Description
Foreliggende oppfinnelse vedrører anvendelse avl,8-oksido-p-mentan eller mentan-3-ol for fremstilling av medikamenter i form av kapsler resistente overfor magesaft, infusjonsløsninger eller som løsninger for intramuskulær injeksjon for behandling av bronkial astma.
Akutte og/eller kroniske inflammatoriske eller akutte allergiske og/eller kroniske allergiske inflammatoriske lesjoner er karakterisert ved inflammatorisk infiltrasjon av forskjellige organsystemer med monocytter/monofager, eosinofiler, basofiler og neutrofiler, granulocytter, mastceller og trombocytter. Infiammasjons-gradaktiviteten korrelerer med inflytelsen av disse inflammasjonsceller i organvev, hvilket forårsaker skade på det berørte organ. Disse prosesser er et kjent trekk i primære inflammatoriske sykdommer i luftveiene, fordøyelsen og leddbrusk ved rheumatoid artritt. Da den primære årsak som utløser de fleste kroniske betennelses-sykdommer ikke er kjent, kan disse sykdommer bare mildnes ved ikke-spesifikk undertrykkelse av betennelsesstimulus. Ved en tilsvarende genetisk predisponering eller ved nærvær av miljøgifter, kan legemets reaksjon på den eksogene gift profylaktisk hemmes ved ikke-spesifikk undertrykkelse av betennelesstimulus før manifesteringen av sykdomssymptomene.
På cellenivået kan den videre vandring av inflammatoriske celler inn i et betennelsesområde reduseres ved å hemme chemotaktiske faktorer. Dette fører til undertrykkelse av betennelesesaktiviteten med reduksjon av morfologiske og funksjonelle forstyrrelser av det berørte organsystem, en virkning som typisk bevirkes med immunosuppresanter, f.eks. corticosteroider. Denne gruppe midler er kjent å ha en aktivitetsprofil som omfatter en sterkt anti-inflammatorisk effekt, men bare tolereres dårlig, da den forårsaker alvorlige bivirkninger, nemlig osteoporose, gastriske og duodenale sår, steroid purpura og lymfopeni.
Corticosteroider er kjent å hemme aktiviteten til fosfolipase og cytokiner i forskjellige betennelsesceller. Etter hemning av fosfolipaseaktiviteten, inhiberes frigjøring av arachidonsyre fra fosfolipidlagrene i cellemembranen. Arachidonsyre er kjent å være et viktig substrat for dannelsen av forskjellige mediatorer.
Den faktiske inhiberingsvirkning på inflammasjon bevirkes med inhibering av cytokinproduksjon og via en redusert produksjon av arachidonsyre. Den sistnevnte er en forløper for dannelsen av de sterke chemotaktiske metabolitter i 5-lipoksygenase-veien (leukotriener) og cyklooksygenaseveien (prostaglandiner, tromboksan) med en sammentrekningsvirkning på glatte muskelceller. På grunn av den vedvarende suppresjon, spesielt av mediatorer for 5-lipoksygenasemetabolisme-veien, reduseres derfor behovet for systemisk aktive corticosteroider, og aktiviteten til den inflammatoriske prosess undertrykkes ved synkende infiltrasjon med betennelesesceller.
Av denne grunn kan sykdomsforløpet av forskjellige akutte, kroniske eller allergiske betennelser påvirkes fordelaktig ved ikke-spesifikk inhibering av inflammasjon.
De mettede, monocykliske og bicykliske terpenstrukturer som kan brukes ifølge oppfinnelsen innbefatter bl.a. monocykliske terpener av mentoltypen ((-)mentan-3-ol)
og de åtte stereoisomerer samt 1,8-terpin, menton (p-mentan-3-on) som eksempler på terpenalkoholer og terpenketoner samt terpentioler og de bicykliske terpener. Disse innbefatter 1,8-cineol, karan, pinan, bornan og deres oksoderivater karon, kamfer, borneol og de respektive stereoisomere. Terpenforbindelsene ifølge foreliggende oppfinnelse er egnet for behandling av inflammatoriske bronkopulmonære sykdommer som krever steroidbehandling og/eller som forverres av infeksjoner.
Overraskende ble det funnet at fra gruppen terpenstrukturer ifølge opp-
finnelsen, er 1,8-cineol egnet forbehandling av inflammatoriske, allergiske og/eller inflammatoriske bronkopulmonære sykdommer som krever steroid behandling og forverres ved infeksjon. Gruppen reduserer produksjonen av arachidonsyre. På denne måten undertrykker den arachidonmetabolisme i 5-lipoksogenaseveien. I tillegg ble en inhibering av interleukin 16 (ILi-6) funnet. Interleukin 16 far menneskeceller til å frigjøre arachidonsyre og er kjent for å være en sterk proinflammasjonsmediator selv. Virkningene kan tilskrives dens lipofile egen-skaper, dvs. diffusjon og lagring i fett-vev. Dette fører til en ny indikasjon for gruppen av substanser som også innbefatter 1,8-cineol, spesielt for behandling av inflammatoriske sykdommer eller inflammatoriske allergiske sykdommer som krever steroid behandling og forverres ved infeksjon f.eks. bronkopulmonære tilstander. For eksempel kan bruken av corticosteroider
reduseres vesentlig som følge av virkningsmåten til 1,8-cineol. Langtidsterapi anbefales derfor.
På grunn av den synergistiske effekt, anbefales en kombinasjonsbehandling omfattende 1,8-cineol og de vanlige corticosteroider prednison, prednisolon, fluocortolon, beklometason, budesonid eller flunisolid.
Cineol er hovedbestanddelen (ca. 80%) av eukalyptusoljene fra forskjellige typer av Eucalyptus globulus-sorten. 1,8-oksidomentan er en bicyklisk eter som har et spenningsfritt ringsystem og kalles 2-oksa-bicyklo/2,2,2/oktan ifølge IUPAC. Det er en fargeløs væske med en aromatisk, kamferlignende lukt som også er tilstede i salvie, myrt, eukalyptol og andre eteriske oljer.
Doseringen varierer i de mettede monocykliske og bicykliske terpenstrukturer, men er lavere enn dosene hvorved skadelige bivirkninger eller til og med forgiftninger manifesteres. Således kan en dose på 200 mg pr. dag til 900 mg pr. dag (fortrinnsvis 600 mg pr. dag) gis i form av mavesaftresistente kapsler med en doseenhet på 100 mg pr. kapsel. 1,8-cineol og mentol er egnet som additiver for langtidsintravenøs infusjon og for intramuskulær administrering. Cineol og mentol kan kombineres med andre substanser.
Dosen av mentol er mellom 100 mg/dag og 450 mg/dag. For eksempel administreres 1,8-cineol for formålet ved oppfinnelsen i en dose på 200 mg/dag til 900 mg/dag (fortrinnsvis 600 mg/dag). Den totale mengde av de nevnte substanser oppdeles hensiktsmessig i 3 doser pr. dag.
De medisinske preparatformer kan være faste eller flytende. I tillegg er det mulig å gi det aktive reagens med bærere, fortynningsmidler og additiver som er vanlige i biofarmasøytika, er farmakologisk harmløse og er forenlige med 1,8-cineol. Additiver innbefatter fyllstoffer, dispergeringsmidler, bindemidler, fuktemidler, stabilisatorer, smøremidler, emulgatorer, søtningsmidler, smaksstoffer o.l. Disse additiver innbefatter f.eks. melantinløsninger, pektinløsninger, laktose, natriumklorid, talkum, stivelse, borsyre, parafinolje, parafin, stearinsyre eller dens derivater, kakao-smør, gummi, sirup, lakrisekstrakter, gjærekstrakter, honning, glycerol, kiseljord, kaolin, magnesiumoksyd, bi voks og planteoljer.
I flytende preparatformer kan vann, glycerol, sukker eller alkoholløsnmger eller blandinger av disse brukes som bærer og hjelpemiddel. Metodene for optimalisering av formuleringen er kjente for spesialisten (Chemie in unserer Zeit 23, 114 og 161
(1989)).
Eksempel 1
Det ble ved in vitro undersøkelser funnet at virkningen av cineol er basert
på inhiberingen av arachidonsyrefrigjøring fra fosfolipidlagrene i nesten alle menneskeceller. Følgelig er bare reduserte mengder av arachidonsyre (som substrat for 5-lipoksygenase og cyklo-oksygenase metabolske veier) tilgjengelige for mediator-dannelse.
I disse eksperimenter kunne en betydelig undertrykkelse av monocytt- LTB4 produksjon ex vivo med forbedring av lungefunksjon påvises etter 4 dagers terapi hos pasienter med allergisk og ikke-allergisk bronkial astma (n=7) (se Tabell I, fig. 1-4). 50 ml veneblod ble tatt fra pasienter med allergisk og ikke-allergisk bronkial astma før terapi, etter 4 dagers terapi og 4 dager etter avbrudd av cineol-administrering (dag 8). Monocytter ble isolert ved hjelp av densitetsgradient-sentrifugering og renset ved ytterligere sentrifugering. 50.000 celler ble stimulert i 1 ml kultursupernatant i plastrør i 30 minutter med kalsiumionofor A23187 i et vannbad ved 37°C. Etter inkubering ble rørene som inneholdt cellene sentrifugert i 5 minutter ved 4°C, og de oppnådde kultursupernatanter ble lagret ved -80°C inntil videre analyse. Etterpå ble kultur-supernatantene tint og LTB4 bestemt med en spesifikk enzymimmunomåling.
Etter behandling av de samme pasienter med 3 x 200 mg cineol i kapsler i 4 dager, ble blod tatt, og den samme analyse for TLB4 ble gjentatt. Behandlingen med cineol ble så avbrutt, og den samme undersøkelse ble gjentatt etter 4 dager til (dag 8). Resultatene er vist i Figur 3 og 4: i alle tilfeller bragte cineol en betydelig reduksjon av LTB4 produksjon. Imidlertid hadde denne steget igjen til begynnelsesverdien etter avbrudd med cineol (dag 8). Tabell I i forbindelse med Figur 1 og Figur 2 viser de kliniske data. FeVi er den påtvungne 1 sekund kapasitet, og RAW er luftveismot-standen.
LTB4 produksjonen i monocytter av syv friske forsøkspersoner (25 ± 2 år) før og etter den samme cineolbehandling ble også brukt til sammenligning (se Figur 5). Sammenlignet med pasienter med astma, hadde LTB4 produksjonen også falt hos friske pasienter, slik at en generell virkning må antas. Figur 6 viser at cineol alene også inhiberer kalsiumionofor A23187-stimulert LTB4 produksjon hos monocytter. Følgelig er cineol i seg selv også egnet for behandling av inflammatorisk bronko-pulmpnær sykdom forverret med infeksjoner.
De høyre spalter i Figur 5 viser den uventede synergistiske virkning av 1,8-cineol i behandlingen av bronkialastma pasienter ifølge oppfinnelsen, fordi selv kortisonbehandling ikke kunne redusere LTB4 produksjon til normal, mens simultan cineolbehandling bringer LTB4 produksjonen inn i det normale området, slik at kortison kan innspares i en tilsvarende grad.
Det samme ble påvist ved langtidsterapi (43 ± 7dager) av pasienter med bronkialastma som krever steroid administrering (n = 5). Disse profitterer på en betydelig reduksjon av steroidbehovet og en forbedring av lungefunksjonen under cineol (3 x 200 mg/dag; Tabell II). Den A23187 stimulerte LTB4 produksjon fra monocytter ex vivo steg heller ikke etter reduksjon av systemisk glucokortikosteroid terapi under langtids cineolbehandling.
Kortikosteroidbehovet før cineol var gjennomsnittlig 8.9 ± 3,3 mg/dag (området 0-24 mg) og kunne reduseres etter 43,5±7,3 dager til 4,0±1,3 mg/dag (område 0-8 mg/ dag). Reduksjonen av steroidterapi hadde ingen negativ virkning på lungefunksjonen. FeVi før cineol (1,83 ± 0,261) steg til 2,7 ± 0,55 1( p = 0,0431) etter langtids terapi (n = 6 forsøkspersoner). Motstanden før cineolbehandling (5,49 ± 1,02 cm H2O/I x sek.) hadde fallt til 3,98 ± 1,2 cm H20/1 x sek. etter langtidsterapi med cineol (p = 0,0273). LTB4 produksjon for cineol hadde også falt etter langtidsterapi. Resultatene av langtidsterapi viser for første gang at det er en markert reduksjon i det daglige steroidbehov under terapi med cineol uten forsemring av lungefunksjonen. Den gjennomsnittlige LTB4 produksjonen under cineol etter langtidsterapi er markert undertrykket. Reduksjonen av systemiske kortikosteroider under langtidsterapi med cineol fører bare til en stigning av LTB4 produksjonen hos pasienter, hvilke som et resultat av en høy steroid-dose hadde vist undertrykking av LTD4 produksjon in vitro før cineoladministrering.
Lipopolysakkarid-(LPS)-stimulert dannelse av IL 1-6 fra monocytter hos friske individer (n = 4) ble bestemt før cineol ble tatt, etter behandling i 4 dager med 3x2 kapsler ( = 3 x 200 mg cineol/dag) og fire dager etter avbrudd med cineol (dag 8). I alle de undersøkte forsøkspersoner ble IL 1-6 produksjon dag 4 og 8 inhibert med et gjennomsnitt på 60%. Da produksjonen av IL1-6 fortsatt ble undertrykket 4 dager etter avbrudd med cineol (= dag 8), må en sterkt anti-inflammatorisk virkning antas som er meget egnet for profylakse (f.eks. hos pasienter med bronkial astma og hos barn) på grunn av langtidsvirkningen.
Eksempel 2
Friske forsøkspersoner ble behandlet i 4 dager med 3 x 150 mg mentol pr. dag p.o (doseenhet =150 mg/kapsel). Etter fire dagers behandling ble kalsiumionofor A23187-stimulert LTB4 produksjon av isolerte monocytter undertrykket med opp til 75%.
Hos pasienter med bronkial astma kunne en forbedring av lungefunksjonen oppnås som under cineolbehandling etter 4 dagers terapi med den ovennevnte dose. Etter langtidsterapi i 12 uker, stabiliseres bronkialastmaen i slik grad at det systemiske kortikosteroidbehov kunne reduseres med opp til 50%. Behandlingen ble godt tolerert uten bivirkninger.
Claims (9)
1. Anvendelse av 1,8-oksido-p-mentan eller mentan-3-ol for fremstilling av medikamenter i form av kapsler resistente overfor magesaft, infusjonsløsninger eller som løsninger for intramuskulær injeksjon for behandling av bronkial astma.
2. Anvendelse ifølge krav 1, for reduksjon av systemisk aktive kortikosteroider.
3. Anvendelse ifølge krav 1 eller 2, for monoterapi i mild eller lett bronkial astma samt for behandling av barn med astma.
4. Anvendelse for profylakse ifølge et av kravene 1 til 3.
5. Anvendelse ifølge kravene 1 til 4, i kombinasjon med systemisk aktive medikamenter, spesielt immunsuppressiver eller kortikosteroider.
6. Anvendelse ifølge et av kravene 1 til 5, med en total daglig inntaksmengde på 1,8-oksido-p-mentan eller mentan-3-ol i området fra 100 mg og 900 mg.
7. Anvendelse av 1,8-oksido-p-mentan for formålene ifølge krav 1, i en total inntaksmengde på mellom 200 mg og 900 mg, fortrinnsvis 600 mg.
8. Anvendelse av mentan-3-ol for formålene ifølge krav 1, i en total daglig inntaksmengde på mellom 100 mg og 450 mg.
9. Anvendelse ifølge et av kravene 1 til 8, med en doseenhet for oral terapi i form av kapsler resistente overfor magesaft på 100 mg.
