【発明の詳細な説明】
留め具を漸進的に付ける外科用留め臭性は装置発明の背景
本発明は外科用留め具を身体組織に付けるための装置に関し、特に比較的多数の
かかる留め具を実質的に同時に付けるための装置に関する。
比較的多数の外科用留め具を同時に又は実質的に同時に付けるための器具は、例
えばヒルスチ等の米国特許第3,275,211号、グリーンの米国゛特許第4
,402,444号およびグリーンの米国特許第4.402.445号で知られ
ている。留め具はヒルスチ等の特許に示すように金属製外科用ホチキス針の形態
を取っても良いし、或は、グリーンの米国特許第4.402,445号に示すよ
うに最初2つの部分の非金属製外科用留め具であっても良い。
「外科用留め具」という用語はここでは、これらの型式の留め具の両方に共通な
用語として用いられる。
成る外科処置では、大変多数の外科用留め具を付ける必要がある9例えば、グリ
ーンの米国特許第4,402,444号に示す装置は間隔の狭い4列の50本、
60本或はもっとそれ以上の留め具を付けるようになっている。これらの留め具
をすべてぴったり同時に打込む鳩舎には、必要とされるピーク力は大変大きくな
る。これは、大変強く、その分コスト高になる器具の設計を必要とする。これは
又、器具を比較的操作しにく(する。
従って本発明の目的は、比較的多数の外科用留め具を付ける器具に必要とされる
ピーク力を減少させることにある。
発明の概要
本発明のこの目的およびその他の目的は、本発明の原理によれば、留め具打込み
組立体の剛性部材の一端に隣接する留め具を前記剛性部材の他端に隣接する留め
具に先行して付けるために、留め具打込み組立体の剛性部材の一端を、最初に剛
性部材の他端よりも早く留め具打込み方向に移動させるための装置を有する外科
用留め臭性は装置を提供することによって達成される。このようにして、留め臭
性は荷重を時間に亘って分布させ、必要とされるピーク力を、留め具のすべてを
ぴったり同時に付けるのに要するピーク力と比較して減少させる。
本発明の一層の特徴、その性質、および種々の利点は添附図面および本発明の以
下の詳細な説明からもっと明らかになろう。

発明の詳細な説明
本発明を採用することができる環境の中で、関節式外科用留め興行は器具が同日
出願の米国特許第662,677号(ケースUSSC1052)に示されている
。これらの器具のうちの1つ(上記の同日出願の第1図乃至第12図と夫々等し
い第1図乃至第12図に示す)を先ず詳細に説明することにする。次いで、本発
明によるその装置の変形例を第13図を参照して説明することにする。
第1図に示すように、本発明により変形しうる外科用留め臭性は器具10は基部
に近い方の作動器組立体20と、末端の方の留め臭性は組立体200と、中間の
シャフト組立体100とを有する。
留め臭性は組立体200は基部に近い方の留め具保持部品210および末端の方
のアンビル部品280を有する。アンビル部品280は、留め具保持部品210
に対して一定の回動および並進運動可能に取付けられ、従ってアンビル部品28
0を第1図に実線で示すように、留め具保持部品210から並進かつ回動させ、
留めるべき組織12を留め具保持部品210の末端の方の表面とアンビル部品2
80の基部に近い方の表面との間に挿入することができる。
留め具保持部品210は、シャフト組立体の関節即ち継手150をちょうど越え
たところでシャフト組立体100の末端の方の端部に連結されている。図示した
装置では、継手150は、球形ボール152 (第2図)を補足し合うソケット
160に回転自在に取つけたものからなる。継手150により、留め臭性は組立
体200を、すべてボール152の中心で交差する互いに直交の3軸線102.
104.106の各々を中心に回転させる。留め臭性は組立体200のいくつか
のありうる位置を第1図に仮想線で示す。シャフト組立体10.0の基部に近い
方の端部を、器具の長手方向軸線である軸vA102を中心に組立体100およ
び200が更に回転運動できるように作動器組立体20に回転自在に取付けても
良い。継手150および作動器組立体20におけるシャフト組立体100の回転
取付は部以外、シャフト組立体100はその長手方向軸線に対して横方向に実質
的に剛性である。シャフト組立体100は又軸線102と平行に実質的に剛性で
ある。
作動器組立体20は、基部に近い方のハンドル30と、手動操作可能なりランプ
作動器レバー40と、手動操作可能な留め具作動器レバー60とを有する。作動
器レバー40を作動器組立体20の中へ下方に回動させると、アンビル部品28
0の下方部分を留め具保持部品210に向かって引っ張る。留め具作動器レバー
60をハンドル30に向かって強く握ると、留め具保持部品210はこの部品の
中に収容されている外科用留め具をアンビル部品280に向かって末端の方向に
打ち込む。安全掛は金90を第3図に仮想線で示すように、レバー60から下方
に回動させるまでは、留め具作動器レバー60を操作することができない。
本装置の構造をより詳細に考える前に、装置の操作全体を概略的に説明すること
にする。装置の最初の状態を第1図、第2図および第8図に示す。留め臭性は組
立体200を、所望ならば、軸線102.104.106のうちの任意の1つを
中心に希望量だけ回転させて、留め臭性は組立体200が作動器組立体20に対
して希望の角度向きをもつようにする。継手150およびシャフト組立体100
と作動器組立体20との間の回転連結部は好ましくは十分に堅いから、装置の次
の操作中、組立体20.200は、これらが取るどんな相対的角度向きをも維持
する。
留めるべき組織12はアンビル部品280と留め具保持部品210との間に置か
れる。アンビル部品280の上方部分を第9図に示すように、留め具保持部品2
10の対向した上方部分に向かって手で移動させ、掛は金ボタン212を末端の
方向に手で押し、その結果、掛は金214がアンビル部品280のキャッチ28
2に係合する。これはアンビル部品280の上方端を留め具保持部品210の上
方端に対して保持して組織をこれらの部品間に締付は始める。
次の操作段階はクランプ作動器レバー40を第3図に示すように作動器組立体2
0の中へ下向に回動させることである。これは第3図および第10図に示すよう
に、アンビル部品280の下方端を留め具保持組立体210の下方端に向かって
引き込む。今や、部品210と280は互いに実質的に平行になって組Wi12
をこれらの部品の封向表面間にしっかりと締付ける。今や、組織は留められよう
としている。
留め具を付けるときには、安全掛は金90を第3図に仮想線で示すように下方に
回動させる。次いで、留め具作動器レバー60を、これ又第3図に仮想線で示す
ように、ハンドル30に向かって回動させる。これにより、留め具保持組立体2
10は第11図に示すように、複数の金属製外科用ホチキス針202の脚部を組
織12にそしてアンビル部品280に向かって同時に或は実質的に同時に打ち込
む。アンビル部品280はホチキス針の脚部の端を折り曲げてホチキス針を組織
に固定する。 (金属製ホチキス針がこの装置に採用されているが、グリーンの
米国特許第4.402,445号に示すもののような2部品形プラスチック留め
具を変形例として使用しうろことは理解されよう)、今や、組織留め手順は完了
である。
留めた組織は第12図に示すように、掛は金ボタン212を手で押し上げること
によ1て装置から取り出される。これにより、掛は金214はキャッチ282を
解放し、これにより、アンビル部品280を留め具保持部品210から回動させ
る。今や、留めた組織を装置から取り出すことができる。
今、装置の内部構造および操作を先ず、第2図を参照して説明すると、クランプ
作動器レバー40は、ピボントビン42によって作動器組立体フレーム22に回
動自在に連結されている。トグルリンク44の一端はピン46によってレバー4
0に回動自在に連結され、他端はピン48によってスライド50に回動自在に連
結されている。スライド50はフレーム22のスロット24に長手方向往復運動
可能に取付けられている(第4図も参照)。レバー40を第2図および第4図に
示すように、作動器組立体20の残部から外に回動させるときには、スライド5
0はスロット24内の末端の方の位置にある。レバー40を第3図に示すように
、作動器組立体20の残部に向かって下方に回動させるときには、スライド50
はスロット24の中で基部に近い方に移動する。レバー40の完全操作位置では
、ピン46はピン42.48を通る直線を僅かに通り過ぎているから、レバー4
0は完全操作位置にとどまるようになる。
第1の横方向に撓む部材110の基部に近い方の端はスライド50に連結される
0部材110は金属ワイヤでも良いし或はケーブルでも良(、ここでは便宜上、
部材110をケーブル110と称する。ケーブル110は、シャフト組立体10
0の長さのほとんどの外側部材である中空管状シャフト120を通過する。ケー
ブル110は又ボール152の長手方向孔154(第7図)を通して留め興行は
組立体200の中へ通る。ケーブル110の末端の方の端はアンビル部品280
に連結される(第5図も参照)。
従って、レバー40を作動器組立体20の残部に向かって下方に回動させるとき
には、ケーブル110はアンビル部品280の下方端を留め具保持部品210に
向かって引き入れる。ケーブル110は横方向に撓むことができるから、ケーブ
ル110は継手150の回動を干渉しない、又、ケーブル110は軸!102.
104.106の交点(第1図)を通過するから、レバー40の操作によって張
力状態を取るときでも、ケーブル110は組立体20および200の相対的な回
転向きを変えない。
第2図に戻ると、留め具作動器レバー60はピン26によってフレーム22に回
動自在に連結されている。安全掛は金90もピン28によってハンドル30に回
動自在に連結されている。第2の横方向に撓む部材112の基部に近い方の端は
アンカー114でフレーム22に固定的に取付けられている0部材112はケー
ブル110と同様であるのが良く、従ってこれをケーブル112と称する。ケー
ブル112の基部に近い方の部分は、軸64によってレバー60に回転自在に取
付けられているローラ62のまわりを通る。ローラ62を通過した後、ケーブル
112はシャフト120および継手150を末端方向に通過して留め具保持部品
210の中へ通る。第5図および第8図で最も良くわかるように、ケーブル11
2が留め具保持部品210に入るとき、ケーブル112はローラ220のまわり
を通る。ローラ220はケーブル112をアンカー222まで向き変えし、この
アンカー222はケーブル112の末端の方の端をカムパー230に連結する。
従ヮてレバー60をハンドル30に向かって回動させるときには、ケーブル11
2はカムパー230を引き下げる。ケーブル110と同じように、ケーブル11
2は軸線 102.104.106の交点を通過し、そして十分に横方向に撓む
ことができるので、ケーブル112は、レバー60の操作によって張力状態を取
るときでも継手150の回動を干渉せず、即ち、組立体20と200の相対的な
回転向きを変える。
カムパー230は、ブツシャ作動器部材240の基部に近い方に向いた溝の中に
配置されている(第6図参照)。カムパー230の末端の方の面は部材240の
この溝の底に当ってこの底に沿って摺動できる。ブツシャ作動器部材240は留
め具保持部品210の2つの平行な側方に間隔をへだてたフレーム部材250
(第5図)間に配置されている。ブツシャ作動器部材240の末端の方の表面は
ブツシャ部材260の基部に近い方の端に当り、ブツシャ部材260は、ホチキ
ス針202の後で、留め具ホルダ270の孔272の中へ延びるブツシャフィン
ガ262を有している。
ブツシャ作動器部材240は、ブツシャ部材260によってホチキス針202が
打ち込まれる軸線と実質的に平行に移動するように構成されている。この拘束は
、部材240からフレーム部材250のガイトスロア)252の中へ側方かつ外
方に延びるガイドピン242によって行なわれる。ブツシャ作動器部材240も
又ピン242を中心に回動することができる。
カムパー230がケーブル112によって引き下げられると、;h ムハ−23
0ヲlji!してフレーム部材250のカムスロット254の中へ側方に突出す
るカムホロワ−ピン232の作動でカムパー230を末端の方向に強制的に移動
させる。カムスロット254は(頃斜しているから、カムパー230がケーフ゛
ル112の運動に応答して下方に移動すると、カムパー230は末端方向にも移
動する。カムパー230はブツシャ作動器部材240に沿って長手方向に摺動す
るから、カムパー230の末端方向の運動だけが部材240に及ぼされる0部材
240が末端方向に移動すると、部材240はブツシャ部材260、従ってホチ
キス針202を末端の方向に駆動する。かくして、要素230.232 、24
0.242.250.252.254および260は、ケーブル112の末端の
方の端の下方運動をホチキス針202の末端方向の運動に変換するための装置を
構成する。
シャフト組立体100と作動器組立体20の相対的回転を可能にするこれらの組
立体間の連結部は第4図で最も良くわかる。シャツ)120の基部に近い方の端
は、作動器組立体20の外側シェルの2つの対象の半部70a、70bに形成さ
れている円筒形ソケット80に回転自在に取付けられている。シェル半部70は
、リベット、接着剤等のような任意適当な手段で互いに保持されている。シャフ
ト120の環状カラー122はソケット80の環状拡大部82に受け入れられて
シャフト120を作動器組立体20に対して的確な長手方向位置に保持する。所
望ならば、作動器組立体20に対するシャフト120の回転量をシャフト120
のストップ124と作動器組立体のシェルフ0のストップ84とによって制限す
るのが良い。シェル半部70は好ましくは、シャフト120に十分な力で係合し
て装置の使用者が確立したシャフト120と作動器組立体20の相対的な角度向
きを摩擦的に維持する。
継手150の詳細な構造を第7図に示す、中空の一般的に截頭円錐形の脚部15
6の末端の方の端にはボール152が形成されている。中空の円筒形シャンク1
58ガ脚部156の基部から基部の方向に延びている。シャンク158はシャフ
ト120の末端の方の端に圧力嵌めされている。ボール152はソケット160
にぴったりと嵌まり、その構造は第9図でもっとも良くわかる。
ソケット160は末端の方の座部材162を有し、座部材162は軸224でロ
ーラ220を支持し、かつピン164によってフレーム部材250に固定的に取
付けられている(第7図)。座部材162は中空であり、そして、ボール152
の末端の方の端部分を受け入れるための基部に近い方に向いた環状座面166を
有している。座部材162の円筒形外表面の基部に近い方の部分はねし山をもっ
ていてカラー170を受け入れる。カラー170は環状保持リング172をボー
ル152の表面の基部に近い方の部分に押しつける。従って、ボール152は座
表面166と保持リング172との間に保持されるが、軸線102.104.1
06の各々を中心にこれらの部材に対して回転ずことができる。表面166およ
び172は好ましくは、ボール152に十分な力で係合して組立体20と200
の相対的な角度向きが装置の使用者によってどんなに確立されても摩擦的に維持
する。特に好ましい実施例では、ボール152はスチールであり、部材162お
よび172はナイロンのような熱可望性材料で作られる。
アンビル部品280は、フレーム部材250の細長いスロット256の中にある
ピン290によって留め具保持部品210に回動自在に連結されている。スロッ
ト256は装置の長手方向軸線と平行に細長く、従ってアンビル部品280はピ
ン290を中心に回動することができるとともにスロット256と平行に並進す
くことができる。アンビル部品280は、アンビル部品280と留め具保持部品
210との間で圧縮される圧縮コイルばね292によって留め具保持部品210
から遠ざかるように弾性的に付勢されている。掛は金214は、掛は金214と
留め具保持部品210の本体とに作用する圧縮コイルばね216によって、例え
ば第8図に示すように、基部の方向に弾性的に付勢されている。
上記の装置は第13図に示すように、本発明によって変更することができる。第
1図乃至第12図の装置に採用されている4つの等しいカムスロット254の代
わりに、第13図では、2つの上方カムスロソ)354 (第13図では、その
うちの1つだけが見える)は互いに等しいが2つの等しい下方のカムスロット4
54(第13図では、そのうちの1つだけが見える)とは異なる。特に、下方カ
ムスロット454の各々は最初の部分454aを有し、この部分は、上方カムス
ロット354の対応する最初の部分354aよりも末端の方向にもっとゆるやか
に傾斜している。これにより、カムパー230の下方端およびプッシャ作動器部
材240の下方端を最初に、これらの部材の上方端よりももっと急速に末端の方
向に移動させる。これにより、留め具保持部品210の底に近いホチキス針20
2を留め具保持部品210の頂部に近いホチキス針202よりももっと急速に組
織12に向かって前進させる。
従って、ホチキス針の端はすべて、アンビル部品280に同時に達しない。むし
ろ、留め具保持部品210の底に近いホチキス針が先ずアンビル部品280に達
して頂部に近いホチキス針の前に、アンビル部品に向かって折曲げを開始する。
従って、各ホチキス針を折曲げるための最大力即ち「ピーク荷重」は他のホチキ
ス針を折曲げるためのピーク荷重よりは異なる時間に起こることになる。これは
個々のホチキス針折曲げピーク荷重を時間に亘って分布させ、これによって、ホ
チキス針を打込むのに要する全体のピーク荷重を減少させる。
最初の部分354a、454aに続いて、カムスロット454の各々は中間部分
454bを有し、この中間部分は、カムスロット354の対応する中間部分35
4bと実質的に平行か或はこれよりもほんの僅かゆるやかに傾斜している。これ
は、ホチキス針202のほとんど又はすべてが少な(とも折曲げを開始しており
、従ってそれらのピーク荷重の要求を終えているまで要素230.240の下方
部分をこれらの要素の上方部分よりももっと末端の方に前進させるような装置の
状態を維持する。
カムスロット354.454の最終部分354C1454Cは、中間部分よりも
もっと幾分かゆるやかに末端の方向に傾斜しているのが良い。必要とする力がピ
ーク荷重よりも実質的に小さいとき、ホチキス針202の最終圧縮中にカムホロ
ワ−ピン232がこれらの最終部分を通る0両カムホロワ−ピン232がそれら
の走行端に達したとき、カムホロワ−ピン232が両方ともそれらの出発位置か
ら同じ末端方向の距離移動しているように、カムスロット部分354Cの傾斜角
度はカムスロット部分454Cの傾斜角度と異なるのが良い。これにより、ホチ
キス針の打込み行程中、部材230.240はアンビル部品280の(垂直)長
手方向軸線に対して計画通り傾斜するが、ホチキス針打込み行程は、部材230
と240がその軸線と実質的に並行になって終わる。
これはホチキス針のすべての圧縮を均一にする。
本発明を、平行な2列の外科用留め具を付ける外科用留め具付は器具について図
示したけれども、付けるべき留め具の数が多ければ多いほど、本発明の利点は大
きくなる。例えば、本発明は、例えば、グリーンの米国特許第4,402,44
4号に示されているように平行な4列の外科用留め具を付ける器具の場合にはも
っと有利である。
本発明は、第1図乃至第12図に示す型式の外科用留め具付は装置の適用に限ら
れないことを理解すべきである。例えば、その装置は関節式シャフト組立体を有
しているが、本発明は、横方向に撓み或は完全に剛性のシャフト組立体のような
他の型式のシャフト組立体をもつ器具に等しく適用できる。同様に、上記の装置
に金属製外科用ホチキス針202を採用しているが、その代わりに、他の型式の
外科用留め具、例えばグリーンの米国特許第4.402.445号に示すものの
ような2部分形プラスチック留め具を使用しても良い。又、採用される金属製ホ
チキス針、ブラスチレク留め具のいずれもが当業者において周知のように、生物
学的に不活性であっても良いし、或は生物学的に吸収できるのが良い。

Claims (7)
- 1.長手方向の列の外科用留め具を身体組織に付けるための装置であって、 長手方向の列の複数本外科用留め具と、長手方向の列と平行でかつこの列と同延 の第1の長手方向剛性部材を有し、留め具を長手方向の列と直交する軸線に沿っ て組織に押し込むための留め具プッシャ装置と、留め具を組織に押し込むのに要 する仕事を供給するための第2の部材と、 第1部材の一端が最初に、第1部材の他端よりも実質的に速く直交軸線と平行に 移動して第1部材の前記一端に隣接する留め具を第1部材の前記他端に隣接する 留め具に先立って組織に付けるために、第2部材によって供給された仕事を第1 部材に伝達するための第1装置と、 からなることを特徴とする装置。
- 2.第1装置は、留め具が付けられるとき、直交軸線と平行な、第1部材の前記 他端の運動速度を増すたあの第2装置を有する、請求の範囲第1項に記載の装置 。

- 6.第1カムおよびカムホロワー組立体のカムホロワーが移動するカム面の最初 の部分は、第2カムおよびカムホロワー組立体の対応するカム面の部分よりもも っと直交軸線と平行に近い、請求の範囲第5項に記載の装置。
- 7.第1カムおよびカムホロワー組立体のカム面の最初の部分の後、第2カムお よびカムホロワー組立体の対応するカム面の部分よりももっと長手方向の列と平 行に近い前記カム面の部分がある、請求の範囲第6項に記載の装置。
