Kompozycja farmaceutyczna do stosowania w zapobieganiu i/lub leczeniu zaburzen plucnych, schorzen skórnych i swedzenia, neurogennego zapalenia i zaburzen centralnego ukladu nerwowego, zaburzen drgawkowych, epilepsji, zaburzen nerek, nietrzymania mo- czu, zapalen oczu, bólów zapalnych, zaburzen jedzenia, alergicznego zapalenia sluzówki nosa, zaburzen neurodegeneracyjnych, luszczycy, choroby Huntigtona, lub depresji, zawie- rajaca substancje czynna oraz farmaceutycznie akceptowalny nosnik, znamienna tym, ze jako substancje czynna zawiera N-(a-etylobenzylo)-3-hydroksy-2-fenylochinolino-4- karboksyamid lub jego farmaceutycznie akceptowalna sól, lub solwat. Description
Przedmiotem wynalazku jest N-(a-etylobenzylo)-3-hydrOksy-2-fenylochinolino-4karboksyamid tj. nowa pochodna chinoliny oraz kompozycja farmaceutyczna zawierająca ten związek.
Peptyd ssaka neurokinina B (NKB) należy do rodziny peptydów tachykinin (TK) obejmującej też substancję P (SP) i neurokininę A (NKA). Farmakologiczne i molekularne biologiczne dowody wykazały istnienie trzech podtypów receptorów TK (NK? i NK3), i NKB wiąże się preferencyjnie z receptorem NK3, chociaż rozpoznaje także oba pozostałe receptory z niż-szym powinowactwem (Maggi i in., 1993, J. Auton. Pharmacol., 13, 23-93).
Znane są selektywne peptydowe związki antagonistyczne receptora NK3 (Drapeau, 1990 Reguł. Pept., 31, 125-135), a odkrycia peptydowych agonistów receptorów NK3 sugerują, że NKB, aktywując receptor NK3, odgrywa kluczową rolę w modulacji wejść nerwowych w drogach oddechowych, skórze, rdzeniu kręgowym i drogach nigro-prążkowiowych (Myers i Undem, 1993, J. Phisiol., 470, 665-679; Counture i in.,1993, Reguł. Peptides, 46, 426-429; Mccarson i Krause, 1994, J. Neurosci., 14(2), 712-720; Arenas i in., 1991, J. Neurosci., 11, 2332-8).
Jednakże peptydopodobna natura znanych antagonistów czyni je niezbyt trwałymi z metabolicznego punktu widzenia, aby mogły służyć jako praktyczne środki lecznicze.
Odkryliśmy obecnie nową klasę selektywnych, niepeptydowych antagonistów NK3 znacznie trwalszych z metabolicznego punktu widzenia niż znane peptydowe związki antagonistyczne receptorów NK3 o potencjalnej leczniczej przydatności w leczeniu zaburzeń płucnych (astma, chroniczne czopujące choroby płuc - COPD, nadwrażliwość, kaszel), zaburzeń skórnych i swędzenia (np., nielokalne zapalenie skóry i skórne bąble i zaczerwienienia), neurogennego zapalenia i zaburzeń centralnego układu nerwowego (choroba Parkinsona, zaburzenia ruchowe, lęki i psychoza). Zaburzenia te określa się dalej jako pierwotne zaburzenia.
Nowe związki antagonistyczne NK3 według wynalazku mają też potencjalną przydatność leczniczą w leczeniu zaburzeń drgawkowych (np. epilepsji), zaburzeń nerek, nietrzymania moczu, zapaleń oczu, bólów zapalnych, zaburzeń jedzenia (inhibicja przyjmowania pokarmu), alergicznego zapalenia śluzówki nosa, zaburzeń neurodegeneracyjnych (np. choroby Alzheimera), łuszczycy, choroby Huntigtona, i depresji (określane dalej jako wtórne zaburzenia).

Przedmiotem jest związek N-(a-etylobenzylo)-3-hydroksy-2-fenylochinolino-4karboksyamid lub jego solwat lub sól, o wzorze I,
w którym Ph oznacza fenyl.
Przedmiotowy związek lub jego sól lub solwat mają korzystnie postać farmaceutycznie dopuszczalną lub zasadniczo czystą. Przez farmaceutycznie dopuszczalną postać rozumie się postać o farmaceutycznie dopuszczalnym poziomie czystości bez zwykłych farmaceutycznych dodatków, takich jak rozcieńczalniki i nośniki, i nie zawierającą substancji uważanych za toksyczne w normalnych dawkach. Zasadniczo czysta postać będzie zwykle zawierała co najmniej 50% (poza zwykłymi farmaceutycznymi dodatkami), korzystnie 75%, korzystniej 90% i jeszcze korzystniej 95% związku lub jego soli lub solwatu. Jedną z korzystnych farmaceutycznie dopuszczalnych postaci jest postać krystaliczna, w tym postać w kompozycji farmaceutycznej, która jest również przedmiotem wynalazku. Inną dogodną pochodną jest mieszany bezwodnik tworzony pomiędzy kwasem i chloromrówczanem alkilu; inną dogodną pochodną jest aktywowany ester, taki jak cyj anometylowy, tiofenylowy, p-nitrofenylowy, p-nitrotiofenylowy, 2,4,6-trichlorofenylowy, pentachlorofenylowy, pentafluorofenylowy, N-hydroksyftalimidowy, N-hydroksypiperydynowy, N-hydroksybursztynowy, N-hydro-ksybenzotriazolowy; lub grupę karboksylową można aktywować stosując karbodiimiid lub N,N'-karbonylodiimidazol.

Ciecz może zawierać środki bakteriostatyczne, antyutleniacze lub inne środki konserwujące, bufory lub substancje rozpuszczalne dla osiągnięcia izotoniczności roztworu z krwią, środki zagęszczające, środki zawiesinujące lub inne farmaceutycznie dopuszczalne dodatki. Takie postaci będą występowały w jednostkowych dawkach takich jak ampułki lub jednorazowe urządzenia do zastrzyków, lub w postaciach wielodawkowych takich jak buteleczki, z których można pobrać właściwą dawkę, lub stałych postaci koncentratów do wytwarzania kompozycji do wstrzykiwania.
Związek według wynalazku można także podawać przez inhalację, drogą donosową lub doustną. Takie sposoby podawania można realizować dzięki kompozycji aerozolowej zawierającej związek według wynalazku i dogodny nośnik, ewentualnie w zawiesinie, np. węglowodorowym propelencie.
Korzystne kompozycje aerozolowe zawierają mikronizowane cząsteczki związku w połączeniu ze środkiem powierzchniowo czynnym, rozpuszczalnikiem lub środkiem dyspergującym w celu zapobiegania sedymentacji zawieszonych cząstek. Korzystnie, cząstki związku mają od około 2 do 10 fam.
Kolejne sposoby podawania związku według wynalazku obejmują przezskóme podawanie wykorzystujące plastry naskóme. Korzystna kompozycja obejmuje związek według wynalazku zdyspergowany we wrażliwym na ciśnienie środku przylepnym, który przywiera
186 075 do skóry pozwalając związkowi dyfundować ze środka przylepnego przez skórę pacjenta. Dla uzyskania stałej szybkości skórnej absorpcji, można stosować znane wrażliwe na ciśnienie środki przylepne takie jak naturalny kauczuk lub silikon.
Jak wspomniano powyżej, skuteczna dawka związku zależy m.in. od stanu pacjenta, częstości, i drogi podawania. Jednostkowa będzie zwykle zawierała od 20 do 1000 mg i korzystnie od 30 do 500 mg, w szczególności 50, 100, 150, 200, 250, 300, 350, 400, 450, lub 500 mg. Kompozycję można podawać raz lub kilka razy dziennie np. 2, 3 lub 4 razy dziennie, a całkowita dzienna dawka dla 70 kg dorosłego będzie zwykle w zakresie od 100 do 3000 mg. Alternatywnie dawka jednostkowa będzie zawierała od 2 do 20 mg składnika aktywnego i będzie podawana wielokrotnie, w razie potrzeby, dając właściwą dzienną dawkę.
Nie oczekuje się żadnych niedopuszczalnych toksykologicznie efektów przy podawaniu związków zgodnie ze sposobem według wynalazku.
Aktywność związku według wynalazku, jako ligandu NK3, jest określana jego zdolnością do hamowania wiązania radioznaczonych ligandów NK3, [l25I]-[Me-Phe7]-NKB lub [3H]-Senktide, na receptorach NK3 ludzi i świnek morskich (Renzetti i in., 1991, Neuropeptide, 18, 104-114; Buell i in., 1992, FEBS, 299(1), 90-95; Chung i in., 1994, Biochem. Biophys. Res. Commun., 198(3), 967-972).
Przykład 1 (R,S) N-(«-eiyk)ben/ylo)-3-hydroksY-2-fenyl()chinolino-4-karboksyamid
Kwas 2-fenylo-3-hydroksy-chinolino-4-karboksylowy (8,0 mmola) rozpuszczono w atmosferze azotu w aprotonowym rozpuszczalniku takim jak suchy tetrahydrofuran (70 ml) i CH3CN (30 ml). Dodano (a-etylobenzylo)aminę (8,4 mmola), N-metylomorfolinę (9,9 mmola) i N-hydroksybenzotriazol (HOBT) (15,5 mmol), mieszaninę reakcyjną mieszano w temperaturze 0°C. Dodano kroplami (9,0 mmol) dicykloheksylokarbodwuimidu (DCC), rozpuszczonego w 10 ml CH2CI2, i roztwór trzymano w temperaturze 0°-5°C przez 1 godzinę i następnie w temperaturze pokojowej przez 2 godziny. Strącony osad odsączono i roztwór odparowano pod zmniejszonym ciśnieniem do suchej masy. Pozostałość rozpuszczono w CH2CI2 i przemyto H2O, nasyconym roztworem NallCO;, 5% kwasem cytrynowym, nasyconym roztworem NaHCCh i nasyconym roztworem NaCl.
Warstwę organiczną oddzielono, osuszono nad NróSCU i odparowano pod zmniejszonym ciśnieniem do suchej masy; pozostałość rozpuszczono w 2C ml CH2CI2 i pozostawiono na noc. Wytrącony jeszcze dicykloheksylomocznik odsączono.
Roztwór odparowano pod zmniejszonym ciśnieniem do suchej masy z wytworzeniem surowego produkt. Po oczyszczeniu otrzymano związek tytułowy z wydaj nością 47%. Temperatura topnienia 168-169°C (rozp. kryst.) (toluen).

Poddając w analogiczny sposób związek o wzorze (II) z odpowiednim izomerem związku o wzorze (III), otrzymano enancjomer (-)-(S)-N-(a_-et^\lobenzylo)-3-hydroksy-2fenylochinolino-4-karboksyarnid z wydajnością 66%. Temperatura topnienia 122-125°C (rozp. kryst.) (iPr?O).

2. Związek według zastrz. 1, znamienny tym, że stanowi go (S)-N-(α-etylobenzy]o)-3hydroksy-2-fenylochinolino-4-karboksyamid lub jego sól, lub solwat.
3. Kompozycja farmaceutyczna do stosowania w zapobieganiu i/lub leczeniu zaburzeń płucnych, schorzeń skórnych i swędzenia, neurogennego zapalenia i zaburzeń centralnego układu nerwowego, zaburzeń drgawkowych, epilepsji, zaburzeń nerek, nietrzymania moczu, zapaleń oczu, bólów zapalnych, zaburzeń jedzenia, alergicznego zapalenia śluzówki nosa, zaburzeń neurodegeneracyjnych, łuszczycy, choroby Huntigtona, lub depresji, zawierająca substancję czynną oraz farmaceutycznie akceptowalny nośnik, znamienna tym, że jako substancję czynną zawiera N-(a-etylobenzylo)-3-hydroksy-2-fenylochinolino-4-karboksyamid lub jego farmaceutycznie akceptowalną sól, lub solwat.
