図1を参考にすれば、血管縫合システム100は、一般的にクリップ102またはプラグなどの血管縫合装置及び展開または前進器具104を備える。プラグまたは縫合装置はまた、密封装置、インプラント、血管アクセス縫合装置、動脈切開部縫合装置、血管縫合装置、及び組織縫合装置を称することもある。図示したように、クリップ102は、展開器具104の遠位端部105上にローディングされる。展開器具104は、血管導入装置108または血管18内に挿入されたカテーテルなどの他の管状医療装置に摺動方式で装着されるか、またはこれに沿って前進し、一般的にこれによって案内される。特定実施形態において、血管導入装置108の挿入のために初期に形成される、皮膚の狭い穴は、摺動自在な案内式組織カッター106により膨脹するか、または拡大して、体内に対する展開器具104の通過を容易に許容するのに十分に大きい経皮性穴12を形成できる。

図40ないし図42は、特定実施形態で血管縫合システム100に使われる、摺動自在な案内式組織カッター106などの組織開口ワイドナー(tissue opening widener)の例を例示したものである。所望の医療過程を完了した後、医療専門家は、図1に示したように、血管導入装置108のチューブセクション110に組織カッターをクリッピングすることで、組織カッター106を臨時に付着できる。この時、組織カッター106は、血管導入装置シース108に沿って摺動式で前進できる。カッター106は、ブレード202の鋭い遠位端部203を用いて経皮用開口12の位置で正確な深さ及び幅の切開を行うように構成される。一般的にカッター106は展開器具104のために、一貫性のあるように、そして容易に寸法が決定される進入点を許容するために、チューブ110から特定配向及び距離にブレード202のエッジ203を位置設定する。機械的な停止部208などの棚(ledge)は、展開器具104の進入を容易にするために要求される切開部がさらに深くならないように保証できる。摺動自在な組織カッター106のためのガイドとして、既存の導入装置シース108を用いることは、切開部の適当な配置を保証するのに役に立つ。切開した以後に、摺動自在な組織カッター106は、血管導入装置の側部から除去される。
図44ないし図46は、特定実施形態で血管縫合システム100に使われる摺動自在な案内式組織拡張器220の例を例示したものである。組織拡張器220は、展開器具104の前方で組織領域を拡張させるように構成され、皮膚にある開口を通じて移動する。組織拡張器220は、一般的にチューブ形状であり、既存の導入装置シースにスナップ結合及びスナップ結合解除されるように構成される。展開器具104の前進以前に組織を拡張することで組織を通じる臨時経路を形成し、これにより、血管壁116に対する前方への展開器具104の前進がさらに容易になる。組織領域を拡張した後、組織拡張器220は、これから後方に向かって摺動しつつ導入装置シース周囲から除去される。

特定実施形態において、内側管状部材の遠位端部は、さらに大きい周り直径を持つ少なくとも一つのセクションを持つか、または(展開中に前方に移動する間に)クリップ枝角を外側に向かって偏向させるためにフレアされ、これは、さらに大きい組織圧迫及び密封のためにクリップが前方に前進する時(直径が増大したセクションがない場合に比べて)、さらに多くの組織を捕獲する。内側管状部材の遠位端部はまた、円周の周りに配される少なくとも一つのバンプ、または隆起した表面のように円周部分ではない拡大部を持つ。このような構造は、単にクリップ枝角の一部のみ前進及び展開中に外側に向かって開放されるか、または偏向させるために、または一部を他の部分よりさらに偏向させるために使われる。

Claims (10)
- 縫合装置であり、
(a)遠位面に形成された複数の突出部を有する可とう性遠位スカートと、ここで前記複数の突出部が、前記可とう性遠位スカートを第1方向において折れ曲がるように構成し、かつ前記第1方向と逆の第2方向において折れ曲がりに抵抗するように構成するものであり、
(b)前記可とう性遠位スカートの近位面と接触する一端部を持つステムと、を備え、
前記可とう性遠位スカートの複数の突出部は、少なくとも1つの中央突出部と当該中央突出部の周りに配置される複数の周り突出部とを備える、縫合装置。 - 前記可とう性遠位スカートの複数の突出部は、前記中央突出部と前記周り突出部との間に形成される一つ以上の折り曲げ可能な特徴部を備える請求項1に記載の縫合装置。
- 前記折り曲げ可能な特徴部は、外力が印加されれば、屈曲のための自然な位置を形成する薄いセクションである請求項2に記載の縫合装置。
- 前記第2方向に力が印加される時に、前記周り突出部が前記中央突出部に接触して、さらなる折り曲げを制限する構成である請求項1に記載の縫合装置。
- 近位に位置設定される一つ以上の半径方向延長要素をさらに含み、前記ステムは、可とう性遠位スカートと一つ以上の半径方向延長要素との間に位置設定される請求項1に記載の縫合装置。
- 前記可とう性遠位スカートの近位面は、アンカーリング突出部、ニブ(nib)またはリブ(rib)のうち少なくとも一つ以上持つ請求項1に記載の縫合装置。
- 前記可とう性遠位スカートは、円形、三角形、タマゴ型(oval)、卵形(ovoid)、楕円形、正方形、皿型または丸いエッジから選択される形状を持つ請求項1に記載の縫合装置。
- 前記ステムは、中央と、中央ではない領域のうち少なくとも一つで可とう性遠位スカートの近位面上に位置設定される請求項1に記載の縫合装置。
- 組織縫合移送システムであり、
管状医療装置と、
組織縫合移送カートリッジと、
請求項1に記載の縫合装置と、
ガイド及びシール組立て体と、
プランジャと、
を備える組織縫合移送システムにおいて、
前記管状医療装置は、近位端部で組織縫合移送カートリッジとリリース可能に結合するように構成され、前記組織縫合移送カートリッジは、近位端部でガイド及びシール組立て体とリリース可能に結合するように構成され、前記プランジャは、ガイド及びシール組立て体、組織縫合移送カートリッジ及び管状医療装置それぞれでルーメンを通じて前進するように構成される組織縫合移送システム。 - 前記管状医療装置は、針、チューブ、ガイドワイヤー、電極ワイヤー、静脈内ワイヤー、導入装置、シース、拡張器、カテーテル、腹腔鏡、内視鏡、套管針、展開ツール、またはカニューレのうち一つ以上である請求項9に記載の組織縫合移送システム。
