DESCRIÇÃO
Enxerto ósseo radiotransparente
ANTECEDENTES DO INVENTO
Este invento refere-se, na generalidade, a melhorias em enxertos ósseos, tais como gaiolas de fusão vertebral do tipo concebido para implantação humana entre vértebras adjacentes da coluna vertebral, para manter as vértebras numa relação afastada substancialmente fixa, enquanto promovem a incorporação óssea intercorporal e a fusão entre si. Mais particularmente, esto invento refere-se a um enxerto ósseo implantável, tal como uma gaiola de fusão vertebral tendo uma melhor combinação de maior resistência mecânica conjuntamente com propriedades osteoindutoras e osteocondutoras, num dispositivo à base de cerâmica que, adicionalmente e vantajosamente promovem a radiotransparência para uma monitorização pós-operatória mais facilitada. 0 documento US-A-6,149,688 representa a arte anterior mais próxima.
Os enxertos ósseos implantáveis intercorpóreos, tais como os dispositivos de fusão vertebral, são conhecidos na arte e são rotineiramente utilizados pelos cirurgiões da coluna para manter as vértebras adjacentes numa relação afastada desejada enquanto a incorporação óssea intercorporal e a fusão têm lugar. Estes dispositivos de fusão vertebral também são usados para proporcionar apoio de peso entre corpos vertebrais adjacentes e, assim, corrigir problemas clinicos. Tais dispositivos de fusão vertebral são indicados para o tratamento médico de doenças degenerativas do disco, lombalgia discogénica 1 e espondilolistese degenerativa. Estas situações foram tratados utilizando-se estruturas, tipicamente feitas a partir de metais tais como o titânio ou ligas de cobalto-cromo, como as utilizadas em implantes ortopédicos e homotransplante (doador) ou autoplastia (paciente) de osso para promover a incorporação óssea e a fusão.
Os dispositivos tipicos de fusão vertebral intercorporal, tal como, por exemplo, pastilhas (plugs) , têm espaços ocos ou abertos que são geralmente cheios com material de enxerto ósseo, quer material ósseo autogéneo fornecida pelo paciente, quer material ósseo alógeno fornecido por um dador terceiro. Esses dispositivos também possuem ranhuras ou aberturas laterais que são usadas principalmente para promover o crescimento interno de fornecimento de sangue e o crescimento activo de osso vivo. Estes implantes podem também ter uma superfície exterior padronizada, tal como uma superfície serrilhada ou estriada ou um fio de rosca para se conseguir um melhor interligação mecânica entre vértebras adjacentes, com um risco minimo de desalojamento do implante do local. Ver, por exemplo, a patente dos EUA 5,785,710 e 5,702,453. Os materiais tipicos de construção de tais dispositivos de fusão vertebral intercorporal incluem polímeros reforçados com fibra de carbono biocompativel, ligas de cobalto-cromo, e aços inoxidáveis ou ligas de titânio. Ver, por exemplo, a Patente dos EUA 5,425,772. A maioria dos implantes de fusão vertebral do estado da arte são feitos em liga de titânio e osso de homotransplante, e têm desfrutado de sucesso clinico, bem como utilização rápida e generalizada devido a melhores resultados para os pacientes. No entanto, os dispositivos de implantes à base de titânio apresentam caracteristicas de radiotransparência pobres, 2 apresentando dificuldades na monitorização e avaliação pós-operatória do processo de fusão, devido à rádio-sombra produzido pelo metal não transparente. Também há evidência clinica de subsidência e colapso do osso que se crê ser atribuível a incompatibilidade mecânica entre o osso natural e o material do implante metálico. Além disso, os dispositivos de implante à base de titânio são principalmente resistentes, mas não são osteocondutores, ou seja, não contribuem para a fixação mecânica directa e forte ao tecido ósseo do paciente, levando a uma potencial necrose do enxerto, e a fusão e estabilidade pobres. Em contraste, os implantes de homotransplantes ósseos apresentam boas propriedades osteocondutoras, mas que podem diminuir ao longo do tempo à medida que assimilam no osso natural. Além disso, sofrem de uma pobre resistência de remoção, resultando numa estabilidade pobre, principalmente devido às opções limitadas na maquinagem das superfícies de contacto. Os implantes de homotransplantes ósseos também têm propriedades de materiais variáveis e, talvez o mais importante de tudo, têm uma oferta muito limitada. Um risco pequeno, mas limitado, de transmissão de doenças com o homotransplante ósseo é também um factor. Em resposta a estes problemas, alguns desenvolvedores estão a tentar usar escoras porosas em metal à base de tântalo, mas estes apresentaram um sucesso muito limitado, devido aos módulos elásticos pobres dos metais porosos.
Um dispositivo de fusão vertebral típico em liga de titânio é construído a partir de uma estrutura do tipo gaiola em metal, roscada externamente, cilíndrica e oca, com fenestrações que permitem a comunicação do tecido hospedeiro esponjoso com o núcleo oco, que é envolvido em material de enxerto ósseo em pedaços. Esta concepção, limitada pelas 3 propriedades de materiais das ligas de titânio, assenta na incorporação óssea para as fenestrações induzidas pelo material de enxerto ósseo. No entanto, a estrutura à base de titânio pode formar uma camada fibrosa fina na interface osso/metal, o que degrada a fixação do osso no metal. Além disso, o núcleo oco no qual o material para enxerto é envolvido, pode ter transmissão de tensão vascularização sub-óptimas conduzindo assim, eventualmente, à falha na incorporação do enxerto. A estabilidade mecânica, a transmissão de tensão do fluido, e a presença de agentes osteoindutores são necessárias para estimular o crescimento interno de gomos vasculares e proliferar as células mesenquimáticas do tecido hospedeiro esponjoso para o material de enxerto. No entanto, a maioria dos dispositivos de fusão vertebral à base de titânio usados hoje em dia têm tampas de extremidade ou nas paredes laterais sólidas para impedir a saída do enxerto para fora do núcleo e a entrada de tecido remanescente do disco e fibroblastos para o núcleo. A fusão óssea autóloga (doente) tem sido utilizada no passado, e tem uma mistura teoricamente ideal de propriedades osteocondutoras e osteoindutoras. No entanto, o fornecimento de material ósseo autólogo é limitado e sabe-se da ocorrência de complicações significativas devido à colheita de osso. Além disso, os custos associados à colheita de material ósseo autólogo são elevados, requerendo duas incisões separadas, o paciente tendo de sofrer mais dor e recuperação devido aos processos de recolha e implantação. Além disso, o material ósseo esponjoso autólogo tem uma resistência mecânica inadequada para suportas por si só as forças intervertebrais, pelo que o material ósseo é normalmente incorporado com uma estrutura à base de metal. 4
Os materiais cerâmicos proporcionam potenciais estruturas alternativas para utilização em dispositivos de implante de fusão vertebral e próteses, ver por exemplo a Patente dos EUA 5,871,517, que descreve uma prótese da articulação da anca que tem uma cabeça em zircónio e uma taça em cerâmica, e a Patente dos EUA 4,000,525, que descreve uma prótese em cerâmica de óxido de aluminio de alta densidade. A este respeito, têm sido propostas estruturas monolíticas em cerâmica, feitas a partir de materiais convencionais, tais como hidroxiapatite (HAP) e/ou fosfato de tricálcio (TCP). Ver, por exemplo, a Patente dos EUA 6,037,519. No entanto, embora estes materiais cerâmicos possam proporcionar propriedades osteocondutoras osteoindutoras satisfatórias, não proporcionam a necessária resistência mecânica para o implante.
Assim, existe uma necessidade siqnificativa de outras melhorias no, e para o desenho de enxertos ósseos, tais como dispositivos de implante de fusão vertebral, em particular para proporcionar um implante de elevada resistência tendo características elevadas de incorporação óssea e de fusão, em conjunto com uma radiotransparência substancial para uma monitorização pós-operatória eficaz e facilitada.
Portanto, um objecto do presente invento consiste em proporcionar um melhor enxerto ósseo, tal como um implante de fusão vertebral intercorporal ou uma qaiola feita a partir de uma estrutura de poros abertos biocompatíveis, que tenha uma radiotransparência semelhante à do osso circundante. É também um objecto do presente invento proporcionar um substrato de elevada resistência biomecânica para suportar aqentes biolóqicos que promovem a incorporação óssea intervertebral, a cura e a fusão. É ainda um outro objectivo do presente invento proporcionar um dispositivo de fusão intervertebral que possua 5 propriedades mecânicas que correspondem substancialmente à do osso natural, usando-se materiais de estrutura em cerâmica, em vez do metal.
RESUMO DO INVENTO
As caracteristicas do invento estão definidas na reivindicação 1. De acordo com as reivindicações, portanto, É proporcionado um enxerto ósseo que compreende um bloco de substrato em cerâmica formado a partir de um material com uma resistência à flexão superior a cerca de 500 mPa, e uma tenacidade à fractura superior a cerca de 5 mPaeirrp O dito enxerto ósseo pode compreender uma gaiola de fusão vertebral. Mais preferencialmente, a composição de bloco de substrato pode compreender cerca de 10% a cerca de 20% ou 25%, em volume, de zircónia na alumina. O dito bloco de substrato em cerâmica pode ter uma primeira zona de menor porosidade e uma segunda zona de maior 7 porosidade, no qual a dita segunda zona de maior porosidade está configurada para facilitar a incorporação óssea e a ligação por fusão a um osso adjacente do paciente. Numa forma preferida, a porosidade do bloco de substrato pode ser graduada de uma primeira zona com uma porosidade relativamente baixa emulando ou mimicando a porosidade do osso cortical, até uma segunda zona de porosidade relativamente, maior emulando ou mimicando a porosidade do osso esponjoso. A dita primeira zona pode ter uma porosidade inferior a cerca de 5%. A dita segunda zona pode ter uma porosidade que varia de cerca de 30% a cerca de 80%. A dita segunda zona pode ter uma porosidade superior a cerca de 40%, de preferência superior a cerca de 45% e, mais preferencialmente, superior a cerca de 50%.
Nalgumas formas de realização, o dito enxerto ósseo pode compreender ainda um revestimento de superfície aplicado ao bloco de substrato, o dito revestimento de superfície tendo propriedades osteocondutoras e osteoindutoras para promover a incorporação óssea e ligação por fusão a um osso adjacente do paciente. O dito revestimento de superfície pode ser aplicado internamente e externamente ao bloco de substrato de cerâmica porosa. Tipicamente, o dito revestimento de superfície pode compreender um revestimento de superfície bioactivo e reabsorvível. O bloco de substrato poroso pode, consequentemente, ser revestido interna e externamente com um material de revestimento de superfície bioactivo seleccionado para propriedades osteocondutoras e osteoindutoras relativamente elevadas, como por exemplo material de hidroxiapatite ou fosfato de cálcio.

Formas preferidas incluem a forma cilíndrica ou parcialmente cilíndrica com extremidades opostas substancialmente planas e uma secção transversal cónica ou lordótica para se adequar à curvatura necessária do espaço intervertebral, no caso de um dispositivo de fusão vertebral. Tipicamente, o dito bloco de substrato pode ainda incluir meios para preensão e manipulação facilitadas com um instrumento cirúrgico para implantação. 0 enxerto ósseo pode incluir, desejavelmente, uma extremidade posterior tendo meios de engate, tal como um caixilho roscado para o engate amovível com uma ferramenta de inserção adequada. Outras configurações alternativas de enxertos ósseos podem incluir gradações de porosidade controladas para definir uma primeira zona com porosidade relativamente baixa ou zonas que emulam substancialmente o osso cortical, para definir uma zona de suporte de carga de elevada resistência ou escora para absorção de carga de impacto e de inserção, em combinação com um ou mais segundas zonas com uma porosidade relativamente elevada, emulando substancialmente o osso esponjoso para entrar em contacto com osso o adjacente do paciente para uma melhor fusão e incorporação ósseas. Em algumas formas de realização, a dita primeira zona pode compreender pelo menos uma escora estrutural de suporte de carga que se estende através do bloco de substrato. A dita pelo menos uma escora pode mimetizar substancialmente as características estruturais do osso natural; a dita pelo menos uma escora pode ser formada a partir de um material cerâmico poroso.
De preferência, a dita primeira zona com menor porosidade pode ser geralmente disposta sobre superfícies anterior e posterior do bloco de substrato e podem anda definir pelo menos 11 uma escora de suporte de carga estrutural que se estende através do dito bloco de substrato entre as ditas superfícies anterior e posterior. A dita segunda zona pode incluir uma área de superfície exposta estendida para entrar em contacto com o osso adjacente. DESCRIÇÃO DETALHADA DE FORMAS DE REALIZAÇÃO PREFERIDAS
Tal como está ilustrado nos desenhos exemplificativos, um enxerto ósseo radiotransparente, tal como uma gaiola de fusão vertebral, genericamente identificado nas Figuras 1-3 pelo número de referência 10, é proporcionado para a implantação encaixada entre um par de ossos adjacentes de um paciente, tal vértebras da coluna vertebral 12 (Fig. 3) para manter as vértebras numa relação afastada enquanto promove a incorporação óssea e a fusão. Em geral, o enxerto ósseo 10 melhorado compreende um substrato de cerâmica biocompativel tendo uma construção porosa para definir uma estrutura aberta propicia à incorporação óssea e à fusão, enquanto proporciona uma forte estrutura de suporte de carga mecânica análoga às propriedades 14 de suporte de cargas do osso cortical e esponjoso. Este substrato em cerâmica de células abertas é revestido interna e externamente com um revestimento de superfície bioactivo seleccionado para propriedades osteocondutoras e osteoindutoras relativamente fortes, pelo que o substrato de cerâmica revestido proporciona um suporte propício à fixação e proliferação de células para promover a incorporação óssea intercorporal e a fixação por fusão. 0 substrato de cerâmica pode também transportar um ou mais agentes terapêuticos seleccionados, adequados para a reparação óssea, aumento e outras utilizações ortopédicas.
As Figs. 1-3 ilustram o enxerto ósseo melhorado, sob a forma de uma melhor gaiola de fusão vertebral 10 de acordo com uma forma de realização preferida, na forma de um corpo geralmente cilíndrico tendo nervuras externas aí formadas na forma de um fio de rosca 14. As extremidades opostas deste corpo cilíndrico são geralmente planas, com uma extremidade posterior 16 ilustrada para incluir uma ranhura superficial que se estende diametralmente 18 em combinação com um caixilho roscado centrado axialmente e aberto para trás 20 para engate amovível com uma ferramenta de inserção 22 (Figs. 2-3), tal como será descrito em mais detalhe. 0 corpo cilíndrico está ainda ilustrado de modo a incluir um recesso ou orifício aberto lateralmente 24 para receber e suportar material de enxerto ósseo em pedaços 26 (Fig. 2), tal como material ósseo de homotransplante de um dador terceiro, ou material ósseo autólogo do paciente. A composição preferida do substrato cerâmico compreende um bloco de substrato cerâmico com uma resistência relativamente elevada. De acordo com uma forma preferida do invento, este bloco de substrato compreende uma composição de alumina- 15 zircónia de duas fases tendo uma porosidade controlada e tendo uma dimensão e forma adequadas para o implante encaixado, tal como no espaço intervertebral no caso da gaiola de fusão vertebral 10. Numa forma preferida, a composição compreende uma composição de alumina-zircónia com uma relação entre alumina e zircónia variando entre cerca de 100% (ou seja, 2:1) e cerca de 25% (ou seja, 1:4). Mais preferencialmente, a composição de substrato compreende cerca de 10% a cerca de 20% em volume de zircónia na alumina. Quer zircónia estabilizada com itria (cerca de 2,5 a cerca de 5% em mol de itria na zircónia) ou zircónia estabilizada com céria (cerca de 2,5 a cerca de 15% em mol de céria em zircónia) é preferencialmente utilizada para a fase de zircónia. A composição de alumina-zircónia é processada para proporcionar uma distribuição homogénea das duas fases, e as temperaturas de sinterização são controladas para fornecer uma dimensão de partícula de cerca de 0,5 micrómetro ou menos, no estado sinterizado. O bloco de substrato cerâmico resultante tem uma porosidade que varia desde cerca de 2% a cerca de 80% em volume, e de preferência entre cerca de 10% a cerca de 50% em volume, com dimensões de poros que variam entre cerca de 5 micrómetro a cerca de 1500 micrómetro, e de preferência entre cerca de 100 a cerca de 500 micrómetro. Na forma preferida, os poros são organizados para a continuidade de fluido entre os mesmos, e com uma superfície exterior poroso rugosa com uma área de superfície grande ou estendida. Além disso, na forma preferida, os poros estão dispostos com um gradiente de porosidade variável para definir uma primeira zona com porosidade relativamente baixa ou reduzida (inferior a cerca de 5%), mimicando a estrutura do osso cortical, e uma segunda zona de porosidade relativamente grande ou maior (variando entre 16 cerca de 30% a cerca de 80%) , que imita a estrutura do osso esponjoso. Numa configuração preferida, as superficies exteriores ou externas do bloco de substrato reticulado compreendem a primeira zona ou zona de baixa porosidade para melhorar a capacidade de suporte de carga, enquanto que as superficies interiores do bloco de substrato compreendem o a segunda zona ou zona de elevada porosidade mimicando o osso esponjoso, para melhorar a incorporação óssea e a fusão. Este material utilizado para formar o bloco de substrato apresenta uma combinação substancialmente óptima de resistência à flexão superior a cerca de 500 mPa, e uma tenacidade à fractura superior a cerca de 5 mPa^m15. Estas resistências são medidas da mesma forma que as barras de dobragem padrão e amostras de viga ranhurada com um bordo único (SENB) espécimes individuais, pelos procedimentos ASTM E-1304 e C-1162. A superfície do bloco de substrato de cerâmica de elevada resistência é revestida internamente e externamente com um material de revestimento orgânico ou inorgânico bioactivo seleccionado para propriedades osteocondutor e osteoindutoras relativamente fortes, para proporcionar um ambiente rico em nutrientes para a actividade celular para promover a incorporação óssea intercorporal e a ligação por fusão. Os materiais de revestimento de superfície preferidos compreendem um material reabsorvivel, como a hidroxiapatite, ou uma cerâmica de fosfato de cálcio. Outros materiais vitreos (amorfos) alternativos tendo uma composição relativamente rica em cálcio e fosfato de cálcio podem também ser utilizados, particularmente quando estes materiais incorporam cálcio e fosfato, numa proporção semelhante ao osso natural ou hidroxiapatite. Tais composições vítreas pode compreender um material osteoindutor parcialmente ou totalmente amorfo, 17 compreendendo um composto de um vidro e composto de cálcio osteoindutor, com uma composição que varia entre cerca de 100% de vidro a 100% de composto de cálcio osteoindutor. O revestimento de superfície pode também compreender aspirados de medula óssea autólogos. O enxerto ósseo resultante 10 compreende, assim, o bloco de substrato formado a partir de material de cerâmica de elevada resistência, com propriedades bio-miméticas e que é não-reabsorvível, ou lentamente ou muito lentamente reabsorvível quando implantado no paciente, em combinação com o revestimento de superfície bioactivo, que é, comparativamente, reabsorvível de forma relativamente rápida para promover uma actividade de incorporação óssea rápida e vigorosa. O bloco de substrato também pode ser, vantajosamente, revestido ou impregnado com um ou mais agentes terapêuticos seleccionados, por exemplo, tal como factores de crescimento autólogos, sintético ou derivados de células estaminais, ou proteínas e factores de crescimento tais como a proteína morfogénica óssea (BMP), ou um seu precursor, o que promove ainda mais a cura, fusão e crescimento. Os agentes terapêuticos alternativos podem também incluir um antibiótico, ou agentes terapêuticos naturais, tais como aspirados de medula óssea, e factores de crescimento ou células progenitoras, tais como células estaminais mesenquimáticas, células hematopoiéticas e células estaminais embrionárias, quer sozinhas quer como uma combinação de diferentes agentes benéfico.
Mais particularmente, um tal agente ou agentes terapêuticos podem ser aplicados ao enxerto ósseo 10 substancialmente no momento de, ou no decurso de, um processo de implante cirúrgico, por exemplo mergulhando o enxerto ósseo no agente terapêutico ou numa solução à base de líquido 18 contendo o agente terapêutico e, em seguida, implantando o enxerto ósseo no paciente. Num procedimento alternativo, as células progenitoras, ou semelhantes, podem ser retiradas de um paciente especifico e então cultivadas in vitro, utilizando o enxerto ósseo como um meio de cultura, e o substrato para produzir uma concentração elevado e terapeuticamente eficaz das células seleccionadas transportadas no, e dentro do enxerto ósseo. Seguidamente, o enxerto ósseo transportando as células de cultura pode ser implantado cirurgicamente. Numa configuração conveniente, o implante de enxerto ósseo pode ser formado no tamanho e forma de uma pequena esfera para o embalamento adequado de diversos implantes num local de regeneração/incorporação ósseas. 0 enxerto ósseo resultante, tal como a gaiola de fusão vertebral 10 ilustrada, apresenta uma resistência biomecânica relativamente alta, semelhante às características de suporte de carga do osso natural. Além disso, a gaiola de fusão vertebral 10 apresenta características osteocondutoras e osteoindutoras relativamente fortes, atribuíveis principalmente ao revestimento de superfície, uma vez mais similares ao osso natural. Importante, a gaiola de fusão 10 é também substancialmente radiotransparente, de modo que a gaiola de fusão não interfere com a análise radiológica pós-operatória da incorporação óssea intercorporal e da fusão. A figura 2 ilustra a gaiola de fusão vertebral 10 com o material de enxerto ósseo 26 suportado por dentro do furo aberto lateralmente 24, para se conseguir uma incorporação óssea e uma fusão ainda melhores quando o dispositivo é implantado no espaço intervertebral. A ferramenta de inserção 22 inclui uma ponta roscada 28 numa sua extremidade dianteira ou de nariz para engate roscado no casquilho roscado 20 na 19 extremidade posterior da gaiola de fusão 10. Um batente de encosto 30 é também proporcionado na extremidade dianteira da ferramenta de inserção 22 para o engate encaixado no entalhe superficial 18 na gaiola de fusão. Assim montado, a ferramenta de inserção 22 é usada por um cirurgião tal como pode ver-se na figura 3 para assentar a gaiola da fusão vertebral 10 no espaço entre um par seleccionado de vértebras adjacentes 12, com a gaiola de fusão encostando à parte substancial das placas de extremidade das duas vértebras nos lados opostos do espaço intervertebral. Este processo de inserção pode ser acompanhado pela rotação adequada da ferramenta de inserção 22, para que o encosto 30 ai accione de forma rotativa a gaiola de fusão 10 para a posição implantada desejada e encaixada de forma segura. Em seguida, o encosto 30 pode ser retraido da gaiola 10, seguido da rotação para trás da ponta roscada 28 para libertação da gaiola de fusão.
As figuras 4-6 ilustram configurações alternativas para enxertos ósseos melhorados, tais como gaiolas de fusão vertebral fabricadas de acordo com o presente invento, sendo reconhecido e entendido que o enxerto ósseo pode ser construído numa ampla variedade de diferentes dimensões e formas geométricas. A figura 4 mostra uma gaiola de fusão vertebral 110 que tem uma forma geralmente cilíndrica similar à da gaiola de fusão 10 ilustrada e descrita nas Figuras 1-3, mas em que o fio de rosca externo 14 é omitido. Tal como está ilustrado, a gaiola de fusão vertebral 110 (Fig. 4) tem uma estrutura de células abertas definida bloco de substrato cerâmico poroso com uma elevada resistência (tal como descrito anteriormente), revestida com o material de revestimento de superfície bioactivo, mas em que a superfície exterior cilíndrica é definida pela estrutura de substrato com uma reticula 20 relativamente grosseira aberta tendo uma descontinuidade de superfície suficiente, e elevada área de superfície para contacto optimizado com o osso esponjoso para se alcançar uma incorporação óssea substancialmente optimizada. A Figura 5 mostra outra gaiola de fusão vertebral alternativa 210 que compreende um bloco de substrato de cerâmica porosa de elevada resistência com a superfície revestida (formado tal como foi previamente descrito) com uma configuração de bloco genericamente rectangular, para incluir pelo menos uma superfície exterior de modo a incluir uma pluralidade de recortes 214 para o travamento mecânico seguro com os ossos adjacentes do paciente, tais como as vértebras adjacentes 12 em lados em lados opostos do espaço intervertebral. A figura 6 ilustra uma gaiola de fusão vertebral alternativa, rectangular e em forma de bloco 310, compreendendo um outro bloco de substrato de cerâmica porosa de elevada resistência com a superfície revestida (formada como previamente descrito), tendo uma dimensão em secção transversal mais estreita e uma superfície exterior definida pelo substrato com uma retícula aberta e uma textura relativamente grosseira (similar à da figura 4) . Cada uma das formas de realização representadas nas figuras 1-3 e figuras 4-6 tem uma dimensão em altura e pode ter uma forma cónica lordótica (tal como está ilustrado na figura 6) para uma melhor ajuste anatómico, por exemplo, no espaço intervertebral ou semelhantes.
As figuras 7-10 ilustram formas outras formas alternativas preferidas do invento, em que o gradiente de porosidade com o bloco de substrato cerâmico poroso de elevada resistência varia de forma controlada para proporcionar benefícios de suporte de carga desejáveis, consistentes com a incorporação óssea e a fusão. Mais particularmente, as figuras 7-8 mostram um enxerto 21 ósseo geralmente rectangular, tal como uma gaiola de fusão vertebral 410 tendo uma dimensão em altura cónica na direcção anterior-posterior. O bloco de substrato é formado com a primeira zona 40 com porosidade relativamente baixa imitando substancialmente o osso cortical para se estender através das faces anterior e posterior, e para incluir ainda pelo menos uma escora de suporte de carga 42 ilustrada nos desenhos ilustrativos para se estender centralmente numa direcção anterior-posterior no interior do corpo do bloco de substrato. O remanescente do bloco de substrato compreende a segunda porção 44 com uma porosidade relativamente elevada imitando substancialmente o osso esponjoso. A primeira zona mais dura 40 incluindo a escora central, 42 proporciona beneficamente uma estrutura de suporte de carga dura e forte, capaz de resistir a forças de impacto e de inserção no sentido anterior-posterior sem danificar o implante, enquanto que a segunda zona mais macia 44 apresenta uma área de superfície exposta e grande para incorporação óssea e fusão optimizadas com o osso adjacente do paciente. Numa aplicação da gaiola de fusão vertebral, as faces medial-lateral do implante são definidas vantajosamente pela segunda zona mais macia 44, em que estas regiões estão assim expostas a formação de imagem aos raios X medial-lateral para análise radiológica pós-operatória da interface implante/osso. Os peritos na especialidade reconhecerão e apreciarão que podem ser utilizadas configurações alternativas para a escora ou escoras de suporte de carga 42, tal como uma configuração de escora em forma de X que se estende numa direcção cranio-caudal, em combinação com, ou em substituição das faces exteriores 40 e/ou a escora central, anterior-posterior, tal como está ilustrado. 22
As figuras 9-10 mostram um outro enxerto ósseo geralmente rectangular, tal como uma gaiola de fusão vertebral 510 tendo a dimensão em altura cónica na direcção anterior-posterior. O bloco de substrato é formado com a primeira zona 40 de porosidade relativamente baixa imitando substancialmente o osso cortical para se estender circunferencialmente sobre o bloco de substrato numa relação circundante relativamente à segunda parte 44 de porosidade relativamente elevada imitando substancialmente o osso esponjoso. A primeira zona mais dura 40 proporciona beneficamente, uma vez mais uma estrutura de suporte de carga dura e forte, capaz de resistir a forças de impacto e inserção no sentido anterior-posterior, sem danos para o implante, e proporciona, adicionalmente, elevada capacidade de suporte de carga vertical. Pelo contrário, a segunda zona mais suave 44 está verticalmente exposta para apresentar uma grande área de superficie para a incorporação e fusão optimizadas com o osso adjacente do paciente. Os cantos externos do bloco de substrato podem incorporar ranhuras abertas lateralmente 46 (fig. 9) para o conveniente acoplamento com, e manipulação de, uma ferramenta cirúrgica adequada (não ilustrada).
Em ambas as formas de realização das figuras 7-10, o bloco de substrato compreende uma cerâmica porosa de elevada resistência, tal como descrito anteriormente, sendo revestida com um material de revestimento de superficie bioactivo, mais uma vez, como foi descrito anteriormente, para aumentar a incorporação óssea e a fusão. O bloco de substrato pode também incluir um ou mais agentes terapêuticos. Os peritos na especialidade reconhecerão e apreciarão que as zonas de porosidade relativamente baixa e alta, 40 e 44, ilustradas nas figuras 7-10, serão integralmente unidas por uma zona de 23 gradiente, adequada apesar de relativamente estreita, em que a porosidade transita entre as mesmas. 0 enxerto ósseo melhor, tal como a gaiola de fusão vertebral ilustrada do presente invento, compreende, assim, uma estrutura de bloco de substrato de células abertas que é revestido com um revestimento de superfície bioactivo, e tem a resistência necessária para a capacidade de suporte de carga exigida por um dispositivo de fusão. A capacidade de ser infundido com o agente de revestimento biológico apropriado confere propriedades osteocondutoras e osteoindutoras desejáveis para o dispositivo, para uma incorporação óssea intercorporal e uma fusão melhores, sem diminuição das características essenciais de suporte de carga. As características radiotransparentes do dispositivo melhorado acomoda beneficamente o exame radiológico pós-operatório para monitorar o progresso da incorporação óssea e da fusão, substancialmente sem a indesejável rádio-sombra atribuível à gaiola de fusão. Os recortes ou roscas externas formados na gaiola de fusão podem ter uma profundidade variável para permitir que a base do dispositivo contacte o osso cortical para uma óptima capacidade de suporte de carga. Além dessas vantagens, o presente invento é fácil de fabricar a um custo competitivo. 0 invento proporciona, assim, uma melhoria substancial na relação com problemas clínicos indicados para o tratamento médico de doença degenerativa do disco, lombalgia discogénica e espondilolistese. 0 enxerto ósseo do presente invento proporciona pelo menos as seguintes vantagens relativamente à técnica anterior: [a] um suporte osteocondutor poroso para melhores velocidades de fusão; 24 [b] uma superestrutura biomimética de suporte de carga proporcionando transmissão de tensão apropriada sem falha por fadiga; [c] uma estrutura e dimensão de poro adequadas para incorporação e vascularização; [d] a capacidade para absorver e reter um agente osteoindutor, tal como aspirado de medula óssea autólogo ou BMP (proteínas morfogénicas ósseas); [e] bio-inerte e bio-compativel com o tecido adjacente e seleccionado para facilitar a reabsorção; [f] fabricável e maquinável em várias formas; [g] esterilizável, e [h] baixo custo de fabrico.
Uma variedade de outras modificações e melhoramentos em, e para a gaiola de fusão vertebral do presente invento serão evidentes para os peritos na especialidade. Assim, não se pretende qualquer limitação do invento por meio da descrição precedente e dos desenhos anexos, excepto como está estabelecido nas reivindicações anexas.

- 4. Enxerto ósseo de acordo com qualquer uma das reivindicações 1-3, no qual a dita segunda zona tem uma porosidade superior a cerca de 40%.
- 5. Enxerto ósseo de acordo com qualquer uma das reivindicações 1-4, no qual a dimensão dos poros varia entre cerca de 5 micrómetros a cerca de 1,500 micrómetros. 1
- 6. Enxerto ósseo de acordo com qualquer uma das reivindicações 1-5, no qual os poros formados no interior do dito bloco de substrato estão em comunicação de fluido substancialmente aberta, suficiente para transmitir a pressão de fluido entre eles.

Enxerto ósseo de acordo com qualquer uma das reivindicações anteriores, compreendendo ainda um revestimento de superfície aplicado ao dito bloco de substrato, o dito revestimento de superfície tendo propriedades osteocondutoras e osteoindutoras para promover a incorporação óssea e ligação por fusão a um osso adjacente do paciente.

Enxerto ósseo de acordo com qualquer uma das reivindicações anteriores, no qual o dito bloco de substrato de cerâmica porosa é substancialmente 2 radiotransparente sendo visível por formação de imagem aos raios X, mas tendo uma densidade que permite a passagem dos raios X através do material de cerâmica do corpo aquando de uma formação de imagem aos raios X medial- lateral facilitando assim a avaliação pós-operatória do crescimento do osso para a zona de maior porosidade do bloco de substrato de cerâmica.
- 11. Enxerto ósseo de acordo com qualquer uma das reivindicações precedentes, no qual o dito corpo de substrato tem um furo (24) nele formado.
- 12. Enxerto ósseo de acordo com qualquer uma das reivindicações precedentes, no qual o dito enxerto ósseo compreende uma gaiola de fusão espinhal (10) para implantação entre, e fusão com vértebras adjacente.
- 13. Enxerto ósseo de acordo com a reivindicação 9, no qual o dito revestimento de superfície é aplicado interna e externamente ao dito bloco de substrato de cerâmica porosa.
- 14. Enxerto ósseo de acordo com qualquer uma das reivindicações 9 ou 13, no qual o dito revestimento de superfície é seleccionado entre a hidroxiapatite e os compostos de cálcio.
- 15. Enxerto ósseo de acordo com qualquer uma das reivindicações 9 ou 13, no qual o dito revestimento de superfície compreende um material osteoindutor 3 parcialmente ou totalmente amorfo, incluindo um vidro e um composto de cálcio osteoindutor.
- 16. Enxerto ósseo de acordo com qualquer uma das reivindicações 9 ou 13, no qual o dito revestimento de superfície compreende um material de revestimento orgânico.
- 17. Enxerto ósseo de acordo com a reivindicação 16, no qual o dito material de revestimento orgânico é seleccionado de entre o grupo constituído por aspirados de medula óssea autólogos, proteínas morfogénicas ósseas, factores de crescimento e células progenitoras, assim como suas mistura.
- 18. Enxerto ósseo de acordo com a reivindicação 17, no qual as ditas células progenitoras incluem células estaminais mesenquimáticas, células hematopoiéticas e células estaminais embrionárias.
- 19. Enxerto ósseo de acordo com qualquer uma das reivindicações precedentes, incluindo ainda um agente terapêutico suportado pelo dito bloco de substrato.
