Claims (1)
- 【特許請求の範囲】 1.先端部(22)と、基端部(82)と、長手方向の軸線とを有するワイヤ( 10)を備え、前記先端部はアンカー(24)に形成され、該アンカーは操作に より前記基端部から解放することができることを特徴とするガイドワイヤ。 2.前記アンカーは前記先端部を曲げ返して曲げ部を形成することにより形成さ れることを特徴とする請求の範囲第1項に記載のガイドワイヤ。 3.前記ガイドワイヤはコイルワイヤ(12)と、外側シース(14)とから構 成されていることを特徴とする請求の範囲第2項に記載のガイドワイヤ。 4.前記曲げ部における前記ガイドワイヤの曲げ性は前記曲げ部に近い前記ガイ ドワイヤの曲げ性よりも大きいことを特徴とする請求の範囲第3項に記載のガイ ドワイヤ。 5.前記増大された曲げ性は前記曲げ部の領域における前記コアワイヤの直径を 小さくすることにより得られることを特徴とする請求の範囲第4項に記載のガイ ドワイヤ。 6.前記コアワイヤの前記縮径領域の周囲に巻回されたワイヤスプリング(96 )を更に備えることを特徴とする請求の範囲第5項に記載のガイドワイヤ。 7.前記コアワイヤは基端部(12)と先端部(12’)との分離され、該2つ の部分間の分離部は前記曲げ部の領域にあることを特徴とする請求の範囲第4項 に記載のガイドワイヤ。 8.前記曲げ部に近接する前記コアワイヤの部分の周囲に配置されたカニューレ (88)を更に備えることを特徴とする請求の範囲第5項に記載のガイドワイヤ 。 9.前記カニューレの端部間において前記外側シースに配設された外部ショルダ (92、94)を更に備えることを特徴とする請求の範囲第8項に記載のガイド ワイヤ。 10.患者の皮膚の穴(50)と、患者の動脈(44)の1つにある穴(52) と、これら2つの穴を接続するチャンネル(46)とから構成される患者の傷へ の止血材(68)の挿入を容易にするガイドワイヤであって、 長手方向の軸線と、基端部(82)と、先端チップ(16)を有する先端部( 22)とを有する細長い中間部を備え、前記先端部は前記長手方向の軸線と直 交する成分を有するアンカーロープ(24)に形成されるようになっており、前 記アンカーループは操作により前記基端部から解放されることを特徴とするガイ ドワイヤ。 11.コアワイヤ(12)と外側シース(14)とを備えることを特徴とする請 求の範囲第10項に記載のガイドワイヤ。 12.前記アンカーの解放は前記コアワイヤを前記シース内で動かすことにより 行われることを特徴とする請求の範囲第11項に記載のガイドワイヤ。 13.前記アンカーの解放は前記コアワイヤを前記シース内で前記先端部から離 れる方向へ動かすことにより行われることを特徴とする請求の範囲第12項に記 載のガイドワイヤ。 14.前記外側シースにはスリットが設けられ、前記アンカーループは前記先端 チップを前記ガイドワイヤに対して曲げ戻しかつ前記シースにスリットが形成さ れている位置において前記コアワイヤと前記外側シースとの間に前記先端チップ を通すことにより形成されることを特徴とする請求の範囲第11項に記載のガイ ドワイヤ。 15.前記外側シースと前記コアワイヤとの間に前記ガイドワイヤの前記先端チ ップを通すことにより前記ガイドワイヤを円または楕円に形成し、その一部は前 記長手方向の軸線の一方の側にあり、その一部は前記長手方向の軸線の別の側に あることを特徴とする請求の範囲第14項に記載のガイドワイヤ。 16.前記外側シースにはスリットが設けられ、前記アンカーループは前記コア ワイヤの中間部(84)を前記スリット介して引っぱり出して前記コアワイヤの 該引っぱり出し部が前記長手方向に対して約180°よりも小さい角度を形成す ることにより形成されることを特徴とする請求の範囲第11項に記載のガイドワ イヤ。 17.前記スリットからの所定の距離を示す位置決め手段(28)を更に備える ことを特徴とする請求の範囲第14項に記載のガイドワイヤ。 18.患者の皮膚の穴(50)と、患者の動脈(44)の1つにある穴(52) と、これら2つの穴を接続する傷チャンネル(46)とから構成される患者の傷 へ止血材(68)を挿入する方法であって、 基端部(82)と、先端部(22)と、中間部と、長手方向の軸線とを有する ガイドワイヤ(10)を前記傷に挿入する工程を備え、前記先端部はアンカー( 24)として形成され、前記挿入は前記アンカーが前記動脈の穴を通過しかつ前 記基端部が患者の身体の外側に位置するまで継続され、更に 前記アンカーが前記動脈の内側に当接するまで前記ガイドワイヤを後方へ引っ 張る工程と、 前記止血材が前記動脈の穴に隣接する前記傷チャンネルに位置するまで前記止 血材を前記ガイドワイヤおよび前記中間部に沿って挿入する工程と、 前記アンカーを解放するとともに前記ガイドワイヤを前記傷から取り除く工程 とを備えることを特徴とする方法。 19.シース(54)/拡張器(56)のセットを前記ガイドワイヤに沿って挿 入するとともに、前記止血剤を前記シースを介して挿入する前に前記シース/拡 張器のセットの拡張器の部分を取り除く工程を更に備えることを特徴とする請求 の範囲第18項に記載の方法。 20.前記止血剤は予め装填されたキャニスタ(66)に収容されかつ前記シー スを介して前記傷チャンネルに挿入されることを特徴とする請求の範囲第19項 に記載の方法。 21.前記シースが前記動脈の穴に対して前記傷チャンネル内に正しく配置され たことを定めるように位置決め手段(28)が配設されており、更に前記シース が前記動脈の穴に対して所定の位置に位置決めされたことを前記位置決め手段が 示すまで前記シース/拡張器のセットを挿入する工程を更に備えることを特徴と する請求の範囲第18項に記載の方法。 22.患者の傷が患者の皮膚の穴(50)と、患者の動脈(44)の1つにある 穴(52)と、これら2つの穴を接続する傷チャンネル(46)とから構成され 、前記動脈の内側に配置される先端チップ(40)を有しかつその基端部(34 )が患者の外側に位置する処置シース(36)を使用する、患者の傷へ止血材( 68)を挿入する方法であって、 ガイドワイヤ(10)を前記処置シースに挿入する工程を備え、前記ガイドワ イヤは基端部(82)と、先端部(22)と、中間部と、長手方向の軸線とを有 し、前記先端部はアンカー(24)として形成され、前記挿入は前記アンカーが 前記処置シースの先端部から現れるまで継続され、更に 前記処置シースを患者から取り除く工程と、 前記アンカーが前記動脈の内側に当接するまで前記ガイドワイヤを後方へ引っ 張る工程と、 前記止血材が前記動脈の穴に隣接する前記傷チャンネルに位置するまで前記止 血材を前記ガイドワイヤおよび前記中間部に沿って挿入する工程と、 前記アンカーを解放するとともに前記ガイドワイヤを前記傷から取り除く工程 とを備えることを特徴とする方法。 23.漏斗(30)を使用して前記アンカーを前記処置シースに挿入する工程を 更に備えることを特徴とする請求の範囲第22項に記載の方法。 24.前記処置シースの前記除去後にガイドシース(54)を前記ガイドワイヤ に沿って挿入する工程を更に備えることを特徴とする請求の範囲第22項に記載 の方法。 25.前記止血材は前記ガイドシースに挿通されることを特徴とする請求の範囲 第24項に記載の方法。 26.前記止血剤は予め装填されたキャニスタ(66)に収容され、更に前記止 血剤を前記キャニスタから前記ガイドシースの中へ付勢する工程を更に備えるこ とを特徴とする請求の範囲第25項に記載の方法。 27.傷チャンネル(46)と皮膚の穴(50)とを介して外界に連通されてい る内部の傷からの血液の流れを止める装置であって、 基端部(82)と、先端部(22)と、長手方向の軸線とを有するガイドワイ ヤ(10)を備え、前記先端部はアンカーの形状(24)に形成され、該アンカ ーの形状は前記ワイヤを操作することにより前記基端部から解放することができ 、更に 前記傷チャンネルを通るサイズに形成され、かつ、前記ガイドワイヤの前記基 端部が摺動通過することができる通しルーメン(67)を有する止血剤(68) のプラグと、 前記止血材を前記傷チャンネルを介して前記内部の傷に付勢する手段(72) とを備えることを特徴とする装置。 28.前記ガイドワイヤの前記基端部が摺動通過することができる通しルーメン を有するシース(54)/拡張器(56)のセットを更に備えることを特徴とす る請求の範囲第27項に記載の装置。 29.前記傷チャンネルを介しての前記止血材の通過を容易にするシース(54 )を更に備えることを特徴とする請求の範囲第27項に記載の装置。 30.前記止血材が前記内部の傷に隣接して位置することを示す手段(28)を 更に備えることを特徴とする請求の範囲第27項に記載の装置。 31.前記ガイドワイヤの前端部の前記傷チャンネルへの挿入を容易にする漏斗 (30)を更に備えることを特徴とする請求の範囲第27項に記載の装置。 32.先端部と基端部とを有するガイドワイヤであって、 基端部(240)および先端部(226、306)を有するコアワイヤ(21 2、302、554)と、 基端部(236)と、先端部(218、316)と、内部に配置されたルーメ ン(248、314)と、該ルーメンを画成する壁と、前記先端部に近接して前 記壁に設けられたノッチ(220、318、368)とを有し、前記コアワイヤ が前記ルーメンに摺動自在に収容されるサイズに形成されたコアワイヤシース( 214、304、352)と、 前記コアワイヤに取着された先端部(230、362)と、基端部(232) と、前記先端部と基端部との間に画成されかつ前記ノッチを摺動自在に通ること ができるサイズに形成された本体部とを有するかえし(228、308、356 )とを備え、 前記かえしの前記基端部が前記ルーメンの外側にあるときに前記ルーメン内で 前記コアワイヤが軸線方向に動くことにより、前記かえしの前記本体部が前記ノ ッチを介して前記ルーメンの中に摺動することを特徴とするガイドワイヤ。 33.前記挿入ガイドワイヤの前記先端部に配設された柔軟なチップ(310) を更に備えることを特徴とする請求の範囲第32項に記載の挿入ガイドワイヤ。 34.前記柔軟なチップは前記コアワイヤシースに取着されていることを特徴と する請求の範囲第33項に記載の挿入ガイドワイヤ。 35.前記柔軟なチップは前記コアワイヤに取着されていることを特徴とする請 求の範囲第33項に記載の挿入ガイドワイヤ。 36.前記柔軟なチップは前記ルーメン内で摺動自在であることを特徴とする請 求の範囲第35項に記載の挿入ガイドワイヤ。 37.前記かえしは前記コアワイヤの軸線方向の動きにより前記ルーメン内に完 全に退却されることを特徴とする請求の範囲第32項に記載の挿入ガイドワイヤ 。 38.前記かえしの前記基端部に配設されたJ−チップ(320、364)を更 に備えることを特徴とする請求の範囲第32項に記載の挿入ガイドワイヤ。 39.前記コアワイヤシースはワイヤコイル(222)から構成されていること を特徴とする請求の範囲第32項に記載の挿入ガイドワイヤ。 40.前記コアワイヤシースは生体親和性プラスチックから構成されていること を特徴とする請求の範囲第32項に記載の挿入ガイドワイヤ。 41.前記ノッチは前記コアワイヤシースを通るスリットからなることを特徴と する請求の範囲第32項に記載の挿入ガイドワイヤ。 42.前記コアワイヤシースは基端部(352a)と先端部(352b)とを有 し、該基端部と先端部は分離部で互いに分離して支持されており、前記ノッチは 前記分離部からなることを特徴とする請求の範囲第32項に記載の挿入ガイドワ イヤ。 43.前記基端部と先端部は少なくとも1つの安全ワイヤ(224、358、3 60)により互いに分離して支持されていることを特徴とする請求の範囲第42 項に記載の挿入ガイドワイヤ。 44.前記基端部と先端部は少なくとも一部が介在するプラスチック充填材(3 72)により分離して支持されていることを特徴とする請求の範囲第42項に記 載の挿入ガイドワイヤ。 45.安全ワイヤ(224、358、360)を更に備えることを特徴とする請 求の範囲第32項に記載の挿入ガイドワイヤ。 46.前記コアワイヤを前記ルーメン内で軸線方向に移動する手段(242)を 更に備えることを特徴とする請求の範囲第32項に記載の挿入ガイドワイヤ。 47.前記コアワイヤ移動手段は前記挿入ガイドワイヤの前記基端部からの前記 コアワイヤの前記軸線方向の動きを制御することを特徴とする請求の範囲第46 項に記載の挿入ガイドワイヤ。 48.前記コアワイヤの先端方向の軸線に沿った動きに一時的に抵抗する手段( 238)を更に備えることを特徴とする請求の範囲第32項に記載の挿入ガイド ワイヤ。 49.前記抵抗手段はスプリットスリーブからなることを特徴とする請求の範囲 第48項に記載の挿入ガイドワイヤ。 50.先端部と基端部とを有する挿入ガイドワイヤであって、 基端部(442)と、先端部と、内部にルーメン(432)とを有するシース (414)を備え、該シースの前記先端部はワイヤコイルからなるとともに該ワ イヤコイルがオーバーラップ部(450)において自身がオーバーラップするよ うに360°よりも大きい全弧を有するループ(426)に形成されており、前 記オーバーラップ部における前記コイルの少なくとも幾つかは交互に重ね合わさ れており、更に 前記ルーメン内に配置されたコアワイヤ(434)を備え、該コアワイヤは基 端部(436)と先端部(438)とを有し、前記コアワイヤの前記先端部は曲 げ戻されてフック(440)を形成し、該フックは前記オーバーラップしたコイ ルを前記重ね合わされた状態に保持することを特徴とする挿入ガイドワイヤ。 51.前記コアワイヤは前記ルーメン内で摺動自在となっていることを特徴とす る請求の範囲第50項に記載の挿入ガイドワイヤ。 52.前記コアワイヤを前記ルーメン内において軸線方向に移動させる手段(4 46)を更に備えることを特徴とする請求の範囲第50項に記載の挿入ガイドワ イヤ。 53.前記コアワイヤは前記オーバーラップしたコイルを前記重ね合わされた状 態から解放することにより前記ループを真っ直ぐに延ばすことができるように前 記シースの前記先端部へ向けて摺動自在となっていることを特徴とする請求の範 囲第52項に記載の挿入ガイドワイヤ。 54.前記コアワイヤ移動手段は前記挿入ガイドワイヤの前記基端部からの前記 コアワイヤの前記軸線方向の動きを制御することを特徴とする請求の範囲第52 項に記載の挿入ガイドワイヤ。 55.前記ワイヤコイルは反対方向に巻かれた2つの隣接するコイル(416、 418)からなり、前記コイルの前記オーバーラップ部は反対方向に巻かれた前 記コイルを有することを特徴とする請求の範囲第52項に記載の挿入ガイドワイ ヤ。 56.安全ワイヤ(412)を更に備えることを特徴とする請求の範囲第50項 に記載の挿入ガイドワイヤ。 57.前記コアワイヤの先端方向への軸線に沿った動きに一時的に抵抗する手段 (444)を更に備えることを特徴とする請求の範囲第52項に記載の挿入ガイ ドワイヤ。 58.前記抵抗手段はスプリットスリーブからなることを特徴とする請求の範囲 第57項に記載の挿入ガイドワイヤ。
