I.概略

単量体TTRまたはそのフラグメント(例えば、アミノ酸残基89〜97)に対する他の非ヒト抗体、例えば、マウス抗体、モルモット抗体、霊長類抗体、ウサギ抗体またはラット抗体の作製は、例えば、動物をTTRまたはそのフラグメントで免疫することによって遂行できる。HarlowおよびLane,Antibodies,A Laboratory Manual(CSHP NY,1988)(あらゆる目的において参照により組み込まれる)を参照されたい。このような免疫原は、天然源から、ペプチド合成によって、または組換え発現によって得ることができる。任意選択で、融合をはじめとする方法で免疫原を担体タンパク質と複合体形成させて投与してもよい。任意選択で、免疫原をアジュバントとともに投与してもよい。のちに記載する通り、いくつかの種類のアジュバントを用いることができる。実験動物の免疫感作には、フロイント完全アジュバント、次いで不完全アジュバントが好ましい。ポリクローナル抗体の作製には通常、ウサギまたはモルモットを用いる。モノクローナル抗体の作製には通常、マウスを用いる。単量体TTRまたはTTR内のエピトープ(例えば、アミノ酸残基89〜97のうちの1つまたは複数のものを含むエピトープ)に特異的に結合する抗体をスクリーニングする。このようなスクリーニングは、単量体TTRバリアント、例えばアミノ酸残基89〜97を有するかその残基内に変異を有するTTRバリアントなどの収集物への抗体の結合を決定し、その抗体にどのTTRバリアント抗体が結合するかを決定することによって遂行できる。結合は、例えば、ウエスタンブロット、FACSまたはELISAによって評価できる。
ヒト化抗体は、遺伝子操作によって非ヒト「ドナー」抗体のCDRをヒト「アクセプター」抗体配列内に移植した抗体である(例えば、Queen,米国特許第5,530,101号および同第5,585,089号;Winter,米国特許第5,225,539号;Carter,米国特許第6,407,213号;Adair,米国特許第5,859,205号;および米国特許第6,881,557号を参照されたい)。アクセプター抗体配列は、例えば、成熟ヒト抗体配列、そのような配列の複合体、ヒト抗体配列のコンセンサス配列、または生殖系列領域の配列であり得る。したがって、ヒト化抗体は、完全にまたは実質的にドナー抗体に由来するCDRを少なくとも3つ、4つ、5つまたは全部ならびに、存在する場合は、完全にまたは実質的にヒト抗体配列に由来する可変領域フレームワーク配列および定常領域を有する抗体である。同じように、ヒト化重鎖は、完全にまたは実質的にドナー抗体重鎖に由来するCDRを少なくとも1つ、2つ、通常は3つ全部ならびに、存在する場合は、実質的にヒト重鎖可変領域フレームワーク配列および定常領域配列に由来する重鎖可変領域フレームワーク配列および重鎖定常領域を有する。同じように、ヒト化軽鎖は、完全にまたは実質的にドナー抗体軽鎖に由来するCDRを少なくとも1つ、2つ、通常は3つ全部ならびに、存在する場合は、実質的にヒト軽鎖可変領域フレームワーク配列および定常領域配列に由来する軽鎖可変領域フレームワーク配列および軽鎖定常領域を有する。ナノボディおよびdAb以外にも、ヒト化抗体はヒト化重鎖およびヒト化軽鎖を含む。ヒト化抗体のCDRは、各CDR間で対応する残基(任意の従来の定義、好ましくはKabatによって定義されるもの)の少なくとも85%、90%、95%または100%が一致する場合、非ヒト抗体での対応するCDRに実質的に由来する。抗体鎖の可変領域フレームワーク配列または抗体鎖の定常領域は、任意の従来の定義、好ましくはKabatによって定義される対応する残基の少なくとも85%、90%、95%または100%が同一である場合、それぞれヒト可変領域フレームワーク配列またはヒト定常領域に実質的に由来する。
(1)直接抗原と非共有結合する;
(2)KabatではなくChothiaによって定義されるCDR領域に隣接するか、そのCDR内にある;
(3)別の方法でCDR領域(例えば、CDR領域の約6Å以内にある)、(例えば、相同な既知の免疫グロブリン鎖で明らかになった構造上で軽鎖もしくは重鎖をモデル化することによって同定される)と相互作用する;または
(4)VL−VH界面に関与する残基である
ことが合理的に予想される。
本発明はさらに、キメラ型およびベニヤ化(veneered)型の非ヒト抗体、具体的には実施例の9D5抗体または14G8抗体を提供する。
以下に記載する様々な技術によって単量体TTRまたはそのフラグメント(例えば、TTRのアミノ酸残基89〜97(配列番号113))に対するヒト抗体が得られる。いくつかのヒト抗体は、特定のマウス抗体、例えば実施例に記載されるマウスモノクローナル抗体の1つなどと同じエピトープ特異性を有するように、上記の競合結合実験により、Winterのファージディスプレイ法により選択される。ヒト抗体はまた、TTRのフラグメント、例えばTTRのアミノ酸残基89〜97のみを含むTTRバリアントなどのみを標的抗原として使用することによって、および/またはTTRバリアントの収集物、例えばTTRのアミノ酸残基89〜97内に様々な変異を有するTTRバリアントなどに対する抗体をスクリーニングすることによって、特定のエピトープ特異性に関してスクリーニングされ得る。
キメラ抗体、ベニヤ化(veneered)抗体またはヒト化抗体の重鎖可変領域および軽鎖可変領域は、ヒト定常領域の少なくとも一部分に連結できる。定常領域の選択は部分的に、抗体依存性細胞媒介性細胞傷害性、抗体依存性細胞食作用および/または補体依存性細胞傷害を所望するかどうかに左右される。例えば、ヒト同位体のIgG1およびIgG3には補体依存性細胞傷害性があり、ヒトアイソタイプのIgG2およびIgG4にはない。ヒトIgG1およびIgG3はまた、ヒトIgG2およびIgG4よりも強力な細胞仲介性エフェクター機能を誘導する。軽鎖定常領域はラムダまたはカッパであり得る。
抗体発現細胞系(例えば、ハイブリドーマ)を用いてキメラ抗体およびヒト化抗体を作製する方法が多数知られている。例えば、周知の方法を用いて抗体の免疫グロブリン可変領域をクローン化し配列を決定できる。1つの方法では、ハイブリドーマ細胞から調製したmRNAを用いるRT−PCRにより重鎖可変VH領域をクローン化する。翻訳開始コドンを含むVH領域リーダーペプチドに対しては、5’プライマーおよびg2b定常領域特異的3’プライマーとしてコンセンサスプライマーを用いる。例示的なプライマーはSchenkによる米国特許公開第2005/0009150号(以下「Schenk」)に記載されている。別個に得た複数のクローンの配列を比較して、増幅の際に変化が導入されないことを確実にできる。VH領域の配列はまた、5’RACE RT−PCR法および3’g2b特異的プライマーによって得られるVHフラグメントの配列のシーケンシングによって、決定または確認することができる。
抗体に結合アッセイ、機能スクリーニング、TTR沈着物による疾患の動物モデルを用いるスクリーニング、および診療試験を含むいくつかのスクリーニングを実施できる。結合アッセイでは、単量体TTRまたはそのフラグメントに対する特異的結合ならびに、任意選択で親和性およびエピトープ特異性を試験する。例えば、結合アッセイによって、天然のTTR四量体では埋没しており単量体形態、ミスフォールド形態、凝集形態、または原線維形態のTTRでは露出しているエピトープである、TTRのアミノ酸残基89〜97(配列番号113)に結合する抗体をスクリーニングできる。抗体はまた、天然のTTRの立体構造ではなく前原線維性の非天然の立体構造のTTRおよびTTRアミロイド原線維を結合する能力についてスクリーニングされる。例えば、抗体は、実施例その他に記載されるように、天然の四量体TTRの解離または脱凝集によって生じる単量体型TTRに結合する能力についてスクリーニングされ、かつ天然の四量体TTRに対してカウンタースクリーニングされる。同じように、抗体はまた、健常組織に対してではなくTTR媒介性アミロイドーシス組織に対するそれらの免疫反応性についてスクリーニングされる。このようなスクリーニングは場合によっては、9D5または14G8(IgG1カッパアイソタイプ)の可変領域を有する抗体などの例示的抗体と競合して実施する。このようなアッセイでは、任意選択で抗体またはTTR標的を固定化する。
本発明はさらに、上記の重鎖および軽鎖のいずれかをコードする核酸(例えば、配列番号40、42、44〜56、87、89、91〜96、および106〜108)を提供する。このような核酸は任意選択で、シグナルペプチドをさらにコードし、定常領域に連結されたシグナルペプチド(例えば、配列番号40および87(重鎖)ならびに配列番号42および89(軽鎖)によってそれぞれコードされ得る配列番号41および88(重鎖)ならびに配列番号43および90(軽鎖)のアミノ酸配列を有するシグナルペプチド)とともに発現させることができる。コード配列を確実に発現させるため、核酸のコード配列はプロモーター、エンハンサー、リボソーム結合部位、転写終止シグナルなどの制御配列と作動可能に連結され得る。重鎖および軽鎖をコードする核酸は、分離形態で存在しても、1つまたは複数のベクター中にクローン化されていてもよい。核酸は、例えば固相合成または重複するオリゴヌクレオチドのPCRによって合成できる。重鎖と軽鎖をコードする核酸は、例えば発現ベクター内で、1つの連続する核酸として連結されていてもよく、または例えばそれぞれの発現ベクター中にクローン化されて、分離していてもよい。
病原性形態のTTRでは露出しているが天然の四量体形態のTTRでは露出していない抗原、例えばTTRのアミノ酸残基89〜97(配列番号113)に特異的に結合するコンジュゲート抗体は、単量体形態、ミスフォールド形態、凝集形態、もしくは原線維形態のTTRの有無の検出;TTRアミロイドーシスと診断された患者の治療に使用される治療剤の有効性のモニタリングおよび評価;TTRの凝集の阻害もしくは軽減;TTR原線維形成の阻害もしくは軽減;TTR沈着物の減少もしくは除去;TTRの非毒性立体構造の安定化;または患者のTTRアミロイドーシスの治療もしくは予防に有用である。例えば、このような抗体は別の治療部分、別のタンパク質、別の抗体、および/または検出可能な標識とコンジュゲートできる。国際公開第03/057838号;米国特許第8,455,622号を参照されたい。
上記の抗体は、トランスサイレチン(TTR)により少なくとも部分的に、および特に単量体形態、ミスフォールド形態、凝集形態、または原線維形態のTTRにより媒介される疾患を有するか、そのような疾患のリスクのある患者における疾患の治療または予防に使用できる。実施にあたって機序を理解する必要はないが、上記の抗体を用いるTTRアミロイドーシスの治療に以下の機序:TTR凝集および原線維形成の抗体介在性阻害、TTRの非毒性立体構造(例えば、テトラマー形態)の抗体介在性安定化、または凝集TTR、オリゴマーTTR、もしくは単量体TTRの抗体介在性除去のいずれかまたは全部が寄与するものと考えられる。抗体−薬物コンジュゲートは、コンジュゲート部分によって決まるさらなる作用機序を有し得る。
本明細書では、トランスサイレチン(TTR)により少なくとも部分的に、および特に単量体形態、ミスフォールド形態、凝集形態、または原線維形態のTTRにより媒介される疾患または病態(例えば、TTRアミロイドーシス)を診断、モニタリング、治療または予防するいくつかの方法が提供される。このような疾患の例としては、家族性アミロイド心筋症(FAC)、家族性アミロイドニューロパチー(FAP)、または中枢神経系選択性アミロイドーシス(CNSA)などの家族性TTRアミロイドーシス、および老人性全身性アミロイドーシス(SSA)または老人性心アミロイドーシス(SCA)などの孤発性TTRアミロイドーシスが挙げられる。上記の抗体は、このような方法で使用する医薬組成物中に組み込まれ得る。一般に、抗体または抗体を含有する医薬組成物が、それを必要とする対象に投与される。治療に適する患者としては、TTRアミロイドーシスのリスクはあるが症状を認めない個体ならびに、現時点で症状を呈する患者が挙げられる。患者によっては、TTRアミロイドーシスの前駆段階の間に治療され得る。
TTR沈着または病原性形態のTTR(例えば、単量体形態、ミスフォールド形態、凝集形態または原線維形態のTTR)に関連する疾患に罹患しているか罹患しやすい患者中でTTRに対する免疫応答を検出する方法も提供される。この方法は、本明細書に提供される薬剤による治療的処置または予防的処置の経過をモニターするのに使用できる。受動免疫後の抗体プロファイルは通常、抗体濃度の直後のピークとそれに続く指数関数的減衰を示す。さらなる投与を実施しなければ、投与した抗体の半減期に応じて数日〜数か月以内に減衰が治療前のレベルに近づく。例えば、いくつかのヒト抗体の半減期は20日程度である。
本発明はさらに、本明細書に開示される抗体と、それに関連する材料、例えば使用指示書(例えば、添付文書)とを含む、キット(例えば、容器)を提供する。使用指示書は、例えば、抗体および任意選択で1つまたは複数の追加の薬剤の投与に関する指示を含み得る。抗体の容器は、単位用量、バルク包装(例えば、複数用量包装)、または小単位用量であり得る。
抗体は、臨床診断、臨床治療または研究において単量体形態、ミスフォールド形態、凝集形態、もしくは原線維形態のトランスサイレチン(TTR)またはそのフラグメントを検出するのに用いることができる。例えば、抗体を用いて、生体試料がTTRアミロイド沈着物を含むことの指標として生体試料中の単量体形態、ミスフォールド形態、凝集形態、または原線維形態のTTRの存在を検出できる。生体試料への抗体の結合を、対照試料への抗体の結合と比較できる。対照試料と生体試料は、同じ組織を起源とする細胞を含み得る。対照試料と生体試料は、同じ個体または異なる個体から同じ機会または異なる機会に採取され得る。必要に応じて、複数の生体試料と複数の対照試料を複数の機会に評価して、試料間の差とは無関係のランダムな変動から保護する。次いで、生体試料(1つまたは複数)と対照試料(1つまたは複数)とを直接比較して、生体試料(1つまたは複数)への抗体の結合(すなわち、単量体形態、ミスフォールド形態、凝集形態、または原線維形態のTTRの存在)が対照試料(1つまたは複数)への抗体の結合に比して増大しているのか、減少しているのか、同じであるのかを決定できる。生体試料(1つまたは複数)への抗体の結合が対照試料(1つまたは複数)に比して増大していることは、生体試料(1つまたは複数)中に単量体形態、ミスフォールド形態、凝集形態、または原線維形態のTTRが存在することを示す。いくつかの場合には、抗体結合の増大は統計的に有意である。任意選択で、生体試料への抗体の結合は、対照試料への抗体の結合より少なくとも1.5倍、2倍、3倍、4倍、5倍、10倍、20倍、または100倍高い。
実施例1.mis−TTRモノクローナル抗体の同定
材料および方法(a〜d)に記載する通りに単量体形態、ミスフォールド形態、原線維形態、または凝集形態のTTR(mis−TTR)に対する立体構造特異的モノクローナル抗体を作製し、スクリーニングし、発現させ、精製した。mis−TTRモノクローナル抗体を作製するため、ヒト四量体TTRの結晶構造を調べて、四量体では埋没しているが、四量体を単量体サブユニットに解離すると露出するタンパク質の領域を明らかにした。特定された領域は、TTRのF鎖内に位置し四量体タンパク質の二量体界面で隔離されている残基89〜97(EHAEVVFTA)(配列番号113)であった。タンパク質データベースのBLAST検索を実施したところ、この配列を有する他のヒトタンパク質は見出されなかった。
4種類のリードmis−TTR mAb(9D5、14G8、6C1および5A1)を0.31〜2.5μg/mlの範囲の濃度で、コーティング抗原としてpH4.0処理TTR(pH4−TTR)および天然TTRの両方を用いるELISAによりアッセイした。TTR抗原の調製およびELISAのプロトコルについては、材料および方法(e〜g)に別途記載する。
9D5および14G8をSDS−PAGE/ウエスタンにより分析し、単量体形態/変性形態のTTRに対する結合特異性を天然TTRと比較して示した。SDS−PAGE、ネイティブPAGE、およびウエスタンブロットのプロトコルについては、方法および材料(h〜j)に別途記載する。
TTR−Y78Fは、タンパク質配列中の78位にTTR四量体を不安定化させる点変異を含むTTRバリアントである。このTTRバリアントは、時間の経過および弱酸性条件下にてその単量体サブユニットへと解離し、次いで凝集して、チオフラビンTに結合できる線維を形成し得る。このため、480nmでチオフラビンTの蛍光を測定することによって線維形成の程度をモニターできる。解離したTTR単量体または凝集体に特異的なmis−TTR抗体を導入すれば、TTR線維の集合が妨げられ、無抗体対照の反応に比してチオフラビンTの蛍光が減少すると考えられる。TTR線維形成の阻害を検討するプロトコルについては、材料および方法(k)に別途記載する。
トランスサイレチンタンパク質のTTR89−97フラグメントに対して産生したリードmis−TTR mAbを、TTR心アミロイドーシスが確定された患者からの新鮮な凍結組織およびパラフィン処理組織を用いて、免疫組織化学的に検討した。心組織試料の採取および調製、免疫組織化学(IHC)、ならびに画像解析のプロトコルについては、材料および方法(l〜o)に別途記載する。IHCに使用した抗体を表5に記載する。
V30M ATTRが確定された患者由来の血漿試料6例(試料番号11、12、15、18、19、20)および正常対象由来の試料6例(番号21、22、23、24、25、27)をM.Saraiva氏(ポルト大学、ポルトガル)から入手した。試料番号C6は商業的供給源(BioreclamationIVT社)から入手した正常ヒト血清試料である。試料をSDS−PAGEとウエスタンブロットにより、またはMesoScale Discovery(MSD)プレートアッセイにより解析した。これらのアッセイのプロトコルについては、材料および方法(p〜r)に別途記載する。6C1を用いてMSDプレートアッセイの標準曲線を作成した。

表12に示すように、ヒト化9D5バリアント、マウス9D5、およびキメラ9D5の全抗体について、組換えヒトTTR F87M/L110Mへの結合に関する結合動態の特徴をBiacoreにより明らかにした。抗ヒト(GE Healthcare社)を、GE Healthcare社の抗ヒトキットに添付された指示書に従って、アミンカップリングによりセンサーチップC5(デキストラン鎖を欠く)上に固定化し、mAbを、分析物の結合最大値が確実に30〜50RUとなるレベルまで捕捉した。様々な濃度の分析物(組換えヒトTTR F87M/L110M)を、3倍希釈でランニング緩衝液(HBS+0.05%P−20、1mg/mL BSA)中50μl/分にて捕捉したリガンド上を通過させた。各濃度について、会合時により高濃度の分析物が平衡状態に達するような、かつ解離時にシグナルの少なくとも10%が減衰するような時間の間、反応を進行させた。少なくとも1種類の濃度(最高濃度でも最低濃度でもない)を二重反復で実施した。分析物の濃度範囲は、予備実験に基づき少なくともKDの10倍〜KDの10分の1に渡るよう選択した。

表18に示すように、Biacoreによって組換えヒトTTR F87M/L110Mに対するヒト化14G8バリアント3種類およびマウス14G8の結合動態の特徴を明らかにした。抗ヒト(GE Healthcare社)を、GE Healthcare社の抗ヒトキットに添付された指示書に従って、アミンカップリングによりセンサーチップC5(デキストラン鎖を欠く)上に固定化し、mAbを、分析物の結合最大値が確実に30〜50RUとなるレベルまで捕捉した。様々な濃度の分析物(組換えヒトTTR F87M/L110M)を、3倍希釈でランニング緩衝液(HBS+0.05%P−20、1mg/mL BSA)中した50μl/分にて捕捉したリガンド上を通過させた。各濃度について、会合時により高濃度の分析物が平衡状態に達するような、かつ解離時にシグナルの少なくとも10%が減衰するような時間の間、反応を進行させた。少なくとも1種類の濃度(最高濃度でも最低濃度でもない)を二重反復で実施した。分析物の濃度範囲は、予備実験に基づき少なくともKDの10倍〜KDの10分の1に渡るよう選択した。
a.抗体作製プロトコル
マウスをRIBIアジュバント中の抗原性ペプチドTTR−MAP、TTR89−97−N−KLHもしくはTTR89−97−C−KLHで週1回、またはTiterMaxアジュバント中の上記の抗原性ペプチドで月1回免疫した。融合の3〜4日前に、選択したマウスに生理食塩水中の免疫原で最後のIV追加免疫を実施した。脾臓をホモジナイズして脾細胞を調製し、標準的な電気融合プロトコルを用いてSP2/0ミエローマ細胞と融合した。選択培地中の融合細胞を96ウェルプレートに播き、7〜10日後にスクリーニングを実施した。
ハイブリドーマ選択は以下のELISAスクリーニングに基づいた:96ウェルELISAプレートをニワトリ抗His(1μg/mL PBS)でコートし、1時間インキュベートした。プレートを1%BSA/PBS溶液(200μL/ウェル)で15分間ブロックした後、0.5μg/mLのpH4−TTR(50μL/ウェル)を加え、1時間インキュベートした。pH4−TTRは、解離/凝集させるために低pH(50mM酢酸ナトリウム、pH4.0)に置かれ、TTR89−97エピトープを露出するTTRである。プレートをTBS−Tで2回洗浄した。融合プレートからの上清を50μL/ウェルで加え、1時間インキュベートした。プレートをTBS−Tで2回洗浄した。0.5%BSA/PBS/TBS−Tで1:5,000に希釈した検出抗体のヤギ抗マウス(IgG1、IgG2a、IgG2b、IgG3特異的)−HRPを50μL/ウェルで加え、1時間インキュベートした。最後に、プレートをTBS−Tで5回洗浄し、TMB基質(100μL/ウェル)を加えた。15分後、2N硫酸(50μL/ウェル)で基質の発色を停止させた。プレートを450nmで読み取った。O.D.が1.0を上回るウェルを選択し、細胞を24ウェルプレートに移した。3日間増殖させた後、上記のアッセイによりクローンにカウンタースクリーニングを実施して結合を確認し、陰性カウンタースクリーニングとしてpH4−TTRを天然TTRに置き換え、非天然型のTTRに特異的なTTR mAbを産生するクローンを選択できた。
ヒト化モノクローナル抗体配列を保有しCMVによって駆動される軽鎖および重鎖のプラスミドをCHO−S1細胞(Life Technology社)中にトランスフェクトした。二重選択を実施して選択プールを作製した。馴化培地の力価および結合をアッセイし、SDS−PAGE/ウエスタンブロット法により解析した。選択プールを、Clonepixシステム(Molecular Devices社)を用いるクローン作製に使用した。抗体価に基づきクローンに順位を付けた。選択したクローンを拡大しバンク化した。
浮遊している細胞を4℃にて重力でWaveバッグの底に最初に沈降させた後、細胞培養物を回収した。回収した培地をデプスフィルター(Millistak Pod COHC、Millipore社)で清澄化し、タンジェンシャルフローろ過(Pelicon 2PLC 30K、Millipore社)により10倍に濃縮し、0.2μmフィルター(Opticap XL、Millipore社)で滅菌ろ過した。次いで、FPLC(Akta Avant、GE Lifesciences社)を用いて、1×PBS(pH7.4)で予め平衡化させたProtein G Sepharose Fast Flowカラム(GE Lifesciences社)に濃縮した馴化培地を負荷した。未結合タンパク質を、OD280がベースラインに達するまでカラム体積の5〜10倍の1×PBS(pH7.4)でカラムから洗い流した。結合抗体を、カラム体積の2倍のIgG Elution Buffer(Thermo Scientific社)でカラムから溶出させた。溶出画分を収集し、2Mトリス(pH9.0)(溶出物1ml当たり60μL)でpHを中性にした。
大腸菌(E.coli)(BL21−A1)細胞を、TTRインサート(Met−hTTR−(His)6またはF87M/L110M二重変異を有するTTRバリアントを含むpET21a(+)プラスミドで形質転換した。細胞を、100μg/mlのアンピシリンを含有する2YTブロスで増殖させた。TTRの発現を、1mM IPTGおよび005%アラビノースの存在下にて20℃で一晩誘導した。
組換えTTR−6Hisの濃縮ストックを1×PBSで最終濃度2.5μg/mlに希釈することにより、天然TTR抗原を調製した。組換えTTRを50mM酢酸ナトリウム(pH3.95)中、濃度0.2mg/mlで72時間、室温でインキュベートすることにより、pH4処理TTRを作製した。この条件下では、TTRが解離して、天然TTRとは構造的に異なるTTR単量体と凝集形態のTTRとの混合物になる。次いで、pH4−TTRを、アッセイで使用する直前に1×PBSで最終濃度2.5μg/mlに希釈した。96ウェルプレート(Costar社、番号3690)を、1×PBS中1.0μg/mlのニワトリ抗hisポリクローナル抗体(Abcam社、番号Ab9107)50μl/ウェルで室温にて1時間コートした。コーティング溶液を捨て、プレートを1×PBSで希釈した体積250μl/ウェルの1×BSA含有ブロッキング緩衝液(G−Biosciences社、番号786−193)でブロックした。
コートしブロックした96ウェルプレートを室温で1時間、2.5μg/mlのTTR抗原(天然TTRまたはpH4−TTRのいずれか)50μl/ウェルで処理した。次いで、プレートを250μl/ウェルの洗浄緩衝液(0.05%Tween−20を含有する1×トリス緩衝生理食塩水)で2回洗浄した。次いで、洗浄したプレートを0.31〜2.5μg/mlの範囲の濃度のしかるべき抗TTRモノクローナル抗体50μl/ウェルで処理した。
SDS−ポリアクリルアミドゲルでの電気泳動を以下の通りに実施した。1×LDS試料緩衝液(Life Technologies社)中0.1〜1μgのTTRまたはpH4.0−TTRを10%NuPAGEビス‐トリスゲル上に負荷し、MES緩衝液中、一定電圧90Vで105分間電気泳動させた。電気泳動後、ゲルをInstant Blue(Expedeon社)で染色するか、ニトロセルロースフィルターに転写してウエスタンブロット解析に供した。
未変性トリス−グリシンゲルでの電気泳動を以下の通りに実施した。1×トリス−グリシン試料緩衝液(Life Technologies社)中0.1〜1μgのTTRまたはpH4.0−TTRを10〜20%のトリス−グリシンゲル上に負荷し、1×ネイティブトリス−グリシンランニング緩衝液中、一定電圧120Vで105分間電気泳動させた。電気泳動後、ゲルをInstant Blue(Expedeon社)で染色するか、ニトロセルロースフィルターに転写してウエスタンブロット解析に供した。
SDS−PAGEゲルまたはネイティブ−PAGEゲルをニトロセルロースフィルターペーパー(iBlot、P7 Program)上にブロットし、ブロッキング緩衝液(Licor社)で30分間ブロックした。次いで、フィルターをブロッキング緩衝液中の0.5μg/mlの一次抗体中にて、室温で1時間(または4℃で一晩)インキュベートした後、1×TBSで10分間、3回洗浄した。フィルターをブロック緩衝液中の1:20,000希釈のIRDye 800CWコンジュゲートヤギ抗マウス二次抗体中でインキュベートした。フィルターを二次抗体溶液中、室温で1時間インキュベートした後、フィルターを洗浄し、Odyssey CLx赤外線撮像装置(Licor社)で撮像した。
50mM酢酸ナトリウム(pH4.8)中の3.6μM(0.2mg/ml)のTTR−Y78Fの溶液を1.4μM(0.2mg/ml)のmis−TTR抗体またはアイソタイプ対照の存在下にて、37℃で72時間インキュベートした。インキュベーション後、混合物に5倍モル過剰のチオフラビンTを加え、30分間結合させた。励起波長を440nmに設定し、発光波長480nmでの蛍光定量的測定値を測定した。0%阻害をアイソタイプ対照抗体存在下での蛍光強度(83a.u.)として設定し、100%阻害点をTTR−Y78Fタンパク質不在下での蛍光(38a.u.)として設定した。
ATTR変異が確定診断された心組織の新鮮な凍結ブロックおよびパラフィン処理ブロックを、インディアナ大学のMerrill Benson博士から入手した。試料は新鮮な凍結試料8例およびFFPE試料6例を含み、各試料はATTRまたは何らかの他の心アミロイドーシスのいずれかと診断された。TTR抗体による特徴付けの前に、Prothena社にてカッパ軽鎖およびラムダ軽鎖ならびにアミロイドAに対する抗体でIHC染色を実施することにより、組織の診断をさらに裏付けた。
免疫組織化学を、パラホルマルドヘイド(paraformaldheyde)で軽く固定しスライドに載せた10μmの凍結切片および5μmのパラフィン切片に実施した。免疫ペルオキシダーゼ法が主な検出系であり、Leica Bond Rx(Leica Biosystems社、バッファローグローブ、イリノイ州)にBond Polymer Refine Detection Kit(DS980、Leica Biosystems社)を用いて実施した。一次抗体を1時間インキュベート(表2の濃度に従って)した後、抗体マウスおよび抗ウサギポリマーHRP−リンカー抗体コンジュゲートとインキュベートした。染色をDAB色素で可視化し、これは褐色の沈着物を生じた。スライドをヘマトキシリンで対比染色し、上昇アルコール系列で脱水し、キシレンで透徹し、CytoSeal 60(Richard Allen Scientific社;カラマズー、ミシガン州)を塗布してカバーガラスをかけた。陰性対照は、非免疫IgGアイソタイプ対照を用いるか一次抗体を省いて、隣接する切片に免疫組織化学的手順全体を実施した。
American MasterTech社(ローダイ、カリフォルニア州ローダイ、カリフォルニア州)のキットを用いてコンゴー赤染色を実施し、組織中のTTRアミロイドの存在を明らかにした。染色は製造業者の手順に従って実施した。スライドをコンゴー赤溶液で1時間染色した後、1%水酸化ナトリウムで約15秒間分別した。次いで、スライドを流水で洗浄し、濃度を漸増させたアルコール系列で脱水し、キシレンを3回交換して透徹し、CytoSeal 60を塗布してカバーガラスをかけた。
スライドを、Olympus BX61顕微鏡、Hamamatsu Nanozoomer 2.0HTデジタルスライドスキャナ、またはLeica SPEスペクトル共焦点システムのいずれかで撮像した。画像を収集し、TIFFファイルとして保存した。
V30M ATTRが確定された患者由来の血漿試料6例(試料番号11、12、15、18、19、20)および正常対象由来の試料6例(番号21、22、23、24、25、27)を、M.Saraiva氏(ポルト大学、ポルトガル)から入手した。試料番号C6は、商業的供給源(BioreclamationIVT社)から入手した正常ヒト血清試料であった。上記の血漿試料を以下の通りにSDS−PAGEによって分離し、9D5または5A1でウエスタンブロットした。体積1.4μlの血漿を、還元剤を含まない1×LDS試料緩衝液(Life Technologies社)中に1:8で希釈した。試料を、既に記載した通りにSDS−PAGEで分離し0.5μg/mlの9D5または5A1でウエスタンブロットした。
96ウェルMSDプレートを、PBS中4μg/mLの濃度のモノクローナル抗体6C1でコートし、振盪しながら室温で2時間、または4℃で一晩インキュベートした。プレートを1×TBSTで3回洗浄した後、振盪しながら3%MSD Blocker A溶液(150μL/ウェル)で1時間ブロックした。ヒト血漿試料を0.6%無グロブリンウシ血清アルブミンと、1.5mMリン酸二水素ナトリウムと、8mMリン酸水素ナトリウムと、145mM塩化ナトリウムと、0.05%Triton X−405と、0.05%チメロサールとからなる試料緩衝液中に1:10で希釈し、ブロックしたMSDプレートに体積30μl/ウェルで1時間加えた。プレートを1×TBSTで3回洗浄した。試料緩衝液中の1μg/mlのスルホタグ化検出抗体(Dakoポリクローナル抗体のいずれかの8C3全TTR抗体)を体積50μl/ウェルで加え、室温で1時間振盪した。プレートを1×TBSTで3回洗浄した後、1×Read Buffer T溶液(Meso Scale Discovery社)を150μl/ウェルで加えた。次いで、プレートをMSD Sector撮像装置で読み取った。
ヒト血漿試料中に存在する非天然の6C1反応性TTRタンパク質の量を定量するため、組換えTTR−F87M/L110Mを6C1反応性TTR標準物質として用いてMSD標準曲線を作成した。このTTRバリアントは、四量体形成を妨げタンパク質を単量体状態に保つ2つのアミノ酸置換を含む(Jiangら,(2001)Biochemistry 40,11442−11452)。したがって、このTTRバリアントは全てのmis−TTR mAbに認識され、このため、MSDアッセイでの参照標準物質としての使用によく適している。
TTR−F87M/L110M、天然TTRまたは低pH凝集TTR−V30Mの1mg/mL試料を、製造業者の仕様書(Thermo Scientific社)に従いpHrodo色素と37℃で15分間、タンパク質:色素の比を約15:4としてアミン結合させた。過剰なpHrodo標識を分子量カットオフ10Kの遠心濃縮器(Pierce Thermo社)を用いたダイアフィルトレーションによって除去し、pHrodo−TTRを1×PBS中に再懸濁させた。
TTR−F87M/L110MをpH感受性蛍光色素pHrodoで共有結合的に標識した。pHrodoタグは生理的pHで最小の蛍光を有するが、エンドサイトーシス小胞の低pH環境中に貪食されると蛍光が増強されるため、タグ化粒子が細胞内に取り込まれたことがわかる。THP−1単球を、14G8またはアイソタイプ対照抗体のいずれかでの処理後のpHrodoタグ化TTR(天然または非天然のTTR−F87M/L110M)に加えた。天然TTRとインキュベートした抗体でも、非天然TTRとの対照抗体のインキュベート後でも、低レベルの蛍光が観察された。しかし、非天然TTRとの14G8のインキュベート後に蛍光が増大し(図7A)、非天然TTRは塩基性条件下では効率的に貪食されないが、mis−TTR抗体を加えると非天然TTRの食作用が特異的に誘発されることが示唆された。
ヒト化トランスジェニックマウスモデルV30M hTTR(Inoueら,(2008)Specific pathogen free conditions prevent transthyretin amyloidosis in mouse models.Transgenic Research 17:817−826)でin vivo研究を実施し、凝集hTTRの結合および除去における抗TTR抗体の有効性を評価する。
本発明者は、予め凝集させたhTTRを、マトリゲル(BD Bioscience社、カタログ番号354263)中に懸濁させ凝固させた後、マウスの皮下に設置できることを明らかにした。移植から4週間後の時点で、マトリゲル埋植物はその構造を維持しており、凝集hTTRは依然として埋植物内に存在していた。さらに、埋植物はマウスによく許容され、抗hTTR抗体はマトリゲルに浸透し、マトリゲル中に懸濁される凝集体に結合できた。上記の所見に基づき、抗体の有効性研究を実施する。マウスを鎮静化させ、マトリゲル中に懸濁させた予め凝集したhTTRを含有する埋植物を、マウスの皮下に設置する。適切な回復期間の後、マウスを3つの治療群に分け(n=10/グループ)、溶媒、対照抗体(アイソタイプ対照、10mpk)または抗hTRR抗体(10mpk)のIP投与で週1回、2〜4週間にわたって処置する。最後の処置の後、マウスを安楽死させ、埋植物が含まれている皮膚を採取して処理した後、組織学的および/または生化学的方法を用いて、残存TTR沈着物の量を評価する。定量分析および統計学を用いて、グループ間でみられる除去の程度を求める。
