ΕΡ 1 980 231 /PT DESCRIÇÃO "Dispositivo de compressão com costura de soldadura para transferência de humidade"
CAMPO DO INVENTO O presente invento dirige-se em geral a um dispositivo de compressão para aplicar terapia de compressão a uma parte do corpo de um utilizador, mais em particular uma manga de compressão.
ANTECEDENTES DO INVENTO
Uma preocupação principal com os pacientes imóveis e as pessoas do género consiste nas condições médicas que formam coágulos no sangue, tais como, tromboses venosas profundas (DVT) e edemas periféricos. Tais pacientes e pessoas incluem aqueles que são sujeitos a cirurgia, anestesia, períodos de descanso prolongado na cama, etc. Estas condições de coagulação do sangue ocorrem em geral nas veias profundas das extremidades inferiores e/ou da pélvis. Estas veias, tal como a ilíaca, a femoral, a popliteal e a tibial, devolvem o sangue desoxigenado ao coração. Por exemplo, quando a circulação do sangue nestas veias é retardada devido a doença, ferimentos ou inactividade, existe uma tendência para o sangue se acumular ou fazer poças. Uma poça estática de sangue pode dar origem à formação de um coágulo de sangue. Um risco importante associado a esta condição é a interferência com a circulação cardiovascular. Mais sério ainda, um fragmento do coágulo de sangue pode romper e soltar-se e migrar. Pode formar-se uma embolia pulmonar a partir do fragmento que bloqueia potencialmente uma artéria pulmonar principal, o que pode ser uma ameaça à vida. O presente invento também pode ser aplicado ao tratamento da limfoedema.
As condições e os riscos resultantes associados à imobilidade do paciente podem ser controlados ou aliviados ao aplicar pressão intermitente a um membro do paciente, tal como, por exemplo, uma perna, de modo a ajudar na circulação do sangue. Por exemplo, foram utilizados dispositivos de 2
ΕΡ 1 980 231 /PT compressão sequenciais, tais como o dispositivo descrito na Patente U.S. N.° 4 091 864 de Hasty. Os dispositivos de compressão sequenciais são tipicamente construídos por duas folhas de material conjuntamente seguras nas costuras de modo a definir uma ou mais bolsas impermeáveis ao fluido, as quais são ligadas a uma fonte de pressão para aplicar pressão sequencial em torno das partes de corpo do paciente para melhorar o retorno do sangue para o coração. As secções infláveis são cobertas com um laminado para melhorar a durabilidade e proteger contra a perfuração. Como parte do dispositivo de compressão, as duas folhas são estruturalmente concebidas para resistirem a uma mudança da pressão ao longo do tempo de acordo com uma utilização repetida. A impermeabilidade da manga torna-a desconfortável para o paciente devido à humidade (isto é, transpiração) ser apanhada entre a folha impermeável e a parte de corpo do paciente. Isto conduz ao desagrado do paciente em vestir a manga, pondo por isso em perigo a saúde do paciente. Além do mais, em geral a manga não se pode esticar e é volumosa uma vez que as bolsas têm de poder reter uma quantidade significativa de pressão de fluido durante o tratamento. Assim, as mangas da arte anterior restringem a mobilidade do paciente. Além disso, pode dar-se a fricção de um membro do paciente devido às concepções da arte anterior reterem as bolsas infláveis numa posição fixa quando estão sob pressão. Dado que a pressão muda durante o tratamento, as bolsas pressionam e soltam novamente o membro do paciente, esfregando e friccionando a pele. Uma bolsa pode enrugar ou dobrar, o que pode provocar mais irritações durante um ciclo de compressão. A construção final de uma manga da arte anterior é volumosa, rígida e pode dar a sensação de ser pesada para uma pessoa ao longo de um período de utilização extenso. O presente invento dirige-se à resolução das deficiências acima mencionadas sem comprometer a durabilidade e a eficácia clínica.
Tal como enunciado acima, os dispositivos da arte anterior são construídos para durarem e serem resistentes. Tal como mostrado na Publicação de Patente U.S. N.° 2005/0187503 AI de Tordella, Tordella descreve uma manga com uma folha de topo e de fundo. As folhas são fixas no 3
ΕΡ 1 980 231 /PT perímetro para formar uma secção inflável ou bolsa, tal como mostrado na FIG. 2. O material que forma as câmaras ou as bolsas é poli(cloreto de vinilo) ou polietileno. Estes materiais são impermeáveis à humidade dado que precisam de ser estanques a fluidos e suficientemente grossos para resistirem a milhares de ciclos de compressão sem rebentarem. Tordella proporciona algum arrefecimento quando o dispositivo proporciona orifícios de ventilação colocados em torno da manga. Além disso, encontra-se introduzida uma fenda através das folhas, mas a fenda de Tordella não se encontra dentro da área definida pelas câmaras (isto é, bolsas). Em geral, o acesso à pele irá proporcionar a evaporação dos fluidos corporais recolhidos nas aberturas, mas o invento de Tordella não proporciona a remoção do fluido apanhado por baixo da folha impermeável para fora das aberturas. A evaporação está limitada às aberturas e a área imediata por baixo da folha impermeável perto da abertura. Pelo menos algumas das concretizações do presente invento proporcionam uma solução para o problema do fluido apanhado ao mover o fluido por debaixo da folha impermeável, a uma velocidade suficiente, para uma pluralidade de aberturas posicionadas, dimensionadas e conformadas para manter o escoamento do sangue e evaporar a humidade tal como descrito abaixo. A construção da manga de Tordella é similar ao dispositivo do Modelo 9529 SCD Expresso (manga com comprimento de joelho) disponível nos Estados Unidos a partir da Tyco Healthcare Group L.P., a qual é discutida em maior detalhe abaixo.
Existem outras tentativas da arte anterior para aperfeiçoar o conforto através da capacidade de respiração e de evaporação. Gillis et al. (US 2005/0187499 AI) descrevem, por exemplo, um dispositivo de acordo com o preâmbulo da reivindicação 1. O dispositivo da arte anterior é um aparelho de compressão com uma bolsa inflável configurada para estimular artificialmente os vasos sanguíneos do pé ou outro membro de um corpo, de modo a impedir e ultrapassar os riscos associados à imobilidade do paciente (por exemplo, edema pulmonar) . Uma manga de pé inclui por exemplo uma camada de contacto com o pé configurada para engatar no pé para aplicar pressão ao mesmo. É contemplado que a camada de contacto com o pé pode ser configurada para absorver por capilaridade fluidos tal como humidade e transpiração a partir de uma 4 ΕΡ 1 980 231 /PT superfície exterior do pé. A camada de contacto com o pé pode ser tratada quimicamente para melhorar o efeito de absorção por capilaridade. Uma camada de bolsa superior e uma camada de bolsa inferior encontram-se unidas de modo fixo através de soldadura em costuras ao longo dos seus perímetros para definir uma bolsa inflável. A patente U.S. N.° 3 824 992 de Nicholas dirige-se a um artigo que proporciona pressão pulsatória a uma extremidade inferior. Encontra-se colocado um número de orifícios na área da biqueira. O ar que entra nos orifícios é puxado através da pele do paciente através de um espaço de ar proporcionado pelo dispositivo quando vestido. Nicholas tem um invólucro externo rígido. O dispositivo de Nicholas carece de um número de desvantagens que não se encontram no presente invento. As mangas de compressão de, pelo menos, algumas concretizações do presente invento, são elásticas na camada interna e na camada externa, para aperfeiçoar a mobilidade e a flexibilidade do paciente. Em vez de um invólucro externo rígido como o de Nicholas, o presente invento tem em algumas concretizações uma cobertura externa elástica e mole, permeável ao ar. A cobertura externa elástica do presente invento ajuda a manga a conformar-se ao membro sob pressão. O presente invento não tem a estrutura para um canal na pele para mover o ar através da pele e para o meio ambiente.
Hasty (Patente U.S. N.° 4 091 804) e Annis (Patente U.S. N.° 4 207 876) descreve uma pluralidade de aberturas em comunicação com um canal de ventilação. O ar é forçado através do canal e das aberturas para cima da pele através de um compressor. O presente invento não utiliza um canal de ventilação dentro das camadas da manga. Além do mais, em concretizações preferidas do presente invento, a manga de compressão não utiliza o seu compressor para forçar o ar através das aberturas para cima da pele através do canal. Nas concretizações do presente invento, o ar nas aberturas faz interface com o material de absorção por capilaridade para evaporar a humidade absorvida por capilaridade tal como descrito mais completamente abaixo. O mecanismo de transporte pode ser o material de absorção por capilaridade no presente invento. Outros dispositivos tais como o de Jacobs (Patente U.S. N.° 5 489 259), proporcionam o acesso directo a uma porção do membro do paciente, mas o dispositivo de Jacobs tem 5 ΕΡ 1 980 231 /PT um problema pelo facto de o arrefecimento (evaporação) estar limitado às aberturas localizadas. A referência a Neal (Patente U.S. N.° 5 695 453), descreve aberturas de várias geometrias, mas o tamanho, forma e distribuição é uma questão de conveniência de utilização. O dispositivo de Neal não se dirige ao tratamento profiláctico. A capacidade de permeabilidade ao ar está associada ao arrefecimento através da evaporação, dado que o ar tem de ter capacidade para passar sobre as aberturas até à pele. Pode ocorrer uma evaporação mais rápida se um dispositivo puder ser permeável ao ar através da sua camada externa, o que é um problema não resolvido nas referências citadas. Um número das referências citadas menciona a permeabilidade ao ar para evitar o aumento do suor, mas nenhuma das referências se dirige ao proporcionar de um tratamento profiláctico pela utilização da compressão sequencial. Um dispositivo de Hall (Patente U.S. N.° 6 520 926) descreve uma meia de suporte que é permeável ao ar, mas Hall não proporciona detalhes adicionais acerca do modo como se torna permeável ao ar. Um dispositivo de Roth (Patente U.S. N.° 7 044 924) descreve que vários orifícios dimensionados podem ser puncionados quer através da folha interna quer através da folha externa 202/204, entre as costuras adjacentes 234 ou 242, para permitir a ventilação. Para além disso, pode ser aplicado um material de revestimento que absorve humidade por capilaridade à superfície da folha interna 204 por razões de conforto. As costuras laterais 230, 232 e 234 e as costuras longitudinais 238 e 240 formam uma pluralidade de bolsas infláveis 250. Os requerentes adaptaram a sua folha interna para proporcionar propriedades de absorção por capilaridade uma vez que os requerentes descobriram que a laminagem ou a aplicação do material de absorção por capilaridade a uma folha pode compreender a capacidade de absorção por capilaridade do material. O dispositivo do requerente foi demonstrado por manter a eficácia clinica tal como abaixo descrito. Roth não proporciona qualquer informação no que diz respeito à eficácia clínica do seu dispositivo e não proporciona quaisquer figuras que mostrem as suas aberturas ou o seu material de absorção por capilaridade. Um dispositivo de meia de Linnane (Publicação da Patente U.S. N.° 2006/0010574), descreve uma meia de compressão com um material de absorção por capilaridade perto da pele da pessoa para absorver humidade por capilaridade ao longo dos canais para o lado de fora da meia. O presente invento dirige a humidade para uma pluralidade de aberturas dimensionadas, conformadas e localizadas ao longo do dispositivo de compressão para maximizar a evaporação enquanto se mantém a eficácia clínica. A elasticidade encontra-se na arte anterior e entende-se em geral como sendo um importante beneficio para meias de compressão tais como as T.E.D®, vendidas pela cessionária do presente invento. Uma desvantagem dos dispositivos de compressão sequenciais da arte anterior, como os mostrados em Hasty, é que o material de bolsa é flexível mas não elástico. As bolsas da arte anterior são formadas como parte de uma construção laminada que adiciona mais rigidez e durabilidade. A referência de Tordella descreve uma manga com secções elásticas flexíveis entre as secções ou porções infláveis para facilitar a mobilidade de um paciente. Tordella não descreve uma concepção elástica circunferencial e longitudinalmente ao longo de todo o comprimento das mangas, o que é solucionado pelo presente invento. O presente invento ajuda a ultrapassar o desconforto do paciente sem diminuir a eficácia clínica, tal como mostrado em testes de laboratório de suporte descritos neste pedido. Um objectivo importante consiste em melhorar a adaptação ao paciente, definida como utilizando a manga tal como prescrita por um médico. Existe uma correlação directa da adaptação do paciente com o conforto do paciente. A adaptação aos 7 ΕΡ 1 980 231 /PT dispositivos de compressão mecânica sempre foi uma preocupação em cuidados de saúde. O pessoal clinico está sobrecarregado com trabalho com os pacientes e deveres e, portanto, o tempo de prestação de cuidados a pacientes um a um fica caro. Foi muitas vezes reportado que os pacientes ficavam desconfortáveis ao usarem mangas de compressão, tendo solicitado que as mangas fossem retiradas, mesmo sabendo que as mesmas eram necessárias para impedirem uma ocorrência fatal de uma embolia pulmonar. O pessoal clinico pode não ter tempo para educar totalmente o paciente acerca da importância de usar a manga, e podem não ter o tempo para assegurar que o paciente esteja a utilizar constantemente a manga. Por exemplo, um estudo de pesquisa efectuado pela CMAJ "Clinicai Practice Guidelines for the Care and Treatment of Breast Câncer", discutiu o tratamento da limfoedema em associação ao cancro do peito. O estudo indica que os pacientes não se adaptaram porque os dispositivos eram em geral difíceis de utilizar e não eram confortáveis. É por esta razão que os fabricantes de mangas de compressão estão a tentar introduzir mangas mais confortáveis ao mesmo tempo que mantêm a eficácia clínica já encontrada nos dispositivos da arte anterior. Com a necessidade de estadias mais curtas no hospital e mais cirurgia ambulatória, a necessidade de um dispositivo mais confortável que seja mais fácil de utilizar, mantendo ao mesmo tempo a eficácia clínica, é uma necessidade sentida há muito tempo na indústria.
Tal como enunciado acima, sente-se há muito tempo a necessidade de melhorar o conforto sem comprometer a eficácia clínica, o que não se encontra nas mangas da arte anterior. Outros dispositivos da arte anterior no mercado, tais como os dispositivos da Aircast®, da Huntleigh® e da Hill-Rom® carecem de um número de desvantagens, descritas abaixo, e ultrapassadas no presente invento. As concretizações preferidas do presente invento proporcionam um substancial arrefecimento sem comprometer a eficácia clínica dos dispositivos da arte anterior tais como as mangas de compressão da Kendall, Modelos 9529 e 9530, que proporcionam a profilaxia DVT. O presente invento dirige-se à melhoria do conforto de um paciente e, assim, à adaptação em termos da utilização prescrita pelo médico.
