本発明は、癌を治療するための腫瘍崩壊性ポックスウイルスの使用に関する。特に、ある特定の程度の腫瘍崩壊を実現するための、GM-CSFを発現するワクシニアウイルスの使用が説明される。別の局面において、血管形成した腫瘍または血管形成している腫瘍の治療により有効な治療レジメンにおいて、ワクシニアウイルスを使用することができる。ある特定のレジメンは、腫瘍崩壊性ワクシニアウイルスを用いて治療した後の抗血管新生剤の使用である。ある特定の局面において、治療レジメンは、ワクシニアウイルスが腫瘍の血管虚脱を誘導した後の、腫瘍の血管再生に起因する再灌流を評価するための腫瘍の画像化を含む。ある特定の局面において、ワクシニアウイルスは、JX-594ウイルス(TKマイナスの、GM-CSFを発現するワクシニアウイルス)である。
本発明の一態様において、JX-594などの腫瘍崩壊性ワクシニアウイルスを送達することによって癌などの過剰増殖疾患を治療する方法が意図される。
本発明の化合物および方法は、癌を含む過剰増殖疾患/状態の状況において用いられてもよく、ある特定の投与順序で、投与間で様々な間隔をあけて用いられてもよい。GM-CSF発現ワクシニアウイルスなどの本発明の組成物を用いた治療の有効性を高めるために、これらの組成物と癌治療において有効な抗血管新生剤および他の剤を組み合わせることが望ましい。例えば、癌治療は、本発明の治療用化合物と抗血管新生剤を併用して実施することができる。

癌療法は、化学物質に基づく治療および放射線に基づく治療の両方を用いた様々な併用療法も含む。併用化学療法には、例えば、シスプラチン(CDDP)、カルボプラチン、プロカルバジン、メクロレタミン、シクロホスファミド、カンプトテシン、イホスファミド、メルファラン、クロランブシル、ブスルファン、ニトロソ尿素、ダクチノマイシン、ダウノルビシン、ドキソルビシン、ブレオマイシン、プリコマイシン(plicomycin)、マイトマイシン、エトポシド(VP16)、タモキシフェン、ラロキシフェン、エストロゲン受容体結合剤、タキソール、ゲムシタビエン(gemcitabien)、ナベルビン、ファルネシル-タンパク質トランスフェラーゼ阻害剤、トランスプラチン(transplatinum)、5-フルオロウラシル、ビンクリスチン、ビンブラスチンおよびメトトレキセート、テマゾロミド(Temazolomide)(DTICの水性型)、または前記の任意の類似体もしくは誘導変異体が含まれる。化学療法と生物学的療法の併用は生物化学療法として知られる。
DNA損傷を引き起こし、広範に用いられてきた他の因子には、γ線、X線、および/または腫瘍細胞への放射性同位体の指向性送達と一般に知られているものが含まれる。DNA損傷因子の他の形態は、マイクロ波および紫外線なども意図される。これらの因子は全て、DNA、DNA前駆体、DNAの複製および修復、ならびに染色体の集合および維持に広範囲の損傷をもたらす可能性が最も高い。X線の線量の範囲は、長期間(3〜4週間)では1日線量50〜200レントゲンから1回線量2000〜6000レントゲンに及ぶ。放射性同位体の線量の範囲は多種多様であり、同位体半減期、発せられる放射線の強度およびタイプ、ならびに新生細胞による取り込みに左右される。
免疫療法は、一般的には、癌細胞を標的化および破壊するために、免疫エフェクター細胞および分子を使用することに頼っている。免疫エフェクターは、例えば、腫瘍細胞表面上の、あるマーカーに特異的な抗体でよい。抗体は単独で療法のエフェクターとして役立ってもよく、細胞を実際に死滅させるように他の細胞を動員してもよい。抗体はまた、薬物または毒素(化学療法剤、放射性核種、リシンA鎖、コレラ毒素、百日咳毒素など)と結合され、単に、標的化剤として役立ってもよい。または、エフェクターは、腫瘍細胞標的と直接的または間接的に相互作用する表面分子を有するリンパ球でもよい。様々なエフェクター細胞には、細胞傷害性T細胞およびNK細胞が含まれる。治療モダリティーの組み合わせ、すなわち、直接的な細胞傷害性活性と、ある特定のポックスウイルスポリペプチドの阻害または低下は、癌の治療において治療上の利益を提供すると考えられる。
癌を有する人の約60%が何らかのタイプの外科手術を受けるであろう。外科手術には、予防的手術、診断的手術または進行期分類のための手術、治癒的手術、および姑息的手術が含まれる。治癒的手術は、本発明の治療、化学療法、放射線療法、ホルモン療法、遺伝子治療、免疫療法、および/または代替治療などの他の治療と併用することができる癌治療である。
現行の方法と併用して使用するための療法の別の形には、患者の組織を高温(106°Fまで)に曝露する処置であるハイパーサーミアが含まれる。局部ハイパーサーミア、局所ハイパーサーミア、または全身ハイパーサーミアの適用には、外部加温装置または内部加温装置が関与してもよい。局部ハイパーサーミアは、腫瘍などの小さな領域に熱を加えることを伴う。熱は、体外の装置からの腫瘍を標的とする高周波によって外部から発生させてもよい。内部の熱は、薄い加温ワイヤまたは温水を満たした中空管、埋め込み型マイクロ波アンテナ、または高周波電極を含む滅菌プローブを含んでもよい。
腫瘍を治療する場合に、本発明の方法は、腫瘍崩壊性ワクシニアウイルスを投与し、次に、しばらく経った後、抗血管新生剤を含む組成物を投与する。当然、投与経路は、腫瘍の位置および種類によって変わり、例えば、皮内投与、経皮投与、非経口投与、静脈内投与、筋肉内投与、鼻腔内投与、皮下投与、局所投与(例えば、腫瘍の近くへの投与、特に、腫瘍の脈管構造または腫瘍の隣接する脈管構造の近くへの投与)、経皮的投与、気管内投与、腹腔内投与、動脈内投与、膀胱内投与、腫瘍内投与、吸入、灌流、洗浄、および経口投与を含む。組成物は、特定の投与経路について製剤化される。
本発明においてポックスウイルスゲノムの全てまたは一部をコードする発現構築物またはウイルスを癌または腫瘍細胞に送達するのに好ましい方法は、腫瘍内注射による方法である。しかしながら、腫瘍内注射の代わりに、本明細書において開示される薬学的組成物を、米国特許第5,543,158号;米国特許第5,641,515号および米国特許第5,399,363号(それぞれ全体が参照により本明細書に特に組み入れられる)に記載の通りに非経口的投与、静脈内投与、皮内投与、筋肉内投与、経皮投与、さらには腹腔内投与することができる。
ポックスウイルスは、何世紀もわたって、この科の名前の由来となった痘瘡ウイルス(天然痘)が作り出す特徴的な痘痕で知られている。天然痘は、2000年以上前に中国および極東において最初に出現したようである。幸運にも、命にかかわることが多いこのウイルスは現在、根絶されており、最後の自然大発生は1977年にソマリアで起こった。
ワクシニアウイルスは、約190Kbpの直鎖二本鎖DNAゲノムを有し、約250の遺伝子をコードする、大きく複雑なエンベロープウイルスである。ワクシニアは、天然痘を根絶したワクチンとして役割を果たしたことがよく知られている。天然痘が根絶された後に、科学者らは、遺伝子を生物組織に送達するための道具(遺伝子療法および遺伝子工学)としてワクシニアを用いることを研究してきた。ワクシニアウイルスは、宿主細胞の細胞質でしか複製しないためにDNAウイルスの中では独特のウイルスである。従って、この大きなゲノムは、ウイルスDNA複製に必要な様々な酵素およびタンパク質をコードする必要がある。複製中に、ワクシニアは、外膜が異なる数種類の感染型:細胞内成熟ビリオン(IMV)、細胞内エンベロープビリオン(IEV)、細胞結合性エンベロープビリオン(CEV)、および細胞外エンベロープビリオン(EEV)を産生する。IMVは最も豊富な感染型であり、宿主間の伝播を担っていると考えられている。他方で、CEVは細胞間の伝播において役割を果たしていると考えられおり、EEVは、宿主生物内での広範囲の播種に重要だと考えられている。

ほとんどの転写された真核生物RNA分子は、一次転写物からイントロンを除去するRNAスプライシングを受ける。転写物を適切にプロセシングして、タンパク質を発現するために、ゲノム真核生物配列を含有するベクターはドナースプライシング部位および/またはアクセプタースプライシング部位と必要とする場合がある(参照により本明細書に組み入れられる、Chandler et al., 1997を参照されたい)。
本発明のベクターまたは構築物は、一般的には、少なくとも1つの終結シグナルを含む。「終結シグナル」または「ターミネーター」は、RNAポリメラーゼによるRNA転写の特異的な終結に関与するDNA配列からなる。従って、ある特定の態様において、RNA転写物の生成を終わらせる終結シグナルが意図される。望ましいメッセージレベルを得るために、ターミネーターがインビボで必要な場合がある。
発現、特に、真核生物での発現において、典型的には、転写物の適切なポリアデニル化をもたらすためにポリアデニル化シグナルが組み込まれる。ポリアデニル化シグナルがどういったものであるかは本発明の実施の成功に重要だと考えられておらず、および/またはこのような任意の配列を使用することができる。好ましい態様には、便利な、および/または様々な標的細胞において良好に機能することが知られている、SV40ポリアデニル化シグナルおよび/またはウシ成長ホルモンポリアデニル化シグナルが含まれる。ポリアデニル化は転写の安定性を高めてもよく、細胞質への輸送を容易にしてもよい。
宿主細胞においてベクターを増殖させるために、ベクターは、1つまたは複数の複製起点部位(しばしば「ori」と呼ばれる)を含有してもよい。複製起点部位は、複製が開始する特定の核酸配列である。または、宿主細胞が酵母であれば、自己複製配列(ARS)を使用することができる。
本発明のある特定の態様において、本発明の核酸構築物を含有する細胞は、発現ベクター内にマーカーを含めることによってインビトロまたはインビボで同定することができる。このようなマーカーは同定可能な変化を細胞に付与し、それにより、発現ベクターを含有する細胞を容易に同定できるようになる。一般的には、選択マーカーは、選択を可能にする特性を付与するものである。正の選択マーカーは、マーカーが存在すると細胞の選択が可能になるものであるのに対して、負の選択マーカーは、マーカーが存在すると細胞の選択が妨げられるものである。正の選択マーカーの一例は薬物耐性マーカーである。
本明細書で使用する「細胞」、「細胞株」、および「細胞培養」という用語は同義に使用することができる。これらの全ての用語は、任意のおよび全ての次世代である子孫も含む。全ての子孫は、故意の変異または故意でない変異のために同一でなくてもよいことが理解される。異種核酸配列の発現という状況において、「宿主細胞」は原核細胞または真核細胞を指し、ベクターを複製することができる、および/またはベクターがコードする異種遺伝子を発現することができる任意の形質転換可能な生物を含む。宿主細胞は、ベクターまたはウイルスのレシピエントとして使用することができ、ベクターまたはウイルスのレシピエントとして用いられてきた(外因性ポリペプチドを発現しなければ、ベクターとしての資格がない)。宿主細胞には、改変タンパク質コード配列などの外因性核酸が宿主細胞に移動または導入されるプロセスを指す「トランスフェクト」または「形質転換」がなされてもよい。形質転換細胞は、初代対象細胞およびその子孫を含む。

インビトロデータ-ソラフェニブはJX-594を阻害する
ヒトHCC細胞株PLC/PRF/5においてJX-594をソラフェニブと組み合わせて試験した。PLC/PRF/5ヒトHCC細胞株はJX-594複製を助ける。しかしながら、JX-594感染の2時間前、JX-594感染中、またはJX-594感染の2時間後にソラフェニブ(10μΜ)を添加すると、バーストサイズは1/100まで減少した(図1)。ヒト卵巣癌細胞株A2780およびヒトHCC株HepG2でも、JX-594をソラフェニブと組み合わせて試験した。
ヒト腫瘍細胞株A2780(卵巣)およびHepG2(HCC)はAmerican Type Culture Collection(ATCC)から入手した。10%FBSおよび1%pen/strepを添加したDMEM中で細胞を培養した。細胞を、5%CO2を含む加湿インキュベーターに入れて37℃で増殖させた。インビトロで使用するために、ソラフェニブをDMSO(Sigma-Aldrich Corp.)に1mg/mlの濃度まで溶解し、10%胎仔ウシ血清を添加したDMEMに溶解して適切な最終濃度(100ng/ml、250ng/ml、500ng/ml、1000ng/ml、および2500ng/ml)までさらに希釈した。最終溶液中のDMSOは0.2%(v/v)を超えなかった。
A2780細胞またはHepG2細胞を6ウェルプレートに4x105細胞/ウェルで播種し、一晩放置した。次いで、100pfuのJX-594を各ウェルに添加し、2時間感染させた。感染が終わったら、培地を除去し、最終濃度が0μg/mL、0.1μg/mL、0.25μg/mL、0.5μg/mL、1.0μg/mL、および2.5μg/mLのソラフェニブを含有する3%カルボキシメチルセルロースDMEMオーバーレイを添加した。3日後に、プレートをクリスタルバイオレットで染色し、プラークを計数した。同時に、複製(バーストサイズ)を評価するために、6ウェルプレートを前記の通りに調製した。染色およびプラーク計数の代わりに、ウイルス精製のために各ウェルから細胞を回収した。3回の凍結融解の後に超音波処理を行うことによって細胞を溶解し、その後に、プラークアッセイ法によってウイルスを滴定するために、粗ウイルス溶解産物の連続希釈液をA2780細胞に添加した。さらに、細胞生存率に対するソラフェニブの直接的効果を評価するために、細胞を96ウェルプレートにプレートし、ソラフェニブのみとインキュベートした。生細胞によるテトラゾリウム塩からホルマザンクロマゲン(chromagen)への還元に基づく比色アッセイ法(Cell Counting Kit-8, Donjindo Laboratories, Kumamoto, Japan)によって、細胞生存率を求めた。
細胞傷害レベル未満のソラフェニブ濃度は、A2780およびHEPG2腫瘍細胞株におけるウイルス増殖および複製を阻害することができる(図2)。さらなる実験から、SNU423、SNU398、SNU475、SNU449、SNU387を含む他のヒトHCC株およびヒト骨肉腫株U20Sにおいて、ソラフェニブはJX-594複製を阻害することが分かった。
インビボデータ-ソラフェニブはJX-594を増強する
インビトロでの知見に反して、前臨床効果モデルから、ソラフェニブとJX-594の組み合わせの効果は剤単独より優れていることが分かった。
皮下移植CT26結腸直腸腫瘍細胞のイムノコンピテントマウスモデルにおいて、JX-594とソラフェニブの組み合わせを試験した。Balb/cマウスに3x105個のCT26細胞を皮下注射し、11日後、ソラフェニブ単独(10mg/kg p.o. 2週間、毎日)、JX594単独(108 pfu IV 1週間、週3回)、または併用(JX594の後にソラフェニブまたはソラフェニブの後にJX594)を用いて治療を開始した(n=4/群) (図3、上パネル)。対照動物にはPBSのみを与えた。この実験のために、マウスGM-CSFを発現するJX-594バージョンを使用した。エンドポイントまで週2回、腫瘍を測定した。治療後、各時点での各研究群の生存率を示した(図3、真ん中のパネル)。治療後、各時点での各研究群の平均腫瘍体積を示した(図3、下パネル)。JX-594とソラフェニブの組み合わせはJX-594単独と比較して生存を延ばし、腫瘍量を減少させた。JX-594とソラフェニブの組み合わせはまたソラフェニブ単独の動物と比較して腫瘍量も減少させた。好ましいレジメンは、JX-594の後のソラフェニブであった。
JX-594とソラフェニブの組み合わせを、B16黒色腫腫瘍のイムノコンピテントマウスモデルにおいて試験した。C57BL/6マウスに3x105個のB16-F10-LacZ細胞をIV注射し、ソラフェニブ単独(HD:10mg/kg、LD:50μg/kg p.o. 2週間、毎日)、JX594単独(107 pfu IV 1週間、週3回)、または併用(LDまたはHDソラフェニブの後にJX-594 IV)によって治療した(n=5/群)(図4、上パネル)。治療の終了時に、マウスを屠殺し、B16表面小結節を検出するために肺を固定および染色した(n=5/群)。各群について、研究終了時の腫瘍小結節の平均数を示した(図4)。50μg/kg p.o.ソラフェニブ+JX-594群では、B16腫瘍はJX-594単独群またはソラフェニブ単独群と比較して有意に少なかった。
SCIDマウスに皮下HepG2ヒト肝細胞癌異種移植片腫瘍を移植した。腫瘍が約12〜14mm最大直径のサイズに達したら、マウスを無作為化して、6つの治療群(n=8/群)(1)PBS単独、(2)ソラフェニブ単独(400μgを用いた標準的な毎日の腹腔内投薬)、(3)JX-594単独(静脈内治療。107pfu、毎週、合計6回の投与)、(4)JX-594およびソラフェニブを用いた同時治療、(5)ソラフェニブの後にJX-594、ならびに(6)JX-594(2回投与)の後にソラフェニブの1つにした。
実験方法、パイロット実験:
パイロット実験は、HepG2モデルにおいてJX-594の後にソラフェニブを用いると急性血管閉塞が誘導されることを示すためにデザインした。3つの群を使用した(それぞれn=5匹の動物)。群1にはPBSのみを与えた。群2には、単一用量の106pfu JX-594を静脈内(IV)投与した。かつ群3には、単一用量の106pfu JX-594を腫瘍内(IT)投与した。VEGFレベルおよびGM-CSFレベルを測定するために、D5において血清試料を収集した。安楽死前に、腫瘍灌流を視覚化するためにマウスに蛍光マイクロスフェアを注射した。分析のために、切除した腫瘍を3つの部分に分けた。(1)VEGFタンパク質レベルを測定するために、瞬間凍結試料、(2)マイクロスフェアおよび血管マーカーCD31を視覚化するために、凍結腫瘍切片のOCT包埋試料調製物、ならびに(3)組織学的分析のために、ホルマリン固定/パラフィン包埋試料。
この研究は、血管分布に対する長期的効果を追跡し、HepG2モデルにおけるJX-594+/-ソラフェニブの効果を評価した。HepG2(ヒトHCC株)腫瘍をヌードマウスに皮下移植した。剤単独および併用を試験するために、動物を7つの治療群(n=5/群)に無作為化した。(図6)に詳しく記載したスケジュールで、群をJX-594(106pfu腫瘍内、D1およびD8)ならびに/もしくはソラフェニブ(400μg i.p.、毎日、D15〜D29)またはPBS(対照)によって治療した。カリパスを用いて、腫瘍を週2回、測定した。VEGFレベルおよびGM-CSFレベルを測定するために、血清試料をD1、D8、D15、D22、およびD29に収集した。D22またはD35にマウスを安楽死させた。安楽死前に、腫瘍灌流を視覚化するためにマウスに蛍光マイクロスフェアを注射した。分析のために、切除した腫瘍を3つの部分に分けた。(1)VEGFタンパク質レベルを測定するために、瞬間凍結試料、(2)マイクロスフェアおよび血管マーカーCD31を視覚化するために、凍結腫瘍切片のOCT包埋試料調製物、ならびに(3)組織学的分析のために、ホルマリン固定/パラフィン包埋試料。
CD31染色OCT腫瘍切片において血管を計数した。200×倍率の3つの視野における平均血管数を(図6)に示した。ソラフェニブ単独は一過的な血管数減少を引き起こしたが(29日目)、時が経つにつれて血管数は再び増えた(35日目)。しかしながら、JX-594の後にソラフェニブを与えた動物では、D35での血管数はソラフェニブ単独群と比較して少なかった。このことは、腫瘍脈管構造に対する併用の効果が長く続くことを示唆している。
JX-594は、選択的に癌細胞の中で複製し、癌細胞を破壊するように設計された画期的な標的化腫瘍崩壊性ポックスウイルスである。直接的な腫瘍崩壊に加えて顆粒球マクロファージ-コロニー刺激因子(GM-CSF)発現も動物腫瘍モデルにおける腫瘍血管閉塞を刺激する。肝細胞癌患者におけるJX-594療法後の腫瘍血管閉塞を評価した。さらに、JX-594後の再灌流を阻止するためにソラフェニブを用いたアジュバント抗血管新生療法の実現可能性を、肝細胞癌(HCC)の前臨床研究および臨床研究の両方において研究した。
JX-594によって誘導された血管閉塞およびその後の再灌流の機構を評価するために、マウスHCCモデルにおいて前臨床試験を行った。ソラフェニブが再灌流を遮る能力を評価した。第2相臨床試験では、HCC患者を、JX-594腫瘍内注射によって2週間毎に3サイクルにわたって治療した。ベースライン、5日目、および8週目に、ダイナミック造影(DCE)-MRIおよびDW-MRIによって腫瘍サイズ、血流、および密度を評価した。8週目に部分的な腫瘍再灌流を有する5人の患者は、標準的なソラフェニブ療法を開始し、連続dce-MRIスキャンが行われた。
マウスHCCモデルにおいてJX-594をIT投与またはIV投与した後に腫瘍血管閉塞が生じる。血管閉塞はウイルス複製に依存した。ウイルスからのmGM-CSF発現および好中球浸潤がこの効果を増強した。時間とともに腫瘍周辺部に再灌流が証明され、腫瘍における高いVEGFレベルと相関関係にあった。アジュバントソラフェニブ療法が血管新生を阻害し、剤単独を超える著しく改善した抗腫瘍効果につながった。JX-594によって治療されたHCC患者は、肝臓内の注射腫瘍および非注射腫瘍の両方で急性の腫瘍血管閉塞および壊死を示した。8週目の評価の後にソラフェニブを用いてアジュバント療法を行うと、少なくとも3人の患者において劇的なかつ持続性の腫瘍壊死および血管閉塞が認められた。ソラフェニブを単独で投与した15人のHCC患者からの連続MRIスキャンの調査から、これらの知見は、JX-594によって前処置した患者において増強および強化されることが証明された。
標的化腫瘍崩壊性ポックスウイルスJX-594は選択的に癌細胞の中で複製するので、腫瘍組織内でウイルス子孫の生成、腫瘍細胞の壊死、放出、蔓延が起こる。JX-594はまた、腫瘍崩壊に起因する抗腫瘍免疫を増強するためにGM-CSF導入遺伝子を発現するように操作されている。ワクシニアバックボーンは、本質的には、EGFR-ras経路依存性およびインターフェロンに対する腫瘍耐性のために腫瘍選択性である。固有の治療指数はTK欠失によって増幅される。JX-594複製は細胞TKに依存し、細胞TKは癌では細胞周期異常によって高レベルになっている。難治性肝臓腫瘍患者におけるJX-594第1相臨床試験からの結果から、JX-594複製の安全性、効果、および機械的概念証明(mechanistic proof-of-concept)、全身播種、ならびに生物学的に活性なGM-CSF発現が証明された。最近公表された前臨床試験から、腫瘍崩壊性ウイルス療法が急性腫瘍血管閉塞も誘導できることが証明された(Breitbach, et al. 2007)。
研究の認可および登録:
研究プロトコールおよびインフォームドコンセントフォームは、米国FDA、韓国FDA、および韓国、釜山にある釜山大学医学部付属病院の施設内審査・感染対策委員会(Institutional Review and Infection Control Committees)にる認可を受けた。このプロトコールは、ワールドワイドウェブを介してclinicaltrials.gov.に登録された。
医薬品の臨床試験の実施に関する基準(GCP)ガイドラインに従って、患者からインフォームドコンセントを得た。組み入れ規準には、標準療法による治療にもかかわらず進行した(難治性の)肝臓内の切除不能、注射可能な肝細胞腫瘍(原発性HCC)、正常な造血機能(白血球数>3、x109細胞/L、ヘモグロビン>100g/L、血小板数>60x109細胞/L)、および臓器機能(クレアチニン<132.6μmol/L、アスパラギン酸アミノトランスフェラーゼ(AST)/アラニンアミノトランスフェラーゼ(ALT)<2・5の正常上限、Child-PughクラスAまたはBを含む)、平均余命>16週、ならびにカルノフスキー・パフォーマンス・ステータス(KPS)>70が含まれた。除外基準には、肝外腫瘍、腫瘍>10cm最大直径、高いワクチン接種合併症(例えば、免疫抑制、湿疹)リスク、4週間以内の免疫抑制剤または癌治療剤による治療、急速進行性の腹水、妊娠または授乳が含まれた。
JX-594は、ヒトCSF2遺伝子およびLacZ遺伝子をそれぞれ、合成初期後期プロモーターおよびp7・5プロモーターの制御下でTK遺伝子領域に挿入することによって改変されたワクシニアWyeth株である。臨床試験材料(CTM)を、医薬品の製造および品質管理に関する基準(GMP)ガイドラインに従ってVero細胞において作製し、スクロース勾配遠心分離法によって精製した。ゲノム対pfu比は約70:1であった。JX-594を、10%グリセロール、138mM塩化ナトリウムを含むリン酸緩衝食塩水,pH7・4において製剤化した。最終製品の品質管理放出試験には、無菌性、エンドトキシン、および抗力のアッセイ法が含まれた。CTMをGM-CSFタンパク質濃度についても試験し、陰性であった(検出下限<14,000pg/mL)。JX-594を、標的腫瘍の推定総体積の25%に相当する体積の0・9%生理食塩水で希釈した。
切除不能なHCCを有する患者を無作為化して、2種類の用量レベル(108pfuまたは109pfu)のうち1つを与えた。ほぼ球状の腫瘍には、いくつかに分岐したQuadrafuse注射針を用いたイメージングガイド下腫瘍内注射を介して、不規則な形状の腫瘍には、21ゲージPEIT(経皮エタノール注射、マルチポア;HAKKO Medicals; Tokyo, Japan)針によってJX-594を投与した。2週間毎に3サイクルにわたって腫瘍(n=1〜5)に注射した。サイクル1において注射した同じ腫瘍に、後の各サイクルにおいて注射をした。
研究をして8週間後に、患者はJX-594 IT臨床試験を終了した。この後に、一部の患者(患者1702、1705、1002、1712、1713)には標準的なソラフェニブ治療(400mg 1日2回 p.o.)を与えた。JX-594治療に対する応答を評価するのに用いられた同じ手順を用いてDCE-MRI画像化によって、これらの患者における腫瘍を追跡した。
切除不能なHCCを有する患者に、JX-594レベル(109pfu)の2種類の用量を与えた。1回目の投与(1日目)の場合、JX-594を静脈内注入によって60分間にわたって投与した。2回目の投与および3回目の投与(8日目および22日目)の場合、ほぼ球状の腫瘍には、いくつかに分岐したQuadrafuse注射針を用いたイメージングガイド下腫瘍内注射を介して、または不規則な形状の腫瘍には、21ゲージPEIT(経皮エタノール注射、マルチポア;HAKKO Medicals; Tokyo, Japan)針によって、JX-594を投与した。25日目から、患者は経口ソラフェニブ療法(400mg 1日2回 p.o.)を模倣した。生存腫瘍組織を有する患者には、12週目にJX-594をIT注射した(このブースター注射の2〜3日前、このブースター注射の間、およびこのブースター注射の4〜5日後に、ソラフェニブ治療は一時中断した)。応答を評価するために、画像化(CT、DECE MIR、および/またはPET-CT)をベースライン、25日目、6週目、および/または12週目に行った。
患者11301には、肝臓に転移した腎細胞癌があった。21ゲージPEIT針を用いた腫瘍内注射によって肝臓腫瘍を治療した。患者に、合計4回のJX-594(109pfu/用量)を3週間あけて与えた(=4サイクル)。2回の治療サイクル毎に、造影CTスキャニングを行い、RECIST基準およびChoi基準を用いて6週目の応答評価を行った。患者は、RECIST基準による安定病態(SD)およびChoi応答(42%のHU減少)を経験した(Park et al., 2008)。
引き続き、患者11301は進行して、続けてスーテント治療が与えられた。患者に、3コースの50mg/日(4週間継続、2週間中断)、次に、3コースの37.5mg/日(4週間継続、2週間中断)を与え、25mg/日(2週間継続、1〜2週間中断)のスケジュールで維持した。
DCE MRI(ダイナミック造影磁気共鳴画像法)を、スクリーニング(ベースライン)、5日目(任意)、および8週目に行った。ソラフェニブを与えた患者には、ソラフェニブ治療を開始して4週間後および/または8週間後にDCE MRIを行った。DCE MRIは、腫瘍サイズ、血管分布、および壊死を評価する。スクリーニング/ベースラインDCE MRIは、腫瘍増殖停止時間および奏効率を求めるための基準として使用した。5日目(任意)DCE MRIを用いて、急性血管閉塞などの初期効果を評価した。腫瘍進行状況およびJX-594に対する腫瘍応答は、8週目の再診時に、修正RECIST基準および修正Choi基準によって放射線学的に評価した。MRIスキャンにおいて肝細胞癌を評価する専門知識のある放射線科医が独立して画像を検査した。肝内腫瘍を評価するために、客観的な「完全」抗腫瘍応答または「部分的」抗腫瘍応答を有する対象の割合を、修正RECIST、および修正Choi基準によって測定された応答(最長直径の合計の>10%減少および/またはMRIにおける平均腫瘍シグナル強度の>15%減少として定義した)に基づいて求めた。
腫瘍応答および腫瘍進行を測定するためのRECISTに対する修正は以下の通りであった。治療中または治療後に肝臓内に発症した新たな腫瘍を測定した。これらの最大直径を、全ての肝内腫瘍の最大直径の合計に算入した。しかしながら、肝臓内の新たな腫瘍は、進行それ自体のエビデンスとみなさなかった。このRECIST基準修正の理由は以下の通りである。放射線撮影によって最初に検出できなかった腫瘍塊にJX-594が感染すると、その腫瘍は、炎症および/または壊死のために新しい腫瘍かつ/または進行した腫瘍のように見える場合がある。しかしながら、これらの変化は真の腫瘍進行を表すものではない。さらに、治療目標は肝内腫瘍量を管理することであったので、腹部において肝外に検出された新たな腫瘍に注目し(場所を記録した)が、全体応答の決定に算入しなかった。従って、腫瘍の応答または進行は、測定可能な肝内腫瘍の最長直径の合計によって求められ、以下の通りに求められた。完全寛解(CR):全ての腫瘍の消失。部分寛解(PR):腫瘍のLD合計の少なくとも30%の減少。ベースライン合計LDを基準とする。進行性疾患(PD):腫瘍のLD合計の少なくとも20%の減少。ベースライン合計LDを基準とする。安定病態(SD):PRとするには縮小が十分でなく、PDとするには増加が十分でない。ベースライン合計LDを基準とする。
Choi応答基準は腫瘍直径に加えて腫瘍密度の変化を考慮に入れ(対RECIST)、この有用性を裏付けるデータが公表されている(Choi et al. 2004)。Choi et al. (2004)が消化管間質腫瘍において記載した通り、初期研究から、JX-594によって治療された肝臓腫瘍はサイズ減少が同時に起こることなく大きな内部壊死を発症できることが分かっている。従って、肝内腫瘍も測定し、修正Choi基準を用いて応答を評価した。DCE MRIが、腫瘍応答測定に使用した画像化モダリティーであったので、基準の修正は必要である。MRIには、CTのハウンスフィールド単位と同様のコントラスト密度測定の標準化システム(standardized system)がないので、ベースラインおよび治療後の腫瘍のパーセントエンハンスメントの比較は、関心対象領域(ROI)シグナル強度(SI)測定を用いて行った。スクリーニング/ベースラインDCE MRIは応答を求めるための基準として使用した。修正Choi基準による応答は、注射腫瘍の最長直径の合計の>10%減少および/またはMRIにおける注射腫瘍の平均シグナル強度の>15%減少と定義した。平均MRIシグナル強度(SI)を腫瘍のパーセントとして測定した。
プロトコールに定められた時点で、各患者に、ダイナミック造影磁気共鳴画像法(DCE MRI)を含む腹部MRIを行う。画像化は、1.5T MRシステムまたは3.0T MRシステムにおいて、患者を仰臥位にして肝臓の全範囲が画像化されるように配置されたボディ/トルソアレイコイルを用いて行う。肝臓の上に誘電体パッド(dielectric pad)を置いてもよい。
以下のパルスシーケンスを行う。
(1)2Dアキシャル同位相(In Phase)および逆位相(Opposed Phase)T1:T1強調スポイルドグラジエントエコー(T1-weighted spoiled gradient echo)(SPGR)デュアル位相アキシャル画像(TR:できるだけ短く;TE:2.1および4.2;フリップ角(FA):80〜90度、スライス厚5〜7mm、スライスギャップ最大1mm;位相エンコード160〜192、256x256に内挿;視野を患者の体型に合わせて最適化する、300〜450mm。(2)2D FSE T2アキシャル:TR:3500〜5000msec(有効);TE:60〜88msec;位相エンコード:160〜256x256;視野は患者の体型に合わせて最適化する、300〜450mm;スライス厚、5〜7mm;最大スライスギャップ1mm;画像化は脂肪抑制を用いて行わなければならない。呼吸同期法または他の体動抑制法が推奨される。
(3)3D T1ダイナミック画像化:このシーケンスの合計6セットを行う:1回の造影前、4回の造影直後、および1回の5分遅延画像セット。3D T1強調脂肪抑制収集のパラメータは以下の通りである:TR=2.0〜4.5msec;TE=1.42〜2.0msec;フリップ角、8〜12°;位相エンコード160〜192、512x512に内挿する;視野を患者の体型に合わせて最適化する、300〜450mm;内挿セクション厚(interpolated section thickness)、1.5〜3mm;肝臓を完全にカバーするスラブ厚。
以下のパルスシーケンスを行う。
(1)T1w 2Dアキシャル同位相(IP)および反対位相(Out-of-Phase)(OP):デュアル位相スポイルドグラジエントエコー(SPGR)
FOV:体型に合わせて最適化する、300〜450mm;TR:肝臓をカバーする最小値;TE:同位相および逆位相TEにおけるデフォルト;フリップ角:80〜90度;スライス厚:5〜7mm;ギャップ:0〜1mm(0mmギャップが好ましい);周波数マトリクス:320;位相エンコード:160〜224、512x512に内挿;脂肪飽和:オフ;帯域幅:デフォルト設定。
FOV:前記の(1)と同じものを使用する;TR:完全な肝臓を画像化するのに有効な最短のTR;TE 有効:60〜88msec;スライス厚:(1)と同じものを使用する;ギャップ:(1)と同じものを使用する;周波数マトリクス:320;位相エンコード:160〜224、512x512に内挿;脂肪飽和:オフ。
この場合、呼吸同期法を用いた自由呼吸下または息止め下の画像化を使用することができる。しかしながら、これは患者試験の中で統一される。例えば、呼吸同期法を用いてベースラインで患者をスキャンするのであれば、後の全てのMR試験では、このシーケンスと共に呼吸同期法が用いられる。呼吸同期法が用いられるのであれば、12〜20のエコートレインレングスを使用しなければならならず、同様に、最適なシグナル対ノイズのために十分な励起/収集でなければならない。息止め下のT2画像化が行われる場合には、24〜32のエコートレインレングスおよび1回の収集を行う。FOV:前記の(1)と同じものを使用する。TR 有効:3500〜5000;スライス厚:(1)と同じものを使用する;ギャップ:(1)と同じものを使用する;周波数マトリクス:320;位相エンコード:160〜224、512x512に内挿;脂肪飽和:オン。DCE MRI
FOV:前記の(1)と同じものを使用する;TR:2〜5msec;TE:1.4〜2.5;フリップ角:8〜15;スライス厚:1.5〜3mm内挿;スラブ厚:肝臓全体をカバーする;周波数マトリクス:288〜320;位相エンコード:160〜224、512x512に内挿;脂肪飽和:オン。スキャンの回数:合計5回(1回の造影前スキャンおよび4回の造影後スキャン、その後に、造影剤を注射して5分後に5回目のスキャン)。全てのスキャンを、標準的な施設での診療1回につき息を吸って止めるか、吐いて止めて呼吸停止中に行う。
灌流CTによって測定された急性血管閉塞は、JX-594腫瘍内注射によって直接治療された腫瘍において以前に見られた(Liu et al., 2008)。本発明者らは、JX-594治療に応答した血管閉塞および腫瘍壊死の経過を追跡するために、腫瘍灌流低下を追跡するDCE MRI分析を適用した。5日目の任意のDCE-MRIスキャニングによる新たな臨床試験に登録した16人の患者のうち13人に、このようなスキャンを行った。肝細胞癌(HCC)患者において、さらなる血管閉塞症例がある(図7)。
患者1703は肝細胞癌を有し、JX-594第2相臨床試験に登録された。単一の大きな腫瘍の中にJX-594を注射した(109pfu/用量)。5日後に、DCE MRIは急性血管閉塞を示した(上部の黒色パネルおよび白パネル)(図9)。下パネルは、血管閉塞/腫瘍壊死の程度を定量するために用いられたセグメンテーション分析の一例を示す(下パネル)(図7)。
患者1708は、肝臓の中に複数の腫瘍がある肝細胞癌を有し、JX-594第2相臨床試験に登録された。肝臓腫瘍の全てではないが一部にJX-594を注射した(108pfu/用量の総用量)。5日後に、DCE MRIは、肝臓をベースとする注射腫瘍および非注射腫瘍において急性壊死/血管閉塞を示した。図8は、JX-594治療前およびJX-594治療後の両方の画像を含む2種類の平面を示す。
患者0204は結腸直腸癌があり、肝臓、肺およびリンパ節に存在する転移があり、JX-594第2相臨床試験に登録された。肝臓転移の全てではないが一部にJX-594を注射した(109pfu/用量の総用量)。5日後に、DCE MRIは、肝臓をベースとする注射腫瘍および非注射腫瘍において急性壊死/血管閉塞を示した(図9)。
JX-594治療終了後8週目に再灌流を示した5人の患者は、続いて標準的なソラフェニブ投薬(400mg 1日2回)を受けた。Choi応答の向上が見られた(表A)。これらの応答は、JX-594治療単独に対する全ての初期Choi応答を上回っていた(図11、図12、および図13)。
歴史的に見て、ソラフェニブ単独に対するRECIST奏効率は1〜2%である(Llovet et al., 2008; Cheng et al., 2009)。Choi基準によって評価された局所対照群については、JX-594を与えず、ソラフェニブを与えたHCC患者のChoi応答を評価した。5人のJX-594患者のうち4人を治療した病院において、同じ期間に他の26人の患者にソラフェニブを与えた。応答評価の前に7人の患者が死亡し、15人の患者のChoi応答を評価した。ソラフェニブ単独患者のうち2人しかChoi+応答を示さなかった(合計26人;15人をChoi応答について評価した)。これらの患者はいずれもソラフェニブ療法と一緒に放射線療法を受けていたことに留意すべきである。比較すると、この期間中にJX-594療法の後にソラフェニブ療法が与えられた2人の患者にはChoi+応答があった(図10)。これは、腫瘍に対するソラフェニブ効果の驚くべきかつ並外れた改善である。
患者1702は肝細胞癌を有し、JX-594第2相臨床試験に登録され、3回のJX-594腫瘍内投与(2週間あけて与えた109pfu/用量)が与えられた。この患者は、修正Choi基準を用いてJX-594に対する応答を評価できなかったが、8週目スキャンは、修正RECIST基準を用いて注射腫瘍において安定病態(SD)を示したが、新たな腫瘍が発生したために進行性疾患(PD)を示した。従って、続いて、患者1702には、8週間にわたる標準的なソラフェニブ治療コース(200mg 1日2回 p.o.)を与えた。ソラフェニブ治療の4週間後および8週間後に撮影したDCE MRIスキャンは急性腫瘍壊死を示した。図は、3種類の平面を示した。左側の画像は、ソラフェニブ治療の4週間後からの画像であり、右側の画像は、ソラフェニブ治療の8週間後からの画像である(図11)。
患者 1705は肝細胞癌を有し、JX-594第2相臨床試験に登録された。JX-594を最初に投与して5日後に、DCE MRIによって、著しい灌流低下から腫瘍に血管閉塞が生じたことが確認された(上パネル)。JX-594投与(2週間あけて与えた108pfu/用量の3回の腫瘍内投与)が終了した後に、8週目スキャンは修正CHOI基準により応答を示したが(-36%)、修正RECIST基準により進行性疾患(PD)を示した。従って、続いて、患者 1705には、4週間にわたる標準的なソラフェニブ治療コース(400mg 1日2回 p.o.)を与えた。4週目に撮影したDCE MRIスキャンは急性腫瘍壊死を示した(下右パネル)。(図12、図15、図16)。
患者1712は肝細胞癌を有し、JX-594第2相臨床試験に登録された。JX-594投与(2週間あけて与えた109pfu/用量の3回の腫瘍内投与)が終了した後に、8週目スキャンは修正CHOI基準により応答を示したが(-33%)、修正RECIST基準により進行性疾患(PD)を示した。患者1712には、4週間にわたる標準的なソラフェニブ治療コース(400mg 1日2回 p.o.)を与えた。4週目に撮影したDCE-MRIスキャンは急性腫瘍壊死を示した(図13)。
スーテントは、VEGF活性および血管新生を阻害する、腎細胞癌(RCC)に対して認可された別の標的化癌療法である。患者11301は、肝臓に転移したRCCを有し、肝臓をベースとする腫瘍を治療するためのJX-594第1相臨床試験に登録された。2コースのJX-594投与(3週間あけて与えた109pfu/用量の腫瘍内投与)が終了した後に、6週目の評価はRECISTにより安定病態を示した。患者には、もう2回のJX-594治療コースを与えたが、1個の肝臓腫瘤および大きな(14cm)腹部腫瘤が進行した。従って、次に、患者1705にスーテント療法を行った。低ヘモグロビンおよび肝臓転移の程度に基づいて患者の予後は不良であった(Motzer et al., 2006)。驚いたことに、この後に、患者の腫瘍の全てが完全寛解した(図14)。全身PETスキャニングはシグナルを示さなかった。JX-594治療後の生存は3年+である(患者は今なお生存している)。比較すると、10cmを超える腫瘍におけるスーテント単独の歴史上のRCC完全寛解率は0%である。予後不良のRCC患者のうち、3年間生存した患者は5%未満である。
患者JX16-HCC-03は肝細胞癌を有し、JX-594第2相臨床試験に登録された。JX-594投与(1回の静脈内投与および2回の腫瘍内投与)が終了した後に、患者にソラフェニブを与えた。ソラフェニブを開始して10日後に、DCE-MRIスキャンは非注射肝外腫瘍における灌流消失を示した(図18)。
Claims (14)
- ソラフェニブおよびスニチニブから選択される抗血管新生チロシンキナーゼ阻害剤を含む薬学的組成物であって、GM-CSFを発現し、かつ機能的チミジンキナーゼ遺伝子を欠如しているWyeth株であるワクシニアウイルスを用いたワクシニアウイルス療法が以前に行われたヒト対象において癌を治療するため、かつ、該ワクシニアウイルス療法から少なくとも1週間後におよび13週間後までに投与するための、薬学的組成物。
- 腫瘍が血管再生しているかどうか判定した後に投与するための、請求項1記載の薬学的組成物。
- 腫瘍の血管再生の判定が、磁気共鳴画像法(MRI)または造影CTスキャニングによる該腫瘍の非侵襲的画像化による、請求項2記載の薬学的組成物。
- 前記ワクシニアウイルス療法から少なくとも5週間後、6週間後、7週間後または8週間後の投与のための、請求項1記載の薬学的組成物。
- 前記癌が、脳腫瘍、頭頸部癌腫瘍、食道腫瘍、皮膚腫瘍、肺腫瘍、胸腺腫瘍、胃腫瘍、結腸腫瘍、肝臓腫瘍、卵巣腫瘍、子宮腫瘍、膀胱腫瘍、精巣腫瘍、直腸腫瘍、乳房腫瘍、腎臓腫瘍、膵臓腫瘍、肝細胞癌、または腎癌である、請求項1記載の薬学的組成物。
- 前記癌が肝細胞癌または腎癌である、請求項5記載の薬学的組成物。
- 前記癌が転移した癌腫である、請求項5記載の薬学的組成物。
- 前記転移した癌腫が結腸または腎臓の転移した癌腫である、請求項7記載の薬学的組成物。
- 前記ワクシニアウイルスがJX-594である、請求項1記載の薬学的組成物。
- 前記ワクシニアウイルス療法から少なくとも2週間後に投与するための、請求項1記載の薬学的組成物。
- 前記癌が肝細胞癌または腎細胞癌である、請求項1記載の薬学的組成物。
- GM-CSFを発現し、かつ機能的チミジンキナーゼ遺伝子を欠如しているWyeth株であるワクシニアウイルス、および、ソラフェニブおよびスニチニブから選択される抗血管新生チロシンキナーゼ阻害剤を含む組み合わせ医薬であって、ヒト対象において癌を治療するため、かつ、最初に該ワクシニアウイルスを用いたワクシニアウイルス療法を行い、該ワクシニアウイルス療法から少なくとも1週間後におよび13週間後までに該抗血管新生チロシンキナーゼ阻害剤を投与するための、組み合わせ医薬。
- 前記ワクシニアウイルスが、腫瘍内に、血管内に、または血管内および腫瘍内の両方に投与される、請求項12記載の組み合わせ医薬。
- 前記対象が、前記ワクシニアウイルスを用いたワクシニアウイルス療法が以前に行われた対象である、請求項12または13記載の組み合わせ医薬。
