Teknisk felt
Oppfinnelsen vedrører en nye sammensetning for behandlingen av sykdomstilstander hvilke er betegnet som "infant respiratory distress syndrome" (IRDS) og "acute" eller "adult respiratory distress syndrome" (ARDS) som angitt i krav 1 samt anvendelse av sammensetningen som angitt i krav 5.
Teknikkens stand
"Adult respiratory distress syndrome" (ARDS) er et beskriv-ende uttrykk hvilket anvendes for et stort antall akutte, diffuse infiltrerende pulmonallesjoner av forskjellig etiologi hvis de er forbundet med en alvorlig gassutbyt-tingsforstyrrelse (spesielt arteriell hypoksemi). Uttrykket ARDS anvendes på grunn av de tallrike kliniske og patologiske trekk vanlige ved "infant respiratory distress syndrome" (ARDS). Hvis, i tilfellet av IRDS, mangel på lungesurfaktant forårsaket av prematur fødsel er dominer-ende, så er i tilfellet med ARDS en lungesurfaktantsvikt forårsaket av lungetilstanden basert på forskjellige etiologier.
Utløsende årsaker til ARDS kan for eksempel være (sitert i overensstemmelse med Harrisons "Principles of Internal Medicine" 10. utg. red. 1983 McGraw-Hill Int. Book Comp) diffuse pulmonale infeksjoner (feks. på grunn av viruser, bak-terier, sopp), aspirasjon av for eksempel magesaft eller i tilfellet av nesten-drukning, inhalasjon av toksiner eller irriterende stoffer (feks. klorgass, nitrogenoksider, røyk), direkte eller indirekte trauma (feks. multiple frak-turer eller pulmonalkontusjon), systemiske reaksjoner på inflammasjoner utenfor lungene (feks. hemoragisk pankrea-titt, gram-negativ sepsis), transfusjon av store blodvol-umer eller alternativt etter kardiopulmonal bypass.
Med en mortalitet på 50-60 % {statistisk undersøkelse i Schuster Chest 1995, 107:1721-26) er fremdeles prognosene til en ARDS-pasient å betegne som ugunstige.
ARDS-terapien består hovedsakelig i den tidligst mulige anvendelse av forskjellige former for ventilasjon [feks. PEEP(positivt ende-ekspiratorisk trykk), å heve oksygen-konsentrasjonen i respirasjonsluften, SIMV (synkronisert intermittent pålagt ventilasjon; Harrisons "Principles of Internal Medicine" 10. utg. red. 1983 McGraw-Hill Int. Book Comp)] opp til ekstrakorporal membranoksygenering (ECMO; Zapol og Lemaire, "Adult Respiratory Distress Syndrome", Marcel Dekker Inc. 1991).
Den spesifikke anvendelse av forskjellige ventilasjonstek-nikker har bare ført til en liten senkning av mortalitet og inkluderer risikoen for å sette en ond sirkel i bevegelse. Ved ventilasjon med trykk og høy Fi02 (fraksjon av innåndet oksygen; del av oksygen i respirasjonsluften), kan lungene i seg selv bli skadet og som et resultat av dette kan enda høyere trykk og høyere FiC>2 være krevet for å oppnå en ade-kvat oksygenering av blodet.
I dag er forskjellige farmakologiske tilnærmingsmåter til løsningen som følger. Disse inkluderer lungesurfaktantsub-stitusjon [statistisk undersøkelse, for eksempel B. Lach-mann, D. Gommers og E.P. Eijking: "Exogenous surfactant therapy in adults", Atemw.-Lungenkrkh. 1993, 19:581-91; T.J. Gregory et al.: "Survanta suplementation in patients with acute respiratory distress syndrom (ARDS)", Am. J. Respir. Crit. Care Med. 1994, 149:A567] opp til ren anti-inflammatorisk terapi med for eksempel prostaglandin Ei (PGEi; Abraham et al., Crit Care Med 1996, 24:10-15) eller glukokortikosteroider (Bernard et al., N Engl Med 1987, 317: 1565-70). Selv om spesifikke suksesser ble oppnådd ved administrasjonen av lungesurfaktant (feks. Walmrath et al., Am J Resp Crit Care Med 1996, 154:57-62), førte de rene antiinflammatoriske terapier til få eller ingen suksesser. Dette er i direkte kontrast med de patologiske og histo-patologiske funn i ARDS. Således ble massive polymorfo-nukleære levkocytt-infiltrasjoner (statistisk undersøkelse, for eksempel Thiel et al., Anesthesist 1996, 45:113-130) funnet i lungene og utskyllingen av pasienter med ARDS og en rekke inflammatoriske mediatorer er detékterbare. Ved testing er i tillegg PGEi tilstede i en liposomal intra-venøs administrasjonsform (Abraham et al., Crit Care Med 1996, 24:10-15) samt substanser som søker inhiberingen av fosfatidsyrer (feks. Lisofyllin, Rice et al., Proe Nati Acad Sei 1994, 91:3857-61) eller rekombinant human inter-leukin 1(IL-1)-reseptor-antagonister (Fisher et al., JAMA 1994, 271:1836-43). Både PGEi og IL-l-reseptor-antagonisten er imidlertid begrenset av bieffekter i sin terapeutiske anvendbarhet.
WO96/09831 indikerer sammensetninger for behandlingen av ARDS og IRDS som inneholder et glukokortikosteroid og en lungesurfaktant. EP-B-0 451 215 beskriver sammensetninger for administrasjonen av en farmasøytisk aktiv forbindelse via lungene. Disse sammensetninger inkluderer liposomer som inneholder en farmasøytisk aktiv forbindelse og et lungesurfaktantprotein. Disse systemer er også foreslått for behandlingen av ARDS og IRDS. EP-B-0 055 041 beskriver preparater for inhalasjon eller infusjon for behandlingen av forstyrrelser i respirasjonsorganene, som inneholder en aktiv forbindelse mot forstyrrelser i respirasjonsorganene og naturlig lungesurfaktant. Sammensetninger for behandlingen av ARDS og IRDS er ikke brakt for dagen.
Beskrivelse av oppfinnelsen
Det har nå overraskende blitt funnet at ved administrasjonen av en kombinasjon av N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamid og lunge-surf akt ant kan en synergistisk effekt oppnås i behandlingen av IRDS og ARDS.
Oppfinnelsen vedrører derfor en sammensetning for behandlingen av IRDS og ARDS omfattende N-{3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamid og/eller dets farmakologisk tolerable salter og en lungesurfaktantsammensetning inneholdende fosfolipider og lungesurfaktantprotein C (SP-C) og/eller dets modifiserte derviater.
Ytterligere utførelser av oppfinnelsen følger av patent-kravene.
Fremstillingen av N-(3,5-diklorpyrid-4-yl)-3-cyklopropyl-metoksy-4-difluormetoksybenzamid og anvendelse som en fos-fodiesterase(PDE) IV-inhibitor er beskrevet i WO 95/01338. Farmakologisk tolerable salter av N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamid som kan nevnes er for eksempel vannløslige og vannuløselige syre-addisjonssalter med syrer slik som for eksempel saltsyre, hydrobromsyre, fosforsyre, salpetersyre, svovelsyre, eddik-syre, sitronsyre, D-glukonsyre, benzosyre, 2-(4-hydroksy-benzoyl)benzosyre, smørsyre, sulfosalisylsyre, maleinsyre, laurinsyre, eplesyre, fumarsyre, ravsyre, oksalsyre, vin-syre, embonsyre, stearinsyre, toluensulfonsyre, metansul-fonsyre eller 3-hydroksy-2-naftosyre, hvor syrene anvendes i saltfremstillingen - avhengig av om det er en mono- eller polybasisk syre og avhengig av hvilket salt som er ønsket - i et ekvimolart kvantitativt forhold eller ett som skiller seg derfra.
Lungesurfaktant forståes i henhold til oppfinnelsen som å bety de tallrike kjente sammensetninger og deres modifika-sjoner som har funksjonen til naturlig lungesurfaktant. Naturlig lungesurfaktant har overflateaktive egenskaper og reduserer overflatespenningen i alveoleregionen av lungene. En enkel og rask kvantitativ in vitro-test for å bestemme surfaktantaktiviteten til surfaktant preparater er feks. Wilhelmy-balansen [Goerke, J Biochim Biophys Acta, 344:241-261 (1974); King R.J. og elements J.A., Am J Physiol 223: 715-726 (1972)]. Den gir en indikasjon på surfaktantkvali-tet uttrykket som evnen til å nærme seg en overflatespen-ning på nærmere null mN/m. Den utføres ved å injisere en surfaktantsuspensjon i definerte konsentrasjoner på fosfolipider inn i en vannholdig løsning. Fosfolipidene spredd på luft/væskefasen danner et såkalt monolag. Dette monolag reduserer overflatespenningen i den vannholdige løsning. En platinaplate dyppes forsiktig i løsningen. Nå kan kraften hvilken trekker ned platinaplaten måles med sensitive transdusere. Denne kraft er proporsjonal med overflatespenningen og avhenger av dimensjonene på platinaplaten. En annen metode for å beskrive surfaktantpreparaters over-flateaktivtet er det pulserende boble-surfaktometer [Poss-mayer F., Yu S. og Weber M., Prog Resp Res, Ed.v. Wichert, Vol 18:112-120(1984)]. Aktiviteten til et surfaktant-preparat kan også vurderes ved en in vivo- test, for eksempel som beskrevet under i farmakologidelen. Måling av lungefunksjon, blodgasser og ventilatortrykk vil frembringe aktivitetsindekser.
Lungesurfaktant skal forståes i henhold til oppfinnelsen som sammensetninger som vil vise aktivitet i en slik test. Spesielt kan nevnes sammensetninger hvilke vil vise en aktivtet i en slik test tilsvarende eller større enn den av naturlig, spesielt human, lungesurfaktant.
Spesielt omfatter lungesurfaktantsammensetninger fosfolipider og kan inter alia i tillegg inneholde lungesurfaktantproteiner. Kommersielt tilgjengelige produkter som kan nevnes er Curosurf® (Serono, Pharma GmbH, Untersc-hleissheim), en godt renset naturlig surfaktant fra homoge-niserte griselunger, Survanta® (Abbott GmbH, Wiesbaden) og Alveofact® (Dr. Karl Thomae GmbH Biberach), begge ekstrak-ter fra bovine lunger, også Exosurf® (Deutsche Wellome GmbH, Burgwedel), et syntetisk fosfolipid med hjelpestoffer. Mulige lungsurfaktantproteiner er både proteiner erholdt fra naturlige kilder, slik som for eksempel pulmonal utskylling eller ekstraksjon fra amniotisk fluid og også de genetisk konstruerte proteiner. I henhold til oppfinnelsen er lungesurfaktantproteinene betegnet SP-B og SP-C og deres modifiserte derivater av spesiell interesse. Aminosyresekvensene til disse lungesurfaktantproteiner, deres isolering eller fremstilling ved genetisk konstruk-sjon er kjent (feks. fra WO86/03408, EP-A-0 251 449, WO89/04326, WO87/06943, WO88/03170, EP-A-0 368 823 og EP-A-0 348 967}. Modifiserte derivater av SP-C som skiller seg fra human SP-C ved utbytting av visse aminosyrer er brakt for dagen i for eksempel WO91/18015 og W095/32992. Spesielt kan nevnes SP-C-derivatene brakt for dagen i W095/32992. I henhold til oppfinnelsen skiller et spesielt foretrukket rekombinant SP-C-derivat [heretter referert til som r-SP-C (FF/I)] seg fra humant SP-C ved utbytting av de to cystei-ner i stilling 4 og 5 med fenylalanin og utbytting av metioninen i stilling 32 med isoleucin. EP-B-0 100 910, EP-A-0 110 498, EP-B-0 119 056, EP-B-0 145 005 og EP-B-0 286 011 beskriver fosfolipidsammensetninger med og uten lungesurfaktant-proteiner som er egnet, for eksempel som komponenter av preparatene i henhold til oppfinnelsen.
Oppfinnelsen vedrører ytterligere anvendelsen av sammensetningene i henhold til oppfinnelsen for fremstilling av medikamenter hvilke anvendes for behandlingen og/eller profylaksen av IRDS eller ARDS.
Oppfinnelsen vedrører videre medikamenter for behandlingen og/eller profylaksen av de nevnte sykdommer, hvilke inneholder sammensetningene i henhold til oppfinnelsen.
Sammensetningene i henhold til oppfinnelsen gjøres tilgjengelige enten i flytende form for intratrakal eller intrabronkial administrasjon eller i pulverform for administrasjon ved inhalasjon. Sammensetningene fremstilles ved prosedyrer kjent for fagmannen, hvis hensiktsmessig ved å bruke egnede farmasøytiske hjelpestoffer. En pulverform erholdes, for eksempel ved å blande flytende lungesurfak-tantpreparåter, feks. vandige suspensjoner med vandige suspensjoner av N-(3,5-diklorpyrid-4-yl)-3-cyklopropyl-metoksy-4-difluormetoksybenzamid og deretter lyofilisere og mikronisere den. Alternativt kan en løsning av en lungesurfaktant og N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmet-oksy-4-difluormetoksybenzamid lyofiliseres i et egnet løse-middel, slik som for eksempel tert-butanol, og deretter mikroniseres. Spraytørking av en blanding av en vandig lungesurfaktantsuspensjon og en vandig N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamidsuspen-sjon eller en løsning av en lungesurfaktant og N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluor-metoksybenzamid i egnede løsemidler, slik som alkoholer (feks. metanol, etanol, 2-propanol), kloroform, diklor-metan, aceton og deres blandinger, som valgfritt i tillegg kan inneholde vann fører til pulveriserte preparater. Administrasjon ved inhalasjon kan også utføres ved å atomisere løsninger eller suspensjoner som inneholder sammensetningene i henhold til oppfinnelsen. Sammensetninger i henhold til oppfinnelsen inneholder fordelaktig 1 til 30 vekt% N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamid.
Under er eksempelvis beskrevet fremstillingen av et pulver-preparat laget ved spraytørking.
Eksempel 1
8,2 g l,2-dipalmitoyl-3-sn-fosfatidylkolin, 3,46 g 1-palmi-toyl-2-oleoyl-3-sn-fosfatidylglyserolammonium, 2,7 g N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluor-metoksybenzamid, 0,56 g palmitinsyre, 0,3 g kalsiumklorid og 0,2 g r-SP-C (FF/I) løses i 700 ml 2-propanol/vann (90:10) og spraytørkes i en Btichi B laboratorium spray-tørker. Spraybetingelser: tørkegass nitrogen, innløpstem-peratur 110°C, utløpstemperatur 59-61°C. Et fint, kremfar-get pulver oppnås.
Eksempel 2

Eksempel 4
10,0 g 1,2-dipalmitoyl-3-sn-fosfatidylkolin, 2,6 g N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksy-benzamid, 0,74 g tyloksapol og 1,1 g 1-heksadekanol løses i 500 ml 2-propanol/vann (90:10) og spraytørkes i en Buchi B 191 laboratorium spraytørker. Spraybetingelser: tørkegass nitrogen, innløpstemperatur 110°C, utløpstemperatur 58-60°C. Et hvit til offwhite pulver oppnås.
Under er eksempelvis beskrevet fremstillingen av en lyofilisert sammensetning.

8,0 g av en renset lungesurfaktant fra bovine lunger og 2,0 g N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluor-metoksybenzamid tilsettes til 90 ml vann. Suspensjonen erholdes lyofilisert ved -25°C for å gi et lyst, offwhite, fluffy materiale.
Oppfinnelsen vedrører ytterligere en fremgangsmåte for behandlingen av pattedyr, inklusive mennesker, som lider av IRDS og ARDS- Fremgangsmåten karakteriseres ved at en tera-peutisk aktiv og farmakologisk tolerabel mengde av sammensetningen i henhold til oppfinnelsen administreres til det syke pattedyr.
Oppfinnelsen vedrører ytterligere sammensetningen i henhold til oppfinnelsen for anvendelse i behandlingen av IRDS og
ARDS.
Preparatene i henhold til oppfinnelsen administreres 3 til 4 ganger daglig i 2 til 4 dager. For eksempel administreres preparater omfattende 4 mg N-(3,5-diklorpyrid-4-yl)-3-cy-klopropylmetoksy-4-difluormetoksybenzamid og 50 mg fosfolipider 6 ganger i et intervall på 6 timer ved inhalasjon eller intratrachealt eller intrabronkialt.
Farmakologi
Voksene Sprague Dawely rotter ventileres kunstig med rent oksygen og et positivt ende-ekspiratorisk trykk (PEEP; for å garantere oksygenering av rottene) og utskylles inntil deres endogene surfaktant er vasket ut (D. Hafner, U. Kilian og R. Beume: "Comparison of four lung surfaktant preparations in an animal model of adult respiratory distress syndrome (ARDS)". Am. Rev. Respir. Dis. 1993, 147:A719; D. Håfner, P.-G. Germann, D. Hauschke, Pulmonary Pharmacology (1994) 7, 319-332). Dette manifisteres ved det faktum at de primære verdier for det arterielle partielle oksygentrykk (Pa02) på 500-550 mmHg (i tilfellet med ren oksygenventilasjon og PEEP) synker til verdier på 50-110 mmHg i dyrene. Dyr i kontrollgruppen, som ikke behandles med lungesurfaktant, forblir ved sine Pa02 på disse lave verdier gjennom observasjonsperioden. Seksti minutter etter at Pa02 har sunket til disse verdier inngis lungesurfaktant sammen med N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamid intratrachelt. Blodgassene bestemmes 30 og 120 minutter etter inngivelse.
I tabell 1 og tabell 2 som følger, er i linje A gjennom-snittsverdiene {± standardavvik) av Pa02 indikert i mmHg for tiden 30 minutter (konstant PEEP på 8 cm H20) etter intratracheal inngivelse og linje B 120 minutter etter intratracheal inngivelse. Det kan sees fra tabell 1 at hele administrasjonen av N-(3,5-diklorpyrid-4-yl)-3-cyklopropyl-metoksy-4-difluormetoksybenzamid ikke har noen innvirkning på PaC>2. Dette følger ved sammenligning med de ubehandlede kontrolldyr. Administrasjonen av lungesurfaktant (25 eller 100 mg/kg) fører til en stigning i Pa02 (lungesurfaktanten anvendt tilsvarer en sammensetning i henhold til eksempel 1 uten N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-di-fluormetoksybenzamid). Tilsettingen av 600 ug/kg eller 6,0 mg/kg N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-di-fluormetoksybenzamid til den respektive lungesurfaktant forbedrer Pa02-verdiene sammenlignet med de respektive lungesurfaktant doser. Det følger fra dette at den sammen-føyde administrasjon av N-(3,5-diklorpyrid-4-yl)-3-cyklo-propylmetoksy-4-difluormetoksybenzamid og lungesurfaktant fører til en uventet ekstra tilleggs virkning. Det er derfor mulig å spare en del av den svært dyre lungesurfaktant, eller ellers å oppnå en økt virkning av hver individuelle komponent.<*> forb. De histologiske opparbeidinger av lungene til disse dyrene utført i forbindelse med eksperimentet viste en kraftig dannelse av såkalte hyalin-membraner (HM) og en kraftig innstrømning av inflammatoriske celler [feks. polymorfo-nukleaere nøytrofile levkocytter (PMNL) ] som et uttrykk for utviklingen av et "acute respiratory distress syndrome".
Ved undersøkelsen av preparater i henhold til oppfinnelsen omfattende N- (3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamid og lungesurfaktant (fosfolipid-blanding) med eller uten surfaktantproteiner i denne modell, ble det funnet at oksygeneringen og de histologiske endringer (inhibering av dannelsen av HM og inhibering av innstrømningen av PMNL) forbedrer superaddetivt sammenlignet med hele administrasjonen av lungesurfaktant eller N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluor-metoksybenzamid. Det følger av dette at som et resultat av denne uventede synergistiske effekt kan behandlingen av IRDS og ARDS forkortes og den høye mortalitet, som ledsager disse syndromer, kan reduseres.
Claims (5)
1. Sammensetning for behandlingen av IRDS og ARDS omfattende N-{3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-di-fluormetoksybenzamid og/eller dets farmakologisk tolerable salter og en lungesurfaktantsammensetning inneholdende fosfolipider og lungesurfaktantprotein C (SP-C) og/eller dets modifiserte derivater.
2. Sammensetning i følge krav 1, hvori det modifiserte derivat av SP-C er r-SP-C (FF/I).
3. Sammensetning i følge et hvilket som helst av kravene 1 til 2, hvori SP-B og/eller dets modifiserte derivater i tillegg inneholdes.
4. Sammensetning i følge krav 1, hvor lungesurfaktant-sammensetningen erholdes ved pulmonal utskylling.
5. Anvendelse av en sammensetning ifølge et hvilket som helst av kravene 1 til 4 for fremstilling av et medikament nyttig for behandlingen av IRDS og ARDS.
