Description
本願は、2010年4月29日に出願された米国特許出願第12/770088号明細書の一部継続出願であり、ここで引用したことによりその開示内容は全部が本明細書に記載されたものとして本明細書に取り込まれる。
顎矯正手術は、概して、構造、成長、睡眠時無呼吸、TMJ疾患に関連する顎(すなわち下顎骨)、上顎骨及び顔面の条件を修正するために又は矯正問題を修正するために行われる。例えば、著しく後退した上顎又は開咬を有する個人に対して、上顎骨切り術が有効である。このような手技において、外科医は、両眼窩の下側で切断を行い、上顎骨の分割された部分を上顎骨の一体の部分から分離させる。口蓋及び全ての上側の歯を含む分割された部分全体は、一つのユニットとして移動することができる。次いで、分割された部分は、上側の歯と下側の歯とが適切に咬み合うまで移動させられる。歯が再位置決めされると、小さなスクリュ及びプレートが使用されて、自然の骨の癒合が生じるまで上顎骨の分割された部分を新たな位置に固定する。
本開示は、概して、顎矯正手術において使用されるインプラントの改良、特に、顎矯正手術において使用するための、特定の患者用のプレートの改良に関する。しかしながら、開示されたインプラントは、この特定の用途に限定されない。
図1を参照すると、顎矯正手術において使用される骨固定インプラント10は、患者の頭骨12、特に、骨切り術のような分割手技によって上顎骨14が第1の"分割された"部分18と第2の"一体の"部分22とに分離させられた後における患者の上顎骨14のような、下に位置する骨に固定されるよう設計されている。上顎骨14の第1の部分18は、通常、上側の歯を支持しており、骨切り術が行われた後に頭骨12から完全に分離されているのに対し、上顎骨14の第2の部分22は頭骨12と一体のままである。骨固定インプラント10は、上顎骨の第1及び第2の部分に取り付けられ、これにより、骨形成が生じる間、上顎骨の第1の部分18を第2の部分22に対して支持及び保持するよう構成されている。インプラント10は、手術中の合併症及び患者が手術室で過ごす時間を最小限に減じるように術前にカスタマイズされている。

本体を有するプレート部材であって、該プレート部材は、曲げられた本体を貫通しかつ前記第2の部分が前記第1の部分から分離された後に前記プレート部材を前記第1の部分及び前記第2の部分に固定するために骨固定エレメントを収容するよう構成された少なくとも1つの固定開口を形成している、プレート部材と、
該プレート部材に結合された少なくとも1つのガイドであって、該少なくとも1つのガイドは、プッシャを有し、前記プレート部材を前記下顎骨と整合させるように、前記プッシャが下顎骨の底部に隣接して構成されるように前記プレート部材に対し相対的に可動である、ガイドと、を備えることを特徴とする、インプラント。 - 少なくとも1つのガイドは、ガイド本体と、該ガイド本体を貫通した固定エレメント開口とを有し、該固定エレメント開口は、前記ガイドを前記プレート部材に結合するための固定エレメントを収容するよう構成されている、請求項1記載のインプラント。
- 前記プレート部材は、前記インプラントを前記下顎骨に固定するための固定エレメントを収容するよう構成された少なくとも1つの開口を有する、請求項1記載のインプラント。
- 前記少なくとも1つのガイドは、ガイド本体と、該ガイド本体を厚さ方向に貫通し、前記下顎骨の底部に当接すべきプッシャが内部に配置されるプッシャ開口と、を有する、請求項1記載のインプラント。
- 前記プッシャは、前記プッシャ開口内で厚さ方向に沿って調節可能である、請求項4記載のインプラント。
- 前記少なくとも1つのガイドは、さらに、ガイド本体を横方向に通ってプッシャ開口内へ延びたボアを有し、該ボアは、セットスクリュを収容しこれにより前記プッシャを前記ガイド本体にロックするよう構成されている、請求項5記載のインプラント。
- 前記少なくとも1つのガイドは、さらに、前記ガイド本体を長手方向に貫通した骨プレート収容チャネルを有し、該骨プレート収容チャネルは、前記少なくとも1つのガイドが骨プレートに結合されたときに該骨プレートを収容するよう構成されている、請求項4記載のインプラント。
- 前記プッシャは、厚さ方向部分と、該厚さ方向部分の遠位端部から前記下顎骨に向かって延びた横方向部分とを有する、請求項4記載のインプラント。
- 前記プッシャは、前記下顎骨に当接すべき骨接触面を形成している、請求項8記載のインプラント。
- 少なくとも第1の下顎骨部分を、骨間隙によって前記第1の下顎骨部分から分離された第2の下顎骨部分に対して固定するよう構成された予め成形されたインプラントをカスタマイズする方法であって、
互いに対して第1の位置を形成する下顎骨の第1のモデル化された部分と下顎骨の第2のモデル化された部分とを形成するために、コンピュータにおいて患者の下顎骨の術前の三次元モデルを獲得する工程と、
前記下顎骨の術前の三次元モデルを、前記下顎骨の前記第1のモデル化された部分と前記下顎骨の前記第2のモデル化された部分とが互いに対して第1の位置と異なる第2の位置を形成している計画された術後形状に操作する工程と、
骨固定インプラントを、該骨固定インプラントが、前記第1の下顎骨部分と前記第2の下顎骨部分とが互いに対して第2の位置を形成するときに第1の下顎骨部分及び第2の下顎骨部分に取り付けられるよう構成されるように、前記下顎骨の計画された術後形状に合致するようにカスタム製造する工程であって、前記骨固定インプラントは、プレート部材を有し、前記少なくとも1つのガイドは、前記プレート部材に結合されるように構成され、前記プレート部材の前側の外面上に配置可能である、工程と、を備え、
前記カスタム製造する工程は、
前記インプラントが前記第2の下顎骨部分が前記第1の下顎骨部分から分割された後に前記第1の下顎骨部分及び前記第2の下顎骨部分に取り付けられるよう、前記第1の下顎骨部分から前記第2の下顎骨部分を分割する前に前記プレート部材を曲げる工程と、
前記第2の下顎骨部分が前記第1の下顎骨部分から分離された後で前記インプラントが前記下顎骨に対して位置決めされるときに前記プレート部材を前記下顎骨と整合させるように、前記第1の下顎骨部分から前記第2の下顎骨部分を分割する前に前記少なくとも1つのガイドを構成する工程と、
前記プレート部材によって定められた第1の開口と前記少なくとも1つのガイドによって定められた第2の開口とを整合させる工程と、
固定エレメントを、整合された第1及び第2の開口を通して挿入して前記プレート部材と前記少なくとも1つのガイドとを結合させる工程と、
を備えることを特徴とする、方法。 - 前記少なくとも1つのガイドは、ガイド本体を有し、前記少なくとも1つのガイドを構成する工程は、前記下顎骨が分離させられた後で前記インプラントが前記下顎骨に対して位置決めされるときに前記プレート部材を前記下顎骨と整合させるために、前記下顎骨の術後形状に対応するようガイド本体を予め成形する工程を含む、請求項10記載の方法。
- 前記ガイド本体を前記プレート部材上にオーバーモールドする工程をさらに含む、請求項11記載の方法。
- 前記ガイド本体は、生体適合性材料から形成されている、請求項11記載の方法。
- 前記少なくとも1つのガイドは、ガイド本体と、該ガイド本体を厚さ方向に貫通したプッシャ開口と、該プッシャ開口内に配置されたプッシャとを有し、
前記少なくとも1つのガイドを構成する工程は、前記下顎骨が分離させられた後で前記インプラントが前記下顎骨に対して位置決めされるときに、前記下顎骨の底部に当接するよう、前記プッシャを術前に位置決めする工程を含む、請求項10記載の方法。 - 前記少なくとも1つのガイドを構成する工程は、さらに、前記厚さ方向開口内でプッシャを術前に調節することを含む、請求項14記載の方法。
- 前記第2の位置は、前記第1の位置と異なる、請求項10記載の方法。
- 前記第1の位置は、前記第2の位置と同じである、請求項10記載の方法。
