Description
Foreliggende oppfinnelse vedrører en fremgangsmåte ved fremstilling av en intravaginal anordning som angitt i krav l's ingress. Oppfinnelsen er karakterisert ved å suspendere 17B-østradiol homogent i en farmakologisk akseptabel elastomer, fremstille en kjerne av nevnte elastomer, anbringe et ytre elastomerlag rundt kjernen, således at forholdet mellom kjernediameteren og tykkelsen på det ytre lag varierer mellom 0,3 og 6, og de relative mengder av 178-østradiol og elastomer velges slik at anordningen vil kontinuerlig og i alt vesentlig konstant i minst én måned frigi 17B-østradiol med en hastighet på ca 0,5 til 25 ug/24 h. Ytterligere karakteriserende trekk er angitt i kravene 2 og 3.
17B-østradiol anvendes ofte for å behandle østrogen-mangel. Denne mangel oppstår ved f.eks. oophorectomia, menopause, strålingsindusert ovarialsvikt, panhypopitui-tarisme og Kallmans syndrom.
Når 17B-østradiol inntas oralt, er den relativt uvirksom (1, 2). Dessuten påvirker orale østrogener leverprotein-ene pga. portal absorpsjon (3), tarm-metabolismen (4, 5)
og "bolus"-strømninger av sirkulerende østrogen etter hver dose (5).
Problemet med oral østrogenterapi kan unngåes ved vaginal administrering, og steroider absorberes godt fra vagina. Administrasjonen av 178-østradiol via intravaginale anordninger har vært foreslått og er blitt grundig studert (6, 7, 8, 9). En av disse studier, nemlig studien av Englund et al. (7) beskriver intravaginale anordninger
som har en in vitro-frisettelseshastighet på 200 ug/24 h, hvilket tilsvarer plasmanivåer i kvinner varierende mellom 80-200 pg/ml. Mot slutten av artikkelen er konklusjo-nen at frisettelseshastigheten på 200 ug pr. døgn er for høy for substitusjonsterapi hos postmenopausale kvinner
da det overskrider østrogen-nivåene som er iakttatt i den folikulære fase hos fertile kvinner. Det foreslåes at anordninger som frisetter 50-100 ug 178-østradiol pr. døgn, muligens vil gi den riktige dosering.
I artiklene av Stumpf (6, 9) og Veldhuls (8) diskuteres ikke frisettelseshastighetene pr. døgn, men plasmanivåene beskrives som liggende i området 50-150 pg/ml, hvilket er i omtrent samme størrelsesorden som beskrevet i Englunds artikkel.
I motsetning til hva som har vært undersøkt og foreslått, er det nå funnet at intravaginale anordninger som frisetter en betraktelig lavere mengde av 178-østradiol, er tilstrekkelige for å lindre eller eliminere symptomer som skriver seg fra østrogen-mangel. Disse symptomener kan f.eks. være vaginaltørrhet, pruritus, dyspareunia, svien-de smerte ved vannlatning, urineringspress og -hyppighet.
Det er også funnet at når lav dosering anvendes, oppnås en drastisk senkning av uønskede bieffekter, såsom flekk-dannelser, gjennombrudd og tilbaketrekningsblødninger.
De intravaginale anordninger fremstilt i henhold til foreliggende oppfinnelse er kjennetegnet ved at de kontinuerlig frisetter 178-østradiol i en hastighet av 0,5-25 ug/24 h.
Fortrinnsvis varierer frisettelseshastigheten mellom 1 og 10 ug/24 h, den mest foretrukne frisettelseshastighet varierer mellom 4 og 8 ug/24 h.
Perioden for kontinuerlig og i det vesentlige konstant frisettelse av østradiol fra anordningene fremstilt i henhold til oppfinnelsen kan omfatte i det vesentlige enhver periode varierende fra et døgn til et år, og denne periode vil være avhengig av flere faktorer. En vanlig periode er 3 til 4 måneder. Fortrinnsvis er frisettelseshastigheten i det vesentlige konstant i minst én måned.
Anordningene fremstilt i henhold til oppfinnelsen kan ha enhver utforming som er tilpasset for intravaginal administrering. En foretrukket type av anordning er den intravaginale ring.
På området intravaginale ringer (IVR) finnes hovedsakelig 5 forskjellige utforminger. Den første utforming vedrører en IVR hvori steroiden er homogent dispergert i en inert elastomer som beskrevet i US patent nr. 3.545.439.
En annen IVR består av en inert elastomer-ring omgitt av en annen ring av inert elastomer impregnert med en steroid (se f.eks US patent nr. 4.012.496).
I en tredje IVR (Rosemans IVR) er et tynt lag av en inert elastomer, som inneholder en steroid, støpt på en sentral inert elastomerkjerne.
I en fjerde type omgir et tynt medikamentpreparert lag av en inert elastomer, som inneholder en steroid, en sentral inert kjerne av syntetisk elastomer, og det preparerte lag er omgitt av et ytre lag av inert elastomer med varierende tykkelse. Denne type ringer er beskrevet i CONTRACEPTION, Vol, 17, No. 3, side 221-230, Mars 1978.
Den femte type IVR består av en sentral elastomerkjerne som er blandet med en steroid omgitt av et ytre lag av inert elastomer. Denne type er beskrevet i US patent nr. 3.545.439.
Elastomerer til.matrisen kan velges blant en rekke mate-rialer som er velkjent i faget, og frisettelseshastigheten for østradiolen reguleres av Ficks lov. Elasto-merene kan f.eks. velges fra hydroksylterminerte organo-polysiloksaner av typen RTV ("room temperature vulcani-zing" ) som herdes til elastomerer ved romstemperatur etter tilsetning av fornetningsmidler i nærvær av herdingskatalysatorer og under atmosfærisk fuktighet. Typiske herdingskatalysatorer er metalliske salter av karboksyl-syrer, fortrinnsvis tinnsaltet, f.eks. tinn(II)oktoat og tinn(II)-etylheksanoat. Andre egnede elastomerer er to-komponent-dimetylpolysiloksanblandinger som platinakata-lyseres ved romstemperatur eller under svakt forhøyede temperaturer, og som er i stand til addisjonsfornetning.

I en foretrukket utførelsesform har anordningen fremstilt i henhold til oppfinnelsen form av en ring som kan fremstilles ved homogent å suspendere 176-østradiol i en formakologisk akseptabel elastomer, å fremstille en kjerne fra nevnte elastomer; støpe et ytre lag av en farma-søytisk akseptabel elastomer rundt kjernen og herde frem-stilte anordning.
Det er klart at herdetemperaturen og herdetiden kan variere innen vide grenser og er avhengig bl.a. av elasto-merene som anvendes. Kjernen og det omgivende umedikamen-terte lag kan - men må ikke - fremstilles av samme elastomer .
Herdetemperaturen kan variere mellom romstemperatur og 150°C og er fortrinnsvis 40-90°C. Herdetiden kan variere mellom et par sekunder og flere døgn. I praksis er dog herdetider på mindre enn 1 time foretrukket.
For å oppnå frisettelseshastigheten i henhold til oppfinnelsen bør forholdet mellom kjernediameter og belegnings-tykkelse variere mellom 0,3 og 6 og mest foretrukket mellom 0,4 og 2.
Oppfinnelsen er ytterligere illustrert ved de følgende eksempler.
Eksempel 1.
En del ren mikronisert 17B-østradiol ble blandet med 250 deler flytende polydimetylsiloksan (382 Silastic" Medical Grade Elastomer). Blandingen ble aktivert med tinn(II)-oktoat og injisert under trykk i 2-delte ringformer, som hver hadde en ytre diameter på 543 mm og indre diameter på 539 mm. Blandingen i formene ble polymerisert, og de faste ringer som hadde en diameter på 2 mm, ble fjernet. Silikonbelegning (av 382 Silastic" Medical Grade Elastomer) med en tykkelse på 3,5 mm ble støpt på ringene, og resulterte således i et første sett med intravaginale ringer bestående av en medikamentpreparert indre 2 mm kjerne og et ikke-medikamentpreparert ytre 3,5 mm lag. Disse ringer frisetter omtrent 8 ug/24 h østradiol i et tidsrom på 150 døgn. Innholdet av østradiol var 1,91 mg/ring og av Silistic" 11 g.
Eksempel 2.
Et annet sett av intravaginale ringer ble fremstilt på lignende måte og frisatte omtrent 20 pg/24 h i et tidsrom på 150 døgn, og forskjellen på ringene var at det ble anvendt et ikke-medikamentpreparert ytre 1,5 mm tykt silikonbelegg og en kjerne som hadde diameter på 6 mm. Frisettelseshastigheten i løpet av en 150 døgns periode var omtrent konstant, men hastigheten har en tendes til å synke noe med tiden. Innholdet av østradiol var 4,79 mg/ring og av Silastic" 11 g.
Frisettelseshastigheten som det henvises til her, er in vitro-frisettelseshastigheter som kan måles i henhold til fremgangsmåten beskrevet på side 225 av marsj-nummeret 1978 av CONTRACEPTION (Vol. 17, No. 3) eller oktober-nummeret 1981 av CONTRACEPTION (Vol. 24, No. 4) eller ved enhver annen sammenlignbar metode.
Det kan således fremstilles intravaginale anordninger som inneholder 17B-østradioler, på en rekke forskjellige måter, og de kan ha en rekke forskjellige former eller utforminger. Den overfor nevnte forsøksprosedyre for frisettelseshastigheten kan anvendes for å bestemme hvor-vidt slike anordninger oppfyller kravene til frisettelseshastigheten i henhold til denne oppfinnelse.
Ringene som frisatte ca. 8 ug/24 h ble testet på 9 postmenopausale kvinner, og ringene som frisatte ca. 20 pg/24 h ble testet på 11 postmenopausale kvinner i et tidsrom på 3 måneder. Ringene ble akseptert godt, og de postmenopausale symptomer forsvant i begge grupper.
Ingen bieffekter ble observert hos kvinnene som fikk østradiolen i en hastighet på 8 ug/24 h. En ubetydelig blødning ble observert hos en kvinne som fikk dosen 20 ug/24 h.
Tabell 1 viser in vitro-frisettelseshastigheten av 17-6-østradiol for de to typer ringer fremstilt i henhold til oppfinnelsen.
Tabellene 2 og 3 angir in vivo-frisettelseshastigheter samt korrelasjonen mellom in vitro- og in vivo-frisettelseshastighetene.
Frisettelseskurver, E2-frisettelse i forhold til tiden, ble fremstilt, og følgende gjennomsnittlige frisettelsesverdier ble beregnet: Ringer av typen A - 20,3 ug E2/24 h når anvendt i 97 eller 98 døgn. - 20,6 ug E2/24 h når anvendt i 92 døgn. - 20,0 ug E2/24 h når anvendt i 105 døgn.
Ringer av typen B - 7,8 ug E2/24 h når anvendt i 93 til 99 døgn.

Claims (3)
1. Fremgangsmåte ved fremstilling av en intravaginal anordning som er effektiv for å behandle hypoøstrogene kvinner for å lindre eller fjerne symptomer på hypoøstrogenitet, og som i alt vesentlig er fri for bivirkninger, karakterisert ved å suspendere 17B-østradiol homogent i en farmakologisk akseptabel elastomer, fremstille en kjerne av nevnte elastomer, anbringe et ytre elastomerlag rundt kjernen, således at forholdet mellom kjernediameteren og tykkelsen på det ytre lag varierer mellom 0,3 og 6, og de relative mengder av 178-østradiol og elastomer velges slik at anordningen vil kontinuerlig og i alt vesentlig konstant i minst én måned frigi 178-østradiol med en hastighet på ca 0,5 til 25 ug/24 h.
2. Fremgangsmåte i henhold til krav 1, karakterisert ved at de relative mengder av 178-østradiol og elastomer velges således at frigivelses-hastigheten varierer fra ca 1 til 10 ug/24 h, fortrinnsvis fra ca 4 til 8 ug/24 h.
3. Fremgangsmåte i henhold til krav 1 eller 2, karakterisert ved at det ytre lag anbringes rundt kjernen således at forholdet mellom kjernediameteren og tykkelsen på det ytre lag varierer mellom 0,4 og 2.
