酸A、コンドロイチン硫酸B(デルマタン硫酸ともいう。)、コンドロイチン硫酸C、コンドロイチン硫酸D、コンドロイチン硫酸E、過硫酸化コンドロイチン硫酸を例示することができる。
これらの硫酸化グリコサミノグリカンのなかでも、ヘパリン又はコンドロイチン硫酸Eが好ましい。
硫酸化グリコサミノグリカンの重量平均分子量も特に限定されないが、通常500~10,000,000程度のもの、好ましくは1,000~8,000,000程度のものを例示することができる。
この本発明抑制剤によって、硫酸化グリコサミノグリカンが有する創傷治癒活性を維持したまま、硫酸化グリコサミノグリカンが有する出血活性を抑制することができる。
本発明複合体は、カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体である。この複合体を構成するカチオン化キトサン、硫酸化グリコサミノグリカンは、前記の「本発明抑制剤」において説明されたものと同じである。
この複合体は、カチオン化キトサンを、硫酸化グリコサミノグリカンと接触させることにより製造することができる。この接触は、カチオン化キトサン分子が、硫酸化グリコサミノグリカン分子に接触し、複合体を形成することとなる態様である限りにおいて特に限定されない。例えば各々の溶液、好ましくは水溶液を製造し、この両者を混合して直接複合体を形成させてもよく、またこの両者を塩の存在下で接触させた後、この塩を透析その他の方法で除去することによって両者の複合体を形成する態様が例示できる。なお、混合手法、条件等は、カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体が形成し得る範囲で、適宜調整され得る。具体的には、本明細書の実施例に記載の方法を参照することができる。このような方法により、両分子間にイオン結合が形成され、ポリイオンコンプレックスを形成させることができる。本発明複合体は、上記のように製造された後、単離、乾燥、精製等を行ってもよい。
この複合体におけるカチオン化キトサンと硫酸化グリコサミノグリカンとの構成比も限定されず、例えば1:0.001~1:1000(モル比)程度を例示することができる。
このように製造された本発明複合体は、以下に説明する本発明被覆材や、本発明医薬の素材として利用することができる。
本発明被覆材は、本発明複合体を含有する創傷被覆材である。本発明被覆材に含有される本発明複合体を構成する分子(カチオン化キトサン、硫酸化グリコサミノグリカン)や、複合体におけるカチオン化キトサン分子と硫酸化グリコサミノグリカン分子間の好ましい結合態様などは、前記の「本発明複合体」における説明のとおりである。この本発明複合体をそのまま、又は必要に応じこれに本発明の効果を損なわない他の成分、例えば医薬上許容可能な担体等を適宜配合することによって、本発明被覆材とすることができる。
本発明被覆材の形状としては、その他、粉末状、顆粒状、シート状、スポンジ状、メッシュ状などを挙げることができる。また軟膏、ペースト、ゲルなどの形状を例示することもできる。
このようにして製造された本発明被覆材で創傷部位を被覆すると、硫酸化グリコサミノグリカンの創傷治癒活性が維持されたまま、硫酸化グリコサミノグリカンの出血活性が抑制されることから、創傷部位からの出血を抑制しつつ創傷治癒を促進することができる。
本発明医薬は、本発明複合体を有効成分とする生体組織治療用医薬である。本発明医薬の有効成分である本発明複合体を構成する分子(カチオン化キトサン、硫酸化グリコサミノグリカン)や、複合体におけるカチオン化キトサン分子と硫酸化グリコサミノグリカン分子間の好ましい結合態様などは、前記の「本発明複合体」における説明のとおりである。この本発明複合体をそのまま、又は必要に応じこれに本発明の効果を損なわない他の成分を適宜配合することによって、本発明医薬とすることができる。
本発明医薬が適用される対象は、生体組織の治療が必要な動物である限りにおいて特に限定されないが、哺乳類が好ましく、なかでもヒトが好ましい。
本発明医薬が適用される生体組織は、治療、組織修復などが必要な生体組織である限りにおいて特に限定されず、例えば皮膚、臓器、骨などを例示することができる。なかでも、生体組織中に血管が多く存在し出血リスクの高い、皮膚や臓器に適用されることが好ましく、皮膚に適用されることが好ましい。また、生体組織において治療、組織修復などが必要な状態も特に限定されないが、創傷、潰瘍などを挙げることができる。なかでも創傷に適用されることが好ましい。すなわち、本発明医薬は創傷治療剤であることが好ましく、皮膚の創傷治療剤であることがより好ましい。
本発明医薬の剤形も、本発明医薬の使用方法に応じて、公知の種々の剤形のなかから当業者が適宜選択することができる。例えば、前記の本発明被覆材と同様に、本発明医薬の有効成分である本発明複合体の溶液や懸濁液の形態での本発明医薬としてもよく、これを凍結乾燥等の方法で乾燥させることによって得られる乾燥物の形態で適宜成形して、本発明医薬としてもよい。前記の本発明被覆材を、そのまま本発明医薬として転用することもできる。
本発明医薬は、生体組織に1回適用するのみでもよく、適用後必要に応じて追加、交換、除去などをしてもよい。また継続的に適用してもよい。
(1)キトサン
キトサンは、市販のもの(和光純薬工業株式会社製、CT300、フレーク状、0.5%酢酸水溶液を溶媒として濃度0.5w/v%としたときの見かけ粘度257mPa・s)を使用した。
カチオン化キトサンを製造する原料として、特開平9-143203号公報に記載の方法にしたがって微粒子化キトサンを製造した。具体的には、上記キトサン10.0gにイオン交換水600mLを添加し、次いで酢酸10mLを添加した。室温で撹拌して溶解させた後、この溶液をグラスフィルターでろ過した。ろ液に対して硫酸アンモニウム20.0 gを添加し、撹拌して混合することによってキトサンの微粒子を析出させた。
析出が完了した時点で、酢酸を中和するため、5M NaOHをpHが8~9程度となるまで添加した。その後、溶媒から、析出した微粒子化キトサンをグラスフィルターで分離した。分離した微粒子化キトサンを含水エタノールで洗浄することによって、これに含まれる塩類を除去した。この洗浄を数回繰り返した後、微粒子化キトサンに付着した水分をアセトンで洗浄することによって除去した。その後、40℃、減圧下で乾燥させることによって、微粒子化キトサン9.8g(粉末状)を得た。
前記の特許文献3に記載の方法にしたがって、カチオン化キトサンを製造した。具体的には、上記(2)で得た微粒子化キトサン9.0gに、80v/v%イソプロピルアルコール150mLを添加し、60℃で撹拌した。その後、グリシジルトリメチルアンモニウムクロリド(阪本薬品工業株式会社製:SY-GTA80)を9.0mL添加し、60℃で7時間撹拌して反応させた。反応終了後、反応溶媒から、反応物をグラスフィルターで分離した。分離した反応物を、20倍量のエタノール、アセトンで洗浄した。その後、反応物を40℃、減圧下で乾燥させることによって、カチオン化キトサン(第4級キトサンである、N-(2-ヒドロキシプロピル)-3-トリメチルアンモニウムキトサンクロリド)14.2 gを得た。
(1)カチオン化キトサンとヘパリンとからなる複合体の製造
カチオン化キトサンと、硫酸化グリコサミノグリカンの一種であるヘパリン(和光純薬工業株式会社製、重量平均分子量約10,000)とからなる複合体(Hep/CT+)を製造した。具体的には、実施例1(3)で製造したカチオン化キトサン220mgと、ヘパリン150mgとを、5.0 w/v%塩化ナトリウム溶液8mLに同時に溶解し、両成分が均一化するまで十分混合した。この混合液をテフロン(登録商標)で表面加工された型に流し込み、混合液が十分に平滑化するまで静置した。次いで、この混合液を型ごと多量のイオン交換水中に投入し、混合液とイオン交換水の接触面から脱塩透析(20時間程度)することで、カチオン化キトサンとヘパリンとの複合体(カチオン化キトサン分子とヘパリン分子とがイオン結合したポリイオンコンプレックス)を形成させ、成形されたゲルを得た。このゲルはいずれも白色状であった。さらにこのゲルを凍結乾燥することで、スポンジ状の乾燥物を得た。この乾燥物はいずれも白色であった。得られた乾燥物の重量は、約270mgであった。
上記(1)におけるヘパリンに替え、他の硫酸化グリコサミノグリカン(コンドロイチン硫酸A、コンドロイチン硫酸B(デルマタン硫酸)、コンドロイチン硫酸C、コンドロイチン硫酸D、コンドロイチン硫酸E、ケラタンポリ硫酸。いずれも生化学工業株式会社製、重量平均分子量はそれぞれ約10,000;約30,000;約40,000;約30,000;約70,000;約10,000)を用いて、上記(1)と同様に、カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体(両分子がイオン結合したポリイオンコンプレックス)を製造し、スポンジ状の乾燥物を得た。得られた乾燥物の重量は、コンドロイチン硫酸Aとの複合体(CS-A/CT+)、コンドロイチン硫酸Bとの複合体(CS-B/CT+)、コンドロイチン硫酸Cとの複合体(CS-C/CT+)、コンドロイチン硫酸Eとの複合体(CS-E/CT+)およびケラタンポリ硫酸との複合体(KPS/CT+)がいずれも約310mg、コンドロイチン硫酸Dとの複合体(CS-D/CT+)が約300mgであった。
実施例2で製造した、カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体の乾燥物を約1cm2に切断して、後述の実施例4に用いた。
(1)スポンジ状のキトサンの調製
前記特許文献1に記載の方法に準じ、以下のとおりスポンジ状のキトサンを得た。
キトサンを2.0v/v% 酢酸水溶液に添加し、十分に撹拌して、2w/v%のキトサン溶液を調製した。
上記キトサン溶液の10.00g(キトサンを200mg含有)を、ポリプロピレン製シャーレ(直径:85mm)に流延し、キトサン溶液が十分平滑化するまで静置した。次いで、キトサン溶液を凍結後、凍結状態のキトサン溶液に2M NaOH(10mL)を添加して、室温下で約3時間静置した。溶液が白色ゲル状となったことを確認後、このゲルをシャーレの底面から剥離して、ゲル全体を2M NaOH溶液に浸漬させ、室温下で4日間静置した。次いでこの白色のゲルの表面をイオン交換水で洗浄後、ゲルを多量のイオン交換水に浸漬し、マグネチックスターラー(600rpm)でイオン交換水を攪拌して、余剰のNaOHを除去した。この操作は、イオン交換水のpHが中性付近で安定化するまで数回繰り返した(最終pH 6.5)。その後、この白色のゲルを凍結乾燥して、スポンジ状のキトサンを得た。この重量は180mgであった。
ヘパリン1067mgを0.2M リン酸緩衝液220ml(pH 6.4)に溶解し、ヘパリン溶液(pH 6.6)を調製した。
このヘパリン溶液20ml(Hep 97mgを含有)を前記のスポンジ状のキトサンと混合し、室温下で約24時間静置した。その後この混合物を取り出し、イオン交換水で洗浄後、ゲルを多量のイオン交換水に浸漬し、マグネチックスターラー(600rpm)でイオン交換水を攪拌して、余剰のヘパリンおよびリン酸無機塩類を除去した。この操作は、イオン交換水の導電率が十分に低下して安定するまで繰り返し行った。この最終的な導電率は100μS/cmであった。洗浄後の混合物を凍結乾燥し、キトサンとヘパリンとからなる複合体(Hep/CT)のスポンジ状の乾燥物を得た。この重量は約250mgであった。
またヘパリンに替えてコンドロイチン硫酸Eを用い、同様に、キトサンとコンドロイチン硫酸Eとからなる複合体(CS-E/CT)のスポンジ状の乾燥物を得た。この重量は約240mgであった。
これらそれぞれの乾燥物を約1cm2に切断して、後述の実施例4に用いた。
(1)全層皮膚欠損創モデルの作成
7~10週齢の雄性Wistar(Crlj:WI)ラット、またはSprague‐Dawley (SD)ラットの背部を除毛し、約1cm2の範囲をメスで切開することで、全層皮膚(表皮層、真皮層、皮下組織)を欠く、全層欠損創を作成した。この全層欠損創モデルは、組織修復を評価するモデルとして一般的であり、広く用いられている。この全層欠損創部分(以下「患部」という。)に被験物質を貼付し、フィルムドレッシング材(テガダーム)と伸縮性包帯で固定した。
また被験物質を貼付せず、フィルムドレッシング材と伸縮性包帯のみで処置したものを対照(Control)とした。
患部に被験物質を貼付した翌日に、患部を写真撮影するとともに、肉眼的に観察し、創部からあふれる程度の出血がみられたものを「++」、滲む程度の出血がみられたものを「+」、出血がみられなかったものを「-」として、各被験物質による出血性を評価した。
被験物質として、参考例1で製造したHep/CT、CS-E/CT、および実施例3で製造したHep/CT+、CS-E/CT+を用いた結果を図1(写真)及び図2に示す。
その結果、Hep/CT(前記特許文献1に記載のものに相当)を貼付した場合、12箇所中12箇所で出血が「++」であり、被験物質を貼付した翌日に全ての動物が死亡した。
これに対し、Hep/CT+を貼付した場合には、顕著な出血(++)はなく、「+」が3箇所、「-」が9箇所で、死亡例もみられなかった。
また、他の硫酸化グリコサミノグリカンであるコンドロイチン硫酸Eを用いた場合についても、CS-E/CTを貼付した患部では12箇所中「++」が0箇所、「+」が6箇所、「-」が6箇所というように、約半数の患部に出血が見られたものの、CS-E/CT+を貼付した患部では8箇所中8箇所が「-」となり、明らかに出血を抑制した。
以上の結果から、カチオン化キトサンを用いることにより、硫酸化グリコサミノグリカンに起因する出血活性を顕著に抑制できることが示された。また、カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体、これを含有する創傷被覆材及びこれを有効成分とする生体組織治療用医薬には、出血抑制効果があることも示された。またこれらカチオン化キトサン、複合体、創傷被覆材、生体治療用医薬を用いることで死亡例もみられなくなったことから、生体組織に対し安全に使用できることも確認された。
患部への被験物質の貼付後7日目に表皮伸展の程度(表皮面積)を計測した。表皮伸展は全層皮膚欠損創の評価において一般的に用いられるものであり、組織修復の程度を反映する評価指標である。表皮面積は、作成した全層欠損創の辺縁から中央部に向かって伸びた表皮の部分を透明シートにトレースし、画像解析ソフトで面積を数値化することにより計測した。
まず、試験例で製造したHep/CT、CS-E/CTと、実施例3で製造したHep/CT+、CS-E/CT+を用いて評価した。結果を図3に示す。
Hep/CT(前記特許文献1に記載のものに相当)を投与した動物については、前記(2)に記載のとおり被験物質投与の翌日に全て死亡したため、表皮面積を測定することができなかった。
Controlにおける表皮面積は20±9.4(SD)mm2であったのに対し、Hep/CT+では39.3±8.6(SD)mm2、CS-E/CT+では34.9±6.2(SD)mm2と、いずれも有意な表皮面積の増加が認められた。またCS-E/CTは前記のとおり出血が見られたが、組織修復の点では30.9±10.1(SD)mm2と有意な表皮面積の増加がみられた。
以上の結果から、カチオン化キトサンを用いても硫酸化グリコサミノグリカンが有する生体組織の修復(創傷治癒)促進効果は何ら損なわれないことが示された。このように、カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体、これを含有する創傷被覆材及びこれを有効成分とする生体組織治療用医薬は、出血を抑制しつつ、優れた生体組織修復(創傷治癒)促進効果を発揮することが示された。
表皮面積率は、Controlが20.0±5.3(SD)%であったのに対して、Hep/CT+は51.5±14.3(SD)%、CS-A/CT+は49.4±7.3(SD)%、CS-B/CT+は57.1±10.8(SD)%、CS-C/CT+は29.2±7.7(SD)%、CS-D/CT+は38.1±11.6(SD)%、CS-E/CT+は47.6±11.4(SD)%、KPS/CT+は48.8±7.0(SD)%であり、いずれも顕著な表皮面積率の増加が確認された。
これらの試験において、患部からの出血、死亡例は確認されなかった。
以上の結果からも、カチオン化キトサンを用いても硫酸化グリコサミノグリカンが有する生体組織の修復(創傷治癒)促進効果は何ら損なわれないことが示された。またカチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体、これを含有する創傷被覆材及びこれを有効成分とする生体組織治療用医薬は、出血を抑制しつつ、優れた生体組織修復(創傷治癒)促進効果を発揮することが示された。
(1)カチオン化キトサンの製造
前記の特許文献3に記載の方法にしたがって、カチオン化キトサンを製造した。具体的には、実施例1と同様の方法で製造した微粒子化キトサン7.5gに、80v/v%イソプロピルアルコール105mLを添加し、60℃で撹拌した。その後、グリシジルトリメチルアンモニウムクロリド(アルドリッチ社製)を10.5mL添加し、60℃で7時間撹拌して反応させた。反応終了後、反応溶媒から、反応物をグラスフィルターで分離した。分離した反応物を、20倍量のエタノール、アセトンで洗浄した。その後、反応物を40℃、減圧下で乾燥させることによって、カチオン化キトサン(第4級キトサンである、N-(2-ヒドロキシプロピル)-3-トリメチルアンモニウムキトサンクロリド)12.5gを得た。
上記(1)で製造した、カチオン化キトサン250mgを注射用水8mLに均一に溶解し、得られた溶液を凍結乾燥することにより、スポンジ状の乾燥物を得た。この乾燥物は白色であり重量は約240mgであった。この乾燥物を適宜切断して、後述の参考例3に用いた。
(1)熱傷創モデルの作成
10週齢の雄性Sprague‐Dawley (SD)ラットの腹部を除毛し、約7cm2の範囲に100℃の熱湯を3秒間処置した。熱傷処置の3日後に処置部の壊死組織を除去して皮膚欠損創を作成した。この熱傷創モデルは組織修復を評価するモデルとして一般的であり、広く用いられている。この熱傷創部分に被験物質を貼付し、フィルムドレッシング材(テガダーム)と伸縮性包帯で固定した。また被験物質を貼付せず、フィルムドレッシング材と伸縮性包帯のみで処置したものを対照(Control)とした。
被験物質の貼付後10日目に表皮伸展の程度(表皮面積、表皮面積率)を計測した。表皮伸展は全層皮膚欠損創の評価において一般的に用いられるものであり、組織修復の程度を反映する評価指標である。表皮面積は、作成した欠損創の辺縁から中央部に向かって伸びた表皮の面積を透明シートにトレースし、画像解析ソフトで面積を数値化することにより計測した。表皮面積率は、創全体の面積に対する表皮で覆われた部分の面積の割合(%)として算出した。
Controlに対して、CT+が創傷治癒効果を示すか評価した。
表皮面積率は、Controlでは17.9±3.2(SD)%であったのに対して、カチオン化キトサン(CT+)単独では2.0±1.3(SD)%と有意な低下がみられた(図5)。

Claims (10)
- カチオン化キトサンを有効成分とする、硫酸化グリコサミノグリカンに由来する出血活性の抑制剤。
- カチオン化キトサンが4級アンモニウム基を有する、請求項1に記載の抑制剤。
- 硫酸化グリコサミノグリカンが、ヘパリン、ヘパラン硫酸、ケラタン硫酸、コンドロイチン硫酸又は硫酸化ヒアルロン酸である、請求項1又は2に記載の抑制剤。
- カチオン化キトサンと硫酸化グリコサミノグリカンとからなる複合体。
- カチオン化キトサンと硫酸化グリコサミノグリカンとがイオン結合している、請求項4に記載の複合体。
- カチオン化キトサンが4級アンモニウム基を有する、請求項4又は5に記載の複合体。
- 硫酸化グリコサミノグリカンが、ヘパリン、ヘパラン硫酸、ケラタン硫酸、コンドロイチン硫酸又は硫酸化ヒアルロン酸である、請求項4~6のいずれか1項に記載の複合体。
- 請求項4~7のいずれか1項に記載の複合体を含有する創傷被覆材。
- 請求項4~7のいずれか1項に記載の複合体を有効成分とする、生体組織治療用医薬。
- 創傷治療剤である、請求項9に記載の医薬。
