GM−1111の調製
下記の研究において評価された改変型ヒアルロナンGM−1111は、本明細書中に参照により援用されている国際公開第WO2009/124266号パンフレットにおいて以前に合成されたものである。GM−1111の構造は図1に提供されており、「SMHA」として示されている。
ユタ大学において全ての動物実験は、完全な承認の下でInstitutional Animal Care and Use Committeeに従って行われた。全ての動物実験に、8〜12週齢の成体雌のC57/Bl6マウス(Charles River,Wilmington,MA)が利用された。全ての動物を、病原体のいない環境に収容して維持し、12時間の光サイクルで食物及び水を自由に(ad libitum)与えた。合成されたLL−37はユタ大学の中核施設(ペプチド配列:LLGDFFRKSKEKIGKEFKRIVQRIKDFLRNLVPRTES)からHPLC均一形態で購入されたものであり、このLL−37を超純水に溶解して320μMの作用濃度にした。各実験群は、6匹のマウスから構成されていた。イソフルラン(Minrad,Bethlehem,Pennsylvania)媒介の全身麻酔に続き、可撓性カテーテル(SILASTIC laboratory tubing,DOW Corning,0.30mm I.D.×0.64mm O.D.,1.5cm長)を無菌条件の下で経尿道的に導入した。尿を完全に排液させた後、発熱物質を含まない0.9%の塩化ナトリウム溶液150μlを1分間滴注し、洗浄ステップとして空にした。次に、LL−37(320μM)を最大容量(150μl)に等しい容積で滴注した。膀胱内接触/滞留時間は45分であった。対照は発熱物質を含まない0.9%の塩化ナトリウム滴注物から構成され、容積及び接触/滞留時間は実験群と同じであった。突然の過膨張に起因する膀胱の外傷の可能性を回避し、潜在的膀胱尿管逆流を予防するため、注入操作を低速に維持し、滴注用シリンジをカテーテル上に維持して、45分間全体で溶液の漏出が起こることのないようにした。45分間の接触/滞留時間の後、膀胱を完全に空にして動物を麻酔から覚ました。実験群に応じて、動物を屠殺し、膀胱内投与後12時間目または24時間目のいずれかに組織を回収した。
2つの実験群をSAGE及びヘパリン処理の両方について調べた。マウスに麻酔をかけ、先に詳述したようにカテーテル処置を行った。第1(SAGE及びヘパリン処理のそれぞれについてn=4)群は、最初にLL−37(320μM)を最大膀胱容量(150μl)で45分間滴注し、次に完全に空にする工程から構成された。その後直ちに、SAGEまたはヘパリンのいずれか10mg/mlを最大膀胱容量(150μl)で45分間滴注した。続いて、SAGEまたはヘパリンを膀胱内投与した後24時間目に、膀胱を回収した。第2(SAGE及びヘパリン処理のそれぞれについてn=4)群は、最初にSAGEまたはヘパリンのいずれか10mg/mlを最大膀胱容量で45分間滴注する工程から構成された。その後、膀胱を空にし、LL−37(320μM)に45分間にわたって曝露(challenge)させた。LL−37投与後24時間目に、組織を回収した。全ての膀胱組織を半切除、処理、固定し、H&Eによる染色または組織MPOアッセイのいずれかを行い、炎症の度合いを数量化する。
GM−1111(表1)50mgとN−ヒドロキシスクシンイミド(NHS)10mgとを脱イオン化(DI)H2O10mL中に溶かした撹拌溶液に、AlexaFluor633ヒドラジド(Invitrogen,Eugene,OR)1mgをジメチルホルムアミド(DMF)4mL中に溶かした溶液を添加した。pHを4.75に調整し、粉末状N,N−ジエチルアミノプロピルカルボジイミド(EDCI)10mgを添加した。3規定のNaOHを滴下により添加して、混合物のpHを4.75に維持した。反応混合物をアルミニウム箔で光線から保護し、室温で一晩撹拌した。その後、反応混合物をNaCl溶液(MWCO 1000)100mmに対して2回透析してから、DI H2Oに対して1回透析した。透析された溶液を凍結乾燥で乾燥させることによって、GM−1111−AlexaFluor633接合体25mgが得られた。Cary50 UV−Vis分光光度計(Varian,Inc.,Palo Alto,CA)を使用して、溶液1mg/mlの吸光度から632nmにおける置換度(SD)を求めた。GM−1111では平均分子量が5000Da、二糖類重量が539(二糖類9つ)AlexaFluor633では1200Da、SDが14%、即ち、二糖類7単位当たりAlexaFluor633の修飾はおよそ1つである。
マウスに麻酔をかけ、先に詳述した方法と同様にカテーテル処置を行った。SAGE AlexaFluor633バイオコンジュゲート(10mg/ml)を用いて、最大膀胱容量(150μl)で45分間膀胱に滴注した。その後直ちに(t=0)または膀胱内SAGE投与後24時間目に(t=24)、膀胱を回収した。全ての組織を処理し、固定して、感光条件下で切断した。全ての細胞核を識別できるように、組織切片をDAPiで対比染色した。ユタ大学の中核施設において、FV1000 Confocal Olympus IX81顕微鏡、及び可変光フィルタ付きHeNeレーザーを利用して、蛍光画像処理が行われた。
膀胱を摘出して長手方向に分割した。膀胱の1切片を4%のパラホルムアルデヒド中に固定し、2切片目を組織ミエロペルオキシダーゼ(MPO:myeloperoxidase)アッセイ用に処理した。解剖用実体顕微鏡を使用して半切除済み膀胱の肉眼的画像の処理(gross images)を行い、Moticam1000デジタルカメラ(Moticam North America,Richmond,CA)で撮影した。膀胱における炎症の変化を評価するため、組織を一連の等級のアルコールで処理してパラフィンに包埋し、5μM切片を切断して、ヘマトキシリン及びエオシン(H&E:hematoxylin and eosin)で染色した。膀胱上皮(尿路上皮)、粘膜下層、固有層、及び平滑筋層における炎症浸潤物(多形核白血球(PMN))の存在及び度合い、浮腫と出血の存在及び度合い、表在性尿路上皮の変化の存在、糜爛、潰瘍化、並びに微小膿瘍形成を基準に、膀胱炎の重篤度を評価した。個々のスライドを検査して、Olympus BX41顕微鏡(オリンパス株式会社,東京,日本)及びImaging Planet 1.4 MPXデジタル顕微鏡カメラ(Imaging Planet Research,Goleta,CA)で撮影した。
半切除済み膀胱を組織MPOアッセイ用に前もって別にしておき、液体窒素中で急速冷凍して、−80℃で貯蔵した。溶解緩衝液(200mMのNaCl、5mMのEDTA、10mMのトリス、10%のグリセリン)(SIGMA,St.Louis,MO)、及びプロテアーゼ阻害剤カクテル(Thermo Scientific,Cat#78140,Rockford,IL)を冷凍組織サンプル(200μl溶解緩衝液/10mg冷凍組織)に添加した。組織をマイクロ剪刀で切り刻み、小型のサンプル実験室用Tissue−Tearor Homogenizer(Biospec Products,Model #985−370,Bartlesville,OK)を使用して4℃で90秒間ホモジナイズした。サンプルを2回にわたってそれぞれ(1500gを4℃で)15分間遠心分離し、上清を新しい無菌管に移した。MPO Sandwich ELISAキット(Cell Sciences,Cat#HK210,Canton,MA)を使用して、膀胱サンプルホモジェネートにおけるMPOの活性を定量した。概括して言うと、ELISAキットDiluent Bufferを用いサンプルを1:5で希釈して、メーカーの推奨事項に従ってアッセイを実行した。標準液のデュプリケート100μl、サンプル及び対照をウェルに添加した。特に明記しない限り、全てのインキュベーションを室温で1時間行い、続いて洗浄した。インキュベーション及び洗浄ステップの後で、トレーサー及びストレプトアビジン−ペルオキシダーゼを順番に添加した。TMB基質を添加して25分間インキュベートした。その後、停止液を添加し、マイクロプレートリーダー(Optimax Tunable,Molecular Devices,Sunnyvale,CA)で450nmにおける吸光度を測定した。各サンプルプレート上の標準曲線を生成し、線形回帰曲線を生成した。全ての値は、平均値+/−SDとして表される。
GM−1111が抗Xa抗凝固活性を全く示さず、0.2U/mg未満の抗IIa抗凝固活性を示す。対照的に、非分画ヘパリンの抗Xa抗凝固活性及び抗IIa抗凝固活性はそれぞれ150U/mgであった。大きな電荷を有するポリアニオン系ポリマーは、ヘパリンとは異なり、第XII因子を活性化し、二次的にキニンを活性化することによって、内因性凝固カスケードを強力に誘発する。第XII因子の活性化に対する検査では、Pセレクチン及びRAGEの薬理学的阻害を遂行するのに必要とされるよりも10倍〜100倍の高濃度においてさえも、医療用ヘパリンよりもGM−1111の方が安全性が高いようである。GM−1111は、最適な安全性及び広範な効能を有するようである。
LL−37による膀胱炎の誘発
膀胱腔内をLL−37に1回曝露させた後、12時間目及び24時間目に、組織を回収して調べた。12時間後の肉眼的検査から、膀胱に中度の炎症があると思われた。この炎症は、紅斑及び出血の病巣領域と共に大域的浮腫によって特徴付けられる(図3C)。24時間後、膀胱に重篤な炎症があるように見えた。この炎症は、大域的浮腫及び出血と共に、有意な組織浮腫及び血管過多によって特徴付けられる(図3D)。生理食塩水に曝露された対照組織は両方の時点において完全に正常であり、炎症の形跡が無いことが観測された(図3A及び図3B)。
硫酸化多糖溶液10mg/mlを滴注してGM−1111がLL−37誘発性膀胱炎を防止または軽減する能力を評価した。2つの群が調査の対象となった。第1(後処理、n=4)群は、LL−37に45分間曝露させ、その後直ちにGM−1111で処理する(滞留時間を45分間とする)工程から構成された。第2(前処理、n=4)群は、GM−1111で処理し(滞留時間を45分間とする)続いてLL−37に45分間曝露させる工程から構成された。第2群は事前のコーティングが予防的効果を発揮するかどうかを評価するのに役立つ。両方の群において、24時間後にマウスを屠殺し、膀胱の回収、撮像、処理を行い、H&Eで染色した。
改変型ヒアルロナンの組織コーティング特性及び貫通特性をよりよく解明するために、蛍光標識されたAlexaFluor633−GM−1111溶液10mg/mを滴注し、45分間の滞留時間を1回経た直後に(t=0)組織を回収して、24時間後に(t=24)再び組織を回収した。また、全ての細胞核を識別できるように、処理済みの組織をDAPiで染色した。即時回収群の最初の結果から、尿路上皮に近接した表在性泌尿器GAG層のコーティングが均一なことと、粘膜下層、基底膜、及び表在性平滑筋層への浸透が深くなったことがわかった(図8A及び図8B)。加えて、細動脈を裏打ちする内皮細胞において、これらの血管構造の基底部及び管腔の両側面にGM−1111で有意なコーティングが施されていることも図示されている。細動脈を裏打ちする内皮においてGM−1111でコーティングされている形跡が無いことが観測された。24時間目の回収群の結果から、即時回収群において前回観測された認められたGM−1111が、泌尿器GAG層沿いに存在する形跡の無いことが明らかにされた。重要な点は、粘膜下組織の領域内、並びに基底部及び管腔の両側面上の小細動脈を裏打ちする内皮沿いに、GM−1111が依然としてはっきり鮮明に見えたことであった(図8C及び図8D)。最後に、GM−1111は依然として、膀胱平滑筋の無作為領域に挿入されていることが可視化された。これらの結果から、24時間後に依然として膀胱内でGM−1111の単回曝露が持続していることがわかった。
塩基処理済みヒアルロナンを原料とする硫酸化低分子量ヒアルロナン(LMW−HA)の調製
a.塩基処理済みLMW HA
100mlのビーカーに入ったNaOH(40%のw/v)20mLにHA(2g、67kDa)を溶解して、混合物を室温にて2時間撹拌し、ストランド開裂を誘発させて部分的にHAを脱重合した。結果として得られた粘性液を、イソプロパノール100mLが入った400mlのビーカーに移し、室温で24時間撹拌した。結果として得られた溶液を重力濾過器(濾紙)に通して粗生成物を収集してから、蒸留水250mL中に溶解させ、pHを7.0に調製した。その溶液を蒸留水に対して24時間透析し、この間にウォーターバスを4回交換してから、凍結乾燥で乾燥させ、1.2gの塩基処理済みHAを得た。この生成物のサイズはHPLC<GPCまたは電気泳動で定量でき、概して5kDa〜20kDaの範囲である。
LMW HAのトリブチルアミン(TBA)塩を得るため、塩基処理済みHA(0.2g)を蒸留水20mL中に溶かしたものに、TBA0.2mLを添加した。その混合物を激しく撹拌して凍結乾燥で乾燥させた。結果として得られた塩(LMW HA−TBA)を20mLのN,N−ジメチルホルムアミド(DMF)中に溶解させ、必要過剰量(6mol/HA中の総ヒドロキシル基(即ち、二糖当たり4)の当量)のピリジン三酸化硫黄複合体(0.325g)を添加した。3時間後40℃にて水20mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール30mLを添加して、粗製物質を沈殿させた。硫酸化された粗生成物を濾過して収集し、蒸留水(30mL)中に溶解させ、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させた。生成物の収率は61%(0.22g)であった。1H NMRに基づく置換度は、およそ0.5〜1であった。元素分析によって硫黄含有率4.13%が求められた。平均分子量はGPCで6,100と定量され、多分散性は2.3であった。
HAのトリブチルアミン(TBA)塩を得るため、塩基処理済みHA(0.2g)を蒸留水20mL中に溶かしたものに、TBA0.2mLを添加した。その混合物を激しく撹拌して凍結乾燥で乾燥させた。結果として得られた塩(LMW HA−TBA)を20mLのN,N−ジメチルホルムアミド(DMF)中に溶解させ、必要過剰量(16mol/HA中で利用可能なヒドロキシ基の当量)のピリジン三酸化硫黄複合体(11.0g)を添加した。3時間後40℃にて水20mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール30mLを添加して、粗生成物を沈殿させた。硫酸化された生成物を濾過して収集し、蒸留水(30mL)中に溶解させ、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させることによって0.26gの生成物が得られた(60%の収率)。生成物を1H NMRにより特性評価し、概算の置換度が約3.5であることがわかった。元素分析によって硫黄含有率13.22%が求められた。平均分子量はGPCで5,900と定量され、多分散性は2.2であった。
a.完全O−硫酸化低MW HA(F−OSHA(1)−10,000)
HAのトリブチルアミン(TBA)塩を得るため、HA(0.2g、約10,000Da、1.3MDaのHAから減成したもの)を蒸留水20mL中に溶かしたものに、0.2mLのTBAを添加した。その混合物を激しく混合して凍結乾燥で乾燥させた。結果として得られた塩(HA−TBA)を20mLのN,N−ジメチルホルムアミド(DMF)中に溶解させ、必要過剰量(6mol/HA中で利用可能なヒドロキシル基の当量)のピリジン三酸化硫黄複合体(0.325g)を添加した。3時間後40℃にて水20mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール30mLを添加して、粗製物質を沈殿させた。硫酸化された生成物を濾過して収集し、蒸留水(30mL)中に溶解させ、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させることによって0.19gの生成物が得られた(58%の収率)。元素分析によって硫黄含有率12.62%が求められた。これは、硫酸化度が3.0〜3.5であることを示している。分子量は3,000Da未満であり、これは硫酸化及びワークアップ中の酸性脱重合を示唆している。
HAのトリブチルアミン(TBA)塩を得るため、酸変性10,000MWのHA(0.2g)を蒸留水20mL中に溶かしたものに、TBA0.2mLを添加した。その混合物を激しく混合して凍結乾燥で乾燥させた。結果として得られた塩(HA−TBA)を20mLのDMF中に溶解させ、必要過剰量(16mol/HA中で利用可能なヒドロキシル基の当量)のピリジン三酸化硫黄複合体(1.1g)を添加した。3時間後40℃にて水20mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール30mLを添加して、粗製物質を沈殿させた。硫酸化された生成物を濾過して収集し、蒸留水(30mL)中に溶解させ、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥させることによって0.23gの生成物が得られた(62%の収率)。その生成物を1H NMRにより特性評価し、置換度が3.0〜3.5であることがわかった。元素分析によって硫黄含有率12.10%が求められた。分子量は3,000Da未満であった。
Kewpie製Hyalo−Oligo HA(200mg、0.5mmole、8.4kDa)をDMF10mL中に溶解させた。撹拌しながらTBA(1当量、0.5mmole、0.12mL)を添加し、更に10分間撹拌した。必要過剰量(6mol/HA中で利用可能なヒドロキシル基の当量数)のピリジン三酸化硫黄複合体(24当量、12mmole、1.916g)を添加した。40℃で3時間撹拌した後、水15mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール25mLを添加して、粗製物質を沈殿させた。硫酸化された生成物を濾過して収集し、蒸留水25mL中に溶解させて、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させることによって0.175gの生成物が得られた(51%の収率)。その生成物を1H NMRにより特性評価し、置換度が3.5超であることがわかった。平均分子量はGPCで6,800Daと定量され、多分散性は1.88であった。
Novozymes製HA(200mg、0.5mmole)を11kDaに減成させたものを、DMF10mL中に溶解させた。撹拌しながらTBA(1当量、0.5mmole、0.12mL)を添加し、混合物を更に10分間撹拌した。次に、必要過剰量(6mol/HA中で利用可能なヒドロキシル基の当量数)のピリジン三酸化硫黄複合体(24当量、12mmole、1.916g)を添加した。40℃で3時間撹拌した後、水15mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール25mLを添加して、粗製物質を沈殿させた。硫酸化された生成物を濾過して収集し、蒸留水25mL中に溶解させて、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させることによって0.194gの生成物が得られた(57%の収率)。その生成物を1H NMRにより特性評価し、置換度が約3.0〜3.5であることがわかった。平均分子量はGPCで8,100Daと定量され、多分散性は2.00であった。
Novozymes製HA(400mg、1.0mmole、11kDa)を11kDaに減成させたものを、DMF25mL中に溶解させた。撹拌しながらTBA(1当量、1.0mmole、0.24mL)を添加し、更に10分間撹拌した。必要過剰量(3mol/HA中で利用可能なヒドロキシル基の当量数)のピリジン三酸化硫黄複合体(12当量、12mmole、1.908g)を添加した。40℃で3時間撹拌した後、水30mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール50mLを添加して、粗製物質を沈殿させてから、濾過して収集した。結果として得られた部分的O−硫酸化粗HAを蒸留水40mL中に溶解させて、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させることによって0.386gの生成物が得られた(56%の収率)。その生成物を1H NMRにより特性評価し、置換度が2.0であることがわかった。平均分子量はGPCで9,500Daと定量され、多分散性は1.77であった。
Novozymes製HA(200mg、0.5mmole)を11kDaに減成させたものを、DMF10mL中に溶解させた。撹拌しながらTBA(1当量、0.5mmole、0.12mL)を添加し、混合物を更に10分間撹拌した。必要過剰量(6mol/HA中で利用可能なヒドロキシル基の当量数)のDMF三酸化硫黄複合体(24当量、12mmole、1.836g)を添加した。30℃で3時間撹拌した後、水15mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール25mLを添加して、粗製物質を沈殿させてから、濾過して収集した。結果として得られた完全O−硫酸化粗HAを蒸留水25mL中に溶解させて、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させることによって0.057gの生成物が得られた(17%の収率)。その生成物を1H NMRにより特性評価し、置換度が約3.0〜3.5であることがわかった。平均分子量はGPCで1,900Daと定量され、多分散性は2.48であった。
Novozymes製HA(5.0g、5.4kDa、950kDaのHAから減成されたもの)をN,N−ジメチルホルムアミド(DMF)400ml中に懸濁させて、3.0ml(1当量)のトリブチルアミン(TBA)を添加した。溶液を5分間撹拌した。ピリジン三酸化硫黄複合体(24当量、48.4g)を6回に分けて添加し、混合物を40℃で3時間撹拌した。水100mLを添加して反応を抑制し、無水酢酸ナトリウムで飽和された低温エタノール500mLを添加して、粗製物質を沈殿させた。硫酸化された生成物を濾過して収集し、蒸留水(650mL)中に溶解させ、2日間にわたってNaCl溶液100mmに対して(溶液を6回交換)及び水に対して(水を2回交換)透析して、1日に4回溶液を交換してから、凍結乾燥で乾燥させることによって4.6gの生成物が得られた(51%の収率)。元素分析によって硫黄含有率13.5%が求められた。これは、硫酸化度が3.0〜3.5 SDであることを示している。分子量はゲル浸透クロマトグラフィで5.1kDaと定量され、多分散性は1.9であった。
GM−0111は耐性が極めて十分であり、C57BL/6Jマウスに2,000mg/kgを1回で経口的に大量に瞬時に投与したときに、毒性の徴候をいっさい呈しなかった。加えて、GM−0111経口投与後24間目に死体解剖の調査結果からは、いかなる異常も認められなかった。
a.ヒト白血球エラスターゼ(HLE:Human Leukocyte Elastase)阻害アッセイ
白血球エラスターゼに対する硫酸化HAの阻害作用を調べるため、7.5μg/mlのHLE100μlを0.001〜100μg/mLの濃度範囲の硫酸化HA100μlと共にインキュベートした。混合物を25℃で10分間インキュベートし、その後にHLE基質suc−Ala−Ala−Val−pNA50μl(1.5mm)を添加した。基質が活性HLEによって開裂され、色原体のpNAが生成された後、動力学的読み出し(kinetic read)によって405nmにおける吸光度の変化を測定した。IC50値は、硫酸化HA濃度に対するVmax(吸収速度)の4パラメータロジスティック非線形回帰方程式を使用して求められる(表1)。
CML−BSA及びRAGE複合体阻害アッセイは、ポリ塩化ビニルプレートを5μg/mlのCML−BSA100μlでコーティングして準備された。別途に、RAGE−Fcキメラ含有PBST−0.1%BSAの1μg/ml溶液を、等量の連続希釈済み硫酸化低分子量ヒアルロナン及びHAオリゴ糖(0.0005μg/ml〜100μg/mlの濃度範囲)と共に4℃で一晩インキュベートした。翌日、RAGE−硫酸化HA混合物50μlをそれぞれリガンドでコーティングされたウェルに移して37℃で1時間インキュベートした。その後、ウェルをPBSTで4回洗浄した。結合したRAGEを検出するため、0.5μg/mLの抗RAGE抗体50μlをそれぞれのウェルに添加した。プレートを室温で1時間インキュベートし、再びウェルをPBSTで4回洗浄した。HRP接合二次抗体(50μl/ウェル)を添加し、ウェルを室温で1時間インキュベートしてからPBSTで4回洗浄した。TMB100μlを添加して比色反応を開始させ、1規定のHClを50μl添加して停止させた。450nmでの吸光度を硫酸化HA濃度に対してプロットし、4パラメータロジスティック非線形回帰方程式を使用してIC50値を求めた(表2)。
ピリジン三酸化硫黄複合体を使用して調製された硫酸化HAサンプルのピリジニウム含有率を、UV吸光度により分析した。1−臭化ブチルピリジニウム(Sigma,St.Louis,MO)を使用して標準曲線を作成し、必要に応じて、UV測定値が標準曲線内に収まるように硫酸化HAサンプルを2mg/mlから0.025mg/mlまで希釈した。標準液及びサンプルについてクォーツ製キュベットにおける255nmでの吸光度値を記録した。標準曲線から、各サンプルについてピリジニウムの重量%値を計算した。これらの計算は表3に記載されている。また、FOSHA−Kewpie−Pyr.SO3のピリジニウム付加体も13C NMR分光法により特性評価し、公開されているデータと比較した(Hintze V,Moeller S,Schnabelrauch M,Beirbaum S,Viola M,Worch H,Scharnweber D.“Modifications of Hyaluronan Influence the Interaction with Human Bone Morphogenetic Protein−4(hBMP−4)”Biomacromolecules 10:3290−3297,2009年)。13C NMRデータを表4に示す。最後に、図5は1H NMRスペクトルのFOSHA−Kewpie−Pyr.SO3で、ピリジニウム付加体のピリジニウムプロトンを示し8.00ppm〜9.10ppmの間の3つのピークを露呈している。
a.マウスの膀胱炎モデル
マウスの膀胱はLL−37(カテリシジンペプチド)をはじめとする様々な催炎物質に対する感受性が高く、膀胱炎を含む炎症疾患において有望な治療剤を研究するための秀逸な動物モデルとして役立つ。硫酸化HAの消炎効果を研究するため、ネズミ膀胱炎モデルでFOSHA誘導体の防護効果を測定した。最初に、C57/BL6成体雌のマウスを麻痺させて、尿道を通してカテーテルを膀胱に挿入した。0.9%の滅菌生理食塩水を注入してから排液させて、膀胱を洗浄した。その後、膀胱に150μLの生理食塩水、または10mg/mlのFOSHA(Kewpie)若しくは10mg/mlのFOSHA(Novozymes)のいずれかを1時間事前滴注した。膀胱を空にしてから、更に1時間かけて320μmのLL−37を150μL滴注した。全ての動物が合併症を伴わずに完全に回復した。手順完了後24時間目に、膀胱を摘出し、撮影して、MPO分析用に冷蔵した。
催炎物質に対する主な細胞応答は、損傷のある細胞から様々なサイトカインを分泌して様々な免疫細胞を標的部位に補充するというものである。MPOは、炎症の初期に炎症部位に補充される一次細胞である多形核細胞において多量に発現するペルオキシダーゼ酵素であり、ゆえに、炎症の度合いを量的に測定するマーカーとして優れている。ネズミ膀胱炎モデルにおいて、完全硫酸化HAの消炎作用を分析するため、酸化HAで前処理された膀胱内で発現したMPOの量を測定し、無処理の膀胱及び生理食塩水対照において発現したMPOの濃度と比較した。膀胱を秤量してホモジナイズした。ホモジナイズされたサンプルを5,000rpmで遠心分離し、可溶な画分を組織破片から分離させてから、マウスMPO ELISAキット(HK210,Hycult biotech,The Netherlands)を使用して組織ホモジェネート内のMPO濃度(ng/mg組織)を測定し、生理食塩水を滴注された対照(炎症の無い正常な膀胱)との差分(%)として表した。結果を図2に示す。
in vitro研究による結果は、RAGE拮抗剤活性に関して硫酸化度が重要なことを実証している(表2)。部分的硫酸化(硫黄分が6%未満)の場合、RAGE拮抗剤の効力が大幅に低下する。硫酸化HA化合物MWが2,000Da未満の場合はまた、in vitroアッセイにおける効力の低減を示す。膀胱炎のマウスモデルにおいて完全硫酸化HA化合物(1%w/w超のピリジニウム付加体を有するものを含む)は、in vivoで炎症を軽減する効果が良好であることを示した。
LL−37により誘発されたネズミ膀胱炎モデルにおいて、GM−0111の治療効果を研究するため、更なる実験を行った。各動物をイソフルランで麻痺し、可撓性カテーテル(SILASTIC laboratory tubing,0.30mm i.d.×0.64mm o.d.,DOW Corning,MI)を、尿道口を通して膀胱中に挿入した。腹部を静かに押圧して尿を排液させた。エンドトキシンを含まない滅菌リン酸緩衝生理食塩水(PBS,Amresco,OH)150μLを滴注し排泄させて、膀胱を洗浄した。その後、空にした膀胱に150μLのPBSを充填するか、または様々な濃度のGM−0111(1、5、10、30、及び100mg/mL)をPBS中に溶解した。膀胱に対するGM−0111の接触を増やすために、膀胱から排液してから、PBSまたはGM−0111のいずれかを再滴注した。GM−0111を1時間滴注した後、膀胱炎を誘発するため、膀胱から排液してから同じ容量のLL−37(250μM)を滴注した。また、GM−0111を滴注するたびにLL−37の滴注手順を繰り返した。LL−37の初回滴注後1時間目に、カテーテルを取り出して動物を回復させた。膀胱の損傷を最小限に抑え、膀胱尿管逆流を低減するため、溶液を流速2μL/sec(または10μL/5sec)で滴注した。溶解した全ての材料を除菌して、微生物汚染の可能性を最小限に抑えた。
LL−37滴注後24時間目に、動物をイソフルランで高度に麻痺させて死体解剖を行った。尾部大静脈を通して全血液を収集し、動物を失血させて、膀胱を回収した。血液をMicrovetteチューブ(Sarstedt,Germany)に移して、血清を収集する。回収された膀胱を秤量して横方向に二等分した。続いて、二等分された膀胱を(血清サンプルと同様)生化学分析の目的に−20℃で貯蔵するか、または組織学的評価の目的に4%ホルマリンで貯蔵した。
体内において誘発性膀胱炎による全般的な反応を定量するため、GM−0111の防護効果を調べて、ELISAキット(ICL Laboratories,OR)を使用して血清アミロイドP(SAP:serum amyloid P)の濃度を測定した。局部組織内の炎症の重篤度を定量するため、プロテアーゼ阻害剤(Halt Protease Inhibitor Cocktail,Thermo Scientific,IL)を補充した溶解緩衝液(200mmのNaCl、10mmのトリス、10%のグリセリン)中で膀胱をホモジナイズした。Fluoro MPO(商標)キット(Cell Technology,CA)を使用してミエロペルオキシダーゼの組織活性を測定し、IL−6及びPTX−3(BioLegend,CA and R&D Systems,MN)の組織中濃度をELISAで測定した。
ホルマリンで固定した組織をパラフィン包理し、4μmの厚さに切断して、ヘマトキシリン及びエオシン(Histology Services by Charles River Laboratories,MA)で染色した。下掲の基準(表5)に従い、浮腫の有無及び程度、多形核好中球(PMN:polymorphonuclear neutrophilic)浸潤、並びに各断層内の尿路上皮の糜爛を評価することによって、各サンプルの炎症の重篤度を評価し、定量した。
様々な測定からのデータを全て個別に識別した。GM−0111の事前滴注が、PBS処理と比較して、観測されたデータに有意に異なる変更をもたらしたかどうかを比較するため、分散試験の分析を行ってからダネットのt検定を行った。死体解剖スコア及び組織学的スコアをクラスカル−ウォリス順位和検定で評価し、続いて、R2.14.0を使用したpgirmessライブラリでkwmc(多重比較)試験を行った。また、データを無処理の正常な動物のデータと比較し、試験を繰り返して、GM−0111の事前滴注が膀胱に炎症性変化が生ずるのを防ぐかどうかを判別した。
GM−0111の事前滴注による、膀胱における膀胱炎の防止
LL−37誘発性膀胱炎モデルに対するGM−0111の防護効果を定量するため、膀胱を数とおりの濃度のGM−0111を膀胱に1時間滴注してコーティングし、続いてLL−37を1時間滴注した。LL−37滴注後24時間目に動物を屠殺し、死体解剖を行い、生化学及び組織学的評価の目的のために膀胱を回収した。

Claims (8)
- 泌尿器の炎症を治療または予防する局所用医薬の製造における、
硫酸化ヒアルロナン、または医薬的に許容可能なその塩若しくはエステルの使用において、前記硫酸化ヒアルロナンのN−アセチル−グルコサミン残基の第一級C−6ヒドロキシルプロトンの100%が硫酸基で置換されており、前記硫酸化ヒアルロナンの硫酸化度が3.5〜4.0であり、前記硫酸化ヒアルロナンの平均分子サイズが2kDa〜10kDaであることを特徴とする使用。 - 請求項1に記載の使用において、前記医薬が更に、消炎剤、解熱性剤、消炎の用に供するステロイド薬及び非ステロイド薬、ホルモン、成長因子、避妊薬、抗ウイルス剤、抗菌剤、抗真菌薬、鎮痛薬、催眠薬、鎮静剤、精神安定剤、抗痙攣薬、筋弛緩薬、局所麻酔薬、鎮痙薬、抗潰瘍薬、ペプチド性アゴニスト、交感神経作動薬、心血管薬剤、制癌薬、またはオリゴヌクレオチドを更に含むことを特徴とする使用。
- 請求項1に記載の使用において、前記医薬が、経粘膜投与に適した製剤を含むことを特徴とする使用。
- 請求項1に記載の使用において、前記医薬が、経膣投与、又は膀胱内注入に適した製剤を含むことを特徴とする使用。
- 請求項1に記載の使用において、前記泌尿器の炎症が前記膀胱、尿道、尿路上皮内膜、前立腺、膣、子宮、またはそれらの任意の組み合わせの炎症を含むことを特徴とする使用。
- 請求項1に記載の使用において、前記硫酸化ヒアルロナンの平均分子サイズが3kDa〜6kDaであることを特徴とする使用。
- 硫酸化ヒアルロナン、または医薬的に許容可能な塩若しくはそのエステルを含む泌尿器の炎症を治療または予防する局所用医薬において、前記硫酸化ヒアルロナンのN−アセチル−グルコサミン残基の第一級C−6ヒドロキシルプロトンの100%が硫酸基で置換されており、前記硫酸化ヒアルロナンの硫酸化度が3.5〜4.0であり、前記硫酸化ヒアルロナンの平均分子サイズが2kDa〜10kDaであることを特徴とする医薬。
- 請求項7に記載の医薬において、前記硫酸化ヒアルロナンの平均分子サイズが3kDa〜6kDaであることを特徴とする医薬。
