Abstract
Det er beskrevet en farmasøytisk sammensetning som omfatter gabapentin eller en analog derav (pregabalin eller tiagabin) og et (-aminoamid og dens anvendelse som smertestillende middel. En synergistisk effekt av de respektive smertestillende aktivitetene uten ledsagende økning av bivirkninger er observert.
Description
Den foreliggende oppfinnelsen vedrører en farmasøytisk sammensetning som innbefatter gabapentin eller en analog derav og et a-aminoamid og dens anvendelse som smertestillende middel. Mer spesifikt er denne oppfinnelsen rettet mot en farmasøytisk sammensetning som innbefatter gabapentin eller pregabalin, og et a-aminoamid som er nyttig ved behandling av smerte.
Smerte lindres vanligvis ved å administrere smertestillende legemidler som virker ved å redusere smertefølelsen ved å heve smerteterskelen, og/eller å endre dens oppfatning.
Selv om et stort antall smertesyndromer behandles ved tilgjengelige terapier, er det funnet at kronisk neuropatiske smertesyndromer som er en følge av perifer nerveskade har en svært redusert sensitivitet for de to hovedklassende av smertestillende midler, opioider og ikke-steroide antiinflammatoriske legemidler (NSAID).
I letingen etter alternative behandlingsformer har krampestillende midler dukket opp som mulige farmakologiske innblandinger (McQuay et al, "Anticonvulsant drug for the management of pain: a systematic review", Br. Med. J., 311: 1047, 1995).
Blant den nye generasjonen av antiepileptiske legemidler (AED) som benyttes ved behandling av neuropatisk smerte har l-(aminometyl)cykloheksaneddiksyre, også kjent som gabapentin (heretter GBP), et amnoacid som er strukturelt beslektet med y-amino-smørsyre (heretter GABA), en fremtredende posisjon sammen med det strukturelle analoger, slik som pregabalin (Tremont et al., "Anticonvulsants for neuropathic pain syndromes: mechanisms of action andplace in therapy.", Drugs, 60 (5): 1029-52, 2000) og tiagabin (Field et al., "Evaluation of Gabapentin and S-(+)-3-Isobutylgaba in a rat modell of postoperative pain", J. Pharmacol. Exp. Ther.; 282 (3): 1242-4, 1997) med hensyn til hyppighet og anvendelsesomfang.
Det er rapportert at GBP er aktiv i flere forskjellige dyremodeller for smerte, der den blokkerer den sene toniske nosisepsjonsfasen som er indusert ved delvis ombinding av hoftenerven (Pan et al., "Gabapentin suppresses ectopic nerve discharges and reverses allodynia in neuropathic rats", J. A randomized controlled trial", JAMA 280 (21): 1837-1842,1998).
Av denne grunn ville det være et ønske å finne andre forbindelser med antinosiseptiv virkningsmekanisme forskjellig fra og/eller komplementær med en av GBP eller dets analoger slik at lavere doser av GBP eller dets analoger kunne benyttes hvilket ville begrense kjente bivirkninger, og samtidig tillate opprettholdelse av dets smertestillende egenskaper eller enda bedre å styrke dem.
Selv om GBP ble utviklet som en strukturell GABA analog, ser den verken ut til å ha direkte "GABA-ergisk" virkning, eller å påvirke GABA opptak eller metabolisme. For å forklare den antihyperalgesieffekten av GBP, peker foreløpige bevis på den mulige effekten av GBP på de spenningsavhengige Ca<++>kanalene ved interaksjon med a/2-8 underenheten. Eksperimentelle bevis støtter involveringen av Ca<++>kanaler i patofysio-logien av smerte. Således kan virkningen av GBP på disse kanalene være en signifikant bidragsyter til det antinosispetive effekt.
Krampestillende legemidler slik som gabapentin, har blitt kombinert med ikke-toksiske blokkere for N-metyl-d-aspartat (NMDA) reseptoren. Slike sammensetninger er beskrevet som nyttige ved behandlingen av neuropatisk smerte. For eksempel beskriver WO 98/07447 kombinasjonen av en neuropatisk smertelindringsmengde av et krampestillende legemiddel, inkludert gabapentin, lamotrigin, valproinsyre, topiramat, famoti-din, fenobarbital, difenylhydantoin, fenytoin, mefenytoin, etotoin, mefobarbital, primi-don, karbamazepin, etoksuksimid, metsuksimid, fensuksimid, trimetadion, benzodiaze-pin, fenacemid, acetazolamid, progabid, klonazepam, divalproeksnatrium, magnesium-sulfatinjeksjon, metarbital, parametedion, fenytoinnatrium, valproatnatrium, klobazam, sultiam, dilantin, difenylan eller L-5-hydroksytryptofan og en krampestillende potensi-erende mengde av en ikke-toksisk blokker for NMDA reseptoren. Denne referansen beskriver imidlertid ikke noen synergistisk effekt av de omtalte sammensetningene.
Krampestillende legemidler kombinert med NSAID eller narkotiske smertestillende midler er også blitt beskrevet som nyttige ved behandling av smerte. WO 99/12537 beskriver en sammensetning av de krampestillende forbindelsene gabapentin eller pregabalin i kombinasjon med NSAID naproksen eller med narkotiske smertestillende mdiler. Kombinasjoner av krampestillende midler og andre legemidler med opoide smertestillende virkning er foreslått (Donnadieu, S., et al. Pain Relief, Presse Medicale, 1998,27/39,2062-2069). Disse referansene beskriver imidlertid heller ikke noen synergistisk effekt av de omtalte sammensetningene.
Kombinasjoner av krampestillende legemidler, inkludert GBP, med det sentralt virk-ende smertestillende tramadol, dvs. (1R,2R eller lS,2S)-2-[(dimetylamino)metyl]-l-[3-metoksyfenyl]cykloheksanol, er beskrevet i WO 01/13904.
WO 00/61188 beskriver en farmasøytisk sammensetning som omfatter en natriumkanal-blokker og gabapentin eller pregabalin eller salter eller kombinasjoner derav; sammensetningen er effektiv ved behandling, forebygging eller forbedring av kronisk smerte eller kramper. Blant natriumkanalblokkeringsmidlene er et antall aminoamider beskrevet, for eksempel de som er omtalt i US 5.449.692 (WO 94/22809), WO 97/05102, US 5.446.066 (WO 94/22808) og US 5.236.957 (WO 90/14334), selv om en sammensetning som omfatter disse aldri har vært eksemplifisert, mens den synergistiske anti-allodinieffekten av gabapentin og natriumkanalblokkeren 4-(4'-fluorfenoksy)benz-aldehydsemikarbazon, i Chung-modellen for neuropatisk smerte hos rotter er illustrert deri.
WO 00/53225 beskriver kombinasjoner av effektive mengder av minst en antielipetisk forbindelse, hvoriblant gabapentin og pregabalin, med smertestillende egenskaper, og minst en forbindelse valgt fra gruppen bestående av N-metyl-D-aspartat (NMDA) reseptorantagonister, ikke-steroide antiinflammatoriske (NSAID) smertestillende midler og kombinasjoner derav, hvoriblant morfin og naproxen.
Det er et formål ved den foreliggende oppfinnelsen å tilveiebringe en sammensetning som omfatter en Na<+->kanalblokker a-aminoamid og GBP, eller pregabalin, med for-bedrede egenskaper for smerteterapi.
Det er også et formål ved den foreliggende oppfinnelsen å tilveiebringe en sammensetning som omfatter et a-aminoamid og GBP, eller pregabalin, der kombinasjonen av de nevnte forbindelsene viser en synergistisk effekt, mens det benyttes mindre av hver av de nevnte forbindelsene.
Videre har søkeren merket seg at ikke alle natriumkanalblokkeringsmidlene som er aktive i en modell for kronisk smerte er synergistisk med gabapentin.
Det er nå funnet at kun noen kjente a-aminoamider eller et farmasøytisk akseptabelt derivat derav, som har smertestillende aktivitet, kombinert med GBP eller pregabalin, eller et farmasøytisk akseptabelt salt derav, betraktelig styrker de smertestillende eller antinosiseptive egenskapene av GBP (eller pregabalin) overraskende tilveiebringer en reell synergistisk effekt sammenlignet med de respektive aktivitetene, og er derfor vel-lykket ved dramatisk begrensning av bivirkninger av GBP (eller pregabalin) ved å tillate å redusere dets farmasøytisk effektive mengde til en uventet lavere dosering.
Ifølge den foreliggende oppfinnelsen tilveiebringes en farmasøytisk sammensetning som omfatter gabapentin, eller pregabalin eller et farmasøytisk akseptabelt syreaddisjonssalt med uorganiske eller organiske syrer derav, og et a-aminoamid valgt fra gruppen bestående av (S)-(+)-2-[4-(2-fluorbenzyloksy)benzylamino]propanamid (intern kode: NW-1029), (R)-(-)-2-[4-benzyloksybenzylamino]-3-fenyl-N-metylpropanamid (intern kode: NW-1037) og (S)-(+)-2-[4-(3-fluorbenzyloksy)-benzylamino]-N-metyl-propanamid (intern kode: NW-1043); enten som en enkeltisomer eller som en blanding derav, eller et farmasøytisk akseptabelt syreaddisjonssalt med uorganiske eller organiske syrer derav;
der a-aminoamidet og gabapentin eller pregabalin eller de farmasøytisk akseptable syreaddisjonssaltene med uorganiske eller organiske syrer derav, foreligger i et forhold basert på en fraksjon av deres respektive ED50verdier, der forholdet strekker seg fra henholdsvis 1:1 til 30:1 eller fra 1:1 til 1:30.
Fortrinnsvis er det ovenfor definerte a-aminoamidet og GBP (eller pregabalin) som er omfattet av den farmasøytiske sammensetningen ifølge den foreliggende oppfinnelsen tilstede i et forhold basert på en fraksjon av deres respektive ED50verdier, der forholdet henholdsvis strekker seg fra 1:1 til 9:1 eller fra 1:1 til 1:9; mest foretrukket strekker forholdet seg henholdsvis fra 1:1 til 3:1 eller 1:1 til 1:3.
Et annet aspekt ifølge oppfinnelsen vedrører anvendelsen av den ovenfor definerte farmasøytiske sammensetningen til fremstilling av et medikament for behandling av en smertetilstand.
I særdeleshet administreres en dose av den farmasøytiske sammensetningen til patte-dyret som har behov for den nevnte behandlingen som strekker seg fra 0,05 til 50 mg/formuleringsenhet per kg kroppsvekt; den farmasøytiske sammensetningen omfatter gabapentin og et a-aminoamid valgt fra gruppen bestående av (S)-(+)-2-[4-(2-fluor-benzyloksy)benzylamino]propanamid, (R)-(-)-2-[4-benzyloksybenzylamino]-3-fenyl-N-metylpropanamid, og (S)-(+)-2-[4-(3-fluorbenzyloksy)benzylamino]-N-metylpropan-amid enten som en enkeltisomer, eller som en blanding derav, eller et farmasøytisk akseptabelt derivat derav, i et forhold av gabapentin og a-aminoamidet, basert på en fraksjon av deres respektive ED50verdier, på fra 1:1 til 30:1 eller fra 1:1 til 1:30, mer foretrukket fra 1:1 til 9:1 eller fra 1:1 til 1:9, mest foretrukket på fra 1:1 til 3:1 eller fra 1:1 til 1:3.
Neuropatiske og kroniske smertetilstander hos et pattedyr, inkludert mennesker, kan således lindres og behandles. Eksempler på smertetilstander hos pattedyr som kan behandles ved sammensetningen ifølge oppfinnelsen inkluderer sentralt mediert smerte, perifer mediert smerte, strukturell eller mykvevskaderelatert smerte, progressiv syk-domsrelatert smerte og neuropatiske smertetilstander, hvorav alle ville inkludere akutt smerte slik som forårsakes ved akutt skade, traume eller kirurgi.
Eksempler på smertetilstander som kan behandles ved sammensetningen ifølge oppfinnelsen inkluderer i særdeleshet perifere neuropatier, slik som trigeminal neuralgi, postherpetisk neuralgi, diabetisk neuropati eller andre metabolske neuropatier, glosso-faryngeus neuralgi, radikulopati, dental smerte, clustersmerte, migrene og enhver av typene av vaskulære hodepiner og neuropati sekundært til metastatisk infiltrering, adipose dolorosa og forbrenningssmerte; sentrale smertetilstander etterfulgt av slag, skade påført ved uhell eller kirguisk eller på annen måte fremstilte talamiske lesjoner og multippel sklerose. Eksempler på smerteinflammatoriske tilstander som kan behandles ved sammensetningen ifølge oppfinnelsen inkluderer reumatoid artritt, alkyloserende spondylitt, osteoartritt, bursitt, tendinitt og akutt giktartritt.
"Behandling" slik det her er benyttet dekker enhver behandling av en tilstand hos et pattedyr, i særdeleshet et menneske, og inkluderer: (i) å forebygge at sykdommen forekommer hos et subjekt som kan være forhåndsdisponert for denne sykdommen, men som ikke ennå har blitt diagnostisert for å ha den;
(ii) å hemme tilstanden, dvs. å stanse dens utvikling; eller
(iii) å lindre tilstanden, dvs. å forårsake tilbakegang av sykdommen.
Både GBP (eller pregabalin) og de nevnte a-aminoamidene og de farmasøytisk akseptable derivatene derav henvises til i den foreliggende beskrivelsen som de "aktive forbindelser".
Farmasøytisk akseptable derivater av de aktive forbindelsene er syreaddisjonssalter med uorganiske syrer, f.eks salpetersyre, saltsyre, hydrobromsyre, svovelsyre, perklorsyre og fosforsyre og lignende, eller organiske syrer, f.eks eddiksyre, propionsyre, glykolsyre, melkesyre, oksalsyre, malonsyre, maleinsyre, vinsyre, sitronsyre, ravsyre, benzosyre, kanelsyre, mandelsyre, metansulfonsyre, p-toluensulfonsyre og salicylsyre og lignende.
a-aminoamidene som finnes i sammensetningen ifølge oppfinnelsen og den smertestillende aktiviteten derav, i særdeleshet mot kronisk og neuropatisk smerte hos pattedyr inkludert mennesker, er beskrevet i henholdsvis WO 90/14334 og WO 99/35125, og kan fremstilles ifølge det som der er beskrevet.
I en farmasøytisk sammensetning ifølge den foreliggende oppfinnelsen finnes et a-aminoamid, GBP (eller pregabalin) i et forhold som er basert på en fraksjon av deres respektive ED50verdier der forholdet kan variere fra 1:1 til 30:1 eller, reversibelt fra 1:1 til 1:30; fortrinnsvis fra 1:1 til 9:1 eller fra 1:1 til 1:9; og mer foretrukket fra 1:1 til 3:1 eller fra 1:1 til 1:3, avhengig av det ønskede resultatet.
En fagmann på området vil forstå at selv om GBP fortrinnsvis benyttes som en av de aktive forbindelsene i sammensetningen ifølge denne oppfinnelsen, så kan pregabalin benyttes istedenfor GBP i den nevnte sammensetningen ved å benytte de samme konseptene og ideene hvorfra denne oppfinnelsen har basis.
Sammensetningen ifølge oppfinnelsen kan fremstilles ved konvensjonelle prosedyrer som er kjent innenfor fagområdet, for eksempel ved å blande de aktive forbindelsene med farmasøytisk, terapeutisk inerte organiske og/eller uorganiske bærermaterialer. Sammensetningen ifølge oppfinnelsen kan være på flytende form, for eksempel på formen av en løsning, suspensjon, emulsjon; eller på fast form, for eksempel tabletter, trocheer, kapsler.
Egnede farmasøytisk, terapeutisk inerte organiske og/eller uorganiske bærermaterialer som er nyttige ved fremstillingen av sammensetningen ifølge den foreliggende oppfinnelsen inkluderer for eksempel vann, gelatin, arabisk gummi, laktose, stivelse, cellulose, magnesiumstearat, talkum, vegetabilske oljer, polyalkylenglykoler og lignende. Den farmasøytiske sammensetningen ifølge oppfinnelsen kan steriliseres og kan inneholde ytterligere komponenter som er velkjente for en fagmann på området slik som for eksempel konserveringsmidler, stabilisatorer, fukte- og emulgeringsmidler, for eksempel parafinolje, mannidmonooleat, salter for å regulere osmotisk trykk, buffere og lignende.
Den farmasøytiske sammensetningen ifølge oppfinnelsen er generelt på formen av en doseenhet.
Optimale terapeutisk effektive doser som skal administreres kan lett bestemmes av en fagmann på området og vil variere med den bestemte kombinasjonen av et a-aminoamid og GBP (eller pregabalin), med mengden av aktive ingredienser som benyttes i et synergistisk forhold basert på en fraksjon av deres respektive ED50verdier, med prepa-ratstyrken, administreringsmåten og med utviklingen av tilstanden eller forstyrrelsen som skal behandles. I tillegg vil faktorer forbundet med det spesielle subjektet som skal behandles, inkludert subjektets alder, vekt, diett og administreringstid resultere i behovet for å justere dosen til et passende terapeutisk effektivt nivå.
Generelt kan en doseenhet av den farmasøytiske sammensetningen ifølge oppfinnelsen inneholde 0,2 til 99,8, fortrinnsvis 0,5 til 99,5 vekt-% av hver av de aktive forbindelsene, i hele blandingen av de aktive forbindelsene. Kombinasjonen av de to aktive forbindelsene kan generelt administreres til et pattedyr som har behov for dette i et bredt doseområde på fra 0,05 til 50 mg/formuleringsenhet per kilo kroppsvekt eller, mer spesifikt til et menneske med en gjennomsnittlig kroppsvekt på 70 kilo, i en dose på fra 3,5 mg til 3.500 mg/formuleringsenhet.
Videre er det også innenfor omfanget av oppfinnelsen å administrere hver aktiv forbindelse i sammensetningen ifølge oppfinnelsen individuelt. Således er det også mulig å formulere hver av de to aktive forbindelsene til separate doseringsformer, i henhold til konvensjonelle prosedyrer som er kjent innenfor fagområdet, og å administrere dem enten samtidig eller sekvensielt.
Med andre ord kan den uventede smertestillende aktiviteten som oppnås ved sammensetningen ifølge oppfinnelsen oppnås ved enten samtidig eller sekvensiell administrering av de aktive forbindelsene.
Administreringsrute av forbindelsen ifølge oppfinnelsen kan både være oral og paren-teral. Sammensetningen bør administreres minst en gang per dag eller mer hyppig om nødvendig, i relasjon til alvorligheten av tilstandene til pasienten. Den smertestillende effekten som observeres er betydelig høyere enn den som ville være resultatet av den additive effekten av de separate aktive forbindelsene. Fordelene ved den antinosiseptive styrkingen er mange, og inkluderer en reduksjon i doseringen av GBP eller pregabalin som er nødvendig for å frembringe smertestillelse, med en konsekvent reduksjon i uønskede bivirkninger.
En enkelt dose av en formulering av en farmasøytisk sammensetning ifølge oppfinnelsen som viser en synergistisk aktivitet inneholder derfor en terapeutisk effektiv dose av aktiv ingrediens som generelt strekker seg fra 3,5 mg til 3.500 mg av en kombinasjon av et a-aminoamid og GBP (eller pregabalin); fortrinnsvis fra 8 mg til 400 mg; mer foretrukket fra 15 mg til 250 mg; og mest foretrukket fra 18 mg til 90 mg. For eksempel vil en 20 mg formulering av en farmasøytisk sammensetning som omfatter GBP (ED50= 12,40 mg) og NW-1029 (ED50= 0,82 mg), i et forhold på henholdsvis 1:3, basert på en fraksjon av deres respektive ED50verdier, inneholde 16,7 mg GBP og 3,3 mg NW-1029.
GENERELLE METODER
Koadministrering av ekvipotente antinosiseptive doser av flere forskjellige av de ovenfor definerte a-aminoamidene (dvs. NW-1029, NW-1027 og NW-1043) og GBP ble undersøkt for å vise at sammensetningen ifølge oppfinnelsen gir en synergistisk effekt som er større enn aktiviteten vist ved hver av de aktive forbindelsene når de gis alene, og større enn den man ville forvente ved kun addisjon av aktivitetene av hver av de aktive forbindelsene, uten ledsagende økning i relaterte bivirkninger. Som tidligere angitt er typiske bivirkninger av GBP hos dyr bevegelsessvekkelse og ataksi.
Den synergistiske antiallodyne effekten av sammensetningen ifølge oppfinnelsen og dens virkning på bevegelsesutøvelse ble evaluert ved å benytte de følgende metodene på rotte: Testingen for synergi av de aktive forbindelsene i sammensetningen ifølge oppfinnelsen ble utført i henhold til tilnærmingen som er utviklet av tallarida, R.J. 1992, Pain, 49: 93-97; Tallarida, R.J. et al. 1997, Life Sciences, vol 61, n. 26, 417-425; Przesmycki, K. et al. 1997, Eur. J. of Pharmacology, 337: 11-17.
Denne prosedyren medfører bestemmelsen av den totale mengden i blandingen som er nødvendig for å frembringe en spesifikk synergistisk antiallodyn effekt ved 50 %-dose-nivået (EDson^) og den tilsvarende totale mengden som ville være forventet ved kun addisjon (ED50add)- Når det er etablert at ED50miks er mindre enn ED50addfor et spesifikt angitt forhold, så har sammensetningen en synergistisk antiallodyn effekt. Både mengdene EDjomiks og EDjoadd er tilfeldige variable. EDjomiks ble bestemt fra doseresponskurven for et spesifikt angitt forhold av komponentene; ED50addble beregnet fra ED50-verdiene fra de individuelle legemidlene.
EDjomiks ble deretter statistisk sammenlignet med EDjoadd: i den foreliggende beskrivelsen er "signifikant lavere enn den teoretisk additive verdien" (EDsoadd) ment å indikere at den eksperimentelle ED50-verdien er utenfor 95 %-sikkerhetsintervallene (CI) for ED50add-
Med andre ord, dersom den aktuelle EDsomiksfaller innenfor CI. av det teoretiske additivet ED50add>så ville effekten av sammensetningen være additiv; ellers, dersom sammensetningens ED5omikser mindre enn den teoretiske additive ED50add(dvs. den faller ikke innenfor det teoretiske CL), så forekommer en signifikant synergistisk interaksjon mellom de aktive forbindelsene.
Derfor ble ED50verdiene ansett å skille seg signifikant (P<0,05) fra hverandre dersom hver ED5o-verdi var utenfor 95 % CI. for den andre.
GENERELL METODE A:
Prosedyre for å teste den antiallodyne effekten av sammensetningen ifølge oppfinnelsen.
Prosedyren som er benyttet for å detektere og sammenligne den synergistiske effekten av sammensetningen ifølge den foreliggende oppfinnelsen for hvilken det er god korrelasjon med human virkeevne for smertebehandlingen, er prosedyren for allodyni-måling som finnes i den monoartritiske rottemodellen for kronisk smerte indusert av fullstendig Freunds adjuvans (CFA) (Butler et al., "A limited arthritic model for chronic pain studies in the rat" Pain; 48: 73-81,1992).
Dyr
Vokse hann Wistar-rotter (kroppsvekt 175 - 200 g, Harlan-Nossan, Italia) ble holdt i adskilte bur med fri tilgang til vann og standard rottefor ved en konstant temperatur (22 ± 0,5°C) og relativ fuktighet (60 - 70 %), med en 6:00 til 18:00 lys-mørke periode.
Monoartittisk modell
Inflammasjonen ble indusert i rottene ved en intra-planetar injeksjon av fullstendig Freunds adjuvans (CF A, Sigma - 100 ul) inn i den venstre bakpote som inneholder Mycobacterium tubercolosis som er drept ved varme og tørket i en blanding av parafinolje og mannidmonooleat som emulsjonsmiddel. En gruppe av kontrolldyr ble injisert med 100 ul mineralolje, ufullstendig Freunds adjuvans (IFA, Sigma). CF A-injeksjonen fremskaffet et areal av lokalisert ødem og inflammasjon starter 48 timer etter injeksjon, med en progressiv reduksjon av den mekaniske tilbaketrekningsterskelen.
Hvert dyr fikk utvikle artritt over en 8 - 9 dagers periode før testing.
Måling av mekanisk allodyni
Rotter ble plassert i individuelle plastbokser på et masket metallgulv og tillatt å aklima-tisere seg i omtrent 30 minutter. En serie av kalibrert von Frey hår med logaritmisk voksende stivhet (fra 2,83 til 5,88 x Logioav bøyekraften, g) ble påført poten med opp-ned metoden. Hvert hår ble presentert vinkelrett mot poten, med tilstrekkelig kraft til å forårsake forsiktig bøying, og holdt tilnærmet 2-3 sekunder. Filamentet ble registrert når en positiv respons var observert (potetiltrekning, slikking eller risting).
Antiallodyneffekten ble uttrykt som %MPE (maksimal mulig effekt) 2 timer etter dosering.
Dyredosering
Rottene ble alle dosert oralt med forskjellige doser av et a-aminoamid alene, GBP alene, kombinerte doser av et a-aminoamid og GBP eller bindemiddel. Doserings-volumet var 2 ml/kg. Doseringsmaterialene ble alle tilberedt i bindemidlet (destillert vann); legemiddelvekter ble beregnet som den frie basen. For sammensetningen ifølge oppfinnelsen ble a-aminoamidet og GBP (eller pregabalin) begge veid som fri base i det valgte forholdet av deres respektive ED50og deretter oppløst i det passende volumet for å gi den endelige doseringssuspensjonen.
Analyse av antiallodyn effekt
Data er presentert som gjennomsnitt av 4/6 dyr per gruppe/dose.
Flere (typisk 4) doser av hver forbindelse alene ble studert for å bestemme ED50. ED50ble definert som doseringen som frembringer 50 % reversering av mekanisk hyperalgesi ved 2 timer etter behandling. En eksperimentell ED50og 95 % sikkerhetsintervaller (CI) ble beregnet for hver forbindelse alene fra de lineære regresjonene som passer til de eksperimentelle dataene i henhold til likningen y = a + bx.
De forskjellige a-aminoamidene ble deretter kombinert med GBP ved forskjellige forhold i relasjon til deres respektive ED50(eksempler av forholdene er 1:1; 1:3; 1:9 og henholdsvis 1:1, 3:1 og 9:9 forhold) for evaluering av den synergistiske effekten.
Flere (typisk 4) doser av hvert utvalgte forhold ble deretter studert oralt for å evaluere synergieffekten av kombinasjonen.
Hver rotte mottok kun en behandling.
GENERELL METODE B:
Rotte roterende stang test
Den roterende stangtesten er en veletablert metode som benyttes som forutsigende for CNS-relaterte bivirkninger hos mennesker, og i særdeleshet bevegelsessvekkelse og ataksi.
Neurologisk svikt ble indikert ved dyrets manglende evne på å forbli på det rullende apparatet i hele testtiden (J. Of the American Pharmaceutical Association, 1957,46(3), 208-209).
Flere (typisk 4-5) doser av hver forbindelse alene ble studert for å bestemme TD50, dvs. dosen av den testede forbindelsen som forårsaker at 50 % av dyrene faller ned fra valsen, beregnet ved Probit analyse. De forskjellige a-aminoamidene ble deretter kombinert med GBP ved forskjellige forhold i relasjon til deres respektive TD50som på forhånd var beregnet (et ikke begrensende eksempel på forholdene er 1:1 forhold) for evalueringen av den mulige synergieffekten. Data er presentert som gjennomsnitt av 8/10 dyr per gruppe/dose.
Testen ble utført 120 minutter etter legemiddeladministrering.
RESULTATER
Ved testen "von Frey" for evalueringen av antiallodynaktivitet var den gjennomsnittlige grunnlinje potetiltrekkingsterskelen som ble oppnådd for de naive dyrene 5,04 ± 0,20 Log[10 x kraft (mg)]; i motsetning til dette var den gjennomsnittlige rottetiltreknings-terskelen for CFA-behandlede rotter signifikant lavere: 3,11 ± 0,11 Log[10 x kraft (mg)]. Bindemiddel (destillert vann)-injeksjon hadde ingen antihyperalgesi effekt i de betente potene.
a-aminoamidene som ble gitt alene i koadministrering med GBP var NW-1029, NW-1037, NW-1043. Gruppene ble lagd av 4/6 dyr.
Alle forbindelsene som ble gitt alene frembrakte en signifikant doserelatert antiallodyn-effekt, (reverserende mekanisk overfølsomhet) i betente poter.
ED5oestimert for GBP alene, 2 timer etter enkel administrering, var 12,40 (CL: 10,3 - 14,3) mg/kg (tabell 1).
ED50estimert for NW-1029, NW-1037 og NW-1043 alene, 2 timer etter enkel administrering, var henholdsvis 0,82 (CL: 0,2 - 4,1), 3,45 (CL: 2,3 - 4,7) og 7,05 (CL: 5,8 - 7,9) mg/kg (tabell 1).
ED50oppnådd eksperimentelt (EDsomiks) fra doseresponskurven etter koadministrering av NW-1029, NW-1037 og NW-1043 med GBP ved gitte forhold er også angitt i tabell 1 og sammenligningen med den additive ED50add-Den siste kolonnen i tabell 1 rapport-erer den beregnede mengden av hver ingrediens i sammensetningen i de eksperimentelle EDsOmiks-
De eksperimentelle verdiene av ED50miksoppnådd fraNW-1029/GBP-sammensetningen ifølge oppfinnelsen var lavere enn den beregnede additive ED50addfor alle angitte dose-forhold av legemidler (se tabell 1). Lignende data ble oppnådd for de andre to forbindelsene NW-1037 og NW-1043 i kombinasjon med GBP ved de angitte doseforholdene på 1:1.
Samtidig administrering av GBP og de aktive a-aminoamidene som er definert over frembringer en antinosiseptiv effekt i en dyremodell for hyperalgesi, som er superadditiv.
I motsetning til dette er effektene oppnådd på bevegelsesfunksjonene og ataksi ikke superadditive slik det er vist ved resultatene oppnådd i den roterende stangtesten (tabell 2). Ved å følge den samme eksperimentelle protokollen som er illustrert over ble ED50av 4-(4'-fluorfenoksy)benzaldehyd semikarbazon, både alene og i kombinasjon med gabapentin i et 1:1 forhold, ifølge beskrivelsen i WO 00/61188, evaluert etter oral behandling.
Verdiene for ED50for den nevnte semikarbazonet alene (14,5 mg/kg - CL: 11,9 - 15,8) og i kombinasjon med gabapentin (13,2 mg/kg - CL: 12,1 - 15,3) ble funnet å ikke være signifikant forskjellige.
I lys av det foregående kan det derfor noteres at ikke alle natriumkanalblokkerings-midler, som er aktiv i en modell for kronisk smerte, slik det er vist ved sammenlignings-data som er illustrert over, er synergistiske med GBP, hvilket strider i mot det som tilfeldig og signifikant ble funnet for a-aminoamidene NW-1029, NW-1037 og NW-1043 i sammensetningen ifølge oppfinnelsen.

1.
Farmasøytisk sammensetning som omfatter gabapentin eller pregabalin eller et farma-søytisk akseptabelt syreaddisjonssalt med uorganiske eller organiske syrer derav og et a-aminoamid valgt fra gruppen bestående av (S)-(+)-2-[4-(2-fluorbenzoyloksy)benzyl-amino]propanamid, (R)-(-)-2-[4-benzyloksybenzylamino]-3-fenyl-N-metylpropanamid og (S)-(+)-2-[4-(3-fluorbenzoyl)-benzylamino]-N-metylpropanamid, som kan foreligge enten som en enkel isomer eller som en blanding derav, eller et farmasøytisk akseptabelt syreaddisjonssalt med uorganiske eller organiske syrer derav;
der a-aminoamidet og gabapentin eller pregabalin eller de farmasøytisk akseptable syreaddisjonssaltene med uorganiske eller organiske syrer derav, foreligger i et forhold basert på en fraksjon av deres respektive ED50-verdier, der forholdet strekker seg fra henholdsvis 1:1 til 30:1 eller fra 1:1 til 1:30.

Farmasøytisk sammensetning ifølge krav 1, der a-aminoamidet og gabapentin eller pregabalin er i et forhold basert på en fraksjon av deres respektive EDso-verdier, der forholdet strekker seg fra henholdsvis 1:1 til 9:1 eller fra 1:1 til 1:9.
