Definitions
- the present invention relates to a cuffed tube, and more particularly to a cuffed tube in which a cuff is provided on the outer periphery of a deformable tube such as a separation lung ventilation device.
- Patent Document 1 For example, in thoracic surgery, as disclosed in Patent Document 1, a thin flexible tube is inserted from the trachea to the bronchus of the patient and the distal end portion is placed at an appropriate position to isolate the damaged lung. There is a need to insert a deformable, flexible cuffed tube to occlude and supply air to other lungs.
- a cuffed tube used for performing a surgical procedure on a patient inserts a cuff attached to a deformable flexible tube to an appropriate position, and then inserts air into the cuff.
- the working fluid is fed and expanded, and the outer surface of the cuff is brought into pressure contact with the inner wall of the trachea, thereby fixing the position of the cuff and the tube and ensuring airtightness.
- the cuff when the cuff is inflated and fixed in a short bronchus located below the tracheal bifurcation as in the case of a separated lung ventilation device, the cuff is displaced and the cuff is located up to the tracheal bifurcation. If they are shifted, the airtightness cannot be maintained and the position adjustment is required again. However, the operation of readjusting the position of the flexible tube is not preferable because it interrupts treatment and increases the burden on the patient.
- a stenosis or the like occurring in a body lumen or a body lumen such as a blood vessel, a bile duct, an esophagus, a trachea, a urethra, or the like is also expanded by a cuff
- a cuff even in such a case, the fixed position of the cuff may be shifted and a desired treatment may not be performed.
- the present invention temporarily inserts a cuffed tube into an affected area, inflates the cuff and positions the cuff and the tube, and then the cuff and the tube are temporarily changed. It is another object of the present invention to provide a cuffed tube that can prevent the movement of the cuff as much as possible even when an external force that causes the tube to move acts.
- the cuffed tube according to the present invention for achieving the above object is A cuffed tube provided on the outer periphery of a flexible tube with a cuff that expands or contracts by introduction or discharge of a working fluid,
- the cuff includes a cuff fixing portion that expands outward by introduction of the working fluid, and one attachment portion and the other attachment portion that are attached to the outer peripheral surface of the tube, and the one attachment portion and the cuff fixation.
- the connecting portion with the portion is fixed to the outer peripheral surface of the tube in a constricted posture, and the connecting portion between the other attachment portion and the cuff fixing portion is fixed to the outer peripheral surface of the tube in a constricted posture,
- the connecting portion between the other attachment portion and the cuff fixing portion is fixed to the outer peripheral surface of the tube in a constricted posture
- the cuff is formed in a substantially barrel shape in which the cuff fixing portion expands on both sides around the axial center, and the one cuff fixing portion and the one attachment portion are disposed outward in the axial direction of the barrel cuff fixing portion.
- the one mounting part is overlapped with the cuff fixing part so that the one mounting part overlaps the other cuff fixing part.
- the other attachment portion is pushed into the one attachment portion side so that the other attachment portion partially overlaps the cuff fixing portion. It may be fixed to the outer peripheral surface of the flexible tube.
- the cuff is formed in a substantially barrel shape in which the cuff fixing portion expands on both sides around the axis, and the one cuff fixing portion and the one attachment portion are arranged outwardly in the axial direction of the barrel cuff fixing portion.
- the other attachment portion extends, and the connection portion between the one attachment portion and the cuff fixing portion is formed in a constricted manner, and the connection portion between the other attachment portion and the cuff fixing portion is further formed.
- the connecting portions may be fixed to the outer peripheral surface of the flexible tube in a constricted posture.
- the cuff is formed in a substantially barrel shape in which the cuff fixing portion expands on both sides around the axis, and the one cuff fixing portion and the one attachment portion are arranged outwardly in the axial direction of the barrel cuff fixing portion.
- Each of the other mounting portions is extended, When the cuff is fixed to the outer peripheral surface of the tube, the one attachment portion and the other attachment portion are turned inside out, and the one attachment portion and the other attachment portion that are turned upside down can be connected to each other. It may be fixed to the outer peripheral surface of the flexible tube.
- the tube with a cuff formed in this way can absorb the external force by the deformation of the cuff itself even if an external force is applied to the cuff fixed in advance.
- the cuff has a substantially cylindrical shape with the cuff fixing portion, the one attachment portion, and the other attachment portion, When the cuff is fixed to the outer peripheral surface of the tube, the connecting portion between the cuff fixing portion and the one attaching portion, and the connecting portion between the cuff fixing portion and the other attaching portion are constricted. It may be pushed inward so as to be fixed to the outer peripheral surface of the flexible tube.
- the cuffed tube according to the present invention even if an external force acts on the cuff in a state where the cuff is once positioned on the abutted portion such as the inner wall of the trachea, even if an external force acts on the cuff by the external force, It is the tube that moves, and the main part of the cuff fixing portion does not move from the original position in contact with the contacted part. Moreover, since the tightness of the cuff is secured, it does not interfere with the treatment.

- the main part 9 of the central cuff fixing part 22 remains fixed in the initial position. Therefore, when the external force applied to the tube 2 is released, the tapered side walls 12 and 13 indicated by the two-dot chain line in FIG. 4 return to the original positions indicated by the solid line. Further, since the main portion 9 of the cuff fixing portion 22 is not moved during the return, the fixing position of the cuff 4 does not move relative to the abutted portion such as the inner wall 15.
- the cuff 4 when inserting into the trachea 14 or the like, the cuff 4 is in a substantially deflated state before insertion. Then, after confirming that the cuff 4 is in a substantially deflated state, the cuffed tube 1 is inserted into the trachea 14. Then, after confirming that the cuff 4 at the distal end has reached a predetermined position, working fluid is introduced into the cuff 4 from the outside via the pilot balloon 6 and the thin tube 5 shown in FIG. Inflate outward. When the cuff 4 is thus expanded, the main portion 9 of the cuff fixing portion 22 of the cuff 4 comes into contact with the inner wall 15 of the trachea 14 as shown in FIG.
- the cuffed tube 1 is positioned and fixed in the trachea 14.
- the cuff 4 remains in that position unless the patient changes his / her posture or an external force is applied to the tube 2. Further, air tightness or liquid tightness at the outer periphery of the cuff is ensured.

Further, in this state, from the bronchus side (left side in FIG. 3) to the mouth side (right side in FIG. 3) from the expanded cuff fixing part 22 or from the cuff fixing part 22 as a boundary. Body fluids do not flow from the mouth side toward the bronchus side.
- the case where the patient's body position changes or the external force acts on the tube 2 is assumed to be within a certain range, and the cuff fixing portion 22 and the contacted portion (in this embodiment) It does not assume that a large force exceeding the frictional force with the inner wall 15) of the trachea 14 acts or a force that causes the tube to move beyond the movable range of the cuff.
- the main portion 9 of the cuff fixing portion 22 of the cuff 4 abuts against the inner wall 15 of the trachea 14 to fix the position. Even if a force for moving in the direction of arrow A is applied, the posture changes only in the tapered side walls 12 and 13, and the posture of the main part 9 does not change.
- One tapered side wall 12 is bent, for example, as shown in FIG. 6, and the other tapered side wall 13 is in a fully extended posture.
- the cuff 4 Since the main portion 9 of the cuff fixing portion 22 expanded at this time remains in contact with the inner wall 15 with a predetermined pressure, the cuff 4 does not move relative to the contacted portion, that is, the inner wall 15 of the trachea 14. . Further, since the cuff 4 remains fixed in a state where the main portion 9 of the cuff fixing portion 22 is expanded, airtightness is also ensured.
- the constricted portion can be used as a liquid pool. Therefore, even if body fluid or the like tries to flow beyond the cuff 4 to the opposite side, the flow of the body fluid can be captured at the constricted portion.
- the fixing position of the cuff 4 with respect to the patient is not shifted. Absent. Therefore, there is no need to perform position adjustment again. Thereby, it is not necessary to perform an operation that increases the burden on the patient.
- the range of the movable range of the tube 2 for keeping the cuff 4 fixed at the initial position can be set by appropriately adjusting the length t shown in FIGS. . Further, in order to improve the frictional force with the contacted portion, the frictional force is reduced by increasing the radial length Y with respect to the axial length X of the cuff fixing portion 22 shown in FIG. The cuff mobility can be adjusted.
- FIGS. 1 to 6 an example is shown in which one attachment portion 10 and the other attachment portion 11 formed on both sides of the cuff fixing portion 22 are pushed and fixed to the center side.
