A presente invenção refere-se aos agrafadores ,
I cirúrgicos para a implantação de dispositivos mecânicos de ligação cirúrgicos no tecido de um paciente e, em particular, aos ' agrafadores cirúrgicos que são accionados por um gás comprimido.
São conhecidos na técnica agrafadores cirúrgji ! cos que implantam dispositivos de fixação cirúrgicos utilizando : a força de um gás comprimido, encontrando-se descritos nas patentes americanas 3 837 555, 3 836 116, 3 815 476, 3 717 294,
662 939, 3 643 851, 3 618 842, 3 613 507, 4 407 432, 4 439 028, e 4 331 277, entre outras. Nos instrumentos representados nestas patentes, um cilindro de gás comprimido está contido na pega do instrumento. 0 gás proveniente do cilindro é conduzido a

Nos agrafadores de gás ooiprimido descritos nas patentes anteriores, a pressão do gás contido no cilindro ê escolhida para proporcionar a força necessária para operar o dispositivo mecânico articulado. 0 dis positivo articulado estende-se através do veio destes instrumentos, entre a pega do instrumento e a cabeça do agrafador. Seria desejável simplificar o aparelho mecânico dos dispositivos da técnica anterior para proporcionar instrumentos mais simples e mais versáteis. Ba particular, seria desejável conduzir o gás comprimido directamente para a cabeça do agrafador, evitando assim a necessidade do dis positivo mecânico articulado entre a fonte de gás comprimido na pega do instrumento e a cabeça do agrafador. Uma outra vantagem resultante de uma tal solução é que o veio do instrumento que liga a pega à cabeça do agrafador pode ser feito flexível, visto que a sua finalidade num tal instrumento aper feiçoado seria ligar tubos pneumáticos flexíveis à cabeça do agrafador, em vez de ccnduzir um dispositivo articulado rígido cano nos dispositivos da técnica anterior.
De acordo ccm os princípios da presente invenção, propor cicnam-se agrafadores cirúrgicos pneumáticos que possuem cabeças de agrafador que respondem ao gás comprimido para fixar o tecido entre as maxilas da cabeça do agrafador e implantar os agrafos no tecido fixado. Numa primeira forma de realização, uma cabeça para agrafador cirúrgico apropriado para implan tar uma fiada de agrafos cirúrgicos no tecido tem a configuração de um dispositivo de êmbolo duplo. Numa primeira actuação do instrumento, o gás ocsm primiâo accicna um primeiro dos êmbolos,contra o tecido a fixar entre as maxilas da cabeça do agrafador. Uma segunda actuação do instrumento faz oom
que o segundo êirbolo accione o impulsor dos agrafos, que implanta agrafos no tecido. Numa, primeira forma de realização preferida, as extremidades do êmbolo são arredondadas para impedir a ligação do êmbolo e garantir a integridade pneumática.
De aoordo oom os princípios da presente invenção numa sua forma de realização preferida, os êmbolos de fi:<ação e de cravação da primeira forma de realização são actuados por ampolas do gênero balão situa das por detrás de cada êmbolo. Numa primeira actuação dos instrumentos, é inflada uma primeira ampola e a sua expansão acciona o êmbolo de fixação para fixar o tecido entre as maxilas da cabeça do agrafador. Orna segunda actuação do instrumento provoca a inflação da segunda ampola para accionar o êmbolo de cravação dos agrafos, que implanta os agrafos no tecido. Esta segunda forma de realização evita a necessidade de proporcionar uma vedação pneumática em tomo dos dois êmbolos, facilitando assim as tolerâncias de fabrico e simplificando ainda mais a construção e a operação do instrumento.
As formas de realização da presente invenção são vantajosas relativamente aos agrafadores pneumáticos da técnica anterior, que for necem gás comprimido a mecanismos mecânicos de agrafamento na pega e na porção do veio do instrumento. Era tais agrafadores da técnica anterior, dispen de-se uma energia considerável na actuação dos dispositivos mecânicos articulados que se estendem da pega e através do veio para a cabeça do agrafador Na presente invenção, a pressão do gás ê fornecida directamente na cabeça do agrafador. Assim, não há qualquer ambiguidade no que respeita â força gerada durante a fixação e a cravação visto que o gás cam a pressão regulada é fornecido directamente nos membros de fixação e de cravação, sem perda de energia.

Oom referência primeiranente â fig. 1, nela estã representado um agrafador cirúrgico actuado pneumaticamente segundo a presente invenção. 0 agrafador inclui três ccrpcnentes principais: uma porção de pega (10), uma porção de veio (108) e uma cabeça de agrafador (110). Os três componentes não unidos nos seus pontos interligação por acessórios pneu mãtioos de desmontagem rápida que permitem desligar e substituir os capcnen tes por veios e cabeças de agrafador ccm outras ocnfigurações. Também localizadas nas uniões (20) e (22) estão as uniões pneumáticas rotativas que permitem a rotação livre dos cccçcnentes principais do agrafador uns em relação aos outros, ocmo estã indicado pelas setas (24) e (26) .Especificamente, o veio (102) pode rodar livremente capletamente em tomo do seu eixo na união (20), ocmo se indica ccm a seta (24). A cabeça de agrafador (110) ê livremente rotativa num ângulo superior a 180° em tomo de um eixo perpendicular ao eixo do veio (108), ocmo se indica pela seta (26).

A primeira fase, num processo de agraf amento, consiste em fixar o tecido a agrafar entre as maxilas da cabeça (110) do agrafador.
O tecido fica situado entre as maxilas e um cursor (74) que se estende a partir da parte traseira do agrafador ê premido. Quando se empurra o cursor para a frente, permite-se a passagem de gãs caprimido para a cabeça do agrafador através de um primeiro tubo de gás para fixar o tecido entre as maxilas da cabeça do agrafador. Quando se libertar o cursor (74), ele desliza para trás para a sua posição estendida representada, por acção da força de uma mola. A posição da porção traseira da cabeça do agrafador depois da fixação estã representada a tracejado em (110*) na fig. 1.
Os agrafos num cartucho (112) podem então ser cravados através do tecido fixado premindo o gatilho (84) da pega. Antes de poder premir-se o gatilho ê no entanto necessário deslocar um dispositivo de segurança (73) do gatilho para cima na sua ranhura (80) a partir da posição
representada na fig. 1 e rcdá-lo para unia extensão dianteira (82) da rauhura ISO). Este mecanismo de segurança impede a implantação intempestiva dos agrafos antes de o utilizador estar pronto para a efectuar. Depois de o dispositivo de segurança (73) do gatilho ter sido deslocado para cima e rodado para a frente, o gatilho pode ser premido para implantar os agrafos no tecido, ou cravando as pernas dos agrafos centra uma bigorna (114) na ca beça do agrafador (110) ou pela interligação das pernas dos agrafos cem receptores dos agrafos de ligação situados na posição da bigorna. Quando se prime o gatilho (84), o dispositivo de segurança e rodado para trás para a sua posição mais vertical e desliza para baixo na ranhura (80) pela força da mola.

Os agrafos (202) estão situados em bolsas formadas no cartucho (112) dos agrafos na maxila (200), oom. cis pernas dos agrafos dirigidas para a maxila fixa (204). Por detrás do cartucho (112) dos agrafos está um impulsor dos agrafos (210). O impulsor dos agrafos tem dedos (212) que estão dirigidos para as coroas dos agrafos respectivos no cartucho de agrafos. Por detrás do impulsor (210) dos agrafos está um êmbolo de cravação (214). O êmbolo de cravação está situado no interior de um êmbolo de fixação (220) e está vedado pneumaticamente no mesmo por dois aneis toroidais periféricos (216) e (218), representados ampliados em pormenor na fig.
3. 0 êmbolo de fixação (220) está situado no interior de uma caixa (230) do êmbolo. O êmbolo de fixação (220) está vedado pneumaticamente no interior da caixa do êmbolo por dois aneis toroidais periféricos (222) e (224), também representados na fig. 3. Entre os dois últimos aneis toroidais há aberturas (226) e (228) que passam através do êmbolo de fixação. Estas
aberturas passam através do êmbolo de fixação. Estas aberturas estão situadas simetricamente de modo que o êmbolo de fixação pode ser introduzido na caixa do êmbolo oom qualquer extremidade na parte inferior.
Situada na parte traseira da caixa do êmbolo há uma passagem (232). Esta passagem está ligada a um primeiro conector da união pneu mãtica em ângulo recto (150), por um segmento de tubo pneumático (34) .Junto da parte dianteira da caixa do êmbolo hã uma segunda passagem (236). Esta passagem está ligada a um segundo oanector de união (150) pelo segmento de tubo (36).
Para o lado esquerdo inferior da cabeça de agrafador (110) está a porção de veio (108) do agrafador. O veio (108) inclui duas secções (30) e (32) de tubo pneumátioo. 0 tubo na porção de veio está ligado a duas aberturas de entrada da união de ângulo recto (150) (uma das quais está representada em (54)) por acessórios pneumáticos de montagem por pressão elástica, um dos quais está representado em (106). Na extremidade traseira do veio (108), que deve ser ligada â pega (10) há urca . união pneumática paralela (120). A união pneumática (120) está ligada de maneira amoví vel ã pega por acessórios pneumáticos de ligação por pressão elástica, um dos quais está representado em (43). O tubo pneumático (30,32) está ligado a duas aberturas de saída (112) no ocnector (120).
Quando o tecido a agrafar estiver situado entre as ruhlas (200) e (204 da cabeça do agrafador e se deseja agrafar o tecido entre as maxilas prime-se o cursor (74), fluindo o gás comprimido da pega para uma das aberturas da união paralela (120). 0 gás comprimido é transportado atra vés da secção de tubo (30) na porção de veio (108) e através da passagem da união em ângulo recto (150). O gás ocsiprimido passa através do segmento de tubo (34) e da passagem (232) para a parte traseira do êmbolo de fixação ( (220). AÍ o gás ê forçado para a interface (240) entre o êmbolo de fixação e a caixa do êmbolo, onde ele se expande e empurra o êmbolo de fixação para a frente como se indica ocm a seta (280) no sentido da maxila fixa. Enquanto o êmbolo de fixação se move, ele transporta o êmbolo de cravação, o impulsor dos agrafos e o cartucho de agrafos consigo isso fixará o tecido entre o cartucho de agrafos e a maxila fixa (204). O êmbolo de fixação (220) está agora numa posição na caixa (230) tal que a passagem; dianteira (236) se opõe ã abertura (228) do êmbolo de fixação ccr.io se indica a tracejado na fig.

O alinhamento da passagem (236) cctn qualquer porção da ranhura permite desse modo o acoplamento pneumático da passagem (236) ccm o espaço na parte traseira do êmbolo de cravação.
Con o tecido firmente fixado entre as maxilas, o utilizador desloca e roda o dispositivo de segurança do gatilho e empurra o gatilho para implantar os agrafos. O gás comprimido flui através da união paralela (120), do tubo pneumático (32), co ccnector (150) e do segmento de tubo (36) para a passagem (236). 0 gás comprimido flui depois através da abertura (228) no êmbolo de fixação e para o espaço (242) na parte traseira do êmbolo de cravação. 0 gás que se expande neste espaço empurra o êmbolo de cravação (214) para a frente contra a parte traseira do impulsor dos agrafos (210) como se indica pela seta (282), de modo que se aplica uma pressão uniforme no impulsor e nos seus dedos. Os dedos cravam então os agrafos que saem das bolsas do cartucho de agrafos, através do tecido e contra a bigorna ou para o interior dos receptores da maxila fixa. Quando se liberta o gatilho, o gás coiprimido fornecido ao êmbolo de cravação e ao êmbolo de fixação escapa-se através da pega, libertando a maxila (200) do tecido agrafado.
Descreve-se um sistema de regulação da pressão pneumática apropriado para o fornecimento de gás ccm pressão regulada para o funcionamento da cabeça de agrafador segundo a presente invenção no pedido de patente americano «9 (ΚΓΗ-742), depositado simultaneamente e intitulado REGULADOR DA PRESSÃO DE AGRAFADOR CIRORGÍCO.
Verificou-se que quando se usam êmbolos de forma rectangular para os êmbolos de fixação e de cravação, os êmbolos tendem a fixar-se quando se movem relativamente à caixa e entre si. Também, as vedações de aneis toroidais nos cantos nos êmbolos rectangulares não proporcionam uma vedação pneumática apropriada. Por conseguinte, as extremidades dos êmbolos e da caixa são arredondadas tomando uma forma oval, como se mostra era (244) e (246) na fig. 4. Quando a ampola de fixação (252) ê infla da quando se prime o cursor (72) ela expande-se e empurra o êmbolo de fixação (220) para a maxila fixa (204) para fixar o tecido entre as maxilas. A Utilização das ampolas evita a necessidade de aneis de ve dação toroidais em tomo dos êmbolos, os quais não têm caracteristicas pneumáticas nesta forma de realização, Esta forma de realização permite a utilização de êmbolos ccmpletamente rectangulares, pois podem desprezar-se as tolerâncias entre os êmbolos não pneumáticos para impedir a prisão dos êmbolos. A utilização dos actuadores de ampolas também proporciona a esta forne. de realização um dispositivo de segurança inerente. No caso de inadvertidamente as ampolas sofrerem uma scbrepressão, elas rebentam no interior da caixa e dos êmbolos. A força desta ocorrência ficará ccaifinada ao interior da caixa da cabeça do agrafador e os gases escapar-se-ão sem perigo através das interfaces entre os. êmbolos e a caixa, devido âs tolerâncias admitidas entre os mesmos. Além disso, esta segunda forma de realização é susceptível de agrafar tecidos ccm uma grande variedade de espessuras, pois o segmento de tubo (238) pode mover-se livremente em sincronismo ccm os dois êmbolos numa ampla gama de posições de fecho quando os êmbolos se deslocam durante o processo de fixação.
Claims (2)
- 'Agrafador cirúrgicojconstituído por: um dispositivo (200,204) para fixar um tecido um dispositivo (210,212) para implantar agrafos (202) no tecido fixado;um dispositivo (250, 220, 252, 214) para activar o referido dispositivo de fixação (200, 204) e o referido dispositivo (210, 212) para implantar agrafos;caracterizado pelo facto de dispor de uma fonte de gás pressurizado; e pelo facto de o referido dispositivo de fixação (200, 204) e o referido dispositivo (210, 212) de_implantação de agrafos serem individualmente activados por insuflação de uma vesícula (250, 252) com gás_pressurizado proveniente da referida fonte.I — 4
