Description
遠位部分および近位部分を有する細長いカテーテル部材(2)と、
カテーテル部材の遠位部分に配置され、収縮した配向と血栓をブロックする拡張した配向との間で径方向に拡張可能な血栓ブロッキング体(5)と、
血栓ブロッキング体に対して軸方向に離間した関係でカテーテル部材の遠位部分に配置され、収縮した配向と血栓を捕捉する拡張した配向との間で径方向に拡張可能な血栓捕捉ケージ(3)であって、ブロッキング体に面しかつ物質を受け入れるための開口を有する、内方に向かって細くなっている前端部を含む、血栓捕捉ケージと、
血栓捕捉ケージおよび血栓ブロッキング体を展開および収縮させるように作動可能な展開手段と、
血栓捕捉ケージに動作可能に接続され、血栓捕捉ケージを体内腔に沿って軸方向に移動させるように構成された細長い制御アームであって、ケージの前端部に動作可能に接続される、細長い制御アームと、
血栓捕捉体内に配置された血栓摘出器またはマセレータ機構と
を備え、
血栓捕捉ケージが理想的には血栓ブロッキング体よりも遠位に配置されるデバイスが提供される。
遠位部分および近位部分を有する細長いカテーテル部材と、
カテーテル部材の遠位部分に配置され、収縮した配向と血栓を捕捉する拡張した配向との間で径方向に拡張可能な血栓捕捉体と、
場合により、カテーテル部材の遠位部分に配置され、収縮した配向と血栓をブロックする拡張した配向との間で径方向に拡張可能な血栓ブロッキング体と、
血栓捕捉体および血栓ブロッキング体を展開および収縮させるように作動可能な展開手段と、
血栓捕捉体に動作可能に接続され、血栓捕捉体を体内腔に沿って軸方向に移動させるように構成された細長い制御アームと
を備え、
血栓捕捉体、血栓ブロッキング体、またはカテーテル体のうちの少なくとも1つを振動させるように構成された振動作動手段を含む振動手段を備えることを特徴とするデバイスを提供する。
カテーテルの長手軸の周りで回転するように構成された振動性回転部材であって、カテーテルの長手軸に対して偏心した重心を有する振動性回転部材、および/または、
カテーテルの長手軸に対してオフセットした軸の周りで(場合により、不均一な円周方向摩擦を含むかまたは含まずに)回転するように構成された振動性回転部材
を含むことが好ましい。
「血栓」は、血管または内腔の壁に通常付着する、固体または半固体または粘性流体の塊を意味すると理解されるべきである。「長い血栓」は、血管径の少なくとも2倍または3倍の軸方向長さを有する血栓を意味すると理解されるべきである。
・ 摘出器ねじの回転方向は、摘出ではなく注入のように変更され得る。
・ 中空の遠位アームを通じて注入する。
・ 遠位制御アームと摘出器チューブ(近位アーム)との間の内腔を通じて注入する(図9)。
・ 摘出器チューブ(近位アーム)とシースとの間で注入される(図10)。
・ シース50内の空洞51を通じて注入される(図11)。
・ シースおよび空洞の位置は、カテーテルの長さに沿って調節され得る。
・ 上記の注入方法のうちの1つまたは組合せ。
Claims (28)
- 体内腔から物質を除去するための血栓切除デバイスであって、
遠位部分および近位部分を有する細長いカテーテル部材(2)と、
前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓をブロックする拡張した配向との間で径方向に拡張可能な血栓ブロッキング体(5)と、
前記血栓ブロッキング体に対して軸方向に離間した関係で前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓を捕捉する拡張した配向との間で径方向に拡張可能な血栓捕捉ケージ(3)であって、物質を受け入れるための開口を有する、血栓捕捉ケージと、
前記血栓捕捉ケージおよび血栓ブロッキング体を展開および収縮させるように作動可能な展開手段と、
前記血栓捕捉ケージに動作可能に接続され、前記血栓捕捉ケージを前記体内腔に沿って軸方向に移動させるように構成された細長い制御アームであって、前記血栓捕捉ケージの前端部に動作可能に接続される、細長い制御アームと、
前記血栓捕捉ケージ内に配置された血栓マセレータ機構と
を備え、
前記血栓ブロッキング体は、前記血栓捕捉ケージより近位側にのみ配置され、
前記細長い制御アームは、血栓を捕捉及び除去するために、前記血栓捕捉ケージを前記血栓ブロッキング体に向かって移動させることを特徴とするデバイス。 - 前記血栓捕捉ケージ内の前記マセレータ機構が、前記血栓捕捉ケージ内にある血栓を分解することを特徴とする請求項1に記載のデバイス。
- 前記血栓捕捉ケージ内の前記マセレータ機構が、前記血栓捕捉ケージ内にある血栓を取り除くことを特徴とする請求項1または請求項2に記載のデバイス。
- 前記分解された血栓が、前記血栓捕捉ケージ内に保持されることを特徴とする請求項2に記載のデバイス。
- 前記血栓捕捉ケージ内の血栓が、前記血栓捕捉ケージから流し出されることを特徴とする請求項1乃至4のいずれか一項に記載のデバイス。
- 前記マセレータ機構が、回転するように構成されたデバイスを備えることを特徴とする請求項1乃至5のいずれか一項に記載のデバイス。
- 前記マセレータ機構が、1つまたは複数の回転要素を備えることを特徴とする請求項6に記載のデバイス。
- 前記1つまたは複数の回転要素が、ワイヤまたはブラシを備えることを特徴とする請求項7に記載のデバイス。
- 前記血栓捕捉ケージ内に血栓摘出器をさらに備え、前記マセレータ機構は、前記血栓摘出器よりも、前記血栓捕捉ケージの後端部に接近して配置されることを特徴とする請求項1乃至8のいずれか一項に記載のデバイス。
- 前記血栓捕捉ケージまたは前記血栓ブロッキング体の一方もしくは両方により血管壁に加えられる力を変化させるように構成された制御機構を含むことを特徴とする請求項1乃至9のいずれか一項に記載のデバイス。
- 前記制御機構は、前記血栓捕捉ケージまたは前記血栓ブロッキング体を拡張した配向に付勢するように構成された付勢要素を含むことを特徴とする請求項10に記載のデバイス。
- 前記血栓捕捉ケージまたは前記血栓ブロッキング体は、それ自体で拡張するものであることを特徴とする請求項1乃至11のいずれか一項に記載のデバイス。
- 前記血栓ブロッキング体(5)は、前記血栓捕捉ケージ(3)の前記前端部(10)とぴったり接合する形状となされることを特徴とする請求項1乃至12のいずれか一項に記載のデバイス。
- 前記血栓ブロッキング体は、外力の印加に応じて前記血栓捕捉ケージの前記前端部とぴったり接合するのに適した形状に変形または再構成するように構成されていることを特徴とする請求項13に記載のデバイス。
- 前記外力は、前記血栓捕捉ケージが前記血栓ブロッキング体に接触することであることを特徴とする請求項14に記載のデバイス。
- 前記血栓ブロッキング体は、前記血栓捕捉ケージと前記血栓ブロッキング体とが離間しているときに前記血栓ブロッキング体の変形または再構成をもたらすように構成された軸方向延長部を含むことを特徴とする請求項14に記載のデバイス。
- 前記血栓捕捉ケージ(3)は、内方に向かって細くなっている後端部(11)を有することを特徴とする請求項1乃至16のいずれか一項に記載のデバイス。
- 前記血栓捕捉ケージ(3)を前記カテーテル部材の長手軸の周りで回転させるように作動可能な回転手段を含むことを特徴とする請求項1乃至17のいずれか一項に記載のデバイス。
- 前記血栓捕捉ケージの前記前端部は、血栓を受け入れる寸法となされたメッシュを含み、前記血栓捕捉ケージの後端部は、血栓を保持する寸法となされたメッシュを含むことを特徴とする請求項1乃至18のいずれか一項に記載のデバイス。
- 前記血栓捕捉ケージは、径方向寸法において弾性的に変形可能であり、それが障害物に遭遇したときに前記血栓捕捉ケージが収縮することを可能にすることを特徴とする請求項1乃至19のいずれか一項に記載のデバイス。
- 前記血栓捕捉ケージ内にまたは前記血栓捕捉ケージと前記血栓ブロッキング体との間に液体を送達するための手段を含むことを特徴とする請求項1乃至20のいずれか一項に記載のデバイス。
- 体内腔から物質を除去するための血栓切除デバイスであって、
遠位部分および近位部分を有する細長いカテーテル部材(2)と、
前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓をブロックする拡張した配向との間で径方向に拡張可能な血栓ブロッキング体(5)と、
前記血栓ブロッキング体に対して軸方向に離間した関係で前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓を捕捉する拡張した配向との間で径方向に拡張可能な血栓捕捉ケージ(3)であって、物質を受け入れるための開口を有する、血栓捕捉ケージと、
前記血栓捕捉ケージおよび前記血栓ブロッキング体を展開および収縮させるように作動可能な展開手段と、
前記血栓捕捉ケージに動作可能に接続され、前記血栓捕捉ケージを前記体内腔に沿って軸方向に移動させるように構成された細長い制御アームであって、前記血栓捕捉ケージの前端部に動作可能に接続される、細長い制御アームと、
を備え、
前記血栓ブロッキング体は、前記血栓捕捉ケージより近位側にのみ配置され、
前記デバイスは、凝血塊を軟化または崩壊させるために、前記血栓捕捉ケージ内にまたは前記血栓捕捉ケージと前記血栓ブロッキング体との間に流体を解放するための手段を含み、
前記細長い制御アームは、血栓を捕捉及び除去するために、前記血栓捕捉ケージを前記血栓ブロッキング体に向かって移動させることを特徴とするデバイス。 - 流体が、回転ねじにより前記カテーテル部材の長さに沿って送達されることを特徴とする請求項22に記載のデバイス。
- 前記回転ねじが、摘出ではなく注入のために回転する摘出器ねじを含むことを特徴とする請求項23に記載のデバイス。
- 流体が外側シース内の空洞を通じて注入されることを特徴とする請求項22乃至24のいずれか一項に記載のデバイス。
- 前記シースおよび前記空洞の位置は、前記カテーテル部材の長さに沿って調整可能であることを特徴とする請求項25に記載のデバイス。
- 前記デバイスは、前記血栓捕捉ケージ内に配置された血栓を分解する、前記血栓捕捉ケージ内の血栓マセレータ機構を備えることを特徴とする請求項22乃至26のいずれか一項に記載のデバイス。
- 前記デバイスが血栓摘出器機構を含む、請求項22乃至27のいずれか一項に記載のデバイス。
