Description
本出願は、2006年3月31日に出願された、“Ocular Allergy Treatment(眼アレルギーの処置)”と題された米国仮出願第60/788185号からの優先権を主張する米国本出願第11/688,016号の一部継続出願である。
本発明は、アルカフタジンを患者の眼に投与することによる眼アレルギーの処置若しくは予防方法を包含する。本明細書に記述される発明は、眼アレルギーの処置若しくは予防にそれをとりわけ有用にする、アルカフタジンが眼アレルギーの多数の異なる症状を処置若しくは予防するという驚くべき発見に少なくとも部分的に基づく。本発明の方法、眼科用組成物およびキットは、有効成分の最小の全身吸収を伴い眼アレルギーおよび眼の炎症の臨床症状を軽減する。特性のこの異例の組合せは、眼への局所投与のため処方される場合に優れた安全性プロファイルおよび忍容性と一緒になって、該薬物を眼アレルギーの処置若しくは予防にとりわけ有用にする。とりわけ、本発明は、アルカフタジン、その製薬学的に許容できる塩、そのN−オキシド、水和物、溶媒和物、多形、プロドラッグ若しくはそれらの混合物の有効量を患者の眼に投与することを含んでなる、眼アレルギーの臨床症状の処置若しくは予防方法を包含する。

20−6064(E.I.du Pont de Nemours and Company、デラウェア州ウィルミントン)であり、好ましいLDPE包装材料は、射出吹込成型工程により眼科用組成物を含有する可撓性滴瓶を製造するのに一般に使用され、そして米国食品医薬品局によりこうした使用に承認されている。該キットは、アルカフタジンを含有する眼科用製剤の複数用量若しくはアルカフタジンの単回使用用量を含有しうる。
薬学的に許容できる塩という用語は全部、それらの前述の意味および好ましい範囲を有する。式IIの化合物、その製薬学的に許容できる塩、そのN−オキシド、水和物、溶媒和物、多形、プロドラッグ若しくはそれらの混合物の「有効量」という用語は、眼アレルギーの症状を処置若しくは予防するのに必要とされる本物質の量である。アルカフタジン、その製薬学的に許容できる塩、そのN−オキシド、水和物、溶媒和物、多形、プロドラッグ若しくはそれらの混合物の有効量に関して以前に記述されたとおり、式IIの化合物、その製薬学的に許容できる塩、そのN−オキシド、水和物、溶媒和物、多形、プロドラッグ若しくはそれらの混合物の有効量は、患者で所望の応答を導き出す、単独または1種若しくはそれ以上の組合せ薬物と組合せの式IIの化合物、その製薬学的に許容できる塩、N−オキシド、水和物、溶媒和物、多形、プロドラッグおよびそれらの混合物の能力に依存して、患者ごとに変動しうる。有効量を決定するのに使用される因子は当業者に既知であり、そしていくつかのそれらの因子が本明細書で挙げられる。式IIの化合物の有効量が約0.25mg未満と約0.015mg以上の間、より好ましくは約0.030mgと約0.14mgの間、より好ましくは約0.075mgと約0.125mgの間であることが好ましい。
アルカフタジンを含有する眼科用溶液を表Iに従って製造した。無菌性を保証するため、該溶液を、以前にエチレンオキサイド滅菌を受けていたLDPE瓶に充填される前に、0.22ミクロン滅菌フィルタを通過させた。
アレルギー性結膜炎の急性期反応(浮腫および紅斑)に対するアルカフタジンの効果を、ウサギ皮膚鱗屑に対し全身性に感作しかつウサギアレルゲンで17日後に局所で攻撃したモルモットにて、他の既知の抗アレルゲンの効果と比較した。
アルカフタジンは、能動アナフィラキシーのマウスモデルでアレルギー性結膜炎の徴候および症状を予防することが示されている。
実施例1からの3濃度のアルカフタジン眼科用溶液のそれぞれの単回投与の抗アレルギー効果を、アレルギー性結膜炎の病歴をもつ成人志願者で実施した結膜アレルゲン攻撃(「CAC」)で評価した。被験体は、アレルゲン攻撃に対する反応性を確認するための2回の来院後に選択した。被験体は、彼らがネコ毛、ネコ鱗屑、樹木花粉、イネ科花粉などのような数種の一般的アレルゲンの最低1種に対し陽性の皮膚試験および眼反応を有した場合に、該試験に適した。被験体をその後、アルカフタジン、PATANOL(R)オロパタジン塩酸塩眼科用溶液0.1%(Alcon,Inc.、テキサス州フォートワース)若しくはベヒクルの両側滴下16時間および15分後の2回の個別の来院で、再構成した商業的に入手可能なアレルゲンを各眼に滴下することにより、アレルゲンで攻撃し、そして臨床応答を評価した。
攻撃16時間前にアルカフタジンを投与した患者は眼のかゆみの用量に関連した阻害を表した。全濃度のアルカフタジンが、5点尺度を使用する被験体評価に基づき、攻撃3、5および7分後にベヒクル若しくはPATANOLより低い平均かゆみスコア(すなわちより少ないかゆみ)を示した。処置15分後に攻撃した場合、アルカフタジン処置した被験体はプラセボに比較して眼のかゆみの用量に関連した阻害を表し、そして、0.25%処置群はベヒクル若しくはPATANOLより低いかゆみスコアを有した。
攻撃16時間前にアルカフタジンを投与した患者は、結膜発赤の用量に関連した阻害もまた表した。評価は、攻撃7、15および20分後に、5点尺度を使用した発赤の研究者
の評価に基づき行った。全濃度のアルカフタジンが、大部分の評価時点で、平均結膜発赤スコアの多様な程度の低下を示した。処置後15分に攻撃した場合、0.25%処置群はベヒクル若しくはPATANOLより低いスコアを有した。
類似の結果が、CACにより誘発した鼻症状の予防について観察された。くしゃみ、鼻漏、掻痒(鼻および耳/口蓋)ならびに鼻うっ血の鼻症状を、標準化した尺度を使用して被験体により評価した。
アルカフタジンは酸化に対し感受性であり、そして該主分解生成物は下のN−オキシド構造と同定されている。主酸化的分解ピークとしてのN−オキシド構造の存在は、合成で製造したN−オキシド構造と同一のHPLC相対保持時間を有するとして確認され、そして質量分析を使用してもまた確認された。

結果:
未滅菌瓶(Rexam若しくはBunder Glas)またはγ線照射瓶(Bund
er Glas)いずれか中に保存したアルカフタジン溶液についての化学的安定性の結果を表2に要約する。全サンプルは50℃で6および14日保存後にHPLCによりアッセイした。光曝露は保存の間制御しなかった。溶液はRexam瓶(滅菌を伴わない)、Bunder Glas瓶(γ線照射で滅菌若しくは滅菌を伴わない)のいずれかおよびガラスバイアル中で保存した。
γ線照射したBunder Glas瓶に保存したアルカフタジンは、ガラスバイアル、ならびに未滅菌RexamおよびBunder Glas瓶と比較して、有意に増大されたレベルのN−オキシド形成を示した。このデータは、γ線照射滅菌方法が、ポリエチレン瓶中で化学若しくは物理的変化を誘発することにより酸化の主原因となりうることを示唆する。
エチレンオキサイドを使用して、未滅菌のRexamおよびBunder Glas双方の瓶を滅菌した。安定性試験を開始し、そして、AppTec(ジョージア州マリエッタ)による水抽出試験法(ANSI/AAMI/ISO:10993−7)を使用して、エチレンオキサイド濃度が1ppm未満(周囲条件下)であることが測定された。
材料および方法:

エチレンオキサイド滅菌したRexamおよびBunder Glas瓶に保存したアルカフタジンの化学的安定性の結果を表4に要約する。
アルカフタジンの細胞安定化能力を、肥満細胞安定化を遂げるその能力を検討する一方法としてRBL−CCR1(ラット好塩基球性白血病ケモカイン受容体−1)細胞株を使用して評価した。抗アレルギー薬を細胞安定化能力について評価するための好塩基球細胞株の使用は十分に確立されている。生理学的に好塩基球は肥満細胞に類似であり、IgE架橋が関わる類似の脱顆粒過程により放出される予め形成された炎症メディエーターを含有する。これらの細胞株は容易に入手可能であるため、それらは、安定化アッセイを肥満細胞株で実施することを上回る有効度を提示する。

11−ジヒドロ−11−(1−メチル−4−ピペルジニリデン)−5H−イミダゾ[2,1−b][3]ベナゼピン−3−カルボン酸(CAS番号147083−93−0)(100μMまで)はそうでない。
結膜上皮の完全性を維持するアルカフタジンの能力を検討するため、密着結合タンパク質ZO−1およびE−カドヘリンの発現の変化(E−カドヘリンについて誘導、およびZO−1について限局性から散在性への質的変化)を特異的結膜アレルゲン攻撃後に評価した。これらの変化は角膜および他の上皮組織の増大された浸透性を伴う。このin vivo実験は、これらのタンパク質の調節に対しアルカフタジン(攻撃1および2時間前に局所投与)が何の影響を有したかを検査した。
アレルギー性炎症は2つの異なる相すなわち初期相および遅延相に分離し得る。初期相の炎症反応は肥満細胞脱顆粒後迅速に発生し、そして血管内皮細胞のgapingおよび漏出(すなわち腫脹)ならびにかゆみを特徴とする。遅延相の炎症反応は肥満細胞脱顆粒のおよそ24時間後に頂点に達し、そして細胞浸潤物(好酸球および好中球)の出現を特徴とする。好酸球および好中球双方は炎症反応の遅延相を累乗することに対する顕著な影響を有する。炎症の部位に到達することに際して、これらの細胞は、多様なペルオキシダーゼ、および侵入する病原体を死滅するよう機能する他の抗菌因子を放出するが、しかし重症若しくは慢性の炎症の場合には、周囲組織を損傷しかつ炎症前メディエーターのさらなる放出を引き起こす。
ー性炎症を低下させ得るかどうかを決定するため、アルカフタジン眼科用溶液を評価した。エバンスブルー色素漏出を使用して、初期相の炎症反応の間の血管透過性に対する影響を評価した。結膜を24時間に採取し、染色し、そして遅延相抗炎症活性を評価するため好中球および好酸球浸潤の存在について評価した。

2.感作、攻撃、処置なし(陽性対照)(N=12)
3.感作、攻撃、アルカフタジン2.5mg/ml処置(N=10)
4.感作、攻撃、ベヒクル処置(N=12)
ことを示す。しかしながら、それは、感作マウスでのアレルゲン攻撃後のアレルギー性結膜炎のマウスモデルにおいて、遅延相応答における好酸球若しくは好中球動員を阻害すると思われない。
患者にアルカフタジンの0.25%眼科用溶液を7日間両側に投与した。それらの患者の血漿濃度を、投与前、ならびに第1および7日の薬剤点眼0.25、0.5、1、1.5、2、3、4、6、8、12および18時間後に評価した。アルカフタジンの血漿濃度は迅速にCmaxに達し、そして投薬後3時間までに定量下限(0.01ng/mL)より下に低下した。平均Cmax値(いずれかの時点で測定された最高値)は極めて低く(第1日に0.051ng/mLおよび第7日に0.060ng/mLの平均);最大血漿濃度は全被験体について0.12ng/mL未満であった。
患者に6,11−ジヒドロ−11−(1−メチル−4−ピペルジニリデン)−5H−イミダゾ[2,1−b][3]ベナゼピン−3−カルボン酸の0.25%眼科用溶液を7日間両側に投与した。それらの患者の血漿濃度を、投与前、ならびに第1および7日の薬剤点眼0.25、0.5、1、1.5、2、3、4、6、8、12および18時間後に評価した。6,11−ジヒドロ−11−(1−メチル−4−ピペルジニリデン)−5H−イミダゾ[2,1−b][3]ベナゼピン−3−カルボン酸の血漿濃度は迅速にCmaxに達し、そして投薬後12時間までに定量下限(0.1ng/ml)より下に低下した。第1日に3.228ng/mLおよび第7日に2.715ng/mLの平均Cmax値;最大血漿濃度は7.23ng/mLであった。
Claims (1)
- 眼の炎症の臨床症状の処置若しくは予防のための医薬の製造における、0.005%から10重量%の間のアルカフタジン、その製薬学的に許容できる塩、そのN−オキシド、その水和物、その溶媒和物、その多形若しくはそれらの混合物、塩化ナトリウム、リン酸ナトリウム、塩化ベンザルコニウム、エデト酸二ナトリウム、および眼科用ビヒクルを含んでなる眼科用組成物、の使用。
