Description
白血球除去療法の対象となる疾患には、さまざまな免疫細胞による免疫反応が深く関与している。免疫反応に大きな役割を担うT細胞は、その表面に存在する抗原レセプターに抗原が結合することによって活動を開始するが、T細胞の反応には抗原提示細胞の存在が必要である。T細胞は抗原提示細胞上の主要組織適合抗原(MHC抗原)と抗原の組み合わせに反応する。
このようにHLA-DR抗原陽性細胞である抗原提示細胞は、免疫反応の最初の段階から、さらに、それに続く反応にも深く関与する重要な免疫細胞であるといえる。
また、白血球除去器の対象疾患である関節リウマチや潰瘍性大腸炎の患者は、健常人に比べてHLA-DR抗原陽性細胞の割合が高いことも知られている(非特許文献1および2を参照)。
また、特許文献2には、単球および/または単球由来のマクロファージを選択的に除去するフィルター装置が開示されている。
しかしながら、臨床で使用できるHLA-DR陽性単球の選択除去器は未だ知られておらず、特許文献1および2にも、HLA-DR陽性単球を選択的に除去することができる手段は開示されていない。

抗血液凝固剤を含む血液からHLA-DR陽性単球を選択的に除去する、HLA-DR陽性単球の選択除去器を製造するための抗酸化剤の使用であって、
前記HLA-DR陽性単球の選択除去器は、表面を改質していないポリエステル不織布からなる細胞除去フィルターを備え、
前記細胞除去フィルターが、抗酸化剤を含む充填液で充填され、かつ、放射線滅菌され、
前記抗血液凝固剤は、クエン酸およびその塩からなる群から選択される少なくとも1種であり、
前記抗酸化剤は、ピロ亜硫酸ナトリウム、ピロ亜硫酸カリウム、チオ硫酸ナトリウム、亜硫酸水素ナトリウム、亜硫酸ナトリウム、次亜リン酸、アスコルビン酸、およびアスコルビン酸誘導体からなる群から選択される少なくとも1種である、HLA-DR陽性単球の選択除去器を製造するための抗酸化剤の使用。

選択除去器としては、特に限定されるものではないが、例えば、細胞除去フィルターを積層状に充填した容器や、円筒状に巻いた細胞除去フィルターを充填した容器などが挙げられる。
選択除去器の容器形状は、特に限定されるものではないが、例えば、円柱状、または三角柱状、四角柱状、六角柱状もしくは八角柱状などの角柱状などが挙げられる。
選択除去器の容器形状は、好ましくは円筒状である。好適な円筒状の選択除去器としては、血液の流れが円筒の外周より入り内側へと流れ、最も内側に集まり血液流出口より出ることを特徴とする容器が挙げられる。また、容器の断面積が血液の流れの入口から出口に向かうに従って、小さくなる形状を有する容器なども好適に用いられる。
本実施の形態において、選択除去器には、さらに、血液を処理する際や選択除去器を洗浄する際に液を流入または流出させるために使用するチューブ、回路などを設けることもできる。
選択除去器の容器の材質としては、ポリカーボネート樹脂などが挙げられる。
ポリエステル不織布を積層状にした積層体や、円筒状にポリエステル不織布を巻くことにより細胞除去フィルターとすることができる。
本実施の形態における「表面を改質していないポリエステル不織布」とは、血液とポリエステルが直接接触するように不織布の表面にポリエステルが配置されていることをいい、ポリエステル不織布の表面に親水化処理などが施されていないことを意味する。
親水化処理には、例えば、ポリマーによるコーティングなどが挙げられる。
ポリエステル不織布の繊維径が、大きすぎる場合には接触効率が低下し、小さすぎる場合には凝集塊などの目詰まり物質の吸着量が増加する観点から、ポリエステル不織布の平均繊維径は、好ましくは0.5μm以上50μm以下の範囲内であり、より好ましくは1μm以上40μm以下の範囲内であり、さらに好ましくは1.5μm以上35μm以下の範囲内である。
細胞除去フィルターを構成する1枚または複数枚のポリエステル不織布から目視で実質的に均一と認められる部分をサンプリングする。
サンプリングに際しては、ポリエステル不織布のうち、血液を濾過する際に、血液が透過する部分に該当する有効濾過断面積部分を、1辺が0.5cmの正方形によって区分し、その中から6ケ所をランダムサンプリングする。ランダムサンプリングするには、例えば、区分した箇所の全てに番地を指定した後、乱数表を使うなどの方法で、必要な数の区分を選ぶことにより行う。3ケ所以上好ましくは5ケ所以上のサンプリングした各区分について、走査電子顕微鏡を用いて拡大倍率2500倍で写真に撮る。
サンプリングした各区分について中央部分及びその近傍の箇所の写真を撮っていき、写真に撮られた繊維の合計本数が区分ごとに100本を超えるまで写真を撮り、繊維の直径を測定する。
但し、複数の繊維が重なり合っており、他の繊維の陰になって繊維の幅が測定できない場合、複数の繊維が溶融するなどして、太い繊維になっている場合、著しく直径の異なる繊維が混在している場合等には、これらのデータは考慮せずに、繊維の幅を測定する。
「複数の繊維が重なり合っており、他の繊維の陰になって繊維の幅が測定できない場合」とは、走査電子顕微鏡写真において陰になっている場合を意味し、「複数の繊維が溶融するなどして、太い繊維になっている場合」とは、他の繊維径の平均の1.5倍以上となるものを溶融などしている場合を意味し、「著しく直径の異なる繊維が混在している場合」とは、他の繊維径の平均の2倍以上となる場合を意味する。
本実施の形態における「抗酸化剤」とは、他の分子などに電子を与え易い性質を持つ原子、分子またはイオンを意味し、抗酸化剤は、細胞除去フィルターが酸素により変化を受けるのを抑制する性質を有する。
抗酸化剤は、ピロ亜硫酸ナトリウム、ピロ亜硫酸カリウム、チオ硫酸ナトリウム、亜硫酸水素ナトリウム、亜硫酸ナトリウム、次亜リン酸、アスコルビン酸、およびアスコルビン酸誘導体からなる群から選択される少なくとも1種である。
アスコルビン酸誘導体としては、特に限定されるものではないが、例えば、5-メチル-3、4-ジヒドロキシテトロン(MDT)などが挙げられる。
抗酸化剤は1種類で用いてもよいし、2種類以上を混合して用いてもよい。
抗酸化剤としては、取扱い性、安全性の観点から、好ましくはピロ亜硫酸ナトリウムである。
本実施の形態における「充填液」とは、選択除去器の製造段階で、選択除去器の内部に充填される液体を意味し、充填液には抗酸化剤が添加されている。
抗酸化剤を含む充填液は、抗酸化剤を水、または生理食塩水などの生理的溶液などの充填液に直接添加して溶解するか、またはあらかじめ少量のアルコールなどの溶剤に抗酸化剤を溶解し、得られた溶液を充填液に添加して作成することができる。
抗酸化剤を含む溶液中の抗酸化剤の濃度は、選択除去器に用いられる抗酸化剤の種類および滅菌の条件によって最適な濃度が決定されるが、好ましくは100ppm以上1000ppm以下であり、より好ましくは300ppm以上1000ppm以下である。
本実施の形態における「放射線」とは、α線、β線、中性子線、または電子線などの粒子線や、X線またはγ線などの電磁波を含む電離放射線を意味し、放射線は、医療用具の滅菌に常用される観点から、好ましくは電子線またはγ線である。
γ線は照射対象物質の原子あるいは原子核との相互作用を介して荷電粒子線を発生させ、二次的に発生した荷電粒子線が電離作用を持つ。
本実施の形態における電子線およびγ線による微生物の滅菌効果は、電離作用により生じたフリーラジカルが微生物のDNA鎖を損傷し、細胞分裂能を阻害することによるものであり基本的な滅菌原理は同一である。
本実施の形態における「D値」とは、特定の微生物数の90%を死滅させる、または特定の微生物数を10分の1に減少させる滅菌処理単位(時間又は吸収線量など)を意味する。
電子線滅菌の方がγ線滅菌よりもポリエステルの分解が減少する可能性は考えられるが、ラジカルが生成しないわけではない。
本実施の形態における「HLA-DR陽性単球」とは、単球であるHLA-DR陽性細胞を意味し、「HLA-DR陽性B細胞」とは、B細胞であるHLA-DR陽性細胞を意味する。
抗血液凝固剤としては、特に限定されるものではないが、例えば、クエン酸ナトリウム溶液、ACD-A液、CPD液などが挙げられる。ACD-A(acid-citrate-dextrose)液は、クエン酸ナトリウム、クエン酸、およびブドウ糖からなる溶液であり、CPD(citrate-phosphate-dextrose)液はクエン酸ナトリウム、クエン酸、ブドウ糖、およびリン酸二水素ナトリウム二水和物からなる溶液である。
抗血液凝固剤は1種類で用いてもよいし、2種類以上を混合して用いてもよい。
前記第1の流路に接続され、かつ、抗血液凝固剤を含む血液を前記細胞除去フィルターに供給することができる、クエン酸およびその塩からなる群から選択される少なくとも1種の抗血液凝固剤を含む容器と、
を備えていることが好適である。
選択除去器10は、例えば図1に記載の構成をなす。第1の流路20は、抗血液凝固剤を含む容器30および細胞除去フィルター40を接続した流路である。第1の流路20は送液ポンプ50に取り付けられ、患者から採血された血液を細胞除去フィルター40に送液することができる。容器30中に貯留された抗血液凝固剤は第1の流路20に供給され、第1の流路20を流れる血液と混合される。そして、この抗血液凝固剤を含む血液が細胞除去フィルター40に導入され、濾過が行われる。第2の流路60は、細胞除去フィルターで濾過された血液を細胞除去フィルター外に導出する流路である。
前記細胞除去フィルターを、抗酸化剤を含む充填液で充填する工程、
前記充填液で充填された細胞除去フィルターを放射線滅菌する工程、および
クエン酸またはその塩を含む抗血液凝固剤を含有する血液を前記フィルター装置に通液して濾過する工程。
放射線滅菌の方法は、特に限定されるものではないが、例えば、HLA-DR陽性単球の選択除去器を包装して照射する方法などが挙げられる。
血液を細胞除去フィルターで濾過することにより、細胞除去フィルターにHLA-DR陽性単球が捕捉され、HLA-DR陽性単球を選択的に除去することができる。
本実施の形態においてHLA-DR陽性単球を選択的に除去した血液を製造する方法は、
細胞除去フィルターを、抗酸化剤を含む充填液で充填する工程、
充填液で充填された細胞除去フィルターを放射線滅菌する工程、および
クエン酸またはその塩を含む抗血液凝固剤を含有する血液をフィルター装置に通液して濾過する工程を含む。
本実施の形態のHLA-DR陽性単球の選択除去器を製造するための抗酸化剤の使用は、前記放射線がγ線または電子線であることが好適である。
関節リウマチ患者または潰瘍性大腸炎患者を対象として臨床で使用される場合には、血液3LをHLA-DR陽性単球の選択除去器で処理することを週1回ずつ計5回繰り返すことが好適である。
まず、採血回路、送液ポンプ、返血回路に接続したHLA-DR陽性単球の選択除去器に、採血回路に接続した生理的溶液1Lを流す。続いて、抗血液凝固剤を添加した生理的溶液500mLをHLA-DR陽性単球の選択除去器に流す。
続いて、採血回路を患者の肘静脈などに接続し、返血回路を患者の別の肘静脈などに接続する。採血回路の分岐には抗凝固剤入りの生理的溶液へつながる導管を接続する。
送液ポンプを50mL/minの流速で1時間動かし、抗血液凝固剤を含む生理的溶液を添加した血液3Lを選択除去器に通す。血液3Lを通し終わったら、生理的溶液を採血回路に接続し、生理的溶液を選択除去器に500mL流して患者に血液を返す。
抗血液凝固剤を含む生理的溶液の添加方法としては、特に限定されるものではないが、例えば、4%(w/v)クエン酸ナトリウムを含む生理的溶液の1容量に対し、血液が5~40容量となるようにポンプを動かして血液に生理的溶液を添加する方法などが挙げられる。
本実施の形態における「血液の導入部」とは血液を導管まで導く部分のことを意味する。
血液の導入部としては、特に限定されるものではないが、例えば、スパイク針、採血針などが挙げられる。
導管としては、特に限定されるものではないが、例えば、塩化ビニル、シリコン、ポリプロピレン、ポリエチレンなどが挙げられる。導管としては、強度の観点から、好ましくは塩化ビニルが用いられ、塩化ビニルの具体例としては、LC-3000N(旭化成クラレメディカル社)が挙げられる。
生理的溶液として、特に限定されるものではないが、例えば、生理食塩液(生理食塩水を含む)、リンゲル液、乳酸リンゲル液、酢酸リンゲル液、タイロード液などの生理的塩類溶液;ブドウ糖、デキストラン、ヘパリンなどの糖類含有液;抗凝固剤含有液などが挙げられる。
生理的溶液としては、上記に例示した生理的塩類溶液、糖類含有液、抗凝固剤含有液などを少なくとも一つ以上加えた混合液を希釈した液も好ましく用いられる。
生理的溶液は1種類で用いてもよいし、2種類以上を任意の割合で混合した混合液として用いてもよく、中でも、生理食塩液が用いられる。
患者から採取した血液としては、特に限定されるものではないが、例えば、全血、血漿、または赤血球製剤もしくは血小板製剤などの血液由来製剤などが挙げられる。
ポリエチレンテレフタレート(PET)不織布(平均繊維径2.3μm、目付90g/m2、厚み0.45mm)を直径6.8mmの円状に切り取り、11枚を重ねて細胞除去フィルターとし、該細胞除去フィルターを直径6.8mmで容器容量1mLのポリエチレン製のカラム(モビコール(登録商標)、モビテック社)に積層した(充填密度 0.14g/cm3)。カラム内をピロ亜硫酸ナトリウム水溶液(ピロ亜硫酸ナトリウム:林純薬)で充填し、該カラムを段ボールケースに入れて包装し、25kGyのγ線を照射して滅菌処理した。得られたカラムを選択除去器とした。水溶液中のピロ亜硫酸ナトリウムの濃度は800ppmとした。
健常人ボランティアより血液を採取し、抗血液凝固剤としてクエン酸ナトリウム水和物を4%(w/v)含むクエン酸ナトリウム液(チトラミン(登録商標)、扶桑薬品)が9.1%(v/v)含まれるようにクエン酸ナトリウム液を添加したヒト新鮮血液(クエン酸ナトリウム水和物の最終濃度0.36g/100mL)を濾過前サンプルとした。
滅菌済みの選択除去器を生理食塩水(大塚生食注(登録商標)、大塚製薬)を通液することで洗浄し、濾過前サンプルを0.18mL/分で10分濾過した。その際出口より流出する血液を濾過後サンプルとして、以下の測定に用いた。
白血球除去率については濾過前後の白血球濃度を自動血球数測定装置(Sysmex SF-3000(東亜医用電子製))を用いて測定し次式(1)より算出した。

充填液として、水(ピロ亜硫酸ナトリウムを含まない)を充填する以外は、実施例1と同様の方法で白血球除去器を得た。
抗血液凝固剤としてメシル酸ナファモスタット10mg/100mLの溶液が12%(v/v)含まれるようにメシル酸ナファモスタット溶液(フサン(登録商標)、鳥居薬品、以下、同じ。)を添加したヒト新鮮血液(メシル酸ナファモスタットの最終濃度1.2mg/100mL)を濾過前サンプルとした。上記白血球除去器を用い、実施例1と同様の方法で濾過前サンプルを濾過した。白血球濃度を測定して、白血球除去率を算出した。
抗血液凝固剤としてメシル酸ナファモスタット10mg/100mLの溶液が12%(v/v)含まれるようにメシル酸ナファモスタット溶液を添加したヒト新鮮血液(メシル酸ナファモスタットの最終濃度1.2mg/100mL)を濾過前サンプルとした。実施例1の選択除去器を用い、実施例1と同様の方法で濾過前サンプルを濾過した。白血球濃度を測定して、白血球除去率を算出した。
比較例1と同様に作成した白血球除去器を用いた以外は、実施例1と同様の方法で濾過を行った。白血球濃度を測定して、白血球除去率を算出した。
実施例1と同様に作成したカラム内を100ppm、300ppmまたは1000ppmのピロ亜硫酸ナトリウム水溶液で充填した以外は実施例1と同様の方法で白血球除去率を算出した。
抗血液凝固剤としてACD-A液(0.8%クエン酸、2.2%クエン酸ナトリウム、2.2%ブドウ糖溶液を含む、テルモ社)が12%(v/v)含まれるようにACD-A液を添加したヒト新鮮血液を濾過前サンプルとした。実施例1の選択除去器を用い、実施例1と同様の方法で濾過前サンプルを濾過した。白血球濃度を測定して、白血球除去率を算出した。
実施例1~5および比較例1~3の結果を表1に示す。
濾過前、濾過後サンプルをそれぞれ蛍光標識された抗HLA-DR抗体(BDバイオサイエンス社)と反応させてフローサイトメトリー(FACS Calibur、BDバイオサイエンス社)で解析した。抗HLA-DR抗体と反応したものをHLA-DR陽性細胞とし、サンプルごとのHLA-DR陽性細胞濃度を算出した。
各実施例および比較例における、除去されたHLA-DR陽性細胞の白血球に占める割合を次式(2)により算出した。

除去されたHLA-DR陽性細胞の白血球に占める割合(%)=(濾過前HLA-DR陽性細胞濃度-濾過後HLA-DR陽性細胞濃度)/(濾過前白血球濃度-濾過後白血球濃度)×100
算出された割合について、比較例1の値を1.0とした結果を表1に示す。
試験例1と同様に、サンプルをそれぞれ蛍光標識された抗HLA-DR抗体、抗CD14抗体(BDバイオサイエンス社)と反応させてフローサイトメトリーで解析した。抗HLA-DR抗体および抗CD14抗体と反応したものをHLA-DR陽性単球とし、サンプルごとのHLA-DR陽性単球濃度を算出した。
各実施例および比較例における、除去されたHLA-DR陽性単球の白血球に占める割合を次式(3)により算出した。

除去されたHLA-DR陽性単球の白血球に占める割合(%)=(濾過前HLA-DR陽性単球濃度-濾過後HLA-DR陽性単球濃度)/(濾過前白血球濃度-濾過後白血球濃度)×100
算出された割合について、比較例1の値を1.0とした結果を表2に示す。
試験例1と同様に、サンプルをそれぞれ蛍光標識された抗HLA-DR抗体、抗CD19抗体(BDバイオサイエンス社)と反応させてフローサイトメトリーで解析した。抗HLA-DR抗体および抗CD19抗体と反応したものをHLA-DR陽性B細胞とし、サンプルごとのHLA-DR陽性B細胞濃度を算出した。
各実施例および比較例における、除去されたHLA-DR陽性B細胞の白血球に占める割合を次式(4)により算出した。

除去されたHLA-DR陽性B細胞の白血球に占める割合(%)=(濾過前HLA-DR陽性B細胞濃度-濾過後HLA-DR陽性B細胞濃度)/(濾過前白血球濃度-濾過後白血球濃度)×100
算出された除去された各分画の白血球に占める割合について、比較例1の値を1.0とした結果を表3に示す。
HLA-DR陽性単球の選択的除去には、クエン酸および/またはクエン酸の塩のカルシウムキレート作用による細胞の接着能低下が関与していると推定される。
また、充填液の抗酸化作用により、不織布表面の陰性荷電が減少した結果、HLA-DR陽性単球に対する選択性が増加したと考えられる。
本発明のHLA-DR陽性単球の選択除去器を製造するための抗酸化剤の使用、HLA-DR陽性単球の選択除去器およびHLA-DR陽性単球を選択的に除去する方法は、全身性エリテマトーデス、関節リウマチ(悪性関節リウマチを含む)、多発性硬化症、潰瘍性大腸炎、クローン病などの自己免疫性疾患、白血病などの治療に産業上の利用可能性を有する。

Claims (7)
- 抗血液凝固剤を含む血液からHLA-DR陽性単球を選択的に除去する、HLA-DR陽性単球の選択除去器を製造するための抗酸化剤の使用であって、
前記HLA-DR陽性単球の選択除去器は、表面を改質していないポリエステル不織布からなる細胞除去フィルターを備え、
前記細胞除去フィルターが、抗酸化剤を含む充填液で充填され、かつ、放射線滅菌され、
前記抗血液凝固剤は、クエン酸およびその塩からなる群から選択される少なくとも1種であり、
前記抗酸化剤は、ピロ亜硫酸ナトリウム、ピロ亜硫酸カリウム、チオ硫酸ナトリウム、亜硫酸水素ナトリウム、亜硫酸ナトリウム、次亜リン酸、アスコルビン酸、およびアスコルビン酸誘導体からなる群から選択される少なくとも1種である、HLA-DR陽性単球の選択除去器を製造するための抗酸化剤の使用。 - 前記充填液が、300ppm以上のピロ亜硫酸ナトリウムを含む、請求項1に記載の抗酸化剤の使用。
- 前記放射線が、γ線または電子線である、請求項1または2に記載の抗酸化剤の使用。
- 抗血液凝固剤を含む血液からHLA-DR陽性単球を選択的に除去するためのHLA-DR陽性単球の選択除去器であって、
表面を改質していないポリエステル不織布からなる細胞除去フィルターを備え、
前記細胞除去フィルターが、抗酸化剤を含む充填液で充填され、かつ、放射線滅菌され、
前記抗血液凝固剤は、クエン酸およびその塩からなる群から選択される少なくとも1種であり、
前記抗酸化剤は、ピロ亜硫酸ナトリウム、ピロ亜硫酸カリウム、チオ硫酸ナトリウム、亜硫酸水素ナトリウム、亜硫酸ナトリウム、次亜リン酸、アスコルビン酸、およびアスコルビン酸誘導体からなる群から選択される少なくとも1種である、HLA-DR陽性単球の選択除去器。 - 前記充填液が、300ppm以上のピロ亜硫酸ナトリウムを含む、請求項4に記載の選択除去器。
- 前記放射線が、γ線または電子線である、請求項4または5に記載の選択除去器。
- 上流側の第1の流路および下流側の第2の流路に接続され、抗酸化剤を含む充填液を充填して放射線滅菌された細胞除去フィルターと、
前記第1の流路に接続され、かつ、抗血液凝固剤を含む血液を前記細胞除去フィルターに供給することができる、クエン酸およびその塩からなる群から選択される少なくとも1種の抗血液凝固剤を含む容器と、
を備えたHLA-DR陽性単球の選択除去器。
