Description
Oppfinnelsen vedrører en farmasøytisk sammensetning for behandling av neonatalt åndenødssyndrom (IRDS) og akutt åndenødssyndrom hos voksne (ARDS) inneholdende en lungesurfaktant.
Teknikkens stand
Det er kjent at ved behandling av gravide kvinner som har en tendens til tidlige fødsler, kan med
glukokortokosteroider, (GCS), følgene av neonatalt åndenødssyndrom mildnes (Infant Respiratory Distress Syndrome, IRDS) hos deres nyfødte barn (f.eks. H. R. Gamsu, B.M. Mullinger, P. Donai og C. H. Dash: Antenatal administration of Betamethasone to prevent respiratory distress syndrome in preterm infants; report of a UK multicentre trial, Brit. J. Obst. Gyn. 1989, 96:410-10; Review: A.N. Papageorgiou og L. Stern: J. Perinat. Med. 1986, 14:75-85). Av denne grunn behandles de vordende mødre med GCS. Deretter forsøker man å utsette fødselen i minst 24 timer for å oppnå GCS' modnende effekt på lungene. I mange år har for tidlig fødte barn blitt behandlet med lungesurfaktanter (LSF) ved intratrakeal eller intrabronkial instillasjon for å forhindre og/eller behandle IRDS (A. Jobe og M. Ikegami: Surfaktant for the treatment of respiratory distress syndrome. Am. Rev. Respir. Dis. 1987, 136:1256-75; M. S. Reynolds og K. A. Wallander. Use of surfactant in the prevention and treatment of neonatal respiratory distress syndrome, Clin. Pharm. 1989, 8:559-76). I den senere tid har stadig flere grunnleggende studier blitt fremlagt hvor man har god erfaring med anvendelse av LSF for behandling av akutt åndenøds syndrom (ARDS) av annen type. (Overview e.g. B. B. Lachmann, D. Gommers and E. P. Eijking: Exogenus surfactant therapy in adults. Atemw. -Lungenkrkh. 1993, 19:581-91; T. J. Gregory et al.: Survanta supplementation in patients with acute respiratory distress syndrome (ARDS), Am. J. Respir. Crit. Care Med. 1994, 149:A567). Kortikostereoider blir anvendt mot ARDS med liten suksess (G. R. Bernard et
al.: High-dose corticosteroids in patients with the adult respiratory distress syndrome, N. Engl. J. Med. 1987, 3171565-70).
Beskrivelse av oppfinnelsen
Det er nå overraskende funnet at ved å anvende en kombinasjon av glukokortikostereoider og lungesurfaktanter kan det oppnås en synergistisk effekt ved behandling av IRDS og
ARDS.
Oppfinnelsens gjenstand er derfor en farmasøytisk sammensetning for behandling av IRDS og ARDS inneholdende minst et glukokortikortikosteroid og en lungesurf aktant.
Ytterligere utførelsesformer av oppfinnelsen fremgår av patentkravene.
Som glukokortikosteroider er de av interesse som egner seg for anvendelse i lungene. Som eksempel kan nevnes betametason, metylprednisolon, dexametason og ciclesonid.
Med lungesurf aktanter menes i henhold til foreliggende opp-finnelse de tallrike kjente sammensetninger som utviser samme funksjon som den naturlige lungesurfaktant. Med disse sammensetninger tenkes det særlig på fosfolipider, som blant annet også kan inneholde lungesurfaktantproteiner. Av de kommersielt tilgjengelige produkter kan nevnes Curo-surf®, (Serono, Pharma GmbH, 85716 Unterschleissheim), som er en høyrenset naturlig surfaktant fra homogeniserte svinelunger, Survanta® (Abbott GmbH, Wiesbaden) og Alveo-fact® (Dr. Karl Thomae GmbH, Biberach) , som begge er ekstrakter fra storfelunger, og Exosurf® (Deutsche Wellcome GmbH, Burgwedel) , et syntetisk fosfolipid inneholdende hjelpestoffer. Som lungesurfaktantproteiner kommer både de fra naturlige kilder, som f.eks. lungeskylling eller ekstraksjoner fra fostervann i betraktning, og også genteknisk fremstilte proteiner. Ifølge oppfinnelsen er særlig de lungesurfaktantproteiner som betegnes som SP-B og SP-C og deres modifiserte derivater av interesse. Amino-syresekvensene for disse lungesurfaktantproteiner, deres isolasjon eller gentekniske fremstilling er kjent (W0-86/03408, EP-A-0251449, WO-89/04326, WO-87/06943, WO-88/03170, EP-A-0368823 og EP-A-0348967). I EP-B-0100910, EP-A-0110498, EP-B-0119056, EP-B-0145005 og EP-B-0286011 er det beskrevet fosfolipidsammensetninger med eller uten lungesurfaktantproteiner som kommer i betraktning, f.eks. som komponenter i sammensetningene ifølge oppfinnelsen.
Sammensetningene ifølge oppfinnelsen kan anvendes enten i pulverform for inhalasjon eller i flytende form for intrarakeal eller intrabronkial administrasjon. Pulver erholdes ved å lyofilisere og deretter mikronisere flytende lungesurfaktantpreparater f.eks. før eller etter tilsetning av glukokortikosteroider. Sammensetninger ifølge oppfinnelsen inneholder 1-3 0 vekt% glukokortikosteroid (avhengig av virkningsstyrken av GCS; en tabell med relative verdier av virkningsstyrken for glukokortikosteroidenes finnes hos Goodman/Gillman, Pharmacological Basis of Therapeutics, Pergamon Press, s. 1447, 8. utg.) og 15-95 vekt% lungesurfaktant regnet på den tørre masse (f.eks. betametason 7% og LSF 92% eller metylprednisolon 37% og LSF 63%).
Sammensetningene ifølge oppfinnelsen blir administrert 3 til 4 ganger daglig i 2 til 4 dager. Eksempelvis blir sammensetninger inneholdende 4 mg betametason og 50 mg fosfolipider administrert ved inhalasjon eller intratrakealt eller intrakronkialt 6 ganger med 6 timers mellomrom.
Farmakologi
Voksne Sprague Dawley-rotter blir kunstig ventilert med rent oksygen og et positivt ekspiratorisk slutt-trykk (=PEEP; for å garantere at rottene blir oksygenisert) og skyllet flere ganger, til deres egen LSF er vasket ut (B. Lachmann, B. Robertson og J. Vogel: In vivo lung-lavage as an experimental model of the respiratory distress syndrome, Acta Anesth. Scand. 1980, 24:231-6; D. Håfner, U. Kilian og R. Beume: Comparison of four lung surfactant preparations in an animal model of adult respiratory distress syndrome (ARDS). Am. Rev. Respir. Dis. 1993, 147-A719: D. Håfner, P. G. Germann, D. Hauschke, Pulmonary Pharmacology (1994) 7, 319-332). Dette viser seg på den måte at dyrenes utgangs-verdi for det arterielle oksygen partialtrykk (Pa02) på 500-550 mmHg (ventilering med rent oksygen og PEEP) faller til verdier på 50-110 mmHg. Dyr i kontrollgruppen som ikke blir behandlet med LSF, forblir med sin Pa02 ved disse lave verdier utover undersøkelsesperioden. Fem minutter etter at Pa02 har falt til disse verdier, blir LSF eller LSF sammen med et glukokortikosteroid tilført dyrene dråpevis intrarakealt. Blodgassene bestemmes 5, 30, 60, 90 og 120 min etter tilførselen. Deretter blir PEEP redusert fra 8 til 6 cm H20 (første reduksjon av PEEP). 15 min senere blir PEEP redusert til 3 cm H20 (andre reduksjon av PEEP) . Blodgassene bestemmes 5 min etter begge reduksjoner av PEEP.
I den etterfølgende Tabell 1 er de midlere verdiene (± standard avvik) for Pa02 i mmHg over perioden 5 til 120 min (konstant PEEP på 8 cm H20) etter den dråpevise intrarakeale tilførsel angitt i linje A. Linje B viser de midlere verdier (± standardavvik) for Pa02 etter den intrarakeale tilførsel. I linje C er de midlere verdier for Pa02 (± standard avvik) under den andre PEEP-reduksjon etter tilførselen angitt. Fra tabellen kan det ses at tilførsel av glukokortikosteroid (her budesonid) alene ikke har noen innflytelse på Pa02. Dette fremgår ved sammenligning med de ubehandlede kontroildyrene. Tilførsel av LSF (25 eller 100 mg/kg) fører til en økning av Pa02. Tilsetning av 600 ug budesonid til hver dose av LSF forbedrer verdiene for Pa02 signifikant sammenlignet med de til-svarende doser av LSF. Ut fra dette kan det ses at til-føriing av de kombinerte glukokortikosteroider og LSF fører til en uventet syneristisk effekt. Det er derfor mulig enten å spare en del av det meget dyre LSF, eller å oppnå en øket effekt av hver av komponentene.
Den histologiske undersøkelse av disse dyrelunger, som ble utført etter at forsøket var avsluttet, viser en alvorlig dannelse av såkalte hyaline membraner (HM) og en sterk infiltrasjon av inflammatoriske celler [f.eks. polymorf-nukleære neutrofile leukocytter (=PMNL)] som sympton på utvikling av et akutt åndenødssyndrom.
Ved undersøkelse av preparater ifølge oppfinnelsen inneholdende dexametason eller ciclesonid og en blanding av fosfolipider med eller uten surfaktantproteiner på denne modell, viste det seg at oksygeneringen og de histologiske forandringer (hemming av dannelse av HM og hemming av innstrømning av PMNL) ved sammenligning med tilførsel av LSF eller GCS, alene blir synergistisk forbedret. Av dette følger at på grunn av denne uventede synergistiske effekt kan behandlingen av IRDS og ARDS forkortes, og den høye dødelighet i forbindelse med disse syndromer kan reduseres.
Claims (8)
1. Farmasøytisk sammensetning for behandling av neonatalt åndenødssyndrom (IRDS) og akutt åndenødssyndrom hos voksne (ARDS) inneholdende minst et glukokortikosteroid samt en lungesurfaktant.
2. Sammensetning ifølge krav 1, hvor sammensetningen omfatter glukokortikosteroidene betametason, metylpred-nison, deksametason og/eller ciclesonid.
3. Sammensetning ifølge krav 1, hvor sammensetningen omfatter en lungesurfaktant som er en blanding av fosfolipider.
4. Sammensetning ifølge krav 3, hvor sammensetningen omfatter lipider som er tilstede i naturlige surfaktanter.
5. Sammensetning ifølge krav 3, hvor sammensetningen ytterligere omfatter lungesurfaktantproteiner.
6. Sammensetning ifølge krav 5, hvor sammensetningen omfatter SP-B og/eller SP-C og/eller deres modifiserte derivater.
7. Sammensetning ifølge krav 4, hvor sammensetningen omfatter lungesurfaktanter fremskaffet ved lungeskylling.
8. Anvendelse av en kombinasjon av lungesurfaktant og minst et glukokortikosteroid ved fremstilling av et medikament som er egnet til behandling av IRDS og ARDS.
