血栓捕捉ケージに動作可能に接続され、血栓捕捉ケージを体内腔に沿って軸方向に移動させるように構成された細長い制御アームであって、ケージの前端部に動作可能に接続される、細長い制御アームと、
血栓捕捉体内に配置された血栓摘出器またはマセレータ機構と
を備え、
血栓捕捉ケージが理想的には血栓ブロッキング体よりも遠位に配置されるデバイスが提供される。
遠位部分および近位部分を有する細長いカテーテル部材と、
カテーテル部材の遠位部分に配置され、収縮した配向と血栓を捕捉する拡張した配向との間で径方向に拡張可能な血栓捕捉体と、
場合により、カテーテル部材の遠位部分に配置され、収縮した配向と血栓をブロックする拡張した配向との間で径方向に拡張可能な血栓ブロッキング体と、
血栓捕捉体および血栓ブロッキング体を展開および収縮させるように作動可能な展開手段と、
血栓捕捉体に動作可能に接続され、血栓捕捉体を体内腔に沿って軸方向に移動させるように構成された細長い制御アームと
を備え、
血栓捕捉体、血栓ブロッキング体、またはカテーテル体のうちの少なくとも1つを振動させるように構成された振動作動手段を含む振動手段を備えることを特徴とするデバイスを提供する。
カテーテルの長手軸の周りで回転するように構成された振動性回転部材であって、カテーテルの長手軸に対して偏心した重心を有する振動性回転部材、および/または、
カテーテルの長手軸に対してオフセットした軸の周りで(場合により、不均一な円周方向摩擦を含むかまたは含まずに)回転するように構成された振動性回転部材
を含むことが好ましい。
「血栓」は、血管または内腔の壁に通常付着する、固体または半固体または粘性流体の塊を意味すると理解されるべきである。「長い血栓」は、血管径の少なくとも2倍または3倍の軸方向長さを有する血栓を意味すると理解されるべきである。
・ 摘出器ねじの回転方向は、摘出ではなく注入のように変更され得る。
・ 中空の遠位アームを通じて注入する。
・ 遠位制御アームと摘出器チューブ(近位アーム)との間の内腔を通じて注入する(図9)。

Claims (21)
- 体内腔から物質を除去するための血栓切除デバイスであって、
遠位部分および近位部分を有する細長いカテーテル部材(2)と、
前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓をブロックする拡張した配向との間で径方向に拡張可能な血栓ブロッキング体(5)と、
前記血栓ブロッキング体に対して軸方向に離間した関係で前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓を捕捉する拡張した配向との間で径方向に拡張可能な血栓捕捉ケージ(3)であって、前記血栓ブロッキング体に面しかつ物質を受け入れるための開口を有する、内方に向かって細くなっている前端部を含む、血栓捕捉ケージと、
前記血栓捕捉ケージおよび血栓ブロッキング体を展開および収縮させるように作動可能な展開手段と、
前記血栓捕捉ケージに動作可能に接続され、前記血栓捕捉ケージを前記体内腔に沿って軸方向に移動させるように構成された細長い制御アームであって、前記血栓捕捉ケージの前記前端部に動作可能に接続される、細長い制御アームと、
前記血栓捕捉ケージ内に配置された血栓摘出器と
を備え、
前記血栓ブロッキング体は、前記血栓捕捉ケージより近位側にのみ配置され、
前記細長い制御アームは、血栓を捕捉及び除去するために、前記血栓捕捉ケージを前記血栓ブロッキング体に向かって近位側に移動させることを特徴とするデバイス。 - 前記血栓捕捉ケージまたは前記血栓ブロッキング体の一方もしくは両方により血管壁に加えられる力を変化させるように構成された制御機構を含むことを特徴とする請求項1に記載のデバイス。
- 前記制御機構は、前記血栓捕捉ケージまたは前記血栓ブロッキング体を拡張した配向に付勢するように構成された付勢要素を含むことを特徴とする請求項2に記載のデバイス。
- 前記血栓捕捉ケージまたは前記血栓ブロッキング体は、それ自体で拡張するものであることを特徴とする請求項1乃至3のいずれか一項に記載のデバイス。

前記振動手段は、前記カテーテル部材の長手軸に対してオフセットされた軸の周りで回転するように構成された回転部材、または、前記カテーテル部材が共振するように、前記カテーテル部材の固有振動数と同じかまたはそれに近い角振動数で回転するように構成された回転部材を含むことを特徴とするデバイス。 - 体内腔から物質を除去するための血栓切除デバイスであって、
遠位部分および近位部分を有する細長いカテーテル部材(2)と、
前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓をブロックする拡張した配向との間で径方向に拡張可能な血栓ブロッキング体(5)と、
前記血栓ブロッキング体に対して軸方向に離間した関係で前記カテーテル部材の前記遠位部分に配置され、収縮した配向と血栓を捕捉する拡張した配向との間で径方向に拡張可能な血栓捕捉ケージ(3)であって、前記血栓ブロッキング体に面しかつ物質を受け入れるための開口を有する、内方に向かって細くなっている前端部を含む、血栓捕捉ケージと、
前記血栓捕捉ケージおよび前記血栓ブロッキング体を展開および収縮させるように作動可能な展開手段と、
前記血栓捕捉ケージに動作可能に接続され、前記血栓捕捉ケージを前記体内腔に沿って軸方向に移動させるように構成された細長い制御アームであって、前記血栓捕捉ケージの前記前端部に動作可能に接続される、細長い制御アームと、
前記径方向に拡張可能な血栓捕捉ケージを前記拡張した配向に付勢するための、力によって制御される機構を備える制御機構と
を備え、
前記血栓ブロッキング体は、前記血栓捕捉ケージより近位側にのみ配置され、
前記細長い制御アームは、血栓を捕捉及び除去するために、前記血栓捕捉ケージを前記血栓ブロッキング体に向かって近位側に移動させることを特徴とするデバイス。
