Description
本発明は一般的に医療装置及び技法に係り、さらに具体的には心血管組織縫合装置、システム、技法及びキットに関する。
図1を参考にすれば、血管縫合システム100は、一般的にクリップ102またはプラグなどの血管縫合装置及び展開または前進器具104を備えることができる。プラグまたは縫合装置はまた、密封装置、インプラント、血管アクセス縫合装置、動脈切開部縫合装置、血管縫合装置、及び組織縫合装置を称することもある。図示されたように、クリップ102は、展開器具104の遠位端部105上にローディングされる。展開器具104は、血管18内に挿入されるカテーテルなどの血管導入装置108または他の管状医療装置により一般的に案内され、これに沿って前進するか、またはこれら装置に摺動自在に装着される。特定実施形態において、血管導入装置108の挿入のために形成される、皮膚の狭い穴は、摺動自在の案内式組織切開装置106により膨脹または拡大して、体内に対する展開器具104の通路を容易に許容するほどに十分に大きい経皮性穴12を形成する。
多様な展開器具が本明細書に図示及び説明されている。当業者ならば、開示された構造に対する変形は、本開示内容の範囲を逸脱せずに行われることを理解できるであろう。
本明細書で開示される組織クリップ、プラグ及び縫合装置は、縫合の目的で、例えば、動脈縫合の目的で、哺乳類身体内への移送の観点で開示されたものである。しかし、当業者ならば理解できるように、他の用例も可能である。例えば、クリップ、プラグ及び縫合装置は、これらクリップ、プラグ及び縫合装置は、移送システムによる移送を必要としないが、出血を止めるための圧着に対する代案として、外傷(刺傷または銃傷などの外傷)の関連分野で多少の移送ができるようにサイズが決定される。エッジ、リッジ、フランジ、ウィング、花弁、半径方向の延長要素及び水平方向の突出部を含むが、これらに限定されない多様な要素が、縫合装置と共に外側面または内側面で使われる。
図40ないし図42は、特定実施形態で血管縫合システム100に使われる摺動自在の案内式組織カッター106などの組織開口ワイドナーの例を例示したものである。所望の医療過程を完了した以後に、医療専門家は、図1に図示されたように、血管導入装置108のチューブセクション110に組織カッターをクリッピングすることで、組織カッター106を臨時で付着できる。この時、組織カッター106は、血管導入装置シース108に沿って摺動式で前進できる。カッター106は、ブレード202の鋭い遠位端部203を利用して、経皮用開口12の位置で正確な深さ及び幅の切開を行うように構成される。一般的にカッター106は、展開器具104のために、一貫性のあるようにそして容易に寸法が決定される進入点を許容するために、チューブ110から特定配向及び距離でブレード202のエッジ203を位置決める。機械的な停止部208などの縁(ledge)は、展開器具104の進入を容易にするために求められる切開部がさらに深くならないように保証できる。摺動自在の組織カッター106のためのガイドとして既存の導入装置シース108を利用することは、切開部の適当な配置を保証するのにも役に立つ。切開を行った以後に、摺動自在の組織カッター106は、血管導入装置の側部から除去される。
図44ないし図46は、特定実施形態で血管縫合システム100に使われる摺動自在の案内式組織拡張器220の例を例示したものである。組織拡張器220は、展開器具104の前で組織領域を拡張させるように構成でき、皮膚にある開口を通じて移動できる。組織拡張器220は、一般的にチューブ形状であり、既存の導入装置シースにスナップ結合及びスナップ結合解除されるように構成できる。展開器具104の前進前に組織を拡張することは、組織を通じる臨時経路を形成し、これにより血管壁16に対して前方への展開器具104の前進がさらに容易になる。組織領域を拡張した後に、組織拡張器220は、これから後方に向かって摺動し、導入装置シースの周囲から除去される。

特定実施形態において、内側管状部材の遠位端部は、さらに大きい周り直径を持つ少なくとも一つの部分を持つか、または(展開中に前方に移動する最中に)クリップ尖叉を外側に向かって偏向させるためにフレアーされ、これは、さらに大きい組織圧縮及び密封のためにクリップが前方に進む時(大径になったセクションがない場合に比べて)、さらに多くの組織を捕獲させる。内側管状部材の遠位端部は、周り周囲に配置される少なくとも一つのバンプまたは隆起した表面のように、周り部分ではない拡大部をさらに持つ。このような構造は、クリップ尖叉のただ一部のみを前進及び展開中に外側に向かって開放または偏向させるために、または一部を他の部分よりさらに偏向させるために使われうる。

Claims (19)
- 組織縫合装置であって、
(a)可とう性遠位キャップと、
(b)内側軸を有するステムと、
前記可とう性遠位キャップは、前記内側軸に対し垂直方向に延びるフェイスプレートと当該フェイスプレートの遠位端部に複数の突出したセクションを含み、当該突出したセクションによって、前記組織縫合装置がより小さい断面形状をとるように、前記可とう性遠位キャップが前記内側軸方向に曲がるように構成され、
前記ステムは、近位で位置決められる貫通開口をさらに備え、
前記可とう性遠位キャップと前記ステムとが単一部品で形成されおり、
前記組織縫合装置が、生体吸収性材料から形成されている、組織縫合装置。 - 近位で位置決められる一つ以上の半径方向延長要素をさらに備え、
前記ステムは、前記可とう性遠位キャップと一つ以上の半径方向延長要素との間に位置決められる、請求項1に記載の組織縫合装置。 - 前記可とう性遠位キャップは、円形、三角形、長円形(oval)、卵形(ovoid)、楕円形、正方形及び皿形から選択される形状を持つことを特徴とする請求項1に記載の組織縫合装置。
- 前記可とう性遠位キャップの形状は、丸いエッジを持つことを特徴とする請求項3に記載の組織縫合装置。
- 前記可とう性遠位キャップ及びステムのうち少なくとも一つは、テザー(tether)と、リリース自在に連通されることを特徴とする請求項1に記載の組織縫合装置。
- 移送カプセルであって、
カートリッジ本体と、
テーパード遠位先端部と、
透明セクションと、
前記透明セクションを貫通して延びるルーメンと、を備える移送カプセルにおいて、
前記透明セクションは請求項1〜5のいずれか1項に記載の組織縫合装置を収納するように構成されるものであり、
前記透明セクションを貫通して延びる前記ルーメンは、前記組織縫合装置の半径方向延長セクションを偏向させることで、前記組織縫合装置の断面直径が低減させるためのテーパ内径を有する、移送カプセル。 - 前記移送カプセルは中央開口を通じてプランジャを収容するように構成されることを特徴とする請求項6に記載の移送カプセル。
- 前記移送カプセルは、プランジャが第1軸線方向に移動するように、そして第1軸線方向とは異なる第2軸線方向への移動に抵抗可能に構成されることを特徴とする請求項7に記載の移送カプセル。
- 組織縫合移送システムであって、
導入装置と、
請求項1〜5のいずれか1項に記載の組織縫合装置を内部に有する組織縫合移送カートリッジと、
ガイド及びシール組立体と、
プランジャと、を備え、
前記導入装置は、近位端部で組織縫合移送カートリッジとリリース自在に噛み合うように構成され、
前記組織縫合移送カートリッジは、近位端部でガイド及びシール組立体とリリース自在に噛み合うように構成され、
前記プランジャは、ガイド及びシール組立体、組織縫合移送カートリッジ及び導入装置それぞれでルーメンを通じて進むように構成されるものであり、
前記プランジャが前記組織縫合移送カートリッジの前記ルーメンを通じて進む際に、前記組織縫合装置がより小さい断面形状プロファイルをとるように、前記組織縫合装置の半径方向延長セクションが進路後方に向かって曲げられるようなテーパ内径を前記ルーメンが有する、組織縫合移送システム。 - 前記可とう性遠位キャップは、円形、三角形、長円形、卵形、楕円形、正方形及び皿形から選択される形状を持つことを特徴とする請求項9に記載の組織縫合移送システム。
- 前記可とう性遠位キャップの形状は、丸いエッジを持つことを特徴とする請求項10に記載の組織縫合移送システム。
- 前記可とう性遠位キャップ及びステムのうち少なくとも一つは、テザーと、リリース自在に連通されることを特徴とする請求項9に記載の組織縫合移送システム。
- 前記組織縫合移送カートリッジは、
カートリッジ本体と、
テーパード遠位先端部と、
透明セクションと、
前記透明セクションを貫通して延びるルーメンを備える移送カプセルとを備え、
前記透明セクションは、前記組織縫合装置を収納するように構成され、
前記透明セクションを貫通して延びる前記ルーメンは、前記組織縫合装置の半径方向延長セクションを偏向させることで、前記組織縫合装置の断面直径が低減させるためのテーパ内径を有することを特徴とする請求項9に記載の組織縫合移送システム。 - 前記移送カプセルは、中央開口を通じてプランジャを収容するように構成されることを特徴とする請求項13に記載の組織縫合移送システム。
- 前記移送カプセルは、プランジャが第1軸線方向に移動するように、そして第1軸線方向とは異なる第2軸線方向への移動に抵抗可能に構成されることを特徴とする請求項14に記載の組織縫合移送システム。
- 前記可とう性遠位キャップ及びステムのうち少なくとも一つは、テザー(tether)に連通されることを特徴とする請求項1に記載の組織縫合装置。
- 前記可とう性遠位キャップ及びステムのうち少なくとも一つは、テザー(tether)に連通されることを特徴とする請求項9に記載の組織縫合移送システム。
- 前記移送カプセルは、前記組織縫合装置の横断面直径を小さくするようにさらに構成される請求項6に記載の移送カプセル。
- 前記プランジャは、前記組織縫合装置に進入するように構成され、前記移送カプセルは、前記組織縫合装置の横断面直径を小さくするように構成される、請求項15に記載の組織縫合移送システム。
