Abstract

Description
본 발명은 신경변성 질환에서 N-[2-히드록시-3-(1-피페리디닐)-프로폭시]-피리딘-1-옥사이드-3-카복시미도일 클로라이드의 용도에 관한 것이다.
알려진 바와 같이, 신경변성 질환(neurogegenerative disease)은 진행성의, 파괴적인, 만성의 연령 관련 장애이다. 평균 수명(life expectancy)의 증가로, 이러한 연령-관련 질환의 발생은 향후 수십년간 극적으로 증가할 것이다. 현재 이러한 만성병의 치료는 단지 대증(symptomatic) 요법이며, 이러한 다병인학적(multietiological) 질환들의 원인이 대부분 알려지지 않았기 때문에 원인(causal) 요법은 존재하지 않는다. 비록 이러한 질환들-알츠하이머병(AD), 파킨슨병(PD), 다발성 경화증(MS), 신경병증, 헌팅톤병(HD), 근위축성 측색경화증(ALS)과 같은-에서, 중추신경계(CNS) 내에서의 세포 손상 및 손실의 원인과 실제 병소부위는 다를 수 있지만, 질환의 진행, 및 세포내 사건에는 많은 공통점이 있다.
비록 많은 신경변성 장애의 대증적인 치료에서 많은 발전이 있었지만, 여전히 이러한 질환들의 진행을 지연시키고 가능하다면 정지시키는 약리학적 및 생약리학적인 치료에 대한 크고, 충족되지 않은 요구가 있다.
AD는 가장 일반적인 신경변성 질환이며, 가장 일반적인 치매의 형태이다(모든 치매의 약 80%의 원인이 됨). AD는 기억력 손실, 언어 감퇴, 시공간 기능의 손상, 판단력 감소, 무관심한 태도를 특징으로 하나, 운동 기능은 보존된다.
먼저 기억력 감퇴로 나타난 알츠하이머병 증상은, 수년 동안, 인지력, 인격, 및 기능에 대한 능력을 파괴한다. 정신착란 및 안절부절증이 또한 발생할 수도 있다. 뇌에서 아밀로이드판(amyloid plaque) 및 신경섬유다발(neurofibrillary tangles)이 이 질환을 구별하는 특징이며, 또한 기억력 및 기타 정신적 능력에 매우 중요한 뇌의 영역에 신경세포의 손실이 있다. 상기 질환은 보통 60세 이후에 시작되며, 나이에 따라 위험이 증가한다. 반면, 그보다 젊은 사람들 또한 알츠하이머병에 걸릴 수는 있지만, 그것은 훨씬 덜 일반적이다. 65 내지 74세의 남성 및 여성의 약 3%가 알츠하이머병을 갖고 있고, 85세 및 그 이상의 연령의 거의 반이 이러한 질환을 갖고 있다.

오늘날까지 실험된 비스테로이드성 항-염증제는 효과적인 것으로 입증되지 않았다.

종종 루 게릭 병이라 불리는, 근위축성 측색경화증(ALS)은, 수의근을 조절하는 역할을 하는 신경 세포(뉴런)를 공격하는, 급속도로 진행하는, 항상 치명적인 신경계 질환이다. 상기 질환은 가장 일반적인 운동 뉴런 질환이며, 운동 뉴런의 점진적인 퇴행과 사멸을 특징으로 한다(Rowland LP, Schneider NA: Amyotrophic lateral sclerosis. N-[2-히드록시-3-(1-피페리디닐)-프로폭시]-피리딘-1-옥사이드-3-카르복시미도일 클로라이드와 관련한 어떠한 특허 문헌도 당뇨병 치료 이외의 이 화합물의 용도를 제안한 적이 없다.

용어 "신경변성 질환"은 근위축성 측색경화증(ALS), 알츠하이머병(AD), 파킨슨병(PD), 헌팅톤병(HD), 다발성 경화증(MS), 및 다양한 유형의 신경병증(neuropathy)들을 포함하는 공지된 유형의 신경변성 질환에 관한 것이다.
실험 화합물로서 (+)-R-N-[2-히드록시-3-(1-피페리디닐)-프로폭시]-피리딘-1-옥사이드-3-카르복시미도일 클로라이드 시트르산염으로 다음의 생물학적 실험을 수행하였다. 상기 화합물은 화합물 A로 언급될 것이다.
양성(both sexes)의 트랜스제닉 mSOD1( G93A ) 마우스가 본 연구에서 이용되었다. 이전에 Zhu et al 2002(Zhu,S. et al.Minocycline inhibits cytochrome c release and delays progression of amyotrophic lateral sclerosis in mice. Nature 417, 74-78(2002))에 기재된 것과 유사한 방식에 따라, 모든 실험 동물들을 연령 35일로부터 매일 화합물 A(10mg/kg, 복강내 주사)로 처리하였다.
근육 기능 및 운동 단위수의 평가
질환의 진행 정도를 측정하기 위해서 후지(hind limb) 근육 기능의 실시간의, 생체 내 전기생리학적 평가를 양 후지의 긴발가락펴짐(EDL) 근육에서 수행하였다. 등척성 신장(isomeric tension) 기록 및 운동 단위 수의 평가
양 트랜스제닉 동물 및 그들의 야생형 한배새끼(littermate)들을 클로랄 하이드레이트(4% 클로랄 하이드레이트; 1ml/100g 체중, 복강내 주사)로 마취시키고, 그들의 수축 특성 및 운동 단위 수의 생체 내 평가를 위해 EDL 근육을 준비하였다. EDL 근육의 말단 힘줄을 절개하여 견사로 등척성 힘 변환기(Dinamometer UFI Devices)에 부착했다. 양다리는 핀으로 테이블에 단단히 고정했다. 좌골 신경은 자유롭게 절개하고, EDL 근육에 대한 신경과는 별도로, 모든 좌골 신경의 가지들을 잘라냈다. 그 후 신경의 말단 끝을 양극 은전극을 이용하여 자극했다. 신경 자극시 최대 수축(maximal twitch)이 될 때까지 근육의 길이를 조절했다. 등척성 수축은 0.02ms 폭의 펄스를 이용하여 운동 신경의 잘린 끝을 자극함에 의해 도출되었다. 강직 수축은 40, 80, 및 100Hz에서 500ms 동안 EDL 근육을 자극함에 의해 도출되었다.

수축 신장의 기록으로부터, EDL의 절반 이완기, 근육이 수축 후 이완하는데 걸리는 시간의 측정을 포함하여 처리 및 미처리의 mSOD1( G93A ) 트랜스제닉 마우스에서의 EDL 근육의 수축 특성의 일부를 평가했다.
피로 패턴
EDL은 보통 속근이므로 지속적으로 자극하여 전형적인 피로 패턴을 생성했을 때 빨리 피로하게 된다. 이러한 실험들에서 EDL의 피로 패턴을 검사하기 위해, 양 후지의 근육들을 40Hz에서 250ms 동안, 3분 걸러서 자극했고, 근수축을 기록했다.

Neuroscience 95, 1101-1109(2000)). 마우스는 깊이 마취시켰고(4% 클로랄 하이드레이트, 1ml/100g 체중, 복강내 주사) 4% 파라포름알데히드를 함유하는 고정제로 심장을 가로질러(transcardially) 관류하였다. 척수를 제거하고, 2시간 동안 동일한 고정제에 부가하고, 수크로스에서 동결방지되고(MPB에서 30%), 횡단면을 냉각한 허리 부분을 저온유지장치에서 30㎛로 절단하고 보조 슬라이드 위로 수집하였다. 이어서 상기 단면을 니슬 염색(갈로시아닌)으로 대비염색제로 엷게 착색했다. 양 전각(ventral horn)에서 니슬 염색된 운동 뉴런의 수를 광학 현미경 하에서 세었다. 연속적인 단면에서 동일한 세포를 두 번 세는 것 을 방지하기 위해서, L2-L5 레벨간 척수의 허리 영역의 모든 제3 단면에서의 좌골 운동 풀에서 운동 뉴런 생존을 평가했다. 고배율에서 핵이 선명하게 보이는 상기 뉴런들만을 카운트에 포함하였다.
통계학적 분석
모든 측정된 파라미터에 대하여, 결과는 독립 표본과 비교하기 위한 만-위트니(Mann-Whitney) U-검정을 이용하여 분석했다. 모든 사례에서 양측검정을 이용했으며, 유의성은 P<0.05로 설정하였다.
결과
연령 35일에 mSOD1( G93A ) 트랜스제닉 마우스는 이미 허리 운동뉴런 퇴행의 미세한 특징을 보였고, 연령 110일까지는 후지(hind limb) 마비가 명확해졌다. mSOD1(G93A) 트랜스제닉 마우스가 질환의 후기 단계에 있는 연령 120일에서 후지 근육 기능에 화합물 A로 처리한 효과뿐만 아니라 운동 단위 및 운동 뉴런 생존을 평가했다.
운동 단위 생존
야생형 마우스에서는 EDL 근육에 보통 28 +/-0.6(평균 +/- S.E.M., n=11)의 운동 단위가 존재한다. 연령 120일의 mSOD1( G93A ) 트랜스제닉 마우스에서는 단 8.3 +/-0.7(평균 +/- S.E.M., n=10)의 운동 단위만이 존재한다. 그러나, 화합물 A로 처리한 mSOD1( G93A ) 트랜스제닉 마우스에서는 운동 단위 생존에 큰 개선이 있으며, 연령 120일에서 14.3 +/-0.6(평균 +/- S.E.M., n=10)의 운동 단위가 생존했다(P=0.003).
EDL 근육의 수축 특성
ⅰ) EDL 근육의 절반 이완기(half relaxation time)
수축 신장의 기록으로부터 처리 및 미처리의 mSOD1( G93A ) 트랜스제닉 마우스에서의 EDL 근육의 수축 특성의 일부를 또한 조사했다. EDL은 보통 속근의, 곧 피로해지는 근육이며, 야생형 마우스에서는 EDL의 절반 이완기, 즉 근육이 수축 후 이완하는데 걸리는 시간을 측정한 것은 25.8ms +/-2.4(평균 +/- S.E.M.,n=10)이다. 반대로, 처리하지 않은 마우스에서는, 절반 이완기가 신경제거(denervation) 및 근육 위축의 결과로 느려졌으며, 43.3ms +/-6.93(평균 +/- S.E.M.,n=10)으로 나타났다. 그러나, 화합물 A로 처리한 마우스에서는, 절반 이완기가 크게 개선되었으며, 32.2ms +/-1.80(평균 +/- S.E.M.,n=10)으로 나타났다(P=0.030).
ⅱ) EDL 근육의 피로 패턴 및 피로 지수
EDL은 보통 속근이므로 지속적으로 자극하여 전형적인 피로 패턴을 생성했을 때 빨리 피로하게 된다. 야생형, mSOD1( G93A ) 트랜스제닉 및 처리된 mSOD1( G93A ) 트랜스제닉 마우스에서 EDL 근육의 피로 패턴을 조사하였다. 자극 3분 후 신장(tension)의 감소를 측정하고, 피로 지수(FI)를 계산했다. 연령 120일의 mSOD1( G93A ) 트랜스제 닉 마우스에서 EDL 근육은 운동뉴런 퇴행, 신경제거 및 근 섬유 표현형에서의 필연적인 변화의 결과로 피로 저항형이 되었다. 그래서, 야생형 마우스에서 0.848 +/-0.028(평균 +/- S.E.M.,n=10)에 비교하여, 연령 120일의 mSOD1( G93A 트랜스제닉 마우스는 0.255 +/-0.04(평균 +/- S.E.M.,n=10)의 피로 지수를 가진다. 그러나, 화합물 A로 처리한 마우스에서는 EDL은 0.416 +/-0.07(평균 +/- S.E.M.,n=10)의 피로 지수를 가진다. 따라서, 처리되지 않은 mSOD1( G93A ) 한배새끼(littermate)에 비해 처리된 mSOD1(G93A) 트랜스제닉 마우스에서 EDL의 피로 지수는 크게 개선되었다(P=<0.05).
운동뉴런 생존
생리학적 실험의 완료에 이어, 허리 척수의 단면으로부터의 전각(ventral horn)에서의 좌골 운동 풀(pool)에서 운동뉴런의 수를 세어서 운동뉴런 생존을 평가하였다. 운동 단위 생존에서 관찰된 증가에 대응하여, 처리되지 않은 mSOD1( G93A ) 한배새끼(littermate)에 비해 처리된 mSOD1( G93A ) 트랜스제닉 마우스의 좌골 운동 풀에서 생존하고 있는 운동뉴런의 수 또한 크게 증가했다. 야생형 마우스에서, 검사된 좌골 운동 풀의 구획(segment)에는 593 +/-15.8(평균 +/- S.E.M.,n=13)의 운동뉴런이 있었다. 연령 120일의 처리되지 않은 mSOD1( G93A ) 트랜스제닉 마우스에서는 많은 수의 운동뉴런이 사멸했고, 237 +/-14(평균 +/- S.E.M.,n=7)의 운동뉴런만 생존 했다. 그러나, 화합물 A로 처리한 mSOD1( G93A ) 트랜스제닉 마우스에서는, 연령 120일에서도, 412 +/-28(평균 +/- S.E.M.,n=4) 운동뉴런 생존으로 운동뉴런 생존에 극적인 증가가 있다(P=0.002).
이러한 결과는 mSOD1( G93A ) 트랜스제닉 마우스를 화합물 A(10mg/kg; 복강내 주사복강내 주사매일 처리함에 따라 운동 단위 및 운동뉴런 생존 모두의 높은 증가뿐만 아니라, 질환의 후기 단계에서(120일) 후지 근육 기능에 개선이 있는 것을 나타낸다.
수명
연령 120일의 처리된 mSOD1( G93A ) 트랜스제닉 마우스에서 관찰된 운동 단위 수 및 운동뉴런 생존에서의 큰 개선을 고려하여, 마우스의 분리된 그룹에서 화합물 A로 처리하는 것이 mSOD1( G93A ) 트랜스제닉 마우스의 수명에 영향을 미칠 것인가를 조사했다. 옆으로 놓았을 때 다시 일어나는 것에 대한 마우스의 무능력 및 대략 20% 체중 손실 양자의 측정에 따라, 처리하지 않은 mSOD1( G93A ) 트랜스제닉 마우스는 125 +/-1.8(평균 +/- S.E.M.,n=18) 일의 평균 수명을 갖는 것을 발견했다. 그러나, 화합물 A로 처리한 그룹에서는 체중의 감소가 지연되었고 수명이 크게 연장되었으며, mSOD1( G93A ) 마우스는 평균 153 +/-2.6(평균 +/- S.E.M.,n=7)일 생존했다. 이것은 수 명에서 22% 이상의 큰 증가를 나타낸다(P=<0.001).

- 청구항 1에 있어서, 상기 화학 물질은 (+)-R-N-[2-히드록시-3-(1-피페리디닐)-프로폭시]-피리딘-1-옥사이드-3-카르복시미도일 클로라이드인 용도.
