Abstract
본 발명은 유효량의 에리트로포이에틴(a), 레시틴 또는 수소화 레시틴(i), 임의로 하전된 양전성 또는 음전성 지질 화합물(ii) 및 콜레스테롤, 또는 콜레스테롤 에스테르, 콜레스테롤의 폴리에틸렌 글리콜 유도체(PEG-콜레스테롤) 및 콜레스테롤의 유기산 유도체로부터 선택된 콜레스테롤 유도체(iii)를 포함하는 지질상(b) 및 인산염 완충액(c)을 포함하는 에리트로포이에틴의 리포솜계 제형에 관한 것이다. 본 발명의 리포솜계 비경구 투여 형태는 에탄올 주입 기법에 의해 제조된다. 당해 조성물은 사람 혈청 알부민을 사용할 필요성이 없고 우수한 안정성을 나타낸다.

발명의 분야
본 발명은 에리트로포이에틴의 리포솜계 제형에 관한 것이다. 특히, 본 발명은 에탄올 주입 기법을 사용하여 제조한 것으로서 안정성이 우수한 에리트로포이에틴의 리포솜계 비경구 투여 제형에 관한 것이다.
발명의 배경
에리트로포이에틴(EPO)은 적혈구 합성을 조절하는데 관여하는 주요 인자로서 작용하는 당단백질이다. 에리트로포이에틴은 신장에서 생산되며, 골수에서 전구체 세포를 자극하여 이들을 성숙한 적혈구로 분리 및 분화시키는 작용을 한다. 재조합에 의해 생산된 165개 아미노산으로 이루어진 당단백질이 만성 신부전과 관련된 빈혈을 포함한 각종 형태의 빈혈, 지도비딘(zidovidine)으로 치료받는 HIV 감염 환자 및 화학요법 중인 암환자를 치료하는데 효과적인 치료제로서 한동안 이용되었다. 당단백질은 정맥내(IV) 주사 또는 피하(SC) 주사에 의해 비경구 투여된다.
최근, 사용 중인 비경구 제형은 담체로서 사람 혈청 알부민(HSA)을 함유하는 IV 또는 SC 주사용의 통상의 멸균된 완충 수용액이다. 이러한 제형은 에포겐(EPOGENR) 및 프로크리트(PROCRITR)라는 상품명으로 미국에서 시판되고 있다. 이들 제품은 방부제를 함유하지 않는 1㎖ 단일 용량 또는 방부처리된 2㎖ 다중 용량 바이알 속에 에리트로포이에틴을 함유한다.
이들 제형이 매우 성공적인 것으로 판명되어 왔으나, 담체로서 사람 혈청 알부민을 사용하는 것과 관련하여 몇가지 단점이 있다. HSA는 천연 공급원으로부터 수득되기 때문에, HIV 또는 간염과 같은 감염 질환 약제용 담체로서는 잠재적인 위험이 있을 수 있으므로 재료를 주의깊게 선별해야한다. 또한, 적당한 품질을 갖는 HSA의 구입용이성 여부가 종종 문제가 될 수 있다. 따라서, 담체로서 HSA를 사용하지 않는 주사용 에리트로포이에틴 제형이 필요하다.
따라서, 담체로서 HSA를 사용하지 않는 개선된 에리트로포이에틴 제형을 제공하려는 시도가 이루어져 왔다. 동시에, 제형은 안정해야 하며, 연장된 저장 수명을 제공할 수 있어야 한다. 또한, 제형은 활성 성분이 바이알 속에 함유되어 이의 표면에 달라붙는 것과 관련한 문제를 피할 수 있어야 한다.
리포솜은 구형 2중층으로 배열되어 있는 양쪽성 지질을 포함하는 작은 소포이다. 리포솜은 수성 채널(다층판 소포 또는 MLV)에 의해 분리되어 있는 다수의 동심성 지질 2중층을 함유하거나, 또는 소단층 소포(SUV) 또는 대단층 소포(LUV)일 수 있는 단일 막 2중층(단층 소포)을 함유할 수 있다. 지질 2중층은 소수성 "꼬리" 영역과 친수성 "머리" 영역을 갖는 2가지 지질 단층으로 이루어진다. 막 2중층에서, 지질 단층의 소수성 "꼬리"는 2중층의 중심을 향하고 있는 반면, 친수성 "머리"는 수성상을 향하고 있다.
리포솜은 수성 내부 또는 2중층들 사이에 친수성 화합물을 가두거나 2중층 내에 소수성 화합물을 가두어 각종 물질을 캡슐화하는데 사용할 수 있다. 그 자체로서, 이들은 수용해도가 불량하거나 치료량에서 허용할 수 없는 독성을 나타내는 화합물을 캡슐화함으로써 생물학적 활성 물질을 운반하는데 특히 유용하다.
단지 1개의 2중 층을 갖는 리포솜의 구체적인 제조방법이 유럽 공개특허공보 제253 619호에 기재되어 있다. 각종 활성제의 리포솜 제형은 오래전부터 공지되어 왔으며, 리포솜성 에리트로포이에틴의 제조방법이 제안되었다. 예를 들면, 문헌(참조; Maitani et al., J. Pharm. Sci., 85: 440-445(1996))에는 경구 투여용 리포솜성 에리트로포이에틴 제형이 기재되어 있으며, 여기서 리포솜은 역상 증발 소포법(reverse phase evaporation vesicle method)에 의해 제조된다. 이러한 제형은 경구 투여용이기 때문에, EPO를 리포솜에 다량으로 혼입하는 것이 바람직하다. 그러나, 작은 소포로의 캡슐화율이 높은 이러한 제형이 간에 집중되어 독성을 일으킬 수 있다. 더욱이, 여기에 사용되는 제조방법에서는 특수한 원료(예를 들면, 폴리글리세린 인지질) 및 유기 용매의 사용을 필요로 한다. 또한, 여기에 사용되는 역상 공정은 캡슐화되지 않은 EPO의 손실율이 높아 바람직하지 못하며 비용이 많이 든다.
따라서, 본 발명의 목표는 담체로서 HSA를 사용하지 않고 연장된 저장 수명을 위한 허용가능한 장기간 안정성을 제공하며 대량 제조하기에 적합한 공정으로 제조할 수 있는, EPO에 적합한 비경구 제형을 제공하는 것이다.
발명의 요지
리포솜계 비경구 조성물은

레시틴 또는 수소화 레시틴(i), 임의로 하전된 양전성 또는 음전성 지질 화합물(ii) 및 콜레스테롤, 또는 콜레스테롤 에스테르, 콜레스테롤의 폴리에틸렌 글리콜 유도체(PEG-콜레스테롤) 및 콜레스테롤의 유기산 유도체로부터 선택된 콜레스테롤 유도체(iii)를 포함하는 지질상(b) 및
수성 완충액(c)을 포함한다.
본 발명에 따라, 당해 조성물은 지질상의 알콜성 용액을 제조하여 가압하에서 이들 용액을 고속 균질화기 속에 담겨있는 수성 완충액에 주입하여 제조한 단일 2중층 리포솜을 포함한다. 이렇게 하여 제조한 리포솜을 에리트로포이에틴 활성 성분과 함께 항온처리하여 본 발명에 따르는 리포솜성 분산액을 형성한다.
바람직하게는, 활성 성분은 골수 세포로 하여금 망상 적혈구 및 적혈구의 생산을 증가시키도록 하는 생물학적 특성을 갖는 에리트로포이에틴 및 이의 유도체이다. EPO 당단백질은 천연 공급원으로부터 수득하거나, 본원에 참고로 인용되어 있는 미국 특허 제4,703,008호, 제5,441,868호, 제5,547,933호, 제5,618,698호 및 제5,621,080호에 기재된 바와 같은 공지된 과정을 사용하여 재조합에 의해 생산할 수 있다.
본 발명에 따라, 예기치 않게도, 본원에 기재되어 있는 적당한 조건하에서 제조된 리포솜성 EPO 조성물이 개선된 안정성을 나타내며, 즉 리포솜 자체가 안정한 동시에 생물학적 유효 물질의 화학적 분해 및 응집이 최소화되는 것으로 밝혀졌다. 추가의 예기치 못한 장점으로서, EPO 활성 성분은, EPO가 실질적으로 리포솜 속에 혼입되어 있지 않고 대신에 리포솜성 분산액으로서 간질액 속에 함유되어 있더라도, 바이알 용기 또는 IV 튜빙의 표면에 달라붙지 않는다.
발명의 상세한 설명
본 발명에 사용되는 활성 성분은, 골수 세포로 하여금 망상 적혈구 및 적혈구의 생산을 증가시키도록 하는 생물학적 특성을 갖는 에리트로포이에틴 및 이의 유도체이다. 본 발명의 리포솜성 분산액은 만성 신부전과 관련된 빈혈을 포함한 각종 형태의 빈혈, 지도비딘으로 치료받는 HIV 감염 환자 및 화학요법 중인 암환자에서와 같이, 적혈구의 생산 감소 또는 부족을 특징으로 하는 혈액 질환을 치료하는데 있어서 비경구 제형으로서 유용하다. 이는 또한 다양한 질환 상태, 혈액학적 이상 장애 및 상태, 예를 들면, 겸상적혈구 질환, 베타-지중해 빈혈, 낭포성 섬유증, 임신 및 월경 장애, 조숙성 초기 빈혈, 척수 손상, 비행, 급성 혈액 손실 및 노화 등을 치료하는데 사용할 수도 있다. 바람직하게는, 본 발명의 EPO 조성물은 비경구(예를 들면, IV, IM, SC 또는 IP) 투여한다. 유효량은 치료받고자 하는 질환 및 투여 경로에 따라 상당히 다를 것으로 예측되지만, 체중 ㎏당 활성 물질 0.1(약 7U) 내지 100㎍(약 7000U)의 범위일 것으로 예측된다. 빈혈 질환을 치료하는데 바람직한 투여량은 1주간 3회 약 50 내지 약 300Unit/㎏이다.
본 발명의 EPO 리포솜성 분산액은 일반적으로 조성물 100g당 EPO 당단백질을 약 200,000 내지 약 1,000,000Unit 함유한다. 활성 EPO 성분은 레시틴 또는 수소화 레시틴(i), 임의로 하전된 양전성 또는 음전성 지질 화합물(ii), 콜레스테롤, 또는 콜레스테롤 에스테르, 콜레스테롤의 폴리에틸렌 글리콜 유도체(PEG-콜레스테롤) 및 콜레스테롤의 유기산 유도체로부터 선택된 콜레스테롤 유도체(iii) 및 탄소수 1 내지 6의 저급 알칸올로 이루어진 알콜 성분(iv)을 포함하는 지질상(a) 및 수성 완충액(b)으로부터 형성된 리포솜성 현탁액 속에 분산되어 있다.
특히 본원에 참고로 인용되어 있는 유럽 공개특허공보 제0 253 619호에 기재된 방법에 따라 제조된 이러한 제형은 선행 기술분야의 HSA 함유 조성물에 대한 대체물로서 적합한 특성을 나타낸다.
레시틴은 정제된 형태의 천연 레시틴으로서 사용하거나, 바람직하게는 보다 안정한 수소화 레시틴으로서 사용할 수 있으며, 후자를 사용할 경우 안정화제의 농도를 낮출 수 있다. 레시틴 성분은 일반적으로 조성물 100g당 약 0.5 내지 5.0g의 양으로 존재한다. 바람직하게는, 수소화 레시틴은 EPO 및 리포솜의 안정성에 부정적인 방식으로 영향을 미칠 수 있는 검출가능한 수준의 촉매를 함유하지 않는 품질이 우수한 것이어야 한다.
콜레스테롤은 조성물 100g당 0.1 내지 1.0g의 양으로 리포솜 안정화제로서 사용된다. 콜레스테롤 이외에, 콜레스테롤 에스테르, 콜레스테롤의 폴리에틸렌 글리콜 유도체(PEG-콜레스테롤) 및 콜레스테롤의 유기산 유도체(예를 들면, 콜레스테롤 헤미석시네이트)와 같은 다른 콜레스테롤 유도체를 사용할 수 있다.
양전성 또는 음전성 지질은 양으로 또는 음으로 하전된 성분을 포함하는 지질 화합물이다. 양전성 지질은 올레일 아민 또는 스테아릴 아민이다. 음전성 지질은 올레산, 포스파티드산, 예를 들면, 디팔미토일 포스파티드산(DPPA), 디팔미토일글리세롤(DPPG), 디스테아로일 포스파티드산(DSPA) 또는 디미리스틸 포스파티드산(DMPA)이다. 이러한 하전된 지질을 사용하면 리포솜의 침강을 방지할 수 있는 유백광의 분산액을 제공하는 하전된 리포솜이 수득된다. 명시한 바와 같이, 활성 에리트로포이에틴 당단백질은, 활성 성분이 리포솜 내에 혼입되어 있지 않고 단지 하전된 리포솜을 포함하는 분산액으로서 존재하더라도, 활성 성분을 투여하는데 사용되는 용기 또는 실리콘 튜빙의 유리벽에 달라붙지 않는다는 결과는 전혀 예측하지 못했던 사실이다.
조성물 100g당 0.5 내지 약 5.0g의 양으로 존재하는, 메탄올, 에탄올, n-프로판올, 이소프로판올, n-부탄올 등과 같은 탄소수 1 내지 6의 저급 알칸올로 이루어진 알콜 성분은 에탄올 주입 기법을 사용하여 제조한 조성물에 일반적으로 포함된다. 에탄올이 바람직하다.
수성 완충액 성분은 비경구 조성물에서 완충액으로서 통상적으로 사용되는 전형적인 산 염으로부터 선택된다. 이의 예로는 시트레이트, 아세테이트 및 인산염이 포함된다. 인산염 완충액이 바람직하다. 이의 예로는 인산이수소나트륨 2수화물, 인산수소이나트륨 2수화물 및 이들의 혼합물이 포함된다. 바람직하게는, 인산이수소나트륨 2수화물과 인산수소이나트륨 2수화물과의 혼합물이 0 내지 2.0g/100g의 양으로 사용된다.
임의로, 글리신과 같은 안정화제를 조성물에 가하여 응집체의 형성을 막을 수 있다. 그러나, 대개의 경우, 리포솜이 조성물에서 담체이자 안정화제로서 작용하기 때문에, 이러한 안정화제는 필요치 않다.
본 발명의 리포솜계 조성물은 본원에 참고로 인용되어 있는 유럽 공개특허공보 제253619호에 기재된 리포솜 조성물을 제조하기 위해 당해 기술분야에 공지된 방법을 사용하여 제조한다. 이러한 방법에서, 단일 2중층 리포솜은, 인지질과 활성 성분의 에탄올 용액을 제조하여 가압하에서 이들 용액을 고속 균질화기 속에 함유된 수성 완충액에 주입함으로써 제조한다. 리포솜이 형성되어 직경이 1㎛ 미만인 리포솜을 자발적으로 제공한다. 특히, 본 발명의 방법에 따라, 리포솜은 정제수 중에 수성 완충액을 형성시킴으로써 제조된다. 별도로, 레시틴, 콜레스테롤 및 하전된 지질 성분은 에탄올과 같은 알콜성 용액에 용해시킨다. 수용액은 고성능 균질화기에 연결하여 순환시키고, 알콜성 용액은 균질화기에 직접 주입한다. 1㎛ 미만의 리포솜이 자발적으로 형성된다. 그후, 이렇게 하여 형성된 리포솜을 EPO 활성 성분과 함께 항온처리하여 본 발명의 리포솜성 분산액을 형성한다.
직경이 명확한 리포솜을 포함하는 투명한 리포솜 분산액을 수득하기 위해서는, 약 0.05 내지 0.08mm의 기공을 갖는 필터를 통해 리포솜을 압출하여 직경이 약 80 내지 100nm인 리포솜을 수득하는 것이 바람직할 수 있다. 이러한 부가적인 입자 분립 단계는 투명 용액의 어떠한 응집도 용이하게 검출하고 이의 혈중 순환 시간을 연장하기 위해 투명 용액을 제공하는데 사용된다.
위에서 명시한 바와 같이, 최근 시판되고 있는 에리트로포이에틴 조성물은 안정성을 유지하고 제한된 저장 수명을 갖도록 하기 위해, 트윈 또는 아미노산 등과 같은 안정화제를 포함시키거나 동결건조하여 저장한다. 본 발명의 리포솜성 조성물은 탁월한 안정성을 나타내며, 즉 리포솜 자체가 안정한 동시에 생물학적 유효 물질의 분해 및 응집이 최소화되는 것으로 밝혀졌다. 2년 이하의 저장 수명이 달성되는데, 이는 산업적인 용도에서 매우 중요하다. 이러한 개선된 안정성은 본 발명의 바람직하면서도 우수한 제조기술 및 제형의 성분과 조성(문헌에 기재된 제형과 비교한 정량적인 측면과 정성적인 측면 둘다로부터)에 기인할 수 있다.
조성물의 안정성은 토코페롤, 부틸화 하이드록시톨루엔, 부틸화 하이드록시아니솔, 아스코빌 팔미테이트 또는 에데테이트(예를 들면, 이나트륨 에데테이트; 에데테이트는 존재하는 중금속과 추가로 결합가능하다)와 같은 산화방지제를 첨가하여 더욱 향상시킬 수 있다. 또한, 안정성은 벤조산 및 파라벤, 예를 들면, 메틸파라벤 및/또는 프로필파라벤과 같은 방부제를 첨가하여 향상시킬 수 있다.

과정
리포솜은 80℃에서 주입용 물 속에서 인산이수소나트륨 2수화물, 인산수소이나트륨 2수화물 및 염화나트륨의 전해질(완충) 수용액을 형성함으로써 제조한다. 별도로, 레시틴 및 콜레스테롤을 55 내지 70℃에서 에탄올과 같은 알콜성 용액에 용해시킨다. 수용액(주전자 1)은 고성능 균질화기에 연결하여 순환시키고, 알콜성 용액(주전자 2)은 균질화기에 직접 주입시킨다. 에탄올 용액을 전 과정 동안 질소로 퍼징시킨다. 1㎛ 미만의 리포솜이 자발적으로 형성된다. 직경이 명확한 리포솜을 형성하기 위해, 리포솜성 분산액을 소정의 기공(예를 들면, 0.8 내지 0.5㎛)을 갖는 핵공 필터(nucleopore filter)를 통해 압출시킨다. 에리트로포이에틴을 리포솜성 분산액과 함께 항온처리하고 후에 1회 살균여과한다. 무균 조건하에서 바이알에 충전한다.

실시예 4
안정성 시험
리포솜성 에리트로포이에틴 제형의 2가지 배치를 실시예 1과 2에 따라 제조한다. 다양한 시간 간격으로 배치의 안정성을 분석한다. 사용되는 시험관내 및 생체내 생검을 위한 절차는 아래에 기재되어 있다. 결과는 표 1과 2에 기재한다.

생체내 생검
저압에 의해 적혈구증가증이 유도된 마우스를 대상으로 한 에리트로포이에틴 생검
마우스를 18시간 동안 감압하에 둔다. 그후 6시간 동안 마우스를 주위압에 둔다. 이러한 과정을 14일간 반복한다. 3일 후, 주위압에서 에리트로포이에틴을 마우스에 투여한다. 1일 후, 59FeCl3를 함유하는 용액을 주사한다. 2일 후, 혈액을 분석하여 59FeCl3의 적혈구로의 유입도를 측정한다.
시험관내 생검
시험관내 생검은 에포에틴 α의 생물학적 활성을 정확하게 정량하기 위해 고안된 세포에 근거한 생검이다.
먼저, 샘플을 조직 배양 배지 속에서 희석시킨 다음, HEP.G2의 세포 배양액으로 처리한다. 이러한 유착 세포주는 탈사이알레이트화(desialated) 단백질 제거능으로 인해 간조직의 기능을 보유한다. 유사한 대사 과정이 생체내에서 일어나면 탈사이알레이트화 에리트로포이에틴의 활성이 감소되는 것으로 공지되어 있다. HEP.G2 세포로 처리할 경우 배지로부터 에포에틴 α 중의 사이알레이트화 에리트로포이에틴이 제거되지 않을 것이다. 따라서, 시험관내 생검은 마우스의 생체내 생검과 유사하다.
제2 단계에서, 나머지 에리트로포이에틴을 HEP.G2 세포로부터 분리하고, B6SUtA 세포주를 사용한 세포 증식 분석법으로 시험한다. 이들 세포는 에리트로포이에틴의 존재하에서 성장하며, 성장도는 에리트로포이에틴의 양에 비례한다. 이어서, MTT를 세포에 가할 때 생성되는 색의 양에 의해 세포 성장도를 측정한다. 생성된 색은 세포수 및 B6SUtA 세포의 활성 감소에 직접 비례한다.
결론
이들 데이타는 2가지 제형이 모두 24개월까지 우수한 안정성을 나타냄을 입증해준다.

- 수성 완충액의 수성 상(a),탄소수 1 내지 6의 알칸올로 이루어진 알콜 용매 중에서 레시틴 또는 수소화 레시틴(i), 하전된 양전성 또는 음전성 지질 화합물(ii) 및 콜레스테롤, 또는 콜레스테롤 에스테르, 콜레스테롤의 폴리에틸렌 글리콜 유도체(PEG-콜레스테롤) 및 콜레스테롤의 유기산 유도체로부터 선택된 콜레스테롤 유도체(iii)를 포함하는 지질상 용액을 제조하고, 가압하에 이들 용액을 고속 균질화기에 함유된 수성 완충액에 주입함으로써 제조된 단일 2중층 리포솜(b) 및골수 세포로 하여금 망상 적혈구 및 적혈구의 생산을 증가시키도록 하는 생물학적 특성을 갖고, 단일 2중층 리포솜(b)과 함께 항온배양되어 외부 수성 상내에 분산되어 있고 리포솜에 혼입되지 않는, 유효량의 에리트로포이에틴(c)을 포함하는 리포솜계 비경구 조성물.
