På grund av det stora intresse; som visats PP har undersökning gjorts i syfte att framställa substanser med liknande aktivitet men resistenta mot hydrolys; Detta ändamål har uppnåtts delvis ge- nom syntes av bifosfonater; dvs ämnen; som innehåller gruppen- P-C-P i stället för gruppen P-O-P. Verkan av bifosfonaterna på kalciumsalter liknar verkan av PP} i själva verket har de redan i låg koncentration följande verkningar: de hämmar utfällning av kalciumfosfat ur lösningarfi de blockerar överföring av amorft kalciumfosfat till kristallina formen utan att dock hämma bildningen av begynnelsefasen; de blockerar aggregationen av kristaller av hydråxiapatitp de retarderar upplösningsgraden för kristaller av hydroxylapa- tit, sedan de senare absorberat bifosfonaterna ur lösningarna.
Många farmakologiska och kliniska studier i den vetenskapliga litteraturen visar emellertid; att trots vissa analogier i fråga om aktivitet de hittills för behandling av bensjukdomar använda många bifosfonaterna har några helt allvarliga olägenheter i fråga om toxicitet hos djur och toleransen eller framkallning av nega- tiva biverkningar på människor.
Det har nu överraskande befunnitsp att några bifosfonsyror med Gllmänna formeln I: i ï°3H2 R-ç-R' (I) PO3H2 10 15 25 i \O i vilken R betecknar fluor eller en linjär eller grenad alkylgrupp med 1-5 kolatomer; som eventuellt kan vara substituerad med en substituent ttert aminogrupper och/eller fluoratomer och R1 hydroxi _eller fluor y och deras salter med olkalimetaller, organiska baser eller basiska aminosyror lämpar sig mycket bra för behand1ing_av urinsten och som inhibitorer för återupptagning i benen ; efter- som de har hög aktivitet, som icke åtföljes av de ovan angivna bi- verkningarna hos pyrofosfatet (PP)l Många bifosfonsyror har beskrivits i litteraturen. I synnerhet kan bifosfonsyror med den allmänna formeln I p i vilken R är en osub- stituerad alkyl och R] hydraki; framställas genom omsättning av en acylhalogenid eller anhydriden av en syra med fosforsyra eller fos- fortriklorid; Även om förfarandet ger goda utbyten i det fall; att R är etylp är detsamma mindre lämpligt för framställning av ana- loger innehållande en alkylgrupp med högre molekylvikt och fungerar praktiskt taget icke; då gruppen R är en med funktionella grupper substituerad alkylgrupp; Dessutom är detta förfarande áfldeles klart icke lämpligt för fram- ställning av föreningar med formeln I; i vilken R och/eller R' är fluoratomerg Det har befunnitsp att det är möjligt att framställa föreningar med allmänna formeln I; i vilken R är en aminoalkyl- grupp och R' hydroxi,med utmärkta utbyten och med hög grad av renhet, om man omsätter en aminosyra med.fosforsyra i syfte att blockera den reaktiva aminogruppen och sedan med fosfortrikloridt Mellan- produkten hydrolyseras därefter och produkten isoleras på lämp- ligtïsätt: I stället för blandningen av fosforsyra och fosfortriklorid är det 10 15 25 .fosforsyrat möjligt att använda endast fosfortriklorid; varvid man tillsätter den stökiometriska mängden vatten i syfte att bilda motsvarande Närhelst det är möjligt? kan aminosyran ersättas med en sikt pre- kursor i stånd att bilda aminosyran genom hydrolys såsom valero- laktam eller motsvarande polyamid i fråga om 5-aminovaleriansyra eller pyrrolidonet ifråga om 4-amino-smörsyra. Reaktionen utföres företrädesvis i närvaro av ett aprotiskt organiskt lösningsmedel såsom ett alifatiskt eller aromatiskt kolväte eller motsvarande klorerade kolväten; men kan också utföras i frånvaro av lösningsme- del.

Använda metoder _ 1. Försök på dädskalleceller Cellkulturz Cellerna odlas enligt den av Fast et al. beskrivna me- toden (Biochem. J; lZ2_ ,97-107 (1978) ). Totalt uppslutes en dag gamla huvudskålar från Wistar-råttor med kollagenas. De frisatta cellerna anbringas på en platta med en koncentration av 200;000 celler per.ml medium i skiv-"klasar" lämpliga för uppodlingj varvid plattorna har 24 urtagningar med ljó cm i diameter innehållande 0,5 ml medium; Cellerna odlas i det väsentliga minsta mediet inne- hållande 10% fetalt kalvserum i en atmosfär av 5% C02 vid 37°C upp till åttonde dagen. Bifosfonaterna tillsättes på den första dagen fram till slutet av försöketg Mediet bytes~på den första; fjärde I och sjunde dagen.
Räkning av celler Cellerna räknas med en Coulter-räknare efter att ha blivit frisatta från skivorna genom uppslutning med en blandning av kolla- genas och trypsin.
Bestämning av laktat' Mediet bytes på sjunde dagen och cellerna inkuberas 16 tim. Det un- der denna period bildade laktatet mätes i ett extrakt i HCIO4 av mediet under användande av laktatodehydrogenas. 10 15 25 30 4% C- \ (Al P J (JJ xD 19 2. Försök med wåterupptagníng och förkalkning in vivo Grupper om fem Wistar-råttor vägande 180-200 gram behandlas 7 dagar med 0,1 , 1j0 och 10 mg P/kg av följande bifosfonsyror: difluormetanbífosfonsyra (F2MBP)(i form av Na-saltet)j 4-amino-1-hydroxibutanbifosfonsyra (AHBUBP); 5-amino-1-hydroxipentanbifosfonsyra (AHPEBP);_ 6-amino-1-hydroxihexanbifosfonsyra (AHExBP); diklormetanbifosfonsyra (Cl2MBP) ( i form av natriumsaltet) ( 1 m9 P/kg).
Kontrolldjuren tillfördes lösningsmedlet med NaCl° Alla behandlingar- na utfördes på subkutan väg; Föreningarna löstes i NaCl ifråga om de lägre koncentrationerna och i vatten i fråga om den högre kon- centrationen samt administrerades i en volym av 0,2 ml/100 gt Djuren matades med Altromine 1314 innehållande 1;1 g/100 g P och _ 250 IU/100 g vitamin D3l Djuren avlivades på åttonde dagen och skenbenet avlögsnades samt fixerades i 50-procentig etanol. Sken- benen dehydratiserades sedan i ökande koncentration etanol och doppades ner i metylmetakrylat efter tillsats av Plastoic N; Fronta- la snitt togs ut och skars till en tjocklek av 70-80/um; varpå snítten underkastades mikroradiografi. Genom detta förfarande kun- de tätheten av mineralet uppskattas.i den trabekulara meta- fysen genom metoden av Shenk et alt; Cala; Tissl Res. ll; 196- 214; 1973)° Resultat 1; Fàšgak med huvqdskalcgíler -Som framgår av tabell 3; ger Cl2MBP upphov till en minskning av antalet celler; Å andra sidan har F2MBP ingen effekt eller en -ïä O\ (N ND Cr! En signifikant skillnad gäller mineraliseringen, eftersom ÅHEXBP framkallar kraftig hämning av míneraliseringen i en dos av 10 mg P/kg , medan AHBUBP icke har någon verkan eller endast en ringa verkan eller endast en effekt i mycket ringa grad.
Dessa sresultat visar, att aminoföreningarna med-udda antal kolatomer är i någon mån toxiska, men mera aktiva ifråga om att hämma benåter- absorptionen; Föreningarna med jämnt antal kolatomer har en aktivi- tet något högre än för Cl2MBPt Ett annat signifikant faktum är, att ÅHB U _ grad av mineraliseringshämning vid höga doser, medan AHEXBP ger BP icke framkallar eller endast framkallar en mycket ringa stor hämning, Följaktligen synes AHBUBP vara mest lämplig för använd- ning ifråga om sjukdomar med ökad återabsorption av ben på männi- skor; Slutligen är det intressant att notera, att F2MBP icke har någon verkan på benabsorptionen eller på benmineraliseringen och med tanke på det faktum, att föreningen hämmar tillväxten in vitro av apatit- kristallerna, kan den användas med framgång då det gäller njurstenar.
I själva verket har sedan lång tid tillbaka ett bifosfonat i stånd att hämma tillväxten av kristallerna utan att påverka benen varit föremål för forskning. Man drar därför slutsatsen, att de båda sub-I stanserna AHBUBP och AHPEBP kan_utvecklas till mediciner i stånd att hämma benåterabsorptionen och att F MBP är användbar för be- 2 handling av urinsten.
De farmaceutiska kompositionerna enligt föreliggande uppfinning 10 4>~ O*- (N NW b I \.C2 26 kan i ordningstöllas för användning i form av kapslar eller tabletter eller i lösning för oral administrering eller för systemisk användning.
Kompositionerna framställes med fördel tillsammans med inerta böra- re såsom sockerarter (sackaros, glukos, laktos), stärkelse och derivat därav, cellulosa och derivat därav, gummin, fettsyror, och deras salter, polyalkoholer, talk, aromatiska estrar.
De farmaceutiska kompositionerna kan administreras oralt i doser från 25 till 3200 mg/dag eller parenteralt i doser från 15 till 300 mg/dag; Behandling utföres i 7 dagar eller i 3 månader och upp- repas efter behov;
Claims (4)
1. Förening med førmeln 'ïO3H2 "R - C - OH PO H 3 2 i vilken R är en rak alkylgrupp med 3-4 kolatomer, varvid alkylen är substituerad med en aminogrupp i ändställning, till användning för behandling av urinsten och för att hämma benvävnadsåterabsorption.
2. Föreningen 5-amino-1-hydroxipentan-1,1-difosfonsyra, till användning enligt krav 1.
3. Föreningen 4-amino-1-hydroxibutan-1,2-difosfonsyra, till användning enligt krav l.
4. Farmaceutisk beredning för behandling av urinsten och för att hämma benvävnadsâterabsorption, k ä n n e t e c k - n a d av att den innehåller en förening med formeln ?93H2 R - C - OH PO3H2 i Vilken R är en rak alkylgrupp med 3-4 kolatomer, varvid alkylen är substituerad med en aminogrupp i ändställning, tillsammans med en farmaceutiskt fördragbar bärare.
